Abstract
Description
本発明は、加齢性認知症または運動機能の低下を含む認知及び運動の障害、及び/または神経変性疾患の治療及び/または予防のための方法及び組成物の特定及び発見に関する。本明細書に記載されているものは、そのような障害を患っている対象の治療のための方法及び組成物であり、それらは本発明の態様である。本明細書に記載されているものはまた、認知または運動の障害を患っている対象にてニューロン新生または神経炎症の減少及び/または認知または運動の改善をもたらす投薬計画である。本明細書に記載されている方法及び組成物は、認知及び運動の障害、加齢性認知症、神経炎症、及び/または神経変性疾患を予防すること、認知及び運動の障害、加齢性認知症、神経炎症、及び/または神経変性疾患の症状を改善すること、加齢に伴う認知もしくは運動の障害、加齢性認知症、神経炎症、及び/または神経変性疾患の進行を減速すること、及び/または加齢に伴う認知及び運動の障害、加齢性認知症、神経炎症、及び/または神経変性疾患の進行を元に戻すことにおいて有用である。本発明の実施には、治療としての血液血漿画分、たとえば、以下に記載されているCohn分画法のような血液分画法から得られる1以上の画分または溶出物を使用することが含まれる。本発明の実施形態には、血漿画分(以後、「血漿画分」と呼ばれる個々にまたは複合体として正常ヒトアルブミンとアルファ及びベータのグロブリンとガンマグロブリンと他のタンパク質とで構成される溶液)を使用することが含まれる。本発明の別の実施形態には、治療として血漿タンパク質画分(PPF)を使用することが含まれる。本発明の別の実施形態には、治療としてヒトアルブミン溶液(HAS)画分を使用することが含まれる。さらに別の実施形態には、以下に記載される溶出物Iまたは溶出物II/IIIのような血液分画法に由来する溶出物を使用することが含まれる。追加の実施形態には、血栓症のリスクを減らす一方で有効性を保持するために実質的にすべての凝固因子を取り除いている血液血漿画分が含まれる(たとえば、全体として参照によって本明細書に組み入れられる米国特許出願公開番号62/236,710及び63/376,529を参照のこと)。

主題の方法を実践することにおいて、血漿を含む血液製剤がそれを必要とする個体、たとえば、認知障害または運動障害、神経炎症及び/または加齢性認知症を患っているまたは患うリスクがある個体に投与される。そのようなものとして、本発明の実施形態に係る方法には、個体(「ドナー個体」または「ドナー」)に由来する血漿成分を含む血液製剤を、認知障害または運動障害、神経炎症、神経変性及び/または加齢性認知症を少なくとも患うリスクがあるまたは患っている個体(「レシピエント個体」または「レシピエント」)に投与することが含まれる。「血漿成分を含む血液製剤」によって、血漿を含む血液(たとえば、全血、血液血漿またはその画分)に由来する製剤を意味する。用語「血漿」は、約92%の水分と、たとえば、アルブミン、ガンマグロブリン、抗血友病因子及び他の凝固因子のような7%のタンパク質と1%の鉱物塩、糖、脂肪、ホルモン及びビタミンとで構成される血液の麦わら色/淡黄色の液体成分を指すために従来の意味で使用される。主題の方法で使用するために好適な血漿を含む血液製剤の非限定例には、抗凝固剤(たとえば、EDTA、クエン酸塩、シュウ酸塩、ヘパリン等)で処理した全血、全血を濾過して白血球を除くことによって作製した血液製剤(「白血球除去」)、血漿交換に由来するまたはアフェレーシスに由来する血漿から成る血液製剤、新鮮凍結血漿、精製血漿から本質的に成る血液製剤、及び血漿画分から本質的に成る血液製剤が挙げられる。場合によっては、採用される血漿製剤は非全血血漿製剤であり、それによって製剤は全血ではないことが意味されるので、それは、たとえば、赤血球、白血球等のような全血で見いだされる1以上の成分を、少なくともこれらの成分が全血に存在する範囲において欠いている。場合によっては、血漿製剤は、完全ではないにしても実質的に無細胞であり、その際、そのような例では、細胞成分は5体積%、たとえば、0.5%以下を含めて1%以下であってもよく、その際、場合によっては、無細胞血漿画分は細胞を完全に欠く組成物であり、すなわち、それらは細胞を含まない。
本明細書に記載されている方法の実施形態には、ヒトボランティアを含むドナーに由来することができる血漿成分を含む血液製剤の投与が含まれる。用語「ヒト由来」はそのような製剤を指すことができる。血漿を含む血液製剤をドナーから採取する方法は当該技術で周知である(たとえば、参照によって本明細書に組み入れられるAABB TECHNICAL MANUAL,(Mark A.Fung,et al.,eds.,18th ed.2014)を参照のこと)。
第二次世界大戦の間に、兵士が大量の血液を失った際、戦場で採用され得る安定な血漿増量剤の必要性が生じた。その結果、凍結乾燥血漿を調製する方法が開発された。しかしながら、再構成には無菌水が必要だったので、凍結乾燥血漿の使用は戦闘の状況では困難だった。代替として、E.J.Cohn博士はアルブミンを使用し、ショックの治療に直ちに導入できる使用準備済みの安定な溶液を調製し得ることを提案した(Johan,Current Approaches to the Preparation of Plasma Fractions in(Biotechnology of Blood) 165 (Jack Goldstein ed.,1st ed.1991)を参照のこと)。血漿画分を精製するCohn博士の手順は変性効果のために冷エタノールを利用したが、pH及び温度の変化を採用して分離を達成する。
当業者にとって、血漿タンパク質画分(「PPF」)とヒトアルブミン溶液(「HAS」)とのアルブミン血漿製剤(「APP」)の2つの一般的なカテゴリーが存在する。PPFはHASよりも収率が高いプロセスに由来するが、HASよりも低い最小アルブミン純度を有する(PPFについては>83%及びHASについては>95%)。(ヒトアルブミン溶液の製造:a continually developing colloid,P.Matejtschuk,et al.,British J.of Anaesthesia,85(6):887−95,at 888(2000))。場合によっては、PPFは83%〜95%の間での、または代わりに83%〜96%の間でのアルブミン純度を有する。アルブミン純度は、電気泳動または、たとえば、質量分析によるような他の定量アッセイによって決定することができる。さらに、一部は、PPFが、たとえば、PKAのようなタンパク質「混入物」の存在のゆえに短所を有することを言及している(同文献)。結果として、PPF製剤はアルブミン血漿製剤としての需要を失っており、特定の国の薬局方からはリストからの削除さえなされている(同文献)。これらの懸念に反して、本発明はこれらの「混入物」の有益な利用を行う。前述のPKAと同様にα、β及びγのグロブリンに加えて、本発明の方法は、たとえば、ニューロン新生、神経細胞の生存、認知または運動の機能の改善、及び神経炎症の減少のようなプロセスを促進する「混入物」内の追加のタンパク質または他の因子を利用する。
米国食品医薬品局(「FDA」)によれば、「血漿タンパク質画分(ヒト)」またはPPFは「ヒト血漿に由来するアルブミンとグロブリンとで構成されるタンパク質の無菌溶液」として定義される製品の適正な名称である(参照によって本明細書に組み入れられる連邦規制のコード“CFR”21 CFR640.90)。PPFの供給源物質は21 CFR640.1〜640.5(参照によって本明細書に組み入れられる)にて処方されたように調製される全血から回収される血漿、または21 CFR640.60〜640.76(参照によって本明細書に組み入れられる)にて処方されたように調製される供給源血漿である。
(a)最終製剤がタンパク質の5.0±0.30パーセント溶液であるべきである。且つ
(b)最終製剤における総タンパク質が少なくとも83パーセントのアルブミンと17パーセント以下のグロブリンとから成るべきである。総タンパク質の1パーセント以下はガンマグロブリンであるべきである。タンパク質の組成は食品医薬品局の生物製剤評価及び研究センターの所長によって各製造元について認可されている方法によって決定される。
FDAによれば、「アルブミン(ヒト)」(本明細書では「HAS」とも呼ばれる)は「ヒト血漿に由来するアルブミンの無菌溶液」として定義される製剤の適正な名称である(参照によって本明細書に組み入れられる連邦規制のコード「CFR21 CFR 640.80)。アルブミン(ヒト)の供給源物質は21 CFR640.1〜640.5(参照によって本明細書に組み入れられる)にて処方されたように調製される全血から回収される血漿、または21 CFR640.60〜640.76(参照によって本明細書に組み入れられる)にて処方されたように調製される供給源血漿である。アルブミン(ヒト)についての他の要件は21 CFR640.80〜640.84(参照によって本明細書に組み入れられる)にてリストにされている。
