Abstract
Description
本発明は、がんに罹患した被験体を、その被験体におけるがん細胞を標的とするα線放射同位体標識された薬剤と関連したBCL−2インヒビターの治療上有効なレジメンを使用して処置することに関する。
BCL−2インヒビター
BCL−2インヒビターは、悪性疾患を処置する可能性がある。1つのこのようなBCL−2インヒビターは、ベネトクラクス(慢性リンパ性白血病(「CLL」)を処置することに関して承認されている薬物)である(1)。ベネトクラクスは、BIMのようなアポトーシス促進タンパク質を置き換えて、BCL−2のBH3結合溝に結合し、ミトコンドリア外膜透過化(「MOMP」)、シトクロムcの放出、およびカスパーゼ活性化を開始して、最終的にプログラムされたがん細胞死滅(すなわち、アポトーシス)を生じる(2)。
放射線は、がんを処置するための認識されている方法である。放射線の細胞効果が細胞周期停止、変異、アポトーシス、壊死および自食作用を含むことは公知である(9)。細胞損傷の放射線関連メディエーターとしては、以下が挙げられる:(i)直接的LED(線形エネルギー蓄積(linear energy deposition));(ii)ROS(活性酸素種);および(iii)RNS(活性窒素種)(9)。
ベネトクラクスおよび放射線の相乗作用のマウス異種移植片モデル(12)において、ベネトクラクスおよび90Yベースの放射免疫療法の組み合わせで処置したマウスは、ベネトクラクスまたは放射免疫療法のいずれか単独で処置したマウスと比較して、より良好な生存率を有した。異種移植マウスコホートにおける生存の転帰は、図8に示される。
BCL−2インヒビター(例えば、ベネトクラクス)および放射線療法(例えば、90Yベースの治療)で認められる課題を解決するがん治療が未だに必要である。
本発明は、がんに罹患した被験体を処置するための方法を提供し、上記方法は、上記被験体に、(i)BCL−2インヒビターを、(ii)上記被験体におけるがん細胞を標的とするα線放射同位体標識された薬剤とともに投与する工程を包含し、ここで上記BCL−2インヒビターおよび標識された薬剤の量は、互いにともに投与される場合に、治療上有効である。
本発明は、がんに罹患した被験体を処置するための方法を提供する。これらの方法は、その被験体に、2タイプの薬剤を互いにともに投与する工程を包含する。その第1のタイプの薬剤は、BCL−2インヒビター(例えば、ベネトクラクス)である。その第2のタイプは、その被験体におけるがん細胞を標的とする、α線放射同位体標識された薬剤(例えば、225Ac標識HuM195)である。
本出願において、ある種の用語が使用され、それら用語は、以下に示される意味を有するものとする。
本発明は、アルファ粒子の使用を用いる。これらの粒子は、標的細胞、例えば、白血病細胞においてアポトーシスを誘導する(10)、(19)。α放射体およびβ放射体は、白血病細胞において匹敵する活性において異なる効率でアポトーシスを誘導する(10)。α粒子は、白血病細胞におけるドキソルビシン抵抗性、CD95抵抗性、ならびにβ線およびγ線に対する放射線抵抗性を克服し得る(10)。その粒子は、以下を介してアポトーシスを誘導する:(i)2本鎖DNA切断(20)、(21);(ii)カスパーゼの活性化;(iii)[213Bi]抗CD45が、CD95リガンド/レセプター系とは無関係のミトコンドリア経路を通じて、カスパーゼ2、3、8および9を活性化するという事実(10)、(19);ならびに(iv)XIAPおよびBcl−XLの不活性化(19)。
225Ac−リンツズマブは、3つの重要な構成要素を含む;ヒト化モノクローナル抗体HuM195(一般名、リンツズマブ)、α線を放射する放射性同位体225Ac、および二官能性キレート化合物2−(p−イソチオシアナトベンジル)−1,4,7,10−テトラアザシクロドデカン−1,4,7,10−四酢酸(p−SCN−Bn−DOTA)。図4に示されるように、HuM195は、225Acに結合しかつその抗体上のリジン残基を介してIgGに共有結合的に付着する二官能性キレート化合物p−SCN−Bn−DOTAを使用して放射性標識される。
DOTA、2−(4−イソチオシアナトベンジル)−1,4,7,10−テトラアザシクロドデカン四酢酸(大環状化合物アイテムコード(Macrocyclics item code)B205−GMP)は、米国特許第4,923,985号に十分に記載される複数工程の有機合成によって合成される。
225Ac−リンツズマブを調製するための手順は、Michael R. McDevitt, 「Design and synthesis of 225Ac radioimmuno−pharmaceuticals, Applied Radiation and Isotope」, 57 (2002), 841−847によって記載される方法に基づく。その手順は、二官能性キレート化合物p−SCN−Bn−DOTAを、放射性同位体225Acで放射性標識し、続いて、その放射性標識したp−SCN−Bn−DOTAを、抗体(HuM195)に結合させる工程を包含する。この構築物225Ac−p−SCN−Bn−DOTA−HuM195を、10 DGサイズ排除クロマトグラフィーを使用して精製し、1% ヒト血清アルブミン(HSA)で溶離する。その得られた薬物生成物Ac225−リンツズマブを、次いで、0.2μm滅菌フィルタに通す。
その手順(図5に示される)を、全ての構成要素の正体、および生産までのその構成要素のその後のQCリリースを確認して開始する。225Acをアッセイして、放射能のレベルを確認し、塩酸で所望の放射能濃度へと再構成する。凍結乾燥したp−SCN−Bn−DOTAのバイアルを、金属非含有水で10mg/mLの濃度へと再構成する。そのアクチニウム反応バイアルに、0.02mlのアスコルビン酸溶液(150mg/mL)および0.05mlの再構成したp−SCN−Bn−DOTAを添加し、そのpHを、5〜5.5の間へと2M テトラメチルアンモニウムアセテート(TMAA)で調節する。次いで、その混合物を、55±4℃において30分間加熱する。
ベネトクラクスは、Genentech(San Francisco, CA)によって商標名VenclextaTMの下で販売される。FDAのVenclextaTM表示によれば、この薬物は、「17p欠失を有し…少なくとも1回の事前の治療を受けたことがある、慢性リンパ性白血病(CLL)を有する患者の処置に関して示されたBCL−2インヒビターである」。VenclextaTMは、10mg、50mgおよび100mgの錠剤形態で販売される。治療は、「7日間にわたって1日に1回、20mgで、続いて、400mgという推奨1日用量への1週間にわたる用量上昇投与スケジュール(weekly ramp−up dosing schedule)」で開始されるべきである。その用量上昇投与スケジュールは、以下のとおりである:1週間目、20mg/日;2週間目、50mg/日;3週間目、100mg/日;4週間目、200mg/日;および5週間目以降、400mg/日。この投与レジメンは、処置される障害に拘わらず、ベネトクラクスの「通常の」ヒト投与レジメンと本明細書でいわれる。より短い継続期間(例えば、21日間)を有するかまたはより少ないベネトクラクスの投与(例えば、合計21日間にわたって20mg/日)を包含する任意の投与レジメンは、「低減した」ヒト投与レジメンと本明細書でいわれる。用語「通常の」ヒト投与レジメンおよび「低減した」ヒト投与レジメンはまた、任意の他のBCL−2インヒビターに、その承認されたかまたは別の慣習的な投与レジメンに関して、必要な変更を加えて適用する。

