脂肪細胞化抑制因子としても知られているインターロイキン11(IL-11)は多形質発現性サイトカインであり、IL-6、IL-11、IL-27、IL-31、オンコスタチン、白血病抑制因子(LIF)、カルジオトロフィン-1(CT-1)、カルジオトロフィン様サイトカイン(CLC)、毛様体神経栄養因子(CNTF)およびneuropoetin(NP-1)を含むIL-6サイトカインファミリーのメンバーである。
IL-11は、低い親和性(Kd=約10nmol/L)でIL-11Rαに結合し、これらの結合パートナー間での相互作用のみでは生体シグナルを伝達することはできない。高い親和性(Kd=約400~800pmol/L)で結合してシグナルを伝達できる受容体の形成には、IL-11Rαとgp130の共発現が必要とされる(Curtis et al Blood 1997; 90 (11):4403-12; Hilton et al., EMBO J 13:4765, 1994; Nandurkar et al., Oncogene 12:585, 1996)。細胞表面のIL-11RαにIL-11が結合すると、ヘテロ二量体化、チロシンのリン酸化、gp130の活性化および下流のシグナル伝達が誘導され、このシグナル伝達は、主として、マイトジェン活性化プロテインキナーゼ(MAPK)カスケードおよびヤヌスキナーゼ/シグナル伝達兼転写活性化因子(Jak/STAT)経路を介して行われる(GarbersおよびScheller、上掲)。
本発明の態様は、肝毒性ならびに肝毒性を特徴とする障害、疾患および状態の診断、治療および予防に関する。
本発明の態様は、IL-11媒介性シグナル伝達の抑制を含む。
いくつかの実施形態において、IL-11媒介性シグナル伝達を抑制することができる薬剤は、IL-11に結合してもよい。いくつかの実施形態において、IL-11媒介性シグナル伝達を抑制することができる薬剤は、IL-11受容体(例えば、IL-11Rα、gp130、またはIL-11Rαおよび/もしくはgp130を含む複合体)に結合してもよい。このような薬剤がIL-11またはIL-11受容体に結合すると、IL-11受容体に対するIL-11の結合能が低減/阻害されて、IL-11媒介性シグナル伝達が抑制され、その結果、下流のシグナル伝達が抑制されてもよい。また、このような薬剤がIL-11またはIL-11受容体に結合すると、IL-11受容体(例えばIL-11Rαおよび/またはgp130)に対するIL-11の結合能が低減/阻害されて、IL-11媒介性シスシグナル伝達および/またはIL-11媒介性トランスシグナル伝達が抑制され、その結果、下流のシグナル伝達が抑制されてもよい。前記薬剤は、IL-11と可溶性IL-11Rαからなる複合体などのトランスシグナル伝達複合体に結合して、gp130媒介性シグナル伝達を抑制してもよい。
いくつかの実施形態において、IL-11もしくはIL-11含有複合体またはIL-11受容体に結合することができる薬剤は、抗体またはその抗原結合断片である。いくつかの実施形態において、IL-11もしくはIL-11含有複合体またはIL-11受容体に結合することができる薬剤は、ポリペプチド、例えばデコイ受容体分子である。いくつかの実施形態において、IL-11もしくはIL-11含有複合体またはIL-11受容体に結合することができる薬剤は、アプタマーであってもよい。
IL-11またはIL-11含有複合体に結合することが可能な、ペプチドベースまたはポリペプチドベースの薬剤は、IL-11受容体に基づいて作製されたものであってもよく、例えばIL-11受容体のIL-11結合断片に基づいて作製されたものであってもよい。
本発明は、IL-11、IL-11含有複合体、IL-11Rα、gp130、またはIL-11Rαおよび/もしくはgp130を含む複合体のうちの1つ以上に結合して、IL-11媒介性シグナル伝達を抑制することができる阻害剤分子の使用を企図する。
いくつかの実施形態において、IL-11もしくはIL-11含有複合体またはIL-11受容体(例えば、IL-11Rα、gp130、またはIL-11Rαおよび/もしくはgp130を含む複合体)に結合することができる薬剤は、アプタマーである。核酸リガンド/ペプチドリガンドとも呼ばれるアプタマーは、高い特異性および高い親和性で標的分子に結合する能力を特徴とする核酸分子またはペプチド分子である。現在までに同定されたアプタマーの大部分は非天然分子である。
IL-11もしくはIL-11含有複合体またはIL-11受容体に結合することができる本発明の薬剤は、以下の特性のいずれか1つ以上を示してもよい。
・IL-11もしくはIL-11含有複合体またはIL-11受容体に対する特異的結合
・10μM以下のKD、好ましくは5μM以下、1μM以下、500nM以下、100nM以下、10nM以下、1nM以下または100pM以下のKDでの、IL-11もしくはIL-11含有複合体またはIL-11受容体への結合

本発明の態様において、IL-11媒介性シグナル伝達を抑制することができる薬剤は、IL-11、IL-11Rαまたはgp130のうちの1つ以上の発現を阻止または低減できるものであってもよい。
標的細胞へのsiRNAの送達に適していると考えられる他の技術としては、米国特許第6,649,192(B)号明細書および米国特許第5,843,509(B)号明細書に記載されているような、ナノ粒子またはナノカプセルを使用した方法が挙げられる。
本発明の実施形態において、IL-11の作用を抑制することができる薬剤は、以下の機能特性のうちの1つ以上を有していてもよい。
・IL-11媒介性シグナル伝達の抑制
・IL-11Rα:gp130受容体複合体へのIL-11の結合を介したシグナル伝達の抑制

本発明は、例えば、本明細書で述べるような、肝毒性ならびに肝毒性に関連する障害、疾患および状態の治療/予防のための方法ならびに発明品(薬剤および組成物)を提供する。さらに、例えば、本明細書で述べるような、肝毒性ならびに肝毒性に関連する障害、疾患および状態の治療/予防のための方法を提供する。

IL-11媒介性シグナル伝達を抑制することができる薬剤の投与は、対象が利益を受けるのに十分な「治療に有効な」量または「予防に有効な」量で行うことが好ましい。
本発明の態様および実施形態のいくつかは、対象から得られた試料における、IL-11またはIL-11受容体(例えば、IL-11Rα、gp130、またはIL-11Rαおよび/もしくはgp130を含む複合体)の発現の検出に関する。
IL-11またはIL-11受容体(例えば、IL-11Rα、gp130、またはIL-11Rαおよび/もしくはgp130を含む複合体)の発現のアップレギュレーションの検出は、肝毒性および/または肝毒性に関連する障害、疾患もしくは状態を診断して、肝毒性および/または肝毒性に関連する障害、疾患もしくは状態の発症のリスクがある対象を特定する方法において使用してもよく、IL-11媒介性シグナル伝達を抑制することができる薬剤を用いた治療に対する対象の反応性の予後判定または予測を行う方法において使用してもよい。

