Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は、関節機構を支持するための固定要素、固定
要素を骨の内部に装着する方法および再構成関節に関す
る。
本発明の好適な実施例を、指関節の再構成に関して以
下詳細に説明する。しかしながら、本発明はこれらに限
定されるものではない。本発明は他の同様な関節、例え
ば足指関節、肘関節等に対しても適用することができ
る。本発明は、また、切断あるいは他の欠損後における
再構成にも適用することができる。
〔従来の技術〕
指−関節の、補綴を伴なう再構成は、最初はリューマ
チにかかって変化したMCP関節に実行された。この目的
として知られる補綴には、スワンソン サイラスチック
（Swanson Silastic）指関節が含まれる。このような関
節はシリコンと同類の材料からなり、細長い形状を有
し、実質的に円形断面の端部を備える。端部は実質的に
円形断面とすることができる。端部は各々の骨の髄腔内
に取付け固定することができる。補綴の中央部分が弾性
的に変形され、これにより事実上の関節機構が構成され
る。
セント ジョージ（The St.George）補綴、接合補綴
も、指−関節として知られている。
〔発明が解決しようとする課題〕
これら従来の補綴の有する問題の１つは、事実上の補
綴機構を支持する固定要素が、補綴が負荷方向へ不都合
に移動されることによって、弛緩し、および／もしくは
周囲の組織を損傷することである。このため、近年、骨
の髄腔内に固定されるチタニウム固定を骨−統合（Osse
o integrate）させる目的で使用する試みがハガード他
による“骨−統合補綴による中手指関節の変換−1986年
スカンジナビア、ジェイ・プラスト・リコンスタ（J.Pl
ast.Reconstr.）外科、20:頁207−208−に記載されてい
るように、行われてきた。口頭あるいは口頭外部の補綴
を骨組織内に永久的に固定することは既に知られてい
る。この歯科骨−統合技術は、この25年来ブランマーク
教授ならびにそのグループによって発展され、顎骨内に
固定物を装着して歯あるいはアーチアタッチメントを保
持するのに優れた効果を挙げてきた。しかしながら、こ
の技術を指関節の再構成に適用しようとしたハガートに
よる実験は、期待を満足することができなかった。この
予期されない結果は、明らかに、この“歯科技術”を指
−関節の補綴再構成に使用する際に遭遇する全く異なる
条件に基因するものである。例えば、既知の技術におい
ては、固定物は骨の長手方向軸線に対して直角に固定さ
れる。指関節においては、固定物は骨の軸線に沿って配
置される。このことは勿論、固定要素上に全く異なる負
荷と応力を発生する。
現在においても、整形外科的補綴外科の主要な問題
は、なお、骨固定要素が弛緩されることである。しかし
ながら、20年間以上に亘る歯科充填の90％以上の成功に
よって今までは考慮する必要がなかった別の問題が多数
発生されてきた。骨−統合方法が適用される場合には、
これまで用いられてきたものとは異なる形式の補綴設計
が要求される。関節機構を、骨固定を妨げることなく取
り換えられるようにするためには、補綴システムは、関
節−機構要素が実際上の骨−固定要素から分離されるよ
うに要素別に分割されなければならない。さらに、もし
２−ステージ法が用いられる場合には、関節機構は、患
者あるいは少くとも患者の再構成された関節が非可動に
保持されない際には、第２ステージにおいて連結可動で
なければならない。したがって、２つの条件が考慮され
なければならない。すなわち、第１図に、関節機構は磨
耗されるので取換え可能でなければならない。第２に、
２−ステージ法を用いるために、関節機構は取換え可能
でなければならない。
〔課題を解決するための手段ならびに作用〕
しかるに、驚くべきことに、前述の技術の問題点ある
いは不利益は、本発明によって除去され得ることが判明
した。
本発明は、疾患中のあるいは骨組織の磨耗もしくは炎
症性分解後の欠陥状態における関節の構造および機能に
関する広範な実験生物学的分析と、骨髄に対する脈管供
給に関する広範な研究とに立脚している。破壊された関
節の軟骨ならびに靭帯の総合的な取換えに際しては、骨
ならびに骨髄組織は構造的ならびに機能的ユニットとし
て取扱わなければならないことが確立された。もし硬質
組織が支持要素として機能される場合には、骨と骨髄組
織との間の協働を絶対に、特に長い期間に亘って、考慮
することが必要である。
このようにして、本発明は、関節に近接する骨格部分
に対する関節表面および靭帯に対して補綴取換えを固定
する場合には、骨髄と骨組織間の相互作用を顧慮しなけ
ればならないという認識に立脚している。このことは、
長い骨の髄腔内における取付け要素は機械的要素によっ
て固定されなければならないことを意味し、前記機械的
要素は、髄腔の壁内における骨髄と緻密骨との間の境界
層に特に注意して、技術的に可能な限り、固定要素の全
