Description

The present invention relates to a hemostatic device, and more particularly, to a hemostatic device for tissue around the cervical sympathetic trunk used for injection of a stellate ganglion block caused by medical treatment or medical treatment.

Stellate ganglion block is one of the treatments frequently used in pain clinic treatment. The stellate ganglion block is used to suppress the function of the sympathetic nerves of the organs controlled by the stellate ganglion by using local anesthetics on the stellate ganglion and the cervical sympathetic trunk around it. A method of treatment, in which the stellate ganglion is located in front of the transverse process of the seventh cervical vertebra. While stellate ganglion blocks are frequently used, deep bleeding may occur postoperatively due to damage to tissues near the cervical sympathetic trunk. Doctors or nurses perform hemostasis for such deep bleeding after surgery, but much work is left for doctors or nurses after surgery, and it is desired to improve the efficiency of such hemostasis work. I have.

As a tool for efficiently performing an arterial hemostasis operation in a puncture part after an artery puncture, there is an invention according to Japanese Patent Application Laid-Open No. 8-140990. The present invention relates to a hemostatic device 100 as shown in FIG. 5, and has a pad 101 large enough to cover a puncture portion for an artery, and the pad 101 can be pressed toward the puncture portion. Such a binding band 102 is provided. The pad 101 is thick and has a thickness of about 3 to 20 mm, has a hardness capable of compressing an artery, and can be temporarily fixed detachably to the binding band 102. Also, if the pad 101 is applied to the puncture part and the binding band 102 is wound and fixed, the pad 101 is pressed against the puncture part. As a result, the puncture portion is compressed and closed to stop the bleeding, the pad 101 can be removed from the binding band 102 and discarded, and can be reused by attaching a new pad 101.
JP-A-8-140990

However, in the hemostatic device according to the above-mentioned Japanese Patent Application Laid-Open No. 8-140990, it is stated that "the pressing action becomes stronger as the thickness of the pad increases, but the pad may shatter or fall down sideways with a small area. It is rare and it is necessary to consider this point. In many cases, a thickness of about 3 to 16 mm is preferable (paragraph number 0009). " Therefore, it is not suitable for use in hemostasis at a bleeding point 20 mm to 30 mm deep from the body surface of the neck.

In view of the above, the present invention is intended to easily and reliably stop deep bleeding near the cervical sympathetic trunk after surgery in the stellate ganglion block used in the treatment of pain clinic. .

In order to solve the above problem, the invention according to claim 1 of the present application is directed to a disc-shaped pressurizer for pressing a puncture hole reaching a bleeding point near the cervical sympathetic trunk from the body surface of the neck, A round rod-shaped adjusting rod connected to the tip end and having a male screw formed entirely or partially, and a predetermined female screw threading hole corresponding to the male screw threading of the adjusting rod is formed substantially in the center. It is characterized by comprising a rectangular pressing plate having a thickness, and a binding band extending from both ends of the pressing plate and having a hook-and-loop fastener attached to an end thereof.

In the hemostatic device in the vicinity of the cervical sympathetic trunk according to the present invention, a pressing plate fixed at a predetermined position by a binding band wound around the patient's neck is used as a reaction force receiver, and the patient's neck is Since the pressurizer in contact with the puncture portion is pressed by a predetermined pressing force, it can be used to stop bleeding at a bleeding point 20 mm to 30 mm deep from the body surface of the neck. In addition, since the distance between the presser and the pressing plate can be freely adjusted by rotating the adjusting rod, the blood can be widely and reliably stopped even for a so-called boar-necked person or a person with a thin neck.

First, the configuration of the hemostatic device according to the present embodiment will be described with reference to FIG.
As described above, the hemostatic device according to the present embodiment includes a pressing plate 21 having a rectangular shape having a predetermined rigidity, binding bands 31 and 32 extending from both ends of the pressing plate 21, and a male screw being threaded. And a compression body 40 in which a compression element 41 is connected to one end of an adjustment rod 42 having a circular cross section.
The pressing plate 21 is a rectangular parallelepiped having a length of 90 mm, a width of 30 mm, and a thickness of 8 mm made of plastic. have. The pressing plate 21 can be formed of a metal plate, a plywood, a ceramic plate or any other suitable plate-shaped body other than the plastic plate. The pressing plate 21 used in the present embodiment is slightly thicker, but has a predetermined rigidity, and is internally threaded with a female screw inside a screw hole formed in a thickness direction of the pressing plate. As long as the thickness is as small as possible, a thin or thick material can be used. A screw hole 22 is formed substantially in the center of the pressing plate 21 in the thickness direction of the pressing plate 21.

