Abstract
Description
本発明は、遺伝子治療の分野に関する。特に、眼に対する遺伝子治療に関する。
眼疾患の治療において、遺伝子治療の普及を妨げてきた課題として、眼への核酸の効果的かつ長期的な導入と発現がある。当技術分野では、眼に大きな損傷を引き起こすことのない、核酸の効果的な導入および長期間にわたる発現のための方法を開発することが、継続的に必要とされている。
本発明者らは、電荷中性ナノ粒子の上脈絡膜への送達を含む、眼障害を治療する方法を開発し、ナノ粒子はそれぞれ、その最小可能サイズに圧縮された単一分子の核酸を含む。簡潔に、製剤は哺乳動物の眼の(SCS)に非外科的に投与される。典型的に、哺乳動物は眼障害を有する。望ましくは、ナノ粒子は、眼障害に対する治療反応を誘発するのに十分な量で送達される。
本発明は、以下の実施例を参照することによりさらに説明される。しかしながら、これらの実施例は、上述の実施形態と同様に、例示的であり、決して本発明の範囲を制限するものとして解釈されるべきではないことに留意されたい。
特に明記しない限り、中空マイクロニードルは、以前に記載されているようにホウケイ酸マイクロピペットチューブ(Sutter Instrument、Novato、Calif.)から作製された(J.Jiang,et al.,Pharm.Res.26:395-403(2009))。マイクロニードルを配置し、その長さを正確に調整できるように、ねじ付きキャップを備えたカスタムのペン状のデバイスを作製した。このデバイスは、注入圧力を加えるために炭酸ガスシリンダーに接続されたチューブを備えたマイクロピペットホルダー(MMP-KIT、World Precision Instruments,Sarasota,Fla.)に取り付けた。ホルダーは、マイクロマニピュレーター(KITE、World Precision Instruments)に取り付けられ、強膜へのマイクロニードルの挿入を制御するために使用された。
ウサギにおける非ウイルス性ナノ粒子の上脈絡膜(SC)送達後の安全性、忍容性、および遺伝子導入試験
目的:安全性、忍容性、および、2種類のレポーター遺伝子をコードする非ウイルス性ナノ粒子の送達経路として上脈絡膜投与後の短期(1週間)研究で、ナノ粒子送達後にトランスフェクトされた網膜細胞タイプを評価する。

