疾患修飾抗リウマチ薬(DMARD)は、疾患進行を減速させるための関節リウマチにおけるそれらの使用により定義される薬物である。
本明細書に記載される方法は、対象に治療有効量の抗hIL-6R抗体を投与することを含む。本明細書で使用されるように、句「有効量」又は「治療有効量」は、関節リウマチ(RA)の処置を生じる治療剤の用量である。本明細書で使用されるように、「処置すること」は、RAに関連する1つもしくはそれ以上の症状における検出可能な改善を引き起こすこと、又は状態もしくは症状を生じる根底にある病態機序と相関する生物学的効果(例えば、特定のバイオマーカーのレベルの減少)を引き起こすことを指す。例えば、RAに関連する以下の症状又は状態のいずれかにおける改善を引き起こす抗IL-6R抗体の用量は、「治療有効量」とみなされる:圧痛関節、関節腫脹、関節のこわばり、疲労、発熱又は食欲不振。
試験サンプルは、Truven MarketScanデータベースにおける1266人の患者及びOptum Clinformaticsデータベースにおける512人の患者からの2012年10月1日と2017年6月30日の間(試験期間)のデータを含んでいた(図1)。2013年10月1日と2016年6月30日との間(患者識別期間)に組入基準を満たす成人を、試験サンプルに含めた。患者識別期間の間の皮下(SC)トシリズマブ(TCZ)の最初の調合(fill)日は指標日(index date)であった。試験において使用された主要分類変数は、メディケア(Medicare)及び民間(Commercial)であり、全試験期間の間メディケア補足(Medicare Supplemental)の適用範囲である患者は「メディケア」群に含まれ、一方で残りの患者は「民間」群に含まれた。
患者は、以下の場合に組み入れられた:患者識別期間の間に≧1のSC TCZの薬局への請求があった;指標日前にRA診断コード(国際疾病分類[ICD]-9:714.XX;ICD 10:M05.XX又はM06.XX)での≧1の入院又は≧2の外来医療請求があった;指標日に≧18歳であった;かつ、指標日の前及び後(それぞれベースライン及び経過観察期間)に民間健康プランへの≧12か月連続的に加入していた場合。
SC TCZの平均月用量(AMD)を、調剤量x強度/供給日数x28として計算した。
記述的分析を全ての試験アウトカムについて行い、そして全ての試験アウトカムについての記述的統計を、試験サンプル全体について、さらには主要分類変数(メディケア及び民間)により報告した。平均、標準偏差(SD)、及び中央値を連続変数について報告し、そして頻度(N及びパーセンテージ)をカテゴリー変数について報告した。
ベースライン特徴
平均(SD)年齢はTruven患者について52.3(±10.7)歳、そしてOptum患者について54.9(±13.3)歳であり;女性の比率はTruvenにおいて82%そしてOptumにおいて83%であり;平均(SD)経過観察はTruven患者について25.8(±9.2)か月及びOptumについて27.9(±9.1)か月であり;そして平均(SD)ECIスコアはTruven患者について1.8(±1.9)、そしてOptum患者について2.3(±2.4)であった(表1)。それぞれ民間及びメディケア適用を有するTruven及びOptumコホートにおける患者は、平均(SD)年齢50.3(9.2)歳及び69.1(6.6)歳(Truven)並びに50.3(11.9)歳及び64.7(10.3)歳(Optum)を有していた;女性の比率は83%及び70%(Truven)並びに80%及び89%(Optum)であり;そして平均(SD)ECIスコアは、1.7(1.8)及び2.8(2.3)(Truven)並びに1.7(7.7)及び3.9(2.9)であった。
試験サンプルにおいて、指標日の90日前に、Truven患者の22及びOptum患者の25%は治療を受けておらず;Truven及びOptum患者の47%ずつが単剤療法を受けており;Truven患者の31%及びOptum患者の29%がcsDMARDと生物製剤との組み合わせ処置を受けており;そしてTruven患者の51%及びOptum患者の57%が副腎皮質ステロイドを使用したことがあった。それぞれ民間及びメディケアの適用を受けるTruven患者及びOptum患者の中で、47%及び44%(Truven)並びに46%及び48%(Optum)が単剤療法を受けており、そして30%及び35%(Truven)及び29%及び27%(Optum)がcsDMARDS及び生物製剤の組み合わせ治療を受けていた(表2)。