(a)タンパク質濃度。最終製剤は、4.0±0.25パーセント、5.0±0.30パーセント、20.0±1.2パーセント、及び25.0±1.5パーセントのタンパク質の溶液の1つの濃度に一致すべきである。
(b)タンパク質組成。食品医薬品局の生物製剤評価及び研究センターの所長によって各製造元について認可されている方法によって判定されるとき、最終製剤における総タンパク質の少なくとも96パーセントがアルブミンであるべきである。
本発明の別の実施形態は、血栓症のリスクを減らした画分の有効性を保持するために実質的にすべての凝固因子が取り除かれる血液血漿画分を使用する。都合上、血液製剤は若齢ドナーまたは若齢ドナーのプールに由来することができ、ABOが適合する若齢血液製剤を提供するためにIgMを欠くようにすることができる。現在、A及びBの抗原に対する天然に存在する抗体の存在が輸血反応を生じ得るので、輸注される血漿はABO血液型について一致する。IgMは、ABOが一致しない血漿を患者が与えられるときの輸血反応に関与すると思われる。血液製剤または血液画分からのIgMの除去は本発明の血液製剤または血液血漿画分を投与される対象にて輸血反応を排除するために役立つ。
本発明の追加の実施形態は、PPFと比べてアルブミン濃度を減らしたが、グロブリンと他の血漿タンパク質(一部によって「混入物」と呼ばれているもの)との量を増やした血漿画分を使用する。実施形態は、PPF、HAS、溶出物I及び溶出物II/IIIと同様に、凝固因子をすべて効果的に欠いている。そのような血漿画分を以後、「タンパク質を濃縮した血漿タンパク質製剤」と呼ぶ。たとえば、本発明の実施形態は、82%のアルブミンと18%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の別の実施形態は、81%のアルブミンと19%のα、β及びγのグロブリン及び/または他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の別の実施形態は、80%のアルブミンと20%のα、β及びγのグロブリン及び/または他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、70〜79%のアルブミンと21〜30%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、60〜69%のアルブミンと31〜40%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、50〜59%のアルブミンと41〜50%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、40〜49%のアルブミンと51〜60%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、30〜39%のアルブミンと61〜70%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、20〜29%のアルブミンと71〜80%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、10〜19%のアルブミンと81〜90%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明の追加の実施形態は、1〜9%のアルブミンと91〜99%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。本発明のさらなる実施形態は、0%のアルブミンと100%のα、β及びγのグロブリン及び他の血漿タンパク質とで構成されるタンパク質を濃縮した血漿タンパク質製剤を使用してもよい。
PPF及び他の血漿画分を調製する方法は当業者に周知である。本発明の実施形態は、ヒト血漿タンパク質画分を調製するために使用される血液が凝固の抑制のためのクエン酸塩または抗凝固剤クエン酸デキストロース溶液と共にフラスコに回収されることを可能にし、さらにHink,et al.(参照によって本明細書に組み入れられるHink,J.H.,Jr.,et al.,Preparation and Properties of a Heat−Treated Human Plasma Protein Fraction,VOX SANGUINIS,2(174)(1957)を参照のこと)で開示された方法のとおりに画分I、II+III、IV及びPPFに分離する。この方法によれば、混合物を2〜8℃に回収することができる。血漿を次いでその後、7℃にて遠心分離によって分離し、取り出し、−20℃で保存することができる。次いで血漿を37℃で解凍し、好ましくは−20℃での保存から取り出した後、8時間以内に分画することができる。
本明細書に記載されている本発明の方法の態様には、血漿を含む血液製剤、たとえば、上記に記載されているような血液血漿画分による対象の治療が含まれる。実施形態には、血漿を含む血液製剤によるヒト対象の治療が含まれる。当業者は、血漿を含む血液製剤による対象の治療の方法が当該技術で認識されていることを認識する。例として且つ限定ではなく、本明細書に記載されている本発明の方法の一実施形態は、認知または運動の障害、神経炎症、神経変性及び/または加齢性認知症の治療のために新鮮凍結血漿を対象に投与することで構成される。一実施形態では、血漿を含む血液製剤はドナーから採取して直ちに、たとえば、12〜48時間以内に、認知または運動の障害、神経炎症、神経変性及び/または加齢性認知症を患っているまたはそのリスクがある個体に投与される。そのような例では、製剤は冷蔵下で、たとえば、0〜10℃で保存されてもよい。別の実施形態では、新鮮凍結血漿は−18℃以下で凍結保存(低温保存)されているものである。投与に先立って、新鮮凍結血漿は解凍され、いったん解凍されると、解凍過程が開始した後60〜75分で対象に投与される。各対象は好ましくは単回単位の新鮮凍結血漿(200〜250mL)を受け取り、新鮮凍結血漿は予め決定された年齢範囲のドナーに由来する。本発明の一実施形態では、新鮮凍結血漿は若齢個体によって提供される(に由来する)。本発明の別の実施形態では、新鮮凍結血漿は同一性別のドナーによって提供される(に由来する)。本発明の別の実施形態では、新鮮凍結血漿は18〜22歳の間での年齢範囲のドナーによって提供される(に由来する)。
本明細書に記載されている本発明の方法の態様には、たとえば、上記に記載されているような血漿を含む血液製剤、たとえば、血液血漿または血漿画分による対象の治療が含まれる。実施形態には、血漿を含む血液製剤によるヒト対象の治療が含まれる。当業者は、血漿を含む血液製剤による対象の治療の方法が当該技術で認識されていることを認識する。例として且つ限定ではなく、本明細書に記載されている本発明の方法の一実施形態は、認知または運動の障害、神経炎症、神経変性及び/または加齢性認知症の治療及び/または予防のために対象に新鮮凍結血漿を投与することで構成される。一実施形態では、血漿を含む血液製剤はドナーから採取して直ちに、たとえば、12〜48時間以内に、認知または運動の障害、神経炎症、神経変性及び/または加齢性認知症を患っているまたはそのリスクがある個体に投与される。そのような例では、製剤は冷蔵下で、たとえば、0〜10℃で保存されてもよい。別の実施形態では、新鮮凍結血漿は−18℃以下で凍結保存(低温保存)されているものである。投与に先立って、新鮮凍結血漿は解凍され、いったん解凍されると、解凍過程が開始した後60〜75分で対象に投与される。各対象は好ましくは単回単位の新鮮凍結血漿(200〜250mL)を受け取り、新鮮凍結血漿は予め決定された年齢範囲のドナーに由来する。本発明の一実施形態では、新鮮凍結血漿は若齢個体によって提供される(に由来する)。本発明の別の実施形態では、新鮮凍結血漿は同一性別のドナーによって提供される(に由来する)。本発明の別の実施形態では、新鮮凍結血漿は18〜22歳の間での年齢範囲のドナーによって提供される(に由来する)。
本発明の方法を実践することにおいて、血漿画分が対象に投与される。実施形態では、血漿画分は血漿タンパク質画分(PPF)である。追加の実施形態では、PPFは市販のPPF製剤から選択される。
本発明の追加の実施形態には、特定の年齢範囲の個体の血漿に由来する血漿タンパク質画分を投与することが含まれる。実施形態には、若齢個体の血漿に由来しているPPFまたはHASを投与することが含まれる。本発明の別の実施形態では、若齢個体は単一の特定の年齢または特定の年齢範囲である。さらに別の実施形態では、ドナーの平均年齢は対象のそれ未満であり、または治療される対象の平均年齢未満である。
