本発明は、概して、組織工学、より具体的には表面が機能化された医療用インプラントを製造する方法に関する。

医療用インプラントの作製
補綴インプラントは、歯のない人を処置するためおよび骨格異常を患った患者において運動能を回復させるために日常的に使用されている。チタン(Ti)は、移植後に周囲の骨と安定な結合を形成することができる(オッセオインテグレーションとして公知となっているプロセス)ことから、補綴分野で好まれる材料である。しかし、補綴インプラントの完全な融合には時間がかかり、限定的な骨の量および/または損なわれた再生能によって特徴づけられる臨床状況においてはならびに危険にさらされている患者では失敗する。したがって、費用効果が高く、安全であり、かつ各臨床状況下の各患者に適する新しいインプラントを開発するために多大な研究努力がなされている。本研究において、幹細胞を用いてTiインプラントを機能化する可能性を調査した。ヒト人工多能性幹細胞由来間葉前駆(iPSC-MP)細胞を、骨形成条件下で2週間、Tiモデルディスク上で培養した。次いでサンプルを、細胞を洗い流しマトリクスを露出させる4つの異なる除細胞化法を用いて処理した。最後に、機能化されたディスクを滅菌し、細胞の挙動に対する幹細胞を通じた表面の機能化の効果を調べるために新しいヒトiPSC-MP細胞を播種した。その結果は、異なる除細胞化法が多様な表面修飾を提供すること、およびこれらの修飾がヒトiPSC-MP細胞の増殖を促進し、発生および分化に関与する遺伝子の発現に影響し、アルカリホスファターゼの放出を刺激することを示している。細胞を通じた機能化は、補綴インプラントの表面を修飾する生物学的妥当性のある魅力的な戦略であり、今までなかった、強化された治療能力を有する新しいデバイスの開発の可能性を切り開く。
チタンディスクの製造および特徴づけ
付属クッションと共に9 mmパンチツールを装備したAmada CNCパンチを用いて、平面シート(グレード2チタン, Edstraco, Sweden)からチタンディスク(面積:0.694 cm2、厚み 0.5 mm)を切り抜いた。70%エタノール(v/v)中でリンスした後、サンプルをアセトン中で30分間超音波処理し、SEM、表面形状測定、EDSおよびXPSを通じて表面特性を調べるために特徴づけた。SEM分析のために、以下の設定でFEI Helios NanoLab(商標)660(FEI, Hillsboro, OR)を用いてサンプルを画像化した:10 kV、0.8 nAおよび5.4 mmの作業距離。表面プロフィールおよび粗さは、0.5倍の倍率でWyko T1100(商標)表面形状測定装置VSIモード(Bruker, Billerica, MA)およびVisionソフトウェアを用いて測定した。表面化学は、10 keVおよび40〜60%の範囲のデッドタイムでFEI Helios NanoLab(商標)660(FEI)および補助的なAZtec(商標)ソフトウェアを用いて調べた。XPSデータは、45°のテイクオフ角で取得した。Multipak(Physical Electronics, Inc. (PHI))を、さらなる定量的化学分析に使用した。200 x 200μmのサイズの領域からC1、N1、O1、P2pおよびCa2pスペクトルを取得した。
播種の前に、ディスクを滅菌ポーチ(Fisher Scientific, Pittsburgh, PA)に入れ、オートクレーブした。次に、滅菌した鉗子を用いて、ディスクを24ウェルプレート(Thermoscientific, Roskilde, Denmark)内に設置し、高グルコースKnockOut(商標)ダルベッコ改変イーグル培地(KO-DMEM; Gibco, Grand Island, NY)、10%(v/v)HyCloneウシ胎仔血清(GE Life Sciences, Pittsburgh, PA)、ベータ線維芽細胞成長因子(1 ng/ml;R&D systems, Minneapolis, MN)、GlutaMax(商標)(1X;Gibco)、非必須アミノ酸(1X;Gibco)、0.1 mM β-メルカプトエタノール(Gibco)および抗菌抗真菌剤(1X;Gibco)からなる1 mlの増殖培地と共に37℃で一晩調整を行った。この処理の後、ディスクを滅菌したキムワイプ(Roswell, CA)で吸い取り、およそ10分間、風乾した。細胞接着性を向上させるために、ディスクを37℃でゼラチン(0.1%;EmbryoMax(登録商標), Millipore, Billerica, MA)により90分間処理した。細胞播種の前に、過剰なゼラチンをディスクから吸い取って除去し、新しい24ウェルプレートに移した。
