【発明の属する技術分野】
本発明は、上下左右任意の方向から被検体を撮影でき、しかも3次元画像再構成に必要なデータを収集するために被検体の周囲を回転することができるようになっているX線診断装置に関する。

【従来の技術】
多くのX線診断装置では、被検体を上下左右任意の方向から撮影できるように、広い範囲の角度付けが可能な支持器にX線管とX線検出器とが搭載されている。この支持器は、例えばC字やU字やΩ字様の形状を有しており、その形状からCアームとかUアームとかΩアームと呼ばれている。

ところでシステムとしての視野は、X線検出器の検出面の大きさ、具体的にはイメーインテンシファイアの入射窓の径によって決まる。もちろん、目的の部位全体が収まる以上の大きな視野が好ましい。しかし、視野を大きくしようとするとそれに伴ってX線検出器も大型化するので、支持器の動きが非常に制約されてしまう。これら両面を考慮して、例えば、腹部検査用の検出器では、その視野は、14[inch](≒355.6[mm])、最大でも、16[inch](≒406.4[mm])のもので構成されている。なお、近年では、22[inch](≒558.8[mm])という超大型のイメージインテンシファイアが登場しているが、支持器の動きが非常に制約されている上に、支持器も含めて非常に大型化されいる。

【発明が解決しようとする問題】
このように視野はあまり大きくできないので、撮影モードによっては様々な制約が化せられることがあった。その顕著な3つの例を以下に説明する。
(1)3次元画像再構成
近年、X線診断装置では、高速回転機構を用いて様々な角度から収集した画像データに基づいて、X線コンピュータトモグラフィ装置のような3次元画像再構成が行われるようになってきた。ところで3次元画像再構成を行うためには、被検体の周囲半周以上、少なくとも180度プラスα(α:X線ビームの広がり角)のデータを収集することが要求される(投影定理)。

ところが、イメージインテンシファイアで主流の14[inch]若しくは16[inch]程度では、被検体の幅に対して短すぎる。つまり、被検体の幅方向に関して全体像が視野(フレーム)に収まらない。そのため、現状では、3次元画像再構成は、造影剤注入前後の画像をサブトラクションしたDSA(Digital Subtraction Angiography)画像データに基づいて血管領域だけを対象に行われている。ところが特に外科的な治療を行うためには、血管系の治療であっても骨や他の臓器との関係が必要になる。このような背景より、支持器の動きを制約しないで、しかも通常撮影の際も邪魔にならないという条件をクリアしながらも、検出面を拡大することが強く望まれている。

(2)下肢血管造影検査
腹部から下肢に至る部分では、主要な大部分の血管は、体軸方向と略平行に延びている。このため下肢血管造影検査にとっては、視野が体軸方向に関して大きければ大きいほど有効である。しかし、視野を大きくすればするほど、上述したように支持器の動きが制約されてしまう。

(3)ステッピング撮影
同様に、腹部から下肢に至る部分では、血管は体軸方向と略平行に延びているので、造影検査では、原理的に、1度の造影剤注入でまかなえる。しかし、視野は上述したように最大でも16[inch]しかないので、腹部から下肢に至る広い範囲を撮影するためには、ステッピング撮影、つまり寝台を一定間隔でスライド→停止→撮影というサイクルを繰り返す撮影である。しかし、寝台スライドの急激で間欠的な動きに伴う被検体の体動によって、いわゆるモーションアーチファクト(ボケ)が発生してしまう。この体動を抑えるためには、被検体を寝台に強固に固定すればよいが、その結果として、被検体に少なからず不快感を与えてしまうことになる。

