他の態様では、本発明は、医薬品として使用するための上記で定義した177−Lu標識ペプチドに関し、癌、特に、限定されるものではないが、前立腺癌、乳癌及び結腸直腸癌の治療用の医薬品の製造のための上記177−Lu標識ペプチドの使用に関し、並びに本発明の177−Lu標識ペプチドを含む医薬組成物に関する。類似の治療法も本発明により包含される。
本発明のペプチド/キレートコンジュゲートは、このコンジュゲートを177−Lu放射性核種に、例えば、好ましくは水溶性の金属塩として、反応させることによって、標識化される。この反応は、当該技術分野において既知の方法によって行われる。
図1は、177Lu−DOTA−AE105の化学構造並びにHT−29腫瘍異種移植片担持ヌードマウス中の選択された臓器及び原発腫瘍病巣内の177Lu−DOTA−AE105の生体分布を示している。マウスを、注射後0.5、1.0、2.0、4.0及び24.0時間に屠殺し、心臓から引き出された血液を含む、臓器のパネルを回収し、秤量し、放射能含量について分析した。各ポイントで3匹のマウスから得たグラム当たりの注入線量の平均パーセンテージ(%ID/g)±SEMが提示される。A:177Lu−DOTA−AE105の化学構造である。B(上図):0.5時間〜24.0時間の間の選択された臓器/組織中の177Lu−DOTA−AE105の生体分布である。B(下図):0.5時間〜4.0時間の間の選択された臓器/組織中の177Lu−DOTA−AE105の生体分布についての初期の時点の増加した時間分解能を示している。
以前に記載されているような、2つの9−merDOTAコンジュゲートペプチド、すなわち、DOTA−AE105(DOTA−Asp−Cha−Phe−(D)Ser−(D)Arg−Tyr−Leu−Trp−Ser−CONH2)(図1A)及びDOTA−AE105mut(DOTA−Asp−Cha−Glu−(D)Ser−(D)Arg−Tyr−Leu−Glu−Ser−CONH2)(図1A)を使用した[25]。64CUによるDOTA−AE105の放射性標識化は、以前に報告されているように行った[25]。177LuによるDOTA−AE105及びDOTA−AE105mutの放射性標識化は、177LuCl3(0.05MのHCl中の水溶液、25μl 約500MBq)を、0.05MのHCl水溶液(450μl)中、ペプチド(4.65ナノモル)及びアスコルビン酸ナトリウム(56mg、0.28ミリモル)を含有する溶液に添加することによって行った。この混合物を、80℃で60分間加熱し、その後、周囲温度に到達させた。この混合物をC18 SepPakカートリッジを通過させて、非標識Lu3+を、水(5ml)で溶出させた。放射性標識ペプチドを、0.5mLのエタノールで最終的に溶出させ、注射できるように、エタノール濃度が5%未満になるように水で希釈した。放射化学純度を、RP−HPLCで決定し、非標識177Lu3+の量を、薄層クロマトグラフィで決定した。この合成は、典型的には、300MBqの放射性標識ペプチドを、95〜97%の放射化学純度で産生した。非標識Lu3+の量は、最終生成物で1%未満であった。
皮下結腸直腸癌異種移植モデル
HT−29結腸直腸癌細胞は、アメリカンタイプカルチャーコレクション(American Type Culrure Collection(バージニア州、マナサス、米国)から入手し、培養培地は、インビトロジェン社(Invitrogen Co.)(カリフォルニア州、カールスバッド、米国)から入手した。細胞系を、10%(v/v)のウシ胎児血清及び1%(v/v)のペニシリン/ストレプトマイシンを補足したマッコイの(Mc.Coy’s)標準培地で、37℃及び5%のCO2で培養した。ヒトHT−29結腸直腸癌細胞の異種移植片を100μlのマトリゲル(Matrigel)(BD バイオサイエンス(BD Biosciences)、カリフォルニア州、サンノゼ、米国)中に懸濁させた200μlの細胞(1×108細胞/ml)を、ヒプノルム(Hypnorm)/ドリクム(doricum)による麻酔下で、タコニック社(Taconic)から得た雌NMRIヌードマウスの左右の側腹部に皮下注射することによって構築した(試験日−3日)。全ての動物実験は、デンマーク法務省実験動物研究委員会(Animal Research Committee of the Danish Ministry of Justice)によって承認されたプロトコールの下で実施された。
