Abstract
Description
図1Aおよび1Bは、歯科インプラントのシナリオを、各ステップを達成するために使用し得るブラストプロトコルの関連する処置ステップと関連付ける表を示す。一般に、図1Aに見られるように、乱れた部位は、その部位から歯またはインプラントが意図的または偶発的に除去された直後またはまもなくしてインプラントを受け入れてよい。代替的に、図1Bに見られるように、乱れのない部位は、歯が除去され、かつ、その部位が治癒後しばらくして、インプラントを受け入れてもよい。
1.インプラント部位の軟組織を切開する。
2.抜歯部位を準備する。
3.滅菌済み超硬ドリルまたはバーを用いて骨切りを行う。
4.骨切り部位の深さを測定する。
5.準備済みインプラント部位へレーザを照射する。
6.インプラントにレーザを照射する(体外)。
7.インプラントを埋入する。
8.バイオスティミュレーションを実施する。
9.必要に応じてメンテナンス治療を行う。

2.抜歯部位を準備する。

4.骨切り部位の深さを測定する。
5.インプラント部位にレーザを照射する

7.インプラントを埋入する。

9.必要に応じてメンテナンス治療を行う。
BLAST処置は、歯科用インプラントの埋入および/またはメンテナンスに関連する複数のステップを含む。例えば、BLASTは、歯の偶発的な喪失に続く歯科インプラントの埋入、または、邪魔されない部位での歯科インプラントの埋入を取り扱ってもよい。以下のステップは、インプラントを埋入するためのBLAST処置を説明する。
インプラント周囲の感染と炎症は、歯垢や歯石(結石)に含まれるある種の細菌が原因である。これらの細菌は、歯肉を刺激する毒素を作り出し、深いポケットを生じさせ、インプラントへの骨の付着の破壊をもたらす。これらの毒素は時間とともに歯茎の組織を破壊し、感染を進行させ、骨が失われることもある。
インプラント周囲粘膜炎とインプラント周囲炎には、初期、浅い、深い骨ポケットを減らし、病的組織、インプラント周囲病原体、病理学的タンパク質、インプラント表面の歯石やその他の結石、金属インプラント劣化の腐食副産物を除去することが含まれる場合がある。これにより、インプラント固定具への新しい骨の再成長、再生、再結合を実現する。上記にかかわらず、全てのインプラントがチタン(例えば、セラミック)で作られているわけではないことに留意すべきであり、このプロセスは、そのような他の種類のインプラントに適用されてもよい。
Claims (5)
- 長期的結果の予測可能性および成功率を高めるべく、インプラントの埋入前、埋入中、埋入後に前記部位と前記インプラントの両方を消毒および処理するためのレーザベースの歯科インプラント方法であって、
患者の口腔内で、インプラント部位へアクセスする工程であって、必要に応じて滅菌メスで軟組織を切開することを含む、ところの工程と、
前記インプラント部位の歯槽骨を露出させるために軟組織フラップを反射させる工程と、
滅菌された回転具によって骨に骨切り部位を作成する工程と、
前記骨切り部位を複数箇所でプロービングし、その深さを決定する工程と、
レーザ光ファイバーの自由長を調節し、前記骨切り部位の深さと一致させる工程と、
相互接続されたレーザを作動させる前に、前記自由長を前記骨切り部位に挿入する工程と、
前記自由長が前記骨切り部位から引き抜かれる際に、レーザを作動させて前記骨切り部位を照射する工程と、
前記骨切り部位に埋入する前に、親水性インプラント表面を得るためにチタンインプラントを照射するのに適した自走パルスレーザを用いて、前記レーザからの光でインプラント固定具を照射する工程と、
前記チタンインプラントを前記骨切り部位に埋入する工程と、
前記チタンインプラントを前記骨切り部位に埋入した後、レーザ光によって前記インプラント部位を生体刺激する工程と
を備える歯科インプラント方法。 - 前記骨切り部位から前記自由長が引き抜かれる工程は、止血するのに十分な時間にわたって行われる、請求項1に記載の歯科インプラント方法。
- 前記骨切り部位から前記自由長が引き抜かれる工程は、止血するのに1回以上繰り返される、請求項1に記載の歯科インプラント方法。
- 前記骨切り部位から前記自由長が引き抜かれる工程は、活性化成長因子を活性化し、遺伝子発現をアップレギュレートし、炎症性サイトカインおよびプロスタグランジンの産生を抑制するのに十分な時間にわたって行われる、請求項1に記載の歯科インプラント方法。
- 前記骨切り部位から前記自由長が引き抜かれる工程は、活性化成長因子を活性化し、遺伝子発現をアップレギュレートし、炎症性サイトカインおよびプロスタグランジンの産生を抑制するために1回以上繰り返される、請求項1に記載の歯科インプラント方法。
