研究デザインおよび患者
本研究は、以下の異なる目的:1)横断的研究コホートIを用いて、痩せ被験体、肥満被験体および糖尿病被験体における循環スクシネートレベルを解析すること;2)腸内微生物叢とスクシネートとの関係性を調べること(発見コホートIIおよび検証コホートIII);3)循環スクシネートと腸内微生物叢との関連を確証すること(食事介入研究コホートIVおよび追跡調査研究コホートV)を果たすための5つの異なるサブ臨床研究(clinical sub−studies)を含んだ。
デザイン:単施設(single−point)観察研究。
参加者:91人の被験体(49人の女性および42人の男性)を横断的研究に含めた(30人の痩せ患者、41人の肥満患者および20人のT2DM患者)。肥満症を世界保健機関(WHO)の基準に従って分類した。T2DM患者を、American Diabetes Associationの基準に従って、安定したグリコシル化ヘモグロビン値によって定義される直前の6ヶ月における安定した代謝調節によって診断した。どの患者にもインスリンで治療しなかった。60%にメトホルミンを投与し、20%をスルホニル尿素で治療し、15%未満をジペプチジルペプチダーゼ−4阻害剤で治療した。被験体を、University Hospital Joan XXIIIのEndocrinology Service(Tarragona,Spain)にリクルートした。
デザイン:単施設観察研究。

参加者:9人の肥満女性(登録研究ISRCTN88315555のサブサンプル)を本研究に含めた。被験体を、University Hospital Virgen de la Victoria de MalagaのEndocrinology Serviceにおける外来診察室にリクルートした。研究開始前の3ヶ月間、研究参加者には、抗生物質治療、プロバイオティクス、プレバイオティクス、または腸管微生物叢に影響する他の任意の医学的治療を行わなかった。
デザイン:自発的観察追跡調査研究。
参加者:19人の患者を2年間追跡して、微生物叢の自発的な進化を評価した。一般的なカウンセリングを被験体に提供した。研究開始前の3ヶ月間または研究中(2年間)、19人のボランティアのいずれにも、抗生物質療法、プレバイオティクス、プロバイオティクス、シンバイオティクス、ビタミンサプリメント、または腸管微生物叢に影響する他の任意の医学的治療を行わなかった。すべての患者が、血液および糞便サンプルの採取の前、追跡調査期間の前および後に、一晩絶食した。人体測定変数および臨床変数を表4に要約する。
血液サンプルを12時間の絶食後に採取した。血清/血漿を分離し、直ちに−80℃で凍結した。血清の生化学的パラメータを2つ組で計測した。血清グルコース、コレステロール、HDLコレステロールおよびトリグリセリドを、標準的な酵素法(Randox Laboratories Ltd.,Antrim,UK)によって計測した。インスリンを、イムノラジオメトリックアッセイ(BioSource International,Camarillo,CA)によって計測した。
全RNAを、RNeasy Lipid Tissue Midi Kit(Qiagen,Hilden,Germany)を用いてSATから抽出した。全RNA量を260nmにおいて計測し、純度をOD260/OD280比によって評価した。遺伝子発現解析の場合、1μgのRNAを、Reverse Transcription System(Applied Byosistems,Foster City,CA)を用いてランダムプライマーで逆転写した。miRNA解析の場合、cDNA合成を、TaqMan MicroRNA Reverse Transcription Kit(ThermoFisher Scientific,Waltham,MA)を用いて行った。リアルタイムPCR(qPCR)を、ATGL(Hs00386101_m1)、ZAG(Hs00426651_m1)、ABHD5(Hs01104373)、HSL(Hs00193510_m1)、CD163(Hs00174705_m1)、HIF1A(Hs00153153_m1)、IL1B(Hs001749097_m1)およびCCL2(Hs00234140_m1)に対するTaqMan Gene Expression Assay(Applied Biosystems)を用いて7900HT Fast Real−Time PCR Systemにおいて行った。結果を、比較Ct法(2−ΔΔCt)を用いて算出し、ハウスキーピング遺伝子18S(Hs03928985_g1)の発現に対して表した。
16Sの配列決定(コホートIIおよびIV)
