Abstract
Description
本発明は、エリスロフェロン(ERFE)の転写物の変異体、並びに、SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を有する患者における、貧血及び鉄の過剰負荷の診断及びモニタリングにおけるその使用に関する。
骨髄異形成症候群は、高齢患者の罹るクローン性造血幹細胞障害であり、症例の40%は急性骨髄性白血病(20%以上の骨髄芽球によって定義される)に進行し、残りの症例は、骨髄不全へと進行する傾向がある。患者は、末梢血の血球減少症、主に貧血(症例の80%)、そして、それほど頻繁ではないが、好中球減少症及び/又は血小板減少症に罹り、これは加齢及びおそらくクローン性造血の発症に関連した合併症を大抵は伴う。骨髄異形成症候群は不均質であり、世界保健機構は、その2016年の分類において7つのサブタイプを認識している(1)。骨髄造血細胞内における反復突然変異の明確に異なる組合せの発見以来、個体間の不均質は増している。これらの突然変異は、後成的な転写調節に関与する遺伝子(TET2、IDH1/2、DNMT3A、ASXL1、EZH2)、スプライシング因子(SF3B1、SRSF2、U2AF1、ZRSR2)、コヒーシン(STAG2)、転写因子(RUNX1、TP53)、及びシグナル伝達分子(KIT、CBL、NRAS、KRAS)に影響を及ぼす。これらの各突然変異は、骨髄のより未熟な幹細胞及び前駆細胞において検出可能な事象を惹起する可能性があり、それらの赤血球系及び顆粒球-単球系の前駆細胞、時にはBリンパ球へと伝達される(2)。リスク因子は、血球減少症の数及び強度、骨髄芽球の比率、並びに細胞遺伝学的異常の種類及び数であり、これらは全て、改訂された国際予後判定システム(R-IPSS;参考文献3)に言及されている。患者は、非常に低い、低い、中間、高い、及び非常に高いリスクと認識される。突然変異の状態を含む、新規な予後判定システムは、研究中である。より低いリスクの患者における処置の目的は、血球減少症を治癒することであるが、より高いリスクの患者では白血病の進行を止めることである。第一選択の処置は、赤血球造血刺激因子製剤(ESA)、及び失敗した場合には輸血である。赤血球造血刺激因子製剤は、組換えエリスロポエチン(Epo)(エポエチンアルファ、エポエチンベータ、及びダルベポエチンを含む)、及びレタクリット(エポエチンゼータ)のようなバイオ後続品を含む(概説については参考文献4)。
本研究では、本発明者らは、患者におけるヘプシジンの抑制及び鉄の蓄積をもたらす、ERFEタンパク質の濃度上昇の原因となる、SF3B1MUT骨髄異形成症候群に特異的なERFEの変異転写物を同定する。この転写物は、追加の4アミノ酸を有する変異タンパク質へと翻訳され得る、終止コドンを誘導しない、フレーム内に付加された12ヌクレオチドのイントロン配列を含有している。精密質量分析を使用することによって、本発明者らは、付加されたポリペプチドVPQF(配列番号5)に対応するペプチドを同定し、SFB1の突然変異したMDS患者の骨髄赤芽球による、変異タンパク質の活発な産生を実証する。組換え変異タンパク質ERFEVPFQは、野生型組換えERFEと同程度にヘプシジンmRNAの発現を抑制する。この変異体は、SF3B1MUT患者における貧血の処置をモニタリングするための、クローン性造血の妥当なバイオマーカーである。
本発明の変異体
本発明の第一の態様は、配列番号2の核酸配列に対して少なくとも70%の相同率を有するERFEの転写物の変異体、及び、配列番号4のアミノ酸配列に対して少なくとも70%の相同率を有するERFEタンパク質の変異体に関する。
本発明の第二の態様は、患者から得られた試料中の、ERFE転写物の変異体又はERFEタンパク質の変異体の発現を決定する工程を含む、SF3B1遺伝子に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における、貧血を診断するための方法に関し、ここで、このような変異体の検出は、患者が、SF3B1遺伝子に少なくとも1つの突然変異を伴う貧血を患っていることを示す。

a)SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍、又はSF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を伴う貧血を患っている患者の試料の収集物を(例えば、これらの疾患の診断後に)準備する工程;

d)該変異体についてのcDNA又はタンパク質の既知の投入量を使用して検量曲線を作成することによってなされるものである、閾値/カットオフ値/基準値に相当する該遺伝子のコピー数を決定することによって、閾値/カットオフ値/基準値を定量的に定める工程;
e)該試料を、それらの発現レベルに従って順位付けされたそれぞれ数の増加しているメンバー、数の減少しているメンバーの部分集合対に分類する工程;
f)工程a)で準備された各試料について、対応するがん患者についての実際の臨床転帰に関連した情報(すなわち、診断、処置に対する応答)を提供する工程;
g)試料の各部分集合対について、診断/反応曲線のカプランマイヤー率を得る工程;
h)試料の各部分集合対について、両方の部分集合間の統計学的有意性(p値)を計算する工程;
i)発現レベルについての基準値として、p値が最小である発現レベルの数値を選択する工程
を含む方法を実施することによって予め決定され得る。
