Abstract
Description
本出願は2018年10月3日に出願された米国特許仮出願第62/740,840号の出願日の利益を主張するものであり、その内容全体が参照により本明細書に取り込まれる。
本開示の1つの態様は、低用量及び/又は複数用量のVEGFポリペプチドと薬剤(例えば診断薬又は治療薬)の併用を含む脳疾患の治療方法を特徴とする。VEGFポリペプチドは低用量での治療を必要とする対象に対して全身投与可能であり、続いてVEGFポリペプチドの投与後の適切な時間枠内に薬剤が投与される。場合によっては、VEGFポリペプチドは薬剤の投与後の適切な時間枠内に対象に対して1回又は複数回投与されてもよい。
血管内皮細胞増殖因子(VEGF)は、脈管形成及び血管形成を刺激する細胞によって産生されるシグナルタンパク質であり、血小板由来増殖因子サブファミリーに属する増殖因子である。VEGFの通常機能は胚発生時の新血管形成、傷害後の新血管形成、運動後の筋肉形成、及び閉塞した血管を迂回するための新血管(側副循環)の形成である。血管内皮細胞増殖因子(VEGF)は、正常な新血管形成、並びに細胞の増殖及び生存の促進の原因となる可溶性ホモ二量体タンパク質である。
本明細書に開示される方法は、BBBを越えた脳への薬剤の送達を促進することを目的としており、それにより薬剤は意図されている活性を及ぼすことができる。いくつかの例では、薬剤は脳障害、例えば脳腫瘍を治療するための治療薬であってもよい。他の例では、薬剤は脳の状態を診断するための診断薬、例えばイメージング剤であってもよい。
本明細書に記載される方法において使用される活性薬剤(例えばVEGF、診断薬、及び/又は治療薬)はいずれも、本明細書に開示される方法のいずれかにおいて使用される医薬組成物を形成するために、緩衝剤を含む薬学的に許容可能な担体(非薬効成分(excipient))と混合され得る。「許容可能な」は、その担体が組成物の有効成分と適合可能でなくてはならず(及び好ましくは有効成分を安定化することができ)、且つ、治療対象に対して有害ではないことを意味する。緩衝剤を含む薬学的に許容可能な非薬効成分(担体)は当技術分野においてよく知られている。例えば、Remington:The Science and Practice of Pharmacy 20th Ed.(2000) Lippincott Williams and Wilkins,Ed.K.E.Hooverを参照されたい。
本明細書に開示されるVEGFポリペプチドはいずれも、例えばVEGF165AなどのVEGF-A(及び他の増殖因子)は、BBBを越えた脳への薬剤の送達を増加させるために、本明細書に同様に開示される任意の薬剤(例えば治療薬又は診断薬)と併用することができる。処理を必要とする対象に対して低用量のVEGFポリペプチドを最初に投与し、VEGFの投与後の適切な時間枠内に適切な用量の薬剤を適切な経路によって対象に投与することができる。いくつかの例では、薬剤の投与後の適切な時間内にVEGFポリペプチドの1回又は複数回の追加用量が対象に投与される場合がある。VEGFの2連続の用量が適切な時間内に、例えば約2~24時間を空けて、対象に体系的に投与されてもよい。
本開示は、脳疾患の治療又は診断のために本明細書に記載される方法において使用されるキットも提示する。このようなキットは少なくとも2つの容器を含むことができ、VEGFを含む第1の製剤を含む容器、及び本明細書に記載されるような治療薬(例えば抗がん剤)又は本明細書に記載されるような診断薬(例えばイメージング剤)を含む第2製剤を含む第2の製剤を含む。いくつかの例では、キットは、VEGFを含む第3製剤を含む第3の製剤を含み、第3の製剤は第2の製剤の投与の2~4時間後に処理を必要とする対象に全身投与されるものであってもよい。