採取した糞便サンプルを直ちに−80℃で凍結した。ゲノムDNAを、International Human Microbiome Standards(IHMS;http://www.microbiome−standards.org)(Santiago et al.,2014,BMC Microbiol.14:112)の推奨に従って抽出した。各サンプルの凍結アリコート(250mg)を250mlのチオシアン酸グアニジン、40mlの10%N−ラウロイルサルコシンおよび500mlの5%N−ラウロイルサルコシンに懸濁した。ビーズを用いた微生物細胞の機械的破砕によってDNAを抽出し、透明の溶解産物からアルコール沈殿によって核酸を回収した。1mg相当の各サンプルを、分光光度計(NanoDrop Technologies,Wilmington,DE)を用いたDNA定量に使用した。DNAの完全性を、DNA12000KitとともにAgilent 2100 Bioanalyzerを用いたマイクロキャピラリー電気泳動(最大17,000bp長の二本鎖DNAフラグメントの分布を分解する)によって調べた。リボソーム16S rRNA遺伝子の配列を、16S Metagenomics Kit(Thermo Fisher Scientific,Italy)を用いてcDNAから増幅した。そのキットには、細菌における16S領域の対応する超可変領域を選択的に増幅する2つのプライマーセットを含んだ:プライマーセットV2−4−8およびプライマーセットV3−6、7−9。用いたPCR条件は、95℃10分、ならびに95℃30秒、58℃30秒および72℃20秒の30サイクルの後、72℃10分であった。各アンプリコンの濃度および平均サイズを、Quant−iT PicoGreen dsDNA Assay Kit(Invitrogen)を用いて測定した;1マイクロリットル当たりのDNAフラグメントの量を算出し、Ion Plus Fragment Library Kit(Thermo Fisher Scientific)を用いてライブラリーを作製した。Ion Xpress Barcode Adapters 1−16キット(Thermo Fisher Scientific)を用いて、各サンプルにバーコードを付加した。ライブラリー濃度を、Ion Universal Library Quantification Kit(Thermo Fisher Scientific)を用いて測定した。アンプリコンライブラリーのエマルジョンPCRおよび配列決定を、Ion Torrent S5TMシステムおよびIon 520TM/530TM Kit−Chef(Thermo Fisher Scientific)を製造者の指示に従って使用して、Ion 520チップ(Ion 520TMChip Kit)において行った。配列決定後、個々の配列リードを、Ion Reporter Software V4.0を用いてフィルタリングして、低品質の配列およびポリクローナル配列を除去した。
全DNAを、QIAamp DNA Stool Mini Kit(Qiagen,Courtaboeuf,France)を用いて凍結ヒト糞便サンプルから抽出した。品質評価を、以下のパラメータを適用するprinseq−liteプログラムを用いて行った:min_length、50、trim_qual_right、20、trim_qual_type、mean;およびtrim_qual_window、20。Illuminaの配列決定からのR1およびR2リードを、ea−tools suiteのfastq−joinを用いてつなぎ合わせた。そのfastqファイルを、FastX−Toolkitプログラムの「fastq_to_fasta」ツールを用いてfastaファイルに変換した。それらのファイルを、NCBIのFTPサイト(ftp://ftp.ncbi.nlm.nih.gov/genomes/H_sapiens/)からダウンロードしたヒトゲノムに対してフィルタリングした。アラインメントされなかったファイル、つまり、ヒトゲノムに対してマッピングしなかったファイルを、Human Microbiome、およびNCBIのFTPサイト(ftp://ftp.ncbi.nlm.nih.gov/genomes/HUMAN MICROBIOM/Bacteria/およびftp://ftp.ncbi.nlm.nih.gov/genomes/archive/old_refseq/Bacteria/)からダウンロードした細菌ゲノムからなるカスタマイズされた細菌データベース(Bacteria_2015_06_09)に対するBLASTn検索の入力ファイルとした。BLASTn出力ファイルの最良のヒットを抽出し、分割表に変換し、BIOM形式に変形して、Quantitative Insights Into Microbial Ecology(QIIME)オープンソースソフトウェアパイプラインバージョン1.9.0(Langmead and Salzberg,2012,9:357−359;Schmieder and Edwards,2011,Bioinformatics 27:863−864)の入力ファイルとして使用した。