主題の方法及び血漿を含む血液製剤及び血液画分は、加齢に関連した状態、たとえば、個体の認知または運動の能力における障害、たとえば、加齢性認知症、免疫状態、がん、及び身体的または機能的な低下を含む(がこれらに限定されない)認知障害、パーキンソン病のような(しかし、これらに限定されない)運動障害を予防することを含めて治療することに使用される。たとえば、本明細書で開示されている方法による主題の血漿を含む血液製剤での治療から効果が得られる、加齢に関連した認知または運動の障害、神経炎症及び/または神経変性を患っているまたはそれを発症するリスクがある個体には、約50歳以上、たとえば、60歳以上、70歳以上、80歳以上、90歳以上及び100歳以上であり、すなわち、約50〜100歳の年齢の間、たとえば、50、55、60、65、70、75、80、85、90、95または約100歳であり、且つ自然な加齢過程に関連する認知または運動の障害、神経炎症及び/または神経変性、たとえば、軽度認知障害(M.C.I)を患っている個体、及び約50歳以上、たとえば、60歳以上、70歳以上、80歳以上、90歳以上及び普通100歳未満であり、すなわち、約50〜90歳の年齢の間、たとえば、50、55、60、65、70、75、80、85、90、95または約100歳であり、認知または運動の障害、神経炎症及び/または神経変性の症状をまだ示し始めていない個体が挙げられる。自然な加齢のせいである認知または運動、神経炎症及び/または神経変性の障害の例には以下が挙げられる。
軽度認知障害は、全体的な精神機能及び日常活動は損傷されていない一方で、経時的に悪化している記憶またはたとえば、計画を立てること、指示に従うこと、または決心することのような他の精神機能での問題として現れる認知の穏やかな崩壊である。従って、著しい神経細胞の死は通常発生していないが、加齢した脳におけるニューロンはシナプスでの構造、シナプスの完全性及びシナプスでの分子処理にて半致死性の加齢に伴う変化の影響を受け易く、そのすべては認知機能を損なう。
アルツハイマー病は、βアミロイドとタウタンパク質から成る神経線維のもつれとを含有する大脳皮質及び皮質下灰白質における過剰な数の老人斑に関連する認知機能の進行性で止められない喪失である。一般型は60歳以上の人々を冒し、その発生率は年齢の進行につれて増加する。それは高齢者における認知症の65%余りを占める。
パーキンソン病(PD)は遅い且つ低下した動き(動作緩慢)、筋肉のこわばり、休止時振戦(筋失調症)、筋肉麻痺、及び姿勢不安定を特徴とする特発性の進行が遅い変性CNS障害である。元々主として運動障害と見なされていたPDは今やうつ病及び情動変化も引き起こすことが認識されている。PDは認知、行動、睡眠、自律神経機能及び感覚機能も冒すことになる。最も一般的な認知障害には、注意及び集中、作業記憶、実行機能、言語を発すること、及び視空間機能における障害が挙げられる。PDの特徴は、認知障害に関連するものに普通先行する運動機能の低下に関連する症状であり、それは疾患の診断に役立つ。
二次パーキンソニズム(非定型パーキンソン病またはパーキンソンプラスとも呼ばれる)は他の特発性変性疾患、薬剤または外因性毒素のせいで大脳基底核におけるドーパミンの作用の喪失または妨害から生じる。二次パーキンソニズムの最も一般的な原因はドーパミン受容体を遮断することによってパーキンソニズムを生じる抗精神病薬またはレセルピンの摂取である。あまり一般的ではない原因には、一酸化炭素またはマンガンの中毒、水頭症、構造的病変(腫瘍、中脳または大脳基底核を冒す梗塞)、硬膜下血種、及び黒質線条体変性を含む変性疾患が挙げられる。進行性核上性麻痺(PSP)、多系統萎縮症(MSA)、大脳皮質基底核変性症(CBD)及びレビー小体型認知症(DLB)のような特定の疾病は、特定診断に必要な主症状が構成され得る前にパーキンソニズムの症状を呈し得るので、「パーキンソニズム」として分類されてもよい。
前頭側頭型認知症(FTD)は、脳の前頭葉の進行性の劣化から生じる状態である。時間をかけて変性は側頭葉に進むこともある。有病率ではアルツハイマー病(AD)に次いで2番目で、FTDは初老期認知症の20%を占める。症状は冒された前頭葉及び側頭葉の機能に基づいて3群に分類される。
ハンチントン病(HD)は、情動的な、行動上の及び精神医学上の異常状態の発生、知的なまたは認知の機能の喪失、及び行動異常(運動障害)を特徴とする遺伝性で進行性の神経変性障害である。HDの従来の兆候には、舞踏病−顔面、腕、脚または胴体に影響を与えてもよい不随意の素早い不規則な痙攣様の動きの発生−と同様に思考処理及び獲得した知的能力の緩慢な喪失を含む認知低下が挙げられる。記憶、抽象的思考、及び判断の損傷、時、場所及び同一性の不適正な知覚(見当識障害)、興奮の増大、ならびに人格変化(人格崩壊)があってもよい。症状は通常、40歳代〜50歳代の間に明らかになるが、発症の年齢は変動し、幼児期から後期成人期(70歳代または80歳代)に及ぶ。
筋萎縮性側索硬化症(ALS)は運動ニューロンを攻撃する進行が早い、常に致死性の神経疾患である。筋肉の衰弱及び委縮及び前角細胞の機能不全の兆候が多くは手で且つあまり多くはないが脚で最初に気付かれる。発症の部位は無作為であり、進行は非対称である。筋痙攣は一般的であり、衰弱に先行することもある。稀に患者は30年生き延びるが、50%は発症の3年以内に死亡し、20%は5年生存し、10%は10年生存する。
多発性硬化症(MS)は、寛解と再発する増悪とを伴う種々の症状及びCNS機能不全の兆候を特徴とする。最も一般的な症候性の症状は1以上の四肢、胴体または顔面の片側における知覚障害、脚または手の衰弱または不器用さ、あるいは視覚障害、たとえば、部分的な失明及び片目における疼痛(球後視神経炎)、視界の暗さまたは暗点である。一般的な認知障害には、記憶(新しい情報を獲得すること、保持すること及び取り戻すこと)、注意及び集中(特に分割的注意)、情報処理、実行機能、視空間機能及び言語流暢性における損傷が挙げられる。一般的な初期症状は、複視(複視)を生じる眼筋麻痺、1以上の四肢における一時的な衰弱、軽いこわばりまたは四肢の異常な易疲労感、軽い歩行困難、膀胱制御の困難、眩暈、及び軽い情緒不安定であり、すべては散在性のCNSの関与を示し、疾患が認識される数ヵ月または数年前に発生することが多い。異常な高温が症状及び兆候を際立たせてもよい。
緑内障は、網膜神経節細胞(RGC)を冒す一般的な神経変性疾患である。証拠は、シナプス及びRGCを含む樹状突起での区切られた変性プログラムの存在を支持している。最近の証拠はまた、高齢者における認知障害と緑内障との間の相関も示している(Yochim BP,et al.Prevalence of cognitive impairment,depression,and anxiety symptoms among older adults with glaucoma.J.Glaucoma.2012;21(4):250−254)。
筋強直性ジストロフィー(DM)は、ジストロフィー筋衰弱及び筋強直を特徴とする常染色体優性の多系統疾病である。分子欠陥は、第19q染色体におけるミオトニンタンパク質キナーゼ遺伝子の3’非翻訳領域での伸長した三ヌクレオチド(CTG)の反復である。症状はどんな年齢でも発生する可能性があり、臨床重症度の範囲は広い。筋強直は手の筋肉で目立ち、眼瞼下垂症は軽い症例でも共通する。重度の症例では、顕著な末梢筋の衰弱が発生し、白内障、早期の脱毛、斧状顔貌、不整脈、精巣委縮及び内分泌異常(たとえば、糖尿病)を伴うことが多い。精神遅滞は重度の先天性形態で一般的である一方で、前頭葉及び側頭葉の認知機能、特に言語及び実行機能の加齢に伴う低下は疾患の軽度成人形態で観察される。重度に冒された人は50歳代前半までに死亡する。
認知症は、日常の機能を妨げるために大幅に十分な思考能力及び社会能力を冒す症状を有する疾患の部類を記載している。上記で議論されている加齢に関連した疾病の後期段階で見られる認知症に加えて認知症の他の例には、以下に記載されている血管性認知症及びレビー小体型認知症が挙げられる。
進行性核上性麻痺(PSP)は、複雑な眼球運動及び思考の問題と共に歩行及び平衡の制御で深刻な且つ進行性の問題を引き起こす脳の障害である。疾患の従来の兆候の1つは眼球運動を調整する脳の領域における病変のために発生する、適正に目を向けることができないことである。一部の人はこの影響をぼやけと表現している。冒された人は、進行性の軽度認知症と同様にうつ及び無気力を含む気分及び行動の変化を示すことが多い。疾患の長い名称は、疾患がゆっくり始まり、悪化し続け(進行性)、眼球運動を制御する核と呼ばれる豆粒大の構造の上(核上)の脳の特定の部分を損傷することによって衰弱(麻痺)を引き起こすことを示す。PSPは、3人の科学者がパーキンソン病から状態を区別した論文を出版した際、1964年に初めて異なる疾病として記載された。それは、疾病を定義した科学者の合わせた名前を反映してSteele−Richardson−Olszewski症候群と呼ばれることもある。PSPは進行性に悪化するが、PSP自体で死亡した人はいない。