上昇したレベルのERFEは、SF3B1の突然変異したMDSにおけるあらゆる輸血に先行する高フェリチン血症を説明することを考えると、ERFEレベルを減少させることは、高フェリチン血症の程度を低減させ、鉄の過剰負荷及び毒性を予防するための治療選択肢であり得る。野生型及び変異体の両方のERFEタンパク質が産生されるので、異常な変異体アイソフォームの標的化は、正常なアイソフォームによるヘプシジン及び日常的な鉄の代謝の調節を止めることなく、ERFEの量を低減させる目的に適う。
本発明の別の目的は、骨髄悪性腫瘍を患っている患者における貧血及び/又は鉄の過剰負荷の処置に使用するための、本発明によるERFEタンパク質の変異体に特異的に結合する抗体又はアンチセンスオリゴヌクレオチドを含む治療用組成物に関する。
材料及び方法
患者
学習コホートについて、2008年から2017年まで低リスクのMDS患者(n=156人)を記録した。各患者が施設内治験審査委員会及び倫理審査委員会の推奨に従って生物学的調査についてのインフォームドメインコンセントを提出した後に(施設内治験審査委員会(IRB)番号:IdF X GFM-LenEpo-08 EudraCT 2008-008262-12; IdFII 2010-A00033-36; IdFV 212-A01395-38 EudraCT 2012-002990-7338; OncoCCH 2015-08-11-DC)、骨髄(BM)穿刺液及び末梢血(PB)血漿を収集した。検証コホートについては、患者が2016年から2018年までフランス(5つのセンター;IRB Onco-CCH 2015-08-11DC)及びドイツ(1つのセンター;IRB Ethikkommission an der TU Dresden; EK 115032015)における血漿の収集について同意した後に、低リスクのMDS患者(n=55)を前向きに記録した。10人の同年齢の対照に由来する骨髄試料、及び20人の健康な対照に由来する骨髄試料も収集した。骨髄単核細胞を、フィコール勾配で精製した。細胞ペレットを、DNA抽出のために処理し、さらにトリゾール(サーモフィッシャーサイエンティフィック、ウォルサム、MA州)に入れ、その後、RNAを抽出した。少ない輸血負荷は、8週間あたり4単位未満の赤血球単位として定義された。
骨髄単核細胞を、フィコール勾配で精製し、DNA抽出のためにDNA/RNAキット(キアゲン社、ヒルデン、ドイツ)を使用して処理した。選択された26個の遺伝子のパネル(ASXL1、CBL、DNMT3A、ETV6、EZH2、FLT3、IDH1、IDH2、JAK2、KIT、KRAS、NRAS、MPL、NPM1、PHF6、PTPN11、RIT1、RUNX1、SETBP1、SF3B1、SRSF2、TET2、TP53、U2AF1、WT1及びZRSR2)における突然変異を、次世代シークエンス(NGS)アッセイによって、イオンAmpliSeq(商標)ライブイラリーキット2 384(ライフテクノロジーズ社、シカゴ、IL州)を使用してスクリーニングした。多重PCR増幅(233プライマー対)を、2×10ngのゲノムDNAから行なった。増幅後、バーコード及びアダプターをアンプリコンにライゲーションによって付加した。生成物を、AMPureビーズ(ライフテクノロジーズ社)上での選択的精製にかけた。エマルジョンPCR(emPCR)を、OneTouchV2(ライフテクノロジーズ社)機器を使用して行なった。シークエンスは、イオンPGM(商標)(ライフテクノロジーズ社)で318V2チップ(1つのチップあたり15個の試料)上で行なわれた。全ての試料はまた、ASXL1(例えばc.1934dupG;p.G646WfsX12)及びSRSF2の突然変異についてもサンガーシークエンスによってスクリーニングした。JAK2、NPM1及びFLT3-ITDの突然変異も、定量PCR及び蛍光PCRによって調べた。分析のための、ベースコールは、トレントブラウザーソフトウェアによって、追加のプラグインと共に含まれているバリアントコーラー(ライフテクノロジーズ社)を使用して生成された。.bam及び.vcfファイルをさらなる分析のために使用した。.vcfファイルに、Ion reporterソフトウェア(ライフテクノロジーズ社)を用いて注釈を付け、NextGENeソフトウェア(Softgenetics社、ステートカレッジ、PA州)を使用してインデックスファイルの二回目の分析のために処理した。結果を比較して、さらに検討される異常を選択した。
cDNA合成を、MuLV逆転写酵素(インビトロジェン社、カールスバッド、CA州)を用いて実施し、品質制御は、Agilent 2100バイオアナライザ(アジレント社、レ・ジュリス、フランス)で実施された。ライブラリーを、TruSeq Stranded mRNAサンプル調製キット(イルミナ社、サンディエゴ、CA州)を使用して構築し、100bpのペアエンドシークエンス戦略を使用して、イルミナ社のHiSeq2500プラットフォームでシークエンスした。平均深度が1億1400万という網羅的シークエンスカバレッジ及び中央値が1億1200万というカバレッジに到達した。分析のために、TopHat(バージョン2.0.6)を使用して、UCSCゲノムブラウザー(http://genome.ucsc.edu)からダウンロードされたヒトリファレンスゲノムHg19RefSeq(RNA配列、GRCh37)に対して、リードをアラインした。
