本発明は、肩補綴具組立体であって、中央管状ブッシングを設けられているプレートによって形成されている、関節窩に固定する部材を伴う後部関節窩エレメント（metaglenoid element）と、後部関節窩エレメント上に嵌められるべく構成されている関節窩球とを具備しているタイプのものに関する。関節窩球は、この関節窩球に対して自由に回転可能である軸方向ねじを設けられている。このねじは、後部関節窩エレメントのブッシング内へ螺入させられるべく構成されている。関節窩球は、それと結び付いている上腕部補綴具の凹状カップと協働する凸状関節面を有している。

このタイプの補綴具は、例えば、Ｍｅｄｉｎｏｖの名前で出願されたフランス特許第８９/１３３６６(２ ６５２ ４９８)号から、知られている。

三角筋、棘上筋、棘下筋、肩胛骨下筋及び小円筋から成る肩の筋の組により、回旋腱板が構成されている、ということは、知られている。広範囲の断裂が腱板に起こると、三角筋のみが、残り、そして、それは、肩の関節が正しく機能するのを可能にするには不十分であり、このことは、肩補綴具が埋め込まれることを必要とする。

補綴具であって、複数のねじによって肩胛骨の関節窩に後部関節窩エレメントが締着され且つ凸状関節面を有している関節窩球をその後部関節窩エレメントが受容するものは、知られており、その関節窩球は、後部関節窩エレメントの周辺にねじ込まれる。

不幸なことに、長時間に亘って使用されると、関節窩球はねじが緩む傾向にある、ということが、見出されている。これは、後部関節窩エレメントの下で関節窩球が傾くことを引き起こし、ねじ山を潰す。補綴具に生じたその損傷は、それを取り替えるべく、外科医が補綴具全体を取り外すことを必要とするに十分であり得る。

この問題を解決すべく、関節窩球と後部関節窩エレメントとの対面している接触面を凸凹にするという提案が、先ず、なされた。この技術は、不十分であると判明している。

次いで、相補的な雄円錐支持面及び雌円錐支持面であって、それぞれ、後部関節窩エレメント及び関節窩球に作られているものの構成と、関節窩球のねじとを結び付けるという提案が、なされた。この場合、関節窩球と後部関節窩エレメントとを具備しているシステムをロックし又はロックを外すべくねじが駆動されるところの方向の関数として、２つの円錐支持面は、互いに他方から離れ又は互いに他方に向かって移動させられ得る。この方法で構成されている肩補綴具は、Ｍｅｄｉｎｏｖの名前でのフランス特許第９５/０９３９５(２ ７３７ １０７)号に記載されている。その補綴具は、満足を与えている。

しかしながら、ねじの頭部は、ねじの締め付けの最後に外科医がその最後であることを適切に感じ得ない場合には、後部関節窩エレメントを通過し得る。何故ならば、特別の保持手段が、設けられていないからである。そのような状況下では、後部関節窩エレメントは、最早、関節窩に固定されていない。
【００１０】
後部関節窩エレメントへのアクセスを外科医に与えることにおける別の問題が、存在し、これは、関節窩球を後部関節窩エレメント上の所定の場所に正しく置くことの困難さを生じる。患者は、手術の間、半分座った位置にあり、このため、外科医は、後部関節窩エレメントに、突き出ている位置から接近しなければならず、この場合、後部関節窩エレメントは、言わば、肩の組織及び筋肉によって画成されている竪穴の底にある。後部関節窩エレメントへのアクセスは、限定されており且つ比較的困難であり、不十分な可視性を伴っており、この不十分な可視性は、後部関節窩エレメントのブッシングの軸上に関節窩球のねじが正確にあるかどうかを確認するのを不可能にする。

それは、外科医が関節窩球を不正確に位置させ得、もって、後部関節窩エレメントにおける対応する穴の軸上に関節窩球の軸方向ねじが正確にはないということを、結果的にもたらす。そのような状況下においては、関節窩球のねじにおける雄ねじ部と後部関節窩エレメントの中央の穴における雌ねじ部とが正しく係合させられていない時に、ねじの締め付けが、開始される。その誤った挿入に外科医が気付いた時には、既に、ねじ部は、補綴具全体が取り替えられることを必要とするに十分に損傷を受けている。２つの補綴具が使用されなければならず、従って、装置のコストをかなり上昇させるだけでなく、手術に必要な時間が、倍になる。

