本発明は、欠如歯部の顎骨内に形成された埋入孔内に埋入して固定される通常歯科インプラントと略称されている歯科用補綴物維持装置におけるインプラントフィクスチャーにアバットメントを中間ボルトを介して固定するための歯科インプラントのアバットメント固定用治具に関するものである。

【従来の技術】
従来、欠如歯部における歯科補綴法としては、ブリッジによる方法と、床義歯による方法とが一般に行われてきた。
しかしながら、ブリッジによる方法は、欠如歯部の両側の健全な天然歯を切削して支台とし、この支台に係合する金属体に欠如歯部に位置する歯科用補綴物を固定する方法であるので、健全な天然歯を切削しなければならないばかりか、欠如歯部に位置する歯科用補綴物部分には咬合圧が直接には加わらないために当該部位で骨吸収を引き起こすという欠点があった。
また、床義歯による方法は、合成樹脂等で作成した義歯床に義歯を固定したものを歯科用補綴物とする方法であるが、この方法では歯科用補綴物に作用する咬合力を残存天然歯及び/又は口腔粘膜によって負担させるものであるので、歯科用補綴物使用中に違和感があることや、口腔粘膜組織中に散在する味覚の受容器を義歯床が覆ってしまうことによる味覚の鈍麻が生じることがあり、更には長期間の使用によって顎堤の吸収を引き起こすという重大な欠点があった。

そこでこれらの欠点を解消する治療方法として、欠如歯部の顎骨内に形成した埋入孔内に歯科用補綴物の維持安定装置となるインプラントフィクスチャーを埋入して天然歯における歯根の機能を代行せしめ、このインプラントフィクスチャー口腔内側に固定されたアバットメントに歯科用補綴物の固定装置を固定して歯科用補綴物維持装置とし、この歯科用補綴物の固定装置に歯科用補綴物を固着する歯科インプラントの技術が開発され、実施されるようになってきている。

このような歯科インプラントによる治療方法を実施すると、口腔粘膜を覆うことなく歯科用補綴物を固定することができるので、歯科用補綴物装着時の違和感や味覚の鈍麻が生じることがなく、天然歯に似た使用感が得られ、更に顎骨に適度な咬合力が付与されるためにインプラントフィクスチャーが埋入されていない場合に想定される骨吸収を最小限に抑制できるという利点があるため、この治療方法は急速な発展を遂げて単独歯欠損,2歯以上の局部欠損及び無歯顎(全歯欠損)に至るまでに適用できるようになりつつある。

この歯科インプラントによる治療方法としては、欠如歯部の顎骨内に形成された埋入孔内に埋入されたインプラントフィクスチャーが欠如歯部の顎骨に充分に結合し且つ埋入孔を形成したことによる手術部分が治癒したら、そのインプラントフィクスチャーが埋入された口腔内側の歯肉部分を再度切開してインプラントフィクスチャーの口腔内側部分に歯科用補綴物の固定装置の取付部分となるアバットメントを固定する2回法が採用されており、このアバットメントはインプラントフィクスチャーに対して回転しないように固定され、その口腔内側部分に取り付けられる歯科用補綴物の固定装置が単独歯用の固定装置である場合にはその固定装置がインプラントフィクスチャーに対して回転したのでは歯科用補綴物を欠如歯部に正確に位置せしめられない現象が生じるため歯科用補綴物の固定装置を回転しないように維持する部分がアバットメントの歯科用補綴物の固定装置が装着される口腔内側部分にも設けられている。

このような歯科インプラントによる治療方法において、欠如歯部の顎骨内に形成された埋入孔内に埋入されたインプラントフィクスチャーの口腔内側部分にアバットメントを固定する操作は、インプラントフィクスチャーの口腔内側部分にアバットメントを位置せしめてインプラントフィクスチャーの口腔内側部分に設けられている係合部(通常、角筒状部)とアバットメントのインプラントフィクスチャー側部分に設けられている係合部(通常、角筒状凹部)とを係合させてアバットメントがインプラントフィクスチャーに対して回転しないようにし、アバットメントの中央部に貫通して設けられている貫通穴を挿通させた中間ボルトをインプラントフィクスチャーの口腔内側部分に螺設されているメネジ部に螺着させることによって行われている。

