Description
するためのパロモマイシンもしくはその誘導体またはそ
の無毒性酸付加塩に係わる。
大熱帯病のなかにはリーシュマニア症が含まれる。
び亜種のLeishmania属の鞭毛虫類寄生虫に起因する。こ
の疾患の臨床症状は感染するLeishmania病原虫に従い、
3つの主な形態、即ち内蔵、粘膜皮膚及び皮膚の症状に
分類される(Seventh Programme Report of Tropical D
iseases Research,第7章,The Leishmaniases−UNDP/Wo
rld Bank,W.H.O.,1985)。
侵入する内蔵リーシュマニア症は致死性であることが多
い。L.donovani、L.donovani infantum及びL.chagasiに
起因する内蔵疾患は、アフリカ、インド亜大陸及びラテ
ンアメリカの幾つかの地域で風土病となっており、中
国、地中海沿岸域、南西アジア及びソビエト連邦南部に
おいて散発的に発生している。
かなく、第一級の薬剤と見なされている五価アンチモン
化合物及び二流の薬剤と見なされてペンタミジンまたは
Amphotericin Bがしばらくは公知であった(McGreevy
P.B.及びMarsden P.D.:American Trypanosomiasis and
Leishmaniasis,in chemotherapy of Parasitic Disease
s,Cambell W.C.及びRew R.S.編集,Plenum Press,ニュー
ヨーク及びロンドン,1986)。しかしながら、上記薬剤
はかなり毒性があり、それらの活性は変動する。
ボグルコン酸ナトリウムは、体重1kg当たり20mgの投与
量で30日間非経口投与すると、約80%の治癒率を与え得
る。しかしながら、主に反応の遅さ及び/又は再発に起
因する困難なケースは、少なくとも更に30日間のアロプ
リノール及びスチボグルコン酸ナトリウムを用いた追加

日に体重1kg当たり16mg、20日間、1日の最高1g)また
は上記2つの薬剤の併用(14日間)を筋肉内ルートで投
与することにより実施した比較実験は、(表1に示すよ
うに)以下の結果を与えた。即ち、Pentostam単独では1
0/13の患者が治癒したが、治癒した患者の3/10は、治療
終了後2カ月以内に再発した。Gabbromycin単独では10/
12の患者が治癒し、治癒した患者のなかで再発はなかっ
た。薬剤を併用した場合には16/16の患者が治癒し、11
日間しか治療しなかった1人の患者に再発が見られた
が、この患者は、第2サイクルの治療の後に完全に治癒
した。
より確証された内臓リーシュマニア症と新たに診断され
た患者をランダムにグループに分け、以下の治療を実施
した。

レルギーを示す者、妊婦、ヘモグロビンアッセイ40g/
以下、異常プロトロンビン時間、肝または腎機能不全、
主要な同時感染症、以前にリーシュマニア症に対する薬
剤治療をしたことがある、脾臓塗抹標本−寄生度数(pa
rasite grading)4+未満とした。
患者評価を、臨床歴記述、身体検査、体重、胸部X線、
心電図(ECG)追跡、肝及び腎機能試験、(マラリアを
除外するための)血液写真及び血液塗抹標本、完全尿検
査、検便、プロトロンビン時間、及び、Chulay及びBryc
esonの方法(Quantitations of amastigotes of leishm
ania donovani in smears of splenic aspirates from
patients with visceral leishmaniasis,American Jour
nal of Tropical Medicine and Hygiene,32,1983,475〜
479)による脾臓吸引物塗抹標本検査といった方法で実
施した。
発は希であることが判っていたので、追跡検査を行なっ
た(Manson Bahr,East Africa Kala−azar with specia
l reference to the pathology,prophylaxis and treat
ment,Transactions of the Royal Society of Tropical
Medicine and Hygiene,53,1956,123〜137及びOster,Ad
vances in clinical diagnosis and chemotherapy of l
eishmaniasis in kenya,Insect Science Applications,
7,1968,235〜240)。

突発性の口腔カンジダ症により、1人は外傷により追跡
調査期間中に死亡し、1人の患者は記録できなかった。
従って53人の評価可能な患者(男性37人及び女性16人、
年齢範囲3〜36才、平均年齢18才)が残った。被検者採
用時の臨床症状は、全ての患者に発熱、衰弱、肝臓及び
脾臓肥大、蒼白及び頻拍が見られ、更に95%に栄養失
調、62%に咳、25%に黄疸及び14%に踝水腫が見られ
た。ChungらのComplications of kala−azar and its t
reatment in Kenya,East African Medical Journal,61,
1984,120〜127に報告されているように、被検者採用時
に存在したまたは治療の過程で出現した合併症(compli
cating factor)は、鼻血、肺炎、ぶどう膜炎、咽頭炎
及び胸部痛であった。