本発明は、さらに、肝組織への損傷を低減する方法、肝細胞死を低減させる方法、肝機能を向上させる方法、血清中ALT濃度を低下させる方法、肝重量を増加させる方法、肝組織を再生させる方法、もしくは肝臓中のERKおよび/もしくはJNKの活性化(すなわちリン酸化)を低減させる方法において使用するための、IL-11媒介性シグナル伝達を抑制することができる薬剤、または前記方法における、IL-11媒介性シグナル伝達を抑制することができる薬剤の使用を提供する。
2つ以上のアミノ酸配列間または核酸配列間の同一性(%)を求めるためのペアワイズ配列アラインメントおよび多重配列アラインメントは、当業者に公知の様々な方法を使用して行うことができ、例えば、ClustalOmegaソフトウェア(Soding, J. 2005, Bioinformatics 21, 951-960)、T-coffeeソフトウェア(Notredame et al. 2000, J. Mol. Biol. (2000) 302, 205-217)、Kalignソフトウェア(Lassmann and Sonnhammer 2005, BMC Bioinformatics, 6(298))、MAFFTソフトウェア(Katoh and Standley 2013, Molecular Biology and Evolution, 30(4) 772-780)などの一般公開されているコンピュータソフトウェアを使用して行うことができる。このようなソフトウェアを使用する場合、例えばギャップペナルティや伸長ペナルティなどにおいて、デフォルトパラメータを使用することが好ましい。
肝細胞に対するIL-11の効果を調べるため、ヒト初代肝細胞を用いて細胞培養を行うことにより実験を行った。
アセトアミノフェン(APAP)誘発性肝障害マウスモデルを用いて、肝毒性に対する抗IL-11療法の効果を調べた。
IL-11媒介性シグナル伝達に対する拮抗作用が肝細胞の生存率に及ぼす効果をインビトロで分析した。
20mg/kgの抗IL11RAアンタゴニスト抗体(X209)またはアイソタイプ適合IgGコントロール抗体を12~14週齢の雄性マウスに腹腔内投与(IP)し、その16時間後に、重度の過剰摂取に相当する量のAPAP(400mg/kg)またはこれと同じ量の生理食塩水を腹腔内注射(IP)により投与した。
重度の過剰摂取に相当する量のAPAP(400mg/kg)またはこれと同じ量の生理食塩水を腹腔内注射(IP)により12~14週齢の雄性マウスに投与し、その10時間に、20mg/kgの抗IL11RAアンタゴニスト抗体(X209)またはアイソタイプ適合IgGコントロール抗体を腹腔内投与(IP)するか、何も処置を行わなかった。
6.1 概要
アセトアミノフェン(APAP)の過剰摂取は、肝不全の主な原因である。APAP誘発性肝障害(AILI)のマウスモデルに対して、組換えヒトインターロイキン11(rhIL11)の投与は保護効果を発揮する。
アセトアミノフェン(N-アセチル-p-アミノフェノール(APAP))は、一般医薬品として販売されている鎮痛剤であり、過剰摂取(OD)に至ることが多く、過剰摂取によりAPAP誘発性肝障害(AILI)を起こす。APAP誘発性肝障害(AILI)は、急性肝不全の主な原因である(1)。抗酸化剤であるN-アセチルシステイン(NAC)は、APAPの過剰摂取の初期の患者には有益であるが(2)、過剰摂取後8時間を超えると、薬物を用いた治療法はなく、肝移植が不可能であれば死に至ることもある(3,4)。
過去の報告と同様に(17)、APAP誘発性肝障害(AILI)は、肝障害マウスにおける血清中IL11濃度の上昇を特徴とすることが確認された(図9A)。次に、本発明者らは、APAP誘発性肝障害マウスモデルにおける血清中IL11濃度の上昇が肝臓に起因するものであるか否かを検討した。APAPによって、肝臓内のIl11転写産物が強くアップレギュレートされた(35倍、P<0.0001)。Il11の開始コドンにルシフェラーゼをクローニングしたレポーターマウスの生物発光を画像化したところ、肝臓全体においてIL11が発現していることが示された(図9B、図9Cおよび図15)。また、ウエスタンブロットにより、APAP誘発性肝障害を発症した後の時間経過全体を通してIL11がタンパク質レベルでアップレギュレートされていたことが確認された(図9D)。EGFPレポーター構築物をIl11の3’UTRに挿入した第2のレポーターマウスを使用した実験(図16)では、APAPの投与後、カスパーゼ3の切断(Cl.CASP3)と一致して、IL11タンパク質が肝小葉中心部の壊死性肝細胞において高発現されることが示され、これは、APAP誘発性肝障害に特有の特徴であった(図9E)。
rhIL11は、げっ歯類の肝損傷モデルにおいて保護効果を有することが一貫して報告されているが(17~20,23)、実施例6.3で述べた結果では、rhIL11がインビトロのヒト肝細胞に対して正反対の効果を及ぼすことが示唆された(図9)。これを受けて、本発明者らは、この結果の不一致をさらに調べるため、ヒトIL11タンパク質とマウスIL11タンパク質の配列相同性が82%に過ぎないことを踏まえ、rhIL11タンパク質を別の生物種に使用した場合を試験した。本発明者らは、まず、マウス肝細胞に対するrhIL11の効果と組換えマウスIL11(rmIL11)の効果を比較した。生物種を一致させたrmIL11は、マウス肝細胞においてERKおよびJNKのリン酸化を刺激し、CASP3の切断を誘導したが、rhIL11は何ら効果を及ぼさなかった(図10A)。これと同様に、rmIL11はマウス肝細胞死を誘導したが、rhIL11はマウス肝細胞死を誘導しなかった。実際に、rhIL11は、高用量においてマウス肝細胞死を抑制する傾向が見られた(図10A)。これらの実験とは関係性を逆にしてヒト肝細胞を用いた実験では、rhIL11がERKおよびJNKのシグナル伝達と肝細胞死を刺激するのに対して、rmIL11はこのような刺激をもたらさないことが見出された(図18Aおよび図18B)。
インビボで肝細胞から分泌されるマウス内因性IL11の効果を試験するため、アルブミンプロモーターにより駆動されるCre構築物をコードするAAV8ウイルスをRosa26Il11/+マウス(15,16)に注射することによって、Il11導入遺伝子を肝細胞において特異的に発現させた(Il11-Tgマウス、図11A)。導入遺伝子の誘導の3週間後に、Il11-Tgマウスの肝臓は、異常な肉眼的所見を呈すとともに縮小しており(38%、P<0.0001)、血清中ALT濃度およびAST濃度も上昇していたが、その他の器官は影響を受けていなかった(図11A~図11Dならびに図19Aおよび図19B)。組織学的試験では、顕著な門脈の拡張および類洞における血液蓄積(類洞閉塞症候群を示唆する所見)が認められ、門脈三管の周囲への浸潤が見られた(図11Eおよび図19C)。肝臓におけるIl11-Tgを分子解析したところ、ERKおよびJNKの活性化とCASP3の切断が見られるとともに、炎症促進遺伝子の発現の増加が認められた(図11F、図19Dおよび図19E)。したがって、APAPの毒性において見られるように(図9)、肝細胞から分泌されるIL11は肝毒性を有する。