本発明の目的は、これらの種々の欠点を排除するような態様で構成されている関節窩補綴具を提供することである。

本発明によれば、肩補綴具は、関節窩球を案内する手段であって、ブッシングの軸上での、後部関節窩エレメント上へのねじの正しい配置を容易にすべく構成されているものを備えている。

本発明の実施形態においては、前記案内手段は、管状ブッシング内を摺動させられ得るべく構成されているピンを具備しており、且つ、関節窩球のねじは、縦軸方向チャンネルであって、ねじの両端部において開口しており且つそのチャンネル内へピンが挿入されるのを可能にするものを提供しており、もって、ピンは、ねじが管状ブッシング内へ挿入されるまで、そのピンに沿ってねじが摺動する際に関節窩球を案内すべく機能する。

後部関節窩エレメントの中央ブッシング内へのピンの予備的な挿入は、後部関節窩エレメントの中央ブッシングの軸上への関節窩球の完全な案内をもたらすことを可能にする。次いで、関節窩球は、ねじ部を傷付けるというリスクなく、関節窩ブッシング内へ正しく螺入させられ得、これにより、第２の補綴具に頼ることを必要とするというリスクが、殆ど完全に排除される。

本発明の別の特徴によると、後部関節窩エレメントを関節窩に固定する手段は、ねじであり、且つ、ねじの締め付けの最後に各ねじが後部関節窩エレメントに当接するのを保証する手段が、設けられている。

本発明の実施形態においては、当接手段は、少なくとも１つのねじの頭部の周りに形成されているカラーであって、ねじ頭部の雄ねじ部に対して半径方向に突出しているものと、雌ねじを螺刻されている穴の縁部の周りに設けられている対応環状支持肩部であって、後部関節窩エレメント内へのねじの挿入のためのものとを具備している。

本発明の別の有り得る特徴によると、当接手段は、凸状外面を呈している少なくとも１つのねじの頭部を具備しており、その凸状外面は、前記ねじを後部関節窩エレメント内に挿入するための穴の壁部に形成されている相補的凹状座部に当接すべく構成されている。

プレート１３は、好適に、円錐状であり、軸Ｘ−Ｘを対称軸とする回転対称であり且つ中央ブッシング１４を設けられており、この中央ブッシング１４は、好適に、プレート１３と一体に形成されている。中央ブッシング１４は、プレート１３から突出していると共に、後部関節窩エレメント７を骨に固定するための円形リブ１５を有している。ブッシング１４は、軸方向の内腔１６を提供し、この内腔１６は、その長さの一部分に雌ねじ部１７を有している。この雌ねじ部１７には、滑らかなゾーンを構成している内腔４９が、続いている。

穴１９は、発散しているねじ２７であって、各々が頭部２８とねじ込軸部２９とによって構成されているものを受容し得る。頭部２８は、雌ねじ部１９内へ螺合するに適している雄ねじ部３１と、頭部２８の周りに形成されている端部カラー３２とを有している。カラー３２は、雌ねじ部３１に対して半径方向に突出していると共に、対応する角張った肩部３３であって、ねじ２７が挿入されるところの穴１８の縁部の周りに配設されているものによって支持され得る。

カラー３２は、関節窩３内でのねじの締め付けの最後における、ねじ２７用の当接手段を構成しており、このカラー３２がそれと結び付いている肩部３３と当接するや否や、カラー３２は、ねじ２７がそれ以上移動させられるのを防止する。同様に、ねじ２５が関節窩３内へ螺入させられ、十分遠くにまでねじ２５が係合させられて頭部２４の面２３が凹状座部２２と接触するや否や、凹状座部２２は、それらと結び付いているねじ２５に対して、それらを後部関節窩エレメント７内に停止させるための当接部として作用する。

