Abstract
Description
以下において、急性腎障害(ARI)という用語は、いくつかの腎機能のうちの1つ又は複数の喪失を引き起こすのに十分な、腎臓内で急性的に発生する任意の細胞傷害を指し、この細胞傷害は、顕微鏡によって検知可能であるかどうかは問わず、又は機能の喪失も、血清クレアチニン等の腎機能マーカー物質の濃度の変化によって検知可能であるかどうかは問わない。そうした腎障害は、有害なプロセスに応答して腎臓内の細胞から放出されるいくつもの腎障害マーカーのうちの1つ又は複数の循環血中濃度又は尿中濃度の上昇によって検知することもできる。
ARIは、多くの異なる原因を有することがあり、通常、3つの異なる項目:1)虚血/再灌流、2)腎毒性、及び3)炎症性にグループ分けされる。「腎毒性」の定義は、腎臓に対するいかなる有害な影響をも表すように拡大されることもあり、したがってこの用語は、虚血/再灌流及び炎症性障害も含む。ここでは、この用語は、身体に本来備わっていない物質、特に薬物によって発揮される腎臓に対する毒性作用を表すために使用される。
敗血症におけるARIの原因の理解が不十分であるのと同様に、損傷が媒介される細胞内経路も全く解明されていない。ARIの原因は一般に、特に腎尿細管上皮細胞において認められる、壊死、アポトーシス、又はネクロプトーシスのいずれかによる腎細胞死に帰着する。細胞の一部は、亜致死的に障害されて生存できる一方で、その他の細胞は、1つ以上の前述の細胞死のプロセスによって死ぬ運命にある。障害性要因の中で、重要であるとみなされるのは、以下である:
シクロスポリン類及びその他の免疫抑制抗生物質
多くの薬物の中で、1つ又は複数の組織又は臓器においてIRIを改善することが実験的に見出されているのは、シクロスポリン類である。これは、一般にカルシニューリン阻害剤と称される、一連の周期性ポリペプチド抗生物質である。カルシニューリンは、カルシウム依存性のセリン・トレオニンフォスファターゼであり、転写因子NFATc(活性化T細胞核内因子)を脱リン酸化し、したがって活性化し、次にNFATcが、インターロイキン2(IL-2)の発現を上方制御し、非自己抗原に応答して、T細胞の増殖及び分化を刺激する。この経路は、cypDと結合してカルシニューリンを阻害するシクロスポリン類によって遮断される。シクロスポリン類は、したがって、臓器移植における免疫抑制剤として使用され、シクロスポリンAは、最も一般的に使用される。しかし、cypD結合のもう1つの結果は、mptpの開口を阻害し、ミトコンドリアが誘発するアポトーシス及び壊死から細胞を保護するのを助ける。
種々の組織及び臓器においてIRI及び炎症の両方を改善することが見出されている薬物の別のクラスは、セラストロール及びベツリン酸等の植物由来の五環性トリテルペノイドからなる。五環性トリテルペノイドは、化合物の大きな群であり、その個々のメンバーは、抗炎症性、抗腫瘍性、抗糖尿病性、及びホルモン活性を含む生物学的活性の選択を示すことがあり、一部はまた昆虫や魚類に有毒であり、また一部は、哺乳類において肝臓及び他の臓器に対して毒性がある。
植物由来薬物の関連クラスは、親油性テルペン誘導体と組み合わされた1つ又は複数の親水性グリコシド部分から構成される、洗剤活性を有する両親媒性グリコシドであるサポニンからなる。多種多様な医薬特性がまた、これらの薬物によるものとされてきた。このクラスには、柑橘類果実の皮中に存在するフラボノイドである化合物ヘスペリジン、及びその誘導体で、静脈の緊張増加及び毛細血管透過性の低減に使用されるジオスミンが、分類される。サポニンであるβ-エスシンは、同じ目的に使用されてきた。これらの化合物は、血管内皮への好中球接着を、したがって炎症組織への好中球の流入を低減し、したがって抗炎症効果を発揮する。抗腫瘍及び抗酸化(フリーラジカル捕捉)効果が報告されている。アメリカニンジン(Panax quinquefolium)から抽出されるサポニンもまた、IRIの結果を改善する有意な効果を有することが示されている(例えば、Li他 2014を参照)。
ARIを含む組織障害を改善することができる前述の免疫抑制剤は、実質的に修飾されない形態で、重大な感染症を有さない患者に対して投与されうる。しかし、相当な数のARI患者は、免疫抑制剤が危険とみなされる重度の感染症を有する。したがって、本発明の目的は、これら物質のリンパ球による取込みを妨げ、腎臓による取込みを促進するであろう、これら物質の誘導体又はコンジュゲートを創製することである。「コンジュゲート」によって、指標物質が、共有結合又は供与/配位結合によってもう1つの物質と化学的に結合していることが意味され、この場合、それが、生じたコンジュゲートが生物学的標的分子と結合することを可能にする。そうした誘導体又はコンジュゲートの腎臓による取込みが、この臓器における薬物濃度を、一般的な全身濃度よりも何倍も大きくすることにつながる場合、腎臓における有効濃度が低投与量で得られる一方で、全身濃度は望まれていない又は有害な影響を生じるには不十分である。