ヒトiPSC由来間葉前駆細胞(1013A株)を以前に記載されたようにして得た[27]。播種前に、細胞を、増殖培地中、ゼラチン(0.1%;EmbryoMax(登録商標), Millipore)コーティングされたプラスチック上で増殖し、その後にトリプシン/EDTA(0.25%;Thermoscientific)を用いて剥離させ、遠心分離し、2 x 104細胞/mlの密度で再懸濁させた。次いで1 mlの細胞懸濁物を各ディスクに添加した。増殖培地中で2日間の後、サンプルを、10%(v/v)HyClone(商標)ウシ胎仔血清(GE Life Sciences)、L-アスコルビン酸(50μM;Sigma-Aldrich, St Louis, MO)、デキサメタゾン(1μM;Sigma-Aldrich)およびβ-グリセロリン酸二ナトリウム塩(10 mM;Sigma-Aldrich)を補充した高グルコースDMEM(Gibco)からなる骨形成培地中でさらに12日間培養した。
Tiディスク上での細胞の分布および増殖を、生/死アッセイ(Life Technologies, Frederick, MD)を用いてモニタリングした。播種から2日および2週間後、サンプルを、37℃で35分間、カルセイン(10 mlのPBS中5μlのカルセイン)を用いて染色した。インキュベート後、臭化エチジウムを各ウェルに添加し(1 mlのPBSあたり2μl)、サンプルをさらに10分間インキュベートした。インキュベート後、サンプルを、フェノールレッド非含有Roswell Park Memorial Institute培地1640(RPMI; Gibco)に移し、落射蛍光および共焦点顕微鏡を通じて画像化した。モザイク画像を作製するため、画像化プログラムOlympus DP2-BSWを備えたOlympus IX71顕微鏡(Olympus, Tokyo, Japan)を用いて倍率4倍で連続する蛍光画像を撮影した。サンプル全体の画像を得るため、MosaicTJ(商標)およびTurboReg(商標)プラグインを導入したImageJ(商標)(National Institute of Health)を用いて連続する蛍光画像からモザイク像を構築した。共焦点画像は、Zen 9000(商標)コンピュータプラットフォームを用いてZeiss(商標)顕微鏡(Zeiss, Oberkochen, Germany)により撮影した。
14日間の培養後、サンプルを収集し、4つの異なる除細胞化プロセスを用いて細胞を溶解および除去するよう処理した。処理前に、すべてのサンプルを、24ウェルプレート内で、0.1% EDTA(w/v)を含む10 mM Tris緩衝液を用いて室温(RT)で30分間処理した。次いでサンプルをRTで5分間、PBSで二度洗浄し、以下に記載されるプロトコルを用いて除細胞化した。
特徴づけの前に、サンプルを、各々10分間、漸増濃度、すなわち20、40、50、70、90および100%(v/v)のエタノール溶液を用いて脱水した。次いでサンプルを風乾し、各方法の除細胞化能を試験するためにSEM、EDSおよびXPSを通じて特徴づけた。非除細胞化サンプルをパラホルムアルデヒド4%(v/v)中4℃で一晩固定し、対照として使用した。SEM分析のために、サンプルの伝導度を増加させるようサンプルを10〜50 nm金層を用いてスパッタし、その後にFEI Helios NanoLab(商標)660(FEI)を5〜10 kVの電圧、0.8 nAの電流および4.9〜5.4 mmの作業距離で用いて画像化した。サンプルの側面画像のために、脱水したサンプルを15分間100%イソプロパノール中で二度インキュベートし、15分間キシレン中で二度洗浄し、そしてEMBed-812キット(Electron Microscopy Sciences, Hatfield, PA)を製造元の説明書にしたがい用いてポリメチルメタクリレート樹脂に包埋し、低速切断機(Isomet(商標), Buehler, Lake Bluff, IL, USA)を用いて垂直に切片化した。およそ300 pm厚の切片を、10 nm金層を用いてスパッタコーティングし、以下の設定でSEMを用いて画像化した:3 kV、80 pAおよび作業距離5 mm。
機能化されたTiディスクに対する細胞の応答を、増殖ならびに中胚葉分化に関与する発生遺伝子およびマーカーの発現を調べることによって調査した。未処理のTiディスクを対照として使用した。漸減濃度のエタノール溶液を用いた再水和後、上記のようにサンプルを24ウェルプレートに移し、1013A-MP細胞(P6)を増殖培地中2 x 104細胞/mlの密度で播種した。播種から2日後、サンプルを新しい24ウェル超低接着性プレートに移し、骨形成培地中でさらに8日間培養した。
