Description

本発明は一般的には抗癌療法の分野に関する。より詳細には、本発明は有機砒素化合物および白血病などの癌治療におけるその使用方法を提供する。
白血病の治療は進歩しているにもかかわらず、大人の白血病患者の多くは依然として病気の進行により死亡しており、2001年には推定31,500の新規症例、および21,500の死亡が予測されている。無機化合物である三酸化砒素が、最近、再発性または難治性の急性骨髄球性白血病(APL)患者の治療のために承認されており、他の白血病型に対する療法としての評価が行われている。しかしながら、毒性のためその使用は限られている。
本発明は当技術分野のこれらのおよび他の欠点を克服し、抗癌特性を有する有機砒素化合物を提供する。いくつかの態様では、本発明は、以下の構造:

I.本発明
本発明は、三酸化砒素を用いる現在の治療と同様のまたはそれ以上の活性、およびより低い毒性を有する、癌治療用の多くの有機砒素化合物を提供することにより、当技術分野における欠点を克服する。より特定的には、本発明は、S-ジアルキルアルシノ-チオカルボン酸、例えば、S-ジメチルアルシノ-チオコハク酸およびS-ジメチルアルシノ-2-チオ安息香酸、ならびに癌治療におけるその使用方法を提供する。本発明はまた、S-ジメチルアルシノ-グルタチオンおよび癌治療におけるその使用方法を提供する。
20年前、多くの有機砒素剤(OA)誘導体が、この出願の共同発明者である、テキサスA&M大学のProf. Dr. Ralph A. Zingaroにより合成され、それらの物理化学特性が決定された(Chenら、1976;Rosenthalら、1980;Chenら、1980;Danielら、1978;Banksら、1979;これらの参考文献の全内容は参照として全体が本明細書に組み込まれる)。これらの化合物の1つが癌細胞に対しインビトロ活性を示した後、新規合成されたOAの多くが、その抗癌活性の評価のために国立健康研究所(NIH)に提出された。これらの化合物はP388リンパ性白血病細胞を有するマウスにおいてインビボで試験された。これらの化合物の活性は、表1に示されるように、異なる薬物で処理した(腹腔内、毎日、5日間)6匹のマウスの群、ならびに対照群の生存時間を記録することにより評価した。有意の活性に対するNIH基準は、%治療/対照(T/C)>125である。これは、薬物を受けた動物の群が対照群よりも少なくとも25%長く生存することを意味する。多くの化合物が著しい抗白血病活性を示し、いくつかが実のところ180の%T/Cに到達した。さらに、化合物は、200mg/kgの用量で使用すると毒性がなかった(三酸化砒素に対するLD50は10mg/kgである)。

三酸化砒素の使用はその毒性により制限される。一方、OAはずっと毒性が低く、無機砒素をインビボでメチル化してOAとするのは無毒化反応であると考えられる程である。OAモノメチルアルシン酸およびジメチルアルシン酸は、無機砒素の主代謝産物である(Hughesら、1998)。三酸化砒素を含む無機砒素剤は、多くの器官系、例えば心臓血管系、消化管、腎臓、皮膚、神経系および血液に様々な影響を及ぼす。無機砒素剤は特に肝臓にとって毒性があり、浸潤、中心壊死および肝硬変を引き起こす(IARC、1980:ACGIH、1991;Belilesら、1994;Goyerら、1996)。現在では、無機砒素化合物がヒトにおいて皮膚および肺発癌物質である証拠が十分存在する(Goyerら、1996)。
本発明の有機砒素剤を使用して、全ての充実性腫瘍および全ての血液癌を含む様々な癌、例えば、白血病、リンパ腫、多発性骨髄腫、骨髄異形成、または骨髄増殖性疾患を治療してもよい。OAを使用して、他の治療形態に対し難治性となっている血液癌を治療することができる。