提案される免疫抑制薬の唯一の反応基は、ヒドロキシ基であるため、コンジュゲーションはこの基を介して行われる。このタイプのコンジュゲーションは水及びアルコールの非存在下で行われなければならない。全ての薬物がジメチルスルホキシド(DMSO)に高度に可溶性であり、したがって乾燥DMSOはコンジュゲーションのための媒質を形成する。ヒドロキシ基は、担体上のアミノ基とカルバメート結合を形成することができるよう、薬剤N,N'-カルボニルジイミダゾール(CDI)、N,N'-ジスクシンイミジルカーボネート(DSC)、又はN-ヒドロキシスクシンイミジルクロロホルメートのうちの1つによって活性化される。この手順及び他の適切な手順が、先行技術に知られている(Yuan他 2007、Hermanson 2013)。活性化後に、薬物は、別法として、更なるより実用的なコンジュゲーションの可能性を与える、4-(アミノメチル)ピリジン等の、部分と結合してよい。これは、プラチナ(II)ベースのUniversal Linkage System (ULS; Kreatech Diagnostics、オランダ、アムステルダム)によってコンジュゲーションの基盤を形成することができるピリジン環を付加する。このプラチナリンカーは、ピリジル基又は他のタイプの芳香族窒素との安定な配位性結合を形成し、リゾチーム等のタンパク質性の担体中のチオール基とコンジュゲートすることができる(Dolman他 2012)。
構造的特徴及びそれらのコンジュゲーションの可能性に関して記述された免疫抑制剤に当てはまる考察の多くはまた、本発明の組成物における五環性トリテルペノイド、フラボノイド及びサポニンにも当てはまる。それらは、中性に近いpHにおいて水又はバッファーに難溶であり、通常は、不溶とみなされ、溶解するためには、強アルカリ又は有機溶媒中での溶解等の特別な条件を必要とする。ジオスミンは、グリコシドであり、より水溶性である。現在、ヒトで使用されるものは、通常、経口投与され、吸収を助け、且つバイオアベイラビリティを増大するために、微粒子化形態であることが多い。それらは、全てDMSOに容易に溶解し、原則として、DMSO/水混合物の静脈内注入として投与することができる。
静脈内注入のための、希釈溶液中低パーセンテージのDMSOを可能にすることは許容されうるが、前述の組合せの従来の静脈内製剤に関するパラダイムは、Cremophor EL(ポリオキシエチレン化ヒマシ油)とのエマルション及び注入前に生理食塩水で5から20倍に希釈される無水エタノール混合物を含む、プラクシタキソール(praxitaxol)又はシクロスポリンA等の、他の水に不溶な天然製品の許容された製剤であろう。しかし、Cremophor ELはそれ自体、アナフィラキシーを含む有害反応に関連するため、純粋なトリグリセリド油及びレシチンに基づく代替製剤もまた使用されうる。
前述の五環性トリテルペノイド及びサポニンを送達する別の方法は、リポソームの水性懸濁液の形態におけるものである。これらの疎水性薬物に関して、リポソームは単層であり、その製造は当業者に周知である。これらは、腎細胞の表面タンパク質に親和性のあるタンパク質分子、例えば腎細胞表面抗原に対する抗体を、親水性の外膜に包埋することによって、腎臓に対してさえも標的化されうる。しかし、リポソームが、含有された薬物の作用が最も重要であると考えられる腎尿細管細胞の頂端膜側に到達することは起こりそうもない。リポソームが、糸球体内皮において、開窓を通過して、糸球体濾液に流入する場合、メサンギウム細胞を標的とする可能性が最も高いことになる。したがって、ARIに関連していかなる標的化も最適でありそうもないため、リポソーム製剤の使用は、いっそう、これら薬物の静脈内送達のためのエマルションの代替とみなされる。
前述の薬物の毒性プロファイルについて知られていることは、全身毒性を低減するために、これら薬物を腎臓に対して標的化する必要性を求めるというようなことではない。しかし、これら薬物を腎臓に対して標的化することは、より低投与量でより大きな腎臓への効果が得られること及び発生しうるいかなる全身的副作用をも低減することを可能にすることになる。これら薬物は実際、免疫抑制薬に関して記述されている方法と非常に類似した方法によって、腎尿細管細胞に対して標的化されうる。トリテルペノイド及びサポニンの標的化についてのパラダイムは、トリプトライドの標的化によって与えられる。トリプトライドは実際、中国において多発性嚢胞腎等の慢性腎疾患の治療に使用されるジテルペノイドエポキシドである。