【解決手段】
本発明は、硫黄代謝物である、感染症診断のためのバイオマーカであり、硫黄代謝物は呼気中に存在するものでよく、硫黄代謝物は呼気の凝縮液から検出できる。感染症のバイオマーカは、細菌・ウイルス感染、特に、新型コロナウイルス感染の診断用バイオマーカ、感染性肺炎、特に、新型コロナウイルス感染の間質性肺炎、その他肺胞性肺炎などの肺炎診断用バイオマーカ、そして、感染による重症化診断、特に、新型コロナウイルス感染の重症化へ移行リスクの診断用バイオマーカを含む。
さらに、硫黄代謝物からなる感染診断用バイオマーカを呼気から検出するための診断システムであって、感染判断、特に、新型コロナウィルス感染診断システムである。

Description
本発明は感染症のバイオマーカに係り、硫黄代謝物に関する。
発明者は、システインヒドロポリスルフィド(CysSSH)、グルタチオンポリスルフィド(GSSH)などの生体内反応性硫黄分子種/活性硫黄分子種(reactive sulfur species, RSS)の生理機能と代謝経路の解明を行ってきた(非特許文献1,2)。
システインヒドロポリスルフィド(CysSSH)は、さまざまな生物で大量に発生するが、その生合成と生理学的機能についてはほとんど知られていない。大腸菌および哺乳動物細胞のシステイン含有タンパク質では広範な過硫化物の形成が明らかであり、これは、生体内の硫黄代謝物に関連する化学反応を含む翻訳後プロセスから生じると考えられている。
原核生物および哺乳類のシステイニル-tRNAシンテターゼ(CARS)によって触媒される反応である、基質L-システインからの効果的なCysSSH合成が存在する。マウスおよびヒト細胞のミトコンドリアCARSをコードする遺伝子の標的破壊は、CARSが内因性のCysSSH産生に重要な役割を果たしていることを示し、これらの酵素がin vivoで主要なシステイン過硫黄合成酵素として機能することを示唆している。CARSは、同時翻訳システイン過硫化も触媒し、ミトコンドリアの生合成と生体エネルギーの調節に関与している。
したがって、CARS依存性過硫化代謝物の産生メカニズムの解明は、生理学的および病態生理学的条件における異常な酸化還元シグナル伝達を理解し、酸化ストレスおよびミトコンドリア機能障害に基づいた治療標的を示唆する可能性がある。
ポリスルフィドの化学的性質は、その反応性または複雑なレドックス活性特性のため、完全には理解され、そして、解明されていない。しかしながら、発明者は、RSS代謝プロファイリングを使用して、活性硫黄メタボロミクス分析を開発し、これにより、原核生物と真核生物の両方で内生的および遍在的に生成されるRSSの生体内動態が明らかになりつつある。RSSは、活性硫黄分子は、ミトコンドリアのエネルギー代謝を維持(非特許文献2)し、一方で、強力な抗酸化(非特許文献1)、抗炎症(非特許文献3)、また、免疫制御機能(非特許文献3)を発揮している。
一方、新型コロナウイルス感染症(COVID-19)の原因となる新型コロナウイルス(SARS-CoV-2)の病態解明と予防・治療法の確立が急務となっている。COVID-19の治療薬としてSARS-CoV-2プロテアーゼ阻害剤が報告されている(非特許文献4,5)。