長に沿って骨髄と骨組織との間に挿通を許容されなけれ
ばならない したがって、本発明は、関節機構を支持するための固
定要素に指向され、この固定要素は、実質的に回転的対
称形で、少くとも部分的に中空部を有すると共に、骨の
組織に適合する材料を含む。固定要素は、少くとも部分
的に組織と骨−統合され得る表面を有し骨の長手方向軸
内で永久的骨内固定を達成する。本発明は、また、再構
成関節にも指向される。
本発明は、また、固定要素を骨内に装着する方法を指
向し、この方法は、骨の髄腔を露出するために関節に近
接する骨を切断し、関節に近接する髄腔内に空間を形成
し、空間内に案内スリーブを装着し、そして案内スリー
ブを用いて固定要素の中心を位置決めしながら固定要素
を髄腔内にねじ込む工程から構成される。
本発明の別の特徴ならびに利点は、本発明の好適な実
施例につき添付参照図面を参照しながら記述する以下の
説明から明らかとなるであろう。
〔実施例〕
第１図に、指関節機構１の両側における長い骨の一部
分を、それぞれ参照符号２および３で示す。
回転的に対称な固定要素４は中空に、実質的にはスリ
ーブ状のボディに形成され、その外側にはねじ６を有す
ると共にその壁厚を次第に減少されている。ボディ５の
開口挿入端部７は先細に形成され、この開口端部７から
始まるスリット８および９を備える。
関節機構１に最近接する固定要素４の端部13（第６
図）は、案内スリーブ11内に配置される。図示されるス
リーブ11は、実質的に円筒状に形成されその半径方向外
表面に沿って長手方向の波形を備える。この波形は、好
適には、歯科医術に使用される先細回転−対称形固定物
の軸方向断面と同一とすることができる。案内スリーブ
11は連結端部13を包囲する。ボディ５の連結端部13は、
これにより、ボディ５が骨組織内に充分ねじ込まれる
と、案内スリーブ11の内面14に対する楔効果によって固
定される。連結端部13は関節機構１の連結部分15を脱着
可能に受入れる。連結部分15は、関節機構１から突出す
る中心部合釘（第７図）とすることができる。この合釘
は、固定要素４内の対応する凹部16あるいは好ましくは
関節機構１との間に配置される連結部材（図示せず）と
協働する。
１つもしくはそれ以上の穴を固定要素４の半径方向外
側表面に直接関連させ、表面に面する穴の端縁部を切断
刃部に形成することができる。このようにすれば、ボデ
ィ５が骨２内にねじ込まれる時に、自己−タッピングが
達成される。除去された骨組織17は、第５図に示すよう
に、固定要素４の内部に取上げられる。
固定要素４の壁厚は挿入端部へ向けて次第に減少され
ており、および／もしくは要素４は長手方向のスリット
８および９をさらに備えているので、必要とされる良好
な変形が達成される。したがって、従来の設計において
大きな問題を提起していた応力集中の危険性が大幅に減
少される。第１図における矢印Ｂ−Ｂは、開口端部７の
柔軟性、すなわち周囲組織に対する適応性を示してい
る。同様のことが、矢印Ａ−Ａに対しても適用される。
スリット８および９は好適には切断刃部を備えられ、
かつその中空空洞部は、第５図に示すように、切除物17
を取上げるので、固定要素４はそれ自体が処理工具を形
成する。これと同時に、正常な解剖学的ならびに生理学
的状態に対する最適条件は、残存する生物学的組織すな
わち髄ならびに骨組織に対する障害を最低限に抑えるこ
とを保証する。
第２図に、固定要素４を配置して達成された関節再構
成を示す。
第３図に、固定要素４を骨２中に配置する方法を示
す。固定要素４は、使用に際して、髄と骨組織の間の境
界域に配置される。スリーブ状のボディ５の壁厚は、ス
リット挿入端部７へ向けて次第に減少され先細り切断刃
部12に終端する。長手方向スリット８の端縁部もまた切
断刃部の形状に形成することができる。これにより、固
定要素４が髄腔内へねじ込まれる時に発生される切除物
17は、第５図に示されるように、ボディ５の内部空洞ス
ペース内に搬出される。
手術に際しては、長い骨２の関節に接続する部分が切
断され、髄腔が露出される。次いで、探り針（図示せ
ず）が適宜の軸方向長さの固定要素４を配置するため
に、髄腔内に挿入される。探り針は、固定要素４の中心
を所望に位置させるための方向測定器として用いられ
る。次いで、案内スリーブ11の穴が露出された髄腔内に
注意して穿孔される。案内スリーブ11を装着した後に、
スリーブ状のボディ５が案内スリーブ11を通して髄腔内
にねじ込まれる。探り針は、その前に勿論取り出され
る。固定要素４の連結部分13が参照符号18で示されてい
るように幾らか外向きかつ上向きに拡開され、その直径
を案内スリーブ11の下側開口よりも少し大きめに形成さ
れる。これにより、連結端部13と案内スリーブ11との間
に（スリーブ11が第６図に破線で示されている位置へ移
動された際に）最終的に楔効果が発生し、案内スリーブ
11とスリーブ状ボディ５の双方が所望の位置に配置され
る。
固定要素４と案内スリーブ11はチタニウムから構成も