本発明の薬学的組成物は、薬学的に許容される担体に溶解または分散させた、有効量の1つまたは複数の有機砒素誘導体、より特定的にはs-ジアルキル-チオ-酢酸、例えばMER-1、SAL1もしくはSGLU1、またはMER-1、SAL1もしくはSGLU1の塩あるいは誘導体、または追加の薬剤を含む。「薬学的にまたは薬理学的に許容される」という句は、動物、例えば、適宜、ヒトに投与した場合に、有害反応、アレルギー反応または他の不適当な反応を起こさない分子物体および組成物を示す。少なくとも1つの有機砒素剤または追加の活性成分を含む薬学的組成物の調製は、Remington's Pharmaceutical Sciences、18版、Mack Printing Company、1990(参照として本明細書に組み込まれる)により例示されているように、本開示に照らせば、当業者には周知であると考えられる。さらに、動物(例えば、ヒト)投与では、調製物は、生物学的基準のFDAオフィスにより要求されるような、無菌性、発熱性、一般的な安全性および純度基準を満たすべきであることは理解されると考えられる。

癌療法は、薬品および放射線を基本とする治療の両方との様々な併用療法を含む。併用化学療法としては、例えば、シスプラチン(CDDP)、カルボプラチン、プロカルバジン、メクロレタミン、シクロホスファミド、カンプトセシン、イフォスファミド、メルファラン、クロラムブシル、ブスルファン、ニトロスウレア、ダクチノマイシン、ダウルノビシン、ドキソルビシン、ブレオマイシン、プリコマイシン、ミトマイシン、エトポシド(VP16)、タモキシフェン、ラロキシフェン、エストロゲン受容体結合剤、タキソール、ゲムシタビエン、ナベルビン、ファネシル-蛋白質トランスフェラーゼ阻害薬、トランスプラチナム、5-フルオロウラシル、ビンクリスチン、ビンブラスチンおよびメトトレキサート、または前記の任意の類似変種または誘導変種が挙げられる。
DNAの損傷を引き起こし、広く使用されている他の因子としては、γ線、X線として通常知られているもの、および/または放射性同位体の腫瘍細胞への直接送達が挙げられる。マイクロ波およびUV-照射などのDNA損傷因子の他の形態もまた企図されている。おそらく、これらの因子は全てDNA、DNAの前駆体、DNAの複製および修復、ならびに染色体の組立ておよび維持に広範囲の損傷を与える。X線の線量は、長期にわたる(3〜4週間)毎日の線量50〜200レントゲンから単一の線量2000〜6000レントゲンまでである。放射性同位体に対する線量範囲は広範囲に変動し、同位体の半減期、放出される放射線の強度および型、ならびに新生物細胞による取り込み量に依存する。細胞に対して適用される「接触」および「曝露」という用語は、本明細書において、治療構築物および化学療法剤または放射線療法剤が標的細胞に送達され、標的細胞と直接並置されるように配置される方法を記述するために使用される。細胞死および鬱血を達成するために、両方の薬剤が、細胞を殺すかまたは細胞が分裂するのを阻止するのに有効な結合量で送達される。
免疫療法は、一般に、癌細胞を標的とし破壊する免疫エフェクター細胞および分子に依存する。免疫エフェクターは、例えば、腫瘍細胞の表面上のいくつかのマーカーに対し特異的な抗体としてもよい。抗体は、それだけで治療のエフェクターとして作用してもよく、または実際に細胞を殺す他の細胞を起用してもよい。抗体はまた、薬物または毒素(化学療法薬、放射性ヌクレオチド、リシンA鎖、コレラ毒素、百日咳毒素、など)に結合させてもよく、それだけでターゲティング薬として機能してもよい。また、エフェクターは、直接または間接的に、腫瘍細胞標的と相互作用する表面分子を有するリンパ球としてもよい。様々なエフェクター細胞が細胞傷害性T細胞およびNK細胞を含む。
さらに他の態様では、第2の治療は第2遺伝子療法であり、この場合、第1の治療薬の前後、またはそれと同時に治療用ポリヌクレオチドが投与される。遺伝子産物をコードするベクターと共に治療薬を送達すると、標的組織に対し複合された抗-過増殖性効果が得られる。
癌患者の約60%が何らかの型の手術を受ける。この手術には、予防、診断または病期分類、治療および緩和手術が含まれる。治療手術は、他の療法、例えば、本発明の治療、化学療法、放射線療法、ホルモン療法、遺伝子療法、免疫療法および/またはその他の療法などと共に使用してもよい癌治療である。治療手術には、癌組織の全てまたはその一部を物理的に除去し、摘出しおよび/または破壊する切除が含まれる。腫瘍切除は腫瘍の少なくとも一部の物理的除去を示す。腫瘍切除の他に、手術による治療には、レーザ手術、凍結手術、電気手術、および顕微鏡下手術(Mohs手術)が含まれる。本発明は、表在性癌、前癌または付随的な量の正常組織の除去と共に使用してもよいことが、さらに企図される。
