Abstract
Description
1.染色体異常および再配列の分子細胞遺伝学的評価。
2.ヘテロ接合性喪失(LOH)または対立遺伝子不均衡を検出するためのDNA多型分析。
3.セグメントコピー数変化を同定するための比較ゲノムハイブリダイゼーション(CGH)アプローチ。
以下、図中の"deletion"は「欠失」、"amplification"は「増幅」、"tumor tissue"は「腫瘍組織」、"tumor adjacent tissue"は「腫瘍隣接組織」、"cell viability"は「細胞生存率」をそれぞれ意味する。
本発明の他の特徴および利点は、以下の実施例においてさらに例示され、記載される。本明細書に記載される実施例は例示のために使用されるものであって本発明を限定するために使用されるものではない。
Infinium CoreExome-24 BeadChip(Illumina)を用いて遺伝子型タイピングを行い、選択した115組のHCCサンプルの遺伝子型を決定する。手順は製造業者の指示に従う。簡潔には、HCC症例のHCC腫瘍組織または非腫瘍組織のいずれかからの約200ngのゲノムDNAを増幅し、断片化し、沈殿させ、ハイブリダイゼーション緩衝液に懸濁する。変性後、サンプルをBeadChipsに48℃で16時間ハイブリダイズさせる。
次に単一塩基伸長を行い、チップを染色し、Illumina Bead Array Reader上でスキャンする。
次いで画像データセットをIllumina GenomeStudio v2ソフトウェアおよび標準パラメータを使用して分析する。
次いで、生の遺伝子型データは、DNAコピー数を計算するためテキストファイルに出力される。遺伝子型決定チップからのデータを用いてDNAコピー数変化を検出するために、B対立遺伝子頻度(BAF)およびlog R比(LRR)をGenomeStudio v2ソフトウェアを用いて抽出し、PennCNV v1.0.3によって分析し、WGAViewerによって可視化する。SNPアレイ結果上にあるDNAコピー数解析の妥当性は、HCC試料についてqPCRアッセイを行うことによって確認される。
HCC腫瘍組織サンプル中のCD36のDNAコピー数を測定するため、定量的PCRを実施し、腫瘍サンプルと隣接する非腫瘍組織との間でCD36 DNAコピー数と参照DNAコピー数を比較する。5’末端領域、CD36遺伝子の3’末端領域、および参照配列をそれぞれ増幅するため、3つのプライマーが設計される。CD36配列を増幅するために使用されるプライマーの配列は、以下の通りである。
1.5’末端領域については、5’GGCTCATTCACCAAGGAC(順方向、配列番号1)および5’GACTTAATGAGAAGGAACAAC(逆方向、配列番号2)、
2.3’末端領域については、5’GTTACTACCTTCTCTTCTG(順方向、配列番号3)および5’GTAAAGTGAATCCAGTTATC(逆方向、配列番号4)、
3.参照配列については、5’GAAACTGTTTTCCTTGTCTG(順方向、配列番号5)および5’GCTTTGTACTGGGAGGAG(逆方向、配列番号6)。
定量的なPCRアッセイはすべてABI StepOneリアルタイムPCRシステムでSensiFAST SYBR(登録商標)Hi-ROXキット(バイオライン)を使用して実行される。腫瘍組織と隣接する非腫瘍組織とのΔCTの差を用い、CD36 DNAコピー数を算出する。
腫瘍(T)組織および非腫瘍(N)組織試料の相対的発現レベルを、逆転写定量PCR(RT‐qPCR)を用いて測定する。100対のHCC試料からの全RNAを、製造業者の説明書に従ってSuperScriptII(Invitrogen)を用いてcDNAに逆転写する。その後、標的遺伝子およびGAPDHに対するqPCR反応は、SensiFAST SYBR(登録商標)Hi-ROXキットを使用して、リアルタイムPCRシステムABI StepOneで三段階的に実行される。
RT−qPCRに使用されるプライマーの配列は以下の通りである。
1.CD36は、5’GAACCTATAACTGGATTCAC(順方向、配列番号7)および5’GTCCCAGTCTCATTAAGC(逆方向、配列番号8)、
2.GAPDHは、5’GTGAAGCAGGCGTCGGAG(順方向、配列番号9)および5’GTTGTCATACCAGGAAATG(逆方向、配列番号10)。
全ての試料を分析し、内部対照遺伝子GAPDHの発現レベルを用いて正規化する。倍率変化に関する相対定量は、腫瘍組織サンプルのΔCTを隣接する非腫瘍組織サンプルのΔCTと比較することによって得られる。