FASTQファイルを、TopHat(バージョン2.0.6)28を使用してマッピングし、UCSCゲノムブラウザー(http://genome.ucsc.edu)からダウンロードされたヒトリファレンスゲノムHg19RefSeq(RNA配列、GRCh37)に対して、リードをアラインした。カウント数を、関連する精度の重みを用いて、100万個あたりのlogカウント数へと変換するDESeq2を使用して、リードカウント数の正規化及びグループの比較を実施し、フラグを、R内のカスタムアルゴリズムを使用してコンピューター計算した(統計的計算についてのR計画[http://www.r-project.org/])。遺伝子の最大80%が発現されると仮定して、本発明者らは、バックグラウンドとして各試料について20%の最低のカウント数を選択した。閾値は、バックグラウンドの平均値に対する2つの標準偏差で固定される。正規化されたカウント数が、コンピューター計算された閾値よりも低かった全ての転写物を、各アレイについてのバックグラウンドとして指定した。2つのグループ間の正規化されたカウント数を比較する場合、転写物のカウント数が、少なくとも1つのグループに由来する試料の少なくとも80%においてバックグランドを上回る場合に、その転写物を分析に含めた。発現変動する転写物を同定するために、本発明者らは、経験的ベイズの推定量を使用した。発現変動の研究のために、Deseq2比較後に得られた結果を、さらなる分析のために選択し、0.05以下のP値、及び1.2、1.5又は2の変化倍率でフィルターにかけた。RefSeq遺伝子記号による、平均化されたRefSeqのエキソンのロバストマルチアレイ平均(RMA)値から計算された主成分分析は、R及びGGplot2パッケージを使用して行なわれた。二変量正規密度は、第一主成分及び第二主成分であるX変数及びY変数の平均値及び標準偏差の関数である。楕円は、各グループへの二変量正規分布の当てはめからコンピューター計算された。junctions.bed TopHatアウトプットからのスプライスジャンクションについてのリードカウント数が考えられた。差異分析が、DESeq2を使用して、ジャンクションリードカウント数に対して実施された(29)。選択的受容スプライス部位(2つ以上の30ssがジャンクションを用いて同50ssへ)及び選択的供与スプライス部位(2つ以上の50ssがジャンクションを用いて同30ssへ)のみが、この分析のために考えられた。Alsafadi et alによると、分岐点配列分析の予測及び分析は、オンラインツールであるSVM-BPfinder(http://regulatorygenomics.upf.edu/ Software/SVM_BP/)及びHuman Splicing Finder(http://www.umd.be/HSF/)を用いて行なわれた(13、57、58)。SVM_BPコードを変更して、svm_getfeat.pyにminidist3ss=6を設定することによって、3’スプライス部位からの分岐点の6塩基対が可能となった。
RNAを、RNeasy Miniキット(キアゲン社)を使用して抽出し、Maxima First Strand cDNA合成キット(サーモフィッシャーサイエンティフィック社)を用いたcDNA合成のための鋳型として使用した。その後、cDNAを、特異的な5’FAM正方向プライマー又は5’FAM逆方向プライマーを使用して、PCR(Phire Hot Start II DNAポリメラーゼ、サーモフィッシャーサイエンティフィック社)によって増幅した。最後に、DNA断片分析を、キャピラリー電気泳動によって実施した。1μlの希釈されたPCR産物を、0.2μlのGeneScan 500 ROX色素サイズスタンダード(サーモフィッシャーサイエンティフィック社)及び18μlのリボヌクレアーゼ非含有水に加えた。DNAを95℃で5分間かけて変性させ、4℃で置いた。断片を、3730xlDNAアナライザを使用して分離し、データをGeneMapperソフトウェア5(サーモフィッシャーサイエンティフィック社)を使用して分析した。逆転写定量PCRについての、各PCRの平均増幅効率は95%であった。MIQE(定量的リアルタイムPCR実験の公表に必要な最低限の情報)指針に従い、関心対象の各遺伝子の発現は、2つのハウスキーピング遺伝子であるB2M及びACTBの発現に対して正規化された。
UT-7/EPO細胞株に、S. Buonamici博士(H3 Biomedicine社、ケンブリッジ、MA州)によって提供された、合成品で完全長のSF3B1WT又は突然変異体SF3B1K700EcDNAを、R溶液中のAmaxa program T-024(ロンザ社、バーゼル、スイス)を使用してトランスフェクトした。
マウス赤血球細胞株G1E-ER4を使用して、CRISPR/Cas9により刺激された相同組換えにより媒介される修復を使用して、同系SF3B1K700E細胞株及びSF3B1WT細胞株を作製した(59)。細胞に、Amaxa Nucleofector(商標)2b装置プログラムA-024及びNucleofectorキットL(ロンザ社)を使用して、1:1:1の比のCas9(参照番号41815、アドジーン社、ケンブリッジ、MA州)、mSf3b1特異的gRNA(gRNAクローニングベクターから構築、参照番号41824、アドジーン社)、及びハイグロマイシン選択カセットをコードしているドナー鋳型をトランスフェクトした。2日後、ハイグロマイシンを800μg/mLで培養液に72時間かけて加え、400μg/mLでさらに72時間維持した。その後、ハイグロマイシン耐性細胞を、FACS Aria III(ベクトンディッキンソンバイオサイエンシーズ社、フランクリン・レイクス、NJ州)を使用して単一細胞選別を行ない、クローンを3週間後に、DNAシークエンスによって分析して、SF3B1K700E突然変異の存在を評価した。選択カセットは、フリッパーゼにより媒介される切断によって除去された。SF3B1K700E及びSF3B1WTハイグロマイシン耐性クローンに、フリッパーゼをコードするベクターをトランスフェクトし、単一細胞選別した。カセットの切断されたクローンをPCRによって評価し、SF3B1K700E及びSF3B1WTの発現を逆転写PCR及びcDNAサンガーシークエンスによって確認した。