本研究は、血液脳関門(BBB)の透過性を高めることによる脳への治療薬の送達の促進における低用量のヒト血管内皮細胞増殖因子A(VEGF-A)の効果を試験する。本明細書では、低用量の全身投与されたヒトVEGFポリペプチドにより、マウス及びブタの両方で短時間のBBB透過性の上昇が誘導されたことが報告された。VEGFは、様々な大きさのナノ粒子、小分子、及びリポソーム化学療法薬を含む様々な例示的分子の脳送達を増加させることがわかった。これらの結果はVEGFによって誘導されるBBB透過性の時間枠は一過性であることを示しており、正常なBBB完全性はマウスモデルにおいて観察されたようにVEGFの投与後4時間以内に回復した。VEGFとリポソーム化ドキソルビシンを組み合わせることでヒト神経膠芽腫のマウスモデルの動物生存期間が予想外に延長された。驚くべきことに、VEGFがマウスに投与された場合、全身にわたる毒性は認められなかった。
(i)動物実験
薬剤生体内分布試験に使用された動物は、体重約25gの8~10週齢の雄フレンド白血病ウイルスB(FVB)マウスであった。約21gの6~8週齢の雄BALB/c NUマウスをヒトGBM腫瘍異種移植実験に使用した。PDAC腫瘍異種移植のためには体重25~30gの8週齢のNOD/SCIDマウスを使用し、機構研究及び安全性試験には約30gの8~10週齢の雌ICRマウスを使用した。全てのマウスを台湾のBioLasco社から購入した。マウスを自由給水給餌で12時間毎の昼夜サイクルで飼育した。大型動物試験のために19~24kgのLanyuミニブタを使用した。全てのマウス実験は台湾中央研究院動物実験委員会(IACUC)によって認可され、ブタは台湾国家実験動物センターの認可済みプロトコルに従って獣医スタッフの監督下で使用された。
マウスでは別段の定めが無い限り30Gのインスリン注射針を使用して尾静脈よりボーラス注射として薬剤を投与した。組換えヒトVEGF165A(Peprotech社、台湾)を0.1%(重量/体積)ウシ血清アルブミン中に懸濁し、別段の定めが無い限り体重1g当たり1.5ngの用量で外側尾静脈から投与した。エバンスブルー(Sigma社、E2129)を通常生理食塩水中に4%(重量/体積)の濃度で懸濁し、4ml/kgの用量で投与した。生理食塩水中に懸濁された塩酸ドキソルビシン(Sigma社、D1515)、又はLipoDox(TTY Bio社、台湾)を外側尾静脈により2~8mg/kgの用量でゆっくりと投与した。テモゾロミド(Sigma社、T2577)を5mg/kg又は20mg/kgの用量で投与した。20nm、100nm、及び500nmのソリッドコア直径を有する蛍光PEG結合黄緑色ポリスチレン微粒子(Life Technologies、Thermo Fisher社)を3mg/kgの用量で投与した。神経炎症の誘導のために使用したリポ多糖(Sigma社、L4391)を、5mg/kgの用量で投与した。ブタではrhVEGF165A(0.2μg/kg、2μg/ml)又はベヒクル対照を右総頸動脈に注入した。5%(重量/体積)ブドウ糖溶液中に0.35mg/mlの濃度に希釈されたLipoDox(TTY Bio社、台湾)を約3.0ml/分の速度のシリンジポンプによる静脈内点滴によって1.5mg/kgの用量で投与した。黄緑色PEG結合ポリスチレンナノ粒子(100nmのコア径)を3mg/kgの用量でボーラス注入により投与した。
以前に記載されたように、mPEGアミン(5kD、Nanocs社、台湾)及びカルボジイミド(Sigma社)を使用して蛍光ナノ粒子をPEG化し、Malvern ZetaSizer ZSを使用してこれらのナノ粒子の特徴を分析した。Lundy et al.,Sci.Rep.,6:25613(2016)。
マウスについては技術員がPharmascan 7T 16cmボア水平システムを操作した。イソフルラン吸入によって麻酔されたFVBマウスにVEGF又は同体積のベヒクル対照を注入した。造影前T1強調スピンエコー画像を撮影した(反復時間(TR)=400ミリ秒、エコー時間(TE)=10.8ミリ秒、視野(FOV)=2×2cm、励起回数(NEX)=8、スライス厚0.8mm)。VEGF投与の45分後又は4時間後に造影剤(Gadovist、Bayer社)をカテーテル挿入された尾静脈に0.2mmol/kgの割合で投与した。造影剤注入の1分後に造影後T1強調画像を撮影した。ROIのシグナル(平均ピクセル強度)をバックグラウンドノイズの標準偏差によって除算することによってSN比を計算した。画像撮影、SN比測定、及び腫瘍容積測定は全て2人のMRI操作技術員により実施あれ、これらの技術員には試験群が伏せられた。
