Description
拍数を増して心拍出量を増加するための装置および方法を提供することである。
心臓活動に反応する信号を検知する、1つ以上のセンサと、
心筋セグメントに非励起刺激パルスを印加する1本以上の刺激電極を備える刺激探針と、
1つ以上のセンサおよび刺激探針と接続しており、1つ以上のセンサからの信号を受信し、この信号に応じて非励起刺激パルスを発生する信号発生回路とを備えている。
検知電極を含むコア部分を備え、環状部分が炭素材料を備えることが好ましい。
1本以上の刺激電極を備える刺激探針の、患者の心臓への装着と、
患者の心筋活動に反応する少なくとも1つのセンサからの信号の受信と、
信号に応じた非励起刺激パルスの発生と、1本以上の電極の少なくとも1本へのパルスの伝達とを備えた方法がさらに得られる。
Claims (37)
- 患者の心臓の心拍出量を変更するための装置であり、
心臓活動に反応する信号を検知する、1つ以上のセンサと、
心筋セグメントに非励起刺激パルスを印加する1本以上の刺激電極を有する刺激探針と、
前記1つ以上のセンサおよび刺激探針と接続しており、前記1つ以上のセンサからの信号を受信し、この信号に応じて非励起刺激パルスを発生する信号発生回路と、
を有し、
前記非励起刺激パルスは、一回拍出量を増加することで、心拍数に直接影響を与えることなく、心拍出量を変更することを特徴とする患者の心臓の心拍出量を変更するための装置。 - 患者の心臓の心拍出量を変更するための装置であり、
心臓活動に反応する信号を検知する、1つ以上のセンサと、
心筋セグメントに非励起刺激パルスを印加する1本以上の刺激電極を有する刺激探針と、
前記1つ以上のセンサおよび刺激探針と接続しており、前記1つ以上のセンサからの信号を受信し、この信号に応じて非励起刺激パルスを発生する信号発生回路と、
を有し、
前記非励起刺激パルスは、心臓細胞の収縮性と、一回拍出量とを、長期にわたり変更することによって、心拍出量を変更することを特徴とする患者の心臓の心拍出量を変更するための装置。 - 前記非励起刺激パルスは、新たな活動電位の伝播を生じさせないことを特徴とする請求項1に記載の装置。
- 前記信号発生回路が患者の身体外のユニットに内臓されていることを特徴とする請求項1から3のいずれか1項に記載の装置。
- 前記信号発生回路が埋込型ケースに内臓されていることを特徴とする請求項1から4のいずれか1項に記載の装置。
- 前記刺激パルスの使用により、心拍出量が増加することを特徴とする請求項1から5のいずれか1項に記載の装置。
- 前記刺激パルスの使用により、心拍出量が減少することを特徴とする請求項1から6のいずれか1項に記載の装置。
- 前記刺激パルスの使用により、心臓の収縮能力が増加することを特徴とする請求項1から5のいずれか1項に記載の装置。
- 前記1つ以上のセンサが心内電極を有し、前記信号発生回路が前記刺激パルスを心臓の電気的活動と同期させることを特徴とする請求項1から8のいずれか1項に記載の装置。
- 前記1つ以上のセンサが体表電極を有し、前記信号発生回路が前記刺激パルスをECG信号と同期させることを特徴とする請求項1から8のいずれか1項に記載の装置。
- 前記信号発生回路が信号内の不規則を確認し、これに応じて前記刺激パルスを制御することを特徴とする請求項1から10のいずれか1項に記載の装置。
- 前記信号発生回路が信号内のQT間隔を検出し、これに応じて前記刺激パルスを制御することを特徴とする請求項1から11のいずれか1項に記載の装置。
- 前記信号発生回路が、電圧、電流、持続時間、時間遅延、波形、波形周波数を有するパラメータのグループから前記刺激パルスの1つ以上のパラメータを変更することを特徴とする請求項1から12のいずれか1項に記載の装置。