ヒトAML患者を、以下のレジメンに従って処置する。ベネトクラクスを、その通常の投与レジメンに従って(すなわち、少なくとも5週間にわたって)経口投与し、続いて、225Ac−HuM195をその通常の投与レジメン(単一投与または分割投与のいずれか)に従って静脈内投与する。一実施形態において、その第1の(かつ適用可能な場合には唯一の)用量の225Ac−HuM195を、ベネトクラクスの最後の用量と同日に、またはその最後の用量の1日後に投与する。

Claims (18)

- 前記BCL−2インヒビターは、ベネトクラクスである、請求項1に記載の方法。
- 前記α線放射同位体標識された薬剤は、α線放射同位体で標識された抗CD33抗体である、請求項1に記載の方法。
- 前記α線放射同位体で標識された抗CD33抗体は、225Ac標識HuM195である、請求項7に記載の方法。
- 急性骨髄性白血病に罹患したヒト被験体を処置するための方法であって、前記方法は、前記被験体に、(i)ベネトクラクスを、(ii)225Ac標識HuM195とともに投与する工程を包含し、ここで前記ベネトクラクスおよび前記225Ac標識HuM195の量は、互いにともに投与される場合に、治療上有効である、方法。
- がん細胞の死滅を誘導するための方法であって、前記方法は、前記細胞を、(i)BCL−2インヒビターと、(ii)前記がん細胞を標的とするα線放射同位体標識された薬剤とともに接触させる工程を包含し、ここで前記BCL−2インヒビターおよび前記標識された薬剤の量は、前記細胞と同時に接触させる場合に、前記細胞の死滅を誘導するために有効である、方法。

- 急性骨髄性白血病細胞の死滅を誘導するための方法であって、前記方法は、前記細胞を、(i)ベネトクラクスと、(ii)225Ac標識HuM195とともに接触させる工程を包含し、ここで前記ベネトクラクスおよび前記225Ac標識HuM195の量は、前記細胞と同時に接触させる場合に、前記細胞の死滅を誘導するために有効である、方法。
