【技術分野】
本発明は、内視鏡観察下で例えば病変部を切除する際に用いる、重力方向視認装置付き磁気誘導手段遠隔誘導システム、及び重力方向視認装置付き内視鏡用磁気誘導装置に関する。

【従来技術及びその問題点】
従来、通常の手術において人体内部の病変部を切除する場合においては、把持鉗子を用いて病変部を持ち上げることにより病変部と隣接する正常組織との間隔を広げ、その状態で病変部と正常組織との間を切除している。しかし、例えば内視鏡粘膜切除術(EMR)では、体内には内視鏡を一台しか挿入できないため、病変を持ち上げることができず、注射針で病変部の周囲の正常粘膜に生理食塩水を注入して病変部を浮き上がらせ、その状態で高周波ナイフやスネアなどを用いて病変部と正常粘膜の間の切除を行っていた。

しかし、このような従来の方法では、病変部を十分な位置まで持ち上げることができなかったため、病変部と正常組織との境界の切除部分を十分確保することができなかった。
また、病変部が扁平な形状である場合は、切除部分を作りだすことができないこともあった。

さらに、切除作業中において、すでに切除した病変部が正常組織上に落ち込むことにより内視鏡による視界を妨げることがあり、特に病変部が大きい場合に顕著であった。そのため、切除部分を見ることができず、盲目的に切除するために正常部分を損傷して穿孔などの合併症が発生したり、血管を損傷して大出血をきたし、また出血時も出血部位の確認ができず止血できないことから重篤な合併症を来すことも考えられ、より安全な装置や処置方法が求められていた。

そこで本出願人は、これらの問題点を解決すべく、人体内部の病変部を掛着する掛着部材と、該掛着部材と連結される磁性体からなる磁気誘導手段(磁気アンカー)と、人体の外部に配置され、磁界を発生して磁気誘導手段に動力を与える磁気誘導手段誘導装置と、を備え、磁気誘導手段誘導装置が発生する磁界によって磁気誘導手段に動力を与えて、掛着部材に掛着された病変部を持ち上げることを特徴とする磁気磁気誘導手段遠隔誘導システムを提案し、特許出願している(特願2002−268239号)。

しかし、この磁気磁気誘導手段遠隔誘導システムを利用した切除作業では、内視鏡によって写しだされた体内の病変部の観察像をテレビモニタに写し出し、テレビモニタを見ながら切除作業を行うが、テレビモニタを見ただけでは、体内の重力方向(上下方向)を把握するのが難しい。このため術者が、磁気誘導手段が、実際に磁気誘導手段誘導装置から生じている磁界によって動いているのか、単に重力を受けて動いているのか分からないことがあった。
そして、このような場合は、磁気誘導手段誘導装置の電源を一旦切ったり、磁気誘導手段装置を移動させたりして、磁力の有無や方向などを確認しなければならなかったので、作業に長時間を要し、患者に与える負担が大きくなるという問題があった。

【発明の目的】
そこで本発明の目的は、簡単に重力方向を確認できるようにすることにより、磁気誘導手段遠隔誘導システムを用いた切除手術を容易に行えるようにした、重力方向視認装置付き磁気誘導手段遠隔誘導システム、及び重力方向視認装置付き内視鏡用磁気誘導装置を提供することにある。

【発明の概要】
本発明の重力方向視認装置付き磁気誘導手段遠隔誘導システムは、対象物内部の対象部位を掛着する掛着部材と、磁性体からなる磁気誘導手段と、該磁気誘導手段と上記掛着部材とを接続するひも状の柔軟連結具と、該柔軟連結具の中間部に固定した、内部が視認可能な透明ケースと、この透明ケース内に挿入した、常時、重力に従って該ケース内を移動する非磁性体からなる視認部材と、対象物外部に配置され、磁界を発生して上記磁気誘導手段に動力を与える磁気誘導手段誘導装置と、を備え、上記磁気誘導手段誘導装置が発生する磁界によって上記磁気誘導手段に動力を与えて、上記掛着部材に掛着された上記対象部位を移動させ、かつ、上記透明ケースと該透明ケース内の視認部材の相対位置により重力方向を認知することを特徴としている。

