【産業上の利用分野】
本発明は、患者の連続断層画像またはシングルスライスの断層画像を撮影する前に位置決め用の画像を撮影する画像撮影装置に関する。

【従来の技術】
従来の比較的大規模な病院では業務の細分化が進んでいる。例えば、画像診断業務においても、従前においては診断及び治療を行う医師が、放射線取扱の有資格者である放射線技師により撮影された画像の読影も行っていたが、近年では、射線技師により撮影された画像を専門の読影医が読影し、医師は読影医による読影結果(読影レポート)を見て診断及び治療を行うようになってきている。

従来の画像診断に関する一連の業務の流れを、以下に説明する。X線コンピュータトモグラフィ装置(X線CT)の場合、この業務は、医師が撮影依頼書を放射線技師宛に発行することにより開始される。撮影依頼書には、患者名や患者年齢等の患者固有の情報の他に、撮影部位等の撮影条件に関する情報が書き込まれる。放射線技師はこの撮影依頼書を受けとり、撮影計画を立案する。まず、スキャノグラムと呼ばれる位置決めの投影画像を撮影する。そして、放射線技師は単独で、または医師の指示にしたがってスキャノグラムを見ながらスキャン・コンソールを介して連続断面の各位置を設定する。これにより連続段面の各位置や架台チルト角(傾斜角度)等の条件がコンソール内のCPUにより撮影装置に自動的に設定される。さらに、この位置情報の他に、X線管電圧、X線管電流、X線管と被検体との距離(X線シフト量)、再構成関数、撮影パルス数等の各条件が設定される。

このような撮影計画を経て、実際に撮影(スキャン)が実行され、全角度的な投影データが各断面について収集される。こうして収集された投影データを用いてコンピュータで各断面のCT画像(組織断層画像)が再構成され、これらのCT画像は読影室の読影用ワークステーションに送られ、読影に供される。この読影結果は読影レポートに纏められて、撮影依頼を行った医師の元に搬送される。医師はこの読影レポートを参照しながら診断を下し治療方針を決定する。
このような一連の流れは磁気共鳴イメージング装置であっても同様である。

【発明が解決しようとする課題】
このような従来の画像診断の流れの中で、読影段階における病変部と撮影断面とのずれ等に起因する情報不足により、スキャン計画を変更して撮影を再度実行しなければならない事態が発生することがある。これは主にスキャン計画の立案に読影医が全く介入していないことに起因すると考えられる。再度の撮影は患者スループットの増大を招く。スキャン計画の立案に読影医を介入させながら、迅速な画像診断作業を実現するような作業の流れに沿って構成された画像撮影装置は確立されていないのが現状である。
本発明の目的は、撮影計画の立案に読影医を介入させながら、迅速な画像診断作業を実現する画像撮影装置を提供することである。

請求項1に記載の発明は、被検体に関する投影データを収集するとともに前記被検体に関するスキャノグラムを撮影するために設けられた撮影室に設置されるX線CT架台と、
スキャノグラムを表示する第1表示装置と、前記第1表示装置に表示されたスキャノグラム上に撮影位置を指定するための第1入力装置と、前記第1入力装置で指定された撮影位置で前記投影データが収集されるように前記架台を制御する制御手段と、前記収集された投影データに基づいて断層画像を再構成する再構成装置と、ネットワーク回線に接続するための第1インタフェースとを備え、スキャノグラムを前記第1インタフェースを介して送信するものであり、撮影室に設置されるX線CTコンソールと、ネットワーク回線に接続するための第2インタフェースと、前記第2インタフェースを介して送られたスキャノグラムを表示する第2表示装置と、前記第2表示装置に表示されたスキャノグラム上に前記撮影位置を指定するための第2入力装置と、前記撮影位置のデータを前記第2インタフェースを介して前記X線CTコンソールに送信する手段を有する前記撮影室とは遠隔の読影室に設置される読影ワークステーションを具備することを特徴とする画像撮影装置である。

本発明によれば、遠隔の読影室に居ながらにしてスキャノグラムを用いて撮影位置の変更を行なうことができ、撮影計画の立案に読影医を介入させながら、迅速な画像診断を実現できる。

【実施例】
以下図面を参照して本発明の好ましい実施例を説明する。
<第1実施例>
図1は本発明の第1実施例による画像撮影装置の全体構成図である。複数のX線コンピュータトモグラフィ装置(X線CT)1,2,3がそれぞれ異なる病院A,B,Cの撮影室に設置されている。撮影室とは遠隔の読影室にはそれぞれ、複数の読影ワークステーション4,5,6が設置される。複数の読影ワークステーション4,5,6は、多くの場合、クライアント・サーバ・モデルとして構築されるLAN(ローカル・エリア・ネットワーク)等のネットワーク回線7を介して複数のX線CT1,2,3に接続される。