月及び6カ月の追跡検査において、それぞれ治療患者の
20%及び5%において脾臓は尚中程度に肥大していた。
体重は治療の最初の10日間は減少し、次いで増加する傾
向にあり、最初(2カ月後)の再検査までに正常に戻っ
た(表2)。踝水腫は治療の最初の10日間で取り除かれ
た。
ビン時間の示度が延長されている患者における鼻血の発
生率は、ビタミンK(1日に10mg、5日間)を用いて治
療することにより著しく低下した。眼球痛及びかすみ目
を伴なうぶどう膜炎は被検者採用時には患者の22.5%に
存在したが、特別な治療をせずとも内臓リーシュマニア
症が治癒したときに取り除かれた。肺炎の発作は9例
(17%)で生じた。このうち5人はSbV単独で治療され
ており、抗生物質治療を必要とし、残りの4人はPMを受
容しており、更に抗生物質薬剤は必要でなかった。

matological abnormalities in visceral leishmaniasi
s,East African Medical Journal,57,1980,634〜640;Wa
sunnaら,Haematological changes in patients with vi
sceral leishmaniasis,Proceedings of the 12th Inter
national Congress of Tropical Medicine and Malari
a,Abstract ThP 5〜3,第270頁)からなり、両者の症状
は、根源のリーシュマニア症が治癒するにつれて改善を
示した。被検者採用時には全ての患者の好酸球数はゼロ
であった。末梢血液の好酸球の再出現は良好な予後を示
唆するものと受け取られた(Lyerlyら,Eosinophilis as
prognostic indicator in East African kala−azar.,
Proceedings of the 2nd Annual Medical Scientific C
onference,KEMRI/KETRI,1981,54〜81)。
変化はなかった。治療によって、総血液ビリルビン及び
BUNアッセイ値は次第に低下し、血清アルブミンはさら
に上昇した。
剤の有効性及び薬剤の安全性のデータを示す。初期の脾
臓寄生度数は3つの全てのグループにおいて平均が5+
であった。終了時は、3つの全てのグループで100%の
臨床的治癒が得られた。PM単独またはSbV単独で治療し
た患者においては数人が部分治癒となったが、これらの
なかでも脾臓寄生度数は決して1+を越えなかった。PM
+SbVで治療した全ての患者は見掛けの治癒を生じた。
あり、PM+SbVグループでは高くなり、SbVグループにお
いては更に高かった。PMとSbVとの差は極めて顕著であ
った(P<0.01)。