試験物質:試験物質(TA)は、ルシフェラーゼまたはeGFPレポーター遺伝子のいずれかをコードする、非ウイルス性の楕円体状またはロッド状のDNAナノ粒子からなる。
群1のウサギは、上脈絡膜(SC)経路を介して溶媒コントロール(生理食塩水)100μLの単回注射を受けた。群2~5のウサギは、SC経路を介してTA100μLの単回注射を受けた。群6~7のウサギは、ポジティブコントロールとして使用され、網膜下(SR)経路を介してTA50μLの注射を受けた。すべての処置は、OSへの投与であった。ODは未処置のままとした。
動物は、麻酔前の最低2週間、研究環境に馴らした。馴化期間の終了時に、各動物は、研究参加への適格性の決定のために実験動物技術者によって物理的に検査された。検査は、限定されないが、皮膚と外耳、眼、腹部、神経学的、行動、および全身状態を含んだ。健康であると判断された動物を研究用に解放した。
動物は、標準操作手順書(SOP)に従って、試験群に割り当てられた。具体的には、各群の平均化された平均体重を得るために設計された層別無作為化スキームによって、動物を群に割り当てた。動物は、対応するケージカード番号と耳のタグ付けによって一意的に識別された。
試験物質(TA)は、ルシフェラーゼまたはeGFP遺伝子のいずれかをコードしている、楕円体またはロッド状のナノ粒子の非ウイルス性ベクターであった。ウサギに、ブプレノルフィン0.01~0.05mg/kg SQを投与した。次に、注射のためにウサギを鎮静化し、局所5%ベタジン溶液を使用して眼を無菌状態で準備し、続いて滅菌眼洗浄剤ですすぎ、プロパラカインHCLとフェニレフリンHCLを1滴適用した。眼瞼の開瞼器を配置し、溶媒コントロールとTAを、長さが約1000μmの30ゲージの針(Clearsideマイクロインジェクター)を使用した上脈絡膜(SC)注射によって投与した。ポジティブコントロール(ルシフェラーゼおよびeGFPロッドナノ粒子)のみ、50μlの量で網膜下(SR)から注入された。注射手順の後、1滴のネオマイシンポリミキシンB硫酸グラミシジン点眼液を、眼の表面に局所的に適用した。
試験:
獣医眼科医は細隙灯生体顕微鏡と倒像検眼鏡を使用して眼の精密検査を行い、注射前のすべての動物について眼の表面形態と前眼部の炎症を評価し、注射の24時間後と採取時にベースラインとして使用した。スコアリングには、HackettとMcDonaldの眼グレーディングシステムを使用した。試験のために動物を鎮静化させなかった(Hackett,R.B.and McDonald,T.O.Ophthalmic Toxicology and Assessing Ocular Irritation.Dermatoxicology,Fifth Edition.Ed.F.N.Marzulli and H.I.Maibach.Washington,D.C.:Hemisphere Publishing Corporation.1996; 299-305 and 557-566.)。
眼圧(IOP)は、両眼について、注射24時間前(ベースライン)、その後採取するまで毎週測定した。局所麻酔剤を使わずに、Tonovetプローブ(iCare Tonometer,Espoo,Finland)を使用して測定を行った。Tonovetプローブの先端は、角膜中央に軽く接触するように向けられた。画面に表示される平均IOPを記録した。次に、この手順をさらに2回繰り返し、測定値を記録して平均化した。
ERGは、ベースライン時および安楽死前にウサギの両眼に対して行った。すべての動物は、ERGの前に、少なくとも15分間暗順応させた。ERGは、ミニGanzfeld光刺激装置(Roland Instruments,Wiesbaden,Germany)を用いて最大強度で送達された0.33Hzの短閃光によって誘発した。各動物について、20の反応を増幅、フィルタリング、および平均化した(Retiport Electrophysiologic Diagnostic Systems,Roland Instruments,Wiesbaden,Germany)。
注射後1週間で、OSとODを安楽死直後に摘出し、Davidson Fixativeで固定し、24時間後に組織を70%エタノールに移し、切片化のためにパラフィンに包埋した。切片をヘマトキシリンとエオシン(H&E)および抗eGFP抗体で染色した。
注射後1週間で、OSおよびOD眼は安楽死直後に除核された。眼圧を下げるために房水を採取し、眼球を瞬間凍結した。凍結されたまま網膜と脈絡膜を各眼から解剖し、事前に秤量したチューブに入れた。次に、チューブの重量を測定して組織の重量を測定して、すぐにドライアイス上に置き、-80℃の冷凍庫に移した。ルシフェラーゼ活性についてアッセイするまで、凍結したサンプルを-80℃で保存した。
この研究は、TAのSCS送達後の短期および長期の忍容性を決定するために設計された。動物の数、データ収集の時点、および測定用のパラメータは、研究の目的を満たすために必要な最小数に基づいて選択された。
プロトコルは、Powered Research IACUCによって承認された。IACUCと施設のSOPによると、ケージ側の検査は、重度の眼瞼けいれん、重度の結膜充血、流涙症、過度の眼のこすり、不食などの明らかな不快感の兆候がないか、少なくとも12時間ごとに行った。これらの状態が12時間続く場合、ウサギは人道的に安楽死させた。