APAPの誘導によるJNKの活性化は、ROSの生成とGSHの枯渇に続いて起こることが知られているが、インビトロでのAPAPの誘導によるJNKの活性化には、IL11のシグナル伝達が必要とされる(図9Iおよび図9J)。Il11-Tgマウスにおいて肝臓中のGSH濃度を調べたところ、低下していることが見出されたが(62%、P<0.0001)、これは、IL11のシグナル伝達によって直接的または間接的にROSが誘導されることを示している(図11G)。
Il11ra1を成体マウスの肝細胞において特異的に欠失させるため、LoxP部位に挟まれたIl11ra1アレルのホモ接合型マウスにAAV8-ALB-Creウイルスを注射することによって条件付きIl11ra1ノックアウト(CKO)を作製した。これと同時に、同じマウスを使用して野生型コントロールも作製した。ウイルスに感染させてから3週間後に、コントロールマウスおよびCKOマウスにAPAP(400mg/kg)を投与した(図12A)。APAP投与の翌日に、肉眼的解剖学的試験を行ったところ、コントロールマウスでは縮小し変色した肝臓が見られたが、CKOマウスの肝臓は正常な状態を呈した(図12B)。組織学的試験では、コントロールマウスは典型的な小葉中心壊死を広範に呈したが、このような小葉中心壊死はCKOでは観察されなかった(図12C)。
次に、IL11のシグナル伝達の治療的抑制がAPAP誘発性肝障害(AILI)の軽減に有効であるか否かを、抗Il11RA(X209)抗体を投与することにより試験した(14)。まず、APAPを投与する16時間前に、X209またはコントロール抗体(10mg/kg)を注射することにより予防処置を行った。このアプローチにより、肝損傷の血清マーカーが70%を超える低下を示し、肝臓中のGSH濃度がほぼ回復し、組織学的試験で確認された小葉中心壊死が抑制された(図13A~図13Dおよび図25A)。
APAPを過剰摂取してから8時間以降に緊急処置室に搬送された患者に対して有効な処置は存在しない。このような事態を踏まえ、本発明者らは、マウスにAPAP(400mg/kg)を投与してから10時間後の抗IL11RAの効果を試験した(図14A)。マウスではAPAPの代謝が加速していることを考慮に入れると、このモデルでの10時間後の治療は、APAPを過剰摂取してから最大24時間後のヒトに対する治療と同等であると見なせる。血清中のAPAPおよびAPAP-グルタチオンを質量分析により定量したところ、予想したとおり、生理食塩水処置コントロールよりも濃度が上昇しており、実験群間では同程度であることが分かった(図26Aおよび図26B)。肉眼的解剖学的試験、組織学的試験ならびに血清中のIL11濃度、ALT濃度およびAST濃度を分析したところ、X209はAPAPの過剰摂取後2日目までには肝損傷を大幅に緩和したが、IgG処置マウスでは深刻かつ持続的な肝損傷が認められることが明らかになった(図14B~図14E、および図27A)。この治療抗体は、実験全体を通してERKおよびJNKの活性化を効果的に阻害し、これは、24時間後の切断型CASP3の低下に先行して見られた(図14Fおよび図27B)。
英国では、毎年最大で50,000人もの患者がAPAPの過剰摂取で緊急処置室を受診するほどAPAPの過剰摂取は一般的であり、その中には移植が必要な肝不全を発症する患者もいる(1)。IL11は、APAPにより誘発された肝不全(17,20)、肝虚血(18,21)、内毒素血症(22)および炎症(19)に対して保護効果を発揮することが過去に報告されているが、実際には、肝毒性を有し、APAPの過剰摂取後の肝不全において中心的かつ重要な役割を担っていることが示されている。

動物実験は、SingHealthの動物実験委員会(IACUC)により承認されたものであり、動物実験委員会のガイドラインに従って実施した。水のみを自由に与えた絶食期間中を除き、すべてのマウスに対して食物および水を自由に与えた。
12~14週齢の雄性マウス(特に明記しない限り、C57BL6/NTACマウス)にアセトアミノフェン(APAP)を投与する前に、一晩絶食させた。次に、重度の過剰摂取に相当する用量(400mg/kg)または致死用量(550mg/kg)のAPAPをマウスに腹腔内(IP)投与した。前述の説明または図面の凡例に示したように、様々な時点および様々な用量で抗IL11RA抗体(X209)またはIgGアイソタイプコントロール抗体をマウスに投与した。前述の説明または図面の凡例に示したように、APAPを投与してから10時間後~8日後の様々な時点でマウスを安楽死させた。
マウスのIl11遺伝子は5つのエキソンからなり、エキソン1に開始コドンATGを有し、エキソン5に終止コドンTGAを有する。マウスIl11では、3種の転写産物が同定されている(ENSMUSG00000004371)。そのうちの1つであるIl11-201は、最も長い転写産物であり、199アミノ酸長のプロペプチドをコードする。残りのIl11-202およびIl11-203は、選択的エキソン1を含み、シグナルペプチドを持たない140アミノ酸長の短いアイソフォームをコードすると予測されている。CRISPR/Cas9技術を使用して、コザック-ルシフェラーゼ-WPRE-ポリA配列をIl11-201(ENSMUST00000094892.11)のエキソン1に導入することにより、開始コドンATGを置換して、この特定の転写産物の翻訳を阻害した。エキソン1に認識部位を有する単鎖ガイドRNA(sgRNA)を、コザック-ルシフェラーゼ-WPRE-ポリA配列を含む標的構築物およびCas9とともに受精接合子に顕微注入し、偽妊娠マウスに移植した(Shanghai Model Organisms Center,Inc)。Il11遺伝子座へのルシフェラーゼカセットの挿入はシーケンシングにより確認した。C57BL/6を遺伝背景とするマウスを用いてIl11-ルシフェラーゼ変異体の子孫を作製し、野生型Il11アレルに対応する818bpの領域を増幅するプライマー(5’-GGAGGGAGGGGACGCCAATGACC-3’(配列番号22)および5’-TCTGCCTCCCCTGCCTGTTTCTCG-3’(配列番号23))、ならびにルシフェラーゼ構築物を含む標的アレルに対応する928bpの領域を増幅する第2のプライマーセット(5’-AATTCCGTGGTGTTGTCG-3’(配列番号24)および5’-TCTGCCTCCCCTGCCTGTTTCTCG-3’(配列番号25))を使用してジェノタイピングによる同定を実施し、エキソン1へのルシフェラーゼ構築物の挿入を検出した。