国際がんゲノムコンソーシアム(ICGC)およびがんゲノムアトラス(TCGA)肝がんデータセットのゲノムデータをhttp://xena.ucsc.eduからダウンロードしている。88対のHCC症例の全ゲノム配列決定(WGS)データをNCBIシークエンスリードアーカイブ(SRA)からダウンロードし、受付番号はPRJEB2869である。
WGSデータを用いてCD36とABCG4のDNAコピー数(CN)変化を解析するために、生のリードをIsacc変異型カルラー20によりヒトゲノムhg19に並べ、DNAコピー数をCNVSegにより解析した。
Mann-WhitneyのU検定を行い、年齢、腫瘍サイズ、腫瘍数、AFP値などの臨床データの意義を算出する。フィッシャーの直接確率検定を用いて、肝硬変比、脂肪肝比、およびDNAコピー数の変化のHCC群間の有意差を決定する。抗CD36抗体治療データについては、両側対応のないStudentのt検定を用いて、異なるHCC群間の大きさ、または抗CD36抗体で治療されたもの間または対照IgGで治療されたもの間の増殖阻害効果を比較する。HCC患者の生存分析はログランク検定により行う。全てのデータは、平均標準偏差および代表的な3つの独立した実験として提示される。Prism(GraphPad)をこれらの分析に使用する。
(症例選択)
ウイルス性肝炎感染の既往がないHCC症例に関連する遺伝的特徴を同定するため、台湾肝癌ネットワーク(TLCN)を介した250例のHCC症例の肝組織試料を得、これらのHCC組織試料のゲノムプロファイルを調査した。このバイオリポジトリは、臨床的および病理学的情報につながる研究を支援するために広範な同意を得て確立される(Chang、IC et al. Medicine(Baltimore)95、e3284、2016)。
試験症例は、HBV感染歴(HBV−HCC)のある被験者100例(男性50例、女性50例)とHCC感染歴(HCV−HCC)のある被験者100例(男性50例、女性50例)である。残りの50例(男性25例、女性25例)のHCC患者には、HBV感染またはHCV感染の既往歴はない(すなわちNBNC−HCC患者)。これらのHCC症例の臨床的特徴を表1に示す。
ゲノム解析を行うため、TP53変異状態に基づいて115症例を選択した。これらは38人のHBV−HCC、42人のHCV−HCC、および35人のNBNC−HCCで構成された(表1)。一般的なTP53ホットスポット変異についてスクリーニングするため、DNA質量分析を使用する検出パネルを設計する。PCRおよび伸長プライマーは、MassArray Assay Design 3.1ソフトウェア(Sequenom)を使用して設計される。
次に単一塩基伸長を行い、チップを染色し、Illumina Bead Array Reader上でスキャンする。
次いで、画像データセットをIllumina GenomeStudio v2ソフトウェアおよび標準パラメータを使用して分析する。
次いで、生の遺伝子型データはDNAコピー数を計算するために、テキストファイルに出力される。遺伝子型決定チップからのデータを用いてDNAコピー数変化を検出するため、B対立遺伝子頻度(BAF)およびlog R比(LRR)をGenomeStudio v2ソフトウェアを用いて抽出し、PennCNV v1.0.3によって分析し、WGAViewerによって可視化する。SNPアレイ結果上にあるDNAコピー数解析の妥当性は、HCC試料についてqPCRアッセイを行うことによって確認される。
DNAコピー数の変化に関連するピークを本実施例においてさらに調査する。本発明はNBNC−HCCに最も関心があるため、79.1Mbから80.7Mbまで拡大した第7色素の領域を詳細に分析する。
図2Aに示すように、この領域を標的とするプローブはDNAコピー数増加を検出した。全ての陽性例において、増幅は80.3Mb付近の位置で、特異的遺伝子、CD36を含む。CD36はスカベンジャー受容体として機能し、それは複数の機能を有する。それらの1つは、脂肪酸取り込みを容易にすることである。このDNA増幅事象の性質を解析すると、コピーの変化は腫瘍組織サンプルでのみ起こることがわかる。さらにこの事象は、NBNC−HCC個体において最も顕著である(図2B)。
11番染色体上のDNA欠失に影響される領域の遺伝子座を検索すると、ABCG4遺伝子領域に及ぶ体細胞欠失(図2C)を同定することができ、NBNC−HCC症例の40%にABCG4の欠失が生じた(図2D)。NBNC−HCC症例におけるこの所見を上記に概説したCD36遺伝子増幅所見と統合すると、ウイルス性肝炎感染の既往がない場合に脂質ホメオスタシス経路を含む遺伝的変化がHCCの病因に何らかの役割を果たしている可能性が高いという仮説を支持している。
SNPアレイから得られた遺伝子型データを検証するため、qPCRアッセイを開発してHCC組織サンプル中のCD36のDNAコピー数およびmRNA発現レベルを調べる。
