【発明の属する技術分野】
本発明は例えば輸液セット、採血セット、輸血セット、体外循環回路等の医療器具の途中に配置される混注部材、薬剤(輸液、生理食塩水溶液、血液保存液等)を封入した医薬容器に装着される口部及び医薬容器に接続されたチューブの途中に配置される混注部材であって、これらの医療器具及び医薬容器に薬剤を混注もしくは採取したり、又は血液を採取もしくは注入する時に使用される混注部材及び口部の改良に関する。

【発明が解決しようとする課題】
血液を患者の体内に入れる輸血治療、血液を体外に抜き出す採血検査等、薬剤を患者の体内に入れる輸液治療、または患者の体内から血液を一時的に抜き出す体外循環治療等を行なう際に、血液や薬剤を患者に注入したり採取する器具(例えば輸血セット、輸液セット、体外循環回路、採血セットなど)がある。
輸血セット、輸液セット、体外循環回路、採血セットなどを構成するラインの途中には、数々の構成部材が装着されているが、混注部材は血液を追加したり、検査のために血液を採取したり、また薬剤の追加や緊急時の薬剤投与など重要な役割を果たしている。

通常これらの混注部材は、管状の本体に突設された胴部に栓体が装着され、前記栓体には中央が開口したキャップが冠着されている。
使用に際しては、キャップの中央開口部に露出した栓体の天面に、先端が鋭利な金属製の針を穿刺し、針の後方に装着された容器の内部に封入されている薬剤や血液の注入または空の容器内に血液の採取を行なう。また、持続的な薬剤や血液の注入の場合、針の後方に薬剤や血液の入った容器と連通された延長チューブまたは輸液セット、輸血セットなどがある。その際混注部材に穿刺した針が抜け落ちないよう、粘着テープで混注部材と針基を固定することもある。
使用する針は、従来より先端が鋭利な金属針を使用する事が多かったが、作業者が金属針の先端に触れ、外傷だけでなく感染症に感染する危険が大きいく、近年、先端が鋭利な金属針に代えて、先端に丸みを帯びた形状を有する穿刺部材を使用する方法が考案されている。
先端に丸みを帯びた形状を有する穿刺部材(以下「穿刺部材」と略記する)を混注部材に穿刺する際に栓体天面にスリットを入れて刺通抵抗を低くして、刺し易くする試みがある。
スリットを入れる事により穿刺のきっかけができ、刺通抵抗が低くなり刺し易くなるが、先端が丸みを帯びた分スリットが拡大して穿刺跡が大きくなり、穿刺部材を引き抜いた後、血液や薬剤の液漏れが発生する虞がある。また、スリットを入れたことにより穿刺部材が抜け易くなる虞がある。
そこで、本発明者は以上の課題を解決するために鋭意検討を重ねた結果、次の発明に到達した。

【課題を解決するための手段】
[1]本発明は、管状の本体(2)の天面に突設された管状の胴部(3)に、弾性を有し且つ少なくとも天面にスリット(6)を形成した栓体(5a)を装着し、
前記胴部(3)の外周にネジ溝(10)とネジ山(11)を螺旋状に形成し、
前記ネジ溝(10)に嵌合する凸部(9)を底部に形成し、且つ正面と背面の二ケ所に表示部(12)を形成した締付部材(7a)を、
前記胴部(3)の外周に該胴部(3)の縦方向に対して、回動により上下するように装着し、
前記締付部材(7a)を回動させて下方向に移動させる事により、前記栓体(5)を外周から前記締付部材(7a)により締付ける事ができるように形成し、
前記栓体(5)の締付け具合が前記表示部(12)の方向により識別できる混注部材(1A)を提供する。

【発明の実施の形態】
以下混注部材について薬剤の混注を行なう場合の一例について説明する。図1は参考例の混注部材1の平面図で、図2は図1の混注部材1の締付部材7に装着したハンドル4を回転させ、栓体5を締付けた時の混注部材1の平面図である。図3は図1の断面図で、図4(ハンドル4は、ネジ溝10、ネジ山11と重なるので省略した。)は図2の断面図である。混注部材1は管状の本体2の天面に突設された管状の胴部3に、弾性を有し且つ少なくとも天面にスリット6を形成した栓体5を装着し、前記胴部3の外周にはネジ溝10とネジ山11が螺旋状に形成され、前記ネジ溝10に嵌合する凸部9を底部に形成し、且つ一対の板状のハンドル4を両側部にした締付部材7を、前記胴部3に装着する事により構成されている。また図1において締付部材7は天面の中央部が開口された筒状の締付部材で、栓体5の天上側面から胴部3の外周に亘って装着されている。

