Abstract
Description
[動物と露光]
実施したすべての実験は、Use of Animals in Ophthalmic and Vision Research(眼病及び視覚研究における動物の利用)についてのARVO基準に準拠したものである。生後年齢130〜160日のアルビノのスプレーグ・ドーリー(Sprague−Dawley,SD)ラットを、1000ルクスの明るい連続光(Bright Continuous Light,BCL)に曝した。ラットの出生及び飼育は、約5ルクスの環境光レベルを有する薄暗い周期光条件(12時間の照明:12時間の暗闇)で行った。BCLへの曝露は、生後日数(P)90〜150日である年齢の動物に対して実行した。BCL曝露前に、ラットは、最低15時間に渡って暗闇に慣らされ、その後、妨げられずに光が進入できるように設計された各ケージに移された。ケージ内に影の領域は存在したいため、瞳孔拡大は生じなかった。BCL曝露は、常に午前9時に開始し、ケージ上方に配置された冷白色蛍光光源(18W、冷白色、TFC)を用いて、ケージ床面での約1000ルクスの光度で実行した。BCL曝露は24時間の期間に渡って続けられ、24時間の経過後、動物は直ちに後露光期間の薄暗い周期光条件に戻された。BCL曝露の後、動物は最大で56日のあいだ薄暗い光条件下に置かれた。
動物は、腹腔内注射によって投与されるバルビツール酸塩の過量(体重1kg当たり60mg、バラバーブ、ビルバックアニマルヘルス(Virbac Animal Health)、リージェンツパーク、ニューサウスウェルズ、オーストラリア)で安楽死させた。各動物の左眼は、定位のために上面に印を付けてから摘出し、凍結切片化用に処理した。右眼の網膜は、角膜切開によって摘出し、RNA抽出用に準備した。
マイクロアレイ解析は、過去の研究(ネィトリ・アール(Natoli R.)、ズー・ワイ(Zhu Y.)、ヴァルター・ケー(Valter .K)、ビスティ・エス(Bisti S.)、エリス・ジェイ(Eells J.)ストーン・ジェイ(Stone J.)、Gene and Noncoding RNA regulation underlying photoreceptor protection:microarry study of dietary antioxidant saffron and photobiomodulation in rat retina(光受容体保護の基礎となる遺伝子及びノンコーディングRNA調節:ラット網膜における食事療法の抗酸化サフラン及び光生体調整のマイクロアレイ研究)Mol Vis.2010;16:1801−1822)から得たマイクロアレイのローデータを利用し、ラット遺伝子マイクロアレイ(Rat Gene 1.0 ST;アフィメトリクス(Affymetrix)、カリフォルニア州サンタクララ)を用いて実行した。マイクロアレイデータの全セットは、アクセス番号GSE22818で、NCBI(National Center for Biotechnology Information(国立生物工学情報センタ)、National Institutes of Health(国立衛生研究所)、メリーランド州ベセスダ)の遺伝子発現情報データベース(Gene Expression Omnibus)レポジトリに預けられている。解析では、薄暗飼育群及び24時間BCL実験群(それぞれn!3)からの試料を比較した。マイクロアレイデータを解析し(Partek Genomics Suite 6.4ソフトウエア、パーテック社(Partek Inc.)、ミズーリ州セントルイス)、プローブ配列及びGC含量(GC−RMA)に合わせて調節されたRobust Multiarray Average(RMA)アルゴリズムを用いて、バックグラウンド補正、正規化、要約の機能を有するソフトウエアにCELファイル(アフィメトリクス)をインポートした。処理された値は、エクソン(exon)コード配列を表す各プローブセットとして表示し、底を2として対数変換した。差次的発現解析は、有意水準P“0.05として分散分析(Analysis Of Variance,ANOVA)の統計手法を用いて実行した。