ヒト前立腺癌細胞でトランスフェクトされたPC−3M.Lucルシフェラーゼは、アメリカンタイプカルチャーコレクション(American Type Culrure Collection(バージニア州、マナサス、米国)から入手し、培養培地は、インビトロジェン社(Invitrogen Co.)(カリフォルニア州、カールスバッド、米国)から入手した。細胞系を、10%(v/v)のウシ胎児血清及び1%(v/v)のペニシリン/ストレプトマイシンを補足したMEMS標準培地で、37℃及び5%のCO2で培養した。
皮下結腸直腸癌異種移植モデル
腫瘍細胞接種(試験0日)後3日目に、それぞれの側腹部でヒト結腸直腸癌異種移植片HT−29を担持する18匹のヌードマウスを、それぞれ6匹の動物からなる3つの群に無作為に分けた。20MBqの177Lu−DOTA−AE105、177Lu−DOTA−AE105mut又は溶媒のいずれかを投与する前に、各群の全ての動物に対し、18F−FLTで最初のベースラインPET/CTスキャンを行った(0日)。その後、試験1、3及び6日に、各群中の全てのマウスの18F−FLTによるPET/CTスキャンを繰り返した。第2の処置線量(20MBq)を、7日に投与し、その後、8、10及び14日にCTスキャンを行い、再構築画像上のマニュアルROI描画を使用して、腫瘍体積を測定した。14日目に、全ての動物を安楽死させて、腫瘍及び腎臓を採集し、更なる分析のために−80℃で保持した。
全てのマウスを3群に分け、0、7及び14日に、セボフルラン麻酔中に、177Lu−DOTA−AE105(18.3±1.1MBq)、177Lu−DOTA−AE105mut(17.9±3.1MBq)又は溶媒のいずれかを、尾静脈内注射によって投与した。試験期間中に、全ての動物を週に二回秤量し、食物/水を自由に摂取させた。週に一回、生物発光撮像法(BLI)を使用して、腫瘍病巣の数を推定した。BLIについては、マウスに、D−ルシフェリン(150mg/kg体重)を腹腔内注射した。IVIS 100(Caliper/Xenogen)を使用して、画像を収集した。D−ルシフェリンの注入後10分で画像が取得され、病巣の総数を計算した。病巣は、バックグラウンドの2倍高いフラックス(ps−1)として定義された。心臓(注入部位)内のいかなる病巣も、これが実際の転移性病巣の反映ではないために、この試験には含まれなかった。
10分間の静態PETスキャンを、セボフラン麻酔中に、約10MBqの64Cu−DOTA−AE105又は18F−FLTのいずれかの静脈内注射の1時間後に、microPET Focus 120スキャナー(シーメンスメディカルソルーションズ社(Siemens Medical Solutions)、ペンシルバニア州マルバーン)で獲得した。全てのPET/CT設定は、以前に詳細に記載されているものを用いた[25]。全ての結果を、Inveonソフトウェア(シーメンスメディカルソルーションズ社(Semens Medical Solutions))を用いて解析し、PETデータを組織1グラム当たりの注入線量のパーセント(%ID/g)として表し、これと共に、CT−データを、立方ミリメートル(mm3)で表わした。
