【発明の属する技術分野】
本発明は、薬液導出部を介して患者に投与可能な状態で加圧された薬液を収容する薬液収容室に対して薬液の導入を許容する一方、薬液収容室外部への薬液の流出を規制する薬液導入装置及びこれを備えた薬液注入装置並びに間歇型投与システムに関するものである。

【従来の技術】
投与対象となる薬液を導入するための薬液導入部と、この薬液導入部から導入された薬液を収容する薬液収容室と、この薬液収容室内の薬液を加圧する加圧手段と、上記薬液収容室内の薬液を患者側へ導出する薬液導出部とを備えた薬液注入装置が一般に知られている。

この種の薬液注入装置では、上記薬液収容室内に薬液が加圧された状態で収容されているため、この圧力を利用して薬液を薬液収容室から流出するのを防止する、いわゆるダックビルと称される逆止弁が上記薬液導入部のハウジング内に組み込まれていることが多い。このダックビルは、弾性を有する合成樹脂等により形成され、略円筒状の本体部と、この本体部から先窄まりの嘴状に突出する嘴部と、この嘴部の先端部において前記本体部の内腔と薬液収容室とを連通可能なスリットとを備えている。そして、上記薬液導入部に対する薬液導入方向の上流側に本体部を配置する一方、薬液導入方向の下流側に嘴部を配置し、さらに上記本体部の外周部とハウジングの内周部とを液密状態で固定した状態で、上記ハウジングに対してダックビルが取り付けられることとなる。一方、上記ハウジングには、上記薬液導入方向の上流側から本体部の内腔へ薬液を導入可能な開口部が形成されている。

上記のようなダックビルを有する薬液導入部を用いて薬液を充填する場合には、上記開口部から導入された薬液が上記本体部の内腔を通り、スリットを押し広げることにより、本体部の内腔と薬液収容室内が連通し、このスリットを介して薬液が薬液収容室内に導入されることとなる。一方、薬液収容室内に充填された薬液は上記加圧手段により加圧されているため、この圧力が上記嘴部を押し縮める方向へ作用する結果、スリットが閉塞して、当該スリットにより薬液収容室外への薬液の流出が規制されることとなる。

このように、上記ダックビルを有する薬液導入部は、薬液収容室内への薬液の導入を許容する一方、薬液収容室からの薬液の流出を規制することができる。また、上記構成のダックビルにおいては、下記特許文献1の図4に示すように、注射針のような略直線状の棒状部材をハウジングの開口部内へ挿入し、この棒状部材によりスリットを抉じ開けることによって、薬液を薬液収容室から流出させることができる。

【発明が解決しようとする課題】
しかしながら、上記ダックビルを有する薬液導入部の構成では、上記特許文献1のように、略直線状の棒状部材を薬液導入部内へ挿入するといった比較的容易な方法で薬液を流出させることができるため、例えば、入院患者が医療従事者の目の届かない所で薬液を薬液収容室の外部へ流出させてしまうといった事態を引き起こすおそれがあり、このことは、患者に対する投与量に加えて流出した量の薬液を薬液収容室から減少させてしまう結果、患者に対する薬液の投与量を薬液収容室内の薬液残量から逆算する医療従事者にとって、注入量の判断を阻害する要因となっていた。

特に、近年では、上記のような薬液注入装置を在宅治療で使用すること、すなわち、医療施設内において予め設定された容量の薬液が充填され、この薬液を予め設定された流量で注入可能な状態とされた薬液注入装置を患者に装着して、帰宅した患者に対する薬液の投与を継続することが望まれている。このように薬液注入装置を在宅治療で用いる場合には、医療従事者の目の届かない時間が長くなり、上記のように薬液を流出させてしまう可能性が高くなる。