Cyagen Biosciences Inc.において、Il11遺伝子にEGFPを構成的にノックインしたトランスジェニックマウスを作製した。簡潔に述べると、エキソン5に挿入された2A-EGFPカセットで終止コドン配列TGAが置換されたノックインマウスを作製し、標的転写産物を翻訳することにより、全長IL11プロペプチドとEGFPの間に2A自己切断ペプチドリンカーが挿入された構築物を得た。C57BL/6ライブラリーから得たBACクローンを使用して、イントロン4に挿入されたNeoカセット(自己欠失アンカー部位(SDA)に挟まれている)およびエキソン5に挿入された2A-EGFPカセットを含むIl11遺伝子の相同アームを含む標的ベクターをPCRにより作製した。C57BL/6 ES細胞を遺伝子標的に使用して、標的が導入されたクローンをC57BL/6アルビノ胚に注入し、CD-1偽妊娠雌性マウスに再移植した。初代マウスを毛色で同定し、C57BL/6雌性マウスと交配し、その子孫をジェノタイピングにより分析して生殖系列移行を確認した。ジェノタイピング用プライマーは、イントロン4の選択された領域(NeoカセットのSDA部位に挟まれた領域)が増幅されるように設計した。プライマー配列は、5’-GAAATGAGAGCCTAGAGTCCAGAG-3’(配列番号26)および5’-GAGGCTTGGAAGAATGCACAATTA-3’(配列番号27)であった。
Creリコンビナーゼを用いた除去によりIl11が細胞種特異的に過剰発現されるように、過去の報告(15)に従って、loxP-Stop-loxP部位の制御下のRosa26遺伝子座にマウスIl11 cDNAを導入した。このマウスは、ジャクソン研究所(C57BL/6N-Gt(ROSA)26Sortm1(CAG-Il11)Cook/J)から入手可能である。肝細胞においてIl11の特異的発現を誘導するため、AAV8-ALB-Null(コントロール)またはAAV8-ALB-Cre(Il11-Tg)のPBS溶液を、マウス1匹あたり4×1011ゲノムコピーでヘテロ接合型Il11-Rosa26マウスに静脈内(IV)注射した(VectorBiolabs)。3週間後に肝臓および血清を評価した。
Il11ra1遺伝子のエキソン4~7がloxP部位に挟まれたIl11ra1-floxedマウスが最近になって作製され、その検証が行われたことから、Creリコンビナーゼを用いた除去によりIl11ra1を空間的かつ一時的に欠失させることが可能となった(Ng et al., Sci Transl Med. (2019) 11(511) pii: eaaw1237)。肝細胞特異的にIl11ra1を欠失させるため、ホモ接合型Il11ra1-floxedマウスに、AAV8-ALB-Creウイルス(マウス1匹あたりPBS中4×1011ゲノムコピー、VectorBiolabs)を尾静脈から静脈内注射した。コントロールとして、同等の量のAAV8-ALB-Nullウイルスをホモ接合型Il11ra1-floxedマウスに注射した。APAP誘発性肝障害の発症の3週間前に、AAV8でマウスを処置することによって肝障害が回復した。ノックダウン効率は、肝臓内のIL11RAのウエスタンブロットによって測定した。
ヒト初代肝細胞およびマウス初代肝細胞をそれぞれ37℃、5%CO2で増殖させ、維持した。増殖培地を2~3日ごとに交換し、標準的なトリプシン処理技術を用いて80%コンフルエントで細胞を継代した。すべての実験は、継代数の少ない細胞(P1~P3)を用いて行った。刺激した細胞は、刺激を与えなかったこと以外は同じ条件下で同じ時間にわたり増殖させた非刺激細胞と比較した。
ヒト肝細胞(5200、ScienCell)は、2%ウシ胎児血清および1%ペニシリン-ストレプトマイシンを添加した肝細胞培地(520、ScienCell)中で維持した。細胞を16時間血清飢餓状態にした後、前述した刺激または図面の凡例に示した刺激を無血清肝細胞培地中で24時間にわたりそれぞれ実施した。
マウス肝細胞(ABC-TC3928、AcceGen Biotech)は、1%ペニシリン-ストレプトマイシンを添加したマウス肝細胞培地(ABC-TM3928、AcceGen Biotech)中で維持した。前述の説明または図面の凡例に示したように、様々な処理条件で細胞を24時間刺激した。
トランスフェクションの16時間前に、ヒト初代肝細胞を6ウェルプレートに播種し、60~70%コンフルエントまで培養した。Lipofectamine RNAiMAXトランスフェクション試薬(13778150、サーモフィッシャー)を含むOptiMEM培地(31985070、サーモフィッシャー)中で37℃で24時間培養することにより、50nMのNOX4 siRNA(ON-TARGETplus SMARTpool siRNA、L-010194-00-0005、Dharmacon)またはコントロールsiRNA(D-001810-10-05、Dharmacon)を細胞にトランスフェクトした。トランスフェクトした細胞をrhIL11で24時間刺激した。ノックダウン効率は、NOX4のイムノブロットにより測定した。
FITC Annexin V/Dead Cell Apoptosis Kit(V13242、サーモフィッシャー)を製造業者の説明書に従って使用して、ヒト初代肝細胞(5×105個)を染色した。陽性細胞をフローサイトメーター(Fortessa、BDバイオサイエンス)で定量し、FlowJoバージョン7ソフトウェア(TreeStar)で分析した。
ALT活性アッセイキット(ab105134、Abcam)またはAST活性アッセイキット(ab105135、Abcam)を使用して、マウス血清中または肝細胞の培養上清中のアラニントランスアミナーゼ(ALT)濃度またはアスパラギン酸アミノトランスフェラーゼ(AST)濃度を測定した。肝臓中のグルタチオンスルフヒドリル(GSH)の測定は、グルタチオン比色定量検出キット(EIAGSHC、サーモフィッシャー)を使用して行った。比色アッセイはいずれも製造業者のプロトコルに従って行った。
製造業者のプロトコルに従ってマウスIL-11 DuoSet(DY418およびDY008、R&Dシステムズ)を使用してマウス血清中のIL-11濃度を定量し、製造業者のプロトコルに従ってヒトIL-11 Quantikine ELISAキット(D1100、R&Dシステムズ)を使用して肝細胞の培養上清中のIL-11濃度を定量した。