【発明の実施の形態】
以下、本発明の第1の実施形態を、図1から図11を参照しつつ詳しく説明する。
本実施形態の磁気誘導手段遠隔誘導システムは、磁気誘導装置1(磁気誘導手段2、クリップ(掛着部材)3、連結ひも4、及び重力方向視認装置7からなる)と、磁気誘導装置1を体外において吸引制御する(磁気誘導手段2に磁力を及ぼす)磁気誘導手段誘導装置10とからなるものである。

まず、図1から図3を参照して、磁気誘導装置1の構成について説明する。
磁気誘導手段2は、略円筒体の本体部2aと、本体部2aより小寸の取付部2bとを具備しており、本体部2aと取付部2bはともに強磁性体によって形成されている。磁性体の具体例としては、純鉄、鉄合金のほか、プラチナマグネット、希土類磁石、テルビウム・ディスプロシウム・鉄合金などの磁石を使用することができる。

図2、図3に詳細を示すクリップ(掛着部材)3は、患者(対象物)体内の病変部(対象部位)Xを掴んで持ち上げるための部材である。このクリップ3は、U字状に折り曲げた弾性材料からなるものであり、その弾性により、互いに離れる方向に付勢された一対の開閉片3aを具備しており、両開閉片3aの先端には先端部3bが形成され、さらに、両開閉片3aの基端部同士は連結部3cによって連結されている。両開閉片3aの相対間隔は可変であり、両開閉片3aには、互いの間隔を調節後に位置固定するラチェット部材3dが備えられている。図3に示すように、このラチェット部材3dは、一対の係合片3d1を具備しており、両係合片3d1の対向面には、互いに係脱可能な係合歯3d2が形成されている。ラチェット部材3dは、対をなす開閉片3aが間隔を縮める方向に弾性変形するときにはその変形を妨げず、かつ両開閉片3aの間隔調整後にその間隔を保持する機能を有する。

連結ひも(柔軟連結具)4は、磁気誘導手段2の取付部2bとクリップ3の連結部3cとを連結するものである。
この連結ひも4の中間部には、無色透明の球形状のケース(透明ケース)5が設けられており、このケース5内には、その直径がケース5より小さい非磁性体からなる球体(視認部材)6が配設されており、このケース5と球体6により重力方向視認装置7が構成されている。非磁性体の具体例としては、金、銅、アルミニウム、ステンレス(オーステナイト系)、真鍮、セラミック、硝子等がある。

次に、図4及び図5を用いて、磁気誘導手段誘導装置10の構成について説明する。
患者Aを載せる床板11aを具備するベッド11の両側部には、一対のXYステージ(一方向移動機構)12、12が配設されている。この一対のXYステージ12は、ベッド11の長手方向に沿って、両者12、12の該長手方向位置が常時同じになるように、直線的に往復移動するものである。さらに、ベッド11の上方には、ベッド11の長手方向と直交する平面内において互いに平行をなす、正面視略逆U字形の二つのレール14、15からなるフレーム/レール(一平面内移動機構)13が配設されており、このフレーム/レール13の両端部は、左右のXYステージ12にそれぞれ固定されている。内側のレール14には、磁気誘導装置1の磁気誘導手段2を体外において吸引制御する(磁気誘導手段2に磁力を及ぼす)磁気誘導部材16が摺動自在に装着されており、磁気誘導部材16は左右のXYステージ12の間を、レール14に沿って移動することができる。磁気誘導部材16は、鉄心にコイルを巻いた構造の電磁石17を基体18上に配置したものであり、その電磁石17は常時、患者A側を向いている(図4参照)。なお、磁気誘導部材16は、永久磁石と電磁石の組み合わせでもよく、また、永久磁石と電磁石を2個以上組み合わせたものでも良い。