しかしながら、このインプラントフィクスチャーの口腔内側部分にアバットメントを固定する操作は、非常に狭い部分において行わなければならないために、インプラントフィクスチャーの口腔内側部分に設けられている係合部とアバットメントのインプラントフィクスチャー側部分に設けられている係合部とを確実に係合したことの確認が困難であるばかりでなく、中間ボルトをインプラントフィクスチャーの口腔内側部分に螺設されているメネジ部に螺着させる際にインプラントフィクスチャーにその締付けトルクが作用してインプラントフィクスチャーが欠如歯部の顎骨内に形成された埋入孔内で回転してインプラントフィクスチャーと欠如歯部の顎骨との結合が破壊されることがないように細心の注意を払って行わなければならず、しかもアバットメントや中間ボルトは非常に小さな部品であるので不用意に口腔内に落下させて患者が誤って飲み込んでしまうような現象が生じないようにすることが必要である。

更に近年、歯科インプラント治療の歯科用補綴物には、単に咬合機能の回復ばかりでなく審美性の回復も求められてきており、より天然歯に近い形態の歯科用補綴物、即ち歯肉縁下より立上り、口腔内方向に向かって拡がるような立上り形態を有する歯科用補綴物が好まれてきている。その結果、アバットメントもインプラントフィクスチャーの口腔内側から離れるに従って連続的にその直径が大きくなるテーパを持つ形態を持つため、欠如歯部の顎骨内に形成された埋入孔内に埋入されたインプラントフィクスチャーの口腔内側部分でアバットメントを安定性良く保持することが困難であり、確実且つ容易にアバットメントをインプラントフィクスチャーの口腔内側の所定位置に固定する手段の開発が切望されていたのである。

【発明が解決しようとする課題】
本発明は、上記した欠如歯部の顎骨内に形成された埋入孔内に埋入されて結合されたインプラントフィクスチャーにアバットメントを中間ボルトを介して確実且つ容易に固定するための歯科インプラントのアバットメント固定用治具を提供することを課題とする。

【課題を解決するための手段】
本発明者は、上記課題を解決すべく鋭意研究の結果、歯科インプラントのアバットメント固定用治具の構造を、
一端に螺設されているオネジ部に続いて角筒状部と中間円筒状部と段差部を介して最下円筒状部とが順次設けられていてその中央長手方向に貫通穴が穿設されていると共にこの貫通穴の最下円筒状部側に中間ボルトの頭部に形成されている角筒部が嵌合する形状の中間ボルト嵌合部が設けられている中間ボルト連結用部材と、該中間ボルト連結用部材にコ状断面の閉塞部側からその中央に穿設されている貫通穴を貫通し前記段差部で支持されているコイルスプリング保持体と、前記中間ボルト連結用部材に外嵌され一端を該コイルスプリング保持体内に挿入されて保持されているコイルスプリングと、前記中間ボルト連結用部材の角筒部にコ状断面の閉塞部の中央に穿設されている角筒状の貫通穴を外嵌され且つ該コイルスプリングの他端がその内部に挿入されている中間ボルト回転操作体と、該中間ボルト回転操作体の貫通穴から突出しているオネジ部に螺合されているナットと、頭部が該ナット側に位置し且つ該中間ボルト連結用部材の貫通穴内にオネジ部を挿入されている中間ボルト保持用ネジ棒とより成る第1連結部材と、
前記中間ボルト嵌合部に中間ボルトの角筒部を嵌合された状態の中間ボルト連結用部材が摺動状態で貫通可能な貫通穴が中央部に穿設されていると共にこの貫通穴の一端にアバットメントの口腔内側に位置する部分に設けられている角筒状部が嵌合する形状のアバットメント嵌合部が設けられていてこのアバットメント嵌合部が設けられている側と反対側に指先で把持できる把持部が突設されているアバットメント係合用部材と、中央部に穿設されている貫通穴に該アバットメント係合用部材が挿入されていて該貫通穴の一端にアバットメントの口腔内側部分に設けられている角筒状部に続く最大外径部を保持する保持部が設けられていると共に該保持部にバネ作用を付与するための軸方向に平行なスリットが設けられているアバットメント保持部材と、該アバットメント係合用部材と該アバットメント保持部材とを一体に固定しているビスとより成る第2連結部材と、
より成る構成とすることにより解決できることを究明して本発明を完成したのである。

