本発明は医療器具の生体管内への導入用具内部の滞留物質を吸引するための治具に関する。
【従来の技術】
一般に、カテーテルはガイドワイヤに導かれて血管などの管状器官内に挿入され、その先端を目的の部位に到達させた後、例えば造影剤などを投与して各種の検査を行ったり、内部に鉗子などを通して患部の組織を採取したり、ステントなどの部材を挿入し留置するのに使用されている。

これらのガイドワイヤやカテーテルを血管内へ導入するための用具であるシースイントロデューサーは、図2に示すようにカテーテルなどの挿通抜去時に血液の流出がない体液流失防止用シール手段(止血弁)6が設けられており、カテーテルの取り替え交換などが容易にできるようになっている。

しかし、長い時間カテーテルやガイドワイヤを血管内に留置した場合、カテーテルやガイドワイヤの表面には血栓層が生成する。そのままカテーテルやガイドワイヤを抜去すると、この止血弁を通過する際にカテーテルやガイドワイヤ表面に付着している血栓の殆どが止血弁で制止され、ハブ内部8に滞留する。従って、抜き出されたカテーテルやガイドワイヤの表面に血栓付着が認められな場合でも、実際には血栓が付着している事が多いのである。カテーテルやガイドワイヤを抜去した後、シースイントロデューサーもすぐに抜去する症例では、シースイントロデューサーハブ内部8に滞留する血栓が体内に戻る恐れはないが、シースイントロデューサーをそのまま長時間留置し、再びカテーテルを挿入する場合は、前記シースイントロデューサーハブ内部8に滞留する血栓を体内に押し出す危険がある。通常は留置中にシースイントロデューサー内の血液が凝固しないように、シースイントロデューサーハブ5から分岐しているサイドポートチューブ1より、滞留血液を一旦吸引除去した後、同所よりヘパリンなどの血液の抗凝固剤、血栓溶解剤などをシース内部に充填するなどの血液凝固防止策が施されている。

しかし、上記のような手段では、シースイントロデューサーハブ内部8に堆積滞留する血栓を除去するのにかなり時間を必要とし、血栓溶解も不完全であるなどの問題点がある。
このような状況にありながら、これまでシースイントロデューサー内部8に滞留する血栓を吸引除去するためのデバイスはない。

シースイントロデューサーなどの生体管内への導入用具内部の血栓などの滞留物質を容易に、安全に、且つ確実に吸引除去する治具を提供する。

【課題を解決するための手段】
本発明者は鋭意研究を重ねた結果、以下の構成からなる医療用具が上記課題を解決し、目的を達成することを見出し、本発明の完成に至った。すなわち本発明は、シリンジと組み合わせて、シースイントロデューサーのハブ内部に滞留する血栓を吸引する血栓吸引具であって、シリンジ先端との嵌合部を有する本体部とシースイントロデューサー内部に挿入する挿入部を備え、少なくとも前記血栓吸引具の一部が透明であり、挿入部の内径が1.2〜4mmであることを特徴とする血栓吸引具である。

【発明の実施の形態】
以下、本発明の実施の形態を図示例と共に説明する。
図3は本発明の実施の形態を示している。29は吸引した血栓を確認する本体部で、30はシースイントロデユーサーの止血弁ゴムを貫通してハブ内部に挿入される挿入部であり、31はシリンジとの嵌合部である。

本発明の血栓吸引具は、図4に示した様にして使用される。すなわち、嵌合部31にシリンジを装着した後、シースイントロデユーサーの止血弁ゴム6を貫通して血栓吸引具の挿入部30をハブ内部に挿入し、ハブ内部あるいはその近辺にある血栓15、16を吸引する。
該血栓吸引具は、後述する図7または8に示す先端部を備え、挿入部30は本体部に比較して細くなっている。該血栓吸引具の全体又は少なくとも一部は透明又は半透明の材質からなる視認領域を有している。

該血栓吸引具の挿入部30の内径は1〜4mmであり、好ましくは1〜2mmで、より好ましくは1.2〜1.5mmである。また、外径は5mmまで可能であるが、より好ましくは3〜4mmである。

挿入部の先端部は図5(A)に示すごとく、テーパ状、面取りあるいは丸みなどをつけても良い。また、図5(B)に示すごとく、大きな血栓を引き裂きながら吸引させることができる様に1以上のスリットを有していてもよい。スリットの幅は、吸引部を構成する管の直径の1/4から2/3とし、深さは同様に1/4から3/4の範囲とすることが望ましいが、この値に限定されるものではない。また、やや大きな血栓であっても全体を余すことなく吸引するように図5(C)のごとく、先端部の内径が、末広がり状、すなわち吸引口から手元側に向かって縮径していてもよい。その角度は特に限定されないが、30度から60度の範囲が好ましく、より好ましくは、40度から50度である。

本発明血栓吸引具は図7の様に挿入部30を含む本体部と嵌合部が分離可能であるものも含む。

本吸引補助具は好ましくは、ポリプロピレン樹脂(PP)、ポリカーボネート樹脂(PC)、ポリエチレン樹脂(PE)、ポリエステルチレンテレフタレート樹脂(PET)、アクリル樹脂、アクリルスチレン樹脂(AS)、塩化ビニル樹脂(PVC)、ポリメチルペンテン樹脂(TPX)などの透明、又は半透明の材料を用いて作られる。用途を間違えなくする識別のため、着色してもよい。

透明ハブを有するシースイントロデユーサーのハブの透明材質としては、ポリエチレンテレフタレート樹脂(PET)、ポリカーボネート樹脂(PC)、ポリアミド樹脂(PA)、ポリメチルペンテン樹脂(TPX)、ポリプロピレン樹脂(PP)等を挙げることができる。

分岐管の材質は医療用として用いられるものであれば特に限定されるものではないが、好ましくは血栓生成しにくい材料表面を有するものが良い。軟質ポリ塩化ビニール樹脂、ポリウレタン樹脂、1−2ポリブタジエン樹脂などが好ましく、内面を抗血栓性や水潤滑性などのコーティングしたものでも良い。

また、シースチューブおよびダイレーターの材質は、テトラフルオロエチレンヘキサフルオロプロピレン樹脂(FEP)、エチレンテトラフルオロエチレン共重合樹脂(ETFE)、パーフルオロアルコキシ樹脂(PFA)、ポリテトラフルオロエチレン樹脂(PTFE)、ポリビニルフルオライド樹脂(PVDF)等のフッ素樹脂や、ナイロン11、ナイロン12などのポリアミド樹脂、ポリエーテルブロックアミド樹脂(PEBA)、またはポリエチレン(PE)等のオレフィン系樹脂を使用することができる。これらの材質は多層に形成したり、2種以上の樹脂を組合せ形成してもよい。さらに、シースチューブにあっては、金属線や繊維等で直線状やメッシュ状あるいはコイル状に形成された補強層が設けられてもよい。また、シースチューブ、ダイレーターの先端部は金、白金、タングステン、ビスマス等のX線不透過物質を含んでいてもよい。

シースチューブキンク防止部材13の材質としては、軟質塩化ビニール樹脂(PVC)、ポリエーテルブロックアミド樹脂(PEBA)、ポリウレタン樹脂(PU)、ポリプロピレンエラストマー樹脂、スチレン系エラストマー樹脂等を挙げることができる。