ERFEミニ遺伝子は、本発明者らがイントロン2~3の100bp上流及びエキソン3の100bp下流にフランキングする選択的3’スプライス部位(AG')の中央にERFE配列を挿入した、鎖状化されたpET01エキソントラップベクター(モビテック社、ゲッティンゲン、ドイツ)から合成された。ERFE挿入断片は、2U/μlのPhusion高忠実度DNAポリメラーゼ(サーモフィッシャーサイエンティフィック社)を使用してMOLM-14細胞のゲノムDNAからPCR増幅され、QIAquick PCR精製キット(キアゲン社)を用いて精製した。本発明者らは、鎖状化ベクターの末端と相同な16bpを、正方向プライマー及び逆方向プライマーの5'末端に導入した。In-Fusion HDクローニングキット(クロンテック社、マウンテンビュー、CA州)を使用して、本発明者らは、機能的なスプライス供与部位を含有している、エキソントラップベクターのXhoI制限酵素部位とXbaI制限酵素部位の間に、精製されたアンプリコンをクローニングした。検証されたENOSF1ミニ遺伝子を、対照として使用した(13)。G1E-ER4 9.2細胞(SF3B1WT)及びG1E-ER4 5.13H細胞(SF3B1K700E)に、電気穿孔法(Amaxa Nucleofector(商標)2b、ロンザ社)によって、L溶液中の400ngのERFE又はENOSF1ミニ遺伝子をトランスフェクトした。24時間後、全RNAを、RNeasyミニキット(キアゲン社)を用いて抽出し、蛍光PCRのために処理した。
赤血球及び顆粒球の増殖のために、CD34+細胞を、MidiMacsシステム(ミルテニーバイオテク社、ベルギッシュ・グラートバッハ、ドイツ)を使用して骨髄試料の単核細胞画分から単離した。赤血球の分化のために、CD34+細胞(その純度は80%より高かった)を、15%BIT9500(ミルテニーバイオテク社)、ペニシリン/ストレプトマイシン、L-グルタミン、2UI/mLのエリスロポエチン(ロシュ社、バーゼル、スイス)、100ng/mLの幹細胞因子(ミルテニーバイオテク社)、10ng/mLのIL6(ミルテニーバイオテク社)、及び2×10-7Mのデキサメタゾン(シグマメルク社、ダルムシュタット、ドイツ)を含有しているイスコフ改変ダルベッコ培地中、1mLあたり0.8×106個で4日間培養した。細胞を毎日同じ培地中で4日目まで希釈した。4日目から、IL6を除去した。10日目から16日目まで、細胞を、エリスロポエチン(2UI/mL)に切り替えて、ターミナル赤血球分化を得て、その後、フローサイトメトリーを行なった(GPA、CD71、CD49d、及びBand3)。顆粒球の分化のために、CD34+細胞を、100ng/mLの幹細胞因子、10ng/mLのIL3(ミルテニーバイオテク社)、及び20ng/mLの顆粒球コロニー刺激因子(サンド社、ホルツキルフェン、ドイツ)を含む同培地中で培養した。2日間毎に、赤血球遺伝子(GATA1、HBB)又は顆粒球遺伝子(MPO、SP11)についてメイ・グリュンワルド・ギムザ染色及び逆転写定量PCRを実施した。
細胞を、FACS ARIA3機器(ベクトンディッキンソン社、フランクリン・レイクス、米国)を使用して、骨髄又は血液試料の単核細胞画分から、CD45-PE抗体、CD3-FITC抗体、CD19-ECD抗体、及びCD5-AA700抗体(ベックマンコールター社、ブレア、米国)を使用して選別した。選別された細胞個体群において、RNAを抽出し、SF3B1遺伝子を、サンガー法によってシークエンスした。
プロメガ社から購入したシークエンス等級のトリプシン(50ng)を使用して、溶液中の赤芽球溶解物のタンパク質を37℃で一晩かけて消化した。結果として得られた産物を、データ依存的スキームで作動している、ナノ液体クロマトグラフィー-Q-Exactive Plus質量分析計(サーモフィッシャーサイエンティフィック社)によって分析した:クロマトグラフィーにローディングされたペプチドを捕捉し、ミリQ水中0.1%トリフルオロ酢酸、2%アセトニトリルを用いてC18逆相プレカラム(Acclaim Pepmap、100Å孔、5μmの粒子、2cm長、75μmの内径)で5μL/分で3分間洗浄した。その後、捕捉されたペプチドをC18逆相分析用カラム(2μmの粒径、100Åの孔径、75μmの内径、25cm長)で、ミリQ水中0.1%ギ酸を含有している99%の溶媒Aで開始し、ミリQ水中80%アセトニトリル及び0.085%のギ酸を含有している40%の溶媒Bで終了する、1.5時間の勾配を用いて分離した。質量分析計は、溶出プロセス全体を通してデータを取得した。質量分析の走査は、350から1500Th(トムソン)におよび、AGCターゲットは1×106、最大注入時間は60ミリ秒、解像度は70000であった。HCD(高エネルギー衝突解離)フラグメンテーションが、30秒のダイナミックエクスクルージョン時間で10個の最も豊富なイオンに対して行なわれた。前駆体イオン選択域は、2Thに設定された。高エネルギー衝突解離の正規化衝突エネルギー(NCE)は27%に設定され、MS/MS走査解像度は17,500、AGCターゲットは1×105、最大注入時間は60ミリ秒以内に設定された。スペクトルはプロファイルモードで記録された。質量分析データは、Mascot 2.5.1(www.matrixscience.com)を使用して分析された。使用されるデータベースは、Uniprot-Swissprotデータベース(2017年10月に公開、20314個の配列)に由来するヒト配列の連結されたもの、及びERFEVPFQ配列をはじめとする修飾された配列のカスタム化されたデータベース、及び社内の一般的なコンタミ配列のリストであった。システインカルバミドメチル化が、定常的な修飾として設定され、メチオニン酸化及びプロリンヒドロキシル化は、変動的な修飾として設定された。スペクトルの注釈は、Roepstorff及びFohlman(60)によって提案されたペプチド断片化の命名法に従って行われた。