細胞の増殖を定量化するため、サンプルを、骨形成培地中10%(v/v)のPrestoBlue(商標)試薬(Life Technologies)と共に2時間インキュベートした。処理後、200μlの培養培地を、黒色、透明、平底96ウェルプレート(Corning)に移した。蛍光を、Gen 5 1.09ソフトウェアを装備した蛍光読み取り装置SYNERGYMx(商標)(BioTek(登録商標), Winooski, VT)を用いて励起560 nmおよび発光590 nmで測定した。
発生遺伝子の発現レベルを、NanoString nCounter(商標)システム(Nanostring Technologies(登録商標), Seattle, WA)を用いて同時に調査し、データを、nSolver(商標)2.5ソフトウェアを用いて分析した(すべての調査した遺伝子のリストについては表S1を参照のこと)。簡単に説明すると、サンプルをRLT緩衝液(Qiagen, Venlo, NE)に溶解し、RNeasy(商標)ミニキット(Qiagen)を製造元の説明書にしたがい用いて総RNAを抽出した。次いで抽出したRNAを、NanoDrop(商標)8000(Thermo Scientific)を用いて定量し、100 ngのサンプルをハイブリダイゼーション(65℃で18時間)に使用した。調査した遺伝子の発現レベルは、nCounter(商標)多重アッセイにおいて検出された、対数スケールの、蛍光数量として表されている。階層的クラスター分析を、R(R-project.orgのワールドワイドウェブで入手可能)のRStudio(商標)パッケージを用いて正規化されたデータから行った。
放出されたアルカリホスファターゼ(ALP)の活性は、アルカリホスファターゼ活性アッセイキット(商標)(Biovision Inc., Milpitas, CA)を製造元のプロトコルにしたがい用いて定量した。反応は、室温の暗所で、150分間かけて行った。蛍光を、Gen 5 1.09ソフトウェアを装備したプレートリーダーSYNERGYMx(商標)(BioTek(登録商標), Winooski, VT)を用いて450 nmで測定し、データを、(2.7.1節に記載される)Prestoアッセイを用いて推定される細胞数あたりで正規化した。ALP活性は、「反応時間あたりに切断されるPNPのモル量」で表される。
分析前に、データを、Prismソフトウェア(Graphpad, La Jolla, CA)を用いて正規性について試験した。ガウス分布を、コルモゴロフ・スミルノフ検定を用いて検出した。有意差を、多重比較のためにボンフェローニ補正を用いるANOVAを用いて評価した。すべての結果が、平均±標準偏差で示されている。各グループの平均値間の差は、そのP値が0.05未満であった場合に、統計学的に有意とみなした。
チタンディスクの表面トポグラフィーおよび組成
Tiディスクのトポグラフィーおよび元素組成を、SEM、表面形状測定、EDS(図2)およびXPSを用いて調べた。SEM顕微鏡写真は、Tiディスクが、ディスク領域を通じて全体に広がるフレーク様構造が存在する滑らかなトポグラフィー(図2B)、ならびにRa = 511.6 nm、Rq = 649.9μmおよびRt = 7.05μmの粗さパラメータ(図2C)を示すことを示している。EDSを通じた元素分析は、このディスクのTi組成を確認し、サンプル内に微量の炭素が存在することを明らかにしている(図2D)。Si、Al、Mo、Ca、P、NaおよびFeを含む他の元素は、無視できる量しか存在しない。同じサンプルから得られたXPSの結果は、Ti、CおよびOならびにN、CaおよびSiの典型的なスペクトルピークを示している。
Tiディスク上での細胞の播種および増殖は、生/死アッセイを通じて示された。図3は、播種から2日後に、細胞が生存していること、健常な形態を示すことおよびディスク領域を通じて均等に分布していることを示している。骨形成条件下での14日間の培養後には、細胞が増殖し、ディスク領域全体を覆う単層組織を形成する。誘導培養条件下での細胞の分化を示す、細胞形態の変化も観察される。