フレーム/レール13の外側のレール15には、フレーム/レール13全体の重量バランスを保つためのカウンターウエイト19がレール15に摺動自在に装着されている。カウンターウエイト19は、磁気誘導部材16の位置に応じて、その位置を変更する。例えば、磁気誘導部材16が患者Aの正面側に位置するときは、カウンターウエイト19は患者Aの背面側に位置し、磁気誘導部材16が患者Aの背面側にあるときは、カウンターウエイト19は患者Aの正面側に位置して、フレーム/レール13全体の重量バランスをとっている。
そして、以上説明した磁気誘導部材16、XYステージ12、フレーム/レール13により磁気誘導手段誘導装置10が構成されている。

図6は、磁気誘導手段遠隔誘導システムを用いた切除術の実施に用いる内視鏡20を示している。
内視鏡20の構造は公知なので詳しい説明は省略するが、体内に挿入される挿入部21の先端面21aには、図9及び図10に示すように、エア及び洗浄水を送るための送気送水ノズル22、切除部及びその周辺を照らすための照明窓23、切除部及びその周辺を観察するために対物レンズを配置した観察窓24、並びに、鉗子チャネル(図示略)の出口25が設けられている。鉗子チャンネルは挿入部21内に設けられており、その入口26aは鉗子挿入口突起26の端面に形成されている。

次に、磁気誘導手段遠隔誘導システムを用いた病変部Xの切除要領について説明する。
磁気誘導手段遠隔誘導システムを用いた切除術の実施に先立っては、まず、図4及び図5に示すように、局所麻酔を施した患者Aをベッド11上に横たわらせる。このとき、XYステージ12を操作して、フレーム/レール13のベッド11の長手方向位置を、患者Aの頭部A1とほぼ同じ位置にしておき、さらに、磁気誘導部材16及びカウンターウエイト19を所定の場所に位置させておく。
次に、XYステージ12を操作してフレーム/レール13を患者Aの患部の正面側に配置させ、さらに、磁気誘導装置16をフレーム/レール13に沿って移動させて、磁気誘導部材16を切除術開始時位置に位置させる(図5参照)。

次いで、図9及び図10に示すオーバーチューブ30を、患者Aの体内に口から挿入し、このオーバーチューブ30の先端部を、臓器B内の病変部Xに近接させる。そして、オーバーチューブ30内に内視鏡20の挿入部21を挿入し、その先端部をオーバーチューブ30の先端から突出させ、病変部Xに近接させる。このように、内視鏡20の挿入部21の先端を臓器B内に挿入すると、観察窓24から得られた臓器B内の観察像が、図示を省略したテレビモニタに写し出される。

この状態で、鉗子挿入口突起26の入口26aから、先端部に注射針を具備するチューブ状の処置具(図示略)を挿入し、その注射針を挿入部21の出口25から突出させて、注射針を病変部Xの周辺から臓器壁の粘膜下層B1に挿入して生理食塩水を注入し、病変部Xを固有筋層B2から浮き上がらせておく。

次に、以下に説明する要領により、磁気誘導装置1を患者Aの臓器B内へ挿入する。
磁気誘導装置1を臓器B内へ挿入する際には、内視鏡20を患者Aの体外に引き抜いておき、さらに、内視鏡20の鉗子チャンネルに、その後端部40aが、溶着されることによって閉じられ、かつ、その内部に、フレキシブル・プッシング・ロッドLが配設された中空筒状のガイドシース40を予め挿入しておき(図8参照)、ガイドシース40の先端部を挿入部21の先端面21aから突出させておく。
その後に、ガイドシース40の先端部40bに、磁気誘導装置1を図7(a)の状態になるように挿入して、磁気誘導装置1がガイドシース40の先端部40bから不意に外れないようにし、この状態のまま、内視鏡20の挿入部21を患者Aの体内に挿入する。

