様々な実施形態において、本発明は、例えば、トシル酸ソラフェニブの形態や、他の薬学的に許容されるソラフェニブの形態、塩、及びエステルなどといったソラフェニブの使用を含む。ソラフェニブは、ソラフェニブのトシル酸塩であるNEXAVAR(R)として市販されている。トシル酸ソラフェニブのIUPAC化学名は4-(4-{3-[4-クロロ-3(トリフルオロメチル)フェニル]ウレイド}フェノキシ)N-メチルピリジン-2-カルボキサミド4メチルベンゼンスルホネートであり、構造式は次の通りである。
一態様では、本発明は、対象の体内の癌細胞または単離された癌細胞のin vitro治療を含む、肝臓癌細胞の治療のための方法を提供する。癌細胞が対象の体内にある場合、対象は、肝臓癌を有するヒトなどの霊長目であってもよい。対象は哺乳動物であってもよい。対象は、成人(すなわち、18歳以上)であってもよく、若年者(18歳未満)であってもよい。様々な実施形態において、肝臓癌は、肝細胞癌(HCC)、線維層板状HCC、胆管癌、血管肉腫、または転移性肝癌であってもよい。
併用療法または多剤療法は、単独の薬剤で行われる療法である単剤療法とは対照的に、複数の薬または他の療法を使用することである。本発明の一態様において、本明細書で提供されるのは、肝臓癌の治療のための薬物の治療的組み合わせであり、その組み合わせにはクロシン及びソラフェニブが含まれる。組み合わせの例示的な実施形態では、クロシン及びソラフェニブは、両方の化合物を含む同じ単一の医薬組成物に一緒に配合される。別の例示的な実施形態においては、クロシン及びソラフェニブは、別個の医薬組成物である。対象に治療量の組み合わせを投与することにより、対象の肝臓癌を治療、抑制、または重症度を低下させるための方法も提供される。
本発明は、肝臓癌を治療するためのキットも提供する。例えば、キットは、上記のようなクロシン及びソラフェニブの1つまたは複数の医薬組成物を含み得る。組成物は、薬学的に許容される賦形剤を含む医薬組成物であってもよい。キットを含む他の実施形態では、クロシンを含む第1の薬学的に許容される組成物、ソラフェニブを含む第2の薬学的に許容される組成物、及び必要に応じて肝臓癌の治療におけるそれらの使用方法の説明を含むキットが提供される。さらに他の実施形態では、1つ以上の医薬組成物およびそのような組成物の投与を達成するための1つ以上の装置を含むキットが提供される。例えば、対象となるキットは、医薬組成物と、組成物を癌へ直接動脈内注射するためのカテーテルとを含んでいてもよい。一実施形態では、デバイスは動脈内カテーテルである。
(in vivoモデル:動物)
オスのWistarラットは、UAE大学医学健康科学部の動物研究施設から入手した。当初、動物の体重は約110gで、24~26℃において12時間の明暗サイクルで飼育され、標準的な実験動物食餌で維持され、餌と水は自由に摂取できた。すべての動物実験は、UAE大学医学健康科学部の動物研究倫理委員会の承認を得て実施した(承認番号A8-15)。
発癌
この研究では、4週齢のオスのWistarラットを使用した。ラットはランダムに2つのグループに分けられた。腹腔内(ip)注射により1xPBSを投与されたコントロールグループ(n=8)と、50mg/kg体重のDEN(Sigma Aldrich、USA)を、PBSに溶解して週に1回、ipを介して16週間投与した実験グループである。このHCC誘導プロトコルは、(DePeralta et al.,2016; Schiffer et al.,2005)によって記述されたプロトコルを改変したものである。
16週目に、実験グループにHCCが確立したので、動物を以下の4つのグループ(n=8)に分けた。HCC単独、HCC+クロシン、HCC+ソラフェニブ、HCC+クロシン+ソラフェニブのグループである(図1参照)。17週目から19週目まで、クロシン及び/またはソラフェニブの投与を1日1回、週5日で開始した。すべての薬剤は胃内チューブを用いて経口投与された(強制経口投与)。投与量および投与経路は、これまでに文献で報告されているものに従った。
血液サンプル