研究の目的は、ルシフェラーゼDNA製剤を含む非ウイルス性ナノ粒子の眼の遺伝子送達を評価することである。上脈絡膜注射は、ヒトにおいて意図される投与経路である。

投与の前に動物を絶食させない。
鎮痛剤は、眼の準備および/または投与後に必要と考えられる場合に投与される。使用される化合物は、限定されないが、以下を含み得る:フルニキシンメグルミンおよびブプレノルフィン。
動物は、ケタミンとデクスメデトミジンの標準的なレジメンを使用して麻酔される。必要に応じて、吸入麻酔剤も投与される。追加(または代替)の麻酔剤と鎮痛剤は、獣医師の推奨により投与され得る。投与されたすべての麻酔剤および鎮痛剤がデータに記録される。
局所麻酔剤を適用した後、眼をヨード溶液で約2分間すすぎ、生理食塩水ですすぐ。
注射部位の準備の後、研究固有の手順に従って、OSODの代表者により、各眼に、5~10秒間かけて100μLの上脈絡膜単回注射(角膜縁から約4mm上側象限)が投与される。注射後、針を、約10秒間眼内に保持した後、引き抜く。マイクロニードルを引き抜いて、先端が綿のアプリケーター(CTA、ドーズワイプ)を約5秒間注射部位に配置し、ドーズワイプを廃棄する。最初に、右眼に投与され、総投与後時間は、第2の眼(左)の投与時間に基づく。
生前観察
到着日に、動物の死亡率および痛みと苦痛の兆候を少なくとも1回観察し、一般的な健康状態と外見についてケージサイドで観察する。到着の翌日から、動物の死亡率および痛みと苦痛の兆候を少なくとも1日2回(午前と午後)観察し、ケージサイドでの一般的な健康状態と外観の観察を1日1回行う。追加の観察が行われる場合があり、異常な知見はすべて、生データに記録される。
必要に応じて、体重は、到着後5日以内に測定し、馴化期間中は毎週行う。必要に応じて、動物の選択時、用量の投与日、および研究の残りの期間中は毎週、動物の体重を測定する。
眼科観察:修正Hackett-Mcdonald(1ラウンド)

観察:両眼を散瞳剤で散大させ、次に細隙灯生体顕微鏡と倒像検眼鏡を使用して検査する。
両眼を全体的に検査して、修正Hackett-Mcdonaldスコアリングシステム(添付資料1に示す)を使用してグレーディングするが、次の評価は除外する。瞳孔対光反射および角膜フルオレセイン染色は、検査する獣医眼科医の裁量でのみ実行される。
異常または正常の兆候を記録する。獣医眼科医の裁量で、他の適切な器具を使用して眼を検査してもよい。
眼組織
群1の1匹の動物は、研究8日目に殺処理する。群2と3の2匹/群の動物は、研究8および22日目に殺処理する。動物は、ペントバルビタールナトリウムの過剰投与により、殺処理する。眼の採取を容易にするために心臓穿刺を介して血液を採取および廃棄し、血液の量は記録しない。殺処理時に、両眼を摘出して角膜上皮を採取し、房水を除去し(廃棄する)、液体窒素で15~20秒間急速冷凍する。摘出された眼をドライアイス上に置くか、または約-70℃で少なくとも2時間保存する。約5日以内に、凍結されたマトリックスを、各マトリックスの右眼と左眼として、リスト化されている特定のチューブタイプに回収される。
眼組織のサンプルは、ドライアイス上で翌日配送によって、以下の住所に発送される。サンプルの発送は、最終的な採取の後に予定される。発送は、月曜日、火曜日、または水曜日に、休日以外にのみ予定される。スタディモニターと受取人には、各出荷時に電子メールで通知される。電子マニフェストは、出荷時に送信される。
統計分析
統計分析には、必要に応じて、平均値や標準偏差などのパラメータが含まれる場合がある。
動物の処分
予定
動物は、最終採取手順の一部として安楽殺される。死体は保持されない。
必要に応じて、研究責任者または実験動物獣医の裁量で、Covance SOPで指定された適切なな方法に従って動物を安楽死させる。
未使用の用量製剤(複数可)は、Covance SOPに従って維持される。
すべてのサンプルは、別の場所に発送され、Covanceにサンプルは残らない。
獣医眼科医が実施する。異常な変化は、以下のスケールで記録される。
注:眼の色素沈着の程度により、このパラメータの正確なスコアリングが妨げられる場合がある。
スコア 説明
0 正常。角膜縁周囲への注射がない場合、薄白から赤みを帯びたピンク色に見え(12:00および6:00の位置を除く)、眼瞼および眼球結膜の血管が容易に観察される。
1 一部の角膜縁周囲注射に伴う、主に眼球結膜に限定された紅潮した赤みを帯びた色。主に、4:00と7:00の位置から眼の下部、および11:00と1:00の位置から眼の上部に限定される。
2 角膜周囲領域の外周の少なくとも75%を覆う角膜縁周囲注射に伴う、眼球結膜および眼瞼結膜の鮮やかな赤色。
3 顕著な角膜縁周囲注射に伴う、眼球結膜および眼瞼結膜の充血を伴う濃い暗赤色。結膜に点状出血がみられることがある。一般に、点状出血は、瞬膜および上部眼瞼結膜に沿って主にみられる。
スコア 説明
0 正常または腫脹のない結膜組織。
1 眼瞼の外反はないが正常よりも腫れている(上眼瞼と下眼瞼が正常の眼のように位置していることに注目することで容易に確認できる)。一般に、腫脹は内眼角近くの結膜嚢下部から始まり、細隙灯検査を必要とする。
2 下眼瞼と上眼瞼の正常な整合(approximation)のずれを伴う腫れで、主に、上眼瞼に限定されているため、初期段階では眼瞼の不整合(misapproximation)は上眼瞼の部分的な外反によって始まる。この段階では、腫脹は一般的に上眼瞼に限定されるが、下結膜嚢にも存在する(細隙灯で最もよく観察される)。
3 上眼瞼と下眼瞼の部分的な外反を伴う腫れは明確で、本質的に同等である。動物を正面から見て、眼瞼の位置に注目することで、これを簡単に確認できる;眼瞼縁が合わない場合は、外反が発生している。
4 上眼瞼の外反は顕著であり、下眼瞼の外反はそれほど顕著ではない。眼瞼を後退させて角膜縁周囲を観察することは難しい。
注:分泌物は、やや白い灰色、漿液性、化膿性、粘液様、および/または血性物質として定義される。通常の分泌物には、相当数の動物の眼の内眼角に見られる少量の透明または粘液様物質が含まれる場合がある。