ヒトERFEWT及びERFE+12のcDNA配列を、以下の修飾を施して、pcDNA3.1にクローニングした:ベクターのシグナル配列(インターロイキン-2)を天然シグナル配列の代わりに使用し、この後にはスペーサー及びFLAGタグがある。組換えタンパク質は、Freestyle F-MAX試薬(ライフテクノロジーズ社)を使用して一過性にトランスフェクトされたFreestyle293F細胞(ライフテクノロジーズ社)の懸濁培養液中で産生された。FLAGタグ化ERFEタンパク質を過剰発現している細胞由来の上清を、5日後に収集し、プロテアーゼ阻害剤混液を補充した。組換えタンパク質を、製造業者(Sigma-Aldrich Chimie社、サン・カンタン・ファラヴィエ、フランス)のプロトコールに従って抗FLAG抗体アフィニティゲルを使用して精製し、150μg/mLのFLAGペプチド(シグマ社)で溶出した。FLAGペプチドを、アミコンウルトラ30K装置(メルクミリポア社、ギュイヤンクール、フランス)を通して調製物をろ過することによって排除し、組換えERFEタンパク質を、食塩水溶液(0.9%NaCl)に懸濁した。タンパク質濃度を、クーマシーインペリアルタンパク染色色素及びピアス社のビシンコニン酸タンパクアッセイ(サーモフィッシャーサイエンティフィック社)を使用して決定した。
ERFEWTを分泌している293F細胞又はERFEVPFQ細胞に由来する上清を、還元条件下(10%の2-メルカプトエタノールを含むレムリ(Laemmli)緩衝液で希釈し、95℃で5分間インキュベートする)及び非還元条件下(2-メルカプトエタノールを含まないレムリ緩衝液で希釈する)でSDS-PAGEにかけ、ニトロセルロース膜(バイオラッド社)にエレクトロブロットした。膜を、TBS-T緩衝液(10mMトリス-HCl、pH7.5、150mM NaCl、0.15%Tween20)中5%ドライミルクを用いてブロッキングし、DYKDDDDKエピトープ(FLAGタグ)を認識するセイヨウワサビペルオキシダーゼ(HRP)にコンジュゲートさせた抗DDKモノクローナル抗体(86861、セルシグナリング社)と共に室温で2時間、又は、ERFEに対するウサギポリクローナル抗体(YenZym社によって作製)と共に4℃で一晩インキュベートし、その後、ヤギ抗ウサギHRP連結抗体(7074S、セルシグナリング社)と共にインキュベートした。酵素活性は、電気化学発光法に基づいた検出システム(GEライフサイエンシーズ社)によって可視化された。ブロットイメージング及び分析は、Chemidoc MPイメージングシステム(バイオラッド社)でImage Labソフトウェアを用いて実施された。
Hep3B肝細胞腫の細胞株を、10%ウシ胎児血清と、対照であるフリースタイル293F細胞又はERFEWT若しくはERFE+12のcDNAをコードしているプラスミドでトランスフェクトされた293F細胞に由来する50%(v/v)上清の補充された、D-MEMグルタマックス(ライフテクノロジーズ社)中で一晩(16時間)インキュベートした。Hep3B細胞に由来する全RNAを、UPzol全RNA単離試薬(バイオテックラビット社)を使用して抽出した。cDNAは、モロニーマウス白血病ウイルス逆転写酵素(プロメガ社)を使用して合成された。定量PCR(qPCR)反応は、LightCycler(登録商標)480DNA SYBRグリーンIマスター反応ミックス(ロシュ・ダイアグノスティックス社)を用いて準備され、LightCycler(登録商標)480システム(ロシュ・ダイアグノスティックス社)で二回実行された。HAMP mRNA転写物の量は、リファレンス遺伝子であるHPRTに対して正規化された。
血漿中の可溶性トランスフェリン受容体アッセイを、Dimension VISTA 1500(シーメンスヘルスケア社、サン・ドニ、フランス)で実施した。循環中のヘプシジンは、以前に記載された質量分析に直列に連結された液体クロマトグラフィー(LC-MS/MS)法によってEDTA血漿試料から定量された(61)。
血漿中エリスロフェロン濃度は、以前に記載28されている通りに決定された。簡潔に言えば、高い結合度の96ウェルプレート(コーニング社)に、50mMの炭酸ナトリウム緩衝液(pH9.6)中1.0μg/mLの捕捉用抗体を100μL/ウェルで4℃で一晩かけてコーティングした。プレートを洗浄(TBS、0.5%Tween-20)し、200μL/ウェルのブロッキング遮断緩衝液(PBS、0.2%カゼインナトリウム、0.05%Tween20、0.1M NaCl)を用いて室温で1時間かけてブロッキングした。組換えヒトERFE標準物質を、10、5、2.5、1.25、及び0.625ng/mLに連続希釈した。室温で1時間インキュベートした後、プレートを洗浄し、1ウェルあたり100mLのヒトエリスロフェロンに対するビオチニル化ウサギモノクローナル抗体(1μg/mL)と共に1時間インキュベートした。その後、プレートを洗浄し、Neutravidin-セイヨウワサビペルオキシダーゼコンジュゲート(サーモフィッシャーサイエンティフィック社、31030番)1/5000(100mL/ウェル)と共に45分間インキュベートし、3回洗浄し、ELISA用の100μLのテトラメチルベンジジン基質システム(サーモフィッシャーサイエンティフィック社、34028番)を用いて暗闇で室温で10分間かけて展開した。反応を、50μlの2N硫酸を添加することによって停止し、プレートを、Spectramax250(モレキュラーデバイス社)で450nmで解読した。