X線CT1,2,3は、患者の連続断層画像(連続断面の断層画像)、またはシングルスライスの断層画像を撮影すると共に、この撮影を実行する前の撮影計画立案の段階において利用される断面の各撮影位置を設定するための位置決め用の画像(スキャノグラム)を撮影する画像撮影手段であり、図1の構成において複数のX線CTの1つまたは複数が磁気共鳴イメージング装置に置換される可能性もある。断層画像の各位置は、スキャノグラム上の任意の位置及び角度で、例えば図5のような複数の平行なラインカーソルを指定することにより指定される。この断層画像の各位置の情報の他に、撮影計画では、X線管電圧、X線管電流、X線管シフト量(X線管と被検体との距離)、再構成関数の選択、撮影パルス数等の諸条件が設定される。これら諸条件は、位置決め用の画像(スキャノグラム)に重ねて表示されると共に、マウス等によってカーソルを任意の条件に合せて、任意の値に修正可能であるのは勿論である。

複数の読影ワークステーション4,5,6は、従来と同様に、読影医がX線CT1,2,3で撮影された断層画像を読影するためにモニタ表示する他に、本発明独自の機能を有する。つまり、読影ワークステーション4,5,6は撮影計画を遠隔操作で立案する、つまり撮影条件を遠隔操作で設定するための機能を有する。読影ワークステーション4,5,6には、X線CT1,2,3で撮影されたスキャノグラムがネットワーク回線7を介して取り込まれ、読影医によりマニュアル操作されるマウスやキーボード等の入力装置を介してスキャノグラム上に撮影断面の位置が設定される。また、読影ワークステーション4,5,6からは、この撮影断面の位置の他に、X線管電圧、X線管電流、X線管と被検体との距離(X線シフト量)、再構成関数、撮影パルス数等の各条件が設定される。読影ワークステーション4,5,6で設定された撮影条件は、X線CT1,2,3に転送される。X線CT1,2,3は読影ワークステーション4,5,6から転送されてきた撮影条件にしたがって撮影を実行する。

図2は図1のX線CT1と読影ワークステーション4のブロック図である。図2に示した構成と同じ構成のX線CT2,3および読影ワークステーション5,6については説明を省略する。X線CT1は大きく分けて架台10、コンソール11、高電圧発生装置12からなる。撮影断面の全角度的な投影データを収集するための構造物であるところの架台10には、台座機構により垂直軸に対して傾斜可能に且つ回転可能に支持された図示しないリング状の回転架台、架台駆動制御装置13、撮影系14、図示しない寝台、寝台駆動制御装置15、データ収集装置16が含まれる。架台駆動制御装置13は、回転架台を回転駆動する。撮影系14は、撮影領域を挟んで対向して回転架台に装着されたX線管と多チャンネル型X線検出器列とからなる。X線管に高電圧発生装置12から管電力が供給されると、X線管からX線が扇状に爆射される。寝台駆動制御装置15は、寝台を駆動して、被検体を撮影領域に挿入または挿出する。データ収集装置16は、多チャンネル型X線検出器列の各チャンネルで検出した検出信号を個々に増幅し、ディジタル信号に変換する。

スキャノグラムを撮影する場合、回転架台は停止され、X線管および多チャンネル型X線検出器列の連続動作中に寝台により被検体が体軸方向に沿って連続的に移動される。これにより、一方向から見たX線投影画像の如きスキャノグラムが撮影される。一方、全角度的な投影データを収集する撮影の場合、回転架台は回転され、X線管および多チャンネル型X線検出器列の連続または間欠動作中に寝台により被検体が体軸方向に沿ってスライスピッチで間欠的に移動される。これにより撮影計画した各断面についての全角度的な投影データが収集される。

コンソール11は、画像再構成装置17、画像ファイル装置18、画像表示装置19、X線CT全体の制御を行うCPU20、入力装置21、ネットワーク回線7に対するLANインタフェース22を含む。データ収集装置16からのディジタル信号は、画像再構成装置17で信号処理される。これにより画像、つまり断層画像が再構成される。この断層画像のデータは、画像ファイル装置18の磁気ディスクや光ディスク等の大容量記憶媒体に一旦収められ、適宜、画像ファイル装置18から画像表示装置19に読み出され表示される。入力装置21はマウス、キーボードおよびジョイスティック(またはトラックボール)等を備え、この入力装置21を介して、撮影条件、つまり、撮影断面の位置、X線管電圧、X線管電流、X線管と被検体との距離(X線シフト量)、再構成関数、撮影パルス数等の各条件が設定される。また入力装置21を介して放射線技師によりスキャノグラムの撮影操作が行われる。