1 分泌物は正常を上回り、眼の表面または内眼角に存在するが、眼瞼または眼瞼の毛には存在しない。
2 分泌物が多く、容易に観察され、眼瞼や眼瞼の毛の周りに集まる。
3 眼の周りの皮膚の毛を実質的に濡らすように眼瞼に分泌物が流れる。
角膜混濁のスコアには、通常2つの数値が必要である;最初の数値は角膜混濁の重症度を示し、2つ目の数値は関与の推定面積を示す。角膜混濁の重症度は次のようにグレーディングされる。

0 正常な角膜。細隙灯を用いて、上皮表面に明るい灰色の線と、間質の大理石様の灰色の外観を伴った、内皮表面の明るい灰色の線が見える。
1 透明度がいくらか失われる。細隙灯の光学切片を観察すると、上皮および/または実質の前半分のみに影響している。散乱光を用いると、多少の曇りが見られる場合があり、下部の構造がはっきりと見える。
2 中程度の透明度の喪失。曇りは実質の前半分を越えて広がる。影響を受けた実質は、大理石様の外観を失い、均一に白色である。散乱光では、下部構造が見えるものの、一部の詳細が失われ得る。
3 実質全体の厚さに影響。光学切片では、内皮表面がまだ見えている。しかし、散乱光では、下部構造はほとんど見えない(観察者がフレアの評価、虹彩血管の充血、瞳孔反応の観察、および水晶体の変化をようやくグレーディングできる程度)。
4 実質全体の厚さに影響。光学切片では、内皮がはっきり見えない。散乱光では、下部構造が見えず、房水フレア、虹彩血管の充血、瞳孔反応、および水晶体の変化の評価は不可能になる。
注:細隙灯ビームが水晶体を通過するときに観察される通常のチンダル現象を前房で見られるものと比較することによって、チンダル現象(房水フレア)の強度がスコアリングされる。房水フレアの存在は、血液房水関門の破壊の推定証拠である。

1 前房で少量のタンパク質が検出される。前房内のタンパク質の存在は、細隙灯生体顕微を用いて、白色光の小さくて明るい集束細隙ビームで、高倍率で、経験豊富な観察者が観察するとすぐにわかるが、肉眼では、そのようなタンパク質は、白色光の小さく明るい集束細隙ビームで注意深く精査した場合にのみ検出される。

眼房または硝子体細胞は、白または茶色として観察される場合があり、次の3つのカテゴリのいずれか一つとして記録される。主に茶色(>75%茶色)、主に白(>75%白)、または混合(他の比率の茶色と白)。細胞の色の種類はカウントされない。むしろ、眼科医は所見を主観的に分類する。
注:次の定義では、一次、二次、および三次の血管は、虹彩充血の主観的な眼のスコアを決定するための補助として使用される。血管の充血が大きくなり、二次および三次血管がより多く関与するほど、虹彩の関与の強度が大きくなるという仮定がなされる。また、眼の色素沈着の程度は、このパラメータの正確なスコアリングを妨げる場合がある。
