Abstract
Description
インスリン様成長因子結合タンパク質は、インスリン様成長因子(IGF)の生体利用能をモジュレートする7つの結合タンパク質のファミリーである。それらの中でIGFBP3は最も豊富であり、ほぼすべての組織に存在し、IGFに対するより高い親和性を有する;実際、IGFのおよそ80~90%が、酸不安定サブユニット(ALS)との三元複合体でIGFBP3に結合している(1)。
1型(T1D)及び2型糖尿病(T2D)は両方とも、インスリンの分泌の低下、血中グルコースレベルを制御できないこと、及び高血糖をもたらす、ベータ細胞の喪失によって特徴付けられる(5、6)。異なる病因メカニズムにもかかわらず、T1Dにおける自己免疫応答及びT2Dにおけるインスリン抵抗性/炎症は両方とも、ベータ細胞量の進行性の低下につながる。実際、生じている免疫活性化がT1Dにおけるベータ細胞喪失を完全に説明するのに十分ではないらしいことが明らかになりつつある(5)。更には、免疫療法がT1Dを治癒することができないことにより(7)、(i)自己免疫が、T1D発病に関与する唯一の要因ではあり得ないこと、及び(ii)T1Dに対する有効な治療を確立するために、ベータ細胞喪失等、疾患の異なるメカニズムを標的にする代替ストラテジーが必要とされることが強調された。長年にわたるT1Dを有する個体において散在性ベータ細胞が検出されるという観察結果は(8)、ベータ細胞代謝回転を保つために新たなベータ細胞が生じているに違いないこと(5、9)、又は破壊されたベータ細胞が「異なり」得、死滅する傾向があり得る(10)ことを裏付ける。このことは、表面ベータ細胞受容体の上方/下方調節された発現が、それらを免疫系に見えるようにすることにおいて主要な役割を有し得ること、並びにより重要なことには、他の非免疫学的決定因子が、ベータ細胞の運命及び機能をモジュレートし得ることを示唆し得る。それ故、T1Dにおける非免疫学的ベータ細胞破壊、及びT2Dにおけるベータ細胞の進行性の喪失を阻止することは、ベータ細胞の生成と破壊の間のバランスを適当なベータ細胞量の回復に傾かせ得、故に疾患の本当に最初の段階を食い止め得る又は遅らせ得る新規の治療的手法のための道を開く。
腸管幹細胞(ISC)は、小腸及び大腸陰窩の底に存在し、陰窩の再生及び代謝回転を制御する。特に、ISCは陰窩に沿って分化して、杯細胞、腸細胞、腸内分泌細胞を生成し得る(4)。
- IGFBP3へのIGF-Iの結合を置き換えない、及び/又はIGFBP3へのIGF-Iの結合を妨げず、
- TMEM219受容体へのIGFBP3の結合を阻害する又は低下させる、単離された抗体又はその抗原結合フラグメントが提供される。

本抗体は、糖尿病、並びに腸管腸疾患(intestinal bowel disease)、吸収不良症候群、炎症性腸疾患、悪液質、クローン病、潰瘍性結腸炎、セリアック病、糖尿病性腸症等の様々な障害又は状態の治療の方法において使用される。
合親和性の増加、アンタゴニスト的若しくはアゴニスト的生物学的特性の改善若しくは増強(場合に応じて)、免疫原性の低下等、1つ又は複数の所望の特性について容易に試験され得る。この一般的様式で獲得された抗体及び抗原結合フラグメントの使用は、本発明に包含される。
患者及び調査設計
憩室症、結腸がん、過敏性腸症候群に対する結腸内視鏡検査又は腸管手術を受けた患者の中で、健常対象(対照)が登録された(n=10)。年齢:41.3±2.2(平均±SEM);性別(M/F):7/3。
組み換えヒトIGFBP3を、Life Technologies社(IGFBP3、Life Technologies社、10430H07H5)から入手した。エクトTMEM219を、Genescript社のカスタマイズされたタンパク質サービスを通じて入手した。大腸菌において産生されたタンパク質は、以下のアミノ酸配列