1匹のマウス(処置試験プロトコールに登録されていない)に、177Lu−DOTA−AE105(20MBq、200μLのPBS)を静脈内注射し、注射後1時間で屠殺した。16mm(5/8インチ)のNaI(TI)結晶を有するMillenium VG(ジェネラルエレクトリック社(General Electric)、ハイファ イスラエル)を用いて、ガンマカメラによる静態画像を作成した。画像は、4.0のズーム倍率で、256×256の行列で24時間にわたって獲得され、エネルギー窓は、113±10%及び208±10%keVに設定した。
生体分布試験を、以前に記載された通りに実施した[25]。簡単に言うと、HT−29異種移植片を担持するヌードマウスに、2〜3MBqの177Lu−DOTA−AE105又は177Lu−DOTA−AE105mutを尾静脈内に注射した。トレーサ注入後0.5、1、2、4及び24時間に、全てのマウスを安楽死させた。血液、腫瘍及び主要な臓器を採集し(湿重量で)、パーキンエルマー社((Perkin Elmer)、マサチューセッツ州、米国)から入手したγカウンタを使用して、放射能を測定した(N=4匹のマウス/群)。
線量計算用に、組織1グラム当たりの滞留時間を得るために、臓器取り込み値を時間積分した。0時間〜24時間の間の積分は、トラペゾイド法によって行われた。全ての時間ポイントを二重指数関数(2−コンパートメントモデル)に適合するよう使用し、これは、24時間から無限大までの滞留時間を推定するために使用された。全ての臓器/組織における推定面積は、腎臓(36.1%)及び脾臓(30.0%)を除いて、計算された総面積の<17%であった。177Luの放射性崩壊は、主として低エネルギーβ粒子を生じる。以前に記載されている通りに、1gの球体についてのS値(0.0233mGy/MBq s)を、それを臓器滞留値に掛けることによって、臓器線量を計算するために用いた[20]。
以前詳細に記載された通りに、uPAR ELISAを切除されたHT−29腫瘍で行った[25]。全ての結果は、2回の測定で実施した。ホルマリン固定パラフィン包埋腫瘍組織中のuPARの発現を、以前に発表された通りに、免疫組織化学染色によって評価した(図7)[25]。一般組織病理学的検査を、H&Eで染色された腎臓組織(各処置群からの1匹の動物から得た一対の腎臓)で実施し、これは訓練された病理医(ODL)によって行われた。
全ての定量的データは、平均±SEM(平均値の標準誤差)として表され、平均値は、一元配置ANOVA法を用いて比較される。相関統計は、線形回帰分析を用いて行われた。≦0.05のp値を統計的に有意とみなした。
結腸直腸HT−29腫瘍担持動物における177Lu−DOTA−AE105の生体内薬物動態の試験は、切除後に観察された、血液及び全ての臓器からの速いクリアランス速度を明らかにした(図1B)。放射能の最高レベルは、腎臓、腫瘍及び血液中で認められた。腫瘍中の放射能のレベルは、1.78±0.21%ID/gで、0.5時間後にピークに達し、4時間で0.17±0.02%ID/gに、24時間後には0.057±0.02%ID/gに急激に減少した(表1)。177Lu−DOTA−AE105の最も高い腫瘍/筋肉比(T/M)は、注射後1時間で12であって、投与後24時間で3.3まで徐々に減少した。177Lu−DOTA−AE105と比べて有意に減少した腫瘍取り込み(p<0.001)は、0.5時間後に、非結合対照ペプチド(177Lu−DOTA−AE105mut)の投与後に観察され、したがって、uPAR標的化ペプチドの特異性を確認した(表1)。