運動失調の人々は、運動及び平衡を制御する神経系の一部が冒されるので協調に問題を有する。運動失調は、指、手、腕、脚、身体、会話及び眼球運動を冒してもよい。単語、運動失調は、中枢神経系における感染、外傷、他の疾患または変性変化に関連し得る非協調の症状を記載するために使用されることが多い。運動失調を用いて、米国運動失調財団の重点である遺伝性及び散発性の運動失調と呼ばれる神経系の特定の変性疾患の群も意味する。
多系統萎縮症(MSA)は変性神経疾患である。MSAは脳の特定の領域における神経細胞の変性に関連する。この細胞変性は、たとえば、膀胱制御または血圧調節のような身体の運動、平衡及び他の自律性の機能に関する問題を引き起こす。
フレイル症候群(「フレイル」)は、可動性の低下、筋肉の衰弱、動作緩慢、乏しい持久力、少ない身体活動、栄養不足状態及び無意識の体重減少を含む機能的な及び身体的な低下を特徴とする老年症候群である。そのような低下は、たとえば、認知機能不全及びがんのような疾患の因果関係を伴うことが多い。しかしながら、フレイルは疾患がなくても発生し得る。フレイルを患っている人は骨折、偶発的な転倒、身体障害、併存疾患及び若年死亡率から見た否定的な予後の高いリスクを有する(C.Buigues,et al.Effect of a Prebiotic Formulation on Frailty Syndrome:A Randomized,Double−Blind Clinical Trial,Int.J.Mol.Sci.2016,17,932)。さらに、フレイルを患っている人は高い医療費の高い発生率を有する。(同文献)。
場合によっては、認知疾患、運動障害、神経変性疾患及び/または神経炎症性疾患における疾患の進行及び改善を診断し、モニターする種々の方法の間で、以下の型の評価が単独で、または所望どおりに神経変性疾患を患っている対象と組み合わせて使用される。以下の型の方法は例として提示され、引用される方法に限定されない。疾患をモニターする好都合な方法を所望どおりに本発明を実践することで使用されてもよい。それらの方法は本発明の方法によっても考慮される。

認知に関連する症状についてのモニタリングの改善に加えて、当業者に周知の技法を用いて、多発性硬化症(MS)に関連する神経変性の進行または改善をモニターすることができる。例として且つ限定ではなく、モニタリングは、たとえば、脳脊髄液(CSF)のモニタリング、病変及び脱髄斑の発生を検出するための磁気共鳴画像診断(MRI)、誘発電位試験、及び歩行モニタリングのような技法を介して行うことができる。
認知に関連する症状についての改善をモニターすることに加えて、当業者に周知の技法を用いてハンチントン病(HD)に関連する神経変性の進行または改善をモニターすることができる。例として且つ限定ではなく、モニタリングは、たとえば、運動機能、行動、機能的評価、及び画像解析のような技法を介して実施することができる。
認知に関連する症状について改善をモニターすることに加えて、筋萎縮性側索硬化症(ALS)に関連する神経変性の進行または改善は当業者に周知の技法を用いてモニターすることができる。例として且つ限定ではなく、モニタリングは、たとえば、機能的評価、筋力を判定すること、呼吸機能を測定すること、下位運動ニューロン(LMN)の喪失を測定すること、及び上位運動ニューロン(UMN)の機能不全を測定することのような技法を介して行うことができる。
認知に関連する症状についての改善をモニターすることに加えて、緑内障に関連する神経変性の進行または改善は、当業者に周知の技法を用いてモニターすることができる。例として且つ限定ではなく、モニタリングは、たとえば、眼内圧を測定すること、損傷についての視神経円板または視神経頭の評価、周辺視野喪失についての視野テスト、及び断層撮影解析のための視神経円板及び網膜の画像化のような技法を介して行うことができる。
認知に関連する症状についての改善をモニターすることに加えて、進行性核上性麻痺(PSP)に関連する神経変性の進行または改善は、当業者に周知の技法を用いてモニターすることができる。例として且つ限定ではなく、モニタリングは、たとえば、機能的な評価(日常生活動作またはADL)、運動評価、精神医学的症状の判定、ならびに容積測定の及び機能的な磁気共鳴画像解析(MRI)のような技法を介して行うことができる。
本発明はまた、たとえば、低下した認知もしくは運動の機能を介して、または神経炎症との関連を介してそれ自体現れてもよい低下したまたは損傷したニューロン新生を伴う対象にてニューロン新生を治療することまたは改善することも考慮する。本発明の実施形態には、例として且つ限定ではなく、パルス状投薬治療計画を用いて、低下したまたは損傷したニューロン新生を伴う対象に血液血漿、血漿画分またはPPFを投与することが含まれる。
本発明はまた、たとえば、低下した認知もしくは運動の機能を介して、または低下したニューロン新生もしくは神経炎症との関連を介してそれ自体現れてもよい亢進した神経炎症を伴う対象にて神経炎症を治療することまたは改善することも考慮する。本発明の実施形態には、例として且つ限定ではなく、パルス状投薬治療計画を用いて、神経炎症を伴う対象に血液血漿、血漿画分またはPPFを投与することが含まれる。
本発明の実施形態には、幹細胞療法を受けている、受ける予定であるまたは受けたことがある対象にて有効量の血液血漿または血漿画分を対象に投与することによって、認知障害、運動機能の障害、神経炎症またはニューロン新生の低下であると診断された対象を治療することが含まれる。本発明の別の実施形態には、有効量の血液血漿または血漿画分を対象に投与することが含まれ、対象は幹細胞療法を受けている、受ける予定であるまたは受けたことがあり、治療法で使用される幹細胞は、胚性幹細胞、非胚性幹細胞、誘導多能性幹細胞(iPSC)、臍帯血幹細胞、羊水幹細胞等であることができる。
提供されるものはまた、認知もしくは運動の障害を治療する、神経炎症を軽減するまたはニューロン新生を増やすことにおける活性のための組成物を選別する方法である。そのような方法は、本発明によって考慮され、それには以下の実験例で記載されている方法が挙げられる。本発明の実施形態によって選別されてもよい組成物には、生物組成物(たとえば、タンパク質、タンパク質の組み合わせ、抗体、小分子アンタゴニスト)、血漿画分または他の血液組成物が挙げられる。組成物を選別する方法からの結果には、海馬(たとえば、歯状回)または他のCNSの領域における炎症/神経炎症マーカーの結果、海馬または他のCNSの領域における細胞増殖の結果、海馬または他のCNSの領域における細胞の生存、海馬または他のCNSの領域における増殖している神経前駆細胞(NPC)の細胞運命(たとえば、星状細胞、新しいニューロン)及び海馬または他のCNSの領域におけるニューロン新生が挙げられるが、これらに限定されない。
提供されるものはまた、上記に記載されている方法の1以上を実践するための試薬、装置及びそれらのキットである。主題の試薬、装置及びそれらのキットは大きく変化してもよい。
運動負荷は有酸素活動または非有酸素活動を特徴とすることができ、高カロリー燃焼活動及び中程度カロリー燃焼活動を含むことができる。運動負荷は筋力トレーニング(たとえば、ウエイトトレーニングまたは等尺性運動負荷)を含んでもよい。運動負荷はまた、たとえば、ランニング、自転車に乗ること、ウォーキング、ダンス、マーチング、水泳、ヨガ、太極拳、平衡運動負荷、脚帯、縄跳び、サーフィン、漕艇、腕または脚を回し、屈曲すること、ガーデニング、清掃、たとえば、ボーリング、エアロビクス、ピラティス及び武道のような活発な競技も含んでもよい。
(a)実施例1
清澄化した若齢ヒト血漿(若齢血漿)または市販のPPF(「PPF1」)を老化した免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。PPF1は、電気泳動で判定したとき、およそ88%の正常ヒトアルブミン(総タンパク質に対して)と12%のアルファ及びベータのグロブリンと1%以下のガンマグロブリンとを伴うPPFである。言及される場合を除いて、PPF1は本明細書の実施例では5%溶液(w/v、50g/L)を用いて生体内に投与される。マウスはすべて4つの異なる基準である自分のケージでの営巣の採点、当初の体重、オープンフィールドでの移動した距離、及びオープンフィールドの中央にいる時間の%に従って処理群にわたって均質化した。群の決定に続いて、5日間150mg/kgで投与される10mg/mLの最終濃度でPBS(リン酸緩衝化生理食塩水)にて処方したBrdU(5−ブロモ−2’−デオキシウリジン)を腹腔内(IP)でマウスに注射した。これに続いて、150μLのPPF1を週に3回4週間マウスに静脈内(IV)で注射した。行動試験は5週目と6週目とに発生したが、その際、マウスは並行する試験日での注射を回避するために週2回の注射を受けた。6週間の期間にわたる合計16回の注射について最後のIV注射の24時間後にマウスを安楽死させた。マウスの追加の2つのコホートには150μLのPPF1または生理食塩水を連続7日間静脈内で(IV)注射した(パルス状投与した)。週群当たり3回と同じ回数で行動試験は5週目と6週目とに発生した。