【発明が解決しようとする課題】
本発明の目的は、支持器の動きを過剰に制約しないで、視野の拡大を図ることのできるX線診断装置を提供することにある。

【課題を解決するための手段】
本発明は、X線管と、長辺と短辺を備えた長方形の検出面を有する平面型のX線検出器と、前記X線管と前記X線検出器とを被検体を挟んで対向するように支持するアームと、前記アームを少なくとも2軸に関して回転可能に保持する保持装置と、前記X線検出器を前記検出面に略垂直な回転軸回りに回転可能に支持する回転機構と、前記X線検出器の長辺と短辺とのいずれかが前記被検体の体軸に略平行になるように前記回転機構を制御する制御部とを具備することを特徴としている。

【発明の実施の形態】
以下、図面を参照して、本発明を好ましい実施形態により詳細に説明する。
図1には、本実施形態に係るX線診断装置の構成を示し、図2には、図1のガントリ部分の外観を示している。ガントリ部分1は、X線管3と平面型X線検出器5とを有している。平面型X線検出器5は、複数のX線検出素子を有している。X線検出素子としては、例えば、半導体層と、半導体層の表面に形成された電圧印加電極と、半導体層の裏面に形成された信号電極とからなるX線を直接的に電気信号に変換することの可能な半導体セルが採用される。その検出原理としては、半導体層にX線が入射すると、その電離作用により発生した電子正孔対が逆バイアスされている電極にそれぞれ引き寄せられ、これにより入射X線の強度に応じた信号電流が発生するというものである。なお、X線検出素子として、シンチレータとフォトダイオードとを組み合わせる等、他の構造を採用してもよい。

この平面型X線検出器5を構成する複数のX線検出素子は、図3及び図4に示すように、検出面63が略長方形になるように、マトリクス状にアレイされている。この検出面63は、例えば、短辺が16インチ(約406.4mm)、長辺が25インチ(約635mm)に形成されている。もちろん、この検出面63のサイズは、16×25インチに限定されないが、短辺の長さが350mm以上450mm以下であって、長辺の長さが550mm以上700mm以下であり、しかも短辺に対する長辺の比が、1.3以上、さらには1.5以上の長方形状が好ましい。

X線管3は、Cアーム51の一端に支持されている。このX線管3のX線放射窓には、被検体に対するX線の照射野を制限するために、X線絞り装置9が配置されている。Cアーム51の他端には、平面型X線検出器5が、ロッド状の回転機構13とスライド機構15とを介して、寝台11上に載置された被検体を挟んで対向する状態で支持される。回転機構13によって平面型X線検出器5は矢印Dで示すようにその検出面に対して実質的に直交する回転軸回りに回転することができるようになっている。また、スライド機構15によって平面型X線検出器5は矢印Cで示すようにその検出面の長辺の向きにスライドすることができるようになっている。

回転機構13のロッド52は、ロッド上部53とロッド下部55とに分かれており、図5に示す内部構造によって、ロッド上部53に対してロッド下部55が回転可能になっている。つまり、ロッド上部53とロッド下部55とはベアリング65を挟んで連結されており、ロッド上部53に固定されたステッピングモータ67の回転がドライブギア69を介してロッド下部55に伝達され、それによってロッド下部55が回転する。なお、ステッピングモータ67とドライブギア69との間には電磁クラッチ71が設けられていて、図3に示すクラッチ解除ボタン59がオペレータによって押されたとき、電磁クラッチ71が解除される。これにより、検出器5の回転はフリーの状態になり、オペレータは、ハンドル57を持って手動で検出器5を回転することができるものである。

この検出器5の回転角度は、ロッド下部55の回転に従って回転する従動ギア75を介してロッド上部53に固定されたロータリーエンコーダ73により検出されるようになっている。さらに、これらロッド上部53とロッド下部55との間の電力や信号の受け渡しはスリップリング77により行われ、これにより検出器5は140度以上回転することが可能になっている。

