【発明の属する技術分野】
本発明は、変形性CM関節症や慢性CMリウマチ、腱鞘炎などを保存的療法によって治療する際、或いはこれらの関節症や腱鞘炎を予防する際に使用される装具の構造に関するものである。

【従来の技術】
現在大菱形骨中手骨関節(これを「CM関節」と呼ぶ)の変形性関節症の治療には、消炎鎮痛剤使用、装具療法(保存的療法)、手術療法、等があり、症状の軽重、或いは治癒の段階に応じて適宜採用されている。この中で装具療法は、患者が手術療法を希望しない場合、手術までの期間の保存的療法のため、術後の部位の可動域制限のため、軽度の症状に対する場合、等々においてしばしば採用される療法である。

装具療法に採用される装具は、症状その他によって種々あるがいずれもCM関節を固定するための金属又は硬質プラスチックを含むいわゆる硬質装具であり、手を洗う、物をつかむ、といった日常生活動作が不可能若しくは困難なものであった。

また装具療法は、手術療法と異なり短期で治癒することは少ないにもかかわらず硬質装具であって煩わしいが故に、長期装用を嫌がる患者も多かった。また硬質装具の場合、患者の患部形状の差異に柔軟に対応できることは少なく、多くは一品製作されるため高価なものとなりやすく、また治癒段階に応じた調整も困難な場合が多かった。

そこで本発明者は、CM関節の動きを制限はするが固定はせず、また日常生活動作を妨げることのない、しかも金属支柱などの硬質部材は使用せず患部形状に応じて一品製作するする必要がないため既製品化でき安価に提供できる、コンパクトな大きさシンプルな構造の装具を開発した(意匠登録第957299号)。またこうした構造であるため、治療用としてだけではなく予防用として用いても好適なものとなった。

【発明が解決しようとする課題】
この装具は、CM関節周辺をすっぽり包み込む母指キャップと手関節を周回させるベルトとを連結したものである。母指キャップを嵌めた後ベルトを手首に巻いて固定すれば、母指キャップを形成するプラスチックの材質の硬度を調整することで、CM関節の動きを制限はするが固定はしない適当な装具となることになる。

ところが実際には、関節の動きを規制することを前提に硬度を決定した時には硬すぎて関節の曲げ伸ばしは
迫しすぎるし、圧迫しない範囲で最も大きな硬度でも関節の動きを好適には規制できるまでには至っていないことが判明した。

【課題を解決するための手段】
そこで本発明者は上記点に鑑み鋭意研究の結果遂に本発明を成したものでありその特徴とするところは、先端は露出させた状態で母指をそのMP関節近傍からCM関節を越えた部位までを包み込む母指キャップと、手関節を周回させて係止することで該母指キャップを固定するベルトとにより構成されるものであって、該母指キャップは母指外側に相当する部分に補強布を内包するシリコーンラバーにより形成される点にある。

即ち、CM関節近傍を覆う母指キャップと手関節を周回するベルトによって、日常生活動作を妨げない範囲で十分な効果を挙げるために、該母指キャップの材質の構造に工夫を凝らしたものであると言える。母指の外側についてはキャップ自体が伸びようとするのを規制し、内側に関してはそうした規制力が小さくなるように、全体はシリコーンラバー製であるが母指外側に相当する部分については補強布が内包されている。

本発明装具に採用されるシリコーンラバーに関して、本発明は限定はしない。当然ながら硬度が高いと保持力は向上するが装用感は劣り、硬度が低くなると逆になる。但し、保持力に関しては補強布によってある程度は確保できるので、やや柔らかめのシリコーンラバーの採用が可能である。本発明者が試作した範囲では、JIS A(Shore A)ゴム硬度検査法による数値で、40〜60度程度の無発泡シリコーンラバーが好適であった。

また補強布についても本発明は特に限定しない。但し、シリコーンラバー内に収まる(即ち、表裏面共露出しない)ものであること、十分な規制力を有していること、が必要であるので、あまる分厚いものや規制方向に関して伸縮性が大きいものは好ましくない。本発明者は試作の段階で、一方向にのみ伸縮性を示す「オペロン(東レ・デュポン株式会社製)」なる布を採用して好適な結果を得た。なお、補強布をシリコーンラバーに内包させるためにどのような方法を採用するかに関しても本発明は何ら限定するものではない。但し、母指キャップに先端の端部から補強布が露出しているような状態では母指MP関節を曲げ伸ばしする際補強布が皮膚に触れ装用感を損なうので、補強布は母指キャップ先端までは達しないような配置が望ましい。

