Abstract
Description

有効性試験
デューテトラベナジンの有効性及び安全性を評価するための第3相、21週間、多施設、ランダム化、二重盲検、プラセボ対照、並行群間試験を、以下のように実施する。
1.患者はベースライン時に6歳~18歳(6、18歳を含む)である。
2.患者は、ベースライン時に体重が少なくとも26ポンド(12kg)である。
3.患者は乳幼児期(2歳以下)以来脳性麻痺(CP)症状を有しており、CPは進行性ではないと治験責任医師によって判断されている(Monbaliu Eら、Clinical presentation and management of dyskinetic cerebral palsy. Lancet Neurol 2017年;16(9):741-9頁; Wimalasundera N, Stevenson VL. Cerebral palsy. Pract Neurol 2016年;16:184~194頁、これらの全体が参照により本明細書に組み込まれる)。
4.患者は、ヨーロッパにおける脳性麻痺の監視基準(Surveillance of Cerebral Palsy in Europe criteria)に従ってDCPと診断されている(Cans, C. Surveillance of cerebral palsy in Europe: a collaboration of cerebral palsy surveys and registers. Developmental Medicine & Child Neurology 2000年;42:816~24頁、その全体は参照により本明細書に組み込まれる)。
5.患者は、治験責任医師による舞踏病のスコアリングに基づいて、ベースライン訪問時にMD-CRSパートII項目の合計スコアが≧10である。
6.患者の症状は、治験責任医師によるスコアリングに基づいて4以上の臨床全般印象重症度(CGI-S)スコアによって決定される機能上の問題を引き起こしている。
7.舞踏病型が、スクリーニング時にEABによって評価される主要な運動障害である。
8.患者は、治験薬を丸ごと嚥下することができる。
9.患者及び介護者/成人は、投薬レジメンを遵守し、すべての試験手順を順守する意思がある。
10.医学的及び精神医学的病歴、並びに身体的及び神経学的検査によって示されるように、患者は良好な全身健康状態にある。
11.治験責任医師の意見では、患者及び介護者/成人は、試験の性質及びその手順を理解する能力を有しており、患者は設計どおりに試験を完了することが予想されている。
12.患者及び介護者/成人は、地域の規制に従って書面によるインフォームドコンセントを提供した(例えば、患者/青年の場合、患者は必要に応じて14歳以上の患者のための書面によるアセント及び/又は共同意を提供している)。
13.初潮後(postmenarchal)又は12歳以上の女性は、ベースライン時にベータヒト絨毛性ゴナドトロピン検査が陰性であるか、不妊である場合にのみ含めることができる。
14.初潮後又は12歳以上で、男性のパートナーに潜在的に生殖能力がある(つまり、精管切除を行っていない)女性は、試験期間中(つまり、スクリーニングを開始して)、及びIMPの最終投与後30日間、非常に効果的な避妊法を使用する必要がある。
1.a.患者はジスキネジア以外の主要な運動障害を有する。
b.患者の主要な運動症状はジストニアである。
c.患者の主要な運動症状は痙性である。
d.患者は、MD-CRSパートIIの運動評価を損なう可能性のある別の運動障害を有する。
e.患者は、患者の生涯にわたって一貫性がない舞踏病型運動障害を有している。
2.患者は、スクリーニング又はベースライン時に臨床的に重大なうつ病を有する。抗うつ療法を受けている患者は、スクリーニング前に少なくとも6週間安定な用量を服用している場合、登録することができる。
3.患者は、スクリーニングから2年以内に自殺念慮又は関連する行動の病歴を有する:
- 希死念慮の時点で、両価性のレベルに関係なく、特定の計画により自殺念慮に基づいて行動するという以前の意図
- 以前の自殺準備行為又は行動
4.患者は、以前実際の、中断された、又は中止された自殺企図の病歴を有する。
5.患者は、自殺を完遂した一等親血縁者を有する。
6.治験中の臨床試験においてボツリヌス神経毒素(BoNT)を現在受けている患者、又はスクリーニング前の過去4か月間に、受けていた患者。