さらに、院内での治療及び上記在宅治療において、上記薬液注入装置から導出された薬液を塞き止めつつ、予め規定された容量だけ貯留して、術後等の痛みを感じた患者自身が操作することにより前記貯留薬液を当該患者に対して投与するように構成されたPCA(Patient Controlled Analgesia)装置と称される間歇投与装置を薬液注入装置と接続して使用する間歇型投与システムも知られている。この間歇型投与システムにおいて、上記のように薬液導入部から薬液を流出させてしまうと、PCA装置による投与量に加えて薬液導入部から流出した量の薬液を薬液収容室から減少させてしまうため、患者によるPCA装置の操作回数を薬液収容室内の薬液残量から逆算する医療従事者にとって、当該操作回数、すなわち、患者が感じた痛みの頻度の判断を阻害する要因となっていた。

本発明は、上記課題に鑑みてなされたものであり、薬液収容室内の薬液の流出を防止することができる薬液導入装置及びこれを備えた薬液注入装置並びに間歇型投与システムを提供することを目的としている。

【課題を解決するための手段】
上記課題を解決するために本発明は、薬液導出部を介して患者に投与可能な状態で加圧された薬液を収容する薬液収容室に対して薬液の導入を許容する一方、薬液収容室外部への薬液の流出を規制する薬液導入装置であって、この薬液導入装置は、逆止弁と、この逆止弁を収容するハウジングとを備え、前記逆止弁は、薬液の導入方向に沿って延びる内腔を有する本体部と、この本体部から薬液導入方向の下流側へ向けて先窄まりの嘴状に延びる嘴部と、この嘴部の先端部に形成されたスリットからなり、薬液導入方向の薬液の導入を許容するとともに薬液導入方向の上流側への薬液の流出を防止する封止手段とを備えている一方、上記ハウジングは、薬液導入方向の上流側から前記封止手段に対して薬液を導入可能な開口部を備え、上記ハウジングの開口部と逆止弁の封止手段との間には、当該開口部に略直線状の棒状部材が挿入された場合に、この棒状部材が逆止弁の封止手段へ移動するのを規制する規制部が形成され、上記ハウジングの開口部及び上記封止手段が薬液導入方向に沿った同一軸線上に配置され、上記規制部は、上記軸線と交差するように配置された規制板により形成されていることを特徴とするものである。

この発明によれば、上記規制部が開口部に挿入された略直線状の棒状部材の封止手段までの移動を規制するため、この棒状部材の挿入により封止手段が開放してしまうことを防止することができる結果、薬液収容室内の薬液が薬液導入装置から流出するのを防止することができる。

この態様によれば、上記薬液導入装置の規制部が開口部に挿入された棒状部材の封止手段への移動を規制するため、この棒状部材の挿入により封止手段が開放してしまうことを防止することができる結果、薬液収容室内の薬液が薬液導入装置から流出するのを防止することができる。したがって、上記薬液注入装置を用いて患者へ薬液を投与する場合に、患者が薬液収容室から薬液を流出させてしまうといった事態を防止することができるため、この薬液収容室内の残存薬液量を、初期充填量から薬液導出部による投与量を減じた量に維持することができる結果、医療従事者は、この残存薬液量に基づいて正確に薬液の投与量を逆算することができる。特に、上記薬液注入装置を在宅治療で使用する場合であっても、医療施設内において薬液収容室に充填された薬液は、薬液導出部を介してのみ導出されることとなるため、医療従事者は、患者への上記薬液投与量を正確に判断することができる。

本発明の別の態様は、上記薬液注入装置と、この薬液注入装置の薬液導出部から導出された薬液を塞き止めつつ、特定容量の薬液を貯留可能に構成され、この貯留薬液を患者の操作に応じて当該患者に対して投与するように構成された間歇投与装置とを備えていることを特徴とする間歇型投与システムである。

