Abstract
Description
本発明は、NIHにより認められたNIGMS GM108505の下、政府支援により行った。政府は、本発明における一定の権利を有する。
本出願は、印刷された紙面による複写の代わりにCD-Rにより提出された「冗長な」配列表を含み、この全体を参照により本明細書に組み込む。2019年12月17日に記録された上記CD-Rは、「CRF」、「Copy 1」及び「Copy 2」とそれぞれ標識し、それぞれが、唯一同一である280,467,968バイトのファイル(MS-0006-P2_SL.txt)を含む。
本出願では、2019年12月20日に出願された米国特許仮出願第62/951,167号の利点を主張し、この全体を参照により組み込む。
本明細書において他に定義しない限り、本開示と関連して使用する科学的及び技術的用語は、当業者により一般に理解される意味を有するものとする。用語の意味及び範囲は、明白であるべきであるが、任意の潜在的曖昧さを有する場合には、本明細書において提供する定義は、任意の辞書又は外部定義に優先する。更に、文脈により他に必要とされない限り、単数形の用語は複数性を含むものとし、複数形の用語は単数形を含むものとする。
以下の説明では、特定の製剤及びこの一部を形成する特定の実施形態に言及する。発明を実施するための形態及び特許請求の範囲において記載する例示的実施形態は、制限することを意図していない。本発明の対象の範囲から逸脱することなく、他の実施形態を利用してもよく、他の変更を行ってもよい。本開示の態様は、非常に多種多様な構成により手配、置換、複合、分離、及び設計することができ、これらのすべてを本明細書において検討する。
発明者らは、ヒト腸微生物叢においてIBDと相関する特異的マーカーを同定した。このようなマーカーを同定するために、発明者らは、無菌C57B1/6Jマウスに、血縁若しくは非血縁の健常ドナー(n=15)又はIBDを有するドナー(n=15)のいずれかの2つの独立的コホート由来の糞便スラリー又は一連の培養糞便微生物叢採取物によりコロニー形成させた。以下のTable 1(表1)を参照されたい。
これまでの研究のように(Faithら2014、Geva-Zatorskyら2017)、微生物叢の大多数により、腸FoxP3+Tregの増殖がベースライン無菌レベルを超えて引き起こされた(図2A及び2B)。発明者らは、FoxP3+Treg及びIL-10+CD4 T細胞の割合は、ドナー微生物叢により有意に影響されるが(p<1×10-15、p<0.0001[FoxP3結腸、回腸]、p=0.006、p=0.02[IL-10結腸、回腸];ANOVA)、健常又はIBD関連微生物叢により誘導されたFoxP3+Treg又はIL-10+CD4 T細胞の平均的割合の間で観察可能な差は存在しないことを発見した(図2C及び2D)。CD4+ T細胞由来IL-10の大多数は、細胞内サイトカイン染色により、FoxP3+ T細胞において検出された(図2E)。加えて、微生物叢により誘導されたRORγt+Th及びFoxP3+Tregの割合は、相関しなかった(図2F)。
最近、FoxP3及びRORγtを同時発現するTreg(RORγt+Treg)の腸特異的サブセットが、微生物叢依存性、腸炎防御性であり、腸エフェクターT細胞応答を制御することが見出された。発明者らは、結腸及び回腸におけるRORγt+Tregの誘導が、種々の微生物叢により有意に異なることを見出した(結腸及び回腸においてp<1×10-15、p<1×10-8、ANOVA;図3A及び3B)。全FoxP3+Treg集団とは対照的に、発明者らは、結腸及び回腸の両方において、健常微生物叢により誘導されたRORγt+Tregの、IBD微生物叢と比較して有意な増殖を観察した(p<0.001、t検定;図3C)。この差は、微生物叢ドナーの2つの独立的コホートにわたって(図3D)及び便又は培養微生物叢によりコロニー形成させたマウスにおいて(図10)有意であった。全FoxP3+Tregの割合は、回腸においてRORγt+Tregと相関し(p<0.001;R2=0.39)、結腸において弱相関した(p=0.04、R2 128=0.1;図10D)。