- 患者は、下肢の痙縮又はジストニアを処置するため、食品医薬品局が承認したBoNTを定期的な間隔で(例えば、3~4か月ごとに)、適度に安定な投与量及び場所で少なくとも2回処置を受けている場合(治験責任医師の判断による)、並びに彼らがこの試験の期間中、BoNT注射のこの安定なレジメンを継続することが予想される場合、患者を試験に含めることができる。患者は、この試験の期間中、3か月ごとに定期的に予定された間隔で、BoNT注射のこの安定なレジメンを継続することが予想される。痙縮又はジストニアの注射は、舞踏病型運動障害の影響を受ける主な領域から離れた筋肉領域に行う必要がある。
- スクリーニングの4か月より前にBoNT注射を受けた患者で、これらの注射を継続する予定がない患者は、この試験の対象と考えられ得る。
7.患者は、指定されたスクリーニングの除外空白期間内に、ジストニア又は舞踏病に対して以下の併用薬のいずれかを受けている:
- 3か月以内:デポー神経弛緩薬

- スクリーニング前に少なくとも4週間投薬が安定である場合に、トピラマート(最高200mg/日)の使用が許容される。
8.患者は、スクリーニング訪問から6か月以内に、異常運動又はCPの処置のために幹細胞、脳深部刺激法、経頭蓋磁気刺激法(transmagnetic stimulation)、若しくは経頭蓋直流電気刺激法による処置を受けているか、又は患者は安定な臨床状態ではない。
9.患者は最近外科的処置を受けているか、又は試験中に外科的処置を受けることが予想され、治験責任医師の意見では、このことによって患者は試験に適さないものになる。
10.患者は、スクリーニング又はベースラインで重度の精神障害又は不安定若しくは重大な医学的疾患(例えば、てんかん)を有しており、治験責任医師の意見では、このことによって、この試験に参加する患者の能力が危険にさらされるか、損なわれる可能性がある。
11.患者は、スクリーニング時に、12誘導心電図でFridericiaの式値>450ミリ秒を使用して心拍数を補正したQT間隔を有する。
12.トルサードドポワント、先天性QT延長症候群、徐脈性不整脈、その他の心不整脈、又は非代償性心不全の病歴を有する患者。
13.以下に示すように、患者は肝機能の低下のエビデンスを有する:
- スクリーニング時のアスパラギン酸アミノトランスフェラーゼ(AST)又はアラニンアミノトランスフェラーゼ(ALT)>2.5×基準範囲上限(ULN)
- スクリーニング時のアルカリホスファターゼ(ALP)又は総ビリルビン>2×ULN
- 注意:ジルベール症候群の患者は、メディカルモニターによって承認された場合に参加する資格がある。
- 注意:以下の臨床検査パラメータのうちの2つ以上に異常がある患者は、メディカルモニターによる登録の承認を受ける必要がある:AST、ALT、ALP、及び総ビリルビン。
14.患者は、スクリーニング時の血清クレアチニン>1.5×ULNによって示される、臨床的に重大な腎機能障害のエビデンスを有する。
15.患者は、治験薬(IMP)の成分のいずれかに対して既知のアレルギーを有する。
16.患者は治験薬又はデバイス試験に参加し、スクリーニングから30日以内又は5つの薬物半減期以内のいずれか長い期間内にIMP/介入を受けた。
17.患者は、妊娠中又は授乳中である。
18.精神障害の診断及び統計マニュアル(Diagnostic and Statistical Manual of Mental Disorders)(第5版)で定義されているように、患者はスクリーニング前の12か月間におけるアルコール又は薬物乱用の病歴を有するか、又はスクリーニング前の12か月間におけるアルコール又は薬物乱用を認めている。
19.患者は、尿中薬物スクリーニング検査の結果が陽性であるか、又は試験全体を通して薬物乱用を控えることができない。
これは、乳幼児期以来(2歳以下)、非進行性の脳性麻痺(CP)症状を有しており、主要な舞踏病型運動障害を伴うDCPの患者(6~18歳)において1日1回6mgの開始用量の経口錠剤として投与された、デューテトラベナジンの有効性及び安全性を評価するための第3相、21週間、多施設、ランダム化、二重盲検、プラセボ対照、並行群間試験である。この試験は、スクリーニング期間(最長31日)と、滴定期間(7週間)及び維持期間(8週間)を含む二重盲検処置期間と、それに続く1週間の休薬期間と、休薬期間の1週間後の、フォローアップ電話連絡とで構成される。試験全体にわたって、患者は、安全性/忍容性、ジスキネジアの重症度、及び行動状態(診療所内のみ)の評価のために、診療所内で及び電話(非録画ライブビデオを用いて)により、定期的に治験センターの担当者と対話する。