本発明者は、生体内に存在する活性硫黄化合物の多彩な生理活性作用に基づいて、ウイルス由来のプロテアーゼを阻害することにより、ウイルス感染症に対する予防・治療効果を奏する薬剤、「活性硫黄化合物を主要薬効成分として含有する抗ウイルス薬」を開発した(特願2020-167343)。本発明は、活性硫黄メタボロミクスに基づいて、硫黄代謝物の有用性を実現することを目標とする。
本発明者が、活性硫黄メタボロミクスに基づいて、硫黄代謝物の有用性を、活性硫黄化合物の抗ウイルス効果に関連させて検討したところ、硫黄代謝物が感染症に関するバイオマーカとして機能することを見出した。
即ち、本発明は、硫黄代謝物を含む、感染症のバイオマーカであることを特徴とする。硫黄代謝物は呼気中に存在するものでよい。硫黄代謝物は呼気の凝縮液から検出できる。硫黄代謝物は、特に、亜硫酸イオン(HSO3 -)、チオ硫酸イオン(HS2O3 -)、そして、二硫化水素イオン(HS2 -)の少なくとも一つ、又は、これらの塩、又は、これらを含む化合物、分子、又は、これらの誘導体でよい。感染症のバイオマーカは、細菌・ウイルス感染、特に、新型コロナウイルス感染の診断用バイオマーカ、感染性肺炎、特に、新型コロナウイルス感染の間質性肺炎、その他肺胞性肺炎などの肺炎診断用バイオマーカ、そして、感染による重症化診断、特に、新型コロナウイルス感染の重症化へ移行リスクの診断用バイオマーカを含む。
即ち、本発明は、硫黄代謝物を含む、感染症のバイオマーカであることを特徴とする。硫黄代謝物は呼気中に存在するものでよい。硫黄代謝物は呼気の凝縮液から検出できる。硫黄代謝物は、特に、亜硫酸イオン(HSO3 -)、チオ硫酸イオン(HS2O3 -)、そして、二硫化水素イオン(HS2 -)の少なくとも一つ、又は、これらの塩、又は、これらを含む化合物、分子、又は、これらの誘導体でよい。感染症のバイオマーカは、細菌・ウイルス感染、特に、新型コロナウイルス感染の診断用バイオマーカ、感染性肺炎、特に、新型コロナウイルス感染の間質性肺炎、その他肺胞性肺炎などの肺炎診断用バイオマーカ、そして、感染による重症化診断、特に、新型コロナウイルス感染の重症化へ移行リスクの診断用バイオマーカを含む。
本発明は、さらに、硫黄代謝物からなる感染診断用バイオマーカを呼気から検出するための診断システムであって、被験者の呼気を回収するための、呼気回収器、呼気回収装置、回収された呼気中の硫黄代謝物の定性、定量分析のための分析装置とを含む、感染判断、特に、新型コロナウィルス感染診断システムである。
本発明によれば、活性硫黄メタボロミクスに基づいて、硫黄代謝物の有用性を実現することができた。
新型コロナ非感染の健常者12名(図1)と新型コロナ感染者12名(東海大学医学部付属病院 入院患者、図2)を被験者として、呼気中の硫黄代謝物の解析を行った。新型コロナ感染の有無はPCR検査に基づいた。新型コロナ感染者12名のうち2名は軽症、10名は中等症であり、そのうち、1名は後に重症化した。軽症、中等症、重症の分類は、飽和酸素濃度、呼吸器症状、そして、胸部CT画像に基づいた。この分類については、例えば、新型コロナウイルス感染症(COVID-19)、診療の手引き・第4.1版に記載されている。
呼気の回収は、呼気回収器(ジーエルサイエンス株式会社製)、及び、呼気回収装置(ジーエルサイエンス株式会社製)を利用した。呼気回収装置を事前に摂氏マイナス20度に冷却しておき、呼気回収器((マウスピース型、マスク型))を介して、被験者が5~10分間呼吸することで、呼気エアロゾルを急速に冷却し呼気凝縮液を約1ml回収した。
呼気凝縮液25μlに親電子性アルキル化剤であるβ-(4-hydroxyphenyl) ethyl iodoacetamide(HPE-IAM)を5mMになるように添加し、摂氏37度で30分間アルキル化反応を起こさせた。その後、1%になるようにギ酸を添加し、硫黄代謝物をHPE-IAM付加体として安定化させた。親電子性アルキル化剤は硫黄の電子対をアルカリ化して、硫黄代謝物の分解を抑制する。
さらに、複数種の硫黄代謝物夫々について、溶液中の濃度が10nMになるように安定同位体標識HPE-IAM付加体スタンダードを添加し、これを質量分析サンプルとした。非文献1(Akaike T, et al. Nat Commun 2017)に従って亜硫酸イオン(HSO3
-)、チオ硫酸イオン(HS2O3
-)、二硫化水素イオン(HS2
-)のHPE-IAMアダクトを、質量分析装置を用いて定量的に解析した。即ち、上記の質量分析サンプル/スタンダードを高速液体クロマトグラフ質量分析計:LCMSTM-8060(Shimadzu)に35μlをインジェクションし、下記のMRM(多重反応モニタリング)条件(表1)により、定量的に解析した。
図3に、健常者12人、新型コロナ感染者12人夫々について、これら3イオンの定量分析結果のデータを示す。呼気凝縮液に含まれる各種硫黄代謝物の濃度は、生体内と比較して全体的に低い生成レベルであった。これは、生体内の硫黄代謝物が、呼気中で希釈されることや、呼気回収の効率が必ずしも良くないことなどが影響していると考えられる。その上で、健常者と比較して、中等症患者では、亜硫酸イオン(HSO3
-)、チオ硫酸イオン(HS2O3
-)、二硫化水素イオン(HS2
-)のレベルが有意に高いことが確認された。
図2の患者(患者番号3)は中等症から重症に移行したが、重症化時の亜硫酸イオン(HSO3
-)、チオ硫酸イオン(HS2O3
-)の測定結果を図4に示す。チオ硫酸イオン(HS2O3
-)については、中等症、重症時でもその生成レベルが、健常者と比較して有意に高いことが分かった。後に、肺炎が重症化した中等症患者(患者番号3)では、重症化する前に採取した検体について、亜硫酸イオン(HSO3
-)の顕著な増加が認められたが、重症化しなかった中等症患者では、そのような増加は見られなかった。従って、亜硫酸イオン(HSO3
-)は、新型コロナウイルス感染症の肺炎の重症化の移行リスクを評価するバイオマーカとして有望である。
活性硫黄は抗酸化活性があり、つまり、酸化ストレス、活性酸素などにより、酸化されるので、肺炎の重症化、つまり、酸化ストレスの悪化、これにより、硫黄代謝物のプロファイルが、酸化の方にシフトすると言える。

- 前記硫黄代謝物が呼気から収集される、請求項1記載のバイオマーカ。
- 前記硫黄代謝物が生体内の活性硫黄化合物の代謝によるものである、請求項2記載のバイオマーカ。
- 前記硫黄代謝物が亜硫酸イオン(HSO3 -)、チオ硫酸イオン(HS2O3 -)、そして、二硫化水素イオン(HS2 -)の少なくとも一つである、請求項3記載のバイオマーカ。
- ウィルス感染の診断のための請求項4記載のバイオマーカ。
- 新型コロナウィルス感染の診断のための請求項5記載のバイオマーカ。
- 感染による肺炎の診断のための請求項5又は6記載のバイオマーカ。
- 新型コロナウィルス感染が重症化するリスクを評価するための請求項6記載のバイオマーカ。
- 前記硫黄代謝物が亜硫酸イオン(HSO3 -)である請求項8記載のバイオマーカ。
