Abstract
Description
本出願は、2019年4月1日出願の米国第62/827,782号及び2018年8月21日出願の米国第62/720,835に対する優先権及びそれらの利益を主張し、それら両方は、それらの全体が本明細書に援用される。
本発明は、アメリカ陸軍医学研究開発部隊により支給された助成金番号W81XWH−14−2−0129の政府支援によりなされたものである。アメリカ合衆国政府は本発明に対して一定の権利を有する。
特定の理論に束縛されるものではないが、本方法及び装置を用いてこれらの形態の失禁及び/または便秘のいずれかを治療することができるものと考えられる。
膀胱及び/または腸の排泄または制御を促進するのに充分な位置、周波数及び強度で脊髄の磁気刺激を与えることを含む、前記方法。
膀胱及び/または腸の排泄または制御を促進するのに充分な位置、周波数及び強度で脊髄の経皮的電気刺激を与えることを含む、前記方法。
本明細書で使用するところの「電気刺激」または「刺激」とは、筋肉またはニューロンに対して、及び/またはニューロン群及び/または介在ニューロン群に対して興奮性または抑制性であり得る電気信号の適用を意味する。電気信号は1つ以上のリターン電極を伴って1つ以上の電極に適用することができる点は理解されよう。
上記で説明したように、感覚、運動、及び自立神経系による膀胱機能の協奏した神経筋制御は、多発性硬化症、脊髄損傷、脳卒中などの変性性または外傷性の変化によって妨げられ得る。脊髄及び場合により付随する神経根の刺激は、膀胱及び/または腸機能の随意制御を回復できることが発見された。
膀胱/腸機能を回復するための磁気刺激
より一般的には、磁場(例えば、約0.5Hzから最大約100Hzの範囲の周波数の)を印可する装置による脊髄の刺激は膀胱機能を調節できることが発見された。詳細には、低周波数の磁気刺激(例えば、0.5Hzから最大約15Hz)が排尿を誘発することができるのに対して、より高い周波数の磁気刺激(例えば、20Hzまたは30Hzから最大100Hz)は排尿を抑制することができる。したがって、例えば、低周波数(例えば1Hz)では、排尿筋圧が尿道圧の最小のまたは小さな変化を伴って増大するため、排尿が促進されると考えられる(これを利用して低活動及び神経因性膀胱を治療することができる)。高周波数(例えば30Hz)では、尿道圧が排尿筋圧の変化を伴わずに増大するため、尿は保持され得る(これを利用して過活動膀胱及び腹圧性失禁を治療することができる)。
上記に述べたように、様々な実施形態では、脊髄の1つ以上の領域を刺激して運動機能(例えば、立つ、足踏みする、姿勢変化、腕及び/または手の制御など)を促進するか、または腸及び/または膀胱の排泄を促進する。所望の機能に応じて、ある特定の実施形態では、刺激は、頸髄の1つ以上の領域に、もしくはこれにわたって、及び/または胸髄の1つ以上の領域に、もしくはこれにわたって、及び/または腰仙髄の1つ以上の領域に、もしくはこれにわたって適用される。
磁気刺激
ある特定の実施形態では、本明細書に記載される方法は、自発運動(例えば、立つ、足踏みする、座る、横になる、座った姿勢を保つ、立ち姿勢を保つ、腕の動作、手の動作、掴む、握力など)を促し、かつ/または腸及び/または膀胱の排泄を誘発または改善するために脊髄(例えば、脊髄回路)を刺激するための磁気刺激装置を用いる。磁気脊髄刺激は、急速に変化する磁場を発生させて対象とする領域(複数可)に電流を誘導することによって得られる。ある特定の実施形態では、有効な脊髄刺激は通常、刺激コイルから放電される約108A/s以上の過渡電流を用いる。刺激コイルに流れる放電電流が磁力線を発生させる。磁力線が組織(例えば、脊髄または脳幹)を通過する際、その組織内に電流が発生する。この誘導電流が、細胞膜が脱分極されるような充分な振幅及び持続時間のものである場合、神経/神経筋組織が刺激される。