SC TCZの162mg Q2W用量で開始した患者の1年間の経過観察期間の間、Truvenからの37%及びOptumからの40%が162mg QWまで増加した。SC TCZの162mg QW用量で開始した患者の中で、3%(Truven)及び4%(Optum)のみが、162mg Q2Wまで用量減少した(表2)。Truven及びOptumにおける患者の全体で60%及び68%は、より高い週用量で開始したか又はより高い週用量に増加した。それぞれ民間及びメディケア適用を有するTruven及びOptum患者は、60%及び53%(Truven)並びに70%及び62%(Optum)がより高い週用量で開始したか又はより高い用量に増加したが、0%及び6%(Truven)並びに3%及び5%(Optum)は、162mg QWから162mg Q2Wに用量減少した。経過観察期間の間の28日あたりのSC TCZ調合の平均(SD)数は、Truven患者について6.7(±4.3)及びOptum患者について7.1(±4.5)であった(表3)。試験サンプルにおける適用された日の平均(SD)比率は、約50%であった(Truven、0.5[±0.3];及びOptum、0.4[±0.3];表3)。
用量漸増を行った患者の中で、用量漸増までの平均(SD)時間は、Truven患者について126(±6.1)日そしてOptum患者について112(±7.7)日であった(図2A及び2B)。
Truven患者の中で、副腎皮質ステロイド使用、年齢、及び貧血(ICD-9/ICD-10診断コードを使用して定義される)は、用量漸増の3つの主要な予測因子であった。指標日から90日以内の副腎皮質ステロイド使用(オッズ比[OR]:0.70;P= .02)、35~44歳の患者対18~34歳の患者(OR:0.54;P= .05)、又は貧血を有する患者対貧血のない患者(OR:0.50;P= .04)は、用量漸増の減少したオッズを有していた(表4)。
TCZは、≧1のDMARDに対して不適切な応答を有したことがある(他の適応症の中で特に)中程度から重度のRAを有する成人に関して多数の国で承認されている。過去10年にわたって、多数の研究が、RAを有する患者におけるTCZの安全性及び有効性を実証した。IV TCZを投与されているRA患者の中で7~12の用量変更パターンが試験された;しかし、SC TCZを投与されているRA患者間で類似したデータは限られている。現在の研究は、実臨床設定においてSC TCZ用量変更を調べる最初のものであり、そして多くの患者が開始時または漸増の際のいずれかにSC TCZのQW用量を使用したが、SC TCZのQW用量で開始した少数の患者は用量減少を行った。
実臨床データを使用して、この研究は、全体的に、開始時または漸増の際のいずれかでSC TCZのQW用量の利用が、それぞれTruven及びOptum患者において60%及び68%であったことを示した。SC TCZの用量漸増は、SC TCZのQ2W用量を開始した患者の3分の1より多くで起こり、そして用量漸増までの時間は約4か月であった。一方で、QW用量で開始した患者の<5%はQ2WへのSC TCZ用量減少を行った。これらの結果は、医師は臨床応答に基づいてSC TCZ用量を増加する選択肢を利用するようであるが、SC TCZの用量を減少する選択することは少なく、結果としてSC TCZを投与されている多くの患者が最終的により高い用量を投与される。

Claims (71)
- 関節リウマチの処置を必要とする対象における関節リウマチの処置における使用のための抗体であって、ここで
(i) 対象は、以前にトシリズマブを投与されたことがないか又はトシリズマブを3か月未満投与されたことがあり、かつ貧血を有しておらず;かつ
(ii) 抗体は、
a. 週に1回トシリズマブ162mgで対象に皮下投与されるか;又は
b. 4週間ごとに1回トシリズマブ8mg/kgで対象に静脈内投与される、
上記抗体。 - 関節リウマチの処置を必要とする対象における関節リウマチの処置における使用のための抗体であって、ここで
(i) 対象は、以前にトシリズマブを投与されたことがないか又はトシリズマブを3か月未満投与されたことがあり、かつ18歳から34歳であり;かつ
(ii) 抗体は、
a. 週に1回トシリズマブ162mgで対象に皮下投与されるか;又は
b. 4週間ごとに1回トシリズマブ8mg/kgで対象に静脈内投与される、

- 対象は、中程度から重度に活動性の関節リウマチを有する、請求項1~6のいずれか1項に記載の使用のための抗体。
- 対象はサリルマブを投与されたことがない、請求項1~7のいずれか1項に記載の使用のための抗体。
- 対象は体重が100kg未満である、請求項1~8のいずれか1項に記載の使用のための抗体。
- 対象は、強直性脊椎炎、クローン病、若年性特発性関節炎、乾癬、乾癬性関節炎、潰瘍性大腸炎、慢性リンパ性白血病、非ホジキンリンパ腫、又は巨細胞動脈炎を有していない、請求項1~9のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、かつ18歳から34歳である、請求項1、2、及び5~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、かつ90日以内に副腎皮質ステロイドを投与されていない、請求項1、3、及び5~10のいずれか1項に記載の使用のための抗体。