量的変数については、数値は、中央値と四分位範囲(IQR)、又は平均値と平均値の標準誤差(SEM)として表現され、マンホイットニー検定を使用して比較した。カウント数又は比率として報告されるカテゴリー変数を、カイ二乗検定又はフィッシャーの正確確率検定を使用して比較した。転写物の定量については、マンホイットニー検定を使用して、各グループの比較のために統計学的有意性を割り当てた。0.05未満のP値を有意と考えた。多重ロジスティック回帰分析を、一変量分析で0.1未満のP値を用いて選ばれた選択変数のために調整した(JMPバージョン10.0.2、SAS Institute社、ケーリー、NC州)。

5q症候群、17人のMDS-SLD、42人のMDS-MLD、及び18人の1型の芽球増加を伴う(EB1)MDSを含む、156人のMDS患者の集団を確立した(表1)。改訂版国際予後スコアリングシステムスコアは21人の患者において非常に低く、98人の患者において低く、29人の患者において中間であり、2人の患者において高かった。骨髄単核細胞(MNC)画分中の、MDSにおいてよく突然変異している26個の遺伝子をシークエンスした。SF3B1遺伝子は、SF3B1K700Eの突然変異に罹っている63(67%)症例を含む、94症例のMDSにおいて突然変異していた。SF3B1の突然変異を全く有さない62人の患者の中には、他のスプライシング遺伝子であるSRSF2、U2AF1、又はZRSR2が、27症例において突然変異し、35症例においてはスプライシング遺伝子の突然変異は全く観察されなかった。いくつかの症例は、2つのスプライシング突然変異を呈した(データは示されていない)。本発明者らは、21人のSF3B1MUTMDS、6人のSF3B1WTMDSを含む、集団の27人の患者、及び5人の健康な対照から単離された骨髄単核細胞のトランスクリプトームをシークエンスすることによって、遺伝子の発現及びスプライシングに対するSF3B1遺伝子の突然変異の結果を評価した。遺伝子発現及びスプライスジャンクションの差異分析を、DESeq(29)を使用して実施した。本発明者らは、SF3B1MUTMDSとSF3B1WTMDSとの間で発現変動を有するとして6,343個の遺伝子を0.05未満のP値で検出した(データは示されていない)。遺伝子発現プロファイルの主成分分析(PCA)は、SF3B1MUTMDSとSF3B1WTMDSとを分離した(データは示されていない)。発現変動する遺伝子を、log2変化倍率(FC)の絶対値>1、及びベンジャミン・ホッホベルク(BH)により補正されたP値<0.05を用いて、73個の特定の遺伝子オントロジータームに集めた(データは示されていない)。log2(変化倍率)<-1を有する遺伝子は、セリン/トレオニンキナーゼシグナル伝達、アポトーシス、骨髄細胞分化、炎症、細胞間接着に関与したが、一方、log2(変化倍率)>1を有する遺伝子は、ヘム代謝、赤血球分化、及び鉄の恒常性に関与していた(データは示されていない)。本発明者らは、IRON_ION_HOMEOSTASIS遺伝子セットGO:0055072(http://amigo.geneontology.org)に属している16個の発現変動する遺伝子のlog2(変化倍率)をプロットし、エリスロフェロンをコードしているFAM132B/ERFE転写物がアップレギュレートされたことを示した(データは示されていない)。
ERFE+12転写物の発現が、SF3B1の突然変異の存在に依存していることを確かめるために、本発明者らは、赤芽球・巨核球細胞株UT7/EPOに、合成で完全長のSF3B1WTのcDNA又は突然変異体のSF3B1K700EのcDNAをコードしているpLVXプラスミドをトランスフェクトした。その後、本発明者らは、キャピラリー電気泳動により162ヌクレオチド(nt)断片及び150ヌクレオチド断片としてERFE+12及びERFEWTの転写物の検出を可能とする、感度の高い蛍光逆転写PCRを設計した(データは示されていない)。トランスフェクションから24時間後、ERFE+12転写物が、SF3B1K700Eのトランスフェクトされた細胞株において検出されたが、SF3B1WTの細胞株には検出されなかった(データは示されていない)。ERFEmRNA前駆体内におけるSF3B1の突然変異の状況で誘発されるスプライスパターンを検証するために、本発明者らは、ミニ遺伝子スプライシングアッセイを実施した。潜在性3’スプライス部位(AG')の両側に位置する約200ヌクレオチドのERFE配列を、エキソントラップベクターにクローニングした。同ベクターにクローニングされたENOSF1遺伝子(第18染色体:683,395~685,920)における選択的ジャンクションを、対照として使用した(13)。SF3B1K700E突然変異が、CRISPR-Cas9技術によって編集されたG1E-ER4マウス細胞株に、これらのミニ遺伝子がトランスフェクトされた。予想された通り、種間のイントロンの相同性が欠落しているために、G1E-ER4 SF3B1K700E細胞株又は同系SF3B1WT細胞株のトランスクリプトームのシークエンスにより、内因性マウスERFE及びENOSF1が、選択的スプライシングを受けなかったことが実証された(示されていない)。トランスフェクション後、選択的3’スプライス部位AG’ ERFE+12は、キャピラリー電気泳動上で、G1E-ER4 SF3B1K700E細胞には検出されたが、同系G1E-ER4 SF3B1WT細胞には検出されなかった(データは示されていない)。選択的3’スプライス部位AG’の使用は、SF3B1WT細胞株におけるENOSF1遺伝子について検出することができ、SF3B1K700E細胞株において優性となり、このことは、突然変異は選択的AG’の使用を選好することを示唆する。