以前に立証されたように、ポジトロン放出放射性核種によるDOTA−AE105の標識化は、uPAR陽性腫瘍を局所化させるためのPET撮像の使用を可能にする[25、26、28]。このことは、図2A(左側)に図示され、ここでは、結腸直腸HT−29腫瘍を担持するマウスが、64Cu−DOTA−AE105を用いてPET/CTスキャンされた。このPETトレーサを使用する最高の取り込みは、肝組織及び腫瘍組織で見られ、したがって、以前に発表されたデータと一致した[25、26]。図2A(右側)に示すものは、177Lu−DOTA−AE105の注射後のHT−29腫瘍担持マウスの代表的平面画像である。融合PET/CT画像に基づくマニュアルROI分析を使用して、64Cu−DOTA−AE105については、注射後1時間の0.86±0.03%ID/gの腫瘍取り込みが確認され、これと共に、177Lu−DOTA−AE105の対応する取り込みは、ガンマカウンター分析に基づいて、0.61±0.15%ID/gであった。腫瘍(白い矢印)は、PET撮像法を使用して、明らかに目視できたが、一方平面画像は、鮮明さに劣る画像品質を有した。
連続的CTスキャンに基づく腫瘍サイズにおける有意差が、放射性標識ペプチドの非結合型(177Lu−DOTA−AE105mut)又は溶媒(対照)のいずれかを投与される対照群と比較して、0日及び7日に177Lu−DOTA−AE105の単回用量を投与された後の、6日(P=0.04)及び8日(p=0.002)の腫瘍(n=12)について観察された(図3A)。10日及び14日では、有意差は観察されなかった。14日で試験が終了するときには、平均腫瘍サイズは、溶媒対照、177Lu−DOTA−AE105mut及び177Lu−DOTA−AE105のそれぞれで、287±44mm3、286±32mm3及び218±22mm3であった(p=0.09)。
18F−FLT PET撮像をuPAR標的化放射性核種療法に応答する指標として使用する能力が、この療法試験においても検討された。全てのマウスを、177Lu−DOTA−AE105を投与する前0日にベースライン18F−FLT PET/CTスキャンし、その後、1、3及び6日に18F−FLT PET/CTスキャンを行った。177Lu−DOTA−AE105を投与されているマウスの群におけるベースラインから6日までの18F−FLTの腫瘍取り込みにおける差と、試験期間(14日間)における総腫瘍増殖との間に有意な相関が認められた(p=0.001、R2=0.71)(図4A)。同様な有意な相関は、2つの対照群でも認められた(図6A、B)。
177Lu−DOTA−AE105の生体分布データに基づいて、線量の推定値を計算した(表1)。放射能の最高の累積被曝量は、腎臓(52.9mGy/MBq)で見られ、次いで腫瘍組織(5.8mGy/MBq)、脾臓(5.5mGy/MBq)及び血液(4.9mGy/MBq)であった。しかしながら、HT−29腫瘍のuPAR陽性分画は、腫瘍の周辺部に位置するに過ぎないことが以前に見出されている[25]。同じ発現パターンが本試験で観察され(図7A)、uPAR−陽性癌細胞は、全腫瘍の約10%の分画であるに過ぎなかった。このこと並びに177Luβ放射の小さい透過範囲(<2mm)の知識を踏まえ、本発明者らは、約58mGy/MBqの局所的uPAR−陽性腫瘍分画線量の推定値を計算し、したがって、観察された全ての臓器/組織についての最高の被爆を結果として得た。
試験中に、いずれの群においても動物は早期に死亡しなかった。動物の体重における差は、いずれの群間でも観察されず(図5A)、したがって、177Lu−DOTAS−AE105の2回の投与(2×25MBq)後に、いかなる急性毒性もないことを示している。生体分布データ及び線量推定値に基づくと、腎臓は、任意の正常な臓器の蓄積放射能の最高レベルを有し、したがって、この処置からの毒性の最高リスクを有した。しかしながら、H&E染色後に、各処置群から試験した一対の腎臓において、著しく異常な組織病理的変化は観察されなかった(図5B)。