なお、図6に示すように、ロッド上部53に対するロッド下部55の回転を、トルクの大きなダイレクトドライブモータ81によって実現してもよい。

Cアーム51は、アーム保持装置7によって、直交3軸に関して回転可能に保持されている。図2では、天井走行式のアーム保持装置7を示しているが、床置式であってもかまわない。天井面に取り付けられた直交2系統の走行レール43,45には走行ベース46が設けられていて、この走行ベース46から懸垂アーム47が矢印Eに示すように回転可能な状態でぶら下げられている。この回転により、Cアーム51は大きく旋回することができる。この懸垂アーム47にはアームホルダー49が矢印Aに示すように回転可能な状態で接続されている。この回転によりCアーム51は被検体の頭頂/足尾(CRA/CAU)に関して回転及び傾斜できるようになっている。さらに、このアームホルダー49には、Cアーム51が矢印Bに示すようにその湾曲形状に沿って走行可能な状態でホールドされている。この走行により、Cアーム51は被検体の左右(RAO/LAO)に関して回転及び傾斜できるようになっている。これら3つの回転軸の回転によってCアーム51を任意の姿勢に保持できる。このCアーム51の姿勢は、3つの回転軸の角度をポジションセンサ17で検出することにより、検知され得る。

上述したX線管3、平面型X線検出器5、アーム保持装置7、X線絞り装置9、寝台11、回転機構13、スライド機構15、ポジションセンサ17は、制御部103,105,107,109,111,113,115,117によってそれぞれ駆動及び制御されるようになっている。これら制御部103,105,107,109,111,113,115,117は、システム制御部19及びオートポジショニング制御部39に接続されている。このシステム制御部19には、オートポジショニング制御部39の他に、表示装置35、操作卓37、ポジションメモリ41が接続されていて、システム制御部19の制御のもとで、オペレータによるコマンド等の入力、オートポジショニング機能、画像表示機能が実現されている。

なお、オートポジショニング機能とは、Cアーム51の姿勢に応じて、平面型X線検出器5を自動的に回転及びスライドさせて、被検体等に対する干渉を回避するのに最適な位置に配置するための便利な機能である。また、オートポジショニング機能には、オペレータが平面型X線検出器5の検出面の全域を使わないで、その検出面内の一部分の領域を使って画像データを収集するような設定をしたときに、その領域(撮像範囲)の中心にX線が垂直に入射するように平面型X線検出器5を自動的にスライドさせ、それと共に不要な被曝を避けるために当該撮像範囲だけにX線が照射するようにX線絞り装置9の開口を調整するという制御動作を実現する機能も含んでいる。このオートポジショニングのために必要なデータは、ポジションメモリ41にプリセットされている。

さらに、システム制御部19には、平面型X線検出器5で撮像したままのオリジナルの画像データを撮像制御部105を介して受け取り、保持するために画像メモリ21が接続されている。そして、この画像メモリ21に保持されたオリジナルの画像データに対して、様々な画像処理を施すために、アフィン変換部23、エッジ強調部25、階調変換部27、サブトラクション部29、さらに被検体の周囲多方向から撮像した2次元の画像データのセットに基づいて3次元画像データを再構成する再構成処理部31、その3次元画像データを対象として3次元画像処理を施すための3次元画像処理部33がシステム制御部19のもとに設けられている。

以上のように構成された本実施形態によるX線診断装置の動作について説明する。まず、平面型X線検出器5の回転動作について説明する。
(第1のモード)
上述したように、平面型X線検出器5のハンドル57に設けられているクラッチ解除ボタン59を押すことにより、電磁クラッチ71が解除され、検出器5が回転フリーの状態になる。これにより、オペレータはハンドル57を持って任意の角度に検出器5を回転することができる。なお、図6のダイレクトドライブモータ81を採用したときには、このボタン59を押すと、ダイレクトドライブモータ81が無励磁状態になって、クラッチ解除と同様に、検出器5が回転フリーの状態になる。