縦軸方向のチャンネル４５が、ねじ３５の中に、そのねじの両端部において、即ち、ソケット４１内の一方の端部において且つオリフィス３９内において、開口している状態で配設されている。補綴具組立体は、また、関節窩球９を案内する手段であって、後部関節窩エレメント７上への組み込みを容易にすべく構成されているものをも具備している。図示されている実施形態においては、それらの案内手段は、ピン４６を具備しており、このピン４６は、多少の可撓性を好適に有しており、先ずブッシング１４の軸方向の内腔１６内へ摺動させられ、次にねじ３５の縦方向のチャンネル４５を貫通して摺動させられるべく、構成されている(図６)。ピン４６は、好適に、金属で作られており、そして、その径は、ねじ３５におけるチャンネル４５の径よりも好都合に僅かだけ小さい。

ねじ３５は、滑らかな自由端部４８を有しており、この自由端部４８の先端部は、チューブ状のブッシング１４の入口内への挿入を容易にすべく、面取りされている。ねじ３５用の案内手段は、更に、ブッシング１４の入口に配設されている滑らかな円筒状のゾーン４９であって、雌ねじ部１７がそれに続くものを具備している。面取りされた先端部５１は、好適に、環状の面５２を有しており、この面５２の縦断面は、直線的(即ち、円錐面)であり、そして、その面５２には、好ましくは球形の丸みによって構成されている、丸みを付された環状の面５３が、先端部５１の自由端部まで続いている。円錐形のセクション５２は、反対側の端部において、滑らかなゾーン５０に合流しており、そのゾーン５０には、雄ねじ部１０が、続いている。

先ず、後部関節窩エレメント７が、ブッシング１４を強く押し付けることにより且つねじ２５,２７を締め付けることにより、関節窩３に固定される。その後、外科医は、ピン４６の一方の端部を、ブッシング１４の内腔１６内に挿入する。カニューレを備えているねじ回しを用いて、外科医は、肩の組織及び筋肉の中に形成されている凹部の入口へ、関節窩球９を供給し、この場合、凹部の底部は、後部関節窩エレメント７によって構成されており、ピン４６の端部は、軸方向のチャンネル４５内へ挿入されている。次いで、外科医は、ねじ３５の自由端部４８が滑らかな入口４９内に係合するまで、関節窩球９をピン４６に沿って摺動させる(図６)。面取りされ且つ丸みを付されている先端部５１は、滑らかなゾーン４９の故に雌ねじ部１７を傷付けることなく且つ滑らかなゾーン５０の故に雄ねじ部１０を傷付けることなく、ねじ３５をブッシング１４内へ挿入するのを、容易にする。ねじ３５がブッシング１４の軸Ｘ−Ｘ上に正しく位置するや否や、外科医は、関節窩球９が後部関節窩エレメント７と衝突するまで、ねじ３５を締め付け(図３)、次いで、ねじ回しとピンとを回収する。

円錐形のゾーン５２の丸みを付された面５３の構成、滑らかなゾーン５０の構成、及びブッシング１４の滑らかな入口４９の構成は、案内ピン４６との組み合わせで(図５及び図６)又はそれを用いることなく、ブッシング１４内へのねじ３５の正しい案内をもたらすべく構成され得る。そのような状況下では、ねじ３５は、縦方向のカニューレ１６を有し得、又は、それは、中実であり得る。それにも拘らず、内腔１６、ピン４６、滑らかなゾーン５０を備えている端部４８、及びブッシング１４の入口ゾーン４９の組み合わせが、後部関節窩エレメント７の軸Ｘ−Ｘ上の関節窩９の正しい位置までの、その関節窩球用の最も満足の行く案内を提供する。

Claims (10)

