本発明は、無侵襲的に生体成分情報を計測する無侵襲生体成分計測装置に関する。

【従来の技術】
生体の循環動態は、個人の行動成績や運動成績と関連することが多い。例えば、血中ヘモグロビン濃度は酸素の運搬能力を直接反映することから、特に有酸素系(エアロビックな)運動能力を示す指標として有用であることが報告されている。

このような指標は定期的に計測することが必要であるが、通常の採血による方法では、頻回の計測は困難である。

そこで、簡単な装置構成で指などの生体の一部に合まれる血管の透過像を得、これを画像解析することによって血管サイズやヘモグロビン濃度およびヘマトクリットのような血液成分の濃度を経皮的、非侵襲的に計測しようとする装置が考案されている(例えば、国際特許出願公開番号WO97/24066)。
【発明が解決しようとする課題】

しかしながら、このような従来の分析装置は、血管幅や血液成分濃度などの計測結果を単純に表示できるが、その計測結果をグラフや表にして時系列的な変化を表示したり、また任意の選択情報と結びつけて表示することはできなかった。

この発明は、このような事情を考慮してなされたもので、複数の光学的情報から計測された同一人の生体成分情報に対応した任意選択情報の時系列データを解析結果として表示させることが可能な無侵襲生体成分計測装置を提供するものである。

【課題を解決するための手段】
この発明は、検査対象とする生体に光を供給するための光源部と、光を受けた生体に光学的情報を検出する受光部と、光学的情報をデータ処理するデータ処理部と、データ処理により得られた生体成分情報を出力する出力部と、データ処理に必要なデータを入力したり操作を行う操作部を備え、データ処理部は同一人の生体成分情報と任意に選択された任意選択情報の時系列データを解析結果として出力部に表示させることを特徴とする。
ここで、任意選択情報とは、生体の状態を表す指標であって、例えば、体重、血圧、心拍数、体脂肪率なども含む。

さらに、任意選択情報が同一人の行動成績、運動成績、タイムトライアル成績であることを特徴とする。
ここで、行動成績とは、行動によって生じた結果や成果であって、例えば、運動成績、作業成績、労働成績などを含む。また、運動成績とは、徒競走、重量上げ、やり投げ等の運動の成績を示す。さらに、タイムトライアル成績とは、時間を競う競技におけるタイムなどであり、例えば、短距離走、長距離走、水泳などのタイムトライアルの成績を示す。

【発明の実施の形態】
この発明の生体検査用装置において、生体とはヒトを合む哺乳動物であり、生体の一部とは生体から分離した組織ではなく、生体のありのままの組織の一部であり、例えば指や耳柔などがあげられる。

本発明品の検出部は、光源部と受光部および保持部から構成される。
光源部には、半導体レーザ(以下、LD)やLEDあるいはハロゲン光源が使用でき、直接生体の一部に照射してもよいし、ファイバーを介して照射してもよい。波長としては生体組織を透過し、水の吸収が大きくない600〜950nmの範囲にあることが好ましい。

受光部は、レンズなどの光学系とフォトダイオードやCCDなどの受光素子から構成できる。受光部素子にCCDを用いると、血管部分の濃度分布情報が得られる。受光素子としては、CCDの他にラインセンサーやフォトダイオード・アレイが使用できる。
また、フォトダイオード1個を血管を横切る方向に駆動させて濃度分布情報を得ることもできる。

受光部の光学系は、受光素子にCCDを用いる場合、単にTV用レンズだけを用いて構成してもよい。

また、検出部はより好適な光学情報を得るために、光源部と受光部を生体の一部に対して保持するための保持部材を備えることが好ましい。生体の一部が例えばヒトの手の指である場合には、保持部材は光源部と撮像部との間にその指を離脱可能に保持するような部材であればよく、それには、指を指の形状に合わせた穴や溝に挿入させる方式のものや、指を可動片で両側から挟む方式のものを用いることができる。

