Description

【0001】

BACKGROUND OF THE INVENTION
The present invention relates to a cover-type endoscope that is detachably attached to an insertion portion of an endoscope and is inserted into a body cavity or the like.
[0002]
[Prior art]
Since the endoscope is inserted into a body cavity such as a human body, it is necessary to clean and disinfect it every time it is used, and it must be able to be completely sterilized. Since this cleaning and disinfection requires a great deal of time and effort, an endoscope, particularly a so-called cover-type endoscope that is inserted into a body cavity or the like with its insertion part covered with a cover, has been developed. Has been. The cover-type endoscope has a great advantage in that the endoscope can be reused without being cleaned and disinfected by replacing the cover.
[0003]
Since an endoscope basically performs inspection in a body cavity, as a minimum endoscope observation mechanism, an illumination section that illuminates the body cavity and the interior of the body cavity under illumination from the illumination section. An observation part to be observed is attached to the distal end portion of the insertion part. In addition, as a result of observing the inside of the body cavity, the endoscope is provided with a treatment instrument insertion channel for inserting a treatment instrument such as forceps in order to perform an appropriate treatment as necessary. Here, since the distal end of the treatment instrument insertion channel is open, dirt, body fluid, or the like in the body cavity adheres. In particular, when this treatment instrument insertion channel is also used as a suction passage, the inner surface of the passage is significantly soiled. Therefore, the treatment instrument insertion channel is not provided on the endoscope side, but a passage functioning as the treatment instrument insertion channel is provided on the cover side.
[0004]
Here, the treatment instrument insertion channel is a passage in a direction substantially parallel to the axis of the insertion portion of the endoscope, and the treatment instrument is led out in the extending direction from the distal end portion. Here, when an appropriate treatment is performed by the treatment tool inserted into the treatment tool insertion channel, the treatment tool is operated under observation by the endoscope observation mechanism. When the endoscope observation mechanism provided at the distal end of the insertion portion of the endoscope is provided at the distal end surface of the insertion portion, that is, the direct viewing type endoscope has its observation direction in front of the insertion portion, The front of the treatment instrument insertion channel can be accommodated in the visual field. However, in the so-called side-view type endoscope in which the endoscope observation mechanism is provided on the side surface of the insertion portion, or in the so-called perspective type endoscope in which the observation field is obliquely forward, the treatment instrument insertion channel is provided. If it is derived in the axial direction of the insertion portion, the treatment tool may not enter the field of view of the endoscope observation mechanism. For this reason, when deriving a treatment instrument, the direction must be changed. Even in a direct-view endoscope, it may be desirable to be able to control the direction of the distal end portion in order to control the directionality of the treatment instrument.
[0005]
For the above reasons, a treatment instrument standing mechanism is provided in a side-view type endoscope or the like. This treatment instrument stand-up mechanism forms a void at a portion where the distal end opening of the treatment instrument insertion channel faces, and a treatment instrument stand is provided in the void, and this void is provided in the direction of the observation visual field, that is, in a side view type. In the case of this endoscope, it is opened at the distal end side surface of the insertion portion. The treatment instrument stand is inserted in the axial direction of the insertion portion to change the direction of the treatment instrument guided into the empty space. In order to control the lead-out direction of the treatment instrument, the treatment instrument stand is remotely connected. It is possible to stand up by operation. That is, in order to make the treatment instrument stand stand up and down, the treatment tool stand is mounted on a pivot shaft provided at a predetermined position, and the standing operation wire is connected to the treatment tool stand. Then, the standing angle of the treatment instrument is controlled by pushing and pulling the operation wire.
[0006]
In the case of a cover-type endoscope, when a treatment instrument insertion channel is provided, the treatment instrument insertion channel is provided on the cover side, so that the treatment instrument stand that constitutes the treatment instrument standing mechanism is provided on the cover side. Become. Thus, what provided the structure which provided the treatment tool standing mechanism in the cover side is disclosed by Unexamined-Japanese-Patent No. 6-319678, Unexamined-Japanese-Patent No. 6-64609, etc., for example.
[0007]
[Problems to be solved by the invention]
As described above, the treatment instrument standing mechanism includes not only the treatment instrument stand, but also an operation wire for raising and lowering the treatment instrument stand and an operation means for pushing and pulling the operation wire at hand. If all these members are provided on the cover side, the structure of the cover becomes extremely complicated and the manufacturing cost becomes extremely high. In the case of a cover-type endoscope, it is optimal from the viewpoint of hygiene and the like that the cover side should be discarded after being used only once. However, as described above, the cover configuration is complicated and expensive. In that case, it is not always appropriate to use as a disposable one.