96ウェルプレートにマウスIL11RA(1μg/mlのPBS溶液)を(4℃で一晩)コーティングし、ブロッキング緩衝液(0.05%Tween20を含む1%BSAのPBS溶液)でブロッキングした。Lightning-Link Rapid Biotin type A kit(Expedeon)を製造業者の説明書に従って使用して、ビオチン化マウスIl11を調製した。rhIL11またはrmIl11をブロッキング緩衝液で2倍段階希釈(5μg/mlから開始)し、0.01μg/mlのビオチン化マウスIL11と混合した。コーティングした前記プレートに、ビオチン化マウスIL11とrhIL11またはrmIL11の混合物を添加し、室温で1時間インキュベートした。ストレプトアビジン-HRP(ブロッキング緩衝液で1:1000希釈)とTMB色原体溶液(002023、サーモフィッシャーサイエンティフィック)を添加して発色させた。
肝細胞の溶解物または肝組織の溶解物を使用してウエスタンブロットを行った。プロテアーゼ阻害剤およびホスファターゼ阻害剤(サーモフィッシャー)を含む放射免疫沈降アッセイ(RIPA)緩衝液中で肝細胞または肝組織をホモジナイズし、遠心分離して溶解物を清澄化した。ブラッドフォードアッセイ(バイオ・ラッド)によりタンパク質濃度を測定した。等量の各タンパク質溶解物をSDS-PAGEで分離し、PVDF膜に転写し、図に示した一次抗体を加えてイムノブロット分析を行った。ECL検出システム(Pierce)を使用して、適切な二次抗体でタンパク質を可視化した。
急速凍結した肝組織または肝細胞溶解物をTrizol(インビトロジェン)で処理し、RNeasyカラム(キアゲン)で精製して全RNAを抽出した。製造業者の説明書に従ってiScriptTM cDNA合成キット(バイオ・ラッド)を用いてcDNAを合成した。StepOnePlusTM(アプライドバイオシステムズ)を使用したTaqMan法(アプライドバイオシステムズ)またはfast SYBR Green法(キアゲン)によって、二連の試料に対して遺伝子発現解析を40サイクルで実施した。発現データはGAPDH mRNAの発現に対して正規化し、2-ΔΔCt法を用いてfold changeを算出した。特異的なTaqManプローブおよびSYBR Greenプライマーの配列は、所望により入手可能である。
表面プラズモン共鳴(SPR)測定は、BIAcore T200(GEヘルスケア)を用いて25℃で行った。緩衝液を使用前に脱気し、0.2μmのフィルターに通してフィルター滅菌した。標準的なアミンカップリング化学反応を使用して、rhIL11またはrmIl11をカルボキシメチル化デキストラン(CM5)センサーチップに固定化した。動態分析を行うため、rhIL11固定化表面、rmIl11固定化表面または基準物質固定化表面に対して、ヒトIL11RAまたはマウスIl11raの濃度系列(3.125nM~100nM)を40μl/分の流速で注入した。分析物はいずれも、1mg/mlのBSAを含むHBS-EP+緩衝液(BR100669、GEヘルスケア)に溶解した。会合を150秒間測定し、解離を200秒間測定した。各分析物の注入後、pH2.5のグリシンを30秒間注入して、5分間安定化させることによって表面を再生した。すべてのセンサーグラムをアライメントし、ダブルリファレンスを取った。親和定数および速度定数は、BIAevaluation v3.0ソフトウェア(GEヘルスケア)を使用して、補正したセンサーグラムをラングミュア型の1:1結合モデルにフィッティングさせることによって決定した。平衡結合定数(KD)は、結合速度定数の比率(kd/ka)から決定した。
ヘマトキシリン・エオシン(H&E)染色
肝臓を10%中性緩衝ホルマリン(NBF)で室温(RT)にて48時間固定し、脱水し、パラフィンブロックに包埋し、7μmに薄切した。標準的なプロトコルに従ってH&E染色で切片を染色し、光学顕微鏡で観察した。
肝臓を冷PBSですすぎ、糸くずの出ない紙で軽く叩いて乾燥させ、OCTコンパウンド(4583、Tissue-Tek(登録商標))中で凍結包埋した。OCTコンパウンドの凍結後、肝臓標本をアルミニウム箔で包み、-80℃で保存した。凍結包埋した肝臓から厚さ7μmの凍結切片を作製し(-20℃)、スライド上で1時間乾燥させた後、製造業者のプロトコルに従ってBaseclick社のEdU IV Imaging Kit 488L(BCK488-IV-IM-L)を使用してEdUを検出した。
前述(「EdU染色」の節)と同様に肝臓を処理し、凍結した。凍結肝組織を-20℃で7μmに薄切し、1時間(RT)乾燥させた。肝臓切片を冷アセトンで15分間固定した後、PBSで短時間洗浄し、0.1%Triton X-100(T8787、シグマ)で透徹し、2.5%正常ヤギ血清(S-1012、Vector Labs)で1時間(RT)ブロッキングした。GFP一次抗体(1:500)またはカスパーゼ3一次抗体(1:1000)とともに肝臓切片を一晩(4℃)インキュベートし、適切なAlexa Fluor 488/647二次抗体(1:250)とともに1時間(RT)インキュベートした。DAPIを使用して核染色し、蛍光顕微鏡(ライカ)で画像化した。
マウス血清試料(20μL)、標準物質およびQC試料を96ディープウェルプレートに移し、10μg/lのAPAP-D4重同位体標準物質50μLを添加(スパイク)した。0.1%ギ酸を含む氷冷アセトニトリル360μLで処理したプレートを混合し(1000rpm/分、10分間)、遠心分離(2270×g、50分間、4℃)した。96マイクロウェルプレートに上清140μLを慎重に移し、オートサンプラーに設置して、LC-MS/MSで分析した。次に、標準曲線による定量を行う前に、APAP-D4重同位体標準物質に対してイオンカウントを補正した。40℃に維持したPEEK被覆SeQuant(登録商標)ZIC(登録商標)-cHILIC HPLCカラム(3mm、100Å、100×2.1mm)(Merck Pte Ltd)を装着したAgilent 1290 Infinity II LCシステム(アジレント・テクノロジー)を用いた液体クロマトグラフィー(LC)より、前記バイオマーカーを分離した。有機溶媒として、0.1%ギ酸を含むアセトニトリル(溶媒A)を使用し、水性溶媒として、20mMギ酸アンモニウム(pH4.0)(溶媒B)を使用した。液体クロマトグラフィー(LC)の線形勾配は、バイナリポンプA(G7120A、アジレント・テクノロジー)を用いて、溶媒Bの割合を、0.4ml/分の流速において、0分で10%、9分で70%、11分で70%および11.1~11.5分で10%に設定した。その後、10%の溶媒Bでカラムを11.5分間平衡化した。Quick-Changeバルブヘッド(2ポジション/10ポート、1,300バール)(5067-4240、アジレント)を取り付けて、追加の高速ポンプとしてバイナリポンプBを使用することにより、交互にカラムを自動再生してサイクル時間を短縮した。バイナリポンプBにおける溶媒Bの割合は、0.3ml/分の流速で10%に維持した。