Abstract
Description

本明細書中に記載の方法における使用のための血漿カリクレイン結合抗体(抗pKal抗体)は、全長(例えばIgG(IgG1、IgG2、IgG3、IgG4を含む)、IgM、IgA(IgA1、IgA2を含む)、IgD及びIgE)であり得るか、又は抗原結合断片(例えばFab、F(ab’)2又はscFv断片のみを含み得る。結合抗体は、2本の重鎖免疫グロブリン及び2本の軽鎖免疫グロブリンを含み得るか、又は単鎖抗体であり得る。血漿カリクレイン結合抗体は、ヒト化、CDR移植、キメラ、脱免疫又はインビトロ作製抗体などの組み換えタンパク質であり得、任意選択によりヒト生殖系列免疫グロブリン配列由来の定常領域を含み得る。一実施形態では、血漿カリクレイン結合抗体はモノクローナル抗体である。

本明細書中に記載される抗体(例えばDX−2930)は、組成物、例えば薬学的に許容可能な組成物又は医薬組成物に存在し得る。本明細書中に記載される抗体(例えばDX−2930)は薬学的に許容可能な担体と共に製剤化され得る。一部の実施形態では、任意選択により薬学的に許容可能な担体を伴う組成物中、例えば薬学的に許容可能な組成物又は医薬組成物中に、DX−2930抗体150mg又は300mgが存在する。
本明細書中に記載される抗体(例えばDX−2930)は、キット中に、例えばキットの構成成分として提供され得る。例えば、本キットは、(a)DX−2930抗体、例えば本抗体を含む組成物(例えば医薬組成物)及び任意選択により(b)情報資料を含む。情報資料は、例えば本明細書中に記載の方法のために、本明細書中に記載の方法及び/又は本明細書中に記載される抗体(例えばDX−2930)の使用に関する、説明、指示、マーケティング又は他の資料であり得る。一部の実施形態では、本キットはDX−2930の1つ以上の用量を含む。一部の実施形態では、1つ以上の用量は150mg又は300mgである。
一部の態様では、本開示は、HAEの処置における本明細書中に記載される抗体(例えばDX−2930)の使用を提供する。
遺伝性血管浮腫(HAE)は「クインケ浮腫」、C1エステラーゼ阻害物質欠乏、C1インヒビター欠乏及び遺伝性血管神経性浮腫(HANE)としても知られる。HAEは、重度の皮下又は粘膜下腫脹(血管浮腫)の予測不可能な、再発性の発作を特徴とし、これは例えば四肢、顔面、生殖器、消化管及び気道に罹患し得る(Zuraw,2008)。HAEの症状としては、例えば、腕、下肢、唇、眼、舌及び/又は喉における腫脹;喉(喉頭)の腫脹、急激な嗄声を含み得、及び/又は窒息による死亡を引き起こし得る気道閉塞(Bork et al.,2012;Bork et al.,2000)が挙げられる。全HAE患者のおよそ50%は、その生涯の間に喉頭発作を経験し、患者が喉頭発作のリスクを有することを予測する方法はない(Bork et al.,2003;Bork et al.,2006)。HAE症状は、明らかな原因のない腹部疝痛;及び/又は重度であり得、腹部疝痛、嘔吐、脱水、下痢、疼痛、ショック及び/又は不要な外科手術につながり得る腹部緊急症状に類似した腸症状に至り得る腸の腫脹の繰り返しエピソードも含む(Zuraw,2008)。腫脹は、最長で5日以上続き得る。このHAEを有する固体の約3分の1が、発作中に有縁性紅斑と呼ばれるかゆみのない発疹を発症する。殆どの患者が1年に複数回の発作を起こす。
