Abstract
Description
12週間にわたって、250mg/日〜500mg/日の安息香酸ナトリウムで、6名のASDsの子供を治療した。ベースラインおよび最後(12週目)の訪問時に、受容的・表出的語彙検査(receptive and expressive vocabulary test、以下、REVTとも称す)、コア語彙コミュニケーションシステムから得られたコア語彙、中国児童発達インベントリー(Chinese child developmental inventory)、適応行動評価システム−II(adaptive behavior assessment system−II、以下、ABAS−IIとも称す)、育児ストレス指数(parenting stress index、以下、PSIとも称す)、臨床全般印象改善尺度(clinical global impression−improvement scale、以下、CGI−Iとも称す)の評価を行った。
本試験は、安息香酸ナトリウムを用いてASDsの非社交的な子供(non−communicative children)を治療するために経験を積むことを目的とした12週間の非盲検試験であった。参加者は、台湾の高雄医学大学病院の小児科から募集された外来小児患者の便宜的なサンプル(convenient sample)であった。
これらの子供たちは全員、DSM−IV(Association,2000)により自閉症障礙と診断された。本試験に入る前に、彼らは全員、DSM−5基準を用いて精密な再評価を受けて、ASD基準(American Psychiatric Association,2013)を満たしていることを確認された。本試験への参加に関する他の選択基準は、(1)子供は現在深刻なコミュニケーション問題を抱えていること;(2)子供はコミュニケーションのために拡張画像交換コミュニケーションシステムを受けていないこと;および(3)親は在宅訓練の要求に協力できることであった。他の精神科薬物治療を受けている子供については、本試験に入る前に、少なくとも2ヶ月間において薬物を安定な投与量で維持しており、臨床試験中においても変更しないことが必要であった。付随する教育的、職業的、身体的または行為的な治療は許可されたが、本試験で提供されるコミュニケーション訓練以外、新たな治療の追加は許可されなかった。研究プロトコルは、上記の病院の施設内審査委員会(登録番号:F(I)−20150003)によって承認された。
年齢が3年7ヶ月〜9年10ヶ月である5人の男の子および1人の女の子からなる6名の子供に、12週間の安息香酸ナトリウム治療を受けさせた。体重が15kg以上である子供には、500mg/日の安息香酸塩を与えた。体重が15kg未満である子供には、250mg/日の安息香酸塩を与えた。安息香酸ナトリウムは、Excelsior Pharmatech Labs社(台湾)から提供された。
この試験に参加する全ての子供は、中国語版のコア語彙コミュニケーションシステム(Unlimiter Assistive Technology Engineering Lab、台湾)を使用したコミュニケーション訓練を受け始めた。親は、このシステムを使用して自宅で1日40分間子供に教えるように要求された。親は、12週間の期間内において、2週間ごとに、子供を病院に連れ戻すように要求された。ベースラインおよび最後(12週目)の訪問時は、以下の評価を受けさせた。
1.中国語版の受容的・表出的語彙検査(REVT)
REVTは、年齢が3歳〜6歳11ヶ月である子供の言語能力を評価し、また、言語発達遅延の7歳以上の子供をも評価する。規範的データを有するREVTの中国語版は、2011年から市販されている(Hwang,2011)。中国語版REVTには、受容的部分および表出的部分という二つの部分がある。検査の結果は、通常、点数であり、基準準拠の標準点数として表示された。しかしながら、これらの非言語的な参加者は検査不可能であったか、またはフロア点数で完成したため、結果は、標準化された基準と比べて、達成された百分位数で二つの領域(すなわち、受容的および表出的)により報告された。
中国語版のコア語彙コミュニケーションシステムにおいて、合計で72個のコア語彙が絵に描かれており、(1)「私の言っている言葉を示す絵に指差すこと」により絵を特定するように参加者に依頼すること、および(2)それぞれの絵に名前を付けるように参加者に依頼すること、という二つの方式で検査された。結果は、子供が12週間の期間内で学習した語彙で報告した。