結果的に得られた差次的発現データの不均度は、ユークリッド距離計量及び主成分分析(Principle Component Analysis,PCA)を用いて(いずれもパーテック製Genomics Suiteによって提供される)、凝集型階層クラスタリングで評価した。次に、差次的発現データは、ソフトウエア(Partec GS Genomics Suite)によって提供される遺伝子オントロジー(Gene Ontology,GO)エンリッチメントによる機能解析を利用して、遺伝子オントロジー協会(Gene Ontology Consortium)によって記載された生物学的過程に従ってクラスタ化した。この後、差次的に発現した遺伝子のリストから補体カスケードに関連のあるものを選別したが、その際に、#50%の差次的発現カットオフを用いると共に、遺伝子オントロジー協会で集計された経路情報及びHUGO Gene Nomenclature Commitee(HUGO遺伝子命名法委員会)から得た遺伝子グループを補助的に用いた。
製造会社の指示に従って、(SuperScript III Reverse Transcriptaseキット、カタログ番号18080−044、インヴィトロジェン)としてファーストストランドcDNA合成を行った。20−μL反応混合物を、1μgのRNA、500ngのオリゴ(dT)18プライマ、及び200U逆転写酵素と組み合わせて使用した。遺伝子増幅は、市販の加水分解プローブ(タックマン(TaqMan)、アプライドバイオシステム社(Applied Biosystems,Inc.)[ABI]、カリフォルニア州フォスターシティ)又はカスタム設計プライマを用いるサイバーグリーン(SYBR Green)法のいずれかを使用して測定した。詳細は表2及び表3にそれぞれ示した。加水分解プローブは、従来の定量PCR(qPCR)プロトコルに従って適用した。サイバーグリーン法での定量PCR用プライマ(表3)は、Primer3ウェブベースの設計プログラム(ローゼン(Rosen)他を参照)を用いて、イントロンを横断するコーディングドメイン配列内で設計した。定量PCRは、市販の定量PCTシステム(StepOnePlus,ABI)を使用して実行した。各生体試料は、実験をトリプリケートで行って増幅し、その後、平均定量サイクル(Quantitation Cycle,Cq)値を用いて、発現の変化率を特定した。両方の定量PCR(タックマンとサイバーグリーン、ABI)において、薄暗飼育試料と比較したパーセント変化をCq法で特定した。対象遺伝子の発現を参照遺伝子のグリセルアルデヒド−3−リン酸デヒドロゲナーゼ(DHDH)の発現に対して正規化したが、本研究においても又は過去の光誘発網膜障害調査においても差次的発現は示されなかった。増幅の特異性はゲル電気泳動法を用いて評価した。一元配置分散分析(One−Way ANOVA)を用いて統計解析を行い、発現傾向の有意性を評価した。P“0.05を有する差を統計上有意であるとした。
BCL後の網膜におけるC3mRNA転写の局在化を研究するため、網膜凍結切片でのin situハイブリダイゼーションのためにC3のリボプローブを生成した。C3は、ラット網膜から用意(前述したように用意)したcDNA、pGEM−T DNAベクター系(Promega、ウィスコンシン州マディソン)、及びTOP10コンピテントセル(One Shot、インビトロゲン)を用いて、PCR産物(483−bpアンプリコン)からコピーした。DIG RNAラベリングキット(SP6/T7,Roche,バーゼル、スイス)を使用して、線状化プラスミドを転写し、DIG標識されたアンチセンスリボプローブ及びセンスリボプローブを生成した。前述したプロトコルでin situハイブリダイゼーションを行い、一晩かけてC3リボプローブを57℃でハイブリダイズし、その後、60℃のクエン酸ナトリウム塩水(pH7.4)で洗浄した。ハイブリダイゼーション後、一部の切片を免疫組織化学的に染色した(これについては後述する)。