本開示は、例えば本明細書中に記載の投与スケジュールに従い、HAEを有するか又は有することが疑われる対象に、本明細書中に記載の抗体(例えば本明細書中に記載の抗体の治療的有効量)を投与することによって、遺伝性血管浮腫(HAE)を処置する(例えば1つ以上の症状を寛解させる、安定化させる、又は除去する)方法を提供する。さらに、例えば本明細書中に記載の投与スケジュールに従い、又は第2の治療、例えば本明細書中に記載の1つの他の薬剤と組み合わせて、本明細書中に記載の抗体(例えば本明細書中に記載の抗体の治療的有効量)を投与することによって、HAEを処置する方法が提供される。本開示は、例えば本明細書中に記載の投与スケジュールに従い、HAEを発症するリスクがある対象(例えばHAEの家族歴があるか又はそれに対する遺伝的素因がある対象)に、本明細書中に記載の抗体(例えば本明細書中に記載の抗体の予防的有効量)を投与することによって、HAE又はその症状を防止する方法も提供する。一部の例では、対象は、処置時にHAE症状がないヒト患者であり得る。一部の実施形態では、対象は、HAE I型又はHAE II型を有するヒト患者である。一部の実施形態では、対象は、処置前に年に少なくとも2回(例えば2、3、4、5回以上)のHAE発作を経験したことがあるヒト患者である。
本明細書中に記載される抗体(例えばDX−2930)は、血漿カリクレイン活性に関連する疾患又は状態、例えば本明細書中に記載の疾患又は状態を処置するために他の治療剤の1つ以上と組み合わせて投与され得る。例えば、本明細書中に記載される抗体(例えばDX−2930)は、別の抗血漿カリクレインFab又はIgG(例えば本明細書中に記載の別のFab又はIgG)、別の血漿カリクレイン阻害剤、ペプチド阻害剤、低分子阻害剤又は外科手術とともに治療的又は予防的に(例えば一連の処置前、処置中又は処置後)使用され得る。本明細書中に記載の抗体に結合する血漿カリクレインの併用療法で使用され得る血漿カリクレイン阻害剤の例としては、例えば国際公開第95/21601号パンフレット又は国際公開第2003/103475号パンフレットに記載の血漿カリクレイン阻害剤が挙げられる。
本明細書中に記載の処置方法の何れかの有効性を評価するためのアッセイ方法も本開示の範囲内である。一部の実施形態では、HAEに関連する1つ以上のバイオマーカー(例えば2−鎖HMWK)の血漿又は血清中濃度を、処置の有効性を評価するために、一連の処置の前及び/又は処置中(例えば初回投与の後)に測定することができる(may be may be measured)。一部の実施形態では、投与量の投与後の時点で得られるHAEに関連する1つ以上のバイオマーカーの血漿又は血清中濃度(レベル)を、投与量の投与後又は初回投与量の投与前のより早い時点で得られる試料中のバイオマーカーの濃度と比較する。一部の実施形態では、バイオマーカーは2−HMWKである。
ラナデルマブは、pH6.0の注射用の滅菌保存剤フリーの溶液である。活性成分である抗体DX−2930は、以下の公定の構成成分:30mM二塩基性リン酸ナトリウム二水和物、19.6mMクエン酸一水和物、50mM L−ヒスチジン、90mM塩化ナトリウム、0.01%ポリソルベート80を使用して製剤化される。各バイアルは、1mL溶液中の150mg DX−2930活性成分の名目濃度を含有する。試験製品は、盲検方式で上腕に皮下(SC)注射により投与される。
1.緊急処置を必要とするHAE発作数
2.中度から重度のHAE発作数。
1.14日目の後に初めて発作が起こるまでの時間、即ち、14日目の後、最初の発作が起こるまでの、対象が発作を起こさなかった持続時間。
2.高い病的状態のHAE発作の週あたりの数;高い病的状態のHAE発作は、以下の特徴のうち少なくとも1つを有する何らかの発作として定義される:入院(<24時間の観察のための入院を除く)を必要とする重度の発作、血行動態学的に重要な発作(収縮期血圧<90、IV水分補給を必要とする、又は失神若しくは失神に近い状態を伴う)又は喉頭発作。
臨床試験に含まれる患者に対して、一般的な安全性パラメーター(血液学、凝固、検尿及び血清化学)、血清学、妊娠検査、C1−INH機能アッセイ、C4アッセイ、C1qアッセイ、PK試料、血漿抗薬物抗体検査及びPD試料を含む臨床検査を行った。全ての臨床検査は、確立され、検証された方法を使用して実施される。