ある特定の実施形態では、本明細書に記載される方法は、自発運動(例えば、立つ、足踏みする、座る、横になる、座った姿勢を保つ、立ち姿勢を保つ、腕の動作、手の動作、掴む、握力など)を促し、かつ/または腸及び/または膀胱の排泄を誘発または改善するために脊髄(例えば、脊髄回路)を刺激するための経皮的電気刺激を用いる。表面電極(複数可)の使用によって、特定の刺激部位の選択または変化、ならびに広範な刺激パラメータの適用を促進することができる。更に、表面刺激を用いて硬膜外刺激を行うための埋め込み式電極または電極アレイの位置を最適化することができる。
様々な実施形態では、本明細書に記載される方法は、自発運動(例えば、立つ、足踏みする、座る、横になる、座った姿勢を保つ、立ち姿勢を保つ、腕の動作、手の動作、掴む、握力など)を促し、かつ/または腸及び/または膀胱の排泄を誘発または改善するために脊髄(例えば、脊髄回路)を刺激するための硬膜外電気刺激を用いることができる。
磁気刺激装置
磁気神経刺激装置は当業者には周知のものである。刺激は、急速に変化する磁場を発生させて対象とする神経に電流を誘導することによって得られる。有効な神経刺激は通常、約108A/sの過渡電流を必要とする。ある特定の実施形態では、この電流は、電子スイッチング素子(例えば、サイリスタまたは絶縁ゲートバイポーラトランジスタ(IGBT))により電流を切り換えることによって得られる。
頸髄の1つ以上の領域に電気信号を与えることが可能ないずれの現在のまたは将来的に開発される刺激システムも、本明細書に示される教示に従って使用することができる。電気刺激システム(例えば、パルス発生器(複数可))は、経皮的刺激及び硬膜外刺激とともに使用することができる。
ある特定の実施形態では、本明細書に記載される経皮的及び/または硬膜外及び/または磁気刺激方法は、様々な薬剤、詳細には神経調節活性を有する(例えば、モノアミン作動性の)薬剤と併用される。ある特定の実施形態では、様々なセロトニン作動性、及び/またはドーパミン作動性、及び/またはノルアドレナリン作動性、及び/またはGABA作動性、及び/またはグリシン作動性薬物の使用が企図される。これらの薬剤は、上記に述べた硬膜外刺激及び/または経皮的刺激及び/または磁気刺激と併用することができる。この複合的アプローチは、本明細書に記載の方法を用いて脊髄を神経調節に最適な生理学的条件とするうえで役立つ。
神経因性膀胱に対する経皮的磁気脊髄刺激の概念実証試験
慢性脊髄損傷(SCI)を有する患者は、自分の意志で排尿ができず、自己カテーテル導入によって膀胱を空にしなければならない。本発明者らは、非侵襲性の経皮的磁気脊髄刺激(TMSCS)がSCIを有する個人の膀胱機能を改善するという仮説を検証した。アメリカ脊髄損傷協会障害スケールがA/Bの慢性SCI及び排尿筋括約筋協調不全を有する5人の個人がこの前向き介入試験に参加した。
被験者は図2に示されるような3つの試験フェーズを行った。5人すべての被験者の人口統計学情報及び膀胱機能の指標を表2に示す。各被験者についてSCIのレベル及び範囲を示す磁気共鳴イメージ(MRI)を図3に示す。SCIの平均期間は8.8±7.5年であった。各被験者における少なくとも3つの事前の尿流動態試験で示されるように被験者のいずれも損傷を受けた時から自発的に排尿することができなかった。
SCI後、球海綿体反射(BCR)は脱抑制され、病的な過活動状態となる(図4)。BCRの振幅は、5人の被験者すべてで1HzのTMSCSの間に有意に減少した(p<0.001)。これに対して、高周波数の刺激はBCRの振幅を増大させるか、または有意な効果はなかった。平均のBCR遅延時間は、1Hz及び30HzのいずれのTMSCSにおいても35.2±5.3msであり、これは正常な個人におけるBCRの遅延時間と同様である(Granata et al.(2013)Func.Neurol.,28:293−295)。