陰窩を、健常対象(健常対照)の腸管サンプルの粘膜若しくは粘膜下組織から摘出した、又は合併症(狭窄症、瘻孔)に対する手術を受けた確証されたクローン病を有する患者から獲得した。粘膜を、抗生物質ノルモシン(Normocin)[Invivogen社、San Diego、California 92121、USA ant-nr]、ゲンタマイシン[Invitrogen社、Carlsbad、CA、USA ant-gn]、及びファンギゾン[Invitrogen社15290018]の混合物と室温で15分間インキュベートし、次いで組織を小片に細かく刻み、PBS中10mMジチオトレイトール(DTT)(Sigma社)と数分間2~3回インキュベートした。次いで、サンプルをPBS中8mM EDTAに移し、37℃で30分間インキュベートした。この工程の後、サンプルの激しい振とうにより、結腸陰窩に富んだ上清がもたらされた。ウシ胎仔血清(FBS、Sigma社12103C-500ML)を5%の最終濃度まで添加し、単一細胞を2分間の40×gでの遠心分離によって取り出した。陰窩を50μlのマトリゲル(BD Biosciences社354234)と混合し、予熱した培養ディッシュにプレーティングした。凝固後、陰窩を、Glutamax、10mM(Life Technologies社35050038)、HEPES(Life Technologies社15630080)、N-2[1×](Life Technologies社17502048)、レチノイン酸なしのB-27[1×](Life Technologies社12587010)、10mMニコチンアミド(Sigma社N0636)、1mM N-アセチル-L-システイン(Sigma社A965)、50ng/mlヒトEGF(Life Technologies社PHG0311)、1μg/ml RSPO1(Sino Biological社11083-H08H)、100ng/mlヒトノギン(Peprotech社12010C)、1μg/mlガストリン(Sigma-Aldrich社SCP0152)、500nM LY2157299(Axon MedChem社1491)、10μM SB202190(Sigma社S7067)、及び0.01μM PGE2(Sigma社P6532)を補充した、完全陰窩培養培地:Wnt3a馴化培地及びAdvanced DMEM/F12(Life Technologies社1263010)50:50で覆った。単離された陰窩を、組み換えタンパク質及び介入調査の節に記載される組み換えタンパク質/抗体のあり/なしで8日間培養した。8日後、陰窩を回収し、形態、ミニ消化管成長、腸管シグネチャーマーカー(EphB2、LGR5、h-TERT)及びカスパーゼ8(Life Technologies社)の発現を、RT-PCRを使用して調べた。少なくとも1つの陰窩ドメインを有する発達したミニ消化管のパーセンテージを、既に記載されているように(4、18)査定した。
陰窩を、健常対象生検サンプルから単離し、以前に記載されているように培養して、ミニ消化管を作出した。病態を模倣するために、普通のFBSの代わりに、糖尿病性腸症(DE)を有する個体のヒト血清を添加することによって陰窩培養培地を改変し、つまり「DE」培地、L-Wnt3細胞を10%「DE」血清中で成長させて馴化培地を作出し、それを、Advanced DMEM/F12培地に50:50で更に添加して、上で記載される陰窩培養培地を獲得した(陰窩単離及びミニ消化管発達を参照されたい)。8日後、陰窩を回収し、形態、ミニ消化管成長、腸管シグネチャーマーカー(EphB2、LGR5、h-TERT)及びカスパーゼ8(Life Technologies社)の発現を、RT-PCRを使用して調べた。これは、腸管幹細胞損傷のモデルとして確立されている(4)。
精製された腸管陰窩由来のRNAを、Trizol Reagent(Invitrogen社)を使用して抽出し、TaqManアッセイ(Life Technologies社、Grand Island、NY)をメーカーの取扱説明書に従って使用して、qRT-PCR分析を実施した。正規化された発現値を、ΔΔCt又はΔCt法を使用して決定した。定量逆転写酵素ポリメラーゼ連鎖反応(qRT-PCR)データをACTBの発現に対して正規化し、ΔCt値を算出した。分析は、技術的及び生物学的な三つ組で実施された。