マトリクスの完全性を維持しつつ細胞を溶解および除去する異なる方法の能力を調べるため、除細胞化の後、サンプルを特徴づけた。非除細胞化サンプルを対照として使用した。SEM画像は、異なる除細胞化法が異なる表面修飾をもたらすことを示している(図4)。diH2Oによるサンプルの処理および凍結解凍サイクルの繰り返しは、細胞を完全に除去しないようである(図4A)。細胞残留物が依然としてディスクの表面上に存在し得るようである。これらのサンプルのトポグラフィーは、実際、非除細胞化サンプルのトポグラフィーと非常に類似しており、密な細胞層がTiディスクの表面を覆っている(図4A)。他方、エタノールおよびトリトンによる処理は、細胞物質の洗浄および繊維マトリクスの露出により効果的なようである(図4A)。興味深いことに、このマトリクスの見た目は異なっており、このことは、異なる化学物質がマトリクスの組成および完全性に異なる影響を及ぼすことを強調している。エタノール処理されたサンプルは、異なるサイズの組織化された繊維の存在によって特徴づけられるマトリクスを示すのに対して、トリトン処理された細胞は、粒状の特徴および孔を有するマトリクスを示す(図4A)。サンプルを側面から画像化したときにも、違いが観察される(図4B)。この結果は、異なる除細胞化法が、異なる見た目および厚みを有する生物学的コーティングの形成をもたらすことを示している。EDS分析により、生物学的物質によるTiディスクの機能化が確認される。図4Bは、除細胞化および非除細胞化の両方のサンプルがC、NおよびOについて陽性であることを示しており、これはディスクの表面を覆う生物学的物質の存在を示している。同様のデータが、XPSを通じて同じサンプルを分析したときに観察されている(図9)。対照Tiディスクとは異なり、すべての除細胞化サンプルは、C、NおよびOのピークを示し、これによりTiディスクの表面上に位置する生物学的物質の存在が実証されている。
細胞の増殖、遺伝子の発現およびアルカリホスファターゼの放出に対する、細胞を通じた表面機能化の効果を調べるため、除細胞化の後、機能化されたサンプルに、新鮮な1013A-MP細胞を播種した。Prestoブルーの結果(図5)は、2日後に、機能化グループと対照グループとの間で細胞数に関して有意差がないことを示している。しかし、誘導培養条件下で7および10日後、すべての機能化されたディスクは、対照グループと比較して有意に増加した細胞数を示しており、これにより、細胞を通じたTiディスクの機能化が1013A-MP細胞の生存または増殖のいずれかに影響することが示唆されている。興味深いことに、異なる細胞化法が異なる表面修飾をもたらすにもかかわらず、4つすべての機能化グループを比較した場合、有意差は観察されない。

Ti補綴インプラントは、修復歯科および整形外科において広く利用されている。骨空洞部への移植後、および望ましい治癒条件下で、これらのデバイスは、周囲組織に完全に融合し、機能を回復する。しかし、オッセオインテグレーションは、高度に組織化された生体力学的プロセスであり、これは時間がかかり、再生能が損なわれた危険な患者において失敗し、緩みおよび感染等の合併症を引き起こし得る。したがって、融合プロセスにおいて好ましい特徴により補綴インプラントの表面を機能化するよう、学術界および産業界の両方で多大な研究努力がなされている。補綴インプラントの物理的および化学的修飾は、例えば体液分子の接着を調整し、骨-インプラント接触面積を増加させることによって、これらのデバイスの表面エネルギーおよび骨伝導特性を変化させるが、細胞応答および組織再生を能動的に制御することはできない。周囲組織環境をさらに良くするよう調整するため、研究者らは最近、「生物学的に活性な」分子(例えば、ECMタンパク質、サイトカイン、成長因子)の固定化または幹細胞の組み込みを通じて補綴インプラントの表面を機能化することを試みており、見込みのある結果が得られている。しかし、生物学的に活性な分子を(それらの機能的な形態で)固定化することは困難であり、表面の活性化を必要とし得、天然組織に典型的な複雑な分子の組織化を模倣することができない。他方、幹細胞の組み込みは、治癒を誘導し、骨形成を促進することが証明されているが、予期せぬ細胞の挙動を伴う潜在的な臨床的リスクを抱えている。これは特に、その細胞をエクスビボで操作するまたはクローニングもしくは再プログラムを通じて多能性幹細胞から誘導する場合に当てはまる。したがって、安全であり、個人向けにすることができかつ向上した骨伝導および骨誘導特性を示す高い生物学的信頼性を有するコーティングで補綴インプラントを機能化する新しい戦略が構築されなければならない。
補綴インプラントのバイオ機能化は、強化された治療能を有するデバイスの開発を推進することが期待される。この研究において、本発明者らは、ヒト幹細胞を使用して高い生物学的複雑性および異なる機能的特性を有するコーティングを作製することができることを実証した。この技術は、より安全であり、規制要件を満たし、高度な歯科整形外科再建のために個人向けにすることができる、新世代の補綴インプラントを開発する可能性を切り開く。
