糖尿病、特にI型およびII型は、すい臓から分泌されるホルモンであるインシュリンの欠乏、または不十分な生成に関する障害である。それは世界中の多数の人々に影響を与える。I型およびII型糖尿病を制御するための管理アプローチは、血糖値を正常化して短期および長期の合併症を防止することを目的とし、これは主に経口薬の投与またはインシュリンまたはインシュリン類似体の注射によってなされる。
したがって、rHIを受け取る糖尿病患者の血清サンプルを分析することにより、かかるNAbの存在を検出するための高度に選択的かつ特異的な方法の必要性がある。
本発明の目的は、血清サンプル中の組み換えヒトインシュリンに対する中和抗体の存在を検出するための、高度に選択的かつ特異的なインビトロの方法を提供することである。
本発明の別の目的は、中和抗体の存在を検出するためのキットを提供することである。
一つの側面において、本発明は、血清サンプルにおいてrHI中和抗体の存在をインビトロで検出する方法に関し、以下のステップ:
(a)血清サンプルを前処理すること;および
(b)細胞ベースのアッセイであって、以下のステップ:
i.rHIの受容体を有する細胞を播種すること;
ii.細胞を飢餓させること;
iii.細胞の集団をrHIおよび前処理血清サンプルで刺激すること;
iv.細胞を溶解させ、透明の溶解物を調製すること;および
v.インシュリン受容体のリン酸化について溶解物を評価すること;を含み、ならびに
(c)リン酸化の量を浮遊切断点(floating cut point)と相関させること、
を含む。

を含み、ここで、キットは、PBS中で作られたPEG6000(40%);PBS中で作られたデキストラン炭(3%);抗インシュリンポリクローナル抗体(PAb);600mM氷酢酸(pH2.5);1Mトリス緩衝液(pH9.5);リン酸緩衝生理食塩水(PBS);および組み換えヒトインシュリンを含む。

「実質的に」という用語は、列挙された特性、パラメータ、または値が正確に達成される必要はないが、例えば許容誤差、測定誤差、測定精度限界、および当業者に知られている他の因子を含む偏差または変動が、特性が提供しようとしている効果を排除しない量で生じ得る。
本明細書中で使用される場合、「含む(comprises)/含む(comprising)」という用語は、記載された特徴、整数、ステップまたは構成要素の存在を特定するために用いられるが、1つまたは複数の他の特徴、整数、ステップ、成分またはその群の存在を排除しない。
「細胞+薬物」(C+RHI)は、薬物の対照として使用された。それは、RHIの存在下におけるIR受容体のリン酸化を表す。
「MoR Nab」は、NaB+C+RHI/CAの比である。それは、NAb存在下において達成されたリン酸化の抑制を表す。
「%中和」は、RHIに誘発されたIR受容体のリン酸化の中和を示す。%中和は、(C+RHIのシグナル−NAbのシグナル)/(C+RHIのシグナル−CAのシグナル)×100として決定される。
「精度」は、正常の操作条件下の方法の再現性の程度を測定する。
「特異性」は、陽性対照だけが中和反応を示し、他のいずれの非特異的抗体もこの反応を示さないことを確立するために決定された。
「選択性」は、分析方法がサンプル中に存在する他の成分(干渉物質)の存在下で分析物を識別し検出する能力である。選択性は、サンプルの異質性および多形性のために試験サンプル間で変化し得る。
本発明の背景技術において述べたように、I型およびII型糖尿病に罹患している患者、特にI型に罹患している患者には、インシュリンまたはインシュリン類似体が投与される。多くの場合、患者の免疫系は、主に患者がrHIを投与された場合に、インシュリンまたはインシュリン類似体が機能しないように妨げる抗体を生成する。かかる抗体は、薬物rHIがインシュリン受容体に結合することを妨げ、炭水化物、タンパク質および脂質代謝を含むその同化機能を行うことができないように失敗させる。これは糖尿病の非効率的な管理につながり、さらに致命的な影響を及ぼし得る。
細胞の前述のインシュリン受容体部位は、rHIに結合する際にリン酸化を受ける。したがって、リン酸化されたインシュリン受容体は、rHIの受容体への結合を示すバイオマーカーとして作用する。いずれの特定の理論にも縛られていないが、細胞中のリン酸化されたインシュリン受容体の量はrHIへの暴露の増加とともに増加すると考えられるが、血清サンプル中の中和抗体の存在は、リン酸化インシュリン受容体の量の減少を導くであろう。インシュリンシグナル伝達の他の下流効果を測定することとは対照的に、リン酸化されたインシュリン受容体の測定は、非常に特異的で正確な評価につながることが、本発明者らによって見出された。
(a)血清サンプルを前処理すること;および
(b)細胞ベースのアッセイであって、以下のステップ:
i.rHIの受容体を有する細胞を播種すること;
ii.細胞を飢餓させること;
iii.細胞の集団をrHIおよび前処理血清サンプルで刺激すること;