したがって、トリプトライド、そのコンジュゲート及び腎疾患を治療するためのそれらの使用は、本発明の範囲外にある。トリプトライドは、非常に低い水溶性及び消化器系、泌尿生殖器系、循環器系及び生殖器系に対する非常に低い毒性作用の両方を有する。トリプトライドは、コハク酸スペーサーによるエステル結合によってリゾチームにコンジュゲートされた。コンジュゲートの腎中濃度は、静脈注射の30分後に等用量の薬物によって得られるものより20倍高かった(Zheng他 2006)。トリプトライド-リゾチームコンジュゲートは、非コンジュゲートトリプトライドが示した肝毒性のわずか約1/5の肝毒性を示し、且つ免疫系に対する有害作用を生じなかった。本発明の目的において、コンジュゲーションに使用される好ましい標的化担体は、組換え型ヒトNGAL、好ましくはその非グリコシル化形態で、且つタンパク質鎖の87位のシステイン残基が欠落している組換え型ヒトNGALである。

2.静脈内投与のために製剤化される、薬物の組合せを含む組成物であって、前記組合せが、セラストロール、プリスチメリン及びベツリン酸を含むリストから選択される五環性トリテルペノイドと共に、ジオスミン、ヘスペリジン及びアメリカニンジン由来のサポニンを含むリストから選択されるサポニン又はフラボノイドからなる、組成物。

14.急性腎障害の予防及び/又は先制的治療及び/又は治療における使用のための、別の薬物と組み合わされた、実施形態1〜13のいずれか一項に記載の組成物。
15.急性腎障害の予防及び/又は先制的治療及び/又は治療における使用のための、静脈内投与のために製剤化される、実施形態1〜2、4〜13のいずれか一項に記載の組成物。
16.急性腎障害の予防及び/又は先制的治療及び/又は治療のための方法であって、そうした予防及び/又は治療を必要とする対象に実施形態1〜2及び4〜13のいずれか一項に記載の組成物を静脈内投与することを含む方法。
本発明の組成物は、好ましくは単独でARIの早期治療、先制的治療又は予防に使用される、オキセクロスポリンを含む、シクロスポリンファミリーのメンバー、及び非シクロスポリン化合物のタクロリムス、セロリムス及びエベロリムスを含むリストから選択される化合物を含む。化合物は、リンパ球による取込みを妨げ、且つ/又は薬物を主として腎臓に標的化することを目的とする、コンジュゲート、又はリポソーム、ミセル若しくはナノ粒子懸濁液の形態で、静脈内に投与されうるような方法で製剤化される。好ましい化合物は、シクロスポリンAであり、より好ましくはシクロスポリンGであり、最も好ましくはオキセクロスポリンである。単一化合物の使用に対する選好にもかかわらず、化合物は、ARI治療に対する相加的又は相乗的な有益な効果を有することが示されている他の化合物と組み合わせられて又は共に使用されてよい。リンパ球による化合物の取込みを妨げ、且つ/又は薬物を主として腎臓に標的化するための好ましい担体又はコンジュゲーションパートナーは、NGALである。
本発明による組成物が使用される適応症は以下の通りである:
エンドユーザーに送達されるときの本発明の組成物の製剤化の主要な側面は、前述されている。組成物は、静脈内注入によって投与される前に希釈用の溶液、エマルション、又はリポソーム調製物として送達される。0.9%塩化ナトリウム溶液、5%ブドウ糖溶液、緩衝食塩水、生理学的適合性のある緩衝液等を含む種々の水性希釈剤が使用されてよいが、これらに限定されるものではない。組成物は、典型的には、水性希釈剤5の中に1、から20の中に1までの容量の希釈液として与えられる。
本発明の組成物は、希釈された調製物の注入による静脈内投与のためのものである。投与のタイミングは重要である。投与は、担当臨床医が使用される適応症が存在するとみなしてすぐに行われるべきである。
本発明の組成物の投与量は、単一用量で投与される各活性物質の量に関係する。標的化されたコンジュゲートが使用される場合、投与量はコンジュゲート中に存在する活性物質の量に関係する。

セラストロール、ベツリン酸及びジオスミンは、Sigma-Aldrich Co.より購入した。セラストロール及びベツリン酸をDMSOに溶解し、静脈注射用に又は別法として、腹腔内注射用に、その溶液を0.9%塩化ナトリウム溶液で希釈して各活性化合物を少なくとも1mg/mLの濃度にする。これらの化合物を、単独で及び組み合わせて、Wen他(2012)によって記述されているように、葉酸の腹腔内注射によってマウスにおいて誘発されたARIの予防及び/又は治療に関して試験した。セラストロール又はベツリン酸の用量は、5〜50mg/kgの範囲であり、ジオスミンの用量は、10〜100mg/kgの範囲である。これらを尾静脈に静脈内投与する。ARIの予防を試験するために、葉酸投与の30分前に化合物を投与し、ARIの治療を試験するために、葉酸投与の30分後に化合物を投与する。投与は、毎日、5日間繰り返す。第2、3、7及び14日に、メタボリックケージ内で尿を採取し、血液試料を伏在静脈から採取する。少なくとも尿、クレアチニン及びNGALを試料において測定する。各群12匹のマウスで、正常対照、葉酸未処置群、及び異なる用量又は薬物の組合せによる3つの治療群を構成する。