図1Aは、PPF1でパルス投与を受けた群が、生理食塩水対照群及びPPF1で週3回処理された群の双方と比べてオープンフィールド試験にて移動距離を増やす傾向があったことを示している。この結果はパルス投与群における移動性を増やす傾向を示している。図1Bは、PPF1でパルス投与を受けた群が、生理食塩水対照群及びPPF1で週3回処理された群の双方と比べてオープンフィールドの中央で過ごす時間の比率を増やす傾向があったことを示している。この結果はパルス投与群における不安低下の傾向を示している。
図2はPPF1の体重に対する影響を示す。双方のPPF1処理群(パルス状投薬または週3回)は注射からの有害効果を示さなかった。
図3は歯状回の顆粒層内でのDCX陽性標識細胞の数を示す。週3回処理群及び生理食塩水群と比べてパルス状投与PPF1処理群との間でニューロン新生に有意な増加があった。示したデータはすべて平均値±s.e.m.であり、**p<0.01である。Dunnettの多重比較事後解析による一元配置ANOVA(n:生理食塩水=8、PPF1パルス状投与=10、PPF1 3×/週=10)。図4は、3つの別々に処理したマウス群の歯状回の顆粒層内におけるBrdU陽性標識細胞の数を示す。週3回処理群及び生理食塩水群と比べてパルス状投与PPF1処理群との間で細胞生存に有意な増加があった。示したデータはすべて平均値±s.e.m.であり、****p<0.0001、 *p<0.05である。Dunnettの多重比較事後解析による一元配置ANOVA(n:生理食塩水=8、PPF1パルス状投与=10、PPF1 3×/週=10)。
清澄化した若齢ヒト血漿(YP)、老齢ヒト血漿(OP)または市販のPPF(「PPF1」)を老化した免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスはすべて4つの異なる基準である自分のケージでの営巣の採点、当初の体重、オープンフィールドでの移動した距離、及びオープンフィールドの中央にいる時間の%に従って処理群にわたって均質化した。群の決定に続いて、5日間150mg/kgで投与される10mg/mLの最終濃度でPBS(リン酸緩衝化生理食塩水)にて処方したBrdUを腹腔内(IP)でマウスに注射した。これに続いて、マウスには、清澄化した若齢ヒト血漿またはPPF1のいずれかの150μLを(1)週3回で6週間(「3×/週」)、(2)週3回で1週間だけ(「3×」)、(3)7日間で1週間、静脈内に(IV)注射した。マウスの追加の群には生理食塩水をパルス状で7日間投与した。若齢血漿または老齢血漿、PPF1または溶媒の投薬の開始の6週間後、マウスをすべて屠殺した。
図5は、若齢ヒト血漿(YP)、老齢ヒト血漿(OP)、PPF1または生理食塩水のいずれかで処理したマウスの9つの別々の処理群での歯状回の顆粒層内におけるDCX陽性標識細胞の数を示す。PPF1処理したマウスはパルス投与または週3回処理の双方とも他の群と比べてニューロン新生の増加を示した。示したデータはすべて平均値±s.e.m.であり、 *p<0.05である。Dunnettの事後解析によるANOVA。PPF1処理(パルス状または3×/週)及び生理食塩水処理(n:生理食塩水=4、PPF1パルス状=5、PPF1 3×/週=5、PPF1 3×=4、YPパルス状=6、YP3×/週=6、YP3×=4、APパルス状=6、AP3×=6)。
清澄化した若齢ヒト血漿(YP)、老齢ヒト血漿(OP)または市販のPPF(「PPF1」)を老化した免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスは1週間の投薬計画にて7日間毎日静脈内(IV)投与で処理した。
図7は、Barnesの迷路によって調べたときの各処理群について1日当たり試行当たり標的穴を見つけるためにかかった時間(待ち時間)を示す。PPF1のパルス状投与で処理したマウスは幾つかの個々の試験区分についての試行待ち時間で有意な減少を示したということは認知能力の改善を示している。 *p<0.05。平均値±s.e.m.、独立t検定(n:生理食塩水=13、PPF1=13、AP=14、YP=14)。
図8は、若齢ヒト血漿(YP)、老齢ヒト血漿(OP)、PPF1または生理食塩水のいずれかで処理した4つの別々の処理群の歯状回の顆粒層内におけるDCX陽性標識細胞の数を示す。生理食塩水処理と比べてパルス状投与のPPF1及びパルス状投与の若齢ヒト血漿ではニューロン新生で有意な増加があった。示したデータはすべて平均値±s.e.m.であり、****p<0.0001、**p<0.01である。Dunnettの多重比較事後解析による一元配置ANOVA。(n:生理食塩水=14、PPF1=14、AP=14、YP=15)。この実験がより長く実施され、最初のパルス状の投与計画から12週間後に組織学的検査がなされるとき、PPF1はより多数のDCX陽性標識細胞を示してPPF1とYPとの乖離がより明示される。
市販のPPF(「PPF1」)を老化した免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。1週間投薬計画にて7日間毎日の尾静脈内(IV)投与で12ヵ月齢のマウスを処理した。処理後、行動試験に先立って4.5週間、マウスを自分のケージ環境に置いたままにした。注射及び行動試験はすべて各コホートについて7週間の経過にわたって行われ、9週間の合計期間にわたって実施された。マウスはすべて最初の投薬に先立って5日間BrdUをIPで受け入れた。最後の行動試験の結論の翌日、マウスを屠殺した。
図10は、Y迷路試験における処理群による各走行路に入る総進入のうち精通走行路または新規走行路に入る進入の総数のパーセントを示す。12ヵ月齢のマウスを生理食塩水、PPF1または5×濃度のPPF1によってパルス投与で処理した。PPF1及びPPF1(×5)のパルス投与で処理したマウスは双方とも生理食塩水で処理したマウスによる新規走行路への進入の量に比べて新規走行路に進入することで有意な増加を示したということは、認知における改善を示している。示したデータはすべて平均値±s.e.m.である。 *p<0.05。対応があるt検定。
図12は、海馬切片すべての中でのBrdU陽性標識細胞の数を示す。PPF1のパルス投与で処理したマウスは生理食塩水及びPPF1(×5)で処理したマウスに比べて細胞生存の増加傾向を示した。示したデータはすべて平均値±s.e.m.である。
市販のPPF(「PPF1」)を老化した(10.5ヵ月齢)免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスはすべて4つの異なる基準である自分のケージでの営巣の採点、当初の体重、オープンフィールドでの移動した距離、及びオープンフィールドの中央にいる時間の%に従って処理群にわたって均質化した。群の決定に続いて、5日間150mg/kgで投与される10mg/mLの最終濃度でPBS(リン酸緩衝化生理食塩水)にて処方したBrdUを腹腔内(IP)でマウスに注射した。これに続いて、マウスには、(1)連続5日間[PPF1−5d]、(2)連続7日間[PPF1−7d]、(3)連続5日間と最初の投薬の完了の6週後に発生する強化(B)投薬の追加の連続5日間[PPF1−5d−B]、(4)連続7日間と最初の投薬の完了の6週後に発生する強化(B)投薬の追加の連続7日間[PPF1−7d−B]のいずれかでPPF1を静脈内(IV)で注射した。追加の群には連続7日間と最初の投薬の完了の6週後に発生する強化(B)投薬の追加の連続7日間[SAL−7d−B]で生理食塩水を注射した。パルス状投薬の5週後、マウスには、30mg/kgで投薬される10mg/mLの最終濃度でのPBSで処方したEdU(5−エチニル−2’−デオキシウリジン)をIPで5日間注射した。マウスはすべて、PPF1または溶媒のパルス投薬の完了の12週後に屠殺した。
市販のPPF(「PPF1」)を成熟(3ヵ月齢及び6ヵ月齢)免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスはすべて4つの異なる基準である自分のケージでの営巣の採点、当初の体重、オープンフィールドでの移動した距離、及びオープンフィールドの中央にいる時間の%に従って処理群にわたって均質化した。群の決定に続いて、5日間150mg/kgで投与される10mg/mLの最終濃度でPBS(リン酸緩衝化生理食塩水)にて処方したBrdUを腹腔内(IP)でマウスに注射した。これに続いて、マウスには、生理食塩水またはPPF1を静脈内で(IV)連続7日間(パルス投薬)注射した。生理食塩水及びPPF1処理の双方に由来するマウスのサブセットには自分のケージ内にて回し車を提供した。パルス投薬の完了の3日後、10日後または42日後にマウスを屠殺した。