下記の実施例は、本発明の好ましい態様を明らかにするために包含される。当業者であれば、下記の実施例で開示した技術は、本発明の実施においてよく機能することが発明者により発見された技術を示し、そのため本発明の実施のための好ましい様式を構成すると考えることができることを理解すべきである。しかしながら、当業者であれば、本開示に照らせば、開示した特定の態様において多くの変更が可能であり、かつ本発明の趣旨および範囲から逸脱せずに同様のまたは類似の結果が得られることを理解すべきである。
S-ジメチルアルシノ-チオコハク酸(MER1)、S-ジメチルアルシノ-サリチル酸(SAL1)、およびS-(ジメチルアルシノ)グルタチオン(SGLU1)の合成
MER-1:メルカプトコハク酸4.5gを、250ml丸底フラスコ内の100mlのグリム(1,2-ジメトキシエタン)に入れた。4mlのジメチルクロロアルシン(0.03mol)を滴下し、その後、4mlのジエチルアミン(0.04mol)を滴下した。反応混合物を20時間室温で撹拌した。ジエチルアミン塩酸塩の白色沈澱が形成され、濾過により分離した。MER1のグリム溶液を減圧下で蒸発させ、真空で大きく体積を減少させた。MER1の白色結晶を濾過により分離し、冷蒸留水で洗浄した。その後、無色結晶生成物をエタノール-水から150℃の一定融点まで再結晶化させた。
インビトロ評価のためのアッセイ法
様々なインビトロアッセイ法を使用して、本発明の砒素化合物、組成物および/または製剤に対する癌細胞の応答を決定した。アッセイした応答の中には、細胞生存、細胞周期、アポトーシス、成熟が含まれた。本発明者らはまた、本発明の砒素化合物に対する感受性に対する癌細胞のPML/RARα遺伝子の要求を評価するアッセイ法を設計した。下記で、これらのアッセイ法について説明する。
様々なヒト癌細胞を、マイクロタイタープレート上で、示された濃度のMER-1、SGLU-1およびSAL-1と共に、またはそれら無しで48時間インキュベートし、その後、スルホローダミンB染料を培養物に添加した。スルホローダミンB染料は、蛋白質結合染料であり、生細胞を標識する。結果を、未処理対照細胞と比較した場合の処理細胞の増殖%として報告する(負のデータは細胞死を示す)。
これらのアッセイ法では、白血病患者および正常ドナーの末梢血試料由来の単核細胞をFicoll Hipaque分画により分離し、DMEM完全培地に再懸濁させた。また、細胞株細胞をいくつかの場合において使用した。様々なヒト細胞株由来の悪性細胞(通常5×104細胞/ml)または白血病患者および健康なドナーの末梢血由来の単核細胞(1×106細胞/ml)をαMEMまたはRPMI1640のいずれかの中で、様々な濃度のMER-1、SAL-1、もしくはSGLU-1と共に、またはそれら無しでインキュベートした。各実験条件を3つ組で実施した。示した日数(通常3日)、MER-1、SAL-1またはSGLU-1に曝露した後、染料(MTTまたはトリパンブルーのいずれか)をウェルに添加することにより細胞生存を評価した。MTT染料はウェル中に生存細胞が存在すると変色する。MTT処理下での細胞の生存を対照細胞増殖の割合として評価した。トリパンブルー染料は死亡細胞を貫通し、生細胞を顕微鏡下で計数することができ、生存率が評価できる。
クローン形成またはコロニー形成を、(正常ドナーまたは白血病患者由来の)末梢血単核細胞を得ることにより分析した。その末梢血単核細胞は、組み換えサイトカインを含む半固体培地中に再懸濁され、96-ウェルマイクロタイタープレート中0.1ml/ウェル、4×104細胞/0.1ml密度で、4つ組で培養された。5%CO2の加湿雰囲気下、37℃で約10日間インキュベートした後、50以上の細胞で構成される細胞集合体を1コロニーとして計数する。増殖阻害を、対照(薬物無し)試料でのコロニー増殖と比較した場合のコロニー増殖の割合として評価した。