(第2のモード)
ガントリ部分1が設置された撮影室の隣の操作室に配置されている操作卓37により、検出器5を遠隔操作で回転することもできる。

(第3のモード)
この第3のモードは、オートポジショニング機能による検出器5の回転及びスライド動作である。例えば、オペレータが検査部位を腹部として入力すると、必要に応じて検出器5が回転し、検出器5の長辺が、被検体の体軸と平行なポジションに設定される。これにより、下肢造影検査においては、体軸方向に延びている血管に対して広い視野を確保することができる。但し、このポジションでは、検出器5が、Cアーム51の被検体の頭頂又は足尾側への傾斜を制限することがある。このような問題は、検出器5をX線管3に対して斜めに傾けることにより軽減される。この場合、X線管3と平面型X線検出器5とが対向しなくなるので、表示画像に歪みが生じるが、これはアフィン変換部23のオブリーク処理により軽減することができる。また、このとき、X線が平面型X線検出器5の検出面の範囲だけに照射されるように、平面型X線検出器5のX線管3に対する向きに応じて、X線絞り装置9の開口が自動的に調整される。

また、例えば、オペレータが検査部位を腹部以外の部位を入力したときには、RAO/LAO(患者の右/左)に関する傾斜角(回転角)θと、CRA/CAU(頭頂/足尾)に関する傾斜回転角度φとに基づいて、検出器5のポジションが変化する。例えば、Cアーム51がCRA/CAU方向に関して35゜以下という回転角であまり傾斜していない過ぎない場合には、検出器5の長辺が体軸と略平行になるように検出器5が回転される。しかし、Cアーム51がCRA/CAU方向に関して35゜を越える程、大きく傾斜している場合には、検出器5が被検体に干渉しないように、検出器5の短辺が体軸と略平行になるように、90゜回転される。

また、この装置では、検出器5の検出面の全域を使わないで、図4に点線で示しているような検出面の一部分を使って画像データを収集することが可能になっている。この場合、X線管3からのX線は、当該一部分の中心に対して斜めに入射するので、画像が歪んでしまう。これを解決するために、図7に示すように、画像データ収集用として、検出面の一部分だけが指定されたとき、そのX線管3からのX線が、当該一部分の中心に対して垂直に入射するように、検出器5がスライドするようになっている。なお、このスライドに代えて、図8(a)、図8(b)に示すように、ロッド52を検出器5の中心からずれた位置に取り付け、その90゜回転によって同様の効果を奏するようにしてもよい。

また、Cアーム51をRAO/LAOとCRA/CAUとの両方向に傾斜させているときには、検出器5の中心とアイソセンタとを結ぶ直線と寝台11の天板面とのなす角度を基に検出器5を回転する。例えば、当該角度が、35゜を越える場合、アイソセンタと検出器5の中心とを通り、且つ寝台11に垂直な平面に対して平面型X線検出器5の短辺が平行になるように、且つ長辺は当該平面に対し垂直になるようにセットされる。この回転により患者の任意の方向(例えば頭頂方向)の画像上での向き(例えば上)が異なってしまった場合、規定の方向を向くように画像処理で回転させても良い。

また、本実施形態では、被検体の周り180゜+α(αはビュー角)以上にわたって繰り返し収集した多方向の画像データに基づいて再構成処理部31で人体内部の3次元構造を表す3次元画像データを再構成することができる。ここで投影定理により人体内部の3次元画像データを再構成するためには、回転中、常に被検体の特に横幅全体が検出器5の検出面に収まっている必要がある。このために、3次元画像再構成時には、被検体の周囲を検出器5が回転する際のCアーム51の回転軸に対して、短辺が略平行になるように検出器5が設定される。これにより、被検体の幅方向に関して広い視野が確保され、回転中、常に被検体の横幅全体が検出器5の検出面に収まるようになる。