この態様によれば、上記薬液注入装置における薬液導入装置の規制部が開口部に挿入された棒状部材の封止手段への移動を規制するため、この棒状部材の挿入により封止手段が開放してしまうことを防止することができる結果、薬液収容室内の薬液が薬液導入装置から流出するのを防止することができる。したがって、上記間歇型投与システムを用いて患者へ間歇的な投与を実行する場合に、患者が薬液収容室から薬液を流出させてしまうといった事態を防止することができるため、この薬液収容室内の残量薬液量を、初期充填量から間歇投与装置の操作回数に応じた間歇投与量(操作回数×上記特定容量)を減じた量に維持することができる結果、医療従事者は、この残存薬液量に基づいて正確に間歇投与量を逆算することができる。特に、上記間歇型投与システムを在宅治療で使用する場合であっても、医療施設内において薬液収容室に充填された薬液は、薬液導出部及び間歇投与装置を介してのみ導出されることとなるため、医療従事者は、患者への上記間歇投与量を正確に判断することができる。

【発明の実施の形態】
以下本発明の好ましい実施形態について図面を参照して説明する。

図1は、本発明の実施形態に係る間歇型投与システムの全体構成を示す正面図である。

図1を参照して、間歇型投与システム1は、薬液注入装置10と、この薬液注入装置10から導出された薬液を案内する可撓性チューブ2と、この可撓性チュ−ブ2から導出された薬液の流量を調整する流量調整装置3と、この流量調整装置3に接続された間歇投与装置としてのPCA(Patient Controlled Analgesia)装置50とを備えている。上記可撓性チューブ2の途中部には、当該チューブ2内を流れる薬液中に混在する気泡を外部へ排出する機能と、薬液のろ過機能とを有するエアベントフィルター4が配設されている。上記流量調整装置3は、可撓性チューブ2の内腔に対して比較的小さな断面積に設定された流路を備え、この流路内に薬液を案内することにより、当該薬液の圧力損失を生じさせ、薬液の流量を調整するように構成されている。この流量調整装置3は、具体的には、例えば、特開平9−225028号公報、特開2000−14777号公報及び、特開2000−14778号公報に開示された構成により具現化できるため、ここでは、詳しい説明を省略する。

図2は、図1に示すPCA装置50の内部構造を示す断面図である。なお、このPCA装置50は、特開2000−185095号公報に開示された構成により具現化できるため、ここでは、構成の詳細な説明を省略し、機能に関する説明を行うこととする。

PCA装置50は、上記流量調整装置3に接続された可撓性チューブ51から導入された薬液をリザーバ52内に案内する案内孔53を備えている。リザーバ52は、予め規定された量(本実施形態では3ml)の薬液を収容可能に寸法設定されている。また、リザーバ52内には、導出孔54の一端部が臨んでおり、この導出孔54の他端部は、圧開放弁55によって行き止まりの状態になっている。すなわち、この圧開放弁55により、薬液は、リザーバ52内に上記規定量だけ収容された時点で、その流通が停止することとなる。上記圧開放弁55は、薬液が所定の圧力まで加圧された場合に、隣接されたバイパス通路56に対して薬液を流出させるように構成され、この所定の圧力は、後述する薬液注入装置10により加圧された薬液の圧力よりも大きく設定されている。この圧開放弁55を開放するには、リザーバ52の上部に配設された押圧子57を下方へ押込操作することとなる。押圧子57を押込むと、リザーバ52が内側に折り返されるように変形して、リザーバ52内の薬液が加圧される結果、圧開放弁55が弾性変形して、導出孔54とバイパス通路56との連通状態が形成される。なお、上記押圧子57の操作により、リザーバ52内の薬液が主に圧開放弁55側へ流出するのは、上記流量調整装置3の流路により、薬液注入装置10側へ逆流する薬液量が制限されているためである。このように、バイパス通路56に流出した薬液は、当該バイパス通路56に接続された可撓性チューブ58内を通過して、この可撓性チューブ58の先端部に接続された図略のカテーテルを介して患者へ投与されることとなる。