栓体5が何もなされてない状態(緩んだ状態)では、前記締付部材7のハンドル4は、管状の本体2と重なるように配置される。
また本体2の側壁には、本体2が指で挾持できるように円形の鍔8が設けられてる。
胴部3の外周に装着した締付部材7の両側部に設けたハンドル4を、図2のように時計方向に90度回転することで、栓体5は前記締付部材7により胴部3の外周から締付けられ、スリット6も同時に締付けられる。
締付けた状態を再び緩める場合は、栓体7に設けたハンドル4を逆時計方向に90度回転させる。
ハンドル4が本体2に対して90度方向(180度、270度、360度、450度も含む。以下の発明の実施形態も同じ)に位置した時に締付力が最大限に達する様に、例えば胴部3に形成したネジ溝10や前記締付部材7に設けられた凸部9の嵌合具合を設定する事ができる。
前記締付部材7や胴部3、栓体5やハンドル4の形状は図1に示したものに限定されず、要するに胴部3の外周から栓体5を締付けることのできる形状であれば何でも良く、ハンドル4と締付部材7を一体にして回動可能な形状であれば何でも良い。また鍔8を設ける位置は本体2の側面に限らず、本体2が指で挾持できるところであれば本体2の天面でもかまわない。

穿刺部材を混注部材1に穿刺する場合、締付部材7の開口部に露出した栓体5の天面のスリット6に前記穿刺部材を穿刺して、その先端を栓体5の底面を越えるまで貫通させる。
穿刺部材の後方に装着されたシリンジ内に封入されている薬剤を投与した後、穿刺部材を栓体5より引き抜いて、締付部材7に設けたハンドル4を時計方向に90度回転させる事により締付部材7の上部から加圧され、栓体5のスリット6を締付ける事により、穿刺部材を引き抜いた跡を完全に締付けて液漏れを防ぐ事ができる。
栓体5の締付け具合は、締付部材7に設けられたハンドル4の方向で識別する事ができる。栓体5が何もなされてない状態(緩んだ状態)では前記ハンドル4は、管状の本体2と並行で重なるような位置になり、前記栓体5が締付けられた状態では、前記ハンドル4は管状の本体2と垂直に交わるような位置になる。
また、穿刺部材を穿刺し、シリンジに入れた薬剤等を持続的に投与する場合は、前記穿刺部材を穿刺したまま、締付部材7に設けたハンドル4を時計方向に90度回転させ、前記と同様に栓体5のスリット6を締付ける事により穿刺部材を締付け、穿刺部材が抜け落ちないように固定することができ、更に穿刺部材とスリットの間から液漏れが無いようにする事ができる。
栓体5の締付け具合は、締付部材7に設けられたハンドル4の方向で識別する事ができる。栓体5が何もなされてない状態(緩んだ状態)では前記ハンドル4は、管状の本体2と並行で重なるような位置になり、前記栓体5が締付けられた状態では、前記ハンドル4は管状の本体2と垂直に交わるような位置になる。

図5は本発明の実施例の混注部材1Aの側面図で、図6は混注部材1Aの締付部材7aに設けたハンドル4を回転させ、栓体5aを締付けた時の混注部材1Aの側面図である。締付部材7aには、締付け具合を識別する為の窓状の表示部12が正面と背面に二ヶ所設けられてる。胴部3には正面と背面に「緩む」、右と左両側面に「締る」の文字が付された栓体5aが装着され、これらの文字は前記表示部12の窓が回動する事により目視できる。栓体5aの締付け具合は、窓状の表示部12の表示により識別できる。栓体5aが何もなされてない状態(緩んだ状態)では、図5のように正面と背面の表示部12に「緩む」と表示される。他方、図6のようにハンドル4を時計方向に90度回転させると、栓体5aは締付部材7aにより上から下に締付けられると同時に右と左両側面の窓状の表示部12に「締る」と表示される。窓状の表示部12を設けた識別表示は、締付部材7(7a)にハンドル4等を設けない場合に特に有益である。

図7はその他の発明の実施例を示す混注部材1Bの平面図で、図8は図7の混注部材1Bの締付部材7bに設けてるハンドル4aを回転させ、栓体5を締付けた時の混注部材1Bの平面図である。図9は図7の側面図で、図10は図8の側面図である。混注部材1Bは管状の本体2の天面に突設された管状の胴部3に弾性を有し且つ少なくとも天面にスリット6を形成した栓体5を装着し、前記胴部3の外周にはネジ溝10とネジ山11が螺旋状に形成され、前記ネジ溝10に嵌合する凸部9を底部に形成した締付部材7bを前記胴部3に装着する事により構成されている。また図7において締付部材7bは天面の中央部が開口された筒状の締付部材で、栓体5の天上側面から胴部3の外周に亘って装着されている。本体2の壁には、胴部3を中心に一対の扇状の表示板13が形成され、締付部材7bには両側部に一対の板状ハンドル4aと底部に一対の扇状の回転板14が装着されている。