  1. 肩補綴具組立体であって、
    中央管状ブッシング(１４)を有するプレート(１３)によって形成されている後部関節窩エレメント(７)であって、該プレート(１３)を関節窩(３)に固定する部材(２５,２７)を有する後部関節窩エレメント(７)と、
    該後部関節窩エレメント上に嵌められるべく構成されている関節窩球(９)であって、前記中央管状ブッシング内へ螺入させられ得る軸方向ねじ(３５)を備えている関節窩球(９)と、
    を有し、
    該関節窩球は、それと結び付いている上腕骨補綴具(２)のカップ(１２)と協働する関節面(１１)を提供し、
    該肩補綴具組立体は、該中央管状ブッシング(１４)の軸(Ｘ−Ｘ)上での、該後部関節窩エレメント上への軸方向ねじ(３５)の正しい配置を容易にすべく構成された、関節窩球を案内する手段(１６,４５,４６；５１,５０,４９・・・)を備え、
    該案内手段は、該管状ブッシング(１４)内へ摺動させられるように構成されているピン(４６)を有し、
    該関節窩球(９)の該軸方向ねじ(３５)は、該軸方向ねじ（３５）の両端部において開口していて且つ該ピン（４６）が挿入できる縦軸方向チャンネル(４５)を有し、それによって、該軸方向ねじ（３５）が該中央管状ブッシング（１４）内へ挿入されるまで該ピン（４６）に沿って該軸方向ねじ（３５）が摺動し、該ピン（４６）は該関節窩球（９）を該後部関節窩エレメント(７)へ案内するよう機能する、
    ことを特徴とする補綴具組立体。
  2. 前記軸方向ねじ(３５)の前記縦軸方向チャンネル(４５)が、一方の端部において、前記軸方向ねじ（３５）の頭部(３７)に設けられており且つカニューレを有しているねじ回しと協働するのに適しているソケット(４１)内へ開口していることを特徴とする請求項１の補綴具組立体。
  3. 前記案内手段が、前記関節窩球(９)の前記軸方向ねじ(３５)の滑らかな自由端部(４８)と、前記中央管状ブッシングへの前記入口に設けられており且つ雌ねじ部(１７)に続いている滑らかなゾーン(４９)とを具備していることを特徴とする請求項１または２の補綴具組立体。
  4. 前記自由端部(４８)が、円錐環状面(５２)を画定している、面取りされた先端部(５１)を有しており、その円錐環状面(５２)には、球面である、丸みを付された環状面(５３)が、続いていることを特徴とする請求項３の補綴具組立体。
  5. 前記後部関節窩エレメント(７)を前記関節窩(３)に固定する前記部材が、ねじ(２５,２７)であり、且つ、前記ねじ(２５,２７)の締め付けの最後に前記ねじ(２５,２７)が前記後部関節窩エレメントと当接するのを保証する手段(２３,２２,３２,３３)が、設けられていることを特徴とする請求項１〜請求項４のいずれか一項の補綴具組立体。
  6. 当接手段が、少なくとも１つのねじ(２７)の頭部(２８)の周りに形成されているカラー(３２)であって、該ねじの頭部の雄ねじ部(３１)に対して半径方向に突出しているものと、雌ねじを螺刻されている穴(１８)の縁部の周りに設けられている対応環状支持肩部(３３)であって、前記後部関節窩エレメント(７)内への該ねじの挿入のためのものとを具備していることを特徴とする請求項５の補綴具組立体。
  7. 当接手段が、凸状外面(２３)を呈している少なくとも１つのねじ(２５)の頭部(２４)を具備しており、その凸状外面(２３)は、前記ねじ（２５）を前記後部関節窩エレメント(７)内に挿入するための穴(２１)の壁部に形成されている相補的凹状座部(２２)に当接すべく構成されていることを特徴とする請求項５又は請求項６の補綴具組立体。
  8. 前記関節窩球(９)の前記軸方向ねじ(３５)を前記後部関節窩エレメント(７)の前記中央管状ブッシング(１４)内に案内する手段(１６,４５,１７,４９,５０,５１,５２,５３)を備えている、請求項１〜請求項７のいずれか一項の補綴具組立体の部分を形成している肩補綴具(１)。
  9. 前記案内手段が、丸みを付された面(５２,５３)を呈している先端部(５１)を有している前記軸方向ねじ(３５)の一方の端部(４８)と、その先端部と該軸方向ねじ(３５)の雄ねじ部(１０)との間の滑らかなゾーン(５０)と、前記中央管状ブッシング(１４)内の前記チャンネル(１６)への入口を構成している滑らかなゾーン(４９)とを具備していることを特徴とする請求項８の補綴具。