BCLの実行中及び実行後に、TUNEL標識を使用し、既に公開されているプロトコルで凍結切片における光受容体のアポトーシスを定量化した。外顆粒層(Outer Nuclear Layer,ONL)内のTUNEL陽性細胞の個数は、1000%1000!mの広さの隣接フィールドにおいて、パラサジタル(上下(superioinferior))面で切断された、視神経乳頭を含む網膜切片の全長に沿って計数した。各動物の最終計数値は、2つの不連続切片内の同等の位置における平均である。統計解析は、一元配置分散分析によって実行した。P“0.05である差異は、統計的に有意であるとした。
各時点の凍結切片は、補体C3(1:50;Abcam、マサチューセッツ州ケンブリッジ)、C3d(1:100;R&D Systems、ミネソタ州ミネアポリス)、ED1(1:200;Millipore、マサチューセッツ州ビルリカ)、及びIBA1(1:1000;Wako、大阪、日本)の一次抗体を用いる免疫組織化学解析に使用した。免疫組織化学法は前述した方法体系で実行した。免疫蛍光は、レーザ走査顕微鏡(カールツァイスメディテックインコーポレイテッド)で視認して、PASCALソフトウエア(バージョン4.0、カールツァイスメディテックインコーポレイテッド)を用いて取得した。画像は、掲載用に画質を向上させた(Photoshop、Adobe、カリフォルニア州サンノゼ)。この画質向上処理は画像間で統一した。
図1に、in vivo実験の結果として、コルチコイドステロイド治療(左側:TA、右側:FA)を行った場合、又は行わなかった場合(ケナログ賦形剤のみ)について、細胞死の測定値(TUNEL)、マクロファージ斬増(IBA1)、及び光酸化損傷後の機能(ERG)を示す。
臨床試験用医薬品、投与分量及び投与経路:フルドロコルチゾン酢酸エステル(FA)1mg/0.1ml&2mg/0.1ml、硝子体内(IVT)注射。同一のプロトコルをTAにも適用。
動物研究を行って、硝子体腔内への硝子体注射後のフルドロコルチゾン酢酸エステルの安全性及び効力を評価した(キヴィルシン(Kivilcim,M.)他、Evaluation of the Retinal Toxicity of Fludrocortisone Acetate After Intravitreal Injection(硝子体内注射後のフルドロコルチゾン酢酸エステルの網膜毒性評価)、Investigative Ophthalmology&Visual Sience,2006年、47(13):p.4281−4281を参照)。外科治療は、25匹のニュージーランドホワイトラビットの眼に対して実行した。フルドロコルチゾン酢酸エステルは、滅菌BSS溶液を用いて、4mg/0.1ml,2mg/0.1ml,1mg/0.1ml,400μg/0.1mLの濃度になるまで滴定して、25匹のラビットの眼の片眼において硝子体内に注射した。コントロール用の眼に、0.1mlの滅菌BSSを与えた。注射前及び注射後に、倒像顕微鏡及びスリットランプ生体顕微鏡検査法を用いて全ての動物を検査した。網膜電位図(ERG)検査は、硝子体内注射の前、及び注射の2週間後に全ての動物に実行した。2週間後の検査時に、倒像顕微鏡検査及びスリットランプ生体顕微鏡検査法で再検査して、動物を安楽死させた。眼を摘出して光学顕微鏡で検査した。
<本研究の論理的根拠>
フルドロコルチゾン酢酸エステルは、PBSから錠剤形式(0.1mg)で入手でき、オーストラリアではヒトへの使用が承認されている。121人の患者の調査で、前部病変にフルドロコルチゾンを用いた良好な結果が報告された(ゴンザレス(Gonzalez,C.)による「Topical fludrocortisone(9−alpha fluorohydrocortisone)in ophthalmology(眼下における局所フルドロコルチゾン(9α−フルオロヒドロコルチゾン)」、Am J Ophthalmol、1960年、49:p.619−22を参照)。この文献には、同様の経験を有する4つの過去の論文が引用された。
本研究では、投与された1mg/0.1ml又は2mg/0.1ml注射の単回投与量について検査する。
この試験実施計画書で採用した安全性モニタリング手法(例えば、全眼科的検査、IOPのモニタリング、OCT、バイタルサイン、血液学検査、血液化学的検査、及びAE質疑)は、被験者の安全を守る十分なものである。
<研究目的>