エレクトロスプレーのイオン源を用いてポジティブイオン化モードまたはネガティブイオン化モードで動作するAgilent 6495 Triple Quadrupole MSシステム(G6495A、アジレント・テクノロジー)にLC溶離液を導入して、質量を検出した。エレクトロスプレーイオン化のイオン源の条件は、キャピラリー電圧:4.0kV、ノズル電圧:500V、iFunnelパラメータ:高圧RF/低圧RF:90V、ネブライザー圧力:60psi、ガス温度:290℃、シースガス温度:350℃、ネブライザー:35psi、およびシースガス流速:12L/分とした。APAPに使用した多重反応モニタリング(MRM)条件は、衝突エネルギー(CE)16eVおよび衝突加速電圧(CAV)5Vにおいて152.1→110であり、APAP-D4に使用した多重反応モニタリング(MRM)条件は、衝突エネルギー(CE)8eVおよび衝突加速電圧(CAV)5Vにおいて156→114であった。APAP-グルタチオンに使用した多重反応モニタリング(MRM)条件は、衝突エネルギー(CE)42eVおよび衝突加速電圧(CAV)5Vにおいて457.1→140であった。
薬物を投与していないマウスの血清にAPAPの希釈標準溶液またはAPAP-グルタチオンの希釈標準溶液を加えて9点の検量線をそれぞれ得た。APAPの最終濃度は、0.32mg/l、0.46mg/l、2.6mg/l、5.2mg/l、10.3mg/l、20.6mg/l、41.3mg/l、82.5mg/lおよび330mg/lとした(低濃度のQC試料:1.29mg/l;高濃度のQC試料:165mg/l)。APAP-グルタチオンの最終濃度は、0.244mg/l、0.49mg/l、0.98mg/l、1.95mg/l、3.91mg/l、7.81mg/l、15.6mg/l、62.5mg/l、125mg/lおよび250mg/lとした(低濃度のQC試料:1.95mg/l;高濃度のQC試料:31.3mg/l)。内部標準(IS)に対する各分析物のピーク面積比に対応する標準曲線は、APAPでは、重み付け1/x2の線形最小二乗回帰を使用し、APAP-グルタチオンでは、重み付け1/xの線形最小二乗回帰を使用し、線形回帰の決定係数(r2)は、APAPではr2=0.97807145であり、APAP-グルタチオンではr2=0.99655914であった。マウス血清試料を使用したアッセイの精度および正確度は、過去に報告されている方法で測定した(32)。
統計分析は、GraphPad Prismソフトウェア(バージョン6.07)を使用して行った。Dunnettの方法(1つの条件に対していくつかの実験群を比較する場合)、Tukeyの方法(1つの実験内でいくつかの条件を比較する場合)、またはSidakの方法(2つの異なる遺伝子型においていくつかの条件を比較する場合)を使用した多重検定を行ってP値を補正した。2つの異なる群を比較する際の2つのパラメータの分析は、二元配置分散分析により行った。生存曲線は、Gehan-Breslow-Wilcoxonの検定により分析した。統計学的有意差の基準はP<0.05とした。

IL11は、インターロイキン6(IL6)サイトカインファミリーのメンバーであり、IL6と同様に、細胞膜に結合したα受容体(IL11RA)と糖タンパク質130(gp130)に結合してシスシグナル伝達を行う。IL6は、肝機能に関連することが報告されており、複数の文献において概して有益な効果が示唆されている(Kleinら、2005;Kroyら、2010;Matthewsら、2010;Schmidt-ArrasおよびRose-John、2016;Wuestefeldら、2003)。しかし、IL6は、可溶性のIL6受容体(sIL6R)とも結合して、トランスシグナル伝達を行うことがあると考えられており、このことから、適切な調節ができないと考えられている(Schmidt-ArrasおよびRose-John、2016)。IL6と同様に、IL11でも、病原性のトランスシグナル伝達が起こる可能性があるが、これまでの実験結果では、腫瘍または生殖組織においてこのようなトランスシグナル伝達が見られたという証拠は報告されていない(Agtheら、2017;Balicら、2017)。
まず、ヒト初代肝細胞におけるIL6R、IL11RAおよびgp130の発現量をフローサイトメトリーで評価した。ヒト初代肝細胞の大部分において、IL11RAの堅牢な発現(92.6%)とgp130の堅牢な発現(91.9%)が観察されたが、IL6Rを発現していた肝細胞はごくわずかしか認められず(3.0%)、その発現量も低かった(図31Aおよび図32A)。この結果と一致して、RNA-seq分析およびRibo-seq分析から、肝細胞においてIL11RA転写産物およびgp130転写産物が高発現されており、活発に翻訳されていることが見出された。これとは対照的に、IL6R転写産物はほとんど観察されず、IL6Rの翻訳もほとんど検出されなかった(図31B~図31D、図32Bおよび図32C)。肝細胞を免疫蛍光染色したところ、Ribo-seqデータを裏付けるように、IL11RAの高発現が認められたが、IL6Rの発現は検出されなかった(図32D)。培養培地中でも有意な量のIL6Rは検出されなかった(検出限界をわずかに上回る量のIL6Rしか検出されなかった)。この結果から、IL6Rのシェディングの可能性は除外された(図32E)。以上のデータから、ヒト初代肝細胞ではIL6Rの発現量が非常に低いことが示され、このことから、ヒト初代肝細胞におけるIL6のシスシグナル伝達の役割は限定的であることが示唆された。一方で、ヒト初代肝細胞は、IL11RAおよびgp130の強い共発現を示す。

市販の組換えタンパク質:ヒトhyper IL6(IL6R:IL6融合タンパク質、8954-SR、R&Dシステムズ)、ヒト可溶性gp130 Fc(671-GP-100、R&Dシステムズ)、ヒトIL11RA(8895-MR-050、R&Dシステムズ)。
カスタム組換えタンパク質:ヒトIL11(UniProtKB:P 20809、Genscript)。トランスシグナル伝達複合体を模倣するヒトhyper IL11(IL11RA:IL11融合タンパク質)は、IL11RAの断片(1~317番目のアミノ酸残基;UniProtKB:Q14626)およびIL11(22~199番目のアミノ酸残基;UniProtKB:P20809)を、20アミノ酸長のリンカー(配列番号20;Schaferら、2017)とともに使用して構築した。
パラホルムアルデヒド(PFA、28908;サーモフィッシャー)、ホルボール12-ミリステート13-アセテート(PMA、P1585、シグマ)、Triton X-100(T8787、シグマ)、および4’,6-ジアミジノ-2-フェニルインドール(D1306;サーモフィッシャー)。