ガイドシース40の全長は、内視鏡20の鉗子挿入口突起26の入口26aから挿入部21の先端面21aまでの距離より長く設定してある。このため、図8に示すように、内視鏡20の先端面21aから磁気誘導手段2の本体部2aが出た状態でガイドシース40を内視鏡20にセットすると、ガイドシース40の後端部40aは鉗子挿入口突起26の外側に位置する。さらに、ガイドシース40の後端部40aには、切り取りひも41が設けられている。切り取りひも41はガイドシース40の外周面に周方向に沿って接着され、その端部41aは作業者が切り取りひも41を引っ張りやすいようにガイドシース40から浮いており、その端部41a以外はガイドシース40と一体となっている。作業者が切り取りひも41を引っ張ると、切り取りひも41がガイドシース40の周方向に沿って取れるため、切り取りひも41が取れた部分でガイドシース40が前後二つに分かれる(図7(b)参照)。
分かれた後に後端部40aを引き抜くと、ガイドシース40の内側に設けられたフレキシブル・プッシング・ロッドLが現れる。このフレキシブル・プッシング・ロッドLをその軸線方向前方に移動させることにより、ガイドシース40の先端部から磁気誘導装置1が患者Aの臓器B内に押し出され(図7(c)参照)、図9に示すように、磁気誘導装置1が臓器B内に載置される。

このように臓器B内に磁気誘導装置1を配置したら、次いで、鉗子チャンネルからガイドシース40を引き抜き、代わりに、図9及び図10に示す把持鉗子50を鉗子チャンネルに挿入し、その先端部を、挿入部21の先端面21aから突出させる。
この把持鉗子50は、可撓性チューブ50aの先端に開閉可能な金属製の把持部材51を具備し、かつ、可撓性チューブ50aの基端部に操作部(図示略)を具備するものであり、この操作部を操作すると把持部材51が開閉する。
そして、この把持鉗子50を操作してクリップ3のラチェット部材3dを締めることにより、クリップ3の両開閉片3aを閉じて、両先端部3bにより病変部Xの端部(対象部位)X1を掴む(図9参照)。

図10に示すように、患者Aの外側には、臓器Bに近接した状態で磁気誘導装置16が配設されているので、磁気誘導装置16から磁界を発生させると、この磁界から生じる磁力P1(図10及び図11参照)によって、磁気誘導手段2が磁気誘導装置16に向かって吸引され、連結ひも4が緊張し、病変部Xの端部X1が磁力P1方向に移動する。

この際、図10及び図11に示すように、ケース5に対する球体6の相対位置を視認することにより、重力P2方向を容易に把握することができる。即ち、ケース5の中心と球体6の中心を結ぶ線が重力P2方向であり、かつ、ケース5内で球体6が寄っている方が下方になる。つまり、図10及び図11(a)に示す状態では、磁力P1と重力P2がほぼ逆向きであり、術者は、病変部Xの端部X1が磁力P1によって上方に引き上げられていることを容易に理解することができる。
一方、図11(b)に示すように、クリップ3を基準にしたときの磁気誘導手段2の方向D1と、重力P2の方向とが一致している場合は、術者は、磁気誘導手段2が、磁力P1によって吸引されているのか重力P2によって吸引されているのかを理解できない。従ってこの場合は、磁気誘導装置16の位置を変えたり、患者Aの体勢を変えることにより、クリップ3を基準にしたときの磁気誘導手段2の方向D1と、重力P2の向きを異ならせてから、磁気誘導装置16から発生する磁力P1により磁気誘導手段2を所望の方向に吸引する。