ープよりもPM及びPM+SbV治療グループにおいて著しく
高かった(両者における差はP<0.01であった)。

消する結果となった(臨床的治癒)。これに反して、幾
つかの重要な差が寄生虫学的治癒の点で現われた。ここ
では、単独のまたは五価アンチモン化合物と併用したPM
の優越性は明らかである。SbV単独の治療は、このなか
でははるかに有効性が小さいことが立証され、優に45.5
%は失敗であった。同じ実験条件で、単独のまたはSbV
と併用したPMは最高の結果をもたらした(表4)。同じ
治療期間でPM単独は部分治癒の割合が最も高かったが、
再発の割合は最も小さく、従って失敗の合計発生率はた
った21%であった。PM+SbV併用療法は、脾臓寄生虫を
実質的により早期に浄化する結果となり、失敗率は最低
であった(部分治癒と再発の中間の13%)。従って、こ
れら2つの薬剤は相乗作用すると見られる。
域によっても患者によっても変化する(WHO Expert Com
mitteeon Leishmaniasis,1984,上掲)。インドではSbV
治療の期間な治癒率と正の相関を示すことが判った。比
較実験においては、1日にSbV 20mg/kgを40日間投与す
ることにより最高の結果(失敗はたった8%)が得られ
た(Thakurら,Rationalization of regimens of treatm
ent of Kala−azar with sodium stibogluconate in In
dia:a randomized study,British Medical Journal,29
6,1988,1557〜1559)。明らかに上記長さの治療期間
は、該疾患が流行している熱帯の国々の条件下では受入
れの問題を惹起するであろう。本発明者らの研究におい
ては、PM+SbV併用療法は、平均14日間投与したときに
最高の結果(失敗13%)を与えた。このことは、PMを単
独で19日間投与した場合の低い再発率に加え、PM+SbV
療法を用いた場合の再発の発生率は、更に数日間の治療
を追加することにより更に低下し得ることを示唆してい
る。
は極めて顕著である(P<0.01)。薬剤療法は、コスト
及び副作用を考慮した上で選択され得る。PMは安価な薬
物であり、本発明者らの研究においては副作用を生じな
かった。SbVは高価であり、両薬剤の最高の投与量を使
用する場合のPM+SbV併用に対しては尚更である。更
に、SbVは明らかにより多くの副作用を生じる。PMとSbV
との作用の相乗効果によって、投与量を適当に少なくし
て同じく良好な治療結果を得ることができ、コストもよ
り小さく且つ副作用もより少ないという明らかな利益が
ある。更に完全治癒率は、幾分延長しても尚SbV治療に
推奨されるより実質的に短い治療期間で、更なる向上を
見せ得るであろう。
Claims (12)
- 【請求項1】式(I): [式中、R1はヒドロキシ基であり、または必要によって
はC1−C4アルキルまたはフェニル−(C1−C4)アルキル
基で置換されたアミノ基であり、 R2は水素またはヒドロキシ基であり、 R3は水素または塩素であり、 R4は水素またはヒドロキシもしくはアミノ基であり、 R5は水素またはヒドロキシ基であり、且つ R6は水素またはヒドロキシ基である] の化合物またはその無毒性酸付加塩と、内臓リーシュマ
ニア症の非経口治療または予防に有効な五価アンチモン
化合物と、医薬的に許容可能な希釈剤とを含有する内臓
リーシュマニア症の非経口治療または予防に使用するた
めの組成物。 - 【請求項2】前記式(I)の化合物が、 (a)R1が−NH2、−NH−CH2CH3または−NH−CH2CH2CH2
−C6H5であり、R2、R4、R5及びR6が各々−OHであり且つ
R3が−Hである、 (b)R1が−NH2であり、R2及びR6が各々−OHであり、R
3及びR4が各々−Hであり且つR5が−OHまたは−Hであ
る、 (c)R1、R2、R5及びR6が各々−OHであり、R3が−Hで
あり且つR4が−NH2である、 (d)R1が−NH2であり、R2が−Hであり、R3が−Hま
たはClであり且つR4、R5及びR6が各々−OHである、また
は (e)R1が−NH2であり、R2、R3及びR6が各々−Hであ
り且つR4及びR5が各々−OHである ものまたはその無毒性酸付加塩である請求項1に記載の
組成物。 - 【請求項3】前記式(I)の化合物がパロモマイシンま
たはその無毒性酸付加塩である請求項1に記載の組成
物。 - 【請求項4】前記酸付加塩が硫酸塩である請求項1に記
載の組成物。 - 【請求項5】前記五価アンチモン化合物がスチボグルコ
ン酸ナトリウムである請求項1から4のいずれか一項に
記載の組成物。 - 【請求項6】前記式(I)の化合物またはその塩と前記
スチボグルコン酸ナトリウムとの重量比が1:9〜9:1であ
る請求項5に記載の組成物。 - 【請求項7】前記重量比が4:6〜6:4である請求項6に記
載の組成物。 - 【請求項8】前記重量比が4:5であって、式(I)の化
合物またはその塩が1日に体重1kg当たり16mg及びスチ
ボグルコン酸ナトリウムが1日に体重1kg当たり20mgの
量で被検者に投与されるようにされた薬剤を提供する請
求項7に記載の組成物。 - 【請求項9】請求項1に記載の式(I)の化合物または
その無毒性酸付加塩と、スチボグルコン酸ナトリウム
と、医薬的に許容可能な希釈剤とを、式(I)の化合物
またはその塩とスチボグルコン酸ナトリウムの重量比1:
9〜9:1で含有する、内臓リーシュマニア症の治療及び予
防ために非経口投与するのに適した組成物。 - 【請求項10】前記重量比が4:6〜6:4である請求項9に
記載の組成物。 - 【請求項11】前記式(I)化合物またはその無毒性酸
付加塩と、スチボグルコン酸ナトリウムと、医薬的に許
容可能な希釈剤とを合わせて混合することからなる請求
