現在入手可能であるヒドロゲル(ゼラチン、ポリ(1−ビニル−2−ピロリジノン)、ポリビニルアルコール(PVA)、ポリアクリルアミド(PAA)、4−アームポリ(エチレングリコール)(テトラ−PEG)を含む)などのポリマーヒドロゲル、架橋ヒアルロン酸、又は自己組織化ペプチドゲルは、天然の硝子体に類似していることから、考え得る硝子体代替物であるが、それらを硝子体タンポナーデとして使用することを妨げるいくつかの欠点が存在する。それらの欠点としては、(i)予備成形したポリマーヒドロゲルが、小孔針を通しての注射後に引張強度を維持することができないこと、(ii)網膜裂孔周囲での脈絡網膜癒着の形成を妨げる素早い生分解性(すなわち、短い滞留時間)、及び(iii)重度の眼内炎症及び長期的な毒性に繋がる低い生体適合性、が挙げられる。多くの場合、網膜裂孔全体に充分な表面張力を及ぼす能力は、評価されてこなかった、又は不充分であることが見出されてきた。例えば、いくつかの市販のヒドロゲルは、網膜裂孔の下に流れる傾向があることが示されており、網膜の再剥離を起こす結果となる。

本明細書で開示される様々な実施形態に従って設計された例示的なポリマーの化学構造を、スキーム1に示す。ポリマーは、ウレタン結合を介して一緒に結合された親水性ポリ(エチレングリコール)(PEG)、感熱性ポリ(プロピレングリコール)(PPG)、及び、限定されないが、生分解性ポリ(ε−カプロラクトン)(PCL)セグメントなどである疎水性生分解性ポリエステル、から成る3成分マルチブロック熱ゲル化ポリマーである。
本明細書で開示される様々な実施形態に従うポリマーを製造するための一般的工程は、スキーム1に示されるように:1又は複数の親水性ポリマー、1又は複数の疎水性ポリマー、及び1又は複数の感熱性ポリマーを、金属触媒(以下の例では、ジブチルスズジラウレートを用いた)及び適切な溶媒(以下の例では、トルエンを用いた)の存在下、カップリング剤(以下の例では、1,6−ジイソシアナートヘキサンを用いた)と混合することを含む。
ポリ(PEG/PPG/PCLウレタン)を、PEG、PPG、及びPCL−ジオールから、1,6−ジイソシアナートヘキサンをカップリング剤として用いて合成した。添加した1,6−ジイソシアナートヘキサンの量は、溶液中の反応性ヒドロキシル基に対して等量であった。典型的には、0.25gのPCL−ジオール(Mn=2040、1.23×10−4mol)、5gのPEG(Mn=1890、2.65×10−3mol)、及び5gのPPG(Mn=1880、2.66×10−3mol)を、250mLの二口フラスコ中、高真空下、50℃で一晩乾燥した。次に、200mLの無水1,2−トルエンをフラスコに添加し、系中のいかなる微量の水も共沸蒸留によって除去し、フラスコ中には僅かに40mLのトルエンしか残らなかった。フラスコが75℃まで冷却された時点で、0.912gの1,6−ジイソシアナートヘキサン(5.43×10−3mol)及び2滴のジブチルスズジラウレート(約8×10−3g)を順に添加した。この反応混合物を、窒素雰囲気下、60〜110℃で24〜48時間にわたって撹拌した。得られたコポリマーを、ジエチルエーテルから沈澱させ、クロロホルムに再溶解することによってさらに精製し、続いてメタノール及びジエチルエーテルの混合物中に沈澱させた。単離及び精製後の収率は、85%であった。

本明細書で開示される様々な実施形態に従って設計されたヒドロゲルを、開発し、特性決定した。特性決定の試験から得られた結果を、以下に提供する。
振動応力掃引測定実験を、本明細書で開示される様々な実施に従って設計された例示的ヒドロゲルに対して行い、結果を図2に提供する。示されるように、ヒドロゲルの機械的存続性を、3桁の応力にわたって維持することができる。
図3は、温度及びポリマー濃度の変動による、本明細書で開示される様々な実施形態に従って設計された例示的ヒドロゲルの粘度の変化を示す。示されるように、粘度は、(1)個々の成分の比を変動させることによって、又は(2)溶液中のポリマー濃度を変動させることによって調節することができる。
図4は、平衡塩類溶液(BSS)中、pH7.4及び37℃で測定した場合の、本明細書で開示される様々な実施形態に従って設計された例示的ヒドロゲルの質量減少率(単位は%)の変化を示す。分解時間は、特定のいずれの臨床的適用にも合わせて変動可能である。例えば、ヒドロゲルは、網膜剥離(RD)修復のための短期的、中期的、又は長期的タンポナーデとして設計、開発することができる。
