Description
The present invention relates to an electronic medical record system, and more particularly to an electronic medical record system having an incident item extraction function.
In hospitals, writing incident reports is indispensable for preventing medical accidents. The incident report is a report for reporting an incident (missing hat) that may not be an actual accident such as a medical accident or an accident, but has some possibility of danger. However, writing and managing incident reports during day-to-day operations is very burdensome for field personnel. For this reason, in recent years, various systems that support the writing of incident reports have been developed and often introduced into actual medical settings.
The most important thing in incident reporting is to report all incidents that occur. As described above, when the daily work is busy, there is a problem that incident reports are often postponed, and many postponed incidents are not described in the report. The presence of incidents that are not reported is a serious problem because risk management cannot be performed properly and can lead to medical accidents.
Conventional incident report support systems include, for example, a device that allows a user to import incident report information from a medical record database to reduce data input when creating an incident report (see, for example, Patent Document 1), or a doctor In order to prevent the nurse from overlooking part of the instruction information or making a self-judgment, the list of items that should be checked in association with the instruction information is presented together with the instruction information so There is a system provided with Patent Document 2).
Even if the conventional incident report support system as described in
The present invention has been made to solve such a problem, and an object thereof is to provide an electronic medical record system capable of improving the reporting rate of incident reports.

The present invention, an electronic medical record database for storing electronic medical records, and incident extraction rule database for storing with the rule identification information to identify it with extraction rules for searching the data described in the electronic chart, the Using the extraction rule stored in the incident extraction rule database, the electronic medical record that includes event data that matches the conditions of the extraction rule is searched from the electronic medical record stored in the electronic medical record database, and the electronic Extracting medical record information as an incident matter and adding incident matter extraction means for adding matter identification information for identifying the incident matter; information on the incident matter extracted by the incident matter extraction means; The case identification information A notification information output means for outputting a cement project notification information, for each of the project identification information, incident information contents of said event data of said incident projects extracted by the extraction rule indicates whether corresponding to incident Based on the incident information of the incident matter confirmation information for each rule identification information, incident information addition means for generating incident matter confirmation information that is input and the incident information is added to the incident matter notification information, Ru electronic medical record system der characterized in that a incident ratio calculating means for calculating an incident ratio indicating a ratio of a number corresponding to the incidents of the number of retrieved the incident Proposal by the incident Proposal extracting means So use extraction rules Since it automatically extracted incidents transactions that may incident from the electronic medical record Te, it is possible to improve the reporting rate of incident reports.
FIG. 1 is a diagram showing a configuration of an electronic medical record system according to
As shown in FIG. 1, in the electronic
On the other hand, the
Next, the operation of the electronic
FIG. 2 shows a process flow of the incident matter extraction unit 3. As shown in the flow of FIG. 2, the incident case extraction unit 3 first acquires one of the predefined extraction rules as shown in FIG. 3 from the incident extraction rule DB 4 ( Step S2).
As shown in FIG. 3, the incident
Returning to the description of FIG. 2, the electronic medical record /
FIG. 4 is an example of the extracted incident item written in the extracted
As described above, when an incident case that may be an incident is extracted by the incident case extraction unit 3 and stored in the extracted
FIG. 5 shows a processing flow of the message creation / transmission unit 6. As shown in FIG. 5, the message creation / transmission unit 6 first searches the extracted incident matter DB 5 (steps S10 and S11), and determines whether or not there is an incident matter that has not been sent (required that the transmission date is empty). If there is an unsent incident case, the data is read for each case (step S13). On the other hand, if there is no unsent incident case, the processing is terminated as it is. Next, it is determined whether the patient ID of the read incident case is the same as the patient ID in the previously read incident case history (step S14). If there is an incident case having the same patient ID, The incident case weight that was read in this time is added to the weight information of the incident case that was read this time. Is updated to a value obtained by adding the weight data (step S15). Further, the ID of related incident cases read in advance is stored as a sub ID in the incident
The incident
The incident
An example of the incident
The incident
In the above description, the example in which the incident
The incident
As shown in the flow of FIG. 7, the message reception /
As described above, the message reception /
The extracted
As shown in FIG. 9, the extracted
Incident ratio to the number of transmissions (%) = number of incidents / number of transmissions x 100
Incident ratio to the number of receptions (%) = number of incidents / number of receptions x 100
Further, in addition to the above, the extracted
For incident cases directly input by the
Also, when the user (user) selects one of the incident cases displayed on the screen and presses (clicks) the “detailed display” button shown in the screen display example of FIG. The extracted
When the “automatic weight adjustment” button shown in the screen display example of FIG. 9 is pressed, the extracted

As described above, according to the electronic