以下の試薬を使用して、新たに作出された抗IGFBP3抗体をスクリーニングした:組み換えヒトIGFBP3(0,223mg/ml R&D System社8874-B3-025)、エクトTMEM219(0,5mg/ml GenScript社)、新たに作出された抗IGFBP3 mAb(Yumab社、契約下)、ヤギポリクローナル抗IGFBP3(LS-C149975 LSBio社)、マウスモノクローナル抗IGFBP3(LS-C45037 LSBio社)、マウス6×HisタグHRP(GTX30506 Genetex社)、ウシ血清アルブミン(BSA A9418 Sigma社)、Tween 20(TW P2287 Sigma社)、ELISA比色TMB試薬(HRP基質、Item H Sigma社、RABTMB3)、ELISA STOP溶液(Item I、Sigma社、RABSTOP3)、ブロッキング試薬液(PBS中3%BSA)、及び希釈液(PBS中0,5%BSA、0,05%Tw)。
エピトープビニングとは、標的タンパク質に対するモノクローナル抗体のライブラリーを特徴付けし、次いで選別するために使用される競合的イムノアッセイである(Abdiche, Y. N.;D. S. Malashock;A. Pinkerton;J. Pons(2009年3月)、「Exploring blocking assays using Octet, ProteOn, and Biacore biosensors」、Analytical Biochemistry、386 (2): 172~180、doi:10.1016/j.ab.2008.11.038、PMID 19111520)。同様の標的に対する抗体を、ライブラリーにおける他のすべての抗体に対して一対形式で試験して、抗体が抗原のエピトープへの互いの結合を遮断するかどうかを見る。各抗体が、ライブラリーにおける他の抗体のすべてに対して創出されたプロファイルを有した後、ライブラリーにおける他のものと比べて、競合的遮断プロファイルが各抗体に対して創出される。密接に関係したビニングプロファイルは、抗体が同じ又は密接に関係したエピトープを有し、一緒に「まとめられる(binned)」ことを示す。エピトープビニングは、エピトープマッピング及びエピトープ特徴付け等の種々の名前の下で文献において参照される(Brooks, B.D. (2014)、「The Importance of Epitope Binning in Drug Discovery」、Current Drug Discovery Technology、11)。
ヒトベータ細胞株であるBetalox-5細胞(36)を、DMEM(グルコース1g/L)、BSA画分V(0.02% wt/vol)、非必須アミノ酸(1×)ペニシリン(100単位/mL)、及びストレプトマイシン(100μg/mL)を含有する培養フラスコ中で成長させた。細胞を、5%CO2の加湿インキュベーターにおいて37℃で培養した。細胞を2週間ごとに1回継代した。ベータ細胞を、IGFBP3の有無で、エクトTMEM219の有無で、新たに作出されたモノクローナル抗体の有無で培養し(組み換えタンパク質及び介入調査を参照されたい)、免疫蛍光調査、RNA抽出、アポトーシス検出、及びタンパク質分析のために細胞を回収した。インスリンの査定のために上清を回収した。インスリンレベルを微粒子酵素イムノアッセイ(Mercodia社Iso-Insulin ELISA、10-1113-01)でアッセイした。
データを平均及び標準偏差(SD)として提示し、正規分布についてはコルモゴロフ-スミルノフ検定で、及び等分散性についてはルビーン検定で検定した。差の統計的有意性を、両側t検定及びカイ二乗(χ2)検定で検定した。2つの群間の有意性を、対応のない両側スチューデントt検定によって判定した。多重比較に関しては、ボンフェローニ補正を有する一元配置ANOVA検定を採用した。統計分析を、GraphPad Prismバージョン5.0(GraphPad Software社、La Jolla、CA)を使用して行った。すべての統計的検定を5%の有意水準で実施した。
モノクローナル抗体開発