配位子のIC50値は、表2に示すように、非コンジュゲートペプチドAE105及びDOTA−コンジュゲート型(DOTA−AE105)の双方に対して6.7nMであることが見出された。uPARに対する結合に重要であることが既知である2つのアミノ酸を置換することによって(すなわち、Phe3−>Glu及びTrp8−>Glu)、IC50値における顕著な低減が観察された(IC50>103nM)。PC−3Mを用いるインビトロ細胞結合試験により、177Lu−DOTA−AE105の高い特異性でヒトPARに結合する能力が確認された。スクランブル型(177Lu−DOTA−AE105mu)と比較して、有意に高い結合が認められた(p<0.01)。
PC−3M細胞を、心内注射によって接種し(PC−3M細胞は、播種性転移性疾患に関する十分に説明されたモデルであり、PC−3M細胞系はルシフェラーゼを安定して発現するので)、全ての3つの処置群において、微小転移の形成を、BLIで追跡した。各群についての代表的画像を図3Aに示している。腫瘍病巣数の増加の明確な傾向が、それぞれ溶媒及び177Lu−DOTA−AE105mut群の双方で観察された。このことはまた、それぞれのマウスにおいて、2日〜30日の腫瘍病巣の数の平均的変化を分析することによって確認された(図9B)。腫瘍病巣の数/マウスにおける有意な減少が、177Lu−DOTA−AE105を投与されている群で観察され(−0.44±0.33)、一方、増加が、それぞれ177Lu−DOTA−AE105mut(0.63±0.53)及び溶媒(1.55±0.51)群の双方で観察された(p<0.05)。更には、無遠隔転移生存期間の延長の明らかな傾向が、最初の転移性病巣(心臓以外で)が存在するまでの時間を分析することによって、uPAR標的化処置群(177Lu−DOTA−AE105)について確認された(図10)。177Lu−DOTA−AE105を投与されたマウスの65%において、最初の投与後65日で存在する遠隔転移はなかった。同様な観察は、177Lu−DOTA−AE105mut(対照)及び溶媒対照群では、それぞれ24%及び33%で見られたにすぎなかった。無転移期間の中央値は、溶媒、177Lu−DOTA−AE105mut及び177Lu−DOTA−AE105について、それぞれ12.5日間、16日間及び>65日間であった。全ての主要な所見は、表3に提示されている。
64Cu−DOTA−AE105を配位子として使用して、微小転移性病巣を同定するためのuPAR PET撮像の能力を、試験中に(試験日31日)、多数のマウスにおいて初めて探索した。各動物を、まず初めにBLIを用いてスキャンし、続いてuPAR PETスキャンを行った。各診断法を用いて提示された微小転移性病巣の数を、それぞれの動物について比較した。BLIを用いて提示された全ての腫瘍病巣は、uPAR PET撮像法を用いても検出された(4/4)(図11)。
処置誘発毒性は、本試験の処置群のいずれにおいても観察されず、このことは、マウス重量曲線でも観察された(図12)。
本試験において、発明者らは、ヒト結腸直腸癌モデル及び播種性転移性前立腺癌モデルにおける、特異的uPAR標的化放射性核種配位子177Lu−DOTA−AE105を用いる放射性核種療法の生体内有効性についての実験的証拠の最初の概念実証を報告する。本発明者らは、この放射性核種療法が、uPAR陽性癌細胞の数における有意な減少及び転移性病巣の数の減少の双方を引き起こして、腫瘍の増殖速度に有意な影響を有することを見出した。相当な数の試験により、異なる癌の反応性腫瘍の間質性部位内で、uPARが強く上方調節されること、並びに侵潤性癌の前線で発現され、予後不良に関連することを特定したために、これらの結果は非常に有望である。更には、この結果は、uPAR発現腫瘍及び転移を局在化させるために、並びに18F−FLT−PETを使用する、177Lu−DOTA−AE105投与後の後続の治療応答の予測のためのPET撮像法の可能性も例示している。

- 請求項1〜5のいずれか一項に記載の177−Lu標識ペプチドコンジュゲートを患者に投与することによる、高いuPAR発現に関連する癌疾患の治療方法。
- 前記癌が、結腸直腸癌及び前立腺癌から選択される、請求項9に記載の方法。