栓体5が何もなされてない状態(緩んだ状態)では、図7のように回転板14は前記表示板13の識別表示部分と重ならない部分に位置すると同時に、ハンドル4aは管状の本体2と並行な位置にあるので、表示板13の識別標識(例えば着色、文字等)が表示される。
他方、締付部材7bの側面に設けたハンドル4aを時計方向に90度回転させると栓体5は、締付部材7bにより上から下に締付けられ、スリット6も同時に締付けられる。この時図8に示すように回転板14も90度回転し表示板13の上に重なるので表示板13の識別表示部分は隠れてしまう。
締付けた状態を再び緩める場合は、回転板14に設けたハンドル4aを逆時計方向に90度回転させる。
ハンドル4aが本体2に対して90度方向(180度、270度、360度、450度も含む。以下の発明の実施形態も同じ)に位置した時に締付力が最大限に達する時に、例えば表示板13の識別表示部分が回転板14で隠れる状態にし、胴部3に形成したネジ溝10や締付部材7bに設けられた凸部9の嵌合具合を、設定する事ができる。
前記締付部材7bや胴部3、栓体5やハンドル4a、表示板13や回転板14の形状は図に示したものに限定されず、要するに胴部3の外周から栓体5を締付けることのできる形状であれば何でも良く、締付け具合が識別表示できる回転可能な形状であれば何でも良い。従ってハンドル4aは必要としない場合も有り得る。また前記表示板13と前記回転板14もそれぞれ一対(二箇所)設ける必要はなく締付け具合を識別表示できれば、それぞれ最低一箇所設けるのみで良い。

穿刺部材を混注部材1Bに穿刺する場合、締付部材7bの開口部に露出した栓体5天面のスリット6に前記穿刺部材を穿刺して、その先端を栓体5の底面を越えるまで貫通させる。

栓体5が何もなされてない状態(緩んだ状態)では、回転板14は前記表示板13と重ならない部分に位置すると同時に、ハンドル4aは管状の本体2と並行な位置にあるので表示板13の識別標識が表示される。他方栓体5が締付けられた状態では、回転板14が表示板13の識別表示部分と重なり、ハンドル4aは管状の本体2と垂直に交わるような位置にあるので識別表示は隠れる。
また、穿刺部材を穿刺し、シリンジに入れた薬剤等を持続的に投与する場合は、前記穿刺部材を穿刺したまま、締付部材7bに設けたハンドル4aを時計方向に90度回転させ、前記と同様に栓体5のスリット6を締付ける事により穿刺部材を締付け、穿刺部材が抜け落ちないように固定することができ、更に穿刺部材とスリットの間から液漏れが無いようにする事ができる。
同時に回転板14が回転し表示板13の識別表示部分と重なり、識別表示は隠れてしまう。
栓体5が何もなされてない状態(緩んだ状態)では、回転板14は前記表示板13と重ならない部分に位置すると同時に、ハンドル4aは管状の本体2と並行な位置にあるので表示板13の識別標識が表示される。他方栓体5が締付けられた状態では、回転板14が表示板13の識別表示部分と重なり、ハンドル4aは管状の本体2と垂直に交わるような位置にあるので識別表示は隠れる。

図11は前記胴部3に代えて、その他の発明の実施例を示す混注部材1Cの断面図で、図12は図11の混注部材1Cの締付部材7に設けたハンドル4を回転させ、栓体5bを締付けた時の混注部材1Cの断面図である。(ハンドル4は、ネジ溝10、ネジ山11と重なるので省略した。)混注部材1Cは管状の本体2の天面に突設された管状の前記胴部3に代えて胴部3aを配置したものである。該胴部3aは弾性を有し且つ少なくとも天面にスリット6を形成した栓体5bを装着し、下部から上部に向けて若干ではあるが大径になるように形成され、外周壁には少なくとも一箇所以上好ましくは二箇所以上のV形切り込み部15を設け、且つネジ溝10の深さを部から部にかけて次第に浅く形成されている。また図11において締付部材7は天面の中央部が開口された筒状の締付部材で、栓体5bの天上側面から胴部3aの外周に亘って装着されている。胴部3aの外周に装着した締付部材7の両側部に設けたハンドル4を図12のように時計方向に90度回転させることで、栓体5bは前記胴部3aを介して締付部材7により締付けられる。同時に前記胴部3aの外周に形成されたV形切り込み部15は、締付部材7を下に回動することにより前記胴部3aの上部より下部方向にかけて前記胴部3aの外周を締付ける力が強く作用するので図12のように閉じられる。締付けた状態を再び緩める場合は、栓体7に設けたハンドル4を逆時計方向に90度回転させる。ハンドル4が本体2に対して90度方向(180度、270度、360度、450度も含む。以下の発明の実施形態も同じ)に位置した時が締付力が最大限に達する様に、例えば胴部3aに形成されるネジ溝10や前記締付部材7に設けられた凸部9の嵌合具合を設定する事ができる。前記胴部3aの内径は、下部から上部に向けて若干ではあるが大径になるように形成されているが、締付部材7や栓体5bやハンドル4の形状は図11に示したものに限定されず、要するに胴部3aの外周から栓体5aを締付けることのできる形状であれば何でも良く、ハンドル4と締付部材7を一体にして回動可能な形状であれば何でも良い。
