Description
図1は、本発明を有効に実施することができるタイプの埋め込み可能医療デバイスの略図である。図1に示すように、例えば、埋め込み可能カーディオバータ/ディフィブリレータ(ICD)のような埋め込み可能医療デバイス10は、1本又は複数本のリード線6、15、及び16によって患者の心臓に結合される。コネクタブロック12は、3心腔又は4心腔において検知し、刺激するために電極を配置するのに使用される、右心室リード線16、右心房リード線15、及び冠状静脈洞リード線6の近位端を受け入れる。図1において、右心室リード線16は、右心室心臓信号を検知し、右心室においてペーシングパルス又はショックパルスを送出するために、その遠位端が右心室内にあるように配置される。これらの目的のために、右心室リード線16は、リング電極24、電極ヘッド28内に伸縮自在に搭載された伸張可能らせん電極26、及びコイル電極20を装備しており、電極のそれぞれは、リード線16の本体内の絶縁導体に接続される。絶縁導体の近位端は、デバイス10に電気接続を提供するために、リード線16の近位端で、分岐コネクタ14によって保持される対応するコネクタに接続される。

に示すように、本発明によるテンプレート生成プロセスは、テンプレート生成部(ブロック162及びブロック164)、テンプレート妥当性検査部(ブロック166及びブロック168)、並びに、テンプレート品質チェック部(ブロック170〜ブロック174)を含む。本発明によるテンプレートの生成は、ランダムアクセスメモリ226に記憶されたデジタル化EGM信号のR波を使用して、ブロック160にて自動又は手動のいずれかでマイクロプロセッサ224によって開始される。テンプレートの生成は、例えば、現在テンプレートが存在しない場合、あるいは、以下で説明するように、埋め込み可能医療デバイスにより自動で、又は、医師により手動でのいずれかで、現在のテンプレートがもはや正確でないことを認識したときに、開始される。自動テンプレート生成プロセスがブロック160で開始されると、マイクロプロセッサ224は、検知拍動のサンプルを生成し(ブロック162)、該検知拍動のサンプルから、以下で詳細に述べるサンプル収集プロセスを使用して、所定数のペーシングされない緩徐拍動(slow, non-paced beat)が識別され、次に、識別されたペーシングされない緩徐拍動からテンプレートが生成される(ブロック164)。

は、本発明の一実施形態による埋め込み可能医療デバイスのためのテンプレートの妥当性検査の流れ図である。本発明によれば、上述したテンプレート生成段階で、テンプレートとして使用するための平均R波が作成されると、テンプレートと進行中の緩徐心調律との間の一致に基づいてテンプレートの品質が評価される。特に、図6に示すように、妥当性検査プロセスは、テンプレートの妥当性検査のためにサンプル収集を始める前に、例えば、15分等の所定の期間遅延させられる(ブロック501)。遅延が終了すると、マイクロプロセッサ224は、適切なカウンタをクリアし、上述したテンプレート生成プロセスと同様に、所定の閾値より長いR−R間隔を有する2つの連続する心室検知(VS)事象が起こったか否かを判定する(ブロック504)。ブロック504において利用される閾値は、プログラム可能であり、例えば、600ミリ秒等の所与のレートとして、又は、短い頻脈間隔を所定のレートXだけ超えるTDI+X(Xは、例えば、60ミリ秒に等しい)として設定されるが、所与のレート又は所定のレートXは、プログラム可能であり、したがって、任意の所望の値に設定されることができる。
Claims (2)
- 埋め込み可能医療デバイスにおけるテンプレートを生成する方法であって、
複数の事象を検知すること、
前記複数の事象のうちの2つの連続する事象が存在するか否かを判定することであって、当該2つの連続する事象が、第1の閾値間隔より大きいR−R間隔を有する心室センス事象であるものと、
通常の鼓動の特徴を有する、前記2つの連続する事象に続く前記複数の事象のうちの所定数の事象を第1の選択事象として識別することであって、通常の鼓動の当該特徴が、心室ペース事象以外の心室センス事象、所定のレートより大きいR−R間隔を有する心室センス事象、心室ペース事象に直ちに引き続く第1の心室センス事象以外の心室センス事象、及び、心房ペース事象が直前に起こり、且つ、該心室センス事象と前記心房ペース事象の間隔が第2の閾値間隔より長い、心室センス事象であるものを含むもの、及び
前記第1の選択事象から前記テンプレートを生成すること、
を含む、埋め込み可能医療デバイスにおけるテンプレートを生成する方法。 - 前記第1の選択事象として識別された前記所定数の事象間のクロスマッチを計算することであって、それによって、クロスマッチを生成する、計算すること、及び
前記生成されたクロスマッチのうちの所定数が、所定のクロスマッチ閾値内にあるか否かを判定することであって、前記テンプレートは、前記所定のクロスマッチ閾値内にあると判定された該クロスマッチに相当する前記所定数の事象のうちの事象から生成される、判定することをさらに含む、請求項1に記載の埋め込み可能医療デバイスにおけるテンプレートを生成する方法。