[0008]
The present invention has been made in view of the above points, and an object of the present invention is to simplify the configuration of the endoscope cover by providing the treatment instrument standing mechanism on the endoscope side, and to reduce the cost. It is to be able to manufacture.
[0009]
[Means for Solving the Problems]
In order to achieve the above-described object, the present invention provides an endoscope having an insertion portion in which an endoscope observation mechanism including an illumination portion and an observation portion is mounted on a distal end hard portion, and the insertion portion is detachably inserted. An endoscope insertion channel comprising an endoscope insertion channel having a sealed structure inside and an endoscope cover provided with a treatment instrument insertion channel through which a distal end is opened as a treatment instrument lead-out portion and the treatment instrument is inserted. In the endoscope, the insertion portion of the endoscope is further provided with a treatment instrument stand so as to be able to protrude and retract with respect to the mounting surface of the endoscope observation mechanism portion, and the treatment is provided in the endoscope cover. It is characterized in that a flexible film is provided to cover the upper part of the tool stand.
[0010]
As the insertion part of the endoscope, in the case where the angle part and the flexible part are connected to the distal end hard part, only the part covering the distal end hard part of the insertion part can be configured with a hard member. The part covering the angle part and the soft part must be flexible in the bending direction. When the endoscope insertion channel is formed of a single member, the endoscope insertion channel must be transparent and flexible in the bending direction, and is extremely high because the inside of the body cavity is observed through the cover. Transparency is required. Therefore, the portion of the insertion portion cover portion that covers the distal end hard portion is formed by a distal end cap, and the entire distal end cap is formed of a transparent member or a non-transparent member, and the distal end hard portion of the insertion portion is formed. It is desirable to provide a configuration in which transparent windows are respectively provided at positions corresponding to the illumination unit and the observation unit. In this case, the treatment instrument stand must protrude outward from the inside of the tip cap. Therefore, an opening is formed at a position covering the treatment tool stand in the entire tip cap so as to be flexible. A film is fixedly provided in the opening. In particular, when the endoscope observation mechanism is applied to a side-view type endoscope in which a flat surface portion is formed on the side surface of the distal end hard portion, a concave portion that accommodates the treatment instrument stand on the side surface of the flat surface distal end hard portion. In addition, an illumination window and an observation window are provided at the distal end portion of the endoscope cover, and an opening is formed at the upper position of the recess so that the opening is covered with a flexible film. The distal end cap is provided with the treatment instrument lead-out portion of the treatment instrument insertion channel opened at a position in front of the flexible membrane mounting portion.

DETAILED DESCRIPTION OF THE INVENTION
Hereinafter, embodiments of the present invention will be described with reference to the drawings. First, FIG. 1 shows an overall configuration of an endoscope constituting the cover-type endoscope of the present invention and an endoscope cover. In the figure, 1 is an endoscope and 20 is an endoscope cover.
[0012]
The endoscope 1 includes a main body operation unit 2 and an insertion unit 3 for insertion into a body cavity, and a universal cord 4 is drawn from the main body operation unit 2. The insertion portion 3 is composed of a distal end rigid portion 3a, an angle portion 3b, and a soft portion 3c in order from the distal end side, and the angle portion 3b can be bent at least in the vertical direction (or the vertical and horizontal directions). Yes.
[0013]
The endoscope 1 is a side-view type endoscope. As shown in FIG. 1, a predetermined length from the distal end side of the distal end hard portion 3 a of the insertion portion 3 is a flat portion 5. The flat surface portion 5 is provided with an illumination portion 6 and an observation portion 7. The illumination unit 6 faces the light guide exit end, the observation unit 7 is equipped with an objective lens, and a solid-state imaging device (or image guide) is provided at the imaging position of the objective lens. . The endoscope observation mechanism is constituted by these, but since the endoscope observation mechanism and the internal configuration of the entire insertion portion are well known, illustration and detailed description thereof are omitted.
[0014]
The illuminating unit 6 and the observing unit 7 are configured such that the illuminating unit 6 is provided in the front and the observing unit 7 is provided in the rear in the flat portion 5 of the distal end hard portion 3a. Upright stand accommodating portions 8 are formed in a line. As shown in FIG. 2, the upright stand accommodating portion 8 is opened in the side surface of the distal end hard portion 3 a, and the treatment instrument upright stand 9 is mounted therein. The treatment instrument stand 9 is made of a substantially triangular member, and its distal end protrudes, and this protrusion serves as a treatment instrument guide portion 9a. The treatment instrument stand 9 is supported at the base end side of the stand support part 8 by the pivot pin 10 so as to be rotatable on the wall surface of the distal end hard part 3a, and is always indicated by a solid line in FIG. As shown in the drawing, the treatment instrument stand 9 is in the retracted state located in the stand support section 8 and the treatment instrument stand 9 is shown in phantom lines in the figure. It is made to stand by rotating about the pivot pin 10 so that the tip portion protrudes from the opening of the stand support portion 8.