本研究の第1の目的は、加齢黄斑変性に伴うGAを有する被験者へのフルドロコルチゾン酢酸エステルのIVT注射の安全性及び認容性を評価して、確認の第二相への更なる発展を支援することである。
<プライマリ安全エンドポイント>
眼底自発蛍光(FAF)によって測定されるGA病変サイズの平均変化に基づいて、フルドロコルチゾン酢酸エステルIVT注射の安全性及び認容性を実証すること。治療で発生した局部性及び全身性の有害事象の数及び深刻度。
FAFによって測定される地図状萎縮(GA)病変サイズの平方根の第6月までのベースラインからの変化。低輝度での最良矯正視力(LL−BCVA)の変化。GA病変サイズ変化とBCVA変化との関係。バイタルサインの変化。IOPの上昇。薬物動態(PK)パラメータ。フルドロコルチゾン酢酸エステルIVT注射の単回投与後の曝露。最高血漿中濃度。最高測定濃度までの時間。消失半減期。
本研究の母集団は、ほぼ1つの施設で登録される約9名(n=9)の被験者を含む。本研究に参加するために、被験者は、両眼にAMDに伴う黄斑GAを有すると診断されなければならない。両方の眼が基準を満たし本研究に適合する場合、スクリーニングビジット時に最良視力を有する眼が調査眼に指定される。両眼の視力が同一である場合は、右眼を調査眼として使用される。
WOCBPは、妊娠することが身体的に可能な閉経前の女性と定義される。
WONCBPは、下記の基準のいずれかを満たす女性と定義される。基準:年齢が45より上で無月経が2年を超えているか、又は年齢が60より上で無月経が1年を超えていることがFSHレベル及びLHレベルによって確認されていること。子宮摘出手術を受けていること。両側卵巣摘出手術を受けていること。両側で卵管切除を受けていること。
承認済の避妊法は、経口避妊薬、子宮内避妊具、医薬的に許容されるバリア法(例えば、コンドーム)、埋め込み可能又は注射可能な避妊薬、又は取り外し可能な避妊具を含む。節制及び中絶性交(膣外射精)を実践している被験者は、研究中、承認済の避妊法を利用することに同意しなければならない。
<治療配分>
スクリーニングの前に、各被験者に一意のスクリーニング番号が割り当てられる。研究スクリーニング評価が完了して全ての適格基準を満たした被験者は、研究への登録が予定され、0日目に、硝子体内フルドロコルチゾン酢酸エステルの治療を受ける。
<投与量レベル及び研究部門>
本研究では、0.1mLにつき1mg及び2mgのフルドロコルチゾン酢酸エステルの単回投与量について検査する。
<臨床試験用医薬品の情報>
フルドロコルチゾン酢酸エステルは、硝子体内投与のために注射溶液用の20mg粉末として調製される。
IVTフルドロコルチゾン酢酸エステル製剤は、研究施設において被指名人に提供され、薬局又は他の鍵のかかる安全な場所において、アクセスが制限/管理されて温度がモニタリングされる冷蔵区域内で2℃から8℃の温度で保管されなければならず、凍結してはならない。IVTフルドロコルチゾン酢酸エステル薬剤は、提供されたボール箱内に保管して、光から保護しなければならない。製剤供給部にアクセスできるのは、PIによって権限を与えられた人物のみである。スポンサーは、本研究を完了できる十分な量のフルドロコルチゾン酢酸エステル製剤を提供する。
積極的治療を受ける被験者は、IPの監督下で、薄肉針27Gを使用してフルドロコルチゾン酢酸エステル1mg/0.1ml又は2mg/0.1mlのIVT注射を受ける。
本研究の開始時に参加者が受け取る併用薬、又は本研究中に何らかの理由により与えられる併用薬(但し、局部的エステティック等、本試験実施計画書で必要とする眼の処置のために与えられる所定薬剤を除く)は、開始終了日時、投与量、投与経路、及び指示と共に、原文書及びCRF(症例報告書)に記録しなければならない。また、外科的措置等、眼及び眼以外の全ての処置(研究治療措置以外)も、開始終了日時と共に原文書に記録しなければならない。外科的なエステティック治療、医療補助者による措置、又は代替医療(例えば、針、マッサージ)も原文書及びCRFに記録しなければならない。フルオレセイン注射によって誘発される吐き気を防ぐメトクロプラミド又は他の薬品は、PIの監督下において投与されてよい。