- 対象は18歳から34歳であり、かつ90日以内に副腎皮質ステロイドを投与されていない、請求項1、2、及び5~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、90日以内に副腎皮質ステロイドを投与されておらず、かつ18歳から34歳である、請求項1~3及び5~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、かつ抑うつを有する、請求項1及び4~10のいずれか1項に記載の使用のための抗体。
- 対象は抑うつを有し、かつ90日以内に副腎皮質ステロイドを投与されていない、請求項3~10のいずれか1項に記載の使用のための抗体。
- 対象は18歳から34歳であり、かつ抑うつを有する、請求項2及び4~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、抑うつを有し、かつ18歳から34歳である、請求項1、2、及び4~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、抑うつを有し、かつ90日以内に副腎皮質ステロイドを投与されていない、請求項1及び4~10のいずれか1項に記載の使用のための抗体。
- 対象は18歳から34歳であり、抑うつを有し、かつ90日以内に副腎皮質ステロイドを投与されていない、請求項1、2、及び4~10のいずれか1項に記載の使用のための抗体。
- 対象は貧血を有しておらず、90日以内に副腎皮質ステロイドを投与されておらず、18歳から34歳であり、かつ抑うつを有する、請求項1~20のいずれか1項に記載の使用のための抗体。
- 90日以内は、対象のトシリズマブの最初の投与の90日以内である、請求項3、5~10、12、13、14、16、及び19~20のいずれか1項に記載の使用のための抗体。
- 副腎皮質ステロイドはプレドニゾンである、請求項3、5~10、12、13、14、16、及び19~22のいずれか1項に記載の使用のための抗体。
- 対象は、トシリズマブの投与の過程でいずれの他のDMARDも投与されない、請求項1~23のいずれか1項に記載の使用のための抗体。
- 対象は、トシリズマブと共に1つ又はそれ以上のさらなるDMARDも投与される、請求項1~23のいずれか1項に記載の使用のための抗体。
- 1つ又はそれ以上のさらなるDMARDはメトトレキサートを含む、請求項25に記載の使用のための抗体。
- 対象は、従来の合成DMARD又は生物学的DMARDに対して不適切な応答を以前に有していた、請求項1~26のいずれか1項に記載の使用のための抗体。
- 従来の合成DMARDはメトトレキサートである、請求項27に記載の使用のための抗体。
- 生物学的DMARDはTNFα阻害剤である、請求項27に記載の使用のための抗体。
- TNFα阻害剤はアダリムマブである、請求項29に記載の使用のための抗体。
- 対象は以前にトシリズマブを投与されたことがない、請求項1~30のいずれか1項に記載の使用のための抗体。
- 対象は3か月未満トシリズマブを投与されたことがある、請求項1~30のいずれか1項に記載の使用のための抗体。
- 対象は2か月未満トシリズマブを投与されたことがある、請求項32に記載の使用のための抗体。
- 対象は1か月未満トシリズマブを投与されたことがある、請求項32に記載の使用のための抗体。
- 対象は女性である、請求項1~35のいずれか1項に記載の使用のための抗体。
- (iii) 以前にトシリズマブを投与されたことがないか又はトシリズマブを3か月未満投与されたことがあり、かつ貧血を有しない対象を選択すること;及び
(iv)
c. 週に1回トシリズマブ162mgを対象に投与すること、[ここでトシリズマブは皮下投与される];又は
d. 4週間ごとに1回トシリズマブ8mg/kgを対象に投与すること[ここでトシリズマブは静脈内投与される]
を含む、関節リウマチを処置する方法。 - (iii) 以前にトシリズマブを投与されたことがないか又はトシリズマブを3か月未満投与されたことがあり、かつ18歳から34歳である対象を選択すること;及び
(iv)
c. 週に1回トシリズマブ162mgを対象に投与すること[ここでトシリズマブは皮下投与される];又は
d. 4週間ごとに1回トシリズマブ8mg/kgを対象に投与すること[ここでトシリズマブは静脈内投与される]
を含む、関節リウマチを処置する方法。 - (iii) 以前にトシリズマブを投与されたことがないか又はトシリズマブを3か月未満投与されたことがあり、かつ90日以内に副腎皮質ステロイドを投与されていない対象を選択すること;及び

- 工程(ii)は、4週間ごとに1回トシリズマブ8mg/kgを対象に投与することを含み、ここでトシリズマブは静脈内投与される、請求項36~40のいずれか1項に記載の方法。
- 対象は中程度から重度に活動性の関節リウマチを有する、請求項36~41のいずれか1項に記載の方法。
- 対象はサリルマブを投与されたことがない、請求項36~42のいずれか1項に記載の方法。
- 対象は体重が100kg未満である、請求項36~43のいずれか1項に記載の方法。