以下、図面により本発明に係る歯科インプラントのアバットメント固定用治具について詳細に説明する。

6は一端に前記ナット2に螺合されるオネジ部6aが螺設されており、このオネジ部6aに続いて前記中間ボルト回転操作体3の閉塞部3aの中央に穿設されている貫通穴3bに嵌合する角筒状部6bが設けられており、この角筒状部6bに続く中間円筒状部6cと更にそれに続く最下円筒状部6dとの間にはコイルスプリング保持体5の閉塞部5aを保持する段差部6eが設けられており、その中央長手方向に前記中間ボルト保持用ネジ棒1の円柱状部1cが嵌挿できる貫通穴6fが穿設されていると共にこの貫通穴6fの最下円筒状部6d側には後記する中間ボルトの頭部に形成されている角筒部が嵌合する形状の中間ボルト嵌合部6gが設けられている中間ボルト連結用部材である。そして、中間ボルト保持用ネジ棒1の頭部1aと先端のオネジ部1bとの間の円柱状部1cにオネジ部1b側が細くなるように段部1dが設けられている場合には、この中間ボルト連結用部材6の貫通穴6fに中間ボルト保持用ネジ棒1の先端のオネジ部1bと螺合するメネジ部6hが螺設されていることが、中間ボルト保持用ネジ棒1の先端のオネジ部1bをこのメネジ部6hと螺合させ更にこのメネジ部6hとの螺合が解除されるまで中間ボルト保持用ネジ棒1を押し込むことにより中間ボルト保持用ネジ棒1が中間ボルト連結用部材6から簡単に脱落しなくなって好ましい。

9はアバットメント係合用部材8のアバットメント嵌合部8bにその角筒状部が嵌合されているアバットメントのその角筒状部に続く最大外径部を保持してアバットメントが落下しないように保持するアバットメント保持部材であり、その中央部にはアバットメント係合用部材8が挿入可能な貫通穴9aが穿設されていて、この貫通穴9aの一端に後記するアバットメントの口腔内側の角筒状部に続く最大外径部を保持する保持部9bが設けられていると共にこの保持部9bにバネ作用を付与するようにその軸方向に平行なスリット9cが設けられており、この保持部9bが設けられている側と反対側の所定位置に螺設されているメネジ部9dにアバットメント係合用部材8のビス穴8dを貫通したビス10を螺着させることによってこのアバットメント保持部材9とアバットメント係合用部材8とが一体に固定されるようになっている。このようなアバットメント保持部材9とアバットメント係合用部材8とをビス10で固定する際に、アバットメント保持部材9のメネジ部9dとアバットメント係合用部材8のビス穴8dとの位置合わせが容易なようにアバットメント保持部材9の保持部9bが設けられている側と反対側の形状はアバットメント係合用部材8のアバットメント嵌合部8bが設けられている側と反対側の部分が丁度係合する形状を成していることが好ましい。

【作用】
上述したような構成部品から成る本発明に係る歯科インプラントのアバットメント固定用治具を使用して歯科インプラントのアバットメント11をインプラントフィクスチャー12に固定する操作について次に説明する。