30分の循環後に腹部大動脈から50mlのリン酸緩衝生理食塩水(PBS)を灌流した。臓器を取り出し、複数の小片に分割し、リンスし、乾燥し、重量測定し、500μlのホルムアミド中でホモジナイズし、60℃で一晩加熱し、21,000gで15分間遠心分離した。上清の吸光度を620nmで測定し、740nmでバックグラウンド補正を行い、検量線により未知濃度を定量した。エバンスブルー注射を受けなかったマウスの臓器を使用してブランク吸光度読み取り値を確定した。Radu et al.,J.Vis.Exp.,No.73,e50062(2013)。
マウスではナノ粒子を30分間循環させた後、上記のように動物に対して灌流を行った。IVISを使用してナノ粒子の保持量を定量した(励起485nm、蛍光530nm)。ナノ粒子投与を受けなかったマウスの脳を使用してバックグラウンド補正を行った。ブタではHPLCを使用してナノ粒子の保持量を定量した。Chen et al.,Nanoscale,7(38),15863-15872(2015)。簡単に説明すると、ナノ粒子の蛍光染料をo-キシレン中に抽出し、Waters e2695分離モジュール及びX-bridge C18(250×4.6mm、5μm)カラムをメタノール:水(77:23)の移動相、流速1ml/分で使用して定量した。検出には、Waters 2475 FLR検出器を505nmの励起及び515nmの蛍光で使用した。
約100mgの組織を酸性化酢酸アンモニウム(200μl、10mM、pH3.5)、硫酸亜鉛(200μl、100mM)、及びメタノール(400μl)中でホモジナイズし、続いて4℃で30分間、10,000gで遠心分離した。上清をHPLC分析のために採取した。80:20の比率の酢酸(0.1%体積/体積)とメタノールを使用して、35℃で、Atlantis T3 3μm HPLCカラム内、0.8ml/分の流速で、Waters e2695分離モジュールにより分離を実施した。Waters 2489 UV/可視光検出器を316nmで使用して検出を実施した。テオフィリンを内部標準として使用し、275nmでこれを測定して、結果をテオフィリンピークに対するTMZピークの比率として計算した。図8bに示されているように、溶解緩衝液中に溶解されたTMZの検量線から未知濃度を計算した。
臓器を取り出し、乾燥し、重量測定し、複数の小片(約100mgの組織又は100μlの血漿)に分割した後、1mlの溶解緩衝液(0.25M ショ糖、5mM Tris-HCl、1mM MgSO4、1mM CaCl2、pH6.7)を使用して完全にホモジナイズした。次に、200μlのホモジネートを1mlの酸性化アルコール(70%エタノール、0.3N HCl)と混合し、-20℃で一晩放置した後、4℃で30分間、10,000gで遠心分離した。上清をHPLC分析のために採取した。Waters e2695分離モジュール、40℃で、X-Bridge 5μmカラム内、35%の10mM KH2PO4と65%のメタノールの移動相、1ml/分の流速より分離を行った。検出には、Waters e2475モジュール(励起480、蛍光600nm)を使用した。
本試験ではDBTRG-05MGヒト神経膠芽腫細胞を使用した。ルシフェラーゼ発現の証拠が図13aに示されている。10%FBS、1mMピルビン酸ナトリウム、及び1%ペニシリン/ストレプトマイシンを添加したRPMI1640培地中において37℃でDBTRG-05MG細胞を日常的に培養した。製造業者のプロトコルに従ってMTTアッセイを実施した。GBM腫瘍の作製のため、6μlの無菌生理食塩水中に懸濁された300,000個のDBTRG-05MG生細胞を定位注入により0.5ml/分で6週齢のBALB/c NUマウスに投与した。注入位置はブレグマの後方2mm、右大脳半球1.5mm側方、及び硬膜より2.5mmの深さであり、細胞を視床に送達した。Baumann et al.,J.Vis.Exp.,No.67,3-6(2012)。ボーンワックスを頭蓋骨に塗り、縫合糸を用いて皮膚を閉じた。異種移植による腫瘍形成の成功率は約90%であった。これらの動物が腫瘍進行のために死んだ場合、又はこれらの動物が重篤な悪液質(開始時の体重の25%超の減少)、摂食不能、及び足指つまみ刺激に対する応答の欠如を含む所定の基準に合致した場合の殺処理時に、動物の死を記録した。Gholamin et al.,Cureus,4,1-14(2013)。腫瘍モデルのその他の特徴解析が図13A~13Cに示されている。