- 前記信号発生回路が、前記非励起刺激パルスの後に、前記刺激パルスと対向する極性の別のパルスを発生し、前記パルスが前記刺激探針によって心筋セグメントに印加されることを特徴とする請求項1から13のいずれか1項に記載の装置。
- 前記1本以上の刺激電極が、少なくとも5mm2の範囲を有する心臓セグメントに対して前記刺激パルスを印加することを特徴とする請求項1から6のいずれか1項に記載の装置。
- 前記1本以上の刺激電極が、少なくとも1cm2の範囲を有する心臓セグメントに対して前記刺激パルスを印加することを特徴とする請求項15に記載の装置。
- 前記1本以上の刺激電極が、少なくとも4cm2の範囲を有する心臓セグメントに対して前記刺激パルスを印加することを特徴とする請求項16に記載の装置。
- 前記信号発生回路が、前記刺激パルスが印加される前記心臓セグメントの範囲を変更することを特徴とする請求項1から17のいずれか1項に記載の装置。
- 前記刺激探針が電極のネットを有することを特徴とする請求項1から18のいずれか1項に記載の装置。
- 前記1本以上の刺激電極が、互いに異なった時間において給電されることができることを特徴とする請求項19に記載の装置。
- 前記ネット内の電極が指定可能であるため、前記刺激パルスが印加される前記セグメントの程度が、前記ネット内の選択された電極を指定することによって制御されることを特徴とする請求項19又は20に記載の装置。
- 前記回路が、前記ネット内の個々の電極に対して、多様な異なる刺激パルスを印加することを特徴とする請求項20又は21に記載の装置。
- 前記多様な異なる刺激パルスがパルスの時間シーケンスを有することを特徴とする請求項22に記載の装置。
- 前記刺激探針が、少なくとも1本以上の刺激電極と共に心内電極を備えたハイブリッド電極を有することを特徴とする請求項9に記載の装置。
- 前記ハイブリッド電極が、少なくとも1本の前記刺激電極を含む環状部内に封入された前記心内電極を含むコア部分を有することを特徴とする請求項24に記載の装置。
- 前記環状部が炭素材料を有することを特徴とする請求項25に記載の装置。
- 前記1本以上の刺激電極が、心臓の血管内に挿入される延長電極を有することを特徴とする請求項1から26のいずれか1項に記載の装置。
- 前記1本以上の刺激電極がガラス質炭素を有することを特徴とする請求項1から27のいずれか1項に記載の装置。
- 前記1本以上の刺激電極が熱分解炭素を有することを特徴とする請求項1から28のいずれか1項に記載の装置。
- 前記1つ以上のセンサが、血流動態パラメータに応じて信号を発生する血流動態センサを有することを特徴とする請求項1から29のいずれか1項に記載の装置。
- 前記血流動態センサが血流に応じて信号を発生することを特徴とする請求項30に記載の装置。
- 前記血流動態センサが血液の酸化に応じて信号を発生することを特徴とする請求項30に記載の装置。
- 前記血流動態センサが温度に応じて信号を発生することを特徴とする請求項30に記載の装置。
- 前記1つ以上のセンサが、心臓電気的活動を検知する電極を有することを特徴とする請求項30に記載の装置。
- 少なくとも1つ以上のセンサからの信号を受信し、前記非励起刺激パルスを調整するために前記信号発生回路を制御する遠隔測定装置を有することを特徴とする請求項1から34のいずれか1項に記載の装置。
- 治療用に構成されたことを特徴とする請求項1から35のいずれか1項に記載の装置。
- 1本以上の刺激電極を有する刺激探針を有し心拍出量を変更する装置を制御する方法であって、
前記装置により、患者の心筋活動に応じた少なくとも1つのセンサからの信号を受信する工程と、
前記装置により、前記信号に応じて、一回拍出量を増加するように構成された非励起刺激パルスを発生し、前記装置により、前記非励起刺激パルスを前記1本以上の刺激電極の少なくとも1本に伝達する工程とを含むことを特徴とする方法。