[0015]
The standing operation of the treatment instrument stand 9 is performed by rotating the operation knob 11 provided in the main body operation unit 2. For this purpose, an operation wire 12 is connected to the treatment instrument stand 9, and by pulling the operation wire 12, the treatment instrument stand 9 can be raised so as to protrude from the stand accommodating portion 8. It is like that. In addition, by adjusting the pulling amount of the operation wire 12, the standing degree of the treatment instrument stand 9 can be controlled.
[0016]
The endoscope cover 20 includes a distal end cap 21, a flexible sleeve 22 provided connected to the distal end cap 21, and an introduction cylinder 23 provided at a proximal end portion of the flexible sleeve 22. As shown in FIG. 3, an endoscope insertion channel 24 and a treatment instrument insertion channel 25 are formed in the endoscope cover 20. The introduction cylinder 23 is formed with an endoscope insertion port portion 23a and a treatment instrument insertion port portion 23b communicating with the endoscope insertion channel 24 and the treatment instrument insertion channel 25.
[0017]
As shown in FIGS. 4 and 5, the connecting portion side of the tip cap 21 to the flexible sleeve 22 is a cylindrical portion 21 a, and the tip side is a flat surface by cutting the cylindrical portion 21 a in the axial direction. The arcuate portion 21b is formed with the portion 28, and the transition portion from the cylindrical portion 21a to the flat portion 28 is a stepped surface 21c. In the distal end cap 21, an endoscope housing portion 26 that communicates with the endoscope insertion channel 24 of the flexible sleeve 22 and a treatment instrument outlet passage 27 that communicates with the treatment instrument insertion channel 25 are formed. The endoscope accommodating portion 26 accommodates the distal end hard portion 3a in the insertion portion 3 of the endoscope 1, and substantially matches the outer shape of the distal end rigid portion 3a, and has a sealed structure. A cavity is formed. The angle portion 3 b and the soft portion 3 c in the insertion portion 3 are positioned in a soft sleeve 22 that is connected to the tip cap 21. On the other hand, the treatment instrument outlet passage 27 is a passage having the same diameter as that of the treatment instrument insertion channel 25 and opens to the step surface 21c of the transition portion from the cylindrical portion 21a to the arc-shaped portion 21b.
[0018]
The distal end cap 21 is made of a non-transparent member, and when the distal end rigid portion 3a of the insertion portion 3 is accommodated therein, the distal end cap 21 is transparent at a position facing the illumination unit 6 and the observation unit 7 provided on the distal end rigid portion 3a. An illumination window 29 and an observation window 30 made of members are formed. An opening 31 having substantially the same shape is formed at a position corresponding to the opening of the upright stand accommodating portion 8 in the distal end hard portion 3a, and a flexible film 32 is attached to the opening 31. . The flexible film 32 is fixed to the inner surface side of the endoscope housing portion 26 by means of adhesion or the like with a slight tension, and when pushed up from the inner side of the endoscope housing portion 26, Can be bulged and deformed.
[0019]
The present embodiment is configured as described above, and when the endoscope 1 is inserted into a body cavity and an endoscopic examination is performed, the insertion portion 3 of the endoscope 1 is directly connected. Instead of being inserted into the body cavity, the endoscope cover 20 is inserted into the endoscope insertion port 23a of the introduction cylinder 23 from the distal end hard portion 3a of the insertion portion 3. Then, as shown in FIG. 3, the distal end hard portion 3 a is fed into the endoscope accommodating portion 26 of the distal end cap 21. As a result, the insertion portion 3 is completely covered with the endoscope cover 20 and is held in a hermetically sealed state. Even if the insertion portion 3 is inserted into a body cavity, each portion constituting the endoscope 1 including the insertion portion 3 Will not be defaced.
[0020]
Since the distal end cap 21 is provided with the illumination window 29 and the observation window 30, the illumination light from the illumination unit 6 provided on the distal end hard portion 3 a is irradiated from the illumination window 29, and the inside of the body cavity by the observation unit 7. Is observed through the observation window 30. Since the tip cap 21 is formed of a non-transparent member except for the illumination window 29 and the observation window 30, even if the illumination light from the illumination unit 6 is reflected by the inner surface of the illumination window 29, the reflected light is observed. There is no risk of entering the part 7 as harmful light. In addition, when the illumination part 6 and the observation part 7 can be made to contact | adhere completely to the illumination window 29 and the observation window 30, you may form the whole front-end | tip cap with a transparent member.