血液サンプルを採取するために、ラットを安楽死させてから断頭し、血液を収集チューブ(BD Vacutainer)に収集した。全血から血清を分離するために、サンプルを1200rpmで10分間遠心分離した。血清を収集し、液体窒素で瞬間冷凍した後、さらなる調査のために-80℃で保存した。
肝臓酵素アラニントランスアミナーゼ(ALT)、及びアスパラギン酸アミノトランスフェラーゼ(AST)のレベルを、市販の比色分析を使用して評価した。キットはAbeam(Cambridge、United Kingdom)から購入し、提供されたプロトコルに従い、Promega GloMax Discoverマイクロプレートリーダーを使用してALTおよびASTの濃度を測定した(図2参照)。
PBSを使用して肝臓組織を洗浄し、肝臓全体の写真を撮影した。次に、採取した各肝臓を2つの部分に分け、1つの部分を空のエッペンドルフチューブに保存し、直ちに液体窒素で瞬間冷凍し、後でさらに調査するために-80℃で保存した。もう一方の肝臓の部分は、組織病理学的検査のために、10%の中性緩衝ホルマリンに浸して室温で保存した(図2参照)。
組織の完全性を維持するために、肝臓組織サンプルを10%の中性緩衝ホルマリン溶液で固定した。続いて、組織を3μmの厚さの切片に切断し、脱水して組織の水分を除去した。脱水のプロセスは、一連のエタノール溶液を、濃度を上げながら使用することにより達成された。その後、キシレンを使用して組織に残っているエタノールをすべて除去した。次に、パラフィンワックスに組織切片を埋め込んでパラフィンブロックを準備し、ワックスが組織に浸透できるようにした。次に、得られたブロックを厚さ3μmの切片に切断した。細胞レベルで形態学的変化を観察するために、H&E染色とレチクリン染色の2つの染色を使用した。製造元の指示書(Abeam)に沿ったプロトコルに従い、組織スライドを光学顕微鏡で観察及び検査した。発癌モデルの有効性と本研究で使用した治療法の影響を評価するために、全ての肝臓組織サンプルは、アラブ首長国連邦のタワム病院の病理学者によって盲検試験された。
肝臓組織サンプル(10mg)を冷やしたRIPA緩衝液(Sigma Aldrich、USA)に入れ、2mLのプロテアーゼ阻害剤とホスファターゼ阻害剤を加えてホモジナイズした。各肝臓サンプルのタンパク質濃度は、Promega GloMax Discoverを用いたブラッドフォード法(Bio-Rad、カリフォルニア州ハーキュリーズ)を用いて測定した。タンパク質をゲルにブロットするために、肝臓組織をホモジナイズして得られた細胞溶解物にローディング用色素である2-メルカプトエタノールを添加した。次に、細胞溶解物とローディング用色素との混合物をSDS-PAGEゲルにロードした。決定するタンパク質のサイズに応じて、ゲルのパーセンテージを変化させた。SDS-PAGEゲルで各サンプルのタンパク質含有量を分離した後、タンパク質をPVDF膜に転写し、5%BSAを加えたTBSTまたは5%(w/v)無脂肪ミルクで1時間、室温でブロックした。ブロッキング段階の後、膜を一次抗体とともに4℃で一晩インキュベートした。本研究では、ここで選択した一次抗体は、抗増殖細胞核抗原(PCNA)、抗カスパーゼ-3(Cell Signaling Technology Inc.)、抗カスパーゼ-9(Nous Biologicals)、抗ポリADPリボースポリメラーゼ(PARP)、抗Bcl-2、抗Bax(サンタクルーズ)である。一次抗体と一晩インキュベートした後、膜をTBSTで広範囲に洗浄し、抗ウサギIgG(Cell Signaling Technology、Inc、MA、USA)や抗マウスIgGなどの二次抗体で再プローブし、室温で1時間培養して西洋ワサビペルオキシダーゼを結合させた。次に、WesternSure PREMIUMとして知られる化学発光溶液を使用してタンパク質バンドを検出し、Bio-Rad ChemiDoc XRS+システムを使用してシグナルを検出し、視覚化した。バンドの強度は、ImageJソフトウェアを使用して測定した。製造元(Thermo Fisher Scientific)の指示するプロトコルに従って、SYPRO Rubyタンパク質ゲル染色を使用して総タンパク質を染色し、内部コントロールとして使用した。