眼内炎又は疑似眼内炎の患者を治療する決定は、PIの臨床的判断によって指導される。治療方法(経毛様体扁平部硝子体切除、硝子体穿刺)及び抗菌剤の選択もPIの指示になるが、現在の標準的慣習パターンに従う必要がある。眼内炎の治療にIVTステロイド(例えば、デキサメタゾン)を使用するかどうかの決定もPIの指示になる。
<研究企図>
これは、加齢黄斑変性に伴うGAを有する被験者へのフルドロコルチゾン酢酸エステルのIVT注射の単回投与量の安全性及び認容性を評価する第Ib相の研究である。
調査官の責任において、登録前と研究期間中に、被験者が本研究への参加に適していることを確認する。スクリーニングプロセスを開始する前に、本研究専用のICF(説明承認同意文書)を用いて作成される、各被験者自身によって署名されて日付が記入されたインフォームドコンセント文書が必要である。説明承認同意書を取得して参加適格性が確定した後、試験施設の担当者が被験者の識別番号を取得する。適格な被験者のみに、ラベル付きで非盲検用の1mg/0.1mL又は2mg/0.1mLのFAが配布される。
下記は、研究ビジット、並びに実行される処置及び審査の要約である。月次のビジットスケジュールに関する処置/評価の詳細スケジュールについては、活動スケジュール(Schedule Of Activities,SoA)テーブルを参照されたい。PIの裁量で必要と判断とされた場合は、本明細書に列挙していない追加の安全評価を行ってよい。
ビジット1−全被験者:全ての眼科処置(撮像を含む)を両眼に対して実行する。
追跡調査ビジット−第7日、第14日、第28日、第60日、第90日:特に指定のない限り、全ての眼の処置(撮像を含む)は、調査眼に対してのみ実行される。全ての追跡調査ビジットで実行される処置は次のとおりである。1.医療的身体状態の変化及び/又は併用薬の使用についての情報を取得する。2.バイタルサインを収集する。3.PK解析用の血液を採取する(第7日、第28日、第90日のみ)。4.角膜、虹彩、前眼房、水晶体(水晶体に何らかの混濁が確認された場合は、LOCSIII)、及び水性反応(細胞及びフレア)についてのスリットランプ試験、硝子体及び網膜の拡大眼底検査、並びにIOP測定を含む完全な眼科検査を実行する。5.有害事象をモニタリングする。終結ビジット(又は早期中止)−第150日:全ての眼の処置を両眼に対して実行する。1.医療的身体状態の変化及び/又は併用薬の使用についての情報を取得する。2.検査室分析用の血液及び尿を採取し、中央研究所にサンプルを送付する。3.PK解析用の血液を採取する。4.バイタルサインを収集する。5.尿妊娠検査を実行する。但し、WOCBPのみ。
被験者が次に予定されているビジットの前に、有害事象の評価のため、又はPIの要求により臨床施設に戻る場合、予定外ビジットで完了した全ての評価は、患者の原記録簿及びeCRF内に記入する必要がある。
参加者が予定されているビジットに1回以上現れず、研究施設の担当者の連絡に対して参加者から何の応答もない場合、当該参加者を追跡調査の対象から外すことが検討される。
活動スケジュールに概説された本研究中に、下記の評価を行う。
各施設の主任調査官は、研究の性質、目的、潜在的リスク、及び潜在的利益についての情報が、口頭及び書面において十分かつ完全に被験者に与えられていることを確認する。被験者には、任意の時点で自由に本研究を中断できることも通知しなければならない。被験者には、質問の機会と、提示された情報を検討する時間的猶予とが与えられなければならない。
注射ビジット時に、投与前の1時間以内及び投与後の30分以内にバイタルサインを測定する。
血液と尿の採取を研究施設で行い、分析のためにサンプルを中央研究所に送付する。
尿妊娠検査は、研究フローチャートに概説されている通りWOCBPにのみ実行する。
有資格の視力検査員によって実行される最良矯正視力(LL−BCVAを含む)検査は、眼球を拡大する点眼薬の投与を必要とする全ての検査、又は眼との接触を必要とする全ての検査より前でなければならない。ETDRSによる最良矯正視力(BCVA)は、スクリーニング時(第1ビジット)において各眼について個別に取得する。この評価は、瞳孔拡張前に実行すべき評価である。正しく読めた文字数(各眼)を適切な研究文書に記録する。研究ビジット(第2〜第8ビジット)の注記事項として、BCVAは、調査眼においてのみ取得する。