3つの異なる方法を使用して、アポトーシス経路における異なる事象をアッセイすることによりアポトーシスを分析した。本発明の砒素誘導体により誘発されるアポトーシス細胞の割合を、フローサイトメーターを使用して評価した。アポトーシスのための異なる細胞染色法を使用して、アポトーシスカスケードの異なる局面を評価した。
アネキシンVは細胞膜の外層上でホスファチジルセリンを発現する細胞に結合し、ヨウ化プロピジウムは易感染性細胞膜を有する細胞の細胞DNAを染色する。これにより、生細胞(どちらの蛍光色素でも染色されない)はアポトーシス細胞(アネキシンVによってのみ染色される)および壊死細胞(アネキシンおよびPIの両方により染色される)から区別される。
示された時間、砒素誘導体で処理した後の、ミトコンドリア膜電位の変化を評価するために、マイクロモル濃度以下のMito Trackerプローブ中で細胞をインキュベートした。Mito Trackerプローブは原形質膜を横切って受動的に拡散し、活性ミトコンドリア中に蓄積する。細胞を2つの色で染色した:Mito Tracker Red CMXRos(分子プローブ)およびMito Tracker Green FM (分子プローブ)。細胞をPBS中で洗浄し、Mito Tracker染料で染色し、37℃で1時間、暗闇でインキュベートした。CMXRosはミトコンドリア膜電位によりミトコンドリア内に組み入れられ、チオール残基と反応し、共有結合のチオールエステル結合を形成する。MitoTracker Green FM染料は、ミトコンドリア膜電位とは関係無くミトコンドリア内に選択的に蓄積し、ミトコンドリア量を決定するのに有益なツールとなる。
フローサイトメトリーによりカスパーゼ活性をモニタするために、蛍光発生基質PhiPhiLux G1D1(Oncoimmunin)を使用した。PhiPhiLux G1D1は生細胞中のカスパーゼ3およびカスパーゼ3様活性の検出および測定のための基質である。示された時間、本発明の砒素誘導体により処理した後、細胞をPBS中で洗浄し、5μlの基質溶液中に再懸濁させ、37℃で1時間、暗闇でインキュベートした。インキュベーション後、細胞を洗浄し、フローサイトメトリー分析数分前に、PIを添加し、分析中に壊死細胞を排除した。
細胞周期を以下の通り分析した:本発明の異なる砒素化合物と共に72時間インキュベートした後、細胞(1×106)をPBS中で2度洗浄した。細胞ペレットを、低張溶液(RNAse溶液、Triton X-100、クエン酸ナトリウム、PEG)とPI(25μg/ml)を含む染色溶液中に再懸濁させた。細胞を15分間、室温の暗闇でインキュベートし、その後、CellQuestプログラム(Becton-Dickinson)を用いフローサイトメトリーにより分析した。
ヒト急性前リンパ球性白血病細胞株NB4を使用して本発明の砒素剤の白血病細胞に対する効果を試験した。フィコエリトリン結合抗CD11bモノクローナル抗体(Becton-Dickinson)を、成熟骨髄球のマーカーとして使用した。薬物と共に72時間インキュベートした後、細胞をPBS中で洗浄した。その後、1×106細胞/mlの密度の細胞を暗闇の室温で、15分間、1:10希釈でモノクローナル抗体と共にインキュベートした。インキュベーション後、細胞をPBS中で洗浄し、ペレットを500μlのPBS中に再懸濁させた。非特異的結合を排除するために、適当な同形対照を同じように調製した。フローサイトメーターを用いて細胞を分類し、CellQuest Document Analysisを用いて分析した。
三酸化砒素は急性前リンパ性白血病の治療薬として承認されており、主にPML/RARα遺伝子および蛋白質の発現によりAPL細胞を死亡させる。白血病細胞中のPML/RARα融合蛋白質の存在が、白血病細胞のSGLUおよびMER1への観察された感受性に寄与するかどうかを確立するために、本発明者らは以下の系を使用した:三酸化砒素に対し耐性があることが知られているU937細胞をPML/RARα遺伝子でトランスフェクトした。トランスフェクトした細胞はU937/PR9と呼ばれ、Michael Andreeffにより寄贈された(M.D.Anderson癌センター)。PML/RARα遺伝子は亜鉛が存在すると機能的になった。U937/PR9細胞株におけるZn2+により誘発されるPML/RARα遺伝子の発現はGrignaniら(1993)において記述されている。PML/RARα発現を確立するために、細胞を0.1mMのZnSO4で3時間処理し、その後に砒素化合物を72時間添加した。PML/RARα発現は典型的には細胞に亜鉛を添加した後約3時間で確立され、48時間安定である。