[0021]
In the insertion portion 3, the distal end hard portion 3a is a hard member, but the angle portion 3b and the soft portion 3c closer to the proximal end than the distal end hard portion 3a are flexible in the bending direction. In the endoscope cover 20, the distal end cap 21 that accommodates the distal end rigid portion 3 a is a hard member, and a flexible sleeve 22 is connected to the distal end cap 21, and the angle portion 3 b and the flexible portion 3 c described above are It is located in this soft sleeve 22. As a result, even if there is a bent portion in the insertion path in the body cavity, the bending smoothly follows the bent portion. In addition, when the angle portion 3b is bent, a large resistance does not occur.
[0022]
When the distal end hard portion 3a of the insertion portion 3 in the endoscope 1 mounted in the endoscope cover 20 is inserted to the observation target portion, the endoscope observation mechanism including the illumination portion 6 and the observation portion 7 is used to enter the body cavity. Perform inspection and diagnosis. When an affected area or the like is discovered, treatment such as extraction of the affected area or collection of cells is performed using a forceps or a treatment tool such as a high-frequency treatment tool. Therefore, FIG. 6 illustrates an example of a treatment tool in which a forceps 33 is inserted and treatment such as extraction of an affected area is performed.
[0023]
The forceps 33 is inserted into the treatment instrument insertion channel 25 from the treatment instrument introduction port 23 b of the introduction cylinder 23 in the endoscope cover 20, and is led out into the body cavity from the treatment instrument extraction passage 27 provided in the distal end cap 21. A treatment instrument stand 9 is provided in the insertion portion 3 of the endoscope 1, and the treatment instrument stand 9 pulls the operation wire 12 by operating an operation knob 11 provided in the main body operation section 2. Operate in the direction. Then, the treatment instrument stand 9 stands up around the pivot pin 10 and protrudes from the opening of the stand support part 8. Since this opening is located below the flexible membrane 32 covering the opening 31 in the distal end cap 21, the treatment instrument guide portion 9a of the treatment instrument stand 9 pushes up the flexible membrane 32, and The flexible film 32 is bulged and deformed. As a result, the treatment instrument guide portion 9 a covered with the flexible film 32 protrudes from the flat portion 28 of the distal end cap 21 so that the forceps 33 can be lifted. As a result, the forceps 33 extending in the axial direction of the insertion portion 3 is erected, so that the forceps 33 is within the visual field of the observation portion 7 obtained from the observation window 30 provided in the flat portion 28 of the distal end cap 21. Turn around. And since the standing angle of the treatment instrument stand 9 changes according to the pulling amount of the operation wire 12, the forceps 33 is controlled to have an arbitrary angle within the range of the observation visual field. Therefore, it is possible to surely snip toward the target portion of the treatment.
[0024]
Here, the treatment instrument stand 9 erects the treatment instrument such as the forceps 32 through the flexible film 32. During the standing operation, the flexible film 32 is elastically deformed to generate tension. Due to this tension, a force to push back the treatment instrument stand 9 in the standing state into the stand support portion 8 acts. Therefore, the treatment instrument guide portion 9a of the treatment instrument stand 9 is released from the stand support section 8 if the pulling force of the operation wire 11 by the operation knob 11 is released without applying a return spring or the like to the treatment instrument stand 9. It returns to the state housed inside.
[0025]
When the inspection of the body cavity by the endoscope 1 is completed with the endoscope cover 20 attached, the insertion portion 3 of the endoscope 1 is taken out from the body cavity, but of course it was covered with the endoscope cover 20. It is taken out of the body in the state. Thereafter, by pulling out the insertion portion 3 of the endoscope 1 from the endoscope cover 20, the endoscope cover 20 that has been used and is contaminated with body fluid or the like is separated from the insertion portion 3. 1 is not fouled. Therefore, the endoscope 1 can be used again without being cleaned or disinfected after use.
[0026]
Here, the endoscope cover 20 basically has a simple configuration in which only two passages are formed, and the treatment instrument insertion channel 25 constituting one of the passages is not provided with a treatment instrument standing mechanism. However, since it is a simple passage, the structure is remarkably simplified and can be manufactured at low cost. Accordingly, the endoscope cover 20 is preferably not reused, but is very advantageous from the viewpoint of hygiene by being disposed of.
[0027]
【The invention's effect】
As described above, the present invention simplifies the configuration of the endoscope cover and reduces the cost by providing the endoscope side with the treatment instrument standing mechanism for raising the treatment instrument derived from the treatment instrument insertion channel. There are effects such as being able to be manufactured.