・患者は、プロトコルで定義された「臨床的に重大な」有害事象(IMPに関連し、強度が中程度若しくは重度であるか、重大な有害事象の基準を満たす有害事象として定義される)を経験するか、又は
・ベースラインでの患者の体重とCYP2D6障害状態に基づいて、最大許容用量に達する。
主要推定値は、用量減少、中断、又は中止が起こったかどうかに関係なく、かつ処置関連の有害事象に関係なく中央判定によるMD-CRSパートIIにおけるベースラインから15週目までの変化についての、標的患者集団におけるデューテトラベナジンとプラセボとの間の平均の差である。
主要評価項目は、中央判定MD-CRSパートIIのベースラインから15週目までの変化である (デューテトラベナジン対プラセボ) 。
主な副次的評価項目は以下の通りである:
1.中央判定によるMD-CRSパートIのベースラインから15週目までの変化(デューテトラベナジン対プラセボ)
2.15週目のCaGI-I(デューテトラベナジン対プラセボ)
3.15週目の臨床全般印象改善度(CGI-I)(デューテトラベナジン対プラセボ)
その他の有効性尺度及び評価項目(デューテトラベナジン対プラセボ)には、以下のものが含まれる:
・MD-CRS全般指標(MD-CRSパートI及びパートIIから計算)
・統一ハンチントン病評価尺度-総合運動スコア(UHDRS-TMS)
・統一ハンチントン病評価尺度-総合最大舞踏病(UHDRS-TMC)
・統一ハンチントン病評価尺度-総合最大ジストニア(UHDRS-TMD)
・小児評価障害目録-コンピュータ適応試験(PEDI-CAT)(日常生活動作、親/介護者完了、カウンターバランスバージョン)
・小児の生活の質の目録(PedsQL)のCPモジュール(生活の質、患者/介護者)
・患者の全般印象改善尺度([PGI-I]、全般的、患者/介護者)
・臨床全般印象重症度([CGI-S]、全般的、医師による評価)
・CaGI-I応答、介護者によって、CaGI-Iスコアにおいて「大幅に改善された」又は「非常に大幅に改善された」と説明された患者として定義されている。
・CGI-I応答、CGI-Iスコアにおいて「大幅に改善された」又は「非常に大幅に改善された」と説明された患者として定義されている。
・CGI-S応答、CGI-Sスコアにおいて≧1ポイント低下した患者として定義されている。
・PGI-I応答、PGI-Iスコアにおいて「大幅に改善された」又は「やや改善された」と説明された患者として定義されている。
主要有効性評価項目を、α=0.025の片側有意水準で試験する。
およそ90人の患者が試験(フォローアップを含む)を完了すると、iDMCは、中央判定によるMD-CRSパートIIに基づく無益性及びサンプルサイズの再推定のために、非盲検IAを実施する。
安全性分析セットに対して、安全性分析を実施する。
患者の15%よりも多くが処置期間の終了前に試験から離脱する場合、試験中止までの日数を、ITT分析セットを使用するカプランマイヤー方法論を使用して分析する。
薬物動態分析のために収集したサンプルを、デューテトラベナジン並びにデューテトラベナジンの活性代謝物としてのアルファ-ジヒドロテトラベナジン(α-HTBZ)及びベータ-ジヒドロテトラベナジン(β-HTBZ)、並びに必要に応じて、その他の代謝物について定量化する。α-HTBZとβ-HTBZの合計を、個々の濃度から計算する。デューテトラベナジン及び代謝物の濃度(個別に及び合計として)は、集団薬物動態手法を使用して分析することができる。デューテトラベナジン試験からの関連するすべての蓄積された薬物動態/薬力学データを使用して、薬力学/安全性評価項目について探索的薬物動態/薬力学的分析を実施することができる。
デューテトラベナジンによる処置は、安全かつ効果的であることが示されている。デューテトラベナジンを用いた処置によってすべての主要及び副次的評価項目における有意な改善がもたらされる。
非盲検継続試験
子供及び青年の脳性麻痺におけるジスキネジアの処置のためのデューテトラベナジンの第3相、55週間、非盲検、単群、長期安全性、忍容性、及び有効性の試験を、以下のように実施する。
1.患者は、親試験を完了している(実施例1、上記)。
2.患者は、この試験の1日目に、体重が少なくとも12kg(26ポンド)である。
3.患者は、デューテトラベナジン錠を丸ごと嚥下することができる。
4.患者及び介護者は、デューテトラベナジンレジメンを遵守し、すべての試験手順を順守する意思がある。