本発明品の解析部は、載置部、データ処理部、出力部および操作部から構成される。
解析部の載置部に載置された検出部は、コネクタによって解析部と直結することができる。これらのコネクタは、検出部の光学情報を解析部へ電気信号として送るポートの役割と同時に、検出部と解析部を固定して一体化する役割を担う。検出部と解析部を切り離す場合、検出部を載置部から取り外し、検出部と解析部の間に接続コードを別途設けることで対処できる。

また、検出部と解析部が、予め収納タイプのリール式接続コードで接続されているか、もしくは、赤外線送受信機などのワイヤレス送受信機を備えることにより、別途、検出部と解析部の載置部にそれぞれに凹凸からなる固定部材、磁石、粘着テープ、ネジ、などにより自由に脱着できる構成にしても良い。

解析部は、得られた同一人の複数の光学情報から血液に関する情報および任意に選択された情報の時系列データを解析結果として出力部に表示させることができる。血液に関する情報とは、血液や血流に関する情報であって、具体的には血管径や血液成分濃度(例えば、ヘモグロビン、ヘマトクリット等)、血液成分濃度比(例えば、酸素化率等)などである。任意に選択された情報とは、同一人の健康状態を表す指標であったり、行動成績、運動成績およびタイムトライアル成績あっても良い。また、解析部は、光学情報を解析するために、CPU、ROM、RAMおよびI/Oポートからなるマイクロコンピュータ並びに市販のパーソナルコンピュータを利用できる。

【実施例】
以下、図面に示す実施例に基づいてこの発明を詳述する。これによってこの発明が限定されるものではない。
図1はこの発明の生体用検査装置の構成図を示すブロック図であり、「血管幅計測モード」、「血液成分濃度計測モード」および「酸素化率計測モード」を選択的に実行できる。図1において、検出部1は、血管を合む生体の一部(ここではヒトの指)を照明するための光源部11と、照明された生体部分の光像(ここでは透過光像)を撮像する受光部12を備える。

解析部3は、データ処理部2、出力部24および操作部35から構成され、データ処理部2は、受光部12が生体の一部を時系列的に複数回撮像するとき、撮像された画像ごとにその画像における生体の一部の位置的特徴(ここでは、指の第1関節位置の輪郭におけるくぼみの座標)を抽出する特徴抽出部31と、抽出された各特徴を記憶する記憶部32と、各特徴を比較する比較部33と、比較結果に基づき複数の画像において同一血管部位を合む解析領域を設定する解析領域設定部34を備える。

図2は図lに示す装置の外観斜視図であり、検出部1を載置部4に載置した状態を示している。検出部1はアーム5とハウジング6からなり、アーム5内に光源部11、アーム5に対向してハウジング6内に受光部12(レンズおよびCCD)を内蔵している。アーム5とハウジング6の間に指16を保持し、指16に光を照射してその透過光を透過像として受光部12で受光するようになっている。出力部(液晶モニター)24は、ヒンジ10で解析部3に繋がっており、折り畳むことができる。解析部3には外部記憶媒体の挿入口36があり、外部記憶媒体を挿入し、測定情報等を記録できる。

図3は図1に示す装置を検出部1側から見た側面図であり、載置部3に載置された検出部1は、検出部1のコネクタ7と解析部3のコネクタ8で直結されている。このように、検出部1と解析部3が載置部4とコネクタ7、8を介して一体化しているために、持ち運ぶときに便利である。

図4は、検出部1を解析部3の載置部4から取り外し、コネクタの付いた接続コードで検出部1と解析部3を接続した図を示している。検出部1を解析部3の載置部4から取り外すことにより、測定の自由度が拡がり、座位または臥位等の自由な姿勢での計測可能となる。

図13は光源部11の正面図であり、LED11aとLED11bとLED11cを備えた発光素子からなる。
LED11aとして、中心波長830nm、半値幅40nmのL3989(浜松ホトニクス(株)製)を使用し、LED11bとして中心波長890nm、半値幅50nmのL25656(同上製)を使用し、LED11cとして、中心波長660nm、半値幅40nmのLED(同上製)を使用している。なお、後述するように、「血管幅計測モード」ではLED11aのみを点灯させ、「血液成分濃度計測モード」ではLED11a、11bを、「酸素化率計測モード」ではLED11a、11b、11cを点灯させる。