75μg/gのルシフェリン基質(Monolight、BD Bioscience社)を腹腔内投与により投与した。イソフルラン吸入によりマウスを麻酔し、Perkin Elmer IVIS Spectrumを使用して5分間隔で反復IVIS像を撮影した。最も強い発光シグナルを示す時点を分析に選択した。各フレームに存在するシャムマウス由来のバックグラウンド読み取り値を差し引いた。
60日目~70日目の間に死んだマウスの脳腫瘍を免疫蛍光染色に選択した。VEGF+対照の試料を参照のために含ませたが、これらのマウスは60日目よりも前に死んだ。使用した一次抗体及び希釈度は、抗Ki67抗体(1:500、GeneTex社、GTX16667)、イソレクチンIB4-AlexaFluor647、抗GFAP抗体(1:500、AbCam社、ab68428)、抗Iba1抗体(1:1000、Wako社、019-19741)、抗P-糖タンパク質抗体(1:100、AbCam社、ab170904)、抗pdgfrβ抗体(1:100、Abcam社、ab32570)、抗クローディン5抗体(1:50、Thermo Fisher Scientific社、34-1600)、抗CD31抗体(1:100、BD Pharmingen社、550274)であった。試料を4%(重量/体積)パラホルムアルデヒド(PFA)(pH6.8)中で一晩固定し、パラフィン中に包埋し、切片を作製した。スライドに対してキシレンを使用して脱パラフィンを行い、アルコールから水への段階希釈物によって再水和を行い、製造業者の指示に従って抗原賦活化、透過処理、及びブロッキングを行った。イソレクチン以外はブロッキング緩衝液中に希釈した一次抗体を4℃で一晩適用し、イソレクチンについては室温で1時間適用した。スライドをPBSで3回洗浄した後、第2の一次抗体を添加して、4℃で一晩放置した。使用した二次抗体は、ヤギ抗ウサギIgG-AlexaFluor 568(Invitrogen社、A-11011)とヤギ抗ラットIgG-AlexaFluor 488(Invitrogen社、A-11006)であり、室温で1時間適用した。少なくとも3枚の別々の切片を試験し、切片当たり少なくとも3枚の画像を撮影した。凍結切片については試料を灌流固定した後、4%パラホルムアルデヒド(PFA)中に浸漬し、30%(重量/体積)ショ糖溶液中で凍結保護して、OCT中で凍結し、切片作製を行い、ブロッキングから続けて上記のように染色した。HE染色(Mayers社)は標準的な実験プロトコルに従った。AxioScop顕微鏡をAxioFluor対物レンズ及びAxioVisionソフトウェアと共に使用して、又はModel-Zeiss LSM 700 Stage共焦点顕微鏡を使用して画像撮影を行った。
マウスを麻酔し、PBSで灌流を行い、続いて100mlの0.1mMリン酸緩衝液(pH7.4)中の4%PFAで灌流を行った。脳を取り出し、4%PFA中で後固定し(一晩、4℃)、PBS中で洗浄した。クライオミクロトームを使用して同日に脳の冠状切片(100μm厚)を切り出し、浮遊切片を作製した。切片をPBS中の4%PFA、2.5%グルタルアルデヒド中に浸漬し(一晩、4℃)、PBSで5分間、3回洗浄した。これらの標本を1%四酸化オスミウム中で45分間浸漬し、脱水し、そしてスパー低粘性樹脂を使用して包埋した。その後、試料をトリミングし、Leica EM UC6ウルトラミクロトームを使用して切片作製を行った。その後、酢酸ウランとクエン酸鉛を使用してこれらの超薄切片を二重染色した。JEOL JEM 1200EX TEMを80KVの加速電圧で使用して画像を撮影した。
血漿S100βについて、マウス血漿を1,500gで15分間の遠心分離により分離し、製造業者の指示(Elabscience社、E-EL-M1033)に従ってELISAを実施した。生理食塩水中の希釈脳ホモジネートを陽性対照として使用した。血漿中rhVEGFを測定するため、製造業者のプロトコルに従って抗ヒトVEGF ELISAキット(Boster社、EK0539)を使用した。VEGF投与前の同じマウスからの試料をブランクとして使用した。