全体として、125人の患者をラナデルマブ(n=84)又はプラセボ(n=41)によって処置した。全患者、従って全ての患者サブグループの平均HAE発作率を決定した。平均HAE発作数を使用して、プラセボを投与した患者と比較して、DX−2930を投与した患者の平均HAE発作率のパーセント低下を決定した。HAE発作率は、全ての患者及び患者サブグループにおいて、プラセボと比較して、DX−2930によって一貫して低下した。しかし、表2で示されるように、2週間ごとに300mgのDX−2930を投与した場合、4週間ごとに300mgのDX−2930(又は4週間ごとに150mgのDX−2930)と比較して、いくつかの患者サブグループについて、プラセボ処置と比較してより大幅のパーセント低下、即ちより治療的に有効な低下が観察された。具体的には、4週間ごとに300mgのDX−2930を投与された<18歳の患者では、プラセボと比較してHAE発作率が20.5%低下し;2週間ごとに300mgのDX−2930を投与された<18歳の患者では、約42パーセンテージポイントのさらなる発作率の低下を認めた(62.3%)(図3A)。4週間ごとに300mgのDX−2930を投与した40〜<65歳の患者では、プラセボと比較してHAE発作率が71.5%低下し;2週間ごとに300mgのDX−2930を投与された40〜<65歳の患者では、発生率が約18パーセンテージポイントより低く、さらに低下した(89.8%)。4週間ごとに300mgのDX−2930を投与された女性患者では、プラセボと比較してHAE発作率が69.6%低下し;2週間ごとに300mgのDX−2930を投与された女性患者では、約16パーセンテージポイントのさらなる低下を認めた(85.8%)。4週間ごとに300mgのDX−2930を投与された喉頭発作の既往歴がある患者では、プラセボと比較してHAE発作率が64.2%低下し;2週間ごとに300mgのDX−2930を投与された喉頭発作の既往歴がある患者では、約21パーセンテージポイントより低い、さらなる低下があった(85.7%)。
この第3相試験及び非盲検長期(OLE)試験で、C1インヒビター欠損があるHAEの青年期の患者において、血漿カリクレインを標的とするモノクローナル抗体であるラナデルマブの有効性及び安全性を調べた。
本明細書で開示される特性は全てあらゆる組み合わせで組み合わせられ得る。本明細書で開示される各特性は、同じ、同等又は同様の目的に役立つ代替的な特性により置き換えられ得る。従って、明らかな別段の記述がない限り、開示される各特性は、包括的な一連の同等物又は類似の特性の単なる例である。

Claims (29)
- 遺伝性血管浮腫(HAE)発作を処置するか、又は発作率を減少させるための方法であって、
それを必要とするヒト対象に、第1の処置期間においてDX−2930と同じ相補性決定領域(CDR)を含む抗体を投与することを含み;
前記第1の処置期間において、前記抗体が前記ヒト対象に複数用量で2週間ごとに約300mgで投与され;
前記ヒト対象が、HAEに罹患しているか、罹患している疑いがあるか又はそのリスクがあり、
(i)女性であり;
(ii)18歳未満又は40〜65歳であり;
(iii)少なくとも1回、以前に喉頭HAE発作を経験したことがあり;
(iv)前記第1の処置期間の初回用量の前の4週間で、1〜2、2〜3回又は3回を超えてHAE発作を起こしたことがあり;及び/又は
(v)前記第1の処置期間の前にC1−インヒビターによる処置を受けたことがある、
方法。 - 遺伝性血管浮腫(HAE)発作を処置するか又はHAE発作の比率を減少させるための方法であって、それを必要とするヒト対象にDX−2930と同じ相補性決定領域(CDRs)を含む抗体を投与することを含み、前記ヒト対象が、
(i)12〜18歳の青年期であり;及び/又は