このような構成において実施される血管幅、血液成分濃度および酸素化率の計測手順を説明する。
(1)血管幅計測モード
このモードにおいては、一つの波長を使って時系列的に複数回血管像を撮像する。
まず、図6において、操作部35を操作して「血管幅計測モード」を設定し(ステップS1)、LED11a(第1波長)によって指16を照明して撮像すると、図8に示すように、指16の輪郭16aと共に、受光部12側の皮膚表面に局在する血管(静脈)像40を合む画像41が得られる(ステップS2)。次に、画像41において解析領域A1を設定する(ステップS3)。

解析領域Alの設定手順は図7に示す手順により実行される。つまり、計測が第1回目であるときには(ステップS31)、血管像40の最もコントラストのよい領域を検索し、決定した領域を解析領域A1として設定する(ステップS32)。なお、解析領域A1は解析領域設定部34により自動的に設定されるが、使用者が出力部24に出力されるモニタ像を見ながら操作部35を操作して手動設定してもよい。

設定された解析領域A1は、画像41の画面をX−Y座標平面として、四角形の各頂点の座標が記憶部32に記憶される(ステップS33)。次に、特徴抽出部31が、画像41において輪郭16aのくぼみから関節位置P1を抽出し、袖出した位置P1の座標を記憶部32に記憶させる(ステップS34、S35)。

また、ステップ31において、計測が第2回目以降である場合には、前のステップにおいて、例えば、図9に示すような画像41aが得られると、記憶されている解析領域A1の座標が読み出されると共に、画像41aから関節位置P2が特徴抽出部31によって抽出される(ステップS36、S37)。

次に、第l回の計測時に設定した関節位置P1と今回抽出した関節位置P2について座標の差△X、△Yが比較部33によって算出される(ステップS38)。そして、△X、△Yがいずれも所定の許容範囲δを越えない場合には(ステップS39)、解析領域設定部34は初斯設定した解析領域A1を△X、△Yだけずらすことにより、新しい解析領域A2を設定する(ステップS40)。

これによって領域A2内の血管部位は、第1回目の計測時に設定された領域A1内の血管部位と実質的に同一となる。このようにして、一人の被検者の指について時系列的(例えば、2時間おき)にn回計測しても、解析領域A1、A2……Anがその都度設定され、常に血管の同一部位についての計測が行われる。なお、ステップS39において、△X、△Yのいずれかが許容値δを越えると、指16が検出部1に対して正常に設置されていないものと判断され、出力部24に「エラー」が表示される。

次に、図6のステップS4においで、プロファイル抽出部21が、設定された解析領域内で血管に垂直な方向の濃度プロファイル(図10)を作成すると、定量化部22は、この濃度プロファイルをベースラインで規格化する。ベースラインは、血管部分以外の濃度プロファイルから、最小二乗法などによって求め、これで図10のプロファイルを図11に示すように規格化する(ステップS5)。このようにすることによって、入射光量に依存しない濃度プロファイルを得ることができる。

演算部23は、この規格化した濃度プロファイル(図10)からピーク高さh1を求め、(1/2)h1における分布輻(半値輻)w1を血管幅として算出し、記憶部25に格納する(ステップS6)。そして、所定回数の計測が完了すると、算出した血管幅からその時系列的変化を表わすグラフや表を作成して表示する(ステップS7〜S9)。

(2)血液成分濃度計測モード
このモードにおいては、2波長について各1回撮像する動作を時系列的に複数回行う。
まず、図6において、操作部35を操作して「血液成分濃度計測モード」を設定し(ステップS11)、LED11a(第1波長)とLED11b(第2波長)とによって順に指16を照明し、それぞれ撮像を行い(ステップS12、S13)、第1波長により得られた画像について、ステップS3と同じ手順、つまり図7に示す手順により解析領域を設定する(ステップS14)。