コロニー形成により、mLNにおいてRORγt+ Tregの割合は増加したが、健常又はIBD微生物叢によるmLN RORγt+ Tregの誘導において有意差は存在しなかった(図3E)。結腸及び回腸におけるRORγt+Tregの割合は相関したが、いずれもmLNにおける割合とは相関しなかった(図3F)。SPFマウスにおけるこれまでの報告と同様に(Sefikら2015)、健常又はIBDのいずれかの微生物叢によりコロニー形成させたマウス由来の結腸RORγt+133Tregは、ex vivoで刺激した場合、FoxP3-RORγt+Thと比較して最小限のIL-17Aを分泌した(図3G)。
腸炎惹起性に対するIBD関連微生物叢の影響の程度を評価するために、発明者らは、腸炎のノトバイオートマウスモデルにおいて健常及びIBDドナー微生物叢を検査した。IBD病態生理におけるT細胞の公知の重要性を考慮して、発明者らは、ともにT細胞及び微生物叢依存性の腸炎のモデルを選択した。Rag欠損マウスへのCD45RBHI(ナイーブ)CD4T細胞の移入により、腸炎様の病態が誘導されるが、免疫原性微生物叢の存在下においてのみである(以降はRagT細胞移入、(RagTCT)モデル)。T細胞移入の4~8週前に、発明者らは、無菌Rag1-/-マウスに、健常(n=16)又はIBD(n=14、Table 1(表1)を参照)の両ヒトドナー由来の糞便微生物叢によりコロニー形成させた。同一のヒトドナー微生物叢によりコロニー形成させたB6及びRag-/-由来の微生物叢のアルファ多様性(シャノン)は、有意に相関し(r2=0.6、p=0.002、f1検定)、マウスモデル間で類似の生着を示した。腸炎モデルの反復毎に含まれた対照微生物叢は、実験間で低い多様性を実証した(図11A)。体重の減少、組織学、及び糞便リポカリン2(LCN2)の上昇により測定した場合、腸炎は、IBDを有する個体由来の糞便微生物叢によりコロニー形成させたマウスでは、健常ドナー由来の微生物叢によりコロニー形成させたマウスよりも重度であった(42日目の体重及びLCN2のそれぞれにおいてp=4.2×10-5、p=0.0058、t検定;図4A及び4B)。体重の減少は、糞便LCN2の上昇と相関した(R2=0.33、p=1.4x10-7;図11B)。
発明者らは、非曝露ノトバイオートB6マウスにおいて観察したCD4 T細胞応答の変動が、同一ドナー微生物叢によりコロニー形成させたRagTCTマウスにおいて、腸炎重症度とどのように相関するかを調べた。合計で健常15種及びIBD 14種の微生物叢を、両モデルにおいて検査した(Table 1(表1))。腸炎重症度がGATA3+Th又はFoxP3+Tregと相関しない一方(図5A)、B6マウスにおいて微生物叢により誘導された結腸及び回腸RORγt+Thの割合は、同一微生物叢によりコロニー形成させたRagTCTマウスにおける腸炎重症度と正に相関した(結腸及び回腸のそれぞれにおいてR2=0.32、p=0.001;R2=0.26、p=0.004;図5A)。B6マウスにおけるRORγt+Tregの誘導は、RagTCTマウスにおける腸炎重症度と逆相関した(結腸及び回腸のそれぞれにおいてR2=0.29、p=0.002;R2=024、p=0.006;図5A)。また、結腸において誘導されたIL-17A+ CD4 T細胞の割合は、腸炎重症度と弱い関連を有した(R2=0.25、p=0.013;図S5)。線形モデルにより、腸炎重症度における53%の変動(6週目における体重減少)が、両組織におけるRORγt+Th及びRORγt+Tregの両方の割合の関数として説明された(R2=0.53、p=0.002;F検定)。腸炎重症度は、Helios+Treg、IL-10+又はIFNγ+237 CD4 T細胞とは関連しなかった(図S5)。受信者動作特性(ROC)曲線は、ロジスティックモデルを使用するヒト微生物叢ドナーの健康状態の予測において、ヒト化微生物叢マウスデータの値を評価するために使用した(図5B)。結腸RORγt+Thの割合は、合理的な予測値を有したが(AUC=0.71)、結腸RORγt+Tregの割合は、より有益であった(AUC=0.92)(図5B)。また、T細胞移入から6週後に体重減少により測定した場合の腸炎重症度は、ドナーの健康を高度に予測した(AUC=0.93)(図5B)。発明者らは、6週における結腸RORγt+Tregの割合及び腸炎重症度をロジスティックモデルにおいて複合する場合に、最良の予測力を見出した(AUC=0.95)(図5B)。