スクリーニングのための抗原として組み換え全長ヒトIGFBP3(R&D社カタログ番号8874-B3)を使用して、モノクローナル抗IGFBP3抗体をナイーブヒトファージディスプレイライブラリーから発見した。簡潔には、ヒトIGFBP3(R&D社カタログ番号8874-B3)をビオチン化し、96ウェルELISAプレートにストレプトアビジンを介して4℃で固定化した。洗浄及びBSAを用いたウェルのブロッキングの後、抗体-ファージライブラリーを添加した。ストレプトアビジンに結合する試験したライブラリーを、抗体-ファージからまず除いた(John deKruif及びTon Logtenberg、Phage Display of Antibodies、Encyclopedia of Immunology(第2版) 1998、1931~1934頁)。抗原特異的抗体を担持したそうしたファージがプレート表面に捕捉された。結合していない/弱く結合しているファージのPBS-Tを用いた洗浄による除去の後、抗原特異的ファージを溶出し、大腸菌におけるファージ感染によって増幅した。この増幅されたライブラリー部分集合を、よりストリンジェントな条件下で標的結合について再度選択した。合計で3回の選択ラウンドを実施して、抗原特異的抗体-ファージを富化した。
市販の抗IGFBP3モノクローナル抗体は、IGFBP3/TMEM219結合に影響を及ぼさない
市販の抗IGFBP3抗体を、IGFBP3へのエクトTMEM219の結合を置き換えるその能力について試験した。結合の置き換えは、(i)天然TMEM219受容体へのIGFBP3の結合を阻害する抗体の能力、及び(ii)エクトTMEM219リガンドトラップ融合タンパク質の中和活性を模倣する抗体の能力、の両方を示すであろう。
モノクローナル及びポリクローナルの両方の市販の抗体が、TMEM219へのIGFBP3の結合を阻止することにより腸管幹細胞(ISC)傷害病状においてミニ消化管成長をレスキューするエクトTMEM219リガンドトラップの能力を模倣し得るかどうかを査定するために、それらをミニ消化管アッセイにおいてインビトロで更に試験した。ミニ消化管を、既に記載されているように(4、18)、健常対照から作出した。図2における最も左の3本の棒に示されるように、糖尿病性腸症を有する患者からの培地(「DE培地」)の存在下でのインキュベーションは、健常個体からの培地(「健常培地」)におけるインキュベーションと比較して、ミニ消化管の形成を有意に低下させ;130ng/mlのエクトTMEM219の更なる添加は、DE培地の存在下でのミニ消化管成長を回復させた。
新規の抗IGFBP3 mAbは、IGFBP3-TMEM219結合を阻害する
実施例1に記述されるように、一連の抗IGFBP3モノクローナル抗体をファージディスプレイによって選択した。次いで、これら新規の抗体を、競合ELISA結合アッセイを使用して、IGFBP3への結合をエクトTMEMと競合するそれらの能力についてスクリーニングした。IGFBP3(コーティング)、エクトTMEM219、及びファージディスプレイによって選択された抗IGFBP3抗体を、すべて1:1のモル比で使用した。図5に示されるように、それらのうちの2つはより低い程度ではあるものの、新規の抗体はIGFBP3-エクトTMEM219結合を阻害し得た。このアッセイにおいて、市販のモノクローナル抗IGFBP3抗体は、IGFBP3-エクトTMEM219結合を阻害するのが不十分であった。
11種の新たに発見されたモノクローナル抗体を、ミニ消化管アッセイにおいても試験した。ミニ消化管を、健常対照から獲得された陰窩から作出し、IGFBP3の存在下で8日間培養し、エクトTMEM219又は新たに発見された抗IGFBP3 mAbのいずれかで処理した。すべての因子は、培養の初日の後に、1:1比(mAb/エクトTMEM219:IGFBP3)で含まれた。
IGFBP3曝露時にミニ消化管発達を促進するのに有効であると示された新たに発見された抗IGFBP3 mAbは、IGFBP3のみで処理されたサンプルと比較して少なくとも50%、IGFBP3の存在下でISCマーカーのEphB2及びLGR5の発現を有意に上方調節し得た(図8)。この結果は、エクトTMEM219に関して観察されたものと類似している。