PPF1または生理食塩水対照を11ヵ月齢の免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)の2つの処理群に投与した。マウスはすべて連続7日間、投与当たり150μLのPPF1または生理食塩水のIV注射を受けた。回し車(MedAssociates)は、試験の7週目に開始するランナーと名付けた(n=8、PPF1及び生理食塩水についてn=8)マウスのケージに入れた。車の回転数は昼夜、連続5日間記録した。
組換えヒトアルブミン(「rhアルブミン、Albumedix,Ltd,Nottingham,UK)、清澄化した若齢ヒト血漿(「YP」)または生理食塩水対照を10.5ヵ月齢の免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスはすべて、7日間のパルス投薬に先立って1週目に50mg/kgのBrdUのIPを受けた。rhアルブミンを注射用水(WFI、0.9%生理食塩水)で50mg/mLに希釈した。マウスはすべて投与当たり150μLのrhアルブミン、YPまたは生理食塩水の連続7日間のIV注射を受けた。処理の最終日の6週後、マウスを屠殺した。
マウスの未発達の皮質に由来する解離混合神経細胞を48穴多重電極アレイプレート(Axion Biosystems)に入れ、増殖させた。各ウェルは入れた神経細胞と物理的に接触し、細胞膜特性のわずかな変化を測定する16の電極を含有する。この設定は、種々の異なるパラメーターを評価して単一電極レベルでのニューロンのスパイク活動及び発火行動についての情報を得るとともに、ウェル内での複数の電極にわたってニューロンの発火特性の同期生を評価することによってニューロンの結合性の程度についての情報を得ることを可能にする。
清澄化した老齢ヒト血漿(OP)または無菌生理食塩水を8週齢の(若齢)免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。各実験では、マウスは体重によって処理群にわたって均質化した。マウスすべてには、無菌PBS中の150mg/kgのBrdUを連続5日間IPで注射した。BrdU注射には投与当たり150μLでの異なる治療規範の老齢血漿のIV投与が続いた。規範はすべて図23で要点を述べる。
老齢ヒト血漿(65〜68歳起源)で連続7日間処理した8週齢のNSGマウスを老齢血漿の最後の注射の4週後に改変Barnes迷路を用いて調べた。図26は、Barnes迷路の脱出にかかった時間経過を示し、老齢血漿及び生理食塩水で処理したNSGマウスについて脱出穴に達し、それに入る時間を示す。群間での脱出にかかった時間には有意差はなかったが、4日目での老齢血漿で処理したマウスは生理食塩水対照よりも上手く実行しなかった。このデータは海馬機能に関連する空間記憶作業における低下した学習と記憶とを示している。示したデータはすべて平均値±s.e.m.である。二元配置ANOVA、Sidak事後検定。
若齢(8週齢)免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)を体重によって処理群にわたって均質化した。マウスには無菌生理食塩水中35mg/kgのカイニン酸(Sigma)または生理食塩水対照を皮下で(s.c.)注射した。カイニン酸の末梢投与は、海馬における急性発作活動、炎症を生じ、マウスのサブセットでは、海馬のCA1領域にてニューロンの欠損も生じた。カイニン酸注射の2時間後、マウスに150μLのPPF1または生理食塩水を静脈内に投与した。PPF1または生理食塩水の投与は合計5日間毎日継続した(図30)。6日目に分析のために組織を回収した。ミクログリアの活性化(CD68)及び星状細胞の活性化(GFAP)についての免疫蛍光染色の後、海馬のCA1領域にて炎症性の変化を分析した。切片は30μmの浮遊切片にて免疫組織化学処理の後、Hamamatsu NanoZoomer HT(Hamamatsu)にて画像化し、Image Pro Software(Media Cybernetics)を用いて解析した。
6ヵ月齢のNSGマウスに150μL(10mg/mL)の用量でPPF1または生理食塩水対照のいずれかをIVで1週間(7日間)毎日注射した。マウスはすべて、PPF1または生理食塩水対照を受け入れるのと同じ日に1日1回50mg/kgのBrdUで処理した。次いでマウスを2つのコホートに分けた。第1のコホートはBrdU及びPPF1による同時処理の7日間の直後の日に屠殺した。第2のコホートは7日後屠殺され、追加の7日間のBrdUの毎日投与を受けた。
3ヵ月齢または6ヵ月齢のNSGマウスにPPF1または生理食塩水溶媒のいずれかをIVで1週間(7日間)毎日注射した。その後、マウスは7日間の毎日投与が投与された3、10または42日後に屠殺した。歯状回のブレードにおける神経幹細胞及び神経前駆細胞を標識する増殖している細胞にしか存在しない核マーカーであるKi67で脳を染色した。図33は、PPF1を用いた連続7日間のパルス投薬計画の終了の10日後、6ヵ月齢のマウスが歯状回にて総前駆細胞(Ki67陽性または「Ki67+」)の増加を呈したことを示す。図34は、PPF1を用いた連続7日間のパルス投薬計画の終了の10日後、NSGマウスにおける歯状回でのKi67の染色(明るい領域)を示す。これは、投薬停止の42日後に細胞生存全体及びニューロン新生を増やすことにおけるPPF1についての考えられる作用機序の1つが総前駆細胞(神経幹細胞)の増加によるものであり得ることを示している。
市販のPPF(「PPF1」)または生理食塩水対照を6ヵ月齢と12ヵ月齢との免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)の2つの異なる集団に投与した。マウスはすべて、試験剤の7日間パルス投薬に先立って1週目に50mg/kgのBrdUを受け入れた。マウスはすべて、投与当たり150μLのPPF1または生理食塩水のIV注射を受けた。各処理群に由来する1コホートを用いて増殖を調べ、最後の施された投与の6週後に屠殺した。
清澄化した老齢ヒト血漿(老齢血漿)または無菌の生理食塩水を3ヵ月齢のマウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。各実験では、マウスは体重によって処理群にわたって均質化した。マウスはすべて無菌生理食塩水中の150mg/kgのBrdUの連続5日間のIP注射を受けた。BrdU注射には、投与当たり150μLの老齢血漿または無菌生理食塩水の連続7日間毎日のIV投与が続き、最後の投与の4週後、組織学的に解析した。
皮質活性化
老齢(18ヵ月齢)C57BL/6マウスは150uLのPPF1または0.9%生理食塩水のIV注射を毎日7日間受けた。最後の試験剤投与の2時間半(2.5時間)後、ケタミンとキシラジンによる深麻酔下での0.9%生理食塩水とそれに続く4%ホルムアルデヒドとによる経心臓潅流によってマウスを屠殺した。脳を切断し、後固定し、iDisco手順によって処理し、2×2×3マイクロメーターのボクセル解像にて光シート蛍光顕微鏡(LSFM)を介してcFos陽性細胞を視覚化した。画像化した脳を三次元体積として割り当て、活性化されたcFos陽性細胞をコンピューターで検出した。群間の統計的比較は、偽発見率により多重比較について補正された負の二項回帰によって行った(*は0.05未満のp値を示す)。
市販のPPF(“PPF1”)または生理食塩水対照を22ヵ月齢の野生型(WT)マウス(C57BL/6J,“WT”,系統コード0664,Jackson Labs,Bar Harbor,ME)に投与した。マウスはすべて7日間パルス投薬に先立って1週目に50mg/kgのBrdUを受け入れた。続いて、マウスはすべて、連続7日間、投与当たり150μLのPPF1または生理食塩水のIV注射を受けた。最後のPPF1または生理食塩水の注射の10日後、マウスを屠殺し、組織検査のために脳を処理した。
市販のPPF(「PPF1)または生理食塩水対照を23ヵ月齢の野生型(C57BL/6J,“WT”,系統コード0664,Jackson Labs,Bar Harbor,ME)に投与した。マウスはすべて連続7日間パルス投薬に先立って1週目に50mg/kgのBrdUを受け入れた。続いて、マウスはすべて、連続7日間、投与当たり150μLのPPF1または生理食塩水のIV注射を受けた。各処理群に由来する1コホートを用いて神経炎症についての組織学マーカーを調べ、最後に施された投与の6週後に屠殺した。