ヒトERFEは、354アミノ酸ポリペプチドをコードしている。ERFEmRNAへの12ヌクレオチドの付加は、コラーゲンドメインのすぐ上流にバリン-プロリン-フェニルアラニン-グルタミン(VPFQ)の挿入配列を含有している、ERFE変異体(さらにERFEVPFQと称される)を生じる(データは示されていない)。突然変異体ERFEVPFQタンパク質がインビボにおいて産生されたかどうかを調べるために、本発明者らは、SF3B1MUTMDS患者の骨髄のCD34+細胞培養液から得られた、赤芽球の細胞溶解液を調製した。LC MS/MSによるタンパク質の同定を通して、本発明者らは、公共データベース(www.proteomicsdb.org/)においてすでに報告されていたALHELGVYYLPDAEGAFRペプチド(配列番号11)を含むいくつかのペプチドに適合した提案を得(データは示されていない)、本発明者らは、ERFEの108位のVPFQ挿入配列に相当する潜在性ペプチドVPFQFGLPGPPGPPGPQGPPGPIIPPEALLK(配列番号7)を同定した(データは示されていない)。このことは、選択的スプライシングによるERFE+12転写物が、SF3B1MUT赤芽球においてERFEVPFQへと翻訳されることを確認する。
その後、本発明者らは、検証されたイムノアッセイを使用して、156人のMDS患者及び20人の健康な非献血者対照の学習コホートにおける、血漿中ERFEのレベルを測定した(28)。本発明者らはまず、ERFEイムノアッセイが、ERFEWT及びERFEVPFQの両方を検出できたことを確認した。実際に、ヒトERFE ELISAは、ERFEWT及びERFEVPFQの発現ベクターを用いて一過性にトランスフェクトされたHEK293F細胞の上清中に、同じような量(1μg/mL)の各組換えタンパク質を検出した。このことは、どちらのアイソフォームもELISAによって検出できることを確立する。SF3B1MUTMDS又はSF3B1WTMDSにおけるERFEの平均濃度は、対照よりも高かった(P<0.0001;図2A)。MDS試料の中で、ERFE濃度は、SF3B1WTMDS(62.1±36.7ng/mL)と比較して、SF3B1MUTMDS(135.0±72.5ng/mL)の方が高かった(P<0.0001;図2A)。一貫して、ERFE濃度は、全ての他の世界保健機構(WHO)のMDSサブタイプと比較して、MDS-RSの方が高かった(データは示されていない)。その後、本発明者らは、血漿中ヘプシジンレベルを測定し、SF3B1MUTMDSにおけるヘプシジンの濃度が、SF3B1WTMDSと比較して有意により低かったことを確認した(P=0.0309;図2B)。本発明者らは、SF3B1MUTMDSと健康な対照との間に差異を全く認めなかった。フェリチンレベルは、SF3B1WT患者と比較して、SF3B1MUT患者の方が有意により高かった(P<0.0001;図2C)。ヘプシジン/フェリチン比は、SF3B1WTMDSに対してSF3B1MUTMDSにおいて(P<0.0001;図2D)、及びまた他のWHOのサブタイプと比較してMDS-RSにおいてさらにより減少し(データは示されていない)、このことは、SF3B1MUT患者におけるより低いレベルのヘプシジン及びより高い濃度のフェリチンの両方に起因していた。ERFEの血漿中濃度は、ヘプシジン/フェリチン比と逆相関していた(ピアソン検定;P<0.0001;r2=0.360)。本発明者らの分析はまた、100ng/mLの閾値を上回るERFE濃度が、ヘプシジンをより効率的に抑制したことを強調する(データは示されていない)。上昇した血漿中ERFE濃度は、SF3B1WTMDS患者と比較してSF3B1MUTMDS患者において、可溶性トランスフェリン受容体(sTfR)の血漿中濃度の有意な上昇によって評価されるように、より顕著な度合の無効造血に関連していた(図3A)。血清中エリスロポエチンレベルは、SF3B1MUT患者及びSF3B1WT患者において同等に増加し(表1)、ERFEはマウスにおいてはエリスロポエチンによって調節されているが、本発明者らは、血清中エリスロポエチンレベルとERFE濃度との間に全く相関を見出さず、このことは、ERFEの産生が、エリスロポエチンによる赤血球細胞の刺激に限定されなかったことを示唆する(図3B)(27)。
マウスにおいて骨髄におけるERFEmRNAの発現は、エリスロポエチン(EPO)によって調節され、これは好塩基性赤芽球及び多染性赤芽球において優性である(27)。ERFE及びERFE+12の発現が赤血球系統に限定されているかどうかを調べるために、本発明者らは、1つのSF3B1MUT及び1つのSF3B1WTの骨髄CD34陽性細胞試料に由来する赤血球前駆細胞又は顆粒球前駆細胞を平行して増幅した。各系統の純度は、メイ・グリュンワルド・ギムザ染色されたサイトスピンの細胞学的検査、及び逆転写定量PCRによる系統に限定されたマーカーの定量によって評価された(データは示されていない)。様々な分化段階における各転写物のアイソフォームの量を比較するために、本発明者らは、逆転写定量PCRによって、カノニカルERFEWT及び異常なERFE+12を定量した(データは示されていない)。正規化された比の量(NRQ)として表現されるカノニカル転写物ERFEWTの発現は、SF3B1MUT及びSF3B1WTの両方のMDS赤芽球において増加した。顆粒球において、ERFEWTの発現は、検出限界に近かった(データは示されていない)。ERFE+12の発現は赤血球系統に限定され、SF3B1MUT赤芽球の分化に沿って増加し、SF3B1WT赤芽球と比較してSF3B1MUT赤芽球の方が高かった(データは示されていない)。これらの結果は、ERFE+12の発現が、赤血球系統に特異的であることを示す。