ヒト初代肝細胞(5200、ScienCell)は、2%ウシ胎児血清および1%ペニシリン-ストレプトマイシンを添加した肝細胞培地(520、ScienCell)中で37℃、5%CO2で維持した。本発明の方法において特に明記しない限り、肝細胞(P2~P3)を血清飢餓状態で一晩培養した後、本明細書および/または図面の凡例に概説したように、様々な用量の様々な組換えタンパク質で24時間刺激した。
10%FBSおよび0.05mM β-メルカプトエタノールを添加したRPMI 1640(A1049101、サーモフィッシャー)中でTHP-1細胞(ATCC)を培養した。次に、RPMI 1640において10ng/ml PMAで48時間刺激することにより、THP-1細胞の分化を誘導した。
細胞表面のIL11RA、IL6Rおよびgp130を分析するため、IL11RA抗体、IL6R抗体またはgp130抗体と、これらに対応するAlexa Fluor 488二次抗体で、ヒト初代肝細胞およびTHP-1細胞を染色した。また、アネキシンV-FITCとPIを使用したDead Cell Apoptosis Kit(V13242、サーモフィッシャー)でヒト初代肝細胞を染色して、細胞の死滅を分析した。次に、陽性細胞をフローサイトメーター(Fortessa、BDバイオサイエンス)で定量し、FlowJoバージョンXソフトウェア(TreeStar)で分析した。
染色の24時間前に、ヒト初代肝細胞を8ウェルのチャンバースライドに播種した(1.5×104個/ウェル)。細胞を4%PFAで20分間固定し、PBSで洗浄し、非特異的部位を5%BSAのPBS溶液で2時間ブロッキングした。IL11RA抗体、IL6R抗体、gp130抗体またはアルブミン抗体とともに細胞を一晩(4℃)インキュベートし、適切なAlexa Fluor 488二次抗体とともに細胞を1時間インキュベートした。チャンバースライドを暗所で乾燥させ、DAPIを含む封入剤5滴をスライドに載せ、15分間経過後に蛍光顕微鏡(ライカ)による画像化を行った。
RNA-seqライブラリーおよびRibo-Seqライブラリーの調製は、過去の報告(Chothaniら、2019)に従って行った。
RNeasyカラム(キアゲン)を使用して、ヒト肝細胞から全RNAを抽出した。Qubit RNA High-Sensitivity Assayキット(Life Technologies)を使用してRNAを定量した。次に、LabChip GX RNA Assay Reagent Kit(パーキンエルマー)を使用して求めたRIN値(RNA integrity number)に基づき、RNAの品質を評価した。製造業者の標準的な説明書に従ってTruSeq Stranded mRNAライブラリー調製キット(イルミナ)を使用して、転写産物量を測定した。
10cmの培養ディッシュで肝細胞を90%コンフルエントまで増殖させ、0.1mg/mLのシクロヘキシミドを添加した1mLの氷冷溶解緩衝液(TruSeq(登録商標)Ribo Profile Mammalian Kit(RPHMR12126、イルミナ社)の処方など)に溶解した。次に、溶解物をホモジナイズおよび清澄化し、製造業者の説明書に従ってTruseq Nuclease(イルミナ)を用いてフットプリント法を行った。イルミナ社のSephacryl S400カラム(GEヘルスケア)を使用してリボソームを精製し、標準的なフェノール・クロロホルム・イソアミルアルコール法を使用して、保護されたRNA断片を抽出した。リボソームRNAを除去した後(Mammalian RiboZero Magnetic Gold、イルミナ)、製造業者のプロトコルに従ってTruSeq(登録商標)Ribo Profile Mammalian Kitを使用して、フットプリント法により抽出されたRNAからシーケンシングライブラリーを調製した。最終的に得られたRNA-seqライブラリーおよびリボソームプロファイリングライブラリーは、製造業者のプロトコルに従って、StepOnePlusリアルタイムPCRシステム(アプライドバイオシステムズ)においてKAPAライブラリー定量キット(KAPA Biosystems)を使用することにより定量した。
生のシーケンシングデータをbcl2fastq V2.19.0.316でデマルチプレックスし、Trimmomatic(Bolgerら、2014)V0.36を用いてアダプターをトリミングして、トリミング後に20ntを超えたリードを保持した。Bowtie(Langmeadら、2009)を用いて、既知のmtRNA配列、rRNA配列およびtRNA配列(RNACentral(The RNAcentral Consortium、2017)、リリース5.0)に対してRibo-seqリードをアラインメントし、アラインメントされなかったリードのみを、リボソームで保護された断片(RPF)として保持した。さらに、STAR(Dobinら、2012)を用いて、ヒトゲノム(hg38)に対するアラインメントを行った。featureCounts(Liaoら、2014)を用いて、Ribo-seqのCDS(コード配列)領域およびRNA-seqのエキソン領域にユニークにマッピングされたリード(Ensemblデータベース、リリースGRCh38 v86)の遺伝子発現を定量した。TPMを算出し、箱ひげ図に可視化することで、IL11RA(ENSG00000137070)、IL6R(ENSG00000160712)およびgp130(ENSG00000134352)のベースラインにおける発現を比較した。IL11RA、IL6Rおよびgp130のRibo-seqリードおよびRNA-seqリードによるリードカバレッジを、鎖特異的アラインメントファイルを用いたGviz Rパッケージ(HahneおよびIvanek、2016)により可視化した。
細胞培養上清中のアラニンアミノトランスフェラーゼ(ALT)活性は、ALT活性アッセイキット(ab105134、Abcam)を製造業者のプロトコルに従って使用して測定した。
肝細胞から得た総タンパク質抽出物に対してウエスタンブロットを行った。プロテアーゼ阻害剤およびホスファターゼ阻害剤(Roche)を含むRIPA Lysis and Extraction Buffer(89901、サーモサイエンティフィック)中で肝細胞の溶解物をホモジナイズした。タンパク質溶解物をSDS-PAGEで分離し、PVDF膜に転写した。ECL検出システム(Pierce)を使用して、適切な二次抗体(抗ウサギHRP抗体または抗マウスHRP抗体)でタンパク質バンドを可視化した。
すべての統計分析は、GraphPad Prismソフトウェア(バージョン6.07)を使用して行った。1つの実験においていくつかの条件を比較する場合、Tukeyの方法により多重検定を行ってP値を補正した。統計学的有意差の基準はP<0.05に設定した。

- 肝毒性および/または肝毒性に関連する障害、疾患もしくは状態の治療方法または予防方法において使用するための、インターロイキン11(IL-11)媒介性シグナル伝達を抑制することができる薬剤。
