Description
本発明は、患者の血行動態における慢性変化又は急性変化に応じて選択されたペーシング出力構成を用いて心臓再同調療法を提供する心臓治療システムを提供する。
ある実施態様において、前記収縮機能のパラメータの値は、少なくとも1つの電気的な心臓特性のタイミング値を含む。
ある実施態様において、前記1つ以上のセンサは、化学的センサ、患者活動センサ、呼吸センサ、体位センサ、及び心臓電位図センサのうちの少なくとも1つを含む。
Claims (6)
- 患者の心臓に心臓再同調療法を提供する心臓治療システムであって、
心臓の左心室内の、左心室と接する、又は左心室の周囲の多数の電極位置にそれぞれ位置決め可能な複数の電極と、
患者の血行動態における慢性変化または急性変化を検出すべく構成された1つ以上のセンサ及び関連付けられた検出回路と、
検出された前記患者の血行動態における慢性変化または急性変化に応じて、脱分極の遅延の分布を含む左心室の収縮機能に関連するパラメータ分布を決定する手段であって、前記決定する手段は前記多数の電極位置のそれぞれについて前記収縮機能のパラメータの値を測定すべく構成された手段と、
選択回路であって、検出された慢性変化に関して決定されたパラメータ分布に基づいて、前記複数の電極のうちの1つ以上の電極を含む第1のペーシング出力構成を選択すべく構成され、かつ、検出された急性変化に関して決定されたパラメータ分布に基づいて、前記複数の電極のうちの1つ以上の電極を含む、第1のペーシング出力構成とは異なる第2のペーシング出力構成を選択すべく構成された選択回路と、
選択された第1のペーシング出力構成または第2のペーシング出力構成を用いて心臓に心臓再同調療法を提供する手段であって、前記提供する手段は選択された1つ以上の電極を用いて左心室をペーシングすべく構成された手段と
を含むシステム。 - 前記提供する手段は、前記患者の血行動態変化が検出された後に、前記複数の電極から選択された1つ以上の電極を用いて左心室をペーシングし、かつ、前記患者の血行動態変化が検出される前に、別の1つ以上の電極を用いて左心室をペーシングすべく構成される請求項1記載のシステム。
- 前記収縮機能のパラメータの値は、少なくとも1つの電気的な心臓特性のタイミング値を含む請求項1記載のシステム。
- 前記1つ以上のセンサは、化学的センサ、患者活動センサ、呼吸センサ、体位センサ、および心臓電位図センサのうちの少なくとも1つを含む請求項1記載のシステム。
- 前記決定する手段はプロセッサを含むとともに、前記システムは、患者の血行動態変化、パラメータ分布、及びペーシング出力構成のうちの少なくとも1つに関する情報を含むルックアップテーブルを格納すべく構成されたメモリを更に含み、
前記プロセッサは、前記ルックアップテーブルの情報を整理し、後続のペーシング出力構成を選択するために前記ルックアップテーブルを使用すべく構成される請求項1記載のシステム。 - 前記選択回路は、心臓の増強されたポンプ機能を提供する第1のペーシング出力構成を決定すべく構成され、かつ、代謝要求の増大に対応した心拍出量の増加を提供する第2のペーシング出力構成を決定すべく構成される請求項1に記載のシステム。