次に、プロファイル抽出部21が、第1および第2波長により得られた各画像について、図10に示すようなそれぞれの第1の濃度プロファイルおよび第2の濃度プロファイルを作成する(ステップS15)。定量化部22は、第1、第2の各濃度プロファイルをベースラインでそれぞれ図11、図12に示すように規格化する(ステップS16)。

算出されたHGBとHCTは記憶部25に格納される。このような計測が所定回数くり返されると、演算部23は、算出した値からその時系列的変化を表わすグラフや表を作成して表示する(ステップS19、S20)。

(3)酸素化率計測モード
このモードでは複数波長について各1回撮像する動作を(時系列的に)複数回行う。
操作部35により「酸素化率計測モード」を設定する。そして、図14に示すようにLED11a(第一波長:830nm)によって指16を照明し撮像を行う(ステップS51)。次に、LED11b(第2波長:890nm)によって指16を照明し、同様に撮像する(ステップ52)。次に、LED11c(第3波:660nm)によって指16を照明し撮像する(ステップS53)。そして、図7に示す手順で、解析領域を設定する(ステップ54)。

図15は出力部24(液晶ディスプレイ)の表示例である。グラフ表示領域51、測定データ表示領域52、測定情報領域53、メッセージ領域54、データ表示形式切替領域55、コマンド領域56がある。グラフ表示領域51は、計測装置より得られた生体成分情報と任意選択情報を時系列データのグラフとして表示できる。測定データ表示領域52は、グラフ上のある測定時点の測定データを表示できる。測定情報領域53は、測定番号、測定者名、生年月日、性別等の測定者情報が表示される。メッセージ領域54はシステムの状態を示すメッセージや、測定者に動作を促すメッセージを表示できる。データ表示形式切替領域55は、データ表示形式を切り替えるためのアイコンを表示し、グラフ、一覧表、画像等のいずれかに表示を切り替えることができる。コマンド領域は、いろいろなコマンドを実行するアイコンが表示され、各アイコンを選択すると、PCカードへの測定データの記録、ファイルの削除、設定、データの転送、印刷、メンテナンス等を実行できる。

図15のグラフ領域51に示されたグラフは、同一の被験者について、毎日の運動前に計測して得られた血管幅、ヘモグロビンHGB、酸素化率SVIと、任意の選択情報中からタイムトライアルのタイムを選択して入力した測定データ基づいた時系列グラフである。グラフ領域51には示されていないが、任意選択情報として乳酸(乳酸値)、血圧H(血圧最大値)、心拍数が選択、データ入力されており、図3に示した「カーソル」キーで表示領域を切り替えて、グラフ領域51に時系列グラフとして表示できる。また、「カーソル」キーで時系列データのグラフ上の計測点を示すバー(棒)を動かすことができ、グラフ領域51に測定日と測定時間とその時点の測定結果とコメントをデータ表示領域52に表示できる。

このように、計測装置の計測結果と任意選択情報との関係が時系列的に一目でわかり、計測結果の予測(増加または減少傾向)が容易となり、便利である。また、コンディションを反映した項目を任意に選択可能であり、目的に応じたコンディションの管理ができる。

【発明の効果】
この発明によれば、計測結果を時系列データのグラフや表として表示したり、また任意の選択情報と結びつけて表示することができる。これにより、計測結果とコンディションが時系列的にわかり、計測結果の予測(増加または減少傾向)が容易となり、便利である。また、コンディションを反映した項目を任意に選択可能であり、目的に応じたコンディションの管理ができる。
このような無侵襲生体成分計測装置は、測定場所を選ばず、どこでも持ち運びができ、定期的に計測したい有酸素系運動能力指標として有用なヘモグロビン濃度の測定が簡単に短時間に行え、測定結果も時系列データとして表示され、その日のコンディションが一目でわかる利点がある。