約50mgの重量のマウス大脳皮質の試料をTrizol中でホモジナイズし、製造業者のプロトコルに従って全RNAを抽出した後、Nanodropによって定量化を行った。SuperScript IIIリバーストランスクリプターゼキット(Life Technology社)を使用して試料をcDNAに逆転写した。OmicsGreen qPCR SYBR Greenマスターミックス(Omics Bio社、台湾)を使用して、Applied Biosystems 7500リアルタイムPCRシステムを用いて増幅をモニターした。GAPDHを内部対照として使用した。使用したプライマーが表1に示されている。
GraphPad Prism 7.0b(Mac)を全ての統計学的分析及びグラフ作成に使用した。事前事後分析には対応t検定を用い、群分析には多重比較の補正のためにテューキーの事後検定と共に一元配置又は二元配置ANOVA(分散分析)を用いた。腫瘍生存分析のために死亡数を記録及び使用してカプラン・マイヤー生存曲線を作成し、マンテル・コックスログランク検定を用いてこれらの生存曲線を比較した。Mac用のLiving Image 4.0ソフトウェアを使用して腫瘍輝度及びナノ粒子蛍光のIVIS像の定量化を行った。MicroDicom(Windows)でMRI DICOM像を分類し、FIJI/ImageJ(Mac)で測定ツールを使用してSN比の計算を実施した。ヒートマップを作成するためにPythonを使用して動物内のボクセルをフレームの隅の64×64ボクセル領域の平均に対して比較し、その差を0~100までのスケールで表した。映像の明瞭さを改善するために免疫蛍光像の輝度とコントラストの調節を行い、この調節をシリーズ内の全ての画像に対して同様に適用した。共局在分析ではZeiss ZENソフトウェアで生画像を分析した。DAPIチャンネル/CD31チャンネルを使用して共局在が無いことに対する閾値を確定し、次にこの閾値をその他のチャンネルに対して適用した。ImageJのフリーハンドライン選択ツールを使用して周皮細胞の整列の分析を実施した。Affinity Designer(Mac)で図をまとめた。
VEGFの投与又は対照の投与の45分後に尾静脈を介して30μlの抗NrCAM一次抗体(abCam社)を注入した。抗体を2時間循環させ、マウスを50mLの生理食塩水で灌流し、続いて50mLのパラホルムアルデヒド(4%重量/体積)で灌流した。脳を取り出し、4%PFA中に一晩保持した後、凍結切片に処理した。陽性対照として、灌流の前に5μlの抗体を脳内に直接注入した。次に、Alexa488結合二次抗体を使用して凍結切片を染色した。陰性対照として、未処理動物の脳切片を使用した。第2の陽性対照として、従来の実験技術(室温1時間)を用いて未処理動物の脳切片を抗nrCAM抗体で染色した。染色された陽性対照以外の全ての画像を固定された露光時間で撮影した。緑色チャンネルの強度をImageJで定量した。
(i)低用量VEGFはBBB透過性の一過性上昇を誘導する
例示的実験計画が図1aに示されている。マウスにVEGF又はベヒクル対照を静脈内注入し、45分後又は4時間後に薬剤を静脈内注入した。図1bはマウス血流中でのヒトVEGFの半減期が約18.67分であることを示している。
次に、異なる種類の治療化合物を脳組織へ送達するために上記同様のアプローチが使用可能であるかどうかを調べることにした。テモゾロミド(TMZ)はGBM治療の第一選択薬剤療法である。図2aに示されているように、10倍高い濃度のVEGFを使用した場合であっても、VEGFは脳内のTMZ濃度を有意に上昇させることがない。TMZ全身投与量を5mg/kgから20mg/kgまで増加させると脳内のTMZ量を増加したが、VEGFによる前処理がTMZの脳内濃度をさらに上昇させることはなかった。これは、TMZは小分子(MW=194.15)且つ高脂溶性分子として、それ自体の臨床効果のために脳内に浸透できるという事実に起因する可能性がある。Ostermann et al.,Clin Cancer Res,10(11),3728-3736(2004)。TMZ定量についての検量線及び試料の高速液体クロマトグラフィー(HPLC)ピークが図8bに示されている。