4〜4.5ヵ月齢の30匹のオスのアルファ−シヌクレイントランスジェニックマウス(ライン61、野生型背景C57BL/6J)を15匹の2群に分け、PPF1または溶媒のいずれかで連続7日間処理した。PPF1処理は体重のグラム当たり5μLにてIVで投与した。アルファ−シヌクレインマウスはパーキンソン病のトランスジェニックモデルとして役立ち、アルファ−シヌクレインタンパク質を過剰発現する。このトランスジェニックモデルはNSGマウスとは異なり、免疫不全ではない。
PPF1、HAS1または生理食塩水対照を12ヵ月齢のマウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。HAS1は冷アルコール分画法によって調製し、ドナーからのプールしたヒト血漿に由来する、5%溶液(w/v、50g/L)にて95%余りがヒトアルブミン(総タンパク質に対して)である市販のHASである。言及された場合を除いて、HAS1は本明細書の実施例では5%を用いて生体内で投与された。マウスには、連続7日間毎日、用量当たり150μLでのPPF1、HAS1または無菌生理食塩水をIV投与で注射し、最後の投与の4週後に行動の面で解析した。
PPFを用いた臨床規範
(1)軽度から中程度のAD
軽度から中程度のADの60歳以上の男女を、6ヵ月の総期間での試験の1週目と13週目とに5日間毎日1回、100mLまたは250mLのPPF1(「パルス状投薬」)を受け入れるように無作為に割り振る。2回の5日間投薬の期間の間、対象は入院観察ユニットに居住し、安全性評価を円滑にし、対象はすべてスクリーニング来診、ベースライン来診、治療来診、経過観察来診、及び試験終了の来診/早期終了の来診を受ける。安全性及び忍容性の評価は来診ごとに発生する。神経認知評価及び運動評価はベースライン及び投薬に続く定期的な中間来診で行う。
軽度から中程度のADであると診断された対象の2群を二重盲検法での積極的な治療に対して無作為化する。対象はすべて、合計6ヵ月間の試験期間で1週目と13週目とにて連続5日間無作為化投与での1日当たり1回の点滴を受ける。対象は、2つの投与レベルである100mL及び250mLのPPF1の1つに対して無作為化される。投薬群を性別によっても階層化する。投与時間は2〜2.5時間であり、投与全体が施されるように投与専用の指針に従って流速を設定する。
パーキンソン病及び認知障害の対象を、2つの群である積極的治療とプラセボとの2つの期間に無作為化する。対象は、試験の第1週の間に連続5日間(「パルス状投薬」)積極的治療またはプラセボ治療の1日1回の点滴を受ける。13週目の間に両群は連続5日間積極的治療を受け、試験期間はおよそ7ヵ月である。投与時間は2〜2.5時間であり、投与全体が施されるように投与専用の指針に従って流速を設定する。
組織の採取及び組織検査が以下の通り行われた。C57BL/6マウスにアベルティン(250mg/kgIP)で深麻酔をかけた。PPF1または対照の投与を受けたマウスに投与終了後10日間麻酔をかけた。脳を生理食塩水による潅流後に採取し、正中のスライスで分割された片方の半分を新たに調整された4%のPFAに固定した。24〜48時間後にPFAを30%スクロースに交換した。二度目の30%スクロースの交換を24時間後に行った。残りの半分を海馬及び皮質に切断し、次いでドライアイス上で急速凍結した。固定された脳の組織を切断し、シナプスのマーカーであるSYNAPSIN1及びPSD−95について分析し、Zeiss LSM800 with Airyscanを使って撮像した。Zスタックをシナプス前及びシナプス後の斑点の数及びサイズを定量するImageJのシナプスカウントマクロを使って解析した。シナプス総数を共存化または並列されたシナプス前及びシナプス後の斑点数を数えることによって定量した。凍結された脳組織をシナプスのマーカーについてqRT-PCRによって解析した。
E16マウスの脳から切断した皮質または海馬を分離し、PDLでコーティングされた96ウェルのガラスボトムプレート上で1ウェルあたり20K細胞の濃度で別々に培養した。ニューロンの培養物を処理条件の存在下で14日間維持した。処理条件は、10%(v/v)の組換えヒトアルブミン(“rhアルブミン”,Albumedix,Ltd,Nottingham,UK)、PPF1またはHAS1を含んだNeurobasal培地にB27補完剤及びBDNFを加えることと、2週に一度の培地の交換からなる。細胞を14日間の培養後固定し、以下のマーカーによる免疫細胞化学を行った。ヘキストを細胞核のすべてを染色するするために使用し、ニューロンを同定するために抗Map2抗体を、星状細胞を同定するために抗Gfap抗体を、前駆細胞を同定するために抗Nestin抗体を使用した。画像をINCell 2000 analyzer(GEHealthcare)を使用して得た。
PPF1または生理食塩水対照を10.5ヵ月齢の免疫不全マウスに投与した(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)。マウスは、150μLのPPF1(n=10)または生理食塩水(n=15)のIV注射を連続7日間受け、マウスが12ヵ月齢になった投与の6週間後に2択水泳試験での認知行動の試験を実施した。2択水泳試験は、脱出プラットフォームまでの待ち時間を定量することによる海馬依存性の記憶と、迷路の開始分枝を脱出するときの最初の方向選択の正誤を記録することによる皮質依存性の実行記憶とを計測する。検定は、T字の両分枝の一方端に脱出プラットフォームを設けて不透明な水に浸漬させたT字迷路を構成した。1日目、マウス群を可視のプラットフォームで訓練し、T字の分枝の底部に徐々に配置し、プラットフォームへと水泳させる実験を4度行った。2日目、脱出プラットフォームを水面の真下に沈めた状態でそのマウス群をT字の分枝の底部に再度配置した。検定の結果は底部の分枝を脱出するときの正誤の選択及び脱出プラットフォームまでの待ち時間として記録された。図58Aは、PPF1で処理したマウスは2日目に脱出時間が短くなる強い傾向がみられ、これは増強された海馬依存性の認知を示す。図58Bは、PPF1で処理したマウスは生理食塩水で処理したマウスと比べてプラットフォームへの正しい選択に大いに成功し、これは増強された皮質依存性の記憶を示す。このデータはカイ2乗検定により0.041のP値に関して重要である。データ全体は、PPF1処理は海馬及び皮質依存性の記憶を高めることを示し、老齢マウスの脳の1以上の領域に有益な効果があることを示唆する。
PPF1、若齢血漿(YP)、生理食塩水対照または老齢血漿(OP)を10.5ヵ月齢の免疫不全マウスに投与した(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)。マウスはBrdUIPを150mg/kg日毎に5日間受けた後、150μLの生理食塩水(n=13)、若齢血漿(YP,n=13)、PPF1(n=15)または老齢血漿(OP,14)のIV注射を連続7日間受け、マウスが12ヵ月齢になったとき屠殺した。BrdU注射はすべての増殖している細胞を標識し、歯状回において主に神経幹細胞及び神経前駆細胞を標識する。処理の投与6週間後及びBrdU注射の7週後、BrdU陽性細胞の数を計測し、その計測値を使って細胞の生存を評価し得る。また、そのとき標識された神経幹細胞及び神経前駆細胞はNeuN陽性ニューロンまたはGFAP陽性星状細胞に分化し得る。BrdU、NeuN及びGFAP共染色することによって、処理済みの動物の脳により多数の新生ニューロンが有るかどうか、処理によって細胞運命に変化があるか否かの判定を行い得る。歯状回の新生ニューロンは海馬のネットワークに一体化され、これを増強させる。図59は、12ヵ月齢のNGSマウスの歯状回におけるBrdU、NeuN及びGFAP染色の代表画像を示す。図60Aは、生理食塩水対照、若齢血漿(YP)、PPF1、または老齢血漿(OP)で処理したマウスにおいて、定量したBrdU/NeuN共標識細胞及びBrdU/GFAPの共標識細胞の総数を示す。生理食塩水またはOPで処理したマウスよりもPPF1で処理したマウスにより明らかに多数のBrdU/NeuNダブル陽性細胞が見られる。YPで処理したマウスとPPF1で処理したマウスとを比べたときにもBrdU/NeuNダブル陽性細胞は増加する傾向があり、これによって、YP、生理食塩水またはOPでの処理に比べてPPF1処理後により多くの新生ニューロンがみられることを示唆する。BrdU/GFAPダブル陽性細胞の数には変化がなかった。図60Bは図60Aと同じデータを示すが、BrdU標識した細胞の総数に対するBrdU/NeuNダブル陽性細胞及びBrdU/GFAPダブル陽性細胞のパーセントとして定量する。定量することによって処理が神経幹細胞又は神経前駆細胞の運命を変えるか否かについて計測できる。ここで、YP及びPPF1で処理したものに関して、BrdU/NeuN共陽性細胞のパーセントの増加傾向が観察され、その傾向はPPF1で処理した動物においてわずかに強い。OP処理後のものにもニューロンの形成を犠牲にして星状細胞の増加傾向がみられ、これによって、OPはYPまたはPPF1と逆の効果を有することを示唆する。これら双方のデータによって、PPF1はYPよりも新生ニューロンの形成をより強く大きく増加させ、幹細胞および前駆細胞の運命を神経細胞系の方にわずかに変化させる。これらの新生ニューロンは海馬のネットワークに一体化されかつ記憶の機能を高める得る。