5.医学的及び精神医学的病歴、並びに身体的及び神経学的検査によって示されるように、患者は良好な全身健康状態にある。
6.治験責任医師の意見では、患者及び介護者は、試験の性質及びその手順を理解する能力を有しており、患者は設計どおりに試験を完了することが予想されている。
7.未成年の患者の場合、親/法定後見人は書面によるインフォームドコンセントを提供し、患者は(地域の規制に従って)アセントを提供する。成人患者は(地域の規制に従って)、独自の書面によるインフォームドコンセントを提供する。
8.介護者は、成人患者によって役割が割り当てられた後、又はこの役割が未成年の患者の親/法定後見人によって委任された場合、書面によるインフォームドコンセントを提供する。
9.初潮後又は12歳以上の女性は、1日目にβ-HCG検査が陰性であるか、又は不妊である場合にのみ含めることができる。
10.初潮後又は12歳以上で、男性のパートナーに潜在的に生殖能力がある(つまり、精管切除を行っていない)女性は、試験期間中(つまり、1日目から)、デューテトラベナジンの最終投与後30日間、非常に効果的な避妊法を使用する必要がある。
1.患者は、この試験の1日目に臨床上重大なうつ病を患っている。
2.患者は、自殺念慮又は関連する行動の病歴を有する:
- 希死念慮の時点で、両価性のレベルに関係なく、特定の計画により自殺念慮に基づいて行動するという以前の意図
3.患者は、以前実際の、中断された、又は中止された自殺企図の病歴を有する。
4.患者は、自殺を完遂した一等親血縁者を有する。
5.治験中の臨床試験においてボツリヌス神経毒(BoNT)を現在受けている患者、又は受けていた患者。
6.患者は、この試験の1日目の指定された除外空白期間内に、ジストニア又は舞踏病に対して以下の併用薬のいずれかを受けている:
- 3か月以内:デポー神経弛緩薬
- 30日以内:テトラベナジン、又はバルベナジン
- 21日以内:レセルピン
- 14日以内:神経弛緩薬(経口)、定型及び非定型抗精神病薬、メトクロプラミド、レボドパ、ドーパミン作動薬、及びモノアミンオキシダーゼ阻害薬
7.患者は、異常運動又はCPの処置のために幹細胞、脳深部刺激法、経頭蓋磁気刺激法、若しくは経頭蓋直流電気刺激法による処置を受けているか、又は患者は安定な臨床状態ではない。
8.患者は最近外科的処置を受けているか、又は試験中に外科的処置を受けることが予想され、治験責任医師の意見では、このことによって患者は試験に適さないものになる。
9.患者は、重度の精神障害又は不安定若しくは重大な医学的疾患(例えば、てんかん)を有しており、治験責任医師の意見では、このことによって、この試験に参加する患者の能力が危険にさらされるか、損なわれる可能性がある。
10.患者は、この試験の1日目に、12誘導心電図でFridericiaの式(QTcF)値>450ミリ秒を使用して心拍数を補正したQT間隔(QTc)を有する。
11.トルサードドポワント、先天性QT延長症候群、徐脈性不整脈、その他の心不整脈、又は非代償性心不全の病歴を有する患者。
12.以下に示すように、患者は肝機能障害のエビデンスを有する:

15.患者は、親試験以外の治験薬又はデバイス試験に参加し、この試験の1日目から30日以内又は5つの薬物半減期以内のいずれか長い期間内にIMP/介入を受けた。
16.患者は、妊娠中又は授乳中である。
17.精神障害の診断及び統計マニュアル(Diagnostic and Statistical Manual of Mental Disorders)第5版(DSM-V(商標))で定義されているように、患者はアルコール又は薬物乱用の病歴を有するか、又はアルコール又は薬物乱用を認めている。
18.患者は、尿中薬物スクリーニング検査の結果が陽性であるか、又は試験全体を通して薬物乱用を控えることができない。
忍容できる量のデューテトラベナジンの長期投与によって、好ましい安全性プロファイルにより、脳性麻痺のジスキネジアに関連する異常不随意運動が減少することが示されている。
成人における有効性試験
デューテトラベナジンの有効性及び安全性を評価するための第3相試験を、DCPと診断された男性及び女性の患者からなる集団に対して実施する。以下の基準のすべてを満たす場合にのみ、患者を試験に含めることができる:
1.患者はベースライン時に18歳を超えている。
2.患者は乳幼児期(2歳以下)以来脳性麻痺(CP)症状を有している。
3.患者は、ヨーロッパにおける脳性麻痺の監視基準に従ってDCPと診断されている。
4.患者は、舞踏病の治験責任医師によるスコアリングに基づいて、ベースライン訪問時にMD-CRSパートII項目の合計スコアが≧10である。