評価フェーズにおいて、随意排尿試行における尿流動態試験において得られた尿道圧(P尿道)及び排尿筋(P排尿筋)は、高い周波数と低い周波数のTMSCSで有意に異なった(図5)。尿道圧及び排尿筋圧をそれぞれ表3及び4に示す。平均尿道圧は1Hzの刺激の間ではベースラインの無刺激値よりも有意に低く(p<0.05)、また、平均尿道圧は、30Hzの刺激の間では無刺激ベースライン値よりも高かった(ただし、これは統計的に有意ではなかった。p<0.10)。これに対して、平均排尿筋圧は、1Hzの刺激の間で、ベースラインの非刺激条件及び30Hzの刺激の両方と比較して有意に上昇し(両方の比較でp<0.01)、また、排尿筋圧は30Hzの刺激の間ではベースライン条件と異ならなかった(p=0.5)。したがって、低周波数の刺激は各被験者の膀胱圧を上昇させるとともに尿道圧を低下させ(尿の流れを促す条件)、30Hzの刺激は逆の作用を有し、尿道圧が有意に増大したが排尿筋圧は30Hz刺激によって変化しなかった(表3及び4)。驚くことではないが、排尿筋収縮及び膀胱圧を上昇させ、同時に尿道圧を低下させることで随意排尿が可能となった(図5)。
5人の被験者のすべてが、TMSCSによる16週間の膀胱リハビリテーション後に少なくともいくらかの随意排尿を行うことができた(図6)。少なくとも4回の週1回のTMSCS治療が行われるまでは随意排尿を行えた被験者はおらず、随意に排尿する能力は、TMSCSの開始の平均で5.6±1.5週間後に5人の被験者すべてで回復した。随意に排尿する能力は、16週間の治療期間の全体を通じて維持された。
随意排尿は、感覚系、運動系、及び自律系による膀胱の複雑で協奏した神経筋肉制御を必要とする。随意排尿では、膀胱収縮の交感神経性抑制が減弱し、排尿筋収縮の副交感神経性活性化が台頭して膀胱内圧を増大させ、尿道括約筋の収縮が抑制されて尿を膀胱から流出させる。この制御は、腹部の副交感神経節に対する、また、尾椎骨内の交感神経性及び体性ニューロンに対する前頭橋路の脊髄投射によって行われる。SCIを有する個人では、副交感神経、交感神経、及び体性神経の活動間の協調が失われ、膀胱圧が上昇するが、外括約筋の収縮が抑制されないために膀胱を完全に空にすることができない。SCIを有する患者は、尿を排出し、高い圧力による腎臓損傷を防止するために毎日複数回の膀胱への自己カテーテル導入を行わなければならず、これにより尿道の感染及び外傷性損傷のリスク及び頻度が増大する。試験の被験者のすべてにおいて実現されたカテーテル導入の頻度のいかなる減少も、カテーテル導入に伴う合併症の潜在的な減少を意味する。
被験者の選択
本発明者らは、5人の被験者でパイロット前向き介入試験を行った。試験のすべての側面は、UCLA IRB(IRB番号14−000932)により承認され、ClinicalTrials.gov(登録番号NCT02331979、登録日06/01/2015)に登録されている。全ての方法を、UCLA IRBにより規定された関連するガイドライン及び規則に従って行った。被験者の参加に先立ってインフォームドコンセントを得た。試験の組み入れ基準は、年齢18〜75才の男性で、安定状態のアメリカ脊髄損傷協会障害スケール(AIS)がA/Bであり、脊髄レベルC2〜T8の間の運動完全麻痺脊髄損傷が1年以上あり、間欠的カテーテル導入を要する神経因性膀胱の文書化された既往歴があること、である。各被験者に、少なくとも3回の尿流動態試験を受けさせて排尿筋括約筋協調不全(DSD)を伴う神経因性膀胱の診断を確認した。排尿筋括約筋協調不全(DSD)は、排尿筋圧の上昇及びそれに伴う針EMG活動と尿道圧の上昇を示す尿流動態試験によって診断した(表3及び4)。自律神経過反射の既往歴を有する患者は試験から除外した。人工呼吸器に依存しているか、薬物乱用があるか、筋骨格機能不全(すなわち、不安定骨折)、心肺疾患、活性感染症または治療を要する進行中の鬱病を有するか、または脊髄刺激への以前の曝露もしく使用のあるすべての患者を試験から除外した。膀胱ボトックス注射歴または膀胱/括約筋手術歴を有する患者は除外した。5人の被験者が採用され、試験を完了した。被験者の離脱はなかった。