先ず、本発明に係る歯科インプラントのアバットメント固定用治具を組み立てる操作について説明すると、コイルスプリング4の一端をコ状断面のコイルスプリング保持体5内に挿入してその閉塞部5aで保持し、中間ボルト連結用部材6をオネジ部6aが螺設されている側の端部からコイルスプリング保持体5の閉塞部5aの中央に穿設されている貫通穴5bとコイルスプリング4とを貫通させて挿入して中間円筒状部6cと更にそれに続く最下円筒状部6dとの間に設けられている段差部6eでコイルスプリング保持体5をその閉塞部5aで保持させた状態とし、中間ボルト回転操作体3の閉塞部3aの中央に穿設されている貫通穴3bに中間ボルト連結用部材6の一端に螺設されているオネジ部6aを貫通させてこのオネジ部6aに続いて設けられている角筒部6bを中間ボルト回転操作体3の閉塞部3aの中央に穿設されている貫通穴3bに嵌合させると共にコイルスプリング4の他端をコ状断面の中間ボルト回転操作体3内に挿入させてその閉塞部3aで保持させ、しかる後に中間ボルト回転操作体3の閉塞部3aから突出している中間ボルト連結用部材6のオネジ部6aにナット2を螺合させる。

かくして本発明に係る歯科インプラントのアバットメント固定用治具が完成すると、中間ボルト連結用部材6の貫通穴6fの最下円筒状部6d側に設けられている中間ボルト嵌合部6gに中間ボルト7の頭部に形成されている角筒部7aを嵌合させ、中間ボルト保持用ネジ棒1の頭部1aを回転させて中間ボルト保持用ネジ棒1のオネジ部1bを中間ボルト7の角筒部7aの中央部から螺設されているメネジ部7bに螺合させてコイルスプリング4が殆ど圧縮されない設定状態で中間ボルト7を中間ボルト連結用部材6に保持させる。しかる後にアバットメント11をその角筒状部11c側からアバットメント保持部材9の保持部9b側に移動させてアバットメント保持部材9の軸方向に平行に設けられたスリット9cによるバネ作用で保持部9bを開かせてアバットメント11の軸心に対して直角な外周側平面11bをこの保持部9bに当接させてアバットメント11の口腔内側の角筒状部11cに続く最大外径部を保持させると共に、アバットメント11の角筒状部11cをアバットメント係合用部材8の貫通穴8a内の一端に設けられているアバットメント嵌合部8bに嵌合させる。最後に、この中間ボルト7が保持されている中間ボルト連結用部材6の最下円筒状部6dをアバットメント係合用部材8の貫通穴8a内に挿入する。

このようにして本発明に係る歯科インプラントのアバットメント固定用治具に中間ボルト7とアバットメント11とを装着すると、そのままの状態で口腔内に移送して、アバットメント11の角筒状凹部11a側をインプラントフィクスチャー12の口腔内側に移送して中間ボルト7のオネジ部7cをインプラントフィクスチャー12の口腔内側に螺設されているメネジ部12bに当接させると、中間ボルト7を保持している中間ボルト連結用部材6は第2連結部材のアバットメント係合用部材8の貫通穴8a内を摺動自在であるので第2連結部材で保持されているアバットメント11をインプラントフィクスチャー12の口腔内側部分に当接させることができる。この状態で、アバットメント係合用部材8の把持部8cを指先で把持しておいて中間ボルト回転操作体3の筒状部を回転させると、中間ボルト7はその頭部に設けられている角筒部7aが中間ボルト連結用部材6の中間ボルト嵌合部6gに嵌合されており且つその角筒部7aの中央部から螺設されているメネジ部7bが中間ボルト保持用ネジ棒1の先端に螺設されているオネジ部1bに螺合されて中間ボルト連結用部材6から抜けない状態となっているので、中間ボルト回転操作体3と一体を成して回転して、中間ボルト7のオネジ部7cがインプラントフィクスチャー12の口腔内側に螺設されているメネジ部12bに螺合されていく。