中国児童発達インベントリー(CCDI中国語版)(Chu,2007;Ko et al.,2008)は、7つの領域(すなわち、粗大運動、微細運動、理解力、表出的言語、状況理解、個人−社会化および自立性)について、320個の項目の発達の親報告尺度であった。「一般的な発達」と称される総合領域は、7つの領域に由来するものであり、通常、全般的な発達の指数として使用された。発達指数(DQ)は、一般的な発達で得られた月数を暦年齢で割り、100を掛けるによって算出した。結果は、前−DQおよび後−DQで報告された。
ABAS−IIは、適応行動の別々に管理される基準準拠尺度である(Harrison and Oakland,2003)。親には、本技術分野におけるコミュニケーション、コミュニティ利用、機能的学問、家庭生活、健康/安全、余暇、自己主導性および社交技能の情報を提供した。全般適応複合(GAC)点数は、9つの技能領域点数全てから算出され、基準準拠の標準点数として表示された。結果は、前−GAC、後−GACの点数および社交点数で報告された。
主な介護者が中国語版の育児ストレス指数(Wen,2003)を記入し、それはAbdinにより開発されたオリジナルの質問紙(Abidin,1986)の検証済みの中国語版であり、親の機能の側面について評価するものであった。PSIにおける親領域の尺度には54個の項目が、子供領域の尺度には47個の項目が、それぞれ含まれた。13個の下位尺度に加えて、親領域および子供領域は、総合点数および派生した原始百分位数点数(derived raw−to−percentile score)を生成した。験證済みの中国語版PSIのマニュアルで報告されているように、「総合ストレス点数」は、異常なバンドのための286のカットオフ点数(85%の百分位数を超える前記派生した原始百分位数点数に相当)を使用した(Wen,2003)。
CGASは、18歳未満の子供および若者に対して、子供の心理および社会機能に関連する側面の評価に基づいて、臨床医が1〜100点における単一の点数を与えることで、完成された。前記点数は、「極度に障害がある」(1〜10)から「非常にうまくやっている」(91〜100)までの10個のカテゴリーのいずれかに分類された。
CGI−Iは、試験への参加時の状態と比べて、全般的な疾患の改善を評価する観察者評価尺度であった(Guy,1976)。前記改善は、(1)極めて著しい改善、(2)著しい改善、(3)僅かな改善、(4)変化なし、(5)僅かな悪化、(6)著しい悪化、および(7)極めて著しい悪化、という1〜7の範囲の応答で評価された。
子供Aは、3歳1ヶ月の時にASDと診断された男の子であった。彼は、すでに共同注意、個別の言語治療、行為訓練および運動訓練を中心とした機軸反応訓練を受けていた。本試験に入る際に、彼は年齢が4歳4ヶ月であり、いくらかの異なる種類の自動車およびブルドーザーの名称を言えた。彼は扉を指差して出かけたいと示したり、数字を指差して親に大きな声で朗読するように要求したりことができた。彼は少し微笑んで、普段は床に横たわったり、自分で車を押し回ったりした。本試験に入ってから12週間後、子供Aは著しく改善したと見なされた。最後の評価の当日に、彼は微笑んで自発的に短い言葉で喋った(例えば、お茶の写真を見ながら「お茶は欲しくない。お水が欲しい。」)。「目」の絵を見せて彼を試す時、彼は「目、僕の目に目薬が入るのはいやだ」と答えた(目の感染症のために眼科医を訪れた経験をご参照)。しかしながら、彼は依然として相互の会話ができなかった。

Claims (15)
- 安息香酸塩とその薬学的に許容される賦形剤とを含有し、必要のある対象における自閉症スペクトラム障害を予防または治療するための組成物。
- 前記安息香酸塩は、安息香酸ナトリウム、安息香酸カリウム、安息香酸カルシウム、2−アミノ安息香酸塩、3−アミノ安息香酸塩または4−アミノ安息香酸塩である、請求項1に記載の組成物。
- 前記安息香酸塩は、安息香酸ナトリウムである、請求項1に記載の組成物。
- 前記自閉症スペクトラム障害は、自閉症、アスペルガー症候群、小児期崩壊性障害または広汎性発達障害から選ばれる、請求項1に記載の組成物。
- 前記対象は、自閉症スペクトラム障害を患っている子供である、請求項1に記載の組成物。
- 前記対象の年齢は、2〜12歳である、請求項5に記載の組成物。
- 前記対象の年齢は、3〜9歳である、請求項5に記載の組成物。
- 前記対象の年齢は、5〜8歳である、請求項5に記載の組成物。
- 前記対象に投与される安息香酸塩の量は、100mg/日〜2000mg/日である、請求項1に記載の組成物。
- 前記対象に投与される安息香酸塩の量は、150mg/日〜1000mg/日である、請求項1に記載の組成物。
- 前記対象に投与される安息香酸塩の量は、200mg/日〜750mg/日である、請求項1に記載の組成物。
- 前記対象に投与される安息香酸塩の量は、250mg/日〜500mg/日である、請求項1に記載の組成物。
- 2ヶ月間〜2年間の範囲の期間で前記対象に投与される、請求項1に記載の組成物。