各被験者に試験開始時に尿流動態検査(UDS)を行ってDSDを伴う神経因性膀胱の診断を確定し、ベースラインの膀胱機能を確立した。試験は、評価フェーズ(2週間)、治療フェーズ(16週間)、及びフォローアップフェーズ(6週間)の3つのフェーズに分けた。評価フェーズにおいては、各被験者に1回/週の経皮的磁気脊髄刺激(TMSCS)を1Hz(低)及び30Hz(高)の両方の周波数(強度40〜60%)で腰椎に行った(後述する)。この試験の前にはヒト被験者における最適刺激周波数は本発明者らにとって未知であったことから1Hz及び30Hzの両方を評価フェーズにおいて与えた。2つの周波数はランダムな順序で与えた。これらの周波数は、SCIを有する動物における排尿を低周波数刺激が促進し、高周波数刺激が抑制した、動物における以前の結果(Alam et al.(2017)Exp.Neurol.,291:141−150)に基づいて選択した。評価フェーズの終了時に、より良好な刺激周波数を決定するために各被験者に別のUDSを行った(最適刺激の特性は下記に定義される)。より良好な周波数が確立された後(5人すべての被験者で1Hzが30Hzの刺激よりも良好であることが分かった)、各被験者は試験の治療フェーズに入り、週1回の経皮的腰髄磁気刺激を合計16週間にわたって行った(後述する)。TMSCSのこの16週間の期間は、膀胱リハビリテーションを構成した。各被験者に非ビデオ尿流動態検査を治療フェーズの間の4週間毎に1回行い、進行を監視して、膀胱機能が更に損なわれないようにした。最初の4週間の刺激期間後、各被験者に膀胱カテーテル導入の前に5〜10分間にわたって随意排尿を試みてもらった。被験者には環境を静かで落ち着いた状態に保ち、排尿に集中してもらうように指示した。詳細には、被験者にいきみ/バルサルバ法、クレーデ法による外部からの圧迫、手で叩く、肛門伸展、または押し込みによる反射の誘発は行わないように指示した。各試行は10分間に制限した。各被験者に尿/大便検体採取パン((Medline DYND36600H,Mundelein,IL)をわたして随意排尿量をすべて採取してもらった。尿閉の可能性を防止するため、各被験者に随意排尿試行の後に自己カテーテル導尿してもらい、カテーテル導尿量を記録してもらった。排尿量及び膀胱内の残留尿量(各患者の所定の膀胱カテーテル導尿による排尿試行の後に採取されたもの)を、毎回の随意排尿試行の後で日誌に記録してもらった。各被験者に、試験期間の全体を通じて気付いた他のあらゆる変化も日誌に記録してもらった。フォローアップ期間では、実際の刺激の音声、部分的な感覚及び機械的キューを模倣した偽の経皮的磁気脊髄刺激(偽)を低い強度(5%)で用いた。各被験者に、治療フェーズで行ってもらったように引き続き随意排尿試行を行ってもらい、フォローアップフェーズの終わりまで各被験者に引き続き詳細な泌尿器学的生活様式の日誌をつけてもらった(図2)。
本発明者らには排尿を生じさせる効果的な刺激のパラメータが分かっていなかったという、試験開始時の知識の状態のため、無作為化試験は行えなかった。したがって、本発明者らは、各被験者が自身の対照となるシングルアーム試験を行った。更に、最初の評価フェーズ(本発明者らは、最初のベースラインの非刺激採取期間を置かなかった)の間に刺激への曝露があったため、刺激の終了時に偽フェーズを行ったが、この試験のパイロット的な性質のためにこの曝露のウオッシュアウト期間は分からなかった。しかしながら、被験者及び実験者を評価、治療、フォローアップフェーズにおけるプロセスの全体を通じて盲検化した。