腎臓に対して標的化されていない化合物の確認試験
前述の化合物の確認試験は、(例えば、Mishra他、2003において記述のように)虚血-再灌流障害のラットモデルにおいて行われてよい。薬物の用量は、実施例1で得られた結果に照らして決定されたものであり、治療効果の確認のために再灌流の開始以降に静脈内投与しなければならない。サンプリングは、最初の24時間の間のタイムポイントにおいて行うものとし、同一のバイオマーカーを測定するものとする。
腎臓に標的化されたコンジュゲート化合物の試験
試験される化合物は、前述の投与量のセクションに列挙したコンジュゲート化合物のいずれか1つであり、そのセクションにおいて示された対応する用量範囲で試験される。担体又はコンジュゲーションパートナーは、コンジュゲートが試験される動物種の非グリコシル化組換え型NGALである。試験のプロトコールは、前述の実施例1及び実施例2において示されたものである。
Claims (16)
- オキセクロスポリンを含む、シクロスポリンファミリーのメンバー、並びに非シクロスポリン化合物のタクロリムス、セロリムス及びエベロリムスを含むリストから選択される薬物を含む組成物であって、リンパ球による取込みを妨げ、且つ/又は前記薬物を主として腎臓に標的化するためのコンジュゲート、又はリポソーム、ミセル若しくはナノ粒子懸濁液の形態での静脈内投与のために製剤化される組成物。
- 静脈内投与のために製剤化される、薬物の組合せを含む組成物であって、組合せが、セラストロール、プリスチメリン及びベツリン酸を含むリストから選択される五環性トリテルペノイドと共に、ジオスミン、ヘスペリジン及びアメリカニンジン(Panax quinquefolium)由来のサポニンを含むリストから選択されるサポニン又はフラボノイドからなる、組成物。
- 急性腎障害の予防及び/又は先制的治療及び/又は治療における使用のための、静脈内投与のために製剤化される、請求項1又は2に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたシクロスポリンAである、請求項1に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたシクロスポリンGである、請求項1に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたオキセクロスポリンである、請求項1に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたタクロリムスである、請求項1に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたセロリムスである、請求項1に記載の組成物。
- 前記薬物が、グリコシル化及び/又は、そのタンパク質鎖の87位のシステイン残基を任意選択で欠いていてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしたエベロリムスである、請求項1に記載の組成物。
- 前記NGALが、グリコシル化を欠くNGAL、又は87位のシステイン残基を欠くNGAL、又はグリコシル化を欠き87位のシステイン残基を欠くNGALのいずれか1つの形態である、請求項4から9のいずれか一項に記載の組成物。
- 前記薬物の組合せが、ベツリン酸及びジオスミンを含む、請求項2に記載の組成物。
- 前記薬物の組合せが、セラストロール及びジオスミンを含む、請求項2に記載の組成物。
- 前記薬物が、グリコシル化及び/若しくはそのタンパク質鎖の87位のシステイン残基を任意選択で欠いてよいヒトNGAL、又はリゾチーム、又は低分子量キトサンとコンジュゲートしている、請求項11又は12に記載の組成物。
- 急性腎障害の予防及び/又は先制的治療及び/又は治療における使用のための、別の薬物と組み合わされた、請求項1〜2及び請求項4〜13のいずれか一項に記載の組成物。
- 急性腎障害の予防及び/又は先制的治療及び/又は治療における使用のための、静脈内投与のために製剤化される、請求項1〜2及び請求項4〜13のいずれか一項に記載の組成物。
- 急性腎障害の予防及び/又は先制的治療及び/又は治療のための方法であって、そうした予防及び/又は治療を必要とする対象に請求項1〜2及び請求項4〜13のいずれか一項に記載の組成物を静脈内投与することを含む、方法。