ISCに対するIGFBP3による有害な影響は、カスパーゼ8媒介性である。興味深いことには、新たに発見された抗IGFBP3 mAbは、IGFBP3のみで処理されたサンプルと比較した場合、IGFBP3処理によって誘導されるカスパーゼ-8上方調節を少なくとも50%阻害し得た(図9)。
新たに発見されたモノクローナル抗IGFBP3抗体がTMEM219発現腸管幹細胞に対するIGFBP3の有害な影響を阻止することを確認するために、本発明者らは、DEの確立されたモデルにおけるミニ消化管アッセイにおいてそれらをインビトロで更に試験した。
エピトープビニング実験を実施して、商業的抗体LSBIO社LS-C45037(クローン83.8F9)及び新たに開発された抗IGFBP3抗体が、IGFBP3の同じエピトープ領域に結合するかどうかをチェックした。簡潔には、マルチウェルプレートを先行技術抗体でコートして、サンドイッチELISAを実施した。ファージディスプレイによって選択された新規の抗IGFBP3モノクローナル抗体のすべては、商業的抗体LSBIO社LS-C45037(クローン83.8F9)の存在下でIGFBP3に結合し得た。
新たに作出されたモノクローナル抗IGFBP3抗体は、IGF-I-IGFBP3結合を置き換えない
競合ELISA結合アッセイ
新たに作出された抗IGFBP3 mAbを、競合ELISAを使用することによって、IGF-I-IGFBP3相互作用を置き換えるそれらの能力についてインビトロで試験した。簡潔には、プレートをIGFBP3(Life Technologies社)4μg/mlでコートした。IGF-I(R&D社、291-G1-01M)を、IGFBP3との用量依存的比率(0:1、0.5:1、及び2:1)で添加した。本発明の新たに作出された抗IGFBP3 mAb(Yumab社)を、IGFBP3と比較して1:1の濃度で添加した。抗IGF-I HRPコンジュゲート抗体を使用することによって、IGFBP3-IGF-I結合を明らかにした。
陰窩単離及びマウスミニ消化管発達
本発明の抗体が、ヒト組織に対して、マウス組織において類似した組織交差反応性プロファイルを有することを確認するために、本発明者らは、マウス陰窩におけるインビトロミニ消化管アッセイにおいて、本発明のモノクローナル抗IGFBP3抗体を更に試験した。陰窩を、C57BL/6Jマウス(632C57BL/6J Charles River Laboratories社、Lyon、France)から獲得した。簡潔には、結腸を2~4mm片にはさみで切り、フラグメントを30mlの氷冷PBS中で洗浄し、次いで20mM EDTA-PBSとともに37℃でインキュベートした。次いで、フラグメントをトリプシン/DNAse溶液で処理して、陰窩を獲得した。この工程の後、サンプルの激しい振とうにより、結腸陰窩に富んだ上清がもたらされた。陰窩をマトリゲルと混合し、予熱した培養ディッシュにプレーティングした。マトリゲルの凝固後(37℃で10~15分間)、陰窩を、培養培地(ADF、10mM HEPES、N-2、レチノイン酸なしのB27、10μM Y-27632、1μM JAG1ペプチド(Anaspec社、Fremont、CA、USA)、1μg/ml R-スポンジン1、50ng/ml EGF(Invitrogen社)、及び100ng/mlノギン(Peprotech社、Rocky Hill、NJ、USA))で覆い、培地を8日目まで1日おきに変えた。
Octet BLIに基づく分析
結合親和性をより詳細に査定するために、バイオレイヤー干渉法(Biolayer Interferometry)(BLI)プラットフォームであるOctet機器(BLItz System)によって実施される生体分子相互作用分析を使用して、親和性測定を実施した。アッセイを確立するために、標的モノクローナル抗体(1%BSA-PBST中30μg/ml)をAHCバイオセンサー上にFcを介して固定化し、種々の濃度の抗原ヒトIGFBP3(R&D社、カタログ番号675 B3)及びマウスIGFBP3(Creative Biomart社、カタログ番号igfbp3 829M)との相互作用を測定した。