PPF1、若齢血漿(YP)または生理食塩水対照を10.5ヵ月齢の免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスは、150μLの生理食塩水(n=13)、若齢血漿(YP、 n=14)またはPPF1(n=13)のIV注射を連続7日間受け、投与6週間後のマウスが12ヵ月齢になったときにBarnes迷路における認知行動の検定を実施した。Barnes迷路は、マウスが迷路の辺りに配置された視覚的な手がかりを使って、逃げ穴の方に移動するために要する時間を測定することによって海馬依存性の記憶を計測する。要するに、この具体的な検定において、マウスは連続3日間逃げ穴を見つけるための試行を1日当たり5度受け、その逃げ穴は3日間同じ場所のままにする。図61Aは、PPF1で処理したマウスは生理食塩水またはYPで処理したマウスよりも明らかにに速く逃げ穴を見つけることを示し、認知機能の改善を示す。図61Bは、Barnes迷路の結果に関する追加的な手段として、14回目及び15回目の試行についての平均脱出時間を示し、ここで、PPF1で処理した動物はYPまたは生理食塩水対照で処理したマウスよりも記憶が改善する強い傾向があることも示している。
PPF1または生理食塩水対照を3ヵ月齢、6ヵ月齢及び10.5ヵ月齢の免疫不全マウスに投与した(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)。マウスは、150μLの生理食塩水対照またはPPF1のIV注射を連続7日間受け、ダブルコルチン免疫組織化学によるニューロン新生の分析のために6週間後に夫々が4.5ヵ月齢、7.5ヵ月齢及び12ヵ月齢で屠殺した。ダブルコルチンは新生ニューロンを標識し、歯状回における新生ニューロンの数を定量することを可能にする。図62Aは、ダブルコルチン(DCX)陽性新生ニューロンの数に年齢依存性の減少が見られることと、PPF1処理が7.5ヵ月齢及び12ヵ月齢の動物において明らかにDCX陽性細胞の数を増加させることとを示す(4.5ヵ月齢、生理食塩水n=8、PPF1n=8;7.5ヵ月齢、生理食塩水n=8、PPF1n=6;12ヵ月齢、生理食塩水n=15、PPF1n=14)。また、4.5ヵ月齢のマウスにDCX陽性細胞の数の増加傾向が見られた。図62Bは、同じデータを異なる態様でとらえ、投与時のニューロン新生のレベルと屠殺時のニューロン新生のレベルとを比較する。このグラフによって、PPF1処理が7.5ヵ月齢及び12ヵ月齢のマウスの年齢依存性のニューロン新生の減少から救出し、夫々の投与時の6ヵ月齢及び10.5ヵ月齢のマウスと同じレベルのDCX陽性細胞の数に回復または維持することが分かる。
PPF1または生理食塩水対照を6ヵ月齢の免疫不全マウス(NOD.Cg−Prkdscid Il2rgtm 1 Wj1/SzJ,「NSG」系統)に投与した。マウスは、150μLPPF1(n=10)または生理食塩水(n=15)のIV注射を連続7日間受け、最後の投与の24時間後に分析のために屠殺した。活性化したミクログリアを標識するCD68免疫組織化学を使って、海馬の炎症性変化を定量した。図63は、生理食塩水(n=10)及びPPF1(n=10)で処理したマウスの海馬におけるCD68陽性領域の定量化を示す。CD68レベルに明らかな低下が見られ、このことは、これらのマウスの脳における炎症の低下を示唆する。
PPF1または生理食塩水対照を24ヵ月齢の野生型マウス(C57BL/6,“wt”)に投与した。マウスは、150μLのPPF1(n=10)または生理食塩水(n=10)のIV注射を連続7日間受け、最後に投与してから10日後に分析のために屠殺した。ミクログリアのマーカーであるCD68及びIba−1の組織学的な分析を、Hamamatsu slidescannerによって取り込んだ免疫蛍光画像を使って行った。図64A及び図64Bは、生理食塩水またはPPF1で処理したマウスのCD68(図64A)及びIba−1(図64B)についての代表画像を示す。両マーカーは、PPF1で処理したマウスの脳において減少し、年齢依存性のミクログリアの活性の減少とそれによる炎症の低下とを示唆する。
6ヵ月齢(図65A)または3ヵ月齢(図65B)の何れか一方で開始する2度の強化パルス投薬計画をNSGマウスに施す。当初6ヵ月齢で静脈を通して処理されたマウスのコホートは、2度の連続7日間の強化投薬を8週間隔で受けた。当初3ヵ月齢で静脈を通して処理されたマウスのコホートは、3度の連続7日間の強化投薬を8週、8週、及び7週間隔で受けた。両方の群に関して、行動試験を最後のパルス投薬計画から6週後、マウスのすべてが12ヵ月齢になったときに行った。
初代マウスの皮質神経細胞(図69A)及び海馬神経細胞(図69B)は、対照治療、rhアルブミン、PPF1(2つの異なるロット)及びHAS1の存在下で維持された。軸索突起の長さは96時間の培養後に評価された。両方のタイプの神経細胞をPPF1またはHAS1で処理することによって、対照処理と比べて神経突起伸長が明らかに促進する結果となる。PPF1またはHAS1での処理はrhアルブミンでの処理と比べても神経突起伸長の促進を増大する結果となる。神経突起伸長は、新生ニューロンが神経ネットワークに接続および一体化する前に生じる関連的な生理的過程である。N=11〜16独立した実験±SEMであって、1元配置ANOVA *p<0.05、**p<0.01である。
第2相臨床試験をアルツハイマー病患者にPPF1を使って実施した。主たる選択規準:年齢60〜90;米国国立老化研究所/アルツハイマー病協会(NIA-AA)の基準に従うprobable AD(参照によって本明細書に組み入れられるJack CR et al.,Alzheimers Dement.,14(4):535−62(2018));ミニメンタルステート検査(MMSE)12〜24点である。主たる除外基準規準は、AD以外の如何なる変性神経疾患;>2磁気共鳴画像法(MRI)でのラクナ梗塞;最後の3ヵ月のコリンエステラーゼ阻害薬またはメマチンの投与における変化である。対象は、夫々ベースライン来診を受け、総期間で6ヵ月の間に2度のの5日間投与期間(IV)とその各々に伴う3ヵ月の無治療期間とを有する。対象を1:1の割合で無作為化しPPF1を100mLまたは250mLだけ受けさせ、投与の割り当ては対象、介護者、査定人及び研究者に対して知らせなかった。プラセボ対照群はなかった。主要評価項目は安全性及び忍容性である一方、副次評価項目にはアルツハイマー病評価スケール・認知サブスケール(ADAS−Cog)、臨床認知症尺度(CDR)、アルツハイマー病共同研究の日常生活動作評価尺度(ADCS−ADL)、MMSE、及びサボニックスコグニティブバッテリが挙げられた。予備的な評価項目には、血液及び脳脊髄液の生物マーカー、及び構造的及び機能的MRIが挙げられた。研究は2018年3月〜2019年3月の期間アメリカ合衆国の9つの場所で行われた。
Claims (22)
- 認知障害と診断された対象を治療する方法であって、
パルス投与される投薬計画を用いて有効量の血漿画分を投与することを含む、方法。 - 前記血漿画分が血漿タンパク質画分である、請求項1に記載の方法。
- 前記血漿タンパク質画分が83%〜95%の間のアルブミンを含む、請求項2に記載の方法。
- 前記血漿タンパク質画分が市販の血漿タンパク質画分である、請求項3に記載の方法。
- 前記血漿画分がタンパク質を濃縮した血漿タンパク質製剤である、請求項1に記載の方法。
- 前記血漿画分が若齢個体のプールからの血漿に由来する、請求項1〜5のいずれか一項に記載の方法。
- 前記血漿画分が哺乳類の血液製剤から製造される、請求項1〜6のいずれか一項に記載の方法。
- 前記哺乳類の血液製剤がヒト血液製剤である、請求項7に記載の方法。
- 認知障害と診断された対象を治療する方法であって、
パルス投与される投薬計画を用いて有効量の若齢血漿を投与することを含む、方法。 - 認知機能の改善について前記対象をモニターすることをさらに含む、請求項1〜9のいずれか一項に記載の方法。

- 前記対象が、前記パルス投与される投薬計画が完全に投与された後、運動負荷計画に従う、請求項1〜14のいずれか一項に記載の方法。

- 前記投与は前記対象のシナプスの統合性を増加させる、請求項1〜17のいずれか一項に記載の方法。
