(1.発明の分野)
本発明は、一般に、特に尿失禁の処置のための、医療デバイス、方法、およびシステムに関する。
本発明は、失禁を処置するための改善されたデバイス、システムおよび方法を提供する。本発明は、しばしば、コラーゲン性の支持組織(例えば、骨盤内筋膜)の堆積を加熱し、この堆積は、代表的に、約100mm3と約800mm3との間である。これらの体積の組織は、コラーゲン性組織の充分な収縮をもたらすために充分な時間にわたって加熱され得、例えば、この体積を、約70℃以上の温度まで約30秒間以上加熱することによる。代替の処置は、この体積を、約65℃以上に約100秒間以上加熱するか、または約75℃以上まで、約10秒間以上加熱し得る。このような加熱は、壊死を誘導し得るが、この加熱はしばしば、組織が治癒後に連続的な促進支持を提供するように、切除を抑止するために制限される。いくつかの実施形態において、プローブ本体は、例えば、内視鏡プローブを骨盤内筋膜に隣接して身体内に導入することによってか、または膣壁からフラップを外科的に取り出すことによって、骨盤内筋膜と直接係合し得る。あるいは、このプローブ本体は、加熱が(例えば)膣壁を通して提供されることによって、子宮内筋膜から離れ得る。いずれの場合においても、組織貫入電極が、骨盤内筋膜を加熱するために使用され得る。
本発明は、一般に、身体の組織によって提供される構造的支持を増強する、特に、失禁を処置するための、方法、デバイス、およびシステムを提供する。本発明の技術は、一般に、プローブから骨盤支持形のコラーゲン組織内へとエネルギーを方向付ける工程を包含する。このエネルギーは、しばしば、コラーゲン組織の収縮を引き起こす。いくつかの実施形態において、瘢痕組織の形成および/または加熱プロセスは、この組織の構造的強化を引き起こし得る。
Claims (18)
- コラーゲン性の骨盤組織を有する患者の失禁を処置するためのシステムであって、該システムは、以下:
該コラーゲン性の骨盤組織と整列可能なプローブ本体であって、該プローブ本体によって支持される1つ以上のレシーバ要素および少なくとも1つのエネルギー送達要素を含み、該少なくとも1つのエネルギー送達要素が、該整列したプローブ本体から、少なくとも300mm3の処置体積のコラーゲン組織を、少なくとも70℃の温度まで、少なくとも30秒間にわたって加熱することが可能であり、その結果、該コラーゲン性の骨盤組織が、自制に寄与する、プローブ本体;および
尿道の軸に沿って処置体積の位置を位置合わせするように該尿道内に配置可能なガイド本体であって、トランスミッタ要素および該トランスミッタ要素を該ガイド本体に沿った所望の位置で該トランスミッタ要素を配置するための構造の調節手段を含むガイド本体を備え、
ここで、該ガイド本体のトランスミッタ要素に対する該プローブ本体の1つ以上のレシーバ要素を整列することが該尿道に対する既知の位置で該処置体積を位置決めする、システム。 - 中間組織が、前記プローブ本体と前記処置体積との間に配置される、請求項1に記載のシステム。
- 前記プローブ本体によって支持される、少なくとも1つの冷却要素をさらに備え、それによって、前記処置て体積を加熱している間に前記中間組織の冷却を提供する、請求項2に記載のシステム。
- 前記少なくとも1つのエネルギー送達要素が、複数の電極を備え、該電極が、少なくとも20mmの幅および8mm未満の長さを有する、請求項1〜3のいずれか1項に記載のシステム。

- 前記少なくとも1つのエネルギー送達要素が、1対の細長電極を備える、請求項1〜3のいずれか1項に記載のシステム。
- 前記少なくとも1つのエネルギー送達要素が、複数の組織貫入要素を備える、請求項1〜3のいずれか1項に記載のシステム。
- 前記組織貫入要素が、針電極、刃電極、平坦な電極、C字型電極、螺旋形状の電極、または組織貫入電極先端を備える、請求項7に記載のシステム。
- 前記組織貫入要素が、2〜20個の組織貫入電極のアレイを備える、請求項7に記載のシステム。
- 前記組織貫入要素が、0mm〜8mmまでの範囲内の距離だけ、組織係合表面から延び、そして0.035インチ(0.889mm)〜0.125インチ(3.175mm)の範囲の直径を有する、請求項7に記載のシステム。
- 前記組織貫入要素の近位部分が絶縁されている、請求項7に記載のシステム。
- 前記組織貫入要素が、拡張可能な電極を備える、請求項7に記載のシステム。
- 前記ガイド本体が、膀胱または膀胱頚部の軸方向位置を識別するための該ガイド本体の遠位端に配置された膨張可能なバルーンをさらに備える、請求項1〜12のいずれか1項に記載のシステム。
- 前記処置体積が、2〜8mmの範囲内の距離だけ前記整列したプローブ本体から離れた組織を含む、請求項1〜13のいずれか1項に記載のシステム。
- 前記処置体積が、少なくとも1cmだけ尿道から離れており、該処置体積が、該尿道から右側または左側に横方向にずれている、請求項1〜14のいずれか1項に記載のシステム。
- 前記処置体積が、尿道に沿って延びる長さ配向、前記コラーゲン組織と該プローブ本体との間に延びる深さ配向、および該処置体積の長さより大きい幅を有する、請求項1〜15のいずれか1項に記載のシステム。
- 前記処置体積が、300mm3 と800mm3との間のコラーゲン組織を含む、請求項1〜16のいずれか1項に記載のシステム。
- 前記少なくとも1つのエネルギー送達要素が、双極無線周波数エネルギーの付与によって、組織の前記処置体積を加熱する、請求項1〜17のいずれか1項に記載のシステム。