蛍光定量的方法
循環血清/血漿スクシネートレベルを、EnzyChromTM Succinate Assay Kit(BioAssay Systems,Hayward,CA)を用いて計測した。アッセイ感度は、12μMであり、アッセイ内およびアッセイ間の変動係数は、それぞれ3.5および6.95%未満であった。
蛍光定量的アッセイによって得られた循環スクシネートレベルを、LC−MS/MSおよびNMR解析を用いて検証した。これを行うために、コホートI由来の血漿サンプルのサブサンプルを、以前に報告されたものにいくつかの修正を加えて調製した(Nagana Gowda et al.,2015,Anal.Chem.87:706−715;Tulipani et al.,2013,Anal.Chem.85:341−348)。重要なことには、蛍光定量的アッセイによって計測されたコハク酸の濃度は、LC−MS/MSによって計測されたコハク酸の濃度(r=0.617、p=0.019)およびNMRによって計測されたコハク酸の濃度(r=0.769、p=0.043)と相関したことから、蛍光定量的アッセイが、ヒトスクシネートレベルを計測するために使用でき、他の2つの方法論よりも迅速かつ経済的であることが示唆された。
血清ゾヌリンを、腸透過性の代用マーカーとして計測した。循環血漿/血清ゾヌリンレベルを、Human Zonulin Elisa Kit(MyBiosource,San Diego,CA)(Smecuol et al.,2005,Clin.Gastroenterol.Hepatol.3:335−341;Wang et al.,2000,J.Cell Sci.113 Pt 24:4435−4440)を用いて評価した。このアッセイは、ゾヌリンの検出に対して高い感度(1ng/ml)および優れた特異性を有し、このアッセイだけが、活性(未切断)型を検出する。これらの測定に対するアッセイ内およびアッセイ間の変動係数は、<10%であった。
統計解析は、Statistical Package for the Social Sciencesソフトウェア、バージョン15(SPSS,Chicago,IL)を用いて行った。臨床変数および人体測定変数については、正規分布したデータを平均値±SDとして表し、ガウス分布値を有しない変数については、中央値(25th−75th四分位数)として表した。スチューデントt検定およびボンフェローニ補正を用いて、正規分布した連続変数の平均値を比較した。ガウス分布を有しなかった変数の場合、クラスカル・ワリス検定をダンの事後多重比較検定とともに用いた。群間の名義変数の差を解析するために、χ2検定を用いた。微生物叢データの場合、統計的有意性を、SPSSソフトウェアパッケージの一部としての、対応のないt検定またはマン・ホイットニーU検定によって検定した。介入研究の場合、適宜、2つのプロスペクティブコホートにおける対解析のために、ウィルコクソン符号付き順位検定または対応のあるt検定を使用した。ピアソン相関係数およびスピアマン相関係数ならびにボンフェローニ補正を用いて、パラメータ間の関係性を解析した。変数が循環スクシネートに関連したかを判定するために、複数の線形回帰分析を使用した(変数増減法)。単変量解析においてスクシネートと関連したすべての変数を、それぞれのモデルに含めた。0.05未満のp値を有意とみなした。機能的研究のために、R統計ソフトウェアバージョン3.3.3を用いて統計解析を行った。2つの群の間の仮説検定解析(群1対群2)のために、ウィルコクソン順位和検定を用いた。階層的クラスタリングアルゴリズムを用いてヒートマップを作成して、メタゲノム機能およびデータセット内の代謝産物の差を可視化した。
被験体における変化した代謝プロファイルを、糖尿病などの代謝病態を発症するリスクに関連するいくつかのパラメータに対する閾値セットとして定義する。変化した代謝プロファイルに特有の値は、以下のとおりである:
・インスリン>25μLU/mL
・グルコース>100(mg/dl)
・HOMA−IR>3,21
・トリグリセリド>1,7(mM)
肥満症およびT2DMに従って層別された91人の患者のコホート(コホート1)において、血漿スクシネートレベルは、痩せ個体よりも肥満個体において有意に高く(図2A、表1)、同BMIのT2DM患者において類似の上昇が検出され、これは、最近の報告とも一致した(van Diepen et al.,2017,Diabetologia 60:1304−1313)。これらの結果から、全身のスクシネートが体重の状態にも関連することが示唆される。したがって、循環スクシネートレベルと、BMIとの間だけでなく(図2B)、インスリン、グルコース、インスリン抵抗性指数(HOMA−IR)およびトリグリセリド(図2B)との間にも、正の関連が見出された。血圧調節におけるスクシネートの実証されている役割と一致して(He et al.,2004,Nature 429:188−193;Sadagopan et al.,2007,Am.J.Hypertens.20:1209−1215)、循環スクシネートは、拡張期血圧とも正に相関した(R=0.386、p=0.039)。年齢および性別に対して調整された複数の回帰分析モデル(R2=0.295)は、BMIおよびグルコース(それぞれβ=0.495 p<0.001およびβ=0.279 p=0.013)が、循環スクシネートレベルの主要な決定因子であることを示した。
