それぞれ上位からの記載情報が同時複写可能に設けられた複写構成を有する送り状と受領票と裏面に粘着剤が塗布されている荷札票とを順に重ねてなる配送伝票を一単位とし、該複数単位の配送伝票が分離可能に連接されている配送伝票連続フォームにおいて、
前記配送伝票それぞれの送り状と受領票と荷札票とがフォーム連接方向に沿う一側辺側に設けた第一綴じ合わせ部で分離可能に綴じ合わされ、かつ、前記送り状と受領票とがフォーム連接方向に沿うもう一方の側辺側に設けた第二綴じ合わせ部で分離可能に綴じ合わされて、この送り状と受領票とが一体として前記第一綴じ合わせ部から分離可能に設けられ、
前記受領票は、フォーム連接方向に直交する上下辺それぞれに沿う切り欠き部を有してフォーム連接方向の縦寸法が送り状と荷札票の縦寸法より短く設けられ、この受領票に重なる送り状は、上下辺が前記切り欠き部を通して荷札票の上下辺に剥離可能に貼り合わされ、受領票の第二綴じ合わせ部側の辺部が荷札票に剥離可能に貼り合わされていることを特徴とする配送伝票連続フォーム。