ハウスキーピングタンパク質は、すべてのウエスタンブロット分析のコントロールとして使用される。ハウスキーピングタンパク質は組織内で安定して発現するため、SDS-PAGEゲルにロードされたタンパク質サンプルの完全性を検証するための優れた指標となり、サンプル中のタンパク質の量を正確に反映する。現在の研究では、これらのハウスキーピングタンパク質がさまざまな実験条件下で安定性を維持できるかどうかが調査されている。この研究では、GAPDH、b-アクチン、b-チューブリンなどのさまざまなハウスキーピングタンパク質がローディングコントロールとして使用され、一貫性がないことがわかった。2003年に発表された研究論文では、GAPDHやb-アクチンなどの一般的に使用されるローディングコントロールの遺伝子発現が増加し、7倍から23倍の間で変動していることが示されており、興味深いことに、これは癌組織で示された(Kim & Kim、2003)。そこで、この問題を解決するために別のアプローチを用いた。ローディングコントロールとして総タンパク質を使用した。このプロトコルは、1つのタンパク質に依存するのではなく、サンプル全体のタンパク質含有量を測定するもので、結腸直腸癌及びHCCにおいて信頼できるローディングコントロールであることが証明されている。
(血清中の肝酵素)
肝細胞から血流に放出される酵素のレベルを評価するために、肝機能分析を実施した。これらのレベルは、肝臓の完全性と機能性を反映している。肝臓が損傷すると、これらの酵素は肝細胞から広範囲に放出される。ALTとASTは、肝臓の効率を評価するために最も一般的にチェックされる酵素である。
ALT(P<0.01)及びASTレベルは、コントロールグループと比較してHCCグループで有意に増加しており、腫瘍形成による肝障害の重症度を示している。クロシンの単独投与及びソラフェニブとの併用投与では、HCCグループと比較してALTレベルの値が低下した(P<0.01)。クロシンの単独投与及びクロシンとソラフェニブとの併用投与では、ソラフェニブ単独投与グループ(HCC+ソラフェニブ)と比較して、ALTレベルが有意に低下した(P<0.05)。
肝臓の肉眼的外観
PBSグループの動物の肝臓は、正常な構造、サイズ、質感、および光沢のある暗褐色を呈し、肉眼で病変は観察されなかった。HCCの肝臓は、肉眼で明らかな複数の病変を伴う顕著な淡い色と外観を示した(図3参照)。治療を受けた肝臓は、HCCグループと比較してストレスが少ないように見えた。DEN誘発HCCラットへの薬物の治療的投与は、HCC単独グループと比較して肝臓の正常な形態を回復させた。クロシン単独投与(HCC+CR)およびクロシンとソラフェニブとの併用投与(HCC+CR+SB)では、HCC動物と比較して病変の数が減少した。クロシンを投与した動物は、ソラフェニブ単独投与(HCC+SB)の肝臓と比較して、有意に少ない病変数を示した(図4参照)。
DEN誘導HCCラットに対するクロシンの治療効果をさらに検討するために、臓組織の組織病理学的検査(図10参照)を実施した。肝臓の切片を光学顕微鏡で観察し、代表的な画像を得た。細胞成分と組織を顕微鏡で可視化するために、核を青色に染色するヘマトキシリン(H)と細胞質をピンク色に染色するエオシン(E)を使用して切片を染色した。H&Eは、組織学で使用される標準的な染色剤である。コントロールPBSグループは、動物の肝臓の正常な構造と組織、中心静脈を伴う正常な肝小葉を示し、肝細胞は中心静脈から放射状に正常なスパンに配置されている。また、組織学的検査により、無傷のコアと正常な血管の関係が門脈路と肝細静脈との間に存在することが明らかとなった。HCCグループの組織学的評価では、HCCの進行と一致する異常な細胞形態が認められ、肝索は正常な肝板よりも広く見え、小葉の正常な構造は失われていた。クロシンの単独投与またはソラフェニブとの併用により、HCCグループでは肝臓の正常な構造が大幅に回復した。