読影ワークステーション4は、CPU23、画像表示装置24、ネットワーク回線7に対するLANインタフェース25、マウスやキーボード等からなる入力装置26を含む。ネットワーク回線7を介してX線CT1から送られてきたスキャノグラムはCPU23を介して画像表示装置24に表示される。スキャノグラム上には、読影医の操作により入力装置21を介して撮影断面の位置が、例えば平行な複数のラインカーソルとして設定される。この複数のラインカーソルは、被検体の体軸に沿って読影医の所望する任意の位置に、被検体の体軸に直交する垂直軸に対して読影医の所望する任意の傾斜角(架台のチルト角に相当する)に、且つ読影医の所望する任意の間隔(スライスピッチに相当する)に設定される。また、読影医の操作により入力装置21を介して、上記撮影断面の位置の他に、必要に応じてX線管電圧、X線管電流、X線管と被検体との距離(X線シフト量)、再構成関数、撮影パルス数等の各条件が設定される。

まず、図3を用いて本実施例の動作を説明する。実際に連続断面のスキャンを開始する前に、放射線技師によりコンソール11の入力装置21を介してスキャノグラム撮影操作がなされる。つまり回転架台は停止され、X線管および多チャンネル型X線検出器列の連続動作中に寝台により被検体が体軸方向に沿って連続的に移動される。これにより、一方向から見たX線投影画像の如きスキャノグラムが撮影される。このスキャノグラムのデータは、コンソール11のCPU20、LANインタフェース22、そしてネットワーク回線7を介して読影医が待機する読影ワークステーション4(または5、若しくは6)に送られる。スキャノグラムは読影ワークステーション4のCPU23を介して画像表示装置24に表示される。読影医はこのスキャノグラムを見ながらスキャン計画を立案する。つまり、読影医の操作により入力装置21を介して連続断面の各位置が、図5に示すように、平行な複数のラインカーソルとしてスキャノグラム上には設定される。この複数のラインカーソルは、被検体の体軸に沿って読影医の所望する任意の位置に、被検体の体軸に直交する垂直軸に対して読影医の所望する任意の傾斜角(架台のチルト角に相当する)に、且つ読影医の所望する任意の間隔(スライスピッチに相当する)に設定される。

次に、図4を参照して、図3を人の動きから見て説明する。医師(主治医)は患者に対して撮影に関して説明し、看護婦に対して撮影を行う際の介護を指示し、撮影依頼書を発行することにより放射線技師に対して撮影を依頼し、読影医に対して読影を依頼する。

医師から指示を受けた看護婦は、撮影準備として、患者を撮影室の寝台に載置し、態動を極力避けるために患者をベッドに固定する。医師から撮影依頼書を受けた放射線技師は、まずX線CT1を操作してスキャノグラムを撮影する。スキャノグラムのデータはX線CT1から、医師から読影を依頼された読影医が待機する読影ワークステーション4に転送される。読影医は、入力装置26を操作して撮影条件を設定する。つまり、読影医の操作により入力装置21を介して複数のラインカーソルがスキャノグラム上に、被検体の体軸に沿って読影医の所望する任意の位置に、被検体の体軸に直交する垂直軸に対して読影医の所望する任意の傾斜角(架台のチルト角に相当する)に、且つ読影医の所望する任意の間隔(スライスピッチに相当する)に設定される。また、任意のピッチ、その他の条件を変えて複数の計画を同一の計画として行うことが出来る。

このように本実施例によれば、読影医が撮影条件の設定、つまり撮影計画の立案に簡単に介入できる。したがって、読影段階での情報不足に起因する再撮影回数が減るので、患者スループットを短縮することができる。また、読影室に居ながらにして読影医が撮影条件を設定できるので、読影医が撮影条件の設定に介入したとしても画像診断の一連の流れが長時間化することも避けられる。また、読影医が読影室に居ながらにして撮影条件を設定することができるので、再度の患者撮影を行う場合でも速やかに撮影することができる。
<第2実施例>
次に第2実施例について説明する。図6は第2実施例による画像撮影装置の全体構成図である。なお、図6において図2と同じ部分には同符号を付して説明を繰り返さない。本実施例では、X線CT1のコンソール11内の入力装置21と同じ撮影操作入力装置30が読影ワークステーション4内に装備され、スキャノグラムの撮影と断層画像の撮影を、放射線取扱有資格者の読影医が読影室から遠隔操作できるようにしたことが第1実施例に無い特徴である。この撮影操作入力装置30は、入力装置21と同様に、マウス、キーボードおよびジョイスティック(またはトラックボール)等を備える。つまり撮影操作入力装置30を介して読影室のX線管電圧、X線管電流、X線管と被写体の距離等の撮影条件が設定され、またスキャノグラムの撮影操作が行われ、また撮影操作が行われる。

A series of work flows related to conventional image diagnosis will be described below. In the case of an X-ray computer tomography apparatus (X-ray CT), this operation is started when a doctor issues an imaging request form to a radiologist.