独立コホート(コホートII。臨床上および人体測定の特徴が表2に要約されている)において、スクシネートの血清濃度は、非肥満個体よりも肥満個体において有意に高かった(43.93±6.16μM 対 23.2±1.57μM、p=0.0020)。注目すべきことに、血清中のスクシネート濃度は、血漿中で見られる濃度よりも約3分の1低い(Ariza et al.,2012,Front.Endocrinol.(Lausanne)3:22および本研究)。
腸内微生物叢の食事誘発性改変が、循環スクシネートレベルの変動に反映され得るかを判定するために、食事介入またはダイエット製品による12週間の前向き研究を、体重減少を目的として、肥満患者において行った(コホートIV、表3)。属および科の豊富さの増大(図6A)と同時に、介入後に血清スクシネートレベルが低下した(図4A)。属または科の多様性に有意差は検出されなかったが(図6B)、以前の食事性体重減少介入研究において報告されたもの(Cotillard et al.,2013,Nature 500:585−588;Dao et al.,2016,Clinical Nutrition Experimental 6:39−58;Healey et al.,2017,Nutr.Rev.75:1059−1080)と同様に、Firmicutes/Bacteroidetes比の低下(図6C)が明らかにされた。
最後に、微生物叢の自発的な進化を評価するために、一般的な健康習慣カウンセリングを提供した19人の被験体を調べた:ベースラインおよびその2年後(方法の項、表4におけるコホートVの説明を参照のこと)。追跡調査において、これらの患者に体重の有意差は観察されなかった。16Sの配列決定ではなく、メタゲノムアプローチを用いて、このコホートにおける腸内微生物叢を解析した。追跡調査の終わりに、比[スクシネート産生]科 対 [スクシネート消費]科の変化に関して、被験体を2群に分類した(群1、比は低下 対 群2、比は増加)。fam[(P+V)/(O+C)]の低下は、スクシネートレベルの有意な低下と関連した(表5、群1)のに対して、この比の有意な上昇は、全身スクシネートの増加と関係した(表5、群2)。
C57/Bl6マウスに16週間、高フルクトース食を与えた。次いで、肥満マウスを、100uLの、PBS+グリセロール1%(ビヒクル)中の1×109CFU/mLの細菌を用いて24日間、経口胃管栄養法にてOdoribacter laneusで毎日治療した。Odoribacter laneusで治療された動物において、耐糖能(図8A)が改善した。曲線下面積(AUC)を(図8B)に示す。

・ 前記プロバイオティクス介入前の前記被験体由来の前記生体液サンプル中の循環スクシネートのレベルより低い、前記プロバイオティクス介入後の前記被験体由来の前記生体液サンプル中の循環スクシネートのレベルが、前記プロバイオティクス介入が有効であったことを示し、
・ 前記プロバイオティクス介入前の前記被験体由来の前記生体液サンプル中の循環スクシネートのレベルと等しいまたはそれより高い、前記プロバイオティクス介入後の前記被験体由来の前記生体液サンプル中の循環スクシネートのレベルが、前記プロバイオティクス介入が有効でなかったことを示す、使用。 - 被験体における循環スクシネートのレベルの低下を目的とする標的化介入が有効であったかを判定するための方法であって、当該方法は、
(a)前記標的化介入前の前記被験体由来の糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比を測定する工程、および
(b)前記標的化介入後の前記被験体由来の糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比を測定する工程
を含み、
・ 前記標的化介入前の前記被験体由来の前記糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比より低い、前記標的化介入後の前記被験体由来の前記糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比が、前記標的化介入が有効であったことを示し、