Although the binding bands 31 and 32 are extended from both ends of the pressing plate 21, in the present embodiment, the ends of the binding bands 31 and 32 are inserted into insertion holes formed in the small edge of the pressing plate. It is fixed. The fixing means may employ other means such as fixing the binding band and the pressing plate by overlapping. In the above, the pressing plate and the binding band are formed separately, but the central portion of the binding band may be thickened to form a pressing plate, and both sides may be gradually thinned to be integrally molded. The banding material in the case of a separate body can be made of cloth, plastic, rubber, non-woven fabric, or other band-shaped materials. These materials are elastic, non-elastic, and elastic. A combination of these and others can be used as appropriate.
Hook-and-loop fasteners 33 and 34 are attached to the tips of the binding bands 31 and 32 extending from both ends of the pressing plate 21. In the present embodiment, the hook-and-loop fastener 33 is attached to the side in contact with the patient's neck 50 when the hemostatic device 10 according to the present embodiment is attached to the neck 50 of the patient, and the hook-and-loop fastener 34 is attached to the neck of the patient. The surface fastener 33 is a male surface fastener, and the surface fastener 34 is a female surface fastener.
Then, the binding bands 31 and 32 are locked so as to be wound around the neck 50 of the patient by the engagement of the male surface fastener and the female surface fastener. In order to hold the binding bands 31 and 32 in a state of being wound around the neck 50 of the patient, a fastening means such as an adhesive material other than the hook-and-loop fastener or a hook may be used. If used, the locking position can be easily changed corresponding to each patient having a different outer peripheral dimension of the neck, and the pressing force of the pressing plate 21 can be finely adjusted, which is convenient in many cases.

In the present embodiment, the binding bands 31 and 32 are extended from both ends of the pressing plate 21, but only the binding band 31 may be extended from one end of the pressing plate 21. In this case, when the locking means is a hook-and-loop fastener, the surface of the pressing plate 21 on the side opposite to the side in contact with the patient's neck 50 and the side opposite to the binding band 31 with the screw hole 22 interposed therebetween. If a male hook-and-loop fastener is attached to the position, the same effect as that of the present embodiment can be obtained.

Here, the compression body 40 will be described. As described above, the compression body 40 includes the round rod-shaped adjustment rod 42 having a male screw threaded thereon, and the compression element 41 connected to one end of the adjustment rod 42. When the hemostatic device 10 according to the present embodiment is attached to the neck 50 of the patient, the compression body 40 is pierced at substantially the center of the pressing plate 21 so that the compression body 41 contacts the neck 50 of the patient. An adjusting rod 42 is screwed into the screw hole 22 provided. The adjusting rod 42 is a plastic round rod having a diameter of 10 mm and a length of 55 mm, and the compression element 41 connected to one end of the adjusting rod 42 is a disk having a diameter of 13 mm. In the present embodiment, the male screw is threaded over the entire length of the adjusting rod 42, but may be formed only partially. Further, the adjusting rod 42 and the pressing element 41 are integrally formed, and the pressing element 41 is fixed to the adjusting rod 42. However, the pressing element 41 can be rotatably connected.
The compression body 40 can be formed of metal, plywood, ceramic, or any other appropriate rod-shaped body other than plastic, similarly to the pressing plate 21. The shape of the presser 41 is a disk in this embodiment, but may be a polygonal column. In addition, the distal end of the compress 41 in contact with the patient's neck 50 may have a convex spherical shape, but is planar in view of the effect of stopping blood in the tissue near the cervical sympathetic trunk. Is more preferable.

Next, the method of use and operation of the hemostatic device 10 according to the present embodiment will be described with reference to FIGS.
First, the stellate ganglion block will be described with reference to FIGS. The stellate ganglion block is performed by inserting a needle of a syringe 60 into the neck 50 of the patient in the supine position and injecting a local anesthetic or the like into the stellate ganglion 52. Since the cervical sympathetic trunk 51 is located close to the stellate ganglion 52, deep bleeding may occur from the cervical sympathetic trunk 51 after surgery. In rare cases, even patients with no abnormalities in the coagulation system may require time to stop bleeding, and in such cases, deep bleeding may be sustained. If this deep bleeding does not stop, it may develop into a fatal complication. Since this deep bleeding cannot be visually observed, it is necessary to stop the bleeding regardless of postoperative deep bleeding.

Next, hemostasis work and operation using the hemostasis device 10 according to the present embodiment will be described with reference to FIGS. 1, 2 and 4.
When performing the hemostatic action, the compression body 40 is screwed into the screw hole 22 of the pressing plate 21, and the hemostatic device 10 according to the present embodiment in which the distance between the compression element 41 and the pressing plate 21 is about 15 mm is prepared in advance. Keep it. The puncture part after removing the needle of the syringe 60 from the neck 50 of the patient in the supine position is disinfected with a disinfecting gauze, and the compression element 41 is brought into contact with the disinfected puncture part via the disinfecting gauze or directly. Then, while holding the pressing plate 21, the binding bands 31, 32 are wound around the neck 50 of the patient, and the hook-and-loop fasteners 33, 34 are overlapped and locked so that the trachea 53 is not pressed.
Then, the adjustment rod 42 of the compression body 40 protruding from the pressing plate is rotated clockwise with a finger to further increase the separation distance between the pressing element 41 and the pressing plate 21 which is initially about 15 mm, and the neck sympathetic nerve is moved. The tissue near the trunk is compressed by the compression element 41 to start the hemostasis. The final separation distance between the presser 41 and the pressing plate 21 depends on the size of each patient, but is generally about 20 mm to 30 mm. If it exceeds 30 mm, the pressing body 40 becomes unstable and may fall down sideways. In such a case, it is preferable to adjust by changing the overlapping position of the binding bands 31 and 32.

The hemostatic operation is completed by leaving the above-mentioned hemostatic state of the patient in the supine position for approximately 15 minutes. After the hemostatic operation is completed, the locking of the binding bands 31 and 32 is released, and the hemostatic device 10 according to the present embodiment is removed from the patient.