水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液のそれぞれの表面張力を、それぞれ24℃及び37℃の特定の温度でDuNouyリングを備えた表面張力計で測定した。熱ゲル化溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。結果を図5に示す。
図6は、せん断速度=1.25秒−1及び温度勾配=3℃/分で測定された、水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液の様々な温度での粘度の変化を示す。熱ゲル化水溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。
水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液のそれぞれの導電率を、pH調節後(7.2〜7.3)、それぞれ24℃及び37℃の特定の温度で測定した。エンドゾルバッファーのpHは、7.6である。熱ゲル化溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。結果を図7に示す。
水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液の温度依存性を、歪み=1%、周波数=1Hz、及び温度勾配=3℃/分で測定した。熱ゲル化水溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。
歪み振幅掃引実験を、水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液に対して、それぞれ25℃及び37℃の特定の温度で行った。熱ゲル化水溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。
周波数掃引実験を、水中に3%(重量/体積)、5%(重量/体積)、及び7%(重量/体積)のゲルを有する熱ゲル化溶液に対して、それぞれ25℃及び37℃の特定の温度で行った。熱ゲル化水溶液において、PEG:PPGの比は、4:1であり、PCLの濃度は、ポリマーの1重量%である。
10重量%ポリマーのサンプルを、誘導結合プラズマ質量分析(ICP−MS)のためにTUV SUD PSB Pte.Ltd.に送付した。サンプルを水に溶解し、続いてICP−MSを用いて元素分析した。表2は、TUV SUDによって提供されたICP−MSの結果を示す。
xi)残留有機溶媒試験
10重量%ポリマーのサンプルを、残留有機溶媒試験のためにTUV SUD PSB Pte Ltdに送付した。およそ1グラムのサンプルを、20mlのガラスバイアル中に密封し、90℃で20分間加熱し、その後水素炎イオン化検出器を備えたヘッドスペースガスクロマトグラフ(GC−FID)によって分析した。表3は、TUV SUDによって提供されたGC−FIDの結果を示す。
例3:ポリマーの生体適合性
本明細書で開示される様々な実施形態に従って設計されたポリマーの生体適合性を評価するために、生体外及び生体内試験を行った。以下の例で示されるように、ヒドロゲルは、生体適合性(眼内において)であり、したがって、眼科的使用に適している。
希釈ゲル溶液を、96ウェルプレート上のコンフルエントな網膜色素上皮ARPE−19細胞(ヒト細胞株)に適用し、24時間インキュベートした。MTTアッセイによって細胞生存率試験を行った。簡潔に述べると、MTTアッセイの前に、培養物を新しい培地で置換する。10μlのMTT溶液(5mg/ml)を各ウェルに添加し、37℃で4時間インキュベートした。MTT溶液の除去後、100μlのDMSOを各ウェルに添加してホルマザン結晶を溶解した。プレートを、室温で10分間暗所に保持し、その後、分光光度計を用いて570nmでの吸光度を読み取った。生体外試験に用いたゲル溶液は、(1)水中の5%(重量/体積)PEG/PPG/PCLコポリマーゲル、(2)水中の5%(重量/体積)PEG/PPG/PHBコポリマーゲル、及び(3)水中の20%(重量/体積)PEG/PPGコポリマーゲルである。ゲル溶液は、それぞれ1:1、1:2、1:4、1:10、及び1:20の希釈率で、BSS溶液を用いて希釈した。結果を図8に示す。
ii)生体内試験