MER1、SAL1およびSGLU1の抗癌活性のインビトロ評価
MER1の抗白血病活性は、6つの異なるヒト白血病細胞株に対する3日MTTアッセイ法/トリパンブルー排除法により評価されている:AML2、AML3およびHL60(AML由来細胞株)、NB4(APL由来細胞株)、K562(CML-BP由来細胞株)およびKBM7(AML由来細胞株)。MER1はNB4細胞に対し最も効果的であり、IC50(未処理対照細胞と比較して、細胞の50%が生存する濃度)は1μMであった(図1)。MTTアッセイ法によるAML2細胞およびKBM7細胞の分析、ならびにトリパンブルーアッセイ法によるAML2細胞(図2を参照のこと)、AML3細胞、K562細胞、およびHL細胞の分析を含む、他の細胞株のMER1処理では、1.5〜4μMのIC50が示された。この活性はこれらの細胞株に対する三酸化砒素の活性と同様であった(三酸化砒素活性の例を図1および図2に示す)。MER1は、国立健康研究所(NIH)により抗癌活性についてもスルホローダミンBアッセイ法を用いて60腫瘍細胞株のパネルに対しインビトロで試験された(図3)。化合物は、様々な腫瘍細胞株に対し、特に試験した白血病細胞に対し低い濃度で活性の証拠を示した。1μMのMER1濃度では、試験した6種の白血病細胞株全ての増殖が著しく阻止された(<20%増殖;図3、第1パネル)。
悪性および正常血液細胞に対するMER1およびSGLU1の毒性決定
本発明者らはまた、5人の白血病患者(AMLが3人、CML-BPが1人、およびALLが1人;図9〜13)由来の血液単核細胞(>80%芽球)に対してMER1を試験した。短期細胞培養では、MER1は三酸化砒素と同じくらい効果的であった(例を図9、10および12に示す)。さらに、正常末梢血単核細胞に対するMER1の毒性を4名の健康ドナー由来の試料で評価した。MTTアッセイ法による短期細胞懸濁液培養では、MER1は白血病患者由来の悪性細胞に比べ正常細胞に対しては毒性が低かった(図14)。最も重要なことに、長期クローン形成アッセイ法では、MER1は三酸化砒素に比べ正常細胞に対し毒性が低かった(図15)。
MER1の製剤化および安定性
MER1はリン酸緩衝生理食塩水に溶解すると少なくとも2ヶ月は安定であり、溶液はこの期間中、実施したインビトロ実験において同じレベルの細胞毒性活性を維持することを示すデータが得られた(図19)。さらに、MER1およびSGLU1の詳細な薬学的評価を実施した。
MER-1は、臨床的状況での投与が許容される十分な溶解性および安定性を有することが見出された(下記データを参照のこと)。MER-1はまた、動物試験および可能であれば前期第I相試験において使用するため、溶液を即座に混合することができるように十分安定である。しかしながら、溶液安定性は、より大規模な臨床試験において使用し、長期の貯蔵が必要とされる市場で流通させるため、より大きなバッチの液体剤形を製造するのに十分ではない。使用するときに再構成される凍結乾燥剤形が、これらの用途では企図されている。そのような凍結乾燥組成物の調製は当技術分野では周知である。
MER-1は約15mg/mLの水溶解度を有する。0.1Nの水酸化ナトリウムを使用してpH6に調整することにより、約150mg/mLまでのより高いMER-1濃度が達成できる。エタノール中では、MER-1は100mg/mlを超える溶解度を有する。
MER-1の水溶液の固有pH値は以下の通りである:

様々なpH値の効果を、0.9%塩化ナトリウム注入薬における10mg/mL濃度で評価した。pH2.3(固有pH)および、水酸化ナトリムを用いてpH5、7.1および8.5に調整した試料を、冷却下3ヶ月の期間にわたり評価した。pH5の試料は、3ヶ月後、最初の濃度の約89%が保持され、良好な安定性が証明された。pH7.1および8.5の溶液は、14日後、それぞれ、約92%および96%を保持したが、その時点が過ぎると90%未満に落ちた。pH2.3試料は7日間安定であったが、その時点を過ぎると沈澱が生じた。表2を参照のこと。