図3Aに示すように、腫瘍組織サンプル中にCD36の3つ以上のコピーを有するHCCサンプル(症例235、243、および247)は、ペアの非腫瘍組織サンプルと比較すると、サンプル中のCD36 mRNA発現レベルが上昇していることが見出される。さらにCD36遺伝子発現は症例235、243および247において、非腫瘍隣接組織(N)に対し、腫瘍組織(T)において上昇し、その各々はCD36遺伝子の3コピー以上を有する。
一方、症例241および242ではCD36コピー数が二倍体ゲノムについて予測されるものから逸脱せず、腫瘍と非腫瘍隣接組織との間のCD36 mRNA発現比は1に近い。
図3に示すように、抗CD36抗体は腫瘍組織中の癌細胞を強く染色できる(図3C)。これは、CD36タンパク質がHCC癌細胞で過剰発現していることを示唆している。しかし、比較してみると、CD36タンパク質は類洞内皮細胞に豊富に発現しているのに対し、良性肝細胞は非常に弱いCD36発現しか示さないことがわかる(図3D)。非腫瘍組織試料中の類洞内皮細胞を、内部陽性対照として用いる。
次に、ICGCおよびTCGAデータベースからの遺伝子型データを調査し、CD36およびABCG4におけるコピー数変化を得る。
図5に示すように、CD36遺伝子増幅およびABCG4遺伝子欠失は、ICGC HCCサンプルのそれぞれ15.6%および10.3%で検出され、TCGA HCCサンプルのそれぞれ15.3%および9.7%で検出される。したがって、公開されているゲノムデータレポジトリーで利用可能な情報は、これら2つの脂質ホメオスタシス遺伝子がHCC発癌において重要な役割を果たすというわれわれの結論を支持する独立した証拠を提供する。
図6Aに示すように、CD36増幅およびABCG4欠失の両方を有する患者の生存期間中央値は1385日であり、これはCD36増幅のみ(2344日)またはABCG4欠失(2045日)のみを有する患者よりも有意に短い(P<0.0001)。CD36増幅は一貫して高α-フェトプロテインレベルと関連し(表3参照)、CD36増幅とABCG4欠失の両方を含むHCCでは腫瘍サイズが有意に増加していることがわかる(図6B)。
最近の研究により、CD36が転移細胞のマーカーであることが報告されている。われわれは、肝がん細胞における脂質取り込みを遮断することで抗CD36抗体が細胞増殖を阻害できると仮定する。これに基づき、本実施例では培養肝癌細胞を用いた実験を行った。
肝細胞癌細胞株を96ウェルプレート(1ウェル当たり104細胞)に24時間播種し、次いで3%FBSおよび抗CD36抗体(Cayman、CAY188150)を含むDMEM培地で72時間処理する。次いで細胞増殖アッセイをユーザマニュアルに従ってalamarBlue細胞生存試薬(Thermo Fisher Scientific)を使用して実施する。
HuH−7細胞をトリプシン処理して200μlの氷冷PBSに再懸濁する。アポトーシスアッセイのため、100μlの細胞懸濁液を微量遠心管にアリコートし、300gで5分間の遠心分離によって収集する。次いで細胞を100μlのアネキシンV FITCアポトーシス検出キット(BD Biosciences)インキュベーション緩衝液に再懸濁し、1μlのアネキシンV+1μlのPIと共に室温で15分間インキュベートする。アポトーシス検出の前に、500μlのインキュベーション緩衝液を細胞に添加する。結果はサンプル当たり5,000事象を記録するように設定されたBD FACSCaliburフローサイトメーターシステム(BD Biosciences)を使用して得られる。
図7Dに示したデータはまた、この効果がおそらくアポトーシス関連機構を介して媒介されることを示唆している。
Claims (11)
- ヒト以外の被験体におけるB型/C型肝炎ウイルス感染陰性の肝細胞癌の予防または治療のため、前記被験体に、脂質ホメオスタシス関連遺伝子の遺伝的変化を制御するための調節剤の治療有効量を投与することを含む、投与方法。
- 前記遺伝的変化がCD36増幅を含む、請求項1に記載の投与方法。
- 前記遺伝的変化がABCG4欠失を含む、請求項1に記載の投与方法。
- 前記遺伝的変化がCD36増幅およびABCG4欠失を含む、請求項1に記載の投与方法。
- 脂質ホメオスタシス関連遺伝子の遺伝的変化を阻害または促進するための治療有効量の調節剤を含む、被験体におけるB型/C型肝炎ウイルス感染陰性の肝細胞癌を予防または治療するための組成物。
- 前記調節剤がCD36の過剰発現を抑制するための薬剤である、請求項5に記載の組成物。
- 前記調節剤が前記肝細胞癌の癌細胞における脂質取り込みをブロックする薬剤である、請求項5または6に記載の組成物。
- 前記調節剤が抗CD36抗体である、請求項7に記載の組成物。