生体内試験では、22体のアルビノウサギを用いた。硝子体切除し、(0.5〜1.0ml)サーモゲルを実験用の眼に注射した。硝子体切除し、平衡塩類溶液(BSS)を充填した眼を、コントロールとして用いた。スリットランプ、スペクトルドメイン光干渉断層撮影法(SD−OCT)、カラー眼底などのイメージング、並びに網膜電図検査(ERG)、及び眼内圧(IOP)などの機能試験を行うことを含む、3ヶ月フォローアップの生体内試験を行った。
IOP測定に用いたサーモゲルは、(1)PEG/PPG/PCLコポリマー、(2)PEG/PPG/PHBコポリマー、及び(3)PEG/PPGである。コントロールとしてはBSSを用いた。測定は28日間にわたって行い、結果を図9に示す。

手術の1ヶ月後のSD−OCT
手術の1ヶ月後に撮影したウサギの眼のスペクトルドメイン光干渉断層撮影法(SD−OCT)画像を、図11に提供する。SD−OCT画像及び組織学的検査を図12に提供する。
ウサギの眼の暗順応網膜電図を図13に提供する。手術後の網膜電図(ERG)で、b波の低下が観察され、それは、3ヶ月後まで、ゆっくり回復していた。硝子体切除及びサーモゲル置換の3ヶ月後のウサギにおける暗順応及び明順応ERGの波形は、正常な網膜構造及び網膜機能の両方を示した。
図14は、本明細書で開示される様々な実施形態に従って設計されたポリマーの生体適合性/無毒性を実証するための実験設定を示す模式的フローチャート200である。工程202では、25℃から37℃への温度変化で、ゾルゲル転移が発生している。ゾル状態のポリマー210は、37℃で素早くサーモゲル212を形成し、25℃に冷却すると、転移してその元の状態210に戻ることができる。工程204では、サーモゲル212が、網膜色素上皮(RPE)細胞に適用され、前記細胞214に対して細胞生存率試験が行われる。工程206では、硝子体切除の過程で、サーモゲル212が、ウサギの眼216に注射される。工程208では、ウサギの眼は、手術後少なくとも6ヶ月までフォローアップされる。
以下の例4、5、及び6では、開発したコポリマーサーモゲルは、ウレタン結合を介して一緒に結合された親水性PEG、感熱性ポリプロピレングリコール)(PPG)、及び疎水性生分解性ポリ(ε−カプロラクトン)(PCL)セグメントから成る(以降「EPC」と称する)。PEG:PPGの比は、4:1であり、PCLの濃度は、EPCポリマーの1重量%である。
異なる濃度(3重量%、7重量%、12重量%)のサーモゲルのゾルゲル転移特性を、レオロジー分析によって特性決定した。
溶液の表面張力測定を、特定の温度でDuNouyリングを用いた表面張力計で行った(図15E)。EPC−12%は、粘度が高過ぎて試験を行うことができなかった。データは、10〜30のデータ点の平均±SDを表す。スチューデントt検定を用いて統計分析を行った。#p<0.05、24℃のエンドゾルバッファーと比較。*p<0.05、37℃のエンドゾルバッファーと比較。
EPCサーモゲルの生体内生体適合性を、ニュージーランドホワイト種(NZW)ウサギの眼科手術モデルを用いて実証した。23ゲージ中心部硝子体切除を、NZWウサギに対して行い、25眼にゾル状態のEPCサーモゲルを注射した。すべてのEPCサーモゲルについて、硝子体網膜手術の過程で、硝子体内でゲルをin−situ形成したことが観察された。これらのウサギを、手術後6ヶ月まで、考え得る合併症についてフォローアップした。
手術非ヒト霊長類(NHP)網膜剥離モデルにおいて、内部タンポナーデ材としての適切性及び有効性を確認した。
EPCサーモゲルの移植後の眼内圧(IOP)を、図21に提供する。図21Aは、ウサギにおけるIOPフォローアップを示す。手術を受けていないウサギの眼の平均IOPは、14.3±1.8mmHg(10.6〜16.4mmHgの範囲)である。3%及7%EPCサーモゲルを移植したウサギの平均IOPは、正常限界内である(13.5±2.6mmHg、7.6〜18mmHgの範囲)。図21Bは、2体のNHPにおけるIOPフォローアップを示す。EPC7%を移植した両方の眼のIOPは、正常限界内に維持されていた(12〜17mmHgの範囲)。
手術後3ヶ月において、硝子体腔は、光学的に透明に維持されていた。前眼部の炎症又は白内障形成の徴候はなかった。重要なことには、IOPが、12〜17mmHgの正常範囲内に維持されていた(図21B)。さらに、12ヶ月において網膜が接着した状態で維持されており、これは、NHPに強制することはできない厳密な腹臥位とすることなく実現された。このことは、本発明で開示されるゲルが、ガス又はオイルによる場合のように浮力に依存する必要なしに、表面張力及び膨張対抗力を通して有効な内部タンポナーデとして機能する能力を実証するものである。