SGLU-1は約60mg/mLの水溶解度を有する。0.1Nの水酸化ナトリウムを使用して溶液pHを上げることにより、より高いSGLU-1濃度が達成できる。しかしながら、薬物はアルカリ環境で不安定であると思われる。SGLU-1はエタノールに不溶である。

様々なpH値の効果を、0.9%塩化ナトリウム注入薬において2.5mg/mL濃度で評価した。pH3(固有pH)および、水酸化ナトリムを用いてpH5および7に調整した試料を、冷却下30日の期間にわたり評価した。pH5の試料は、30日後、約90%の濃度が保持され、わずかに良好な安定性が証明された。pH3および7の溶液は、それぞれ、約84%および82%を保持した。表4を参照のこと。
MER1、SAL1およびSGLU1に関する機序
アポトーシスの誘発、細胞周期への効果、成熟の誘導、および異常PML/RARα融合蛋白質の分解はすべて、三酸化砒素の作用機序であることを示す。本発明者らは、HL60ヒト白血病細胞においてアポトーシスを誘発するMER1の可能性を調べた(アッセイ時間1〜3日)。アポトーシスの誘発は、生存細胞の割合の減少に密接に追従した(図20、21および22)。MER1およびSGLUの両方を使用した追加の試験により、これらの化合物によるアポトーシスの誘発(アネキシンV染色)はミトコンドリア膜電位の変化(CMXRos染色)およびカスパーゼ活性の変化(PhiPhiLux染色)が関係することが確認された。図23A、23B、23C、23D、23Eおよび23Fを参照のこと。
MER1、SAL1およびSGLU1の治療能力のインビボ評価
ヒト白血病の動物モデルは、ヒト白血病細胞を有する重症複合型免疫不全(SCID)マウスにより代表される。このモデルは、患者において見られる様式と同様の様式で動物においてヒト白血病の増殖が可能であるという点で独特である。このモデルにより、異なる投与レベルおよび投与計画で新規薬物のインビボ効能を迅速に試験する機会が得られる。さらに、動物の生存がモニタできるだけでなく、疾患の蔓延パターンに対する治療の効果もモニタできる。SCIDマウスの治療は典型的には、ヒト白血病細胞を接種後2日に開始される。1つのヒト白血病細胞株を注入したSCIDマウスにおける最初のインビボ実験で、他のマウスモデルならびに最初の人体試験のためのMER1、SAL1またはSGLU1の用量および計画が決定される。
第1のインビボ実験は4群のSCIDマウスを必要とした。1群あたり5匹のマウスに、異なる型のヒト白血病細胞を腹腔内接種した:HL60(AML)、KBM5(CML-BP)、KBM7-急性骨髄性白血病、およびZ119(ALL)。HL60およびKBM5細胞は優れた移植を示した:HL60群では、接種後31日および36日以内に全てのマウスが死亡し、KBM5群では、34日および36日以内に死亡した。ヒトHLA-DQαのDNA配列に対しPCRを実施することにより移植を確認した(全てのマウス由来の全ての組織において試験は陽性であった)。100日目では、KBM7では5匹のマウスのうち依然として4匹が、Z119群では5匹中5匹が生存していた。その日、全てのマウスを屠殺し、HLA-DQαに対するPCRにより組織分析を行った。試験は陰性であり、白血病細胞の移植の欠如が示された。計画した治療研究には、同じ型の他の細胞株が必要であると考えられる。
毒性試験では、免疫適格性Swiss Websterマウスを使用した。本発明者らは、三酸化砒素のLD50濃度が10mg/kgであることを確認した。
SGLU1の毒性を試験するためにSwiss Websterマウスに対し2つの試験を実施した。第1の試験では、SGLU1を178mg/kg;285mg/kg;および357mg/kgの用量でIP経路を介して投与した。毒性はマウスの死亡数により測定した。マウスは、SGLU1の178mg/kgおよび285mg/kg用量に対し良好に耐性を示すことがわかった。この試験データを表6にまとめる。