本明細書中で報告された結果が臨床的により適切な意義のある薬剤用量までスケールアップ可能であることを確認するために、脳への薬剤送達の促進におけるVEGFの効果をブタモデルにおいてさらに試験した。
BBB透過性は、タイトジャンクションタンパク質の発現又は局在の変化、内皮細胞からの周皮細胞の解離、星状細胞の喪失、並びにBBBトランスポーター及び排出ポンプの活性変化を含む多くの変化を特徴とする場合がある。VEGF又は生理食塩水の注入から45分後又は4時間後にマウスの脳を採取し、BBB透過性に対するVEGFの潜在的な影響について分析した。
VEGF前処理と組み合わせてLipoDoxを使用して、マウス神経膠芽腫モデルにおいて神経膠芽腫の治療実験を図5aに略述されているように実施した。LipoDoxの循環半減期が長いこと(44.72時間)、及びVEGF誘導BBB開放が一過性であることを考慮すると、(VEGF前処理に加えて)LipoDox投与の後に複数用量のVEGF(MV)を投与することで、複数回のLipoDoxの脳送達の機会が得られると予期された。MVマウスには最初にVEGFを投与し、次にその最初のVEGF投与の45分後にLipoDoxを投与した。このLipoDox投与から3時間後と6時間後に2回用量のVEGFによってこれらのMVマウスをさらに処理した。MV+VEGFマウスにおけるLipoDoxの生体内分布は図12Bに示されている。VEGF又は対照で前処理されたマウスにおけるドキソルビシンの生体内分布を比較として図12Aに示した。
VEGFの投与は脳の区画化を破綻させ、結果として脳の構成要素が体循環中へ漏出することが示唆された。脳への治療薬の送達を促進するためにVEGFを使用することの潜在的な副作用が試験される。本研究ではカルシウム結合タンパク質S100β(S100β)をマーカーとして使用した。血液中のこのタンパク質の存在が脳損傷及びBBB完全性の喪失の末梢マーカーとしての役割を果たすことが以前に示されている。Marchi et al.,Clin.Chim.Acta.,342(1-2),1-12(2004);及びPlog et al.,J Neurosci,35(2),518-526(2015)。図6aに示されている結果には、本明細書に開示される低用量又は10倍高用量のVEGFでは、VEGFの投与から2時間後のマウス血漿中S100βに有意な変化が無いことが示されている。BBB透過性を上昇させる神経炎症の強力な誘導因子であるリポ多糖(LPS)は陽性対照として使用され、投与2時間後に血漿中S100βの有意な増加を引き起こした。
VEGFは20nm~100nmのナノ粒子及びLipoDox(直径が約95nm)などの分子のBBB透過の向上において特に効果的であり、全てがVEGF前処理後に脳内に容易に進入したことは興味深いことである。LipoDoxは現在では乳房及び卵巣の固形腫瘍の治療に使用されているが、GBMの治療には認可されていない。LipoDoxは、PEG修飾によりこの薬剤分子が排出から保護されて、この薬剤分子が脳組織内をより容易にすることが可能になる場合があるため、P-糖タンパク質を発現する腫瘍を有する患者ではドキソルビシンよりも効果的な場合がある。Nance et al.,Sci Transl Med,4(149),149ra119(2012)。

Claims (25)
- (i)投与を必要とする対象に血管内皮細胞増殖因子(VEGF)ポリペプチドの1回目の用量を全身投与すること、
(ii)ステップ(i)の15分~3時間後に前記対象に有効量の治療薬を全身投与すること、及び
(iii)ステップ(ii)の2~24時間後に前記対象に前記VEGFポリペプチドの2回目の用量を全身投与すること
を含む、対象の脳に治療薬を送達する方法。 - ステップ(iii)の前記VEGFポリペプチドの前記2回目の用量が、ステップ(ii)の前記治療薬の投与の2~8時間後に投与される(所望によりステップ(ii)の前記治療薬の投与の3~5時間後に投与される)、請求項1に記載の方法。