Claims (18)
- RORγt+Th細胞の量を所定レベル未満に低下させるか、又は対象におけるRORγt+Treg細胞レベルの量を、所定レベルを超えて上昇させる治療組成物を投与する工程を含む、対象において炎症性腸疾患を治療する方法。
- 対象のGIにおいて細菌群1を所定レベルまで生着させる対象に、治療組成物を投与する工程を含む、請求項4に記載の方法。
- 対象のGIにおいて細菌群2を所定レベルまで生着させる対象に、治療組成物を投与する工程を含む、請求項4に記載の方法。
- 対象のGIにおいて細菌群3を所定レベルまで生着させる対象に、治療組成物を投与する工程を含む、請求項4に記載の方法。
- 対象から試料を得る工程と、無菌環境において前記試料をインキュベートする工程と、RORγt+Th細胞又はRORγt+Treg細胞の存在をスクリーニングする工程と、前記RORγt+Th又はRORγt+Treg細胞レベルを所定の量と比較する工程とを含む、対象において炎症性腸疾患を診断する方法。
- 核酸配列の単離を含む、請求項1に記載の方法。
- 試料に対してフローサイトメトリーを実施する工程を含む、請求項1に記載の方法。
- (a)対象のGIにおける細菌群1、細菌群2又は細菌群3のいずれかのレベルを決定する工程と、(b)前記量を所定レベルと比較する工程と、(c)対象のGIにおける細菌群1、細菌群2又は細菌群3のいずれかのレベルを所定レベルまで拡大させる対象に、治療組成物を投与する工程とを含む、対象において炎症性腸疾患を治療する方法。
- 細菌群1のレベルを、所定レベルを超えて上昇させる工程を含む、請求項8に記載の方法。
- 細菌群2のレベルを、所定レベルを超えて上昇させる工程を含む、請求項8に記載の方法。
- 細菌群3のレベルを、所定レベルを超えて上昇させる工程を含む、請求項8に記載の方法。
- 治療組成物が、ワクチン、アジュバント、生物製剤、医薬組成物、プロバイオティクス、食品、飲料、糞便移植、細菌組成物、若しくは動物モデルにおいて使用される試薬、又はこのような成分の組合せを含む、請求項1に記載の方法。
- (a)対象のGIにおける細菌群1、細菌群2又は細菌群3のいずれかのレベルを決定する工程と、(b)治療組成物を前記対象に投与する工程と、(c)前記対象の前記GIにおける細菌群1、細菌群2又は細菌群3のいずれかの前記レベルに対する前記治療組成物の作用をアッセイする工程と、(c)前記対象の前記GIにおける細菌群1、細菌群2又は細菌群3のいずれかの前記レベルに対する前記治療薬の前記作用を判定する工程とを含む、炎症性腸疾患を治療するための治療組成物を同定する方法。
- (a)対象のGIにおけるRORγt+Th細胞又はRORγt+Treg細胞のレベルを決定する工程と、(b)治療組成物を前記対象に投与する工程と、(c)前記対象の前記GIにおけるRORγt+Th細胞又はRORγt+Treg細胞の前記レベルに対する前記治療組成物の作用をアッセイする工程と、(c)前記対象の前記GIにおけるRORγt+Th細胞又はRORγt+Treg細胞の前記レベルに対する前記組成物の前記作用を判定する工程とを含む、炎症性腸疾患を治療するための治療組成物を同定する方法。
- 対象におけるRORγt+Treg細胞の生成を刺激する治療薬を投与することにより、対象において疾患を治療する方法であって、前記治療薬が、ワクチン、アジュバント、生物製剤、医薬組成物、プロバイオティクス、食品、飲料、糞便移植、細菌組成物、若しくは動物モデルにおいて使用される試薬、又はこのような成分の組合せを含む、方法。