・ 前記標的化介入前の前記被験体由来の前記糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比と等しいまたはそれより高い、前記標的化介入後の前記被験体由来の前記糞便サンプル中のスクシネート産生細菌とスクシネート消費細菌との比が、前記標的化介入が有効でなかったことを示す、
方法。 - 前記標的化介入が、食事介入またはダイエット製品、薬理学的介入およびプロバイオティクス介入からなる群から選択される、請求項8に記載の方法。
- 前記患者が、肥満である、請求項8または9のいずれか一項に記載の方法。
- 前記患者が、2型糖尿病を有する、請求項8から10のいずれか一項に記載の方法。
- 患者における高レベルの循環スクシネートに関連する疾患の予防および/または治療において使用するための食事介入またはダイエット製品であって、前記介入が、前記患者の腸管におけるスクシネート産生細菌とスクシネート消費細菌との比を低下させる、食事介入またはダイエット製品。
- 請求項12に記載の使用のための食事介入またはダイエット製品であって、前記介入が、
・ 脂肪が、1日当たりの総カロリー摂取量の35〜40%であること;および
・ 炭水化物が、1日当たりの総カロリー摂取量の40〜45%であること
を特徴とする低カロリー食を含み;
前記食事介入またはダイエット製品は、少なくとも12週間投与され、
前記食事介入またはダイエット製品は、場合により、身体的運動プログラムと組み合わせて投与される
食事介入またはダイエット製品。 - 高レベルの循環スクシネートに関連する疾患の予防および/または治療において使用するための製品であって、前記製品は、患者の腸管におけるスクシネート産生細菌とスクシネート消費細菌との比を低下させ、前記製品は、薬理学的製品およびプロバイオティクス製品からなる群から選択される、製品。
- 高レベルの循環スクシネートに関連する疾患の予防および/または治療において使用するための製品であって、前記製品は、患者の循環スクシネートのレベルを低下させ、前記製品は、薬理学的製品およびプロバイオティクス製品からなる群から選択される、製品。
- 前記患者が、肥満である、請求項12または13のいずれか一項に記載の使用のための食事介入またはダイエット製品、あるいは請求項14または15のいずれか一項に記載の使用のための製品。
- 患者における高レベルの循環スクシネートに関連する前記疾患が、肥満症、循環器疾患、高血圧症、2型糖尿病、慢性心不全、虚血性心疾患、虚血/再灌流傷害および糖尿病性腎症からなる群から選択される、請求項12、13または16のいずれか一項に記載の使用のための食事介入またはダイエット製品、あるいは請求項14から15または16のいずれか一項に記載の使用のための製品。
- スクシネート産生細菌とスクシネート消費細菌との比が、(Prevotellaceae+Veillonellaceae)/(Odoribacteriaceae+Clostridiaceae)比である、請求項12から13または16から17のいずれか一項に記載の使用のための食事介入またはダイエット製品、あるいは請求項14から17のいずれか一項に記載の使用のための製品。
- 前記薬理学的製品が、スクシネート産生細菌を特異的に標的化し、かつ前記薬理学的製品が、抗生物質、抗菌性抗体およびバクテリオファージからなる群から選択される、請求項14から18のいずれか一項に記載の使用のための製品。
- 前記プロバイオティクス製品が、スクシネート消費細菌を含む、請求項14から18のいずれか一項に記載の使用のための製品。
- 前記スクシネート消費細菌が、Odoribacter spp.、Clostridium spp.、Phascolarctobacterium spp.、Ruminococcus spp.およびそれらの組み合わせからなる群から選択される、請求項20に記載の使用のための製品。
- 前記スクシネート消費細菌が、Phascolarctobacterium succinatutens、Phascolarctobacterium faecium、Ruminococcus bromiiおよびOdoribacter laneusからなる群から選択される、請求項21に記載の使用のための製品。
- 有効量のスクシネート消費細菌を含むプロバイオティクス製品であって、前記スクシネート消費細菌が、Odoribacter spp.、Phascolarctobacterium spp.、Ruminococcus spp.およびそれらの組み合わせからなる群から選択される、プロバイオティクス製品。
- 前記スクシネート消費細菌が、Phascolarctobacterium succinatutens、Phascolarctobacterium faecium、Ruminococcus bromiiおよびOdoribacter laneusからなる群から選択される、請求項23に記載のプロバイオティクス製品。