なお、上記カテーテルは、通常、可撓性チューブ58の内腔よりも小さな断面積に設定されているため、このカテーテルを介して薬液を流出させるには、上記押圧子57を大きな力で操作する必要が生じることとなる。そこで、上記可撓性チューブ58の途中部には、薬液を一旦貯留するためのバルーン59が設けられている。このバルーン59は、上記押圧子57の操作によって加圧された薬液が内部に導入されることにより、弾性的に膨らんで、その内部に薬液を収容するようになっている。このように収容された薬液は、バルーン59の復元力によりカテーテル側へ徐々に押し出されることとなる。上記バルーン59の構成については、例えば、特開平11−169458号公報に開示される様々な変形が可能である。

上記のようにPCA装置50は、薬液注入装置10から導出された薬液を塞き止めつつ、貯留するとともに、押圧子57の操作に応じて、予め規定された量の薬液を排出するようになっている。

上記液体側シリンダ11は、光透過性を有する材質で形成され、略円筒状のシリンダ本体12と、このシリンダ本体12の内腔を左側で閉塞する底部13と、この底部13に取り付けられた薬液導入ユニット(薬液導入装置)16と、この薬液導入ユニット16を前記底部13に対して固定する蓋部材17と、上記シリンダ本体12の外周面に配設された目盛り部(図1参照)Mとを備えている。上記底部13の周縁部近傍には、左方向へ延びる略円筒状のチューブ接続部(薬液導出部)14と、このチューブ接続部14及び上記シリンダ本体12の内腔をそれぞれ連通させるように、底部13を左右方向に貫通する導出口13aが形成されている。チューブ接続部14は、上記導出口13aから導出された薬液を可撓性チューブ2の内腔へ案内可能となるように、周知の接着等の手段により、当該可撓性チューブ2と接続可能に構成されている。

また、上記底部13の略中心位置には、左方向へ延びる略円筒状のユニット収容部15と、このユニット収容部15及び上記シリンダ本体12の内腔をそれぞれ連通させるように、底部13を左右方向に貫通する導入口13bが形成されている。ユニット収容部15の内腔には、弾性帯部材20を介して薬液導入ユニット16が挿入されている。この弾性帯部材20は、弾性を有する合成樹脂等で形成された円筒状の部材であり、上記ユニット収容部15の内周部と後述する下流側ハウジング22における小外径部24の外周部との間に縮設され、これら両部間に液密状態を形成するようになっている。

上記上流側ハウジング32は、外周にねじ山(図示せず)が形成された略円柱状の雄ねじ部26aを左側に有するポート部26と、このポート部26の右側に連結された円盤状のフランジ部27と、このフランジ部27の右側に連結され、上記大内径部25a内に収容可能な外径寸法を有する略円柱状の押込部28と、これらポート部26、フランジ部27及び、押込部28を左右方向に貫通する注入孔29とを備えている。

上記注入孔29は、断面略円形の孔であり、注射筒等に形成されたオスルアーテーパーを受入可能となるように、左方向へ先窄まりとなる傾斜を有するテーパー部29a(開口部)と、このテーパー部29aの右側に配設され、当該テーパー部29aの右端部の孔よりも大きな内径に設定された規制孔29bとを備え、これらテーパー部29aと規制孔29bが肩部29cを介して同心に連結されている。上記規制孔29bには、その軸線周り90度間隔で当該軸線へ向けて4本のリブ30が突設され、これらリブ30は、上記肩部29cから規制孔29bの右端部より若干左位置まで延びているとともに、上記規制孔29bの内周面から軸線よりも若干外側の位置まで延びている。また、各リブ30の内側(規制孔29aの軸線側)には、上記規制孔29bの軸線と直交するように配置された規制板31が形成されている。この規制板31は、右方向へ向けて先窄まりの円錐台形状に形成され、上記各リブ30と一体に形成されている。また、規制板31の左側面は、上記ダックビル21における本体部21aの内腔の断面積と略同一の面積に設定されている。

