Abstract
Description
本願は、2018年3月6日出願の米国特許出願第62/639,282号の利益を主張するものであり、その開示内容は、その全体があたかも本明細書に陳述されているかのように、参照により本明細書に組み込まれる。
本開示は、植込み型整形外科用デバイスに関し、より具体的には、脊椎を安定させるための植込み型デバイスに関する。更により具体的には、本開示は、縮小されたサイズを有する第1の挿入構成から、拡張されたサイズを有する第2の植込み構成へのケージの拡張を可能にする一体型ラチェット組立体を備える角度調節式椎間ケージに関する。椎間ケージは、脊椎の矢状面のバランス及びアライメントを回復させながら、前弯角度、特により大きな弯曲角度を調節及び適応させるように構成されている。
(1) 拡張可能な脊椎用インプラントであって、
第1の椎体の終板に接して配置されるように構成された上部プレートと、隣接する第2の椎体の終板に接して配置されるように構成された下部プレートと、を備えるハウジングと、
前記ハウジング内にあり、前記脊椎用インプラントの角度調節をもたらすように構成されている一体型ラチェット組立体であって、シャフトと、前記シャフトによって支持される作動部材と、前記シャフトから外に延在する複数のフランジと、前記シャフトを受容するように構成された拡張可能な開口部を有するスリーブと、を含む一体型ラチェット組立体と、を備え、
前記シャフトが前記スリーブから移動することにより、前記作動部材が前記上部プレート及び前記下部プレートを圧迫し、また前記上部プレート及び前記下部プレートが互いに対して角度をつける、拡張可能な脊椎用インプラント。
(2) 前記ハウジングは、前記上部プレート及び前記下部プレートに付勢力を加えるように構成された少なくとも1つのばね部材を更に含む、実施態様1に記載の拡張可能な脊椎用インプラント。
(3) 前記ばね部材は、前記上部プレート及び前記下部プレートの各々に接続された変形可能なストリップとして構成されている、実施態様2に記載の拡張可能な脊椎用インプラント。
(4) 前記上部プレート及び前記下部プレートは、円錐ヒンジにおいて前記ハウジングに接続されている、実施態様1に記載の拡張可能な脊椎用インプラント。
(5) 前記ハウジングは器具受容開口部を含む、実施態様1に記載の拡張可能な脊椎用インプラント。
(7) 前記ハウジングは1つを超える作動部材を更に含む、実施態様1に記載の拡張可能な脊椎用インプラント。
(8) 前記インプラントは、後方経路腰椎椎体間固定(posterior lumbar interbody fusion)(PLIF)用に構成されている、実施態様1に記載の拡張可能な脊椎用インプラント。
(9) 前記インプラントは、前方経路腰椎椎体間固定(anterior lumbar interbody fusion)(ALIF)用に構成されている、実施態様1に記載の拡張可能な脊椎用インプラント。
(10) 拡張可能な脊椎用インプラントであって、
第1の椎体の終板に接して配置されるように構成された上部プレートと、隣接する第2の椎体の終板に接して配置されるように構成された下部プレートと、を備えるハウジングと、
前記ハウジング内にあり、前記脊椎用インプラントの角度調節をもたらすように構成されている一体型ラチェット組立体であって、前記上部プレートと前記下部プレートとを接続する弾性変形可能なプレートを備え、前記弾性変形可能なプレートは、ラチェットシャフトと解放可能に係合するように構成された縁部と、前記ラチェットシャフトを捕捉するようにサイズ決めされた軟質開口部を有するスリーブと、を有する、一体型ラチェット組立体と、を備え、
前記スリーブから前記ラチェットシャフトを解放することにより、前記上部プレート及び前記下部プレートは離れるように移動し、また前記プレートの互いに対する角度調節が引き起こされる、拡張可能な脊椎用インプラント。
(12) 前記多孔性表面は、前記上部プレート及び前記下部プレートのうちの少なくとも一方の上にある、実施態様11に記載の拡張可能な脊椎用インプラント。
(13) 前記インプラントの先端部は先細になっている、実施態様10に記載の拡張可能な脊椎用インプラント。
(14) 前記ハウジングは骨グラフト窓を更に含む、実施態様10に記載の拡張可能な脊椎用インプラント。
(15) 前記ラチェットシャフトは、前記上部プレート及び前記下部プレートのうちの少なくとも一方を圧迫して前記上部プレート及び前記下部プレートのうちの他方から離すように構成された係合部材を支持する、実施態様10に記載の拡張可能な脊椎用インプラント。
(17) 前記上部プレート及び前記下部プレートは、前記係合部材と係合するように構成された段を更に含む、実施態様15に記載の拡張可能な脊椎用インプラント。
(18) 前記インプラントは、後方経路腰椎椎体間固定(PLIF)用に構成されている、実施態様12に記載の拡張可能な脊椎用インプラント。
(19) 前記インプラントは、前方経路腰椎椎体間固定(ALIF)用に構成されている、実施態様12に記載の拡張可能な脊椎用インプラント。
Claims (15)
- 拡張可能な脊椎用インプラントであって、
第1の椎体の終板に接して配置されるように構成された上部プレートと、隣接する第2の椎体の終板に接して配置されるように構成された下部プレートと、を備えるハウジングと、
前記ハウジング内にあり、前記脊椎用インプラントの角度調節をもたらすように構成されている一体型ラチェット組立体であって、シャフトと、前記シャフトによって支持される作動部材と、前記シャフトから外に延在する複数のフランジと、前記シャフトを受容するように構成された拡張可能な開口部を有するスリーブと、を含む一体型ラチェット組立体と、を備え、
前記シャフトが前記スリーブから移動することにより、前記作動部材が前記上部プレート及び前記下部プレートを圧迫し、また前記上部プレート及び前記下部プレートが互いに対して角度をつける、拡張可能な脊椎用インプラント。 - 前記ハウジングは、前記上部プレート及び前記下部プレートに付勢力を加えるように構成された少なくとも1つのばね部材を更に含む、請求項1に記載の拡張可能な脊椎用インプラント。