なお、既述した変形性CM関節症や慢性CMリウマチ、腱鞘炎などは、多くの場合、手関節部で長母指外転腱と短母指伸筋腱と局部的に圧迫して該腱の動きを制限してやると、最大ピンチ力が増加し母指の不安定感や力の入りにくさが軽減されることが試験的にも確認できた。そこで本発明においては、ベルト裏面に上記目的のためのパッドを着脱自在に配置するという発明をも含めている。パッドの材質はどのようなものであっても良いが、皮膚とのなじみの良さを考慮するとシリコーンラバーが最適であり且つその硬度は母指キャップの硬度よりも柔らかなものが最適である。

【発明の実施の形態】
以下図面に基づき本発明を更に詳細に説明する。

図1及び図2は、本発明に係る母指関節装具1(以下本発明装具1という)の一例を示すものであり、図1はその斜視図、図2はその使用状態を示す斜視図である。これらの図より明らかなように本発明装具1は、いびつな筒形状をした母指キャップ2と、該母指キャップ2の下端近傍に連結されたベルト3とにより構成されたものである。母指キャップ2は、無発泡シリコーンラバーよりなるものであるがその片側(本発明装具1装着時外側になる側)には補強布4が埋め込まれている。その結果図の矢印方向Aに関する伸縮性は、母指キャップ2の内側と外側とでは異なり、外側の「伸びにくい」性質のものとなっている。対して内側は、柔らかく皮膚とのなじみの良いいわゆる「しっくりとくる」ものとなっている。また、補強布4が存在することによって、元来が変形に対する強度の低い(即ち脆弱な)シリコーンラバーの強度向上に役立っている。なお、本例ではシリコーンラバーを半透明としたため補強布4がある程度観察できるものとなっているがこの補強布4はあくまで「埋め込まれた」ものであり母指キャップ2の表層には露出しない。しかも本例の場合、母指キャップ2の先端まで補強布4は達しておらず、表裏両面及び端面いずれでも布材は露出していない。母指を曲げ伸ばしする際皮膚と接触するのはシリコーンラバーのみである。

ベルト3は、母指キャップ2に対しその中心線と約130度の角度となるように固定されたもので、裏面(内側)が面ファスナーの雌側であり、且つ幾分かの伸縮性を有するものである。また母指キャップ2はベルト3に直接固定されてはおらず、母指キャップ2を先ずベルト係止片5にホック6(ハトメ状ホック)にて固定し、このベルト係止片5をベルト3に縫合するという方法で連結した。ベルト係止片5の表面は、面ファスナーの雄側となっており、手関節を周回させたベルト3はこのベルト係止片5位置で係止されることになる。

次に図3(a)(b)は、本発明装具1に付属させて好適な部材であるパッド7とその取り付け状態を概略的に示したものである。このパッド7は、柔らかいシリコーンラバー製の楕円柱状或いは長円柱状のものでありその一面には面ファスナー雄側である固定用シート8が固着されている。固定用シート8は、ベルト3の裏面(面ファスナー雌側である)のどこにでも係止できるので、手関節部で長母指外転腱と短母指伸筋腱と局部的に圧迫して該腱の動きを制限してやるというパッド7の目的にかなった位置への配置が容易なものとなる。なおこのパッド7は本発明に不可欠の部材ではない。また同図(b)においてはベルト係止片5及びホック6の描出は省略している。

【発明の効果】
以上詳細に説明したように本発明に係る母指関節装具は、先端は露出させた状態で母指をそのMP関節近傍からCM関節を越えた部位までを包み込む母指キャップと、手関節を周回させて係止することで該母指キャップを固定するベルトとにより構成されるものであって、該母指キャップは母指外側に相当する部分に補強布を内包するシリコーンラバーにより形成されるものであることを特徴とするものであり、以下述べる如き種々の効果を有する極めて高度な発明である。

(1) 金属支柱等を使用しない軟質装具であるので装用感が良好である。
(2) 母指キャップはシリコーンラバー製であるので皮膚とのなじみが良く、発汗による炎症などを起こしにくい。
(3) 母指キャップの外側には補強布が配置されているので、シリコーンラバーの硬度を柔らかめにしても所定の規制力が得られるし、変形に対する強度も確保される。
(4) 母指関節を覆う母指キャップと手関節を周回するベルトだけで構成されているため軽量でかさばらず、日常生活動作の妨げとなりにくい。

This orthosis is a combination of a thumb cap that completely wraps around the CM joint and a belt that circulates the wrist joint.