- 肝毒性および/または肝毒性に関連する障害、疾患もしくは状態の治療方法または予防方法において使用するための医薬品の製造における、インターロイキン11(IL-11)媒介性シグナル伝達を抑制することができる薬剤の使用。
- 肝毒性および/または肝毒性に関連する障害、疾患もしくは状態を治療または予防する方法であって、インターロイキン11(IL-11)媒介性シグナル伝達を抑制することができる薬剤の治療有効量または予防有効量を対象に投与することを含む方法。
- 前記薬剤が、インターロイキン11受容体(IL-11R)へのインターロイキン11(IL-11)の結合を阻止または低減することができる薬剤である、請求項1に記載の薬剤、請求項2に記載の使用または請求項3に記載の方法。
- 前記薬剤が、インターロイキン11(IL-11)またはインターロイキン11受容体(IL-11R)に結合することができる薬剤である、請求項1もしくは4に記載の薬剤、請求項2もしくは4に記載の使用または請求項3もしくは4に記載の方法。
- 前記薬剤が、抗体またはその抗原結合断片、ポリペプチド、ペプチド、核酸、オリゴヌクレオチド、アプタマーおよび小分子からなる群から選択される、請求項5に記載の薬剤、使用または方法。
- 前記薬剤が、抗体またはその抗原結合断片である、請求項5または6に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11媒介性シグナル伝達に対する抗IL-11アンタゴニスト抗体またはその抗原結合断片である、請求項7に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11媒介性シグナル伝達に対する抗IL-11Rαアンタゴニスト抗体またはその抗原結合断片である、請求項7に記載の薬剤、使用または方法。
- 前記薬剤が、デコイ受容体である、請求項5または6に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11のデコイ受容体である、請求項10に記載の薬剤、使用または方法。
- 前記IL-11のデコイ受容体が、(i)gp130のサイトカイン結合モジュールに対応するアミノ酸配列と、(ii)IL-11Rαのサイトカイン結合モジュールに対応するアミノ酸配列とを含む、請求項11に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11のムテインである、請求項5または6に記載の薬剤、使用または方法。
- 前記IL-11のムテインが、W147A置換を有するムテインである、請求項13に記載の薬剤、使用または方法。
- 前記薬剤が、インターロイキン11(IL-11)またはインターロイキン11受容体(IL-11R)の発現を阻止または低減することができる薬剤である、請求項1に記載の薬剤、請求項2に記載の使用または請求項3に記載の方法。
- 前記薬剤が、オリゴヌクレオチドまたは小分子である、請求項15に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11の発現を阻止または低減することができるアンチセンスオリゴヌクレオチドである、請求項16に記載の薬剤、使用または方法。
- IL-11の発現を阻止または低減することができる前記アンチセンスオリゴヌクレオチドが、配列番号12、13、14または15の配列を含むIL-11標的siRNAである、請求項17に記載の薬剤、使用または方法。
- 前記薬剤が、IL-11Rαの発現を阻止または低減することができるアンチセンスオリゴヌクレオチドである、請求項16に記載の薬剤、使用または方法。
- IL-11Rαの発現を阻止または低減することができる前記アンチセンスオリゴヌクレオチドが、配列番号16、17、18または19の配列を含むIL-11Rα標的siRNAである、請求項19に記載の薬剤、使用または方法。
- 前記インターロイキン11受容体がIL-11Rαであるか、またはIL-11Rαを含む、請求項4~20のいずれか1項に記載の薬剤、使用または方法。
- 前記肝毒性に関連する障害、疾患または状態が、肝毒性が病理学的に関与する疾患である、請求項1および4~21のいずれか1項に記載の薬剤、請求項2および4~21のいずれか1項に記載の使用、または請求項3~21のいずれか1項に記載の方法。
- 前記肝毒性が病理学的に関与する疾患が、薬物誘発性肝障害(DILI)、急性肝障害(ALI)、急性肝不全、急性肝疾患、慢性肝疾患、肝損傷、肝炎、ウイルス性肝炎、アルコール性肝炎、肝虚血再灌流障害(IRI)、温虚血再灌流(WIR)、放射線誘発性肝疾患(RILD)、薬物誘発性特異体質性肝障害(IDILI)、自己免疫性肝障害、胆汁うっ滞性肝疾患、HIVおよびがんから選択される、請求項22に記載の薬剤、使用または方法。
- 前記肝毒性または前記肝毒性に関連する障害、疾患もしくは状態が、薬物誘発性肝障害(DILI)である、請求項1および4~23のいずれか1項に記載の薬剤、請求項2および4~23のいずれか1項に記載の使用、または請求項3~23のいずれか1項に記載の方法。
- 前記肝毒性または前記肝毒性に関連する障害、疾患もしくは状態が、アセトアミノフェン(APAP)誘発性肝毒性である、請求項1および4~24のいずれか1項に記載の薬剤、請求項2および4~24のいずれか1項に記載の使用、または請求項3~24のいずれか1項に記載の方法。
- APAP誘発性肝毒性を治療または予防する方法が、N-アセチルシステインで処置することをさらに含む、請求項25に記載の薬剤、使用または方法。
- 前記治療方法または前記予防方法が、インターロイキン11(IL-11)またはIL-11受容体(IL-11R)の発現がアップレギュレートされている対象に前記薬剤を投与することを含む、請求項1および4~26のいずれか1項に記載の薬剤、請求項2および4~26のいずれか1項に記載の使用、または請求項3~26のいずれか1項に記載の方法。
- 前記治療方法または前記予防方法が、インターロイキン11(IL-11)またはインターロイキン11受容体(IL-11R)の発現がアップレギュレートされていることが判明している対象に前記薬剤を投与することを含む、請求項1および4~27のいずれか1項に記載の薬剤、請求項2および4~27のいずれか1項に記載の使用、または請求項3~27のいずれか1項に記載の方法。
- 前記治療方法または前記予防方法が、
インターロイキン11(IL-11)またはIL-11受容体(IL-11R)の発現が対象においてアップレギュレートされているか否かを判定すること、および
インターロイキン11(IL-11)またはIL-11受容体(IL-11R)の発現がアップレギュレートされている対象に前記薬剤を投与すること
を含む、請求項1および4~28のいずれか1項に記載の薬剤、請求項2および4~28のいずれか1項に記載の使用、または請求項3~28のいずれか1項に記載の方法。