5.患者の症状は、治験責任医師によるスコアリングに基づいて4以上の臨床全般印象重症度(CGI-S)スコアによって決定される機能上の問題を引き起こしている。
6.舞踏病型が、スクリーニング時にEABによって評価される主要な運動障害である。
成人患者への忍容できる量のデューテトラベナジンの投与によって、好ましい安全性プロファイルにより、脳性麻痺のジスキネジアに関連する異常不随意運動が減少することが示されている。
有効性試験
デューテトラベナジンの有効性及び安全性を評価するための第3相試験を、DCPと診断された男性及び女性の患者からなる集団に対して実施する。以下の基準のすべてを満たす場合にのみ、患者を試験に含めることができる:
1.患者はベースライン時に6歳~18歳(6、18歳を含む)である。
2.患者は乳幼児期(2歳以下)以来脳性麻痺(CP)症状を有している。
3.患者は、ヨーロッパにおける脳性麻痺の監視基準に従ってDCPと診断されている。
4.患者は、舞踏病の治験責任医師によるスコアリングに基づいて、ベースライン訪問時にMD-CRSパートII項目の合計スコアが≧10である。
5.患者の症状は、治験責任医師によるスコアリングに基づいて4以上の臨床全般印象重症度(CGI-S)スコアによって決定される機能上の問題を引き起こしている。
6.患者の主要な運動症状はジストニアである。
小児患者への忍容できる量のデューテトラベナジンの投与によって、好ましい安全性プロファイルにより、脳性麻痺のジスキネジアに関連するジストニアの異常不随意運動が減少することが示されている。
Claims (17)
- 患者への治療有効量の投与が、患者に、1日量約6mg/日~約48mg/日を、1又は2用量で投与することである、請求項1に記載の方法。
- 処置によって、ベースラインでの患者の異常不随意運動と比較して、患者の脳性麻痺におけるジスキネジアに関連する異常不随意運動が維持されるか又は減少する、請求項1又は2に記載の方法。
- 脳性麻痺におけるジスキネジアに関連する異常不随意運動が、ジストニア型のものである、請求項3に記載の方法。
- 脳性麻痺におけるジスキネジアに関連する異常不随意運動が、舞踏病型のものである、請求項3に記載の方法。
- ヒト患者が小児患者である、請求項1から5のいずれか一項に記載の方法。
- 脳性麻痺におけるジスキネジアに関連する異常不随意運動を、評価尺度:
a)MD-CRSパートII;
b)MD-CRSパートI;
c)CaGI-I;及び
d)CGI-I、
のうちの少なくとも1つによって測定する、請求項3から6のいずれか一項に記載の方法。 - 脳性麻痺におけるジスキネジアに関連する異常不随意運動が、ベースラインでの患者の異常不随意運動と比較して少なくとも20%低下する、請求項7に記載の方法。
- 患者が、ベースラインと比較して、少なくとも0.5ポイントのMD-CRSパートII合計スコアの低下を呈する、請求項7に記載の方法。
- 患者が、ベースラインと比較して、少なくとも2ポイントのMD-CRSパートI合計スコアの低下を呈する、請求項7に記載の方法。
- 患者が、ベースラインで体重が少なくとも12kgである、請求項3から10のいずれか一項に記載の方法。
- 患者が、ベースラインで体重が12kg~17kg未満であり、患者にCYP2D6障害がない場合、投与されるデューテトラベナジンの1日量は約24mg以下であるか;又は患者にCYP2D6障害がある場合、約12mg以下である、請求項11に記載の方法。
- 患者が、ベースラインで体重が17kg~30kg未満であり、患者にCYP2D6障害がない場合、投与されるデューテトラベナジンの1日量は約30mg以下であるか;又は患者にCYP2D6障害がある場合、約18mg以下である、請求項11に記載の方法。
- 患者が、ベースラインで体重が30kg~40kg未満であり、患者にCYP2D6障害がない場合、投与されるデューテトラベナジンの1日量は約42mg以下であるか;又は患者にCYP2D6障害がある場合、約24mg以下である、請求項11に記載の方法。
- 患者が、ベースラインで体重が40kg以上であり、患者にCYP2D6障害がない場合、投与されるデューテトラベナジンの1日量は約48mg以下であるか;又は患者にCYP2D6障害がある場合、約36mg以下である、請求項11に記載の方法。
- Dと表されている各位置に、約50%以上、約90%以上、約95%以上、約98%以上、又は約99%以上の重水素濃縮がある、請求項1から16のいずれか一項に記載の方法。