上記薬液導入ユニット16を組み立てる場合には、ダックビル21の嘴部21cを下流側ハウジング22の小内径部25内に配置する一方、ダックビル21の突出部21bを下流側ハウジング22の傾斜部25cの左側に配置して、この状態で上流側ハウジング32の押込部28を下流側ハウジング22の大内径部25aに挿入して、上流側ハウジング32のフランジ部27と下流側ハウジング22のフランジ部23aとを熱溶着等により固定する。このように組み立てると、ダックビル21の突出部21bが下流側ハウジング22の傾斜部25cと上流側ハウジング32の押込部28の右側面との間で挟圧された状態で保持されることとなる。そして、この挟圧された突出部21bによって、薬液導入ユニット16は、貫通孔25に対して右側から導入された薬液がダックビル21の側面を通過して上流側ハウジング32側へ流出すること防止するようになっている。

また、上記のように組み立てられた薬液導入ユニット16は、上流側ハウジング32の注入孔29、下流側ハウジング22の貫通孔25及び、ダックビル21の本体部21aの軸線方向がそれぞれ同一となるように構成されている(すなわち、上流側ハウジング32のテーパー部29aとダックビル21の内腔とが薬液導入方向に沿って配設されている)。一方、上記規制板31は、上記のようにダックビル21における本体部21aの内腔の断面積と略同一の面積に設定された左側面を有するとともに、当該ダックビル21の若干左側で、上記軸線方向を遮るように配置されている。したがって、上記薬液導入ユニット16の右側面視において、上記ダックビル21の本体部21aの内腔が規制板31によって隠蔽されることとなる。換言すると、上記テーパー部29a(開口部)に対してダックビル21の内腔が隠蔽されている。

上記のように構成された薬液導入ユニット16は、その下流側ハウジング22のフランジ部23aの右側面がユニット収容部15の左側面と当接した状態で、当該ユニット収容部15内に収容されている。このように収容された薬液導入ユニット16がユニット収容部15に対する左方向への移動するのを規制するように、ユニット収容部15には、蓋部材17が外嵌されている。

蓋部材17は、上記ユニット収容部15に外嵌される外装筒33と、この外装筒33に対して一端部が連結された連結帯34と、この連結帯34の他端部が接続され、上記外装筒33に対して外嵌可能な蓋体35とを備えている。外装筒33は、剛性樹脂等で形成され、略円筒状で有底の部材である。この外装筒33の底部には、上記ポート部26が挿通可能となるように左右方向へ貫通するポート挿通孔33aが形成されている。また、外装筒33の側面には、上記ユニット収容部15の側面に形成された図略の係止爪と係合可能な固定孔33bが形成され、この固定孔33bと係止爪との係合により、外装筒33がユニット収容部15に対して左右方向で位置決めされるようになっている。この位置決め状態では、外装筒33の底部がユニット収容部15の左側面との間で上記両フランジ部27、23aを挟圧して、薬液導入ユニット16のユニット収容部15に対する左方向への移動を規制している。上記蓋体35は、外装筒33の外周部に対して外嵌可能な内径寸法を有する有底の略円筒状部材であり、上記連結帯34の可撓性を利用して、外装筒33に対して着脱可能に構成されている。

上記気体側シリンダ36は、略円筒状のシリンダ本体37と、このシリンダ本体37の内腔を左側で閉塞する底部38と、この底部38の左側に配設された左右一対の気密部材39とを備えている。底部38の略中央位置には、左方向へ延びる略円筒状に形成され、上記気密部材39を左右方向に位置決めした状態で収容する気密部材収容部38aと、この気密部材収容部38a及びシリンダ本体37の内腔を連通するように左右方向へ貫通する挿通孔38bが形成されている。上記各気密部材39は、弾性を有する合成樹脂等で形成され、挿通孔38bよりも若干小さな内径寸法に設定されるとともに、気密部材収容部38aの内径寸法よりも若干大きな外形寸法に設定されたリング状部材である。