これらの脊髄損傷患者は損傷のために感覚が弱い/ないことを考えると、各被験者は脊髄損傷によって感覚能力が弱くなっているために治療において用いた刺激レベルの1Hzまたは30Hzでは何も感覚がなかった。本発明者らは、痛みのある感覚を避けるため比較的低い強度の刺激を意図的に選択し、刺激レベルを感覚及び運動閾値よりも低くした。各被験者には、各刺激の間に「クリック」音が聞こえた(特にクリックが予測可能な1Hzで)。この聴覚キューを偽刺激においても同様に再現した。おおよそT11〜L3のレベル(高さ)。コイルの寸法は、172×92×51mmの8の字形であり、2個のリングがそれぞれ直径75mmである。標的(焦点)は8の字の中心であり、本発明者らのケースではヒトの脊髄円錐にわたるT12〜L1領域である。本発明者らは、実験用コイル(偽面と治療面が同じもの)を使用し、これにより実験者及び被験者の両方の盲検化が可能となることで試験のフォローアップフェーズは二重盲検化された。刺激装置及び刺激量の制御を行う要員は、試験の異なるフェーズで刺激パラメータを操作したことから盲検化しなかったが、この要員は被験者に影響しないようにし(カーテンの後ろに座らせた)、各要員には、使用される特定の刺激の値によらず、異なるタスクを行う際に同じスクリプトに従ってもらった。盲検化の完全性を評価するため、本発明者らは試験終了時に被験者に各試験フェーズが何から構成されていたかを訊ねたが、被験者の答えは偶然以上のものではなかった。
本発明者らは、市販の尿流動態検査装置(Laborie Aquarius(登録商標)XT、Laborie International,Mississauga,ON,Canada)を用いた。尿流動態検査に先立ち、各被験者に直接カテーテル導尿により膀胱を空にしてもらった。尿量を記録した。次いで、患者に仰臥位になってもらい、トリプルルーメンカテーテル(TLC−7M,Laborie International,Mississauga,ON,Canada)を挿入した。2本の記録用針電極(1512A−M, Laborie International,Mississauga,ON,Canada)を睾丸の付け根と肛門のほぼ中間の正中線の1cm外側において会陰筋に両側性に挿入した。EMG接地パッドを膝関節に配置した。腹圧を記録するための直腸カテーテル(RPC−9,Laborie International,Mississauga,ON,Canada)を挿入した。次に被験者に左側臥位になってもらった。コンドームカテーテルを使用してすべての排尿量を採取した。排尿は漏斗を通じて秤(UROCAP IV,Laborie International,Mississauga,ON,Canada)に載せたメスシリンダー(DIS173,Laborie International,Mississauga,ON,Canada)に案内して排出された尿量及び尿流速度を記録した。
活性/プラセボ型8の字実験用コイル(Cool−B65 A/Pコイル)を備えたMagVenture磁気刺激装置(MagPro R30,Atlanta,GA)をすべての経皮的磁気刺激セッションで使用した。各被験者の下部脊椎の棘突起を触診し、第11胸椎〜第4腰椎をマークした。刺激の間には、コイルを中心が正中線に沿うように脊椎レベルL1に置き、発生した磁場が脊髄に平行になるような向き(体軸方向)とした。本発明者らは、二相性、単パルス(長さ250μ秒)の連続した磁気刺激のトレインを用いた。各刺激セッションは、3つの4分間の連続刺激時間で構成され、各刺激時間の間に30秒間の休みを置いて全体で13分間(合計12分間の刺激と1分間の休み)とした。最初の2週間では各被験者に周波数1Hz及び30Hzの刺激(第1週目:1Hz/30Hz/1Hz、第2週目:30Hz/1Hz/30Hz)を行い、2週間の刺激後の最初のフォローアップ尿流動態試験(UDS)でその患者にとってより良好な周波数を決定した。1Hz及び30Hzの周波数は、本発明者らによる動物及びヒトにおける以前の研究に基づいて選択した(Lu et al.