完全な眼科検査は、眼及び付属器の外部検査、眼瞼/瞳孔反応(眼瞼下垂症、異常な瞳孔形状、瞳孔間の不均等、光に対する異常な反応、及び求心性瞳孔障害を含む)の所定スクリーニング、スリットランプ検査(角膜、前眼房、虹彩、水晶体、水性反応(細胞及びフレア))から成る。いずれのビジットにおいても、スリットランプ検査中に水晶体に異常が見つかった場合は、その異常についてLOCSIIIを用いて詳しく調べなければならない。異常が見つかった被験者の以降の全ビジットにはLOCSIIIを含めなければならない。LOCSIIIの標準的手順及び評価尺度の完全な説明については、MOPに概説されている。拡大眼底検査は、網膜及び硝子体(すなわち、後区異常、網膜出血/剥離、及び硝子体出血密度及び硝子体細胞)の評価、硝子体出血密度及び硝子体細胞の段階評価を含む。眼圧(Intraocular Pressure,IOP)は、第1ビジットで、研究施設の通常の慣例に従って両眼において測定し、該当する研究文書に記録する。研究ビジット(第2〜第9ビジット)の注記事項として、IOPは、調査眼においてのみ取得する。
前述のビジットスケジュールに概説されるように下記の眼の画像を取得する。SOAも参照されたい。取得する画像は、デジタルカラー眼底写真、蛍光眼底造影、スペクトル領域光干渉断層撮影、眼底自発蛍光、赤外線反射率の画像である。指定の臨床施設においてのみハイデルバルグスペクトラリス(Hidelberg Spectralis(登録商標))システムを用いて実行する。
注射の前及び後に調査眼を評価して、注射処置及び/又は治験薬が眼の健康を危険にさらしていないことを確認する。最初の注射後評価は、注射後15分以内に行わなければならず、この注射後評価には視力(光覚)の総合評価及びIPOのモニタリングが含まれる。被験者が総合視力検査に合格し、IOP<30mmHgである場合、被験者は施設を退出できる。被験者が総合視力検査で不合格及び/又はIOP>30mmHgとなった場合は、被験者が総合視力検査に合格し、IOPが30mmHgとなるまで約30分ごとにこの評価処理を続行する。
研究中(第150日まで)の血液量については表6を参照されたい。
PI又はPIの医療協力者のうちの1人によって観察された、参加者から自発的に報告された、又は直接質疑において報告された有害事象(AE)を全てレポートする。全ての有害事象(眼、眼以外、重篤、非重篤、自発的、及び誘発的な有害事象)は、研究記録に記載しなければならない。
有害事象は、医薬製品を受け取った被験者における不都合な医療的発生事象である。この発生は、必ずしも治療と因果関係がある必要はない。したがって、AEは、薬剤に関連があると考えられるか否かに関わらず、薬剤の使用に伴って一時的に生じる好ましくない非企図の兆候、症状、又は疾病である。
重篤有害事象(Serious Adverse Event,SAE)は、不都合な医療的発生事象であり、いずれかの投与量において死に至る事象;生命を危うくする事象(但し、この事象は、当該事象の発生時において被験者が死亡する危険が実際にあったものを意味し、より深刻な形で生じた場合に死に至った可能性のある事象を意味するものではない);入院加療又は現在の入院加療の延長が必要になる事象;持続的若しくは深刻な無能力に至る事象;通常の生活機能を遂行する能力の実質的な破綻に至る事象;又は先天性異常若しくは先天性欠損として定義される。
特に関心のある有害事象は、スポンサーの製品又はプログラムに固有の科学的医療懸案事項の1つであり、調査官による持続的モニタリング及びスポンサーとの迅速な意思伝達が必要になり得る。これらの有害事象は、重篤である場合又は重篤でない場合がある。該当する有害事象では、事象を特定して理解するための更なる調査が必要になり得、事象の性質によっては、治験スポンサーから第三者への迅速な意思伝達も必要になり得る。特定関心事の有害事象は、重篤事象に関して記載したものと同じ方式且つ同じ時間枠(すなわち、調査官又は委員による事象の把握から1作業日以内)で報告しなければならない。特定関心事の有害事象は、眼内炎;4+の眼炎症;事象の発生から30日以内に1+以下に低下しない2〜3+の眼炎症;FA注射後の光覚損失の持続(5分を超える持続);注射後90分/90分以降のIOPの継続的上昇(30mmHg);外科的介入(すなわち穿刺)を必要とするIOPの上昇;事象の発生から14日以内に消失しない2+重要度を上回る新たな硝子体出血;白内障の進行が挙げられる。
