Abstract

Description
本発明は、一般に、脊髄損傷、脳損傷、脳麻痺、または痙縮などの後天性または発達性の神経疾患を有する人のための神経リハビリテーションシステムに関する。脊髄損傷(SCI)への適用のための本発明の好ましい実施形態が、提示される。

何千年もの間、SCIは不可逆的であると考えられていた。この考えは、現在変わりつつある。最近の科学的証拠は、思考制御のリハビリテーションロボットが、SCIにおけるいくつかの随意運動を回復させることができることを示している。さらなる証拠は、運動感覚フィードバックを提供することで、損傷した脊髄経路を回復できることを示唆している。
他の研究でもまた、脊髄や筋肉を電気的に刺激することが有効であることが示されている。しかし、このような刺激は、筋肉を活性化して、使用されないことによって時間の経過とともに弱くなった骨を骨折させることが知られている。

各参加者102の個人用のデジタルツインは、磁気共鳴画像法(MRI)[2]と人工知能法とを組み合わせて、開発することができる。脳波(EEG)は、携帯無線ヘッドセット(例えば、スイスのウェアラブルセンシングDSI7又はDSI-VR300)122を介して取得され、この例ではサイクリング運動を、患者が実行したいかどうか、及び患者がどの程度強く実行したいかを識別するために、AI方法を使用してBCI124で処理されうる。
サイクリングの例では、自転車データに対する患者の運動意図124がデジタルツインを制御し、デジタルツインは、次いで電気刺激112を介して筋肉を最適に刺激し、サイクリングを達成するために適切な電動支援112を提供する。重要なことに、デジタルツインは、刺激された筋肉活性化が、運動を実行するための電動作動に対抗するのではなく、補助することを保証するように、電気刺激112および電動支援112を調整する。複数のウェアラブルバイオセンサ106からの生体力学的および生理学的情報108(例えば、筋電図、慣性測定ユニット、心拍数、心電図、および呼吸)は、患者のデジタルツインによって解釈され、筋骨格組織の負荷及び心血管需要を安全なレベに維持しながら、患者102を最大限に運動に従事させるために、エルゴメータペダル支援の量を、段階的に適応させることができる[3、4]。
最後に、患者のデジタルツインは、体性感覚情報116を合成することができ、これはエクステンデッドリアリティ118、視覚123、聴覚120、及び/又は触覚121フィードバックを介して、より高い体性感覚野にリダイレクトされる[4、5]。
既存の研究[6~10]で使用され、確立された安全性プロファイルを有する既製で既知の薬理学的補助剤(例えば、ブスピロン)を加えて、神経可塑性に対するそれらの相加効果を測定することができる。システム100は、既存の市販の機器104を改造し更新するように設計されている。

システム100は、運動器具104を作動させるために、駆動クランクに連結されたロボットモータなどのアクチュエータを更に含むことができる。使用時には、モデル110は、アクチュエータをある程度まで作動させて、人102を支援する。