しくは製造されるか或いはチタニウムを被覆される。チ
タニウム表面の構造は、少くとも重要な表面部分に関し
ては、統合を促進するよう形成される。好ましくは、10
ないし1000nm、好適には10−300nmの点食径を有する。
スウェーデン国特許第7902035−０号に記載されるよう
な、微細−点食からなる不規則性を付与する。代案とし
て、化学構造のチタニウム表面層が、例えばスウェーデ
ン国特許第8505158−９号に記載されるように、明確な
組織反応に与えられることができる。
第８図に、固定要素４のフランジ20に対面するスリー
ブ11の上端部19に突出端縁部21が設けられ、前記フラン
ジ20上のねじ22と協働するよう配置されている状況を示
す。これにより、要素４とスリーブ11とは、ボディ５が
骨髄内に充分にねじ込まれた際には、非−回転的に固定
される。
代案として、スリーブ11と要素４との間の固定効果は
第９図に示される装置によっても達成されるが、この場
合には、軸方向に配置される波形がスリーブ11の内面と
協働するよう配列されている。
さらに、固定効果は、第10図に示す装置によっても要
素４の上部リム上の特殊ねじ24を介して達成されるが、
この場合、前記特殊ねじは組織内にねじ込まれる。
以上、本発明を好適な実施例について説明したが、多
くの変形もしくは変更態様ならびに他の用途は、同業者
には自明のことである。したがって、本発明は、特定の
開示に限定されるものではなく、添付される特許請求の
範囲によってのみ限定されるものである。

Claims (14)

    (57)【特許請求の範囲】
  1. 【請求項１】関節機構を支持するための固定要素におい
    て、前記固定要素は実質的に回転的対称形で、少なくと
    も部分的に中空部を有すると共に、骨の組織に適合する
    材料を含み、前記固定要素は少なくとも部分的に骨の組
    織と骨−統合され得る表面を有し骨の長手方向軸内で永
    久的骨内固定を達成し、さらに前記固定要素は変形可能
    な開口挿入端部を有している、関節機構を支持するため
    の固定要素。
  2. 【請求項２】前記固定要素の前記材料はチタニウムであ
    る請求項１記載の固定要素。
  3. 【請求項３】前記固定要素の前記表面はチタニウムで被
    覆されている請求項１記載の固定要素。
  4. 【請求項４】関節機構は再構成された指関節の要素であ
    る請求項１記載の固定要素。
  5. 【請求項５】前記固定要素の前記表面は半径方向外側表
    面であり、少なくとも部分的に10−300nmの範囲の直径
    で微細点食されている請求項１記載の固定要素。
  6. 【請求項６】前記固定要素はスリーブ状のボディからな
    り、このボディは半径方向外側表面に外ねじを有し前記
    固定要素を骨内に螺入せしめる請求項１記載の固定要
    素。
  7. 【請求項７】さらに、前記半径方向外側表面に直接的に
    関連する穴を有し、前記穴は前記半径方向外側表面に切
    断刃部を備える請求項６記載の固定要素。
  8. 【請求項８】前記固定要素はスリーブ状のボディからな
    り、このボディは壁部を有し、前記壁部の厚さは前記挿
    入端部に向けて次第に減少される請求項１記載の固定要
    素。
  9. 【請求項９】前記固定要素は壁部を有するスリーブ状の
    ボディとさらに前記壁部を通る長手方向のスリットから
    なり、前記スリットは前記挿入端部から関節機構へ向け
    て延在する請求項１記載の固定要素。
  10. 【請求項１０】前記スリットは切断刃部を有する請求項
    ９記載の固定要素。
  11. 【請求項１１】前記スリーブ状ボディは関節機構に近接
    して位置される連結端部とおよび案内スリーブとを含
    み、前記連結端部は、前記案内スリーブが前記連結端部
    を包囲するように、前記案内スリーブ内に配置されかつ
    非回転的に固定される請求項10記載の固定要素。
  12. 【請求項１２】前記連結端部は関節機構に対面する開口
    端部を画成し、関節機構を前記固定要素に対して脱着可
    能に適合せしめる請求項11記載の固定要素。
  13. 【請求項１３】骨と、 前記骨に連結される関節機構と、 前記関節機構を支持する固定要素とからなり、前記固定
    要素は実質的に回転的対称形で少なくとも部分的に中空
    部を有すると共に、前記骨の組織に適合する材料を含
    み、前記固定要素は少なくとも部分的に骨の組織と骨−
    統合され得る表面を有し、これにより前記骨の長手方向
    軸内で永久的骨内固定を達成し、さらに前記固定要素は
    変形可能な開口挿入端部を有している関節再構成。
  14. 【請求項１４】前記固定要素は関節機構に近接して位置
    される連結端部とおよび案内スリーブとを含み、前記連
    結端部は前記案内スリーブ内に配置されかつ非−回転的
    に固定される請求項13記載の関節再構成。