より低リスクのMDS患者の50%(MDS-RS患者を含む)が、貧血を治癒するために受けた処置に対して、一次耐性又は続発性の効力低下を経験する。耐性機序がクローン性造血の存続に関与するかどうかは常に不明である。本発明者らは以前に、赤血球造血刺激因子製剤に耐性である5番染色体長腕欠失を伴わないMDSに投与されたレナリドミドが、悪性クローンに標的化し、場合によっては、反応の持続時間中、優性なSF3B1MUTクローンを排除することを示した(31)。しかしながら、赤血球細胞及び骨髄細胞に発現されている、SF3B1変異対立遺伝子の出現頻度は、クローン性赤芽球の量を反映するだけではない。ここで、本発明者らは、GFM-LenEpo-08臨床試験(クリニカルトライアルズ・ドット・ガブにおいてNCT01718379)に含まれる患者の経過観察のために、赤血球に特異的なERFE+12の転写物の発現を使用した。この目的のために、本発明者らは、対を形成したRNA試料を入手することのできた、14人のSF3B1MUTMDS患者における、蛍光PCRを実施し、ERFE+12及びERFEWTのピーク高さを、ERFE+12/ERFE+12+ERFEWTの比として統合した。ERFE+12/ERFE+12+ERFEWTの比は、6人の応答患者においては減少したが、8人の非応答患者においては常に安定であり続けた(図5A、5B)。スクリーニング時と、4サイクルの処置後の評価時との間の比の変動率は、応答患者と非応答患者の間で有意差があった(マンホイットニー検定P=0.0013;図5C、左)。対照的に、スクリーニング時と4サイクル後の評価時との間の骨髄単核細胞のSF3B1変異対立遺伝子の出現頻度の変動率は、8人中6人の非応答患者と、6人中4人の応答患者との間であまり変化がなかった(図5C、右)。要するに、本発明者らの結果は、ERFE+12の発現が、治療下にあるクローン性造血の評価を可能とすることを実証する。

Claims (15)
- 配列番号2の核酸配列に対して少なくとも70%の相同率を有するエリスロフェロン(ERFE)の転写物の変異体、又は、配列番号4のアミノ酸配列に対して少なくとも70%の相同率を有するERFEタンパク質の変異体。
- 変異体が、配列番号2の核酸配列(ERFE+12)を有する、請求項1記載のERFE転写物の変異体。
- 変異体が、配列番号4のアミノ酸配列(ERFEVPFQ)を有する、請求項1記載のERFEタンパク質の変異体。
- 変異体が、そのアミノ酸配列内に少なくとも配列番号7のアミノ酸配列を含む、請求項1記載のERFEタンパク質の変異体。
- 変異体が配列番号3に対して少なくとも70%の相同率を有し、配列内に配列番号5の4つのアミノ酸VPFQを含む、請求項1記載のERFEタンパク質の変異体。
- 患者から得られた試料中における、ERFE転写物の変異体又はERFEタンパク質の変異体の発現を決定する工程を含む、SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における貧血を診断するための方法であって、ここでのこのような変異体の検出は、該患者が、SF3B1遺伝子内の少なくとも1つの突然変異を伴う貧血を患っていることを示す、方法。
- 患者から得られた試料中における、ERFE転写物の変異体又はERFEタンパク質の変異体の発現を決定する工程を含む、貧血の処置が、SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における、SF3B1の突然変異した前駆細胞及び/又は赤血球前駆細胞に標的化するであろうか否かを示すことを可能とする方法であって、ここでのこのような変異体の検出は、該処置が、クローン性の/異常なSF3B1の突然変異した造血に標的化するのに有効であるか否かを示す、方法。
- i)患者から得られた試料中における、レナリドミドによる処置の前後における、ERFE転写物の変異体又はERFEタンパク質の変異体の発現レベルを決定する工程、ii)レナリドミドによる処置の前後に得られた該発現レベルを比較する工程を含む、SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における、レナリドミドによる貧血の処置をモニタリングする方法であって、ここで、レナリドミドによる処置後に得られた該変異体の発現レベルが、処置前に得られた該変異体の発現レベルと比較して減少している場合、このことは、SF3B1の突然変異した造血が減少し、患者はレナリドミドによる処置に応答していることを示す、方法。
- 患者から得られた試料中における、ERFE転写物の変異体又はERFEタンパク質の変異体の発現を決定する工程を含む、SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における、全身の鉄の過剰負荷を診断するための方法であって、ここで、このような変異体の検出は、該患者が、全身の鉄の過剰負荷を有することを示す、方法。

- SF3B1遺伝子内に少なくとも1つの突然変異を有する骨髄悪性腫瘍を患っている患者における、貧血及び/又は全身の鉄の過剰負荷の処置に使用するための、ERFEの転写物又はタンパク質の変異体の阻害剤。
- 以下の配列:AACTGAAAGGGAAC(配列番号8)、AAAGGGAACCTTGGCAGTGAGGACA(配列番号9)又はACCTTGGCAGTGAGGACATGT(配列番号10)を有する、アンチセンスオリゴヌクレオチド。
