Claims (17)
- 虚血性イベントを示す1つ又は複数の信号を感知する感知回路と、
前記1つ又は複数の信号から前記虚血性イベントを検出するために前記感知回路に接続された虚血検出器と、
ペーシング・パルスを送るパルス出力回路と、
前記ペーシング・パルスの送出を制御するために、前記虚血検出器と前記パルス出力回路に接続された制御回路とを備え、前記制御回路が、
前記虚血性イベントの前記検出に応答して、1つ又は複数の心臓保護ペーシング・シーケンスを始めるようになされた心臓保護ペーシング・シーケンスのイニシエータであって、前記1つ又は複数の心臓保護ペーシング・シーケンスがそれぞれ、交互になったペーシングと非ペーシング期間を含み、前記ペーシング期間がそれぞれペーシング時間を有し、その間に複数の前記ペーシング・パルスが送られ、前記非ペーシング期間がそれぞれ非ペーシング時間を有し、その間に前記ペーシング・パルスが送られない、心臓保護ペーシング・シーケンスのイニシエータと、
前記1つ又は複数の心臓保護ペーシング・シーケンスの時間を調整するようになされた心臓保護ペーシングのタイマと、を備え、各心臓保護ペーシングは、交互になったペーシング期間と非ペ−シング期間とを含み、前記ペーシング期間が約5秒から10分のペーシング時間を有し、前記非ペーシング期間が約5秒から10分の非ペーシング時間を有する、
心臓ペーシング・システム。 - 前記制御回路が、前記1つ又は複数の心臓保護ペーシング・シーケンスうちの1つが始められたとき、ペーシング・モードを長期的なペーシング・モードから一時的なペーシング・モードに切り替え、前記1つ又は複数の心臓保護ペーシング・シーケンスのうちの前記1つが完了したとき、前記ペーシング・モードを前記一時的なペーシング・モードから前記長期的なペーシング・モードに切り替えるようになされたペーシング・モード・スイッチを備え、前記一時的なペーシング・モードが実質的に前記長期的なペーシング・モードとは異なる、請求項1に記載のシステム。
- 前記心臓保護ペーシング・シーケンスのイニシエータが、前記虚血性イベントの前記検出に応答して少なくとも1つのポストコンディショニング・シーケンスを始めるようになされたポストコンディショニング・シーケンス・イニシエータを備え、前記ポストコンディショニング・シーケンスが前記1つ又は複数の心臓保護ペーシング・シーケンスのうちの1つになっている、請求項1又は2に記載のシステム。
- 前記虚血性イベントの終了が前記虚血検出器によって検出された後、前記ポストコンディショニング・シーケンスのイニシエータが、前記少なくとも1つのポストコンディショニング・シーケンスを始めるようになされた、請求項3に記載のシステム。
- 虚血後の時限が切れたとき、前記ポストコンディショニング・シーケンスのイニシエータが、前記少なくとも1つのポストコンディショニング・ペーシング・シーケンスを始めるようになされ、前記虚血性イベントの終了が前記虚血検出器によって検出されたとき、前記虚血後の時限が始まる、請求項4に記載のシステム。
- 前記心臓保護ペーシング・シーケンスのイニシエータが、前記虚血性イベントの前記検出に応答して複数のプレコンディショニング・シーケンスを始めるようになされたプレコンディショニング・シーケンス・イニシエータを備え、前記プレコンディショニング・シーケンスがそれぞれ、前記1つ又は複数の心臓保護ペーシング・シーケンスのうちの1つになっている、請求項1から請求項5のいずれか一項に記載のシステム。
- 前記プレコンディショニング・シーケンス・イニシエータが、所定の期間を使用して断続的なベースで予防のプレコンディショニング・シーケンスを始めるようになされた、請求項6に記載のシステム。
- 前記プレコンディショニング・シーケンス・イニシエータが、1つ又は複数のプレコンディショニング命令に応答して前記予防のプレコンディショニング・シーケンスを始めるようになされた、請求項6に記載のシステム。
- 前記制御回路が、心不全を検出し、前記心不全の前記検出に応答して、前記1つ又は複数の心臓保護ペーシング・シーケンスを中止するようになされた、請求項1から請求項8のいずれか一項に記載のシステム。
- 前記感知回路が1つ又は複数の心臓の信号を感知する心臓の感知回路を備え、前記虚血検出器が前記1つ又は複数の心臓の信号から前記虚血性イベントを検出するようになされた、請求項1から請求項9のいずれか一項に記載のシステム。
- 前記感知回路が、心臓のインピーダンス又は経胸腔的なインピーダンスを示すインピーダンス信号を感知するインピーダンス感知回路を備え、前記虚血検出器が前記インピーダンス信号から前記虚血性イベントを検出するようになされた、請求項1から9のいずれか一項に記載のシステム。
- 前記感知回路が心音を示す1つ又は複数の信号を感知する心音感知回路を備え、前記虚血検出器が心音を示す前記1つ又は複数の信号から前記虚血性イベントを検出するようになされた、請求項1から9のいずれか一項に記載のシステム。
- 前記感知回路が心音を示す1つ又は複数の圧力信号を感知する圧力感知回路を備え、前記虚血検出器が心音を示す前記1つ又は複数の信号から前記虚血性イベントを検出するようになされた、請求項の1から9のいずれか一項に記載のシステム。
- 前記感知回路が、それぞれが領域的な心壁の動きを示す1つ又は複数の加速度信号を感知する心臓の動きの感知回路を備え、前記虚血検出器が心音を示す前記1つ又は複数の加速度信号から前記虚血性イベントを検出するようになされた、請求項1から9のいずれか一項に記載のシステム。
- 前記感知回路が、心拍変動(HRV)を感知し、前記HRVの基準であるHRVパラメータを生成する心拍変動(HRV)感知回路を備え、前記虚血検出器が前記HRVパラメータを使用して前記虚血性イベントを検出するようになされた、請求項1から9のいずれか一項に記載のシステム。
- 少なくとも前記虚血検出器、前記パルス出力回路、前記制御回路を備え、前記虚血検出器が前記虚血性イベントの前記検出を示す虚血警報信号を生成するようになされる埋込可能な医療機器と、
前記埋込可能な医療機器に通信的に接続された外部システムとをさらに備え、この外部システムが、前記埋込可能な医療機器から前記虚血警報信号を受ける虚血警報信号の受信機と、前記虚血性警報信号が受信された場合に、少なくとも1つのアラーム信号及び警告メッセージを生成するように構成された表示機器とを備える、請求項1から15のいずれか一項に記載のシステム。 - 前記外部システムが、1つ又は複数のペーシング命令を受けるユーザ入力機器を備え、前記心臓保護ペーシング・シーケンスのイニシエータが、前記虚血性イベントと前記1つ又は複数のペーシング命令の前記検出に応答して、前記1つ又は複数の心臓保護ペーシング・シーケンスを始めるようになされた、請求項16に記載のシステム。