C57BL/6Jマウスにおいてデキストラン硫酸ナトリウム(DSS)によって誘導される結腸炎のモデルは、IBDにおける薬物の抗炎症特性を評価し及び確認もするための有効な動物モデルである。DSS(飲用水での経口投与)は、顕著な下痢、それに続く炎症を誘導する。このモデルは十分に特徴付けされ、信頼性が高く、再現性があり、死亡がないことをIBDにおける規制当局によって認められている。この調査をC57BL/6Jマウスにおいて実施した。実際、この特定の遺伝的背景において、マウスは、最後のDSS投与の3日後に分析された場合には急性結腸炎を、又は最後のDSS投与の7日後に分析された場合には慢性様炎症を発症する。
雄C57BL/6Jマウスは、Charles River laboratories社、l'Arbresle、Franceによって供給された。マウスを20±5℃で飼育し、水及び食料を自由裁量で提供した。すべての実験プロトコールは、政府ガイドラインに従って、リールのパスツール研究所における認定施設で実施された。
飲用水に溶解した2.5%(w/v)DSS(45kD;TDB Consultancy AB社、Uppsala、Sweden、バッチ番号DB001-41)をマウスに5日間給餌することによって、急性結腸炎を誘導した。マウスを8つの群にランダムに分けた:対照群; DSS+ビヒクル;DSS+Yu139-CO1 0.6mg/マウス、DSS+エクトTMEM 0.1mg/マウス、DSS+マウス抗TNFα 0.1mg/マウス(Ozyme社、B270358)、DSS+ペントキシフィリン(Sigma社P1784、BCBQ05 15)、DSS+ヒュミラ(Abbvie社、1108722)ip、DSS+ヒュミラsc。C57BL/6JマウスにおけるDSSにより誘導される急性結腸炎に対する抗IGFBP3 mAbの予防的治療効果を査定するために、マウスを、結腸炎誘導の3日前に開始する表示される用量のYu139-CO1を用いた腹腔内投与によって毎日処理し、最後のDSS投与の7日後に生じる安楽死まで実施した。動物モデルの実験スケジュールは、図15Aに記載されている。
すべての群において、マウスの重量、便の硬さ、及び血便を毎日記録した。疾患活動性指標(DAI)スコアは、標準的採点システムに従って、体重、便の硬さ、及び潜血の変化に基づいた。これらのパラメーターを、Table 10(表10)に記載される尺度で査定した。DAIデータは、毎日取られたこれらのパラメーターの平均スコアとして提示されている。動物を、麻酔下での頸椎脱臼によって屠殺した。安楽死時に、結腸を慎重に切除し、その重量及びサイズを測定した。安楽死時に、潜血(OB)の存在を、潜血法を使用して記録する。
2つの独立したサンプルに対する並べ替え検定を使用して、すべての比較を分析した。統計値は、GraphPad Prismバージョン7.0(GraphPad Software社、San Diego、CA)を使用して算出されている。p値が<0.05である場合、差を統計的に有意であると見なした。
動物
雌非肥満糖尿病(NOD)マウス(10週齢)をJackson Laboratory社、Bar Harbor、Maine(JAXストック#001976)から入手した。すべてのマウスを、Jackson Laboratory社施設内動物実験委員会によって承認された施設ガイドラインに従って世話し及び使用した。
明らかな糖尿病(空腹時血中グルコース濃度の上昇及び古典的症状によって特徴付けられる最も進行した段階)を、300mg/dLを上回る血中グルコースレベルとして規定した。血中グルコースを、Breeze2(Bayer S.p.A社)血中グルコース計測器を使用して測定した。
データを平均及び標準偏差(SD)として提示し、正規分布についてはコルモゴロフ-スミルノフ検定で、及び等分散性についてはルビーン検定で検定した。差の統計的有意性を、両側t検定及びカイ二乗(χ2)検定で検定した。2つの群間の有意性を、対応のない両側スチューデントt検定によって判定した。多重比較に関しては、ボンフェローニ補正を有する一元配置ANOVA検定を採用した。異なる群間での糖尿病発生率を、ログ-ランク(マンテル-コックス)検定で分析した。統計分析を、GraphPad Prismバージョン7.0(GraphPad Software社、La Jolla、CA)を使用して行った。すべての統計的検定を5%の有意水準で実施した。