Claims (69)
- 骨組織に移植されたときにオッセオインテグレーションを促進するようインプラントの表面を機能化するための方法であって、
(a)インプラントの表面に間葉前駆(MP)細胞を播種する工程、
(b)該表面上で細胞外マトリクス(ECM)を生成するよう該細胞を培養および増殖する工程、ならびに
(c)該表面を除細胞化する工程であって、それによってインプラントの表面を機能化する、工程
を含む、方法。 - 除細胞化する工程が、ECMを維持しつつ表面から細胞を除去する、請求項1記載の方法。

- 除細胞化後に表面に再播種する工程をさらに含み、再播種する工程が、表面にMP細胞を播種することを含む、請求項1記載の方法。
- 再播種されたMP細胞を培養および増殖する工程をさらに含む、請求項17記載の方法。
- (b)の培養する工程が、約5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21日超である、または約2〜14日間である、請求項1記載の方法。
- インプラントを滅菌する工程、および任意でインプラントをパッケージングする工程をさらに含む、請求項1記載の方法。
- インプラントを対象の骨組織に移植する工程をさらに含む、請求項1記載の方法。
- 除細胞化後に表面上のECMを分析する工程をさらに含む、請求項1記載の方法。
- (a)の前に、MP細胞を生成するよう多能性幹(PS)細胞を分化および増殖する工程をさらに含む、請求項1記載の方法。

- ネジ山を含む、請求項25記載のインプラント。
- (a)請求項25〜29のいずれか一項記載のインプラント、および
(b)三次元足場を有する骨組織移植片
を含む、ハイブリッド骨インプラント。 - 骨組織移植片が人工物である、請求項30記載のハイブリッド骨インプラント。
- 足場上に播種されかつ細胞とインプラントとのオッセオインテグレーションを促進するよう培養された細胞を、足場が含む、請求項30記載のハイブリッド骨インプラント。
- インプラント材料が、チタン、鋼鉄、PMMA、シリコーン、PEEK、アルミナ、ジルコニア、リン酸カルシウムまたはそれらの1つもしくは複数の組み合わせを含む、請求項30記載のハイブリッド骨インプラント。
- 移植片が、幹細胞由来または前駆細胞由来の細胞を含む、請求項30記載のハイブリッド骨インプラント。
- 移植片が、人工多能性幹細胞由来の細胞を含む、請求項30記載のハイブリッド骨インプラント。
- 足場が、除細胞化された骨組織から本質的になる、請求項30記載のハイブリッド骨インプラント。
- 骨組織が、ウシ骨組織である、請求項36記載のハイブリッド骨インプラント。

(c)間葉前駆(MP)細胞を生成するよう該iPS細胞を分化および増殖する工程、
(d)該MP細胞をインプラントの表面に播種する工程、
(e)表面上で細胞外マトリクス(ECM)を生成するよう該MP細胞を培養および増殖する工程、
(f)表面を除細胞化する工程であって、それによって、骨組織に移植したときにオッセオインテグレーションを促進するようインプラントの表面を機能化する、工程、ならびに
(g)インプラントを対象の骨組織に移植する工程
を含む、医療処置を行うための方法。 - 除細胞化する工程が、ECMを維持しつつ表面から細胞を除去する、請求項46記載の方法。
- 除細胞化する工程が、水、アルコールまたは非イオン性界面活性剤を含む処理溶液中で表面をインキュベートすることを含む、請求項47記載の方法。
- 処理溶液が、脱イオン水からなる、請求項48記載の方法。
- 処理溶液が、エタノールと水との混合物である、請求項48記載の方法。
- 非イオン性界面活性剤が、ポリエチレングリコールtert-オクチルフェニルエーテル(トリトンX-100(商標))である、請求項51記載の方法。
- 表面が、37℃で約30〜60分間インキュベートされる、請求項48記載の方法。
- 除細胞化する工程が、生理学的緩衝液中で表面を凍結させることを含む、請求項46記載の方法。

- 除細胞化後に表面に再播種する工程をさらに含み、再播種する工程が、表面にMP細胞を播種することを含む、請求項46記載の方法。
- 再播種されたMP細胞を培養および増殖する工程をさらに含む、請求項62記載の方法。
- (b)の培養工程が、約5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、21日超である、請求項46記載の方法。
- 培養が、約14日間である、請求項46記載の方法。