(2016)Neurorehabil.Neural Repair,30:951−962;Gad et al.(2014)PloS one 9:e108184)。排尿試行の際の尿道圧(直接測定される)及び排尿筋圧(膀胱内圧−腹圧)の変化を、低周波数刺激(1Hz)及び高周波数刺激(30Hz)の両方の間に測定した。排尿試行の間に排尿筋圧の増大と尿道圧の減少の組み合わせをもたらした(したがって、膀胱の排出を促す)刺激周波数を最適として選択した。刺激の強度は、各被験者で局所的な傍脊椎筋の収縮を誘発した強度よりも20%低く設定した(筋収縮は二重盲検化を解除してしまう可能性があるため)。この刺激強度は、最大磁場強度である2テスラの通常、約40〜50%とした。最適周波数が決定された後、すべての被験者に最適刺激周波数のみを一定の強度で残り16回の週1回の膀胱リハビリテーションセッションにおいて与えた。
評価フェーズの終了時に、各被験者で低周波数(1Hz)及び高周波数(30Hz)の経皮的磁気刺激の前、その間、及びその後で以下の電気生理学的データ、すなわち、球海綿体反射(BCR)、ならびに骨盤底筋、腓腹筋、臀筋、及び大腿四頭筋の筋電位図(EMG)及び脊髄誘発電位(SEP)を取得した。
主要評価項目は、1日当たりの随意排尿量とした。予め指定された副次評価項目には、尿流速、膀胱容量、1日当たりのカテーテル導入、男性性機能問診票(SHIM)、及び尿失禁クオリティー・オブ・ライフスケール(iQOL)を含めた。TDTシステム(Tucker Davis Technologies,Alachua,FL)からのすべての電気生理学的データをコンピュータにエクスポートし、MatLab(Matlab2015b,MathWorks,Natick,MA)を使用して分析した。
神経因性膀胱に対する経皮的磁気脊髄刺激
図7は、非侵襲性磁気脊髄刺激で治療した術後オピオイド誘導性尿閉患者の残留膀胱量を示す。手術後の尿閉を有する68歳の男性患者は排尿ができないと訴えた。治療前膀胱超音波検査では、420cc強の残留膀胱量を示した。2時間にわたる不成功裏に終わった随意排尿試行の結果、膀胱量は500cc強にまで増加した。被験者のL1/2脊柱椎体領域(脊髄円錐)を経皮的磁気刺激で15分間治療した(2テスラの磁場強度の60%の強度、二相性、単パルス、250μ秒)。治療セッションの終了後、患者は随意に排尿することができ、残量膀胱量はおよそ100ccとなった。この後、退院前に、患者は自発的に排尿するための別の随意排尿試行を行った。
術後腸閉塞に対する経皮的磁気脊髄刺激
図10は、脊髄変性を治療するために、術後腸閉塞(例えば便秘)を誘発する腹部後腹膜への外科的アプローチを必要とする手術である腰椎前方椎体間固定術(ALIF)を行った7人の患者の結果を示している。これらの患者のうち、3人の患者を磁気刺激で治療し(治療群)、4人の患者を偽刺激で治療し(偽群)、その後、術後腸雑音及び排便について評価した。詳細には、術後に腸雑音の評価を1時間おきに看護士により腹部聴診によって行った。刺激は、3人の治療患者には脊髄の脊髄円錐に2時間おきに適用し、各治療セッションは、1Hz、2テスラの磁場の60〜75%の強度で15分間の刺激の持続時間であり、二相性、単パルス、250μsで行った。脊髄円錐を各患者で術前MRIにより特定し、局在化を棘突起間空間の表面ランドマークに関連付けられたAP X線画像により特定し、このコホートの中でL1〜L3の椎体レベルに局在させた。図10に示されるように、磁気刺激は、偽刺激で治療した患者と比較して治療患者で腸雑音及び排便までの時間を有意に短縮し、これにより術後腸閉塞が解消された。**対応のあるt検定でP<0.05

- 脊髄損傷または脳損傷のない、機能不全の膀胱及び/または腸機能を有する対象の膀胱及び/または腸の排泄または制御を促進する方法であって、