手術の3ヶ月後に眼球除去したウサギの眼球を解剖すると、自然の硝子体に類似の堅牢性を有する硝子体様体が形成されていた(図24)。生分解特性と一致して、3ヶ月までにゲルがもはや存在しないことを示すために、ゲルポリマーは、質量分析(MS)(図25)又は核磁気共鳴(NMR)(図26)によって、3ヶ月で検出されていない。
当業者であれば、広く記載した本開示の趣旨又は範囲から逸脱することなく、本明細書で開示される実施形態に対して他の変更及び/又は改変が成されてよいことは理解される。例えば、本明細書の記述において、異なる例示的実施形態の特徴について、異なる例示的実施形態にわたって混合、組み合わせ、交換、組み込み、適合、改変、包含などが成されてよい。本発明の実施形態は、したがって、あらゆる点において、限定的ではなく、例示的として見なされるべきである。
Claims (25)
- マルチブロック熱ゲル化ポリマーを含む材料であって、前記マルチブロック熱ゲル化ポリマーは、親水性ポリマーブロック、感熱性ポリマーブロック、及び疎水性ポリマーブロックを含み、
前記親水性ポリマーブロック、前記感熱性ポリマーブロック、及び前記疎水性ポリマーブロックは、ウレタン/カルバメート結合、カーボネート結合、エステル結合、又はこれらの組み合わせのうちの少なくとも1つによって化学的に一緒にカップリングされており、
並びに、前記材料は、硝子体代替物として用いるのに適している、
材料。 - 前記親水性ポリマーブロック及び前記感熱性ポリマーブロックが、各々、ポリ(アルキレングリコール)を含む、請求項1に記載の材料。
- 前記親水性ポリマーブロックの前記ポリ(アルキレングリコール)が、前記感熱性ポリマーブロックの前記ポリ(アルキレングリコール)と異なる、請求項2に記載の材料。
- 前記親水性ポリマーブロックが、ポリ(エチレングリコール)を含み、前記感熱性ポリマーブロックが、ポリ(プロピレングリコール)を含む、請求項1から3のいずれか一項に記載の材料。
- 前記疎水性ポリマーブロックが、ポリエステルを含む、請求項1から4のいずれか一項に記載の材料。
- 前記疎水性ポリマーブロックが、ポリ(カプロラクトン)を含む、請求項1から5のいずれか一項に記載の材料。
- 前記親水性ポリマーブロックの前記感熱性ポリマーブロックに対するモル比が、1:1〜10:1の範囲内である、請求項1から6のいずれか一項に記載の材料。
- 前記疎水性ポリマーブロックが、前記マルチブロック熱ゲル化ポリマーの1重量%〜10重量%の量で存在する、請求項1から7のいずれか一項に記載の材料。
- 前記材料が、水性媒体中に1重量/体積%〜30重量/体積%の前記マルチブロック熱ゲル化ポリマーを含む、請求項1から8のいずれか一項に記載の材料。
- 前記材料が、60重量%超の高い含水率を有する、請求項1から9のいずれか一項に記載の材料。
- 前記材料が、7.1〜7.7の範囲内のpH値を有する、請求項1から10のいずれか一項に記載の材料。
- 前記材料が、20℃〜30℃の範囲内に入る温度では、流動性状態であり、36℃〜40℃の範囲内に入る温度では、非流動性ゲル様状態である、請求項1から11のいずれか一項に記載の材料。
- 前記材料が、実質的に透明である、請求項1から12のいずれか一項に記載の材料。
- 前記材料が、1.20〜1.48の範囲内に入る屈折率を有する、請求項1から13のいずれか一項に記載の材料。
- 前記生体材料が、実質的に金属を含まない、請求項1から14のいずれか一項に記載の材料。
- 請求項1から15のいずれか一項に記載の材料を製造する方法であって、
1又は複数の親水性ポリマー、1又は複数の感熱性ポリマー、及び1又は複数の疎水性ポリマーを、前記親水性ポリマーブロック、前記感熱性ポリマーブロック、及び前記疎水性ポリマーブロックがウレタン/カルバメート結合、カーボネート結合、エステル結合、又はこれらの組み合わせのうちの少なくとも1つによって化学的に一緒にカップリングされるように一緒にカップリングして、マルチブロックポリマーを形成すること、
を含む、方法。 - 前記1又は複数の親水性ブロック、1又は複数の感熱性ブロック、及び1又は複数の疎水性ブロックが、1〜10:1:0.01〜1.5のモル比で混合される、請求項16に記載の方法。
- 前記混合工程が、70℃〜150℃の範囲内の高められた温度で行われる、請求項17に記載の方法。