このように病変部Xの端部X1を磁力P1方向に移動させると、病変部Xと正常組織との境界部に十分な大きさの切除部分が確保されるので、把持鉗子50を内視鏡20から取り出した後、図12に示すように、高周波メス60などの切開具を鉗子チャネルから臓器B内に挿入し、病変部Xを粘膜とともにその端部X1から切除し、病変部X全体を完全に切除する(図示略)。なお、高周波メス60による切除作業時においては、切除領域が拡がるにつれて、高周波メス60の先端60aの位置の確認がより容易となる。
以上のように切除作業を終えると、クリップ3に病変部Xが取り付いた状態になるので、病変部Xが紛失することが防止される。切除した病変部Xを回収するには、クリップ3に把持された病変部Xを把持鉗子50で掛着し、そのままの状態で、内視鏡20を体内から抜き去る。そして、その後に、切除した部分の縫合、消毒などの処置を行う。

以上のように、磁気誘導装置1を用いれば、球体6のケース5に対する相対位置を視認することにより、重力P2方向を容易に把握することができるため、磁気誘導装置16を的確に操作できるようになる。その結果、磁気誘導手段2を所望の方向に移動させることができるので、病変部Xと正常組織との境界部に十分な大きさの切除部分を確保することができ、また、病変部Xが扁平な形状であっても、十分な大きさの切除部分を作りだすことができるので、病変部Xを容易に切除することが可能となる。
さらに、病変部Xはクリップ3により磁力P1方向に移動するため、切除部分を十分確保することができ、すでに切除した病変部Xが固有筋層B2上に落ち込むことを防止できる。
また、任意の位置にクリップ3を配置できるため、切除した病変部Xにより内視鏡の視界が妨げられることがない。

次に、本発明の第2の実施形態について、図13を参照しながら説明する。

本実施形態の磁気誘導装置70の重力方向視認装置71は、ケース5と、ケース5内に充填された、水等の非磁性体からなる液体(視認部材)Wとからなるものであり、液体Wの体積はケース5の容積より小さい。なお、液体Wの色は、血液や臓器等と区別が付きやすく、かつ、目立つ色(例えば緑)が好ましいが、視認できるのであれば、無色透明等であっても構わない。

このような磁気誘導装置70は、第1の実施形態と同様の要領により、臓器B内に挿入され、そのクリップ3で病変部Xを把持する。そして、磁気誘導装置16から発生した磁界から生じる磁力P1によって、磁気誘導手段2が磁気誘導装置16に向かって吸引されるので、病変部Xの端部X1を磁力P1方向に移動させることができる。

そして、磁気誘導装置16を操作する際に、図13に示すように、ケース5に対する液体Wの相対位置を視認することにより、重力P2方向を容易に把握することができる。即ち、液体Wの液面に直交する方向が重力P2方向であり、かつ、液体Wの液面と反対側が下方になる。即ち、図13(a)の場合は、磁力P1と重力P2がほぼ逆向きであり、術者は、病変部Xの端部X1が磁力P1によって上方に引き上げられていることを容易に理解することができる。
一方、図13(b)に示すように、クリップ3を基準にしたときの磁気誘導手段2の方向D2と、重力P2の方向とが一致している場合は、術者は、磁気誘導手段2が、磁力P1によって吸引されているのか重力P2によって吸引されているのかを理解できない。従ってこの場合は、磁気誘導装置16の位置を変えたり、患者Aの体勢を変えることにより、クリップ3を基準にしたときの磁気誘導手段2の方向D2と、重力P2の向きを異ならせてから、磁気誘導装置16から生じる磁力P1により磁気誘導手段2を所望の方向に吸引する。

このように本実施形態によっても、術者が重力P2方向を容易に把握できるので、第1の実施形態と同様の効果が得られる。

以上、本発明について各実施形態を参照しつつ説明したが、本発明は上記実施形態に限定されるものではなく、改良の目的または本発明の思想の範囲内において改良または変更が可能である。例えば、クリップ3の代わりに、病変部Xに突き刺すことが可能な釣り針状の針部材(掛着部材)(図示略)を用意し、この針部材を病変部Xの端部X1に突き刺して実施することも可能である。

以上説明したように、本発明によると、簡単に重力方向を確認できるので、磁気誘導手段遠隔誘導システムを用いた切除手術を容易に行えるようになる。