レチクリンは、肝臓の組織病理学を調査するために主に使用される免疫染色である。細網繊維の可視化に役立ち、十分に分化したHCCの診断に有用である。全てのグループの肝臓切片を処理してレチクリンで染色し、光学顕微鏡で観察した(図5参照)。コントロールPBSグループは正常な網状ネットワークを示した。実験HCCグループでは、網状組織の明らかな崩壊が見られた。クロシン単独投与(HCC+クロシン)及びソラフェニブとの併用投与(HCC+クロシン+ソラフェニブ)では、DEN誘発の癌による損傷を回復し、HCCグループと比較して繊維の破損はそれほど頻繁に見られなかった。
DEN誘発HCCのアポトーシス経路におけるクロシンの役割を、特にミトコンドリアを介する(内因性)アポトーシスに関与する重要なマーカーの発現を評価することによって調べた。その結果、クロシンを投与した場合に、抗アポトーシスタンパク質Bcl-2の発現がHCCグループに比べて有意に(P<0.05)減少した一方で、アポトーシス経路の活性化に重要な役割を果たす促進タンパク質Baxの発現が有意に(P<0.05)増加することがわかった。また、ソラフェニブとクロシンとを組み合わせて投与することで、クロシン単独またはソラフェニブ単独のいずれかと比較して、DEN誘発HCCに対してより強い(P<0.05)効果があることが分かった。アポトーシスに対するクロシンの役割を徹底的に調査するために、複数のウエスタンブロット実験を行った結果、プロカスパーゼ-3、プロカスパーゼ-9、及びPARPが内因性アポトーシス経路を活性化することを確認した。これらのタンパク質の発現は、クロシン投与グループ及びクロシンをソラフェニブと組み合わせて使用した場合に、HCCグループ単独と比較して、クロシン投与及びクロシンとソラフェニブとの併用投与で有意に(それぞれP<0.01、P<0.01、P<0.001)減少した(図8、図9参照)。
肝臓は、恒常性を維持するために不可欠な機能を果たす複雑な器官である。肝臓で起こる主要な生物学的プロセスには、様々な異物の取り込み、代謝、及び排泄が含まれる。肝臓は、免疫応答、食作用、微生物の除去にも関与している。また、タンパク質、炭水化物、脂肪を代謝する場所でもある。肝機能のルーチン分析では、ALTやASTなどのバイオマーカーの値をチェックする。これらの値は、肝臓の損傷や機能障害のレベルを大きく反映している。ALTは肝臓に高濃度で存在するが、腎臓や心臓などの他の部位にも存在する。肝細胞の細胞質では、ALTはアミノ基転移反応の触媒として重要な役割を果たしている。ALTはアミノ基転移反応にも関与しており、肝細胞のミトコンドリアや細胞質に存在しているが、主に心臓組織に集中している。肝臓にわずかな損傷があると、血清中のALTとASTの値は大幅に上昇する。
肝臓は小葉に分かれており、それぞれの小葉は、中心静脈と肝動脈の分岐、及び胆管を備えた六角形の構造を備えている。肝細胞は通常、中心静脈から放射状に広がる単細胞のプレート状に配置される。クロシンの有効性を評価するために、肝生検を分析した。肝生検は伝統的に、肝疾患の原因や重症度を特定するための診断ツールとして実施されている。この研究では、図3に示すように、DEN処理した動物から収集された肝臓において、肉眼で見える結節が観察された。DEN誘発動物の肝臓を組織病理学的に検査したところ、索状型を示し、透明な細胞質を特徴とするHCC病変の明確な特徴が認められた。肝細胞の正常な六角形は失われていた。プレートは、単細胞の厚さに配置されなくなった。リンパ球の浸潤が観察された。これらはすべて、HCCの発生を示している。他の研究でも同様の結果が観察された。クロシンの単独投与及びソラフェニブの補助剤としての投与は、観察される肝結節の数を減らすことに大きな影響を与えた(図4参照)。クロシンでコーティングされたD-MNPsナノ粒子を使用してマウスのDEN誘発マウス前癌性肝臓を治療した研究では、クロシンを投与された動物の肝臓が、明らかな肝臓組織学回復を示したことが明らかとなった。