調査官は、被験者のビジット中に毎回、被験者との対話によりAEの発生を厳密に調査して、施設の原文書にその情報を記録する。各AEについて、PIは、開始及び解決の日付、深刻度、SAEの定義を満たすかどうかの判断、治験薬と事象との関連性、治験薬に関して遂行された行動、及び事象の結果を記述しなければならない。データは、CRFの指示通りに原文書からCRFに複写しなければならない。
PIは、報告された有害事象全ての深刻度を下記の5つの区分のいずれかに等級分けしなければならない。5つの区分は、第1等級(軽度)、第2等級(中度)、第3等級(重度)、第4等級(生命の危険)、及び第5等級(死に関わるAE)である。適合するCTCAE用語を利用できる場合は、報告された特定タイプの有害事象についての標準CTCAE深刻度等級尺度を使用しなければならない。標準の等級尺度を参照できない場合、又は即時の参照が可能でない場合は、下記の指標を用いるものとする。
PI(又は有資格の試験医)は、報告された有害事象の因果関係について、試験物品又は試験処置への帰属性を提示しなければならない。
全てのSAE(本明細書に定義)は、研究薬剤との関連性が決定したかどうかに関わらず、調査官がSAEを把握してから24時間以内に、電話、電子メール、又はファクシミリによってスポンサー(又は指定のメディカルモニター)に報告される。連絡先の詳細は、重篤有害事象フォームで確認できる。
<試験物品に関して予想されるAE>
本試験実施計画書において提案された投与量で予想される、調査試験薬に関連のある眼又は全体のAEは皆無である。
注射処置(開瞼器、麻酔点眼薬、散瞳薬、抗生物質点眼薬、ポビドンヨード点眼薬又は洗浄、及び結膜下注射麻酔の使用、並びにIVT針の実際の挿入を含む)に関する軽度の不快症状が予想される。これらの処置関連有害事象は、限定ではないが、赤み、軽い眼の痛み、眼刺激症状、視力障害、眼の知覚異常等を含み、本明細書に指定したように等級付けされる。
調査眼又は他眼におけるAMD疾病の明らかな進行としてPIによって判断された状態は、参加者の原文書にその状態が特定されるべきで、病変の拡大、病変の出血、病変の流体滲出、RPE裂傷、及び脂質の広範付着等の状態は、有害事象としてCRFに記録されない。他の全ての状態は有害事象として記録しなければならない。疾病進行の疑う余地のない特徴は、原文書に示されていなければならない。研究中における医療状態の通常の発達又は悪化(例えば、AMDの進行に起因する視力喪失)については、その変化が、単なる効力不足に起因するのではなく、試験物品の作用に帰属すると合理的に判断できる場合を除き、その変化自体は必ずしも有害事象を構成するものではない。
参加者は、任意の時点において任意の理由により本研究から脱退することを選択でき、その際に罰則が課せられることも、他の臨床試験実施計画への登録が禁止されることもない。
WOCBPは、適切な妊娠調節方法が利用される限り本研究から除外されない。臨床治験への登録前に、WOCBPは、治験中に妊娠を回避することの重要性、及び意図しない妊娠に関わる潜在的リスクについての助言を受けなければならない。WOCBP及びWOCBPである伴侶を有する男性は、研究期間中に受容可能な妊娠調節方法(上記に定義)を実践するように指示される。男性被験者は、第1日の投与後から最後の終了ビジットまで、精液の提供を行わないように助言される。
記述的要約は、平均値、標準偏差値、中央値、連続変数範囲、カテゴリー変数の個数、及びカテゴリー変数のパーセンテージを含む。
安全性解析:少なくとも1回の投与の治療を受けた全ての被験者は、FAの安全性評価対象に組み込まれる。
参加者の人口統計ベースライン変数(年齢、性別、民族、人種、身長、体重、及びBMI)は、記述統計を用いて集計される。性別、民族、及び人種は、回数カウント及びパーセンテージを用いて集計される。ベースライン眼評価も同様に記述式で集計される。