MER-1の毒性を試験するためにSwiss Websterマウスに対し2つの試験を実施した。第1の試験では、MER-1を71mg/kg;107mg/kg;および143mg/kgの用量でIP経路を介して投与した。毒性はマウスの死亡数により測定した。マウスは、MER-1の71mg/kgおよび107mg/kg用量に対し良好に耐性を示し、死亡数は0であることがわかった。この試験データを表8にまとめる。
上記実験と同様に、SAL1の簡単な毒性試験により、SAL1に対するLD50濃度が50mg/kgであることを立証した。
MER1、SAL1およびSGLU1の薬物動態学
MER1、SAL1およびSGLU1の薬物体内動態を、尾部血管を介する静脈内投与後のマウスにおいて評価する。以前に決定したMTDに近い用量を最初に調べる。血液試料は、薬物投与後、異なるサンプリング時点で採取する(0(前)、5、10、15、30、45、60分および2、3、4、6、8、12、16、24、48、72時間)(8マウス/時間点)。血液採取では、マウスをCO2吸入により安楽死させ、首を落とし、放血により血液採取する。ヘパリンを含む試験管内に血液試料を採取し、遠心分離し、血漿分離し、分析まで-80℃で保存する。試験を繰り返し、血漿限外濾過液を、Amicon Centrifreeマイクロパーティションユニット内で2000g×20分の遠心分離により回収する。分析まで限外濾過液を-80℃で保存する。選択した群において、様々な組織を検死後収穫し、組織体内動態の分析のために凍結した。血漿および限外濾過液試料中の砒素量を黒鉛炉(フレームレス)原子吸光分析により測定する。測定した薬物濃度を部門別に分析し、薬物動態学的パラメータを得る。
毒性試験

HPLC分析法開発および確認
有機砒素剤の使用に関する方法開発および確認にHPLCを使用する。HPLC法は、標準曲線および線形性、再現性(最低10注入)、感度(最低定量化濃度;最低検出可能濃度)、精度(例えば0.025mg/mL、0.1mg/mL、1mg/mLの3つの別個に調製した溶液)、熱、塩基性溶液、酸性溶液およびH2O2による意図的な変性、ならびに完全な薬物、バルク不純物、および開始材料に対するピーク規定、ならびに生成物変性を含む。バルク原料薬物を参照基準ロットにおいて、純度のHPLC分析、乾燥による損失、旋光度、融点および外観により分析する。
剤形開発
薬理学研究所(Pharmacology Laboratory)により開発された製剤化溶媒系に従い、有機砒素剤の調剤を開発する。これには、潜在的な水性溶剤中での安定性および濾過に対する安定性、さらなる開発のための標的濃度の選択、必要であれば浸透圧およびpH試験ならびに調節、パッケージおよび密閉構造の選択、熱安定性の決定(オートクレーブ)、外観および微粒子量の試験、ならびに標的pH値および許容される標的濃度範囲の決定が含まれる。
臨床試験
この実施例は、本発明の砒素化合物、MER1、SGLUおよびSAL-1、ならびに組成物またはその製剤を使用するヒト治療プロトコルの開発に関する。これらの組成物は、白血病および充実性癌および腫瘍の他の形態を含む様々な癌の臨床治療において使用される。

R4は-OH、-H、-CH3、またはグルタミン置換基であるか;または
R3およびR4は、C1およびC2と非置換もしくは置換芳香環を形成し;かつ
R5は-OH、またはグリシン置換基である。 - R5が-OHである、請求項14記載の使用。
- R3およびR4が、C1およびC2と非置換もしくは置換芳香環を形成する、請求項14または15記載の使用。
- XがSである、請求項14から16のいずれか一項記載の使用。
- R1およびR2が両方ともメチルである、請求項14から17のいずれか一項記載の使用。
- R5がグリシン置換基である、請求項14記載の使用。
- R4が-OH、-H、または-CH3である、請求項14から19のいずれか一項記載の使用。
- 化合物がピリジン塩酸塩と複合体を形成している、請求項14から20のいずれか一項記載の使用。
- 化合物が薬学的担体をさらに含む製剤として提供され、製剤のpHが5から7である、請求項14から24のいずれか一項記載の使用。