- 前記方法が、
(iv)ステップ(iii)の前記VEGFポリペプチドの前記2回目の用量の投与の2~24時間後、好ましくはステップ(iii)の前記VEGFポリペプチドの前記2回目の用量の投与の2~12時間後、より好ましくはステップ(iii)の前記VEGFポリペプチドの前記2回目の用量の投与の3~5時間後に、前記VEGFポリペプチドの3回目の用量を投与すること
をさらに含む、請求項1又は請求項2に記載の方法。 - 前記治療薬が、ステップ(i)の前記VEGFポリペプチドの前記1回目の用量の投与の約45分後に前記対象に投与される、請求項1~請求項3のいずれか一項に記載の方法。

- ステップ(iv)の前記VEGFポリペプチドの前記3回目の用量が、ステップ(iii)の前記VEGFポリペプチドの前記2回目の用量の投与の約3時間後に前記対象に投与される、請求項3~請求項5のいずれか一項に記載の方法。
- 前記VEGFポリペプチドの前記1回目の用量、前記2回目の用量、及び/又は前記3回目の用量が約50~200ng/kgである、請求項1~請求項6のいずれか一項に記載の方法。
- 前記VEGFポリペプチドの前記1回目の用量、前記2回目の用量、及び/又は前記3回目の用量が約100~150ng/kgである、請求項7に記載の方法。
- 前記VEGFポリペプチドがVEGF-Aポリペプチドである(所望により前記VEGF-AポリペプチドがヒトVEGF165Aである)、請求項1~請求項8のいずれか一項に記載の方法。
- 前記治療薬がリポソーム若しくはナノ粒子に被包されているか、又はリポソーム若しくはナノ粒子に結合している、請求項1~請求項9のいずれか一項に記載の方法。
- 前記リポソーム又は前記ナノ粒子がPEG化されている、請求項10に記載の方法。
- 前記リポソーム又は前記ナノ粒子が約20~500nm(所望により約20~300nm)のソリッドコア直径を有する、請求項10又は請求項11に記載の方法。
- 前記治療薬が医薬組成物として製剤されており、薬学的に許容可能な担体が前記医薬組成物にさらに含まれている、請求項1~請求項12のいずれか一項に記載の方法。
- 前記治療薬が遊離型である、請求項1~請求項12のいずれか一項に記載の方法。
- 前記治療薬が小分子、タンパク質、又は核酸である、請求項1~請求項4のいずれか一項に記載の方法。
- 前記治療薬が水溶性であり、且つ、500ダルトンを超える分子量を有しており、所望により前記治療薬がドキソルビシンである、請求項1~請求項15のいずれか一項に記載の方法。
- 前記対象が脳疾患を有することが疑われているヒト患者、脳疾患のリスクがあるヒト患者、又は脳疾患のヒト患者であり、所望により前記脳疾患が脳腫瘍、脳卒中、精神神経障害、及び神経変性疾患からなる群から選択される、請求項1~請求項16のいずれか一項に記載の方法。
- VEGFの前記1回目の用量及び/又はVEGFの前記2回目の用量が静脈内注射又は動脈内注射により投与される、請求項1~請求項17のいずれか一項に記載の方法。
- (i)投与を必要とする対象に約50~200ng/kgの用量で血管内皮細胞増殖因子(VEGF)ポリペプチドを全身投与すること、
(ii)ステップ(i)の15分~3時間後に前記対象に治療薬を投与すること
を含む、対象の脳に治療薬を送達する方法。 - 前記治療薬が、ステップ(i)の約45分後に前記対象に投与される、請求項19に記載の方法。
- ステップ(i)の前記VEGFポリペプチドの前記用量が約100~150ng/kgである、請求項19又は請求項20に記載の方法。
- 前記治療薬がリポソームに被包されているか、又はリポソームに結合している、請求項19~請求項21のいずれか一項に記載の方法。
- 前記治療薬がドキソルビシンである、請求項22に記載の方法。
- 前記対象が脳疾患を有することが疑われているヒト患者、脳疾患のリスクがあるヒト患者、又は脳疾患のヒト患者であり、所望により前記脳疾患が脳腫瘍、脳卒中、精神神経障害、及び神経変性疾患からなる群から選択される、請求項19~請求項23のいずれか一項に記載の方法。
- 前記VEGFが静脈内注射又は動脈内注射により投与される、請求項19~請求項24のいずれか一項に記載の方法。