正規推計統計は安全性評価で実行しない。安全性解析の統計手法は、性質上、主に記述式である。フルドロコルチゾン酢酸エステル1mg/0.1mL及び2mg/0.1mLの注射は、治験薬に関連して重篤AEを示した被験者の数に基づいて、安全で認容できると考えられる。安全性は、詳細に予測定義することができない医療判断であると理解されている。ただし、一般的な指針として、臨床的に深刻な薬剤関連有害事象又は検査所見の異常が被験者に生じない場合に、被験者は投与量を認容したと考える。逆に言えば、治験薬の投与中又は投与後の追跡調査期間中に、臨床的に深刻な薬剤関連有害事象又は検査所見の異常が被験者に生じた場合、被験者は投与量を許容したと考えない。
効力性エンドポイントは、本研究の第2のエンドポイントで、完全な眼の検査を含む。
本研究について、正規中間解析は計画されていない。
本研究では、3+3法に従って、加齢黄斑変性に続発する地図状萎縮を有し、スネレン換算視力が20/32〜20/2000である参加者を最大で12名登録することが計画される。
欠測データは、通常、安全性又は効率性データに対して分配されない。
<データ収集指示>
調査官は、全ての観察内容及び他の関連データを本研究薬で治療する参加者毎に記録するように設計された適切で正確な原文書を準備して維持する。
データは検証されたデータベースに登録される。データ管理グループは、手順書通りにデータを処理する責任を有する。データベースのロックは、品質保証処理が完了したときに行われる。
研究データベースへのデータの登録後、電子化されたデータ妥当性チェックシステムが定期的に実行されてデータベースに適用される。
データベースは、所定のセキュリティプロシージャによって、無許可のアクセスから保護され、データベースと関連ソフトウェアファイルの適切なバックアップコピーが保持される。データベースは、データベースに対する各種の更新及び変更内容と共に、データベース管理者によってバックアップされる。
調査官は、要求があったときに、モニター、スポンサー(又は被指名人)の他の公認代理人、IRB/IEC調査員、及び規制当局(例えば、FDA,TGA)調査員が研究データにアクセスできるようにしなければならない。各参加者のファイルは、署名された説明承認同意文書、HIPAA許可及び同意書、並びに参加者に関する全原文書のコピーを含み、これらを含んだ各参加者のファイルが保管されなければならない。調査官は、eCRFに関する情報の入手元である原文書の信頼性及び利用可能性を確保しなければならない。
モニタリングビジットは、米国CFR21章第50,56,312節と、GCP(Good Clinical Research Practice:臨床試験実施基準)(E6)に関するICH(International Council for Harmonisation:医薬品規制調和国際会議)ガイドラインとに従って、スポンサーの代理人によって実施される。この試験実施計画書に署名することによって、調査官は、スポンサー(又は被指名人)、及び該当する規制当局から、施設内でのモニタリング及び/又は全ての該当研究文書の監査を行う権限を得る。
独立したDSMBが召集される。DSMBの役割は、本研究における治験の道義的運営を保証し、患者の安全を保護する(protect the safety interests of patients)ことである。
参加者の秘密保持のために、eCRF及びスポンサーに提出される他の文書において、全ての研究参加者は、施設番号、参加者番号、及び参加者のイニシャルのみによって識別される。追加の参加者秘密保持事項(存在する場合)は、臨床研究同意書に記載される。
本研究は、ヘルシンキ宣言(Declaration of Helsinki)、志願者の保護(Protection of Human Volunteers)(21CFR50)、治験審査委員会(21CRF56)、及び臨床調査官の義務(Obligations of Clinical Investigators)(21CFR312)に従って実施される。

試験実施計画書及び同意書は、研究開始前に各参加センタのIRB/IECによって審査及び承認される。理由の如何に寄らず、重篤有害事象は、標準運営手続き及びIRB/IECの方針に従ってIRB/IECに報告され、調査官は、研究の進行状態をIRB/IECに継続的に通知する。調査官は、規制頓首についてのIRB/IECの承認を得る。
