Description
カフの1、2カ所から可撓性のプラスチックチューブにより気密に接続されており、患者の肢の目的とする場所に巻き付けられたときに、外科手術の施術に適切な一定時間動脈を閉鎖するために必要とされる最小値を超える圧力で気密に接続される。このような空気圧式カフ装置は、たとえばMcEwenによる米国特許第4469099号、第4479494号、第5439477号明細書や、McEwen及びJamesonによる米国特許第5556415号、第5855589号明細書に開示されている。さらに、従来技術におけるデュアルポート式手術用タニケットシステムに適したデュアルポート式タニケットカフの代表例は、米国特許第4635635号、第5454831号、第5741295号及び第5649954号明細書に開示されている。

Claims (11)
- 患者の肢部に巻き付けられ、第1のカフポートと気密に接続され、かつ前記第1のカフポートから独立した第2のカフポートと気密に接続された加圧部を含むタニケットカフとタニケット装置とを含む閉塞検出装置を有するデュアルポート式手術用タニケットシステムであって、
前記タニケット装置は、前記第1および前記第2のカフポートに別々に結合されて前記タニケットカフと前記タニケット装置との間に独立した前記第1および第2の空気通路を確立し、
前記カフの前記加圧部内の圧力を表示するための圧力感知手段と、
前記カフを加圧する加圧手段と、
前記カフを減圧する圧力緩和手段と、
外科処理を行うのに適当な長さの期間にわたり、前記カフ内の圧力を基準圧力レベルに近い値に調整するために前記加圧手段および前記圧力緩和手段を選択的に作動させる前記圧力感知手段に応答する圧力制御手段と、
閉塞検出期間中に前記第1の空気通路へ空気圧力振動を導入し、前記圧力振動を前記加圧部を通して前記第2の空気通路内へと向かわせ、前記閉塞検出期間中の前記第2の空気通路内の圧力を監視し、かつ、前記圧力振動に起因する前記第2の空気通路内での圧力変動が前記閉塞検出期間内に検知されない場合には閉塞アラームを発生させる閉塞検出手段とを含む、デュアルポート式手術用タニケットシステム。 - 前記圧力振動は、前記閉塞検出期間より短い期間において、前記閉塞検出前記基準圧力レベルと比べて少なくとも既定の圧力差は異なる、請求項1に記載のタニケットシステム。
- 前記加圧手段は前記第1の空気通路と気密に通じており、前記圧力緩和手段は前記第2の空気通路と気密に通じており、前記閉塞検出手段は前記圧力振動を前記基準圧力レベルより大きい既定の圧力レベルで導入すべく前記加圧手段を作動させる、請求項1に記載のタニケットシステム。
- 前記圧力緩和手段は前記第1の空気通路と気密に通じており、前記加圧手段は前記第2の空気通路と気密に通じており、前記閉塞検出手段は前記基準圧力レベルより低い既定の圧力レベルで前記圧力振動を導入すべく前記圧力緩和手段を作動させる、請求項1に記載のタニケットシステム。
- 前記閉塞検出手段は、さらに前記閉塞検出期間中に前記圧力制御手段の稼働を一時的に停止させる、請求項1に記載の閉塞検出装置。
- 前記閉塞検出期間の開始時に、前記閉塞検出手段は、前記第1の空気通路に導入される圧力振動のレベルを、初期レベルから、既定の最小圧力変動レベルよりも大きい圧力変動を前記第2の空気通路に生じさせるレベルまで徐々に増加させていく、請求項1に記載の閉塞検出装置。
- 加圧部を有し、患者の肢部に巻き付けるのに適しており、手術が円滑に行われるように動脈が前記カフを通って前記肢部へ流れるのを阻止するのに十分な基準圧力レベルにまで加圧されるタニケットカフであって、前記加圧部は第1のカフポートと第2のカフポートとに気密に通じているタニケットカフと、
前記第1のカフポートと着脱自在に接続され、閉塞検出期間中に前記第1のカフポートへ空気圧力振動を導入し。その結果、前記圧力振動が前記加圧部内に導入され、前記加圧部を出て前記第2のカフポート内に至るようにした閉塞検出部とを有し、
前記閉塞検出部は、前記閉塞検出期間中に前記第2のカフポートにおける圧力を監視すべく前記第2のカフポートと着脱自在に接続され、前記圧力振動に起因する前記第2のカフポートでの圧力変動が前記閉塞検出期間内に検出されない場合に閉塞アラームを発生させる、デュアルポート式手術用タニケットカフの閉塞検出装置。 - 前記タニケットカフ内の圧力を前記基準圧力レベルに近い値に調整するために前記第1および前記第2のカフポートと気密に接続されているタニケット圧制御手段を含み、該圧力制御手段により前記閉塞検出部はさらに前記閉塞検出期間中に圧力の制御を一時的に停止させる、請求項7に記載の閉塞検出装置。
- 前記圧力振動は、前記閉塞検出期間より短い期間において、前記基準圧力レベルと比べて少なくとも既定の圧力差は異なる、請求項7に記載の閉塞検出装置。
- デュアルポート式タニケットカフにおける閉塞を検知する閉塞検知装置を制御する方法において、
閉塞検出期間中に空気圧力振動をタニケットカフの加圧部と気密に接続された第1のカフポートに導入するステップと、
前記圧力振動を、前記加圧部を通して、前記第1のカフポートから独立し且つ前記加圧部と気密に接続された第2のカフポート内に向かわせるステップと、
前記閉塞検出期間中に前記第2のカフポートにおける圧力変動を監視するステップと、
前記圧力振動に起因する前記第2のカフポート内での圧力変動が前記閉塞検出期間内に検知されない場合には閉塞アラームを発生させるステップとを含む、閉塞検知装置を制御する方法。 - 前記閉塞検出期間中に前記タニケットカフの前記加圧部における圧力制御をタニケット装置により一時停止させるステップを含む、請求項11に記載の閉塞検知装置を制御する方法。