結合組織は、体内の4つの主要なタイプの組織の1つである。結合組織は、免疫防御、成長、修復、及び機械的支持の提供において重要な役割を担う。結合組織は、上皮組織のバックボーンとなる。結合組織は、主に細胞外マトリックスと様々な種類の細胞とで構成されている。細胞外繊維には、コラーゲン、エラスチン、細網線維を含む3種類がある。細網線維は主にIII型コラーゲンで構成されており、主に肝臓と筋線維とに見られる細網ネットワークを形成している。レチクリン染色は、高分化型肝細胞癌と良性肝病変を区別するための鑑別診断に有用であることが示されている。HCCの場合、細網ネットワークは完全に失われるか、異常なパターンを示す。そのパターンには、細胞層の厚さが増すことに伴う索状の拡大が含まれる。
増殖に対するクロシンの影響
PCNAは、DNA複製、DNA修復、細胞周期の進行など、多くの細胞プロセスで重要な役割を果たすことが分かっている。DNA複製中の変異は、PCNAの翻訳後修飾に反映され、それによってPCNAの機能が変化する。PCNAは、複製中にその役割を果たすことから、増殖のマーカーとされている。複製の間、PCNA分子はDNA螺旋の周りにスライディングクランプを形成し、複製のプロセスを制御するためのプラットフォームを作成する。PCNAの発現は、乳癌および肝臓癌の転移で増加する。PCNAはS期にピークに達するため、細胞増殖の指標となる。PCNAの発現レベルを評価することは、癌を評価するための診断および予後ツールとして使用される。乳癌患者を対象に実施された研究では、隣接する正常組織と比較して、患者の癌組織でPCNAが高発現していることが明らかとなった。また、ミツバチの抗腫瘍効果を評価するためにSprague Dawleyラットを用いた他の研究では、コントロールグループと比較してDEN誘発動物においてPCNAが高レベルで発現していた。本研究では、ウエスタンブロット分析により、DEN誘発HCC動物におけるPCNAのレベルがコントロールグループと比較して有意に高いことが示された。クロシン及びクロシンとソラフェニブと両方を投与すると、PCNAの発現が減少した(図6参照)。クロシンは、A549及びSPC-A1肺癌細胞において細胞増殖を抑制し、アポトーシスを誘導することがわかっている。乳癌細胞を対象とした別の研究では、クロシンは微小管ネットワークを破壊することによって増殖を抑制した。B16G10癌細胞を移植したC57BL/6マウスのメラノーマ治療に対するクロシンの抗腫瘍効果を調査した研究では、クロシンを21日間投与したところ、腫瘍が縮小し、生存期間が有意に延長したことが示された。
アポトーシスの誘導におけるクロシンの役割
プログラムされた細胞死であるアポトーシスは、細胞生物学で最も研究されているトピックの1つである。アポトーシスは、生理学的状態と病理学的状態との両方で発生する複雑なプロセスである。アポトーシスは、恒常性を維持し、細胞の発達を調節するために不可欠である。アポトーシスの不均衡は、自己免疫疾患や癌を含む多くの疾患の発症につながる。癌の場合、細胞死と細胞分裂の間の平衡が失われる。この問題は、癌細胞が非常に賢く適応性があり、死のシグナルをスキップして無限に増殖し続ける能力を持っているために発生する。これにより、変異した細胞の塊である腫瘍が形成される。
本明細書及び付随する特許請求の範囲で使用されるように、以下の用語は、文脈上別段の必要がない限り、次の意味を有するものとする。
Claims (20)
- 対象の肝臓癌を治療、抑制、または重症度を軽減する方法であって、
前記対象に第1の量のソラフェニブを投与することと、
前記対象に第2の量のクロシンまたはその薬学的に許容されるプロドラッグを投与することを含む方法。 - 前記クロシンがα-クロシンである、請求項1に記載の方法。
- 前記クロシンまたはそのプロドラッグ及び前記ソラフェニブが、両方の化合物を含む1つの組成物に一緒に配合されている、請求項1に記載の方法。
- 前記クロシンまたはそのプロドラッグ及び前記ソラフェニブが、別々の医薬組成物で別々に投与される、請求項1に記載の方法。
- 前記クロシンまたはそのプロドラッグ及び前記ソラフェニブが順次投与される、請求項1に記載の方法。
- 前記ソラフェニブに曝露する前に、前記クロシンまたはそのプロドラッグを最初に投与して癌細胞を感作し、次に前記ソラフェニブを投与する、請求項1に記載の方法。