- 他の薬剤が、全トランス-レチノイン酸、9-シス-レチノイン酸、Am-80、およびアスコルビン酸から選択される、請求項14から25のいずれか一項記載の使用。
- 他の薬剤が、化学療法薬である、請求項14から25のいずれか一項記載の使用。
- 他の薬剤が、シスプラチン(CDDP)、カルボプラチン、プロカルバジン、メクロレタミン、シクロホスファミド、カンプトセシン、イフォスファミド、メルファラン、クロラムブシル、ブスルファン、ニトロスウレア、ダクチノマイシン、ダウノルビシン、ドキソルビシン、ブレオマイシン、プリコマイシン、ミトマイシン、エトポシド(VP16)、タモキシフェン、ラロキシフェン、エストロゲン受容体結合剤、タキソール、ゲムシタビエン、ナベルビン、ファネシル-蛋白質トランスフェラーゼ阻害薬、トランスプラチナム、5-フルオロウラシル、ビンクリスチン、ビンブラスチン、およびメトトレキサート、またはそれらの任意の類似変種もしくは誘導変種から選択される化学療法薬である、請求項27記載の使用。
- 他の薬剤が、免疫療法剤である、請求項14から25のいずれか一項記載の使用。
- 他の薬剤が抗体である、請求項29記載の使用。
- 抗体が薬物または毒素に結合されている、請求項30記載の使用。
- 薬物または毒素が、全トランス-レチノイン酸、9-シス-レチノイン酸、Am-80、およびアスコルビン酸から選択される、請求項31記載の使用。
- 薬物または毒素が、化学療法薬、放射性ヌクレオチド、リシンA鎖、コレラ毒素、および百日咳毒素から選択される、請求項31記載の使用。
- 薬物または毒素が、シスプラチン(CDDP)、カルボプラチン、プロカルバジン、メクロレタミン、シクロホスファミド、カンプトセシン、イフォスファミド、メルファラン、クロラムブシル、ブスルファン、ニトロスウレア、ダクチノマイシン、ダウノルビシン、ドキソルビシン、ブレオマイシン、プリコマイシン、ミトマイシン、エトポシド(VP16)、タモキシフェン、ラロキシフェン、エストロゲン受容体結合剤、タキソール、ゲムシタビエン、ナベルビン、ファネシル-蛋白質トランスフェラーゼ阻害薬、トランスプラチナム、5-フルオロウラシル、ビンクリスチン、ビンブラスチン、およびメトトレキサート、またはそれらの任意の類似変種もしくは誘導変種から選択される化学療法薬である、請求項33記載の使用。
- 抗体が、癌胎児抗原、前立腺特異抗原、泌尿器腫瘍関連抗原、胎児性抗原、チロシナーゼ(p97)、gp68、TAG-72、HMFG、Sialyl Lewis抗原、MucA、MucB、PLAP、エストロゲン受容体、ラミニン受容体、erbB、およびp155から選択される腫瘍マーカーを標的とする、請求項31記載の使用。
- 癌が、脳癌、肺癌、肝臓癌、脾臓癌、腎臓癌、リンパ節癌、小腸癌、膵臓癌、血液細胞癌、骨癌、結腸癌、胃癌、乳癌、子宮内膜癌、前立腺癌、精巣癌、卵巣癌、中枢神経系癌、皮膚癌、頭頸部癌、食道癌、および骨髄癌から選択される、請求項14から25のいずれか一項記載の使用。
- 癌が血液癌である、請求項36記載の使用。
- 癌が、白血病、リンパ腫、多発性骨髄腫、骨髄異形成、骨髄増殖性疾患、および不応性白血病から選択される、請求項37記載の使用。
- 癌が急性前骨髄球性白血病である、請求項38記載の使用。
- 癌が不応性白血病である、請求項38記載の使用。
- 化合物および一つまたは複数の他の薬剤が同時に投与される、請求項14から25のいずれか一項記載の使用。
- 一つまたは複数の他の薬剤が、化合物の投与前または投与後の5分以内から48時間以内に投与される薬剤である、請求項14から25のいずれか一項記載の使用。
- 一つまたは複数の他の薬剤が、化合物の投与前または投与後の5分以内から1時間以内に投与される薬剤である、請求項41記載の使用。
- R5が-OHである、請求項45記載の使用。
- R3およびR4が、C1およびC2と非置換もしくは置換芳香環を形成する、請求項45または46記載の使用。
- XがSである、請求項45から47のいずれか一項記載の使用。
- R1およびR2が両方ともメチルである、請求項45から48のいずれか一項記載の使用。