この3次元画像再構成時には、Cアーム51が高速回転しながら、180゜+αの範囲にわたって例えば1゜間隔で撮影が繰り返される。この多方向の画像データ(投影データ)は、撮像制御部105でディジタル信号に変換された後、画像メモリ21に記憶される。また同様に画像メモリ21には、X線管3と検出器5との間に何も挟まないで撮影しておいた画像データが予め記憶されている。画像メモリ21に被検体に関する画像データが蓄積されると、予め記憶されていた画像データとの間でサブトラクション処理がサブトラクション部29で行われる。サブトラクションにより得られた差分データは再構成処理部31に送られ、ここで離散化された再構成領域の再構成を行う。再構成方法の1例としては、ここではFeldkamp等によって提案されたフィルタードバックプロジェクション法で説明すると、180゜+α分の多方向の画像データに対して例えばShepp&LoganやRamachandranのような適当なコンボリューションフィルターをかける。

次に一般的には逆投影演算を行うことにより再構成データが得られる。ここで再構成領域は、X線管球の全方向へのX線束に内接する円筒として定義される。この円筒内は、例えばディテクタの1検出素子の幅に投影される再構成領域中心部での長さdで3次元的に離散化され、離散点のデータの再構成像を得る必要がある。但しここでは離散間隔の1例を示したが、これは装置やメーカーによって違うこともあるので、基本的には装置によって定義された離散間隔を用いれば良い。再構成された3次元画像は、例えばボリュームレンダリングやサーフェイスレンダリングのような適切な画像処理を3次元画像処理部33で施された後、表示装置35に表示される。

次に、例えば腹部から下肢に至る広い範囲を撮影するのに効果的なステッピング撮影時の動作について説明する。ステッピング撮影では、撮影→寝台11のスライド→停止というサイクルが繰り返される。このとき、検出器5は、その長辺が体軸と略平行になるようにセットされる。

ステッピング撮影では、例えばお腹から足首までを撮影領域として決定し、その長さと検出器5の長辺の長さ、及びX線光学系の拡大率を元にステップ数と撮影位置を決定する。この時ステッピングの回数は検出器5の長さに反比例するので、本実施形態のような長い検出器5を使用するとステッピングの回数が少なくて済む。次に足首方向からお腹まで決められた撮影位置で順次透視(線量の非常に少ない撮影)を行い、各位置での透過光を元にその位置での撮影線量を算出する。そして造影剤を注入後、その流れを追跡するように線量決めと逆の順序でお腹から足首方向へ向けて決められた撮影位置で順次撮影を行う。

またステッピングDSAと言う手技もある。この時は、線量決めの時はお腹から足首方向へ向けて行い、次に造影剤を注入しないで足首方向からお腹まで決められた撮影位置で順次撮影を行う(マスク像撮影)。最後に造影剤を注入後、その流れを追跡するようにお腹から足首方向へ向けて決められた撮影位置で順次撮影を行う。そして同じ位置で撮影した造影像とマスク像をサブトラクション(DSA:Digital Subtraction Angiography)することにより、骨や軟組織等の背景が消え、血管のみが強調された画像を得ることができる。

このように本実施形態によれば、Cアームの動きに対する制約は格段に縮小される。しかも、視野の拡大も図ることができる。

本発明は、上述した実施形態に限定されることなく、種々変形して実施可能である。

【発明の効果】
本発明のX線検出器は、従来のイメージインテンシファイアを使ったものよりも、平面型で薄い。しかも、その検出面は略長方形であり、さらに回転可能である。これにより必要に応じてX線検出器を回転して、支持機構や被検体に対する干渉を回避することができる。また、X線検出器を回転してその検出面の長辺を所望の向きに合わせれば、その向きに大きな視野を確保することができる。さらに、検出面の長辺を被検体の幅方向に合わせれば、被検体の全幅を視野に収めることができるので、3次元画像再構成の適用範囲を従来の血管だけに縛られることなく拡大することができる。また、ステッピング撮影時には、X線検出器の長辺を被検体の体軸方向に合わせることにより、ステップ回数を減らすことができる。

As described above, since the field of view cannot be so large, various restrictions may be imposed depending on the shooting mode. Three prominent examples are described below.