を含む。
該方法は、細胞をDMEM:F12増殖培地に播種し、約5%のCO2を含む加湿空気中で37℃にて12時間〜16時間培養し、続いて細胞を飢餓状態にすることを含む。細胞を約18時間〜約26時間、好ましくは約18時間〜約22時間の範囲の期間飢餓状態にする。飢餓は、完全増殖培地をグルコースを含まない培地で約24時間置換することによって引き起こされる。飢餓のために使用される培地は、グルコースを含まないDMEM培地である。
キットが実行され、分析され得る手段は、本明細書でなされた開示を考慮すると容易に理解することができる。
例1
サンプルの前処理
血清サンプルを600mMの酢酸で処理した。循環インシュリンの消耗は、3%デキストラン炭溶液により起こり、室温にて約30分間培養し、および180rpmにてオービタルシェイクする。その後、4℃、10,000rpmで10分間スピンさせ、その後、上清を移す。150μlの1Mトリス緩衝液、375μlの40%PEG6000溶液を550μlの上清に添加する。これを180rpmでオービタルシェイクしながら室温で約30分間培養した。PEG沈殿させたサンプルの上清を、NAbの存在について評価したが、これはPEG沈殿中に損失がないことを示した。ペレットの再懸濁を180μlのPBS溶液中、4℃で6000rpmにて約20分間、超音波処理バス中で10分間の超音波処理によって実施した。
サンプル処理後のNAbの回収率は一定であり、50〜70%の範囲であった。
図6は、本方法の前処理ステップの概略図である。
サンプルの細胞ベースのアッセイ
MCF−7細胞を96ウェルプレートに完全増殖培地において播種し、14±2時間表面に接着させた。2日目に、完全増殖培地をグルコースを含まないDMEM培地(FBSおよびRHIを含まない)と20±2時間交換することによって、接着した細胞を飢餓状態に保った。3日目に、グルコースを含まない培地を除去し、実験の設計ごとにサンプルをプレートに添加する。10分間の短時間の刺激後、細胞を溶解し、MSDベースのリン酸化タンパク質シグナリングパネルキット(図2)によってリン酸化されたインシュリン受容体について細胞溶解物を評価する。
0.1μg/mlのRHI濃度での市販の抗体によるIR受容体のリン酸化の中和の評価
細胞をウェル当たり0.5×106個の播種密度で播種した。rHIの2つの濃度(1および0.1μg/mlであった)を細胞の刺激に使用した。3つの利用可能な抗RHI抗体をIRリン酸化の中和について評価した。2つの抗体は商業的供給源、すなわち、それぞれPeninsulaおよびInvitrogenから得られ、3番目は社内で生成された抗体であった。すべての抗体を、2、1および0.5μg/mlの濃度で評価した。
発明者らは、正常なプールされた血清中において、抗RHI抗体の中和活性を評価した。
抗RHI抗体は、血清中でスパイクされ、例1に従って前処理された。処理NAbサンプルは、表2に記載されるようにアッセイにおいて評価された。
結果:アッセイ培地でスパイクされたNAbによるIRリン酸化反応の中和−
処理された血清マトリクス中の細胞に対するRHIの投与量依存的な反応を評価し、抗RHI抗体の中和反応を決定するためのRHIの濃度を選択すること
正常なプールされたヒト血清を、例1に記載のサンプル前処理手順に従って処理した。RHIの範囲は、0.3〜1000ng/mlで調製し、飢餓状態の細胞(0.2×106個/ウェル)に添加した。細胞溶解物をMSDアッセイで分析した。平均MSDデータは、以下の表5および図1に報告される。
実験の詳細:切断点は、バランスの取れた設計において、36の正常なヒト個体血清サンプルを用いて決定された。2人の分析者(A1およびA2)は、3日の異なる日にサンプルを評価し、各分析者はサンプルを1回分析した。切断点の決定は、以下に参照する設計に従って実行された。したがって、切断点を決定するために合計72(36×2)個のデータ点が収集された。
i.データをボックスプロット分析によって異常値について評価した。サンプル19および35は異常値として同定され、したがって、さらなる分析から除外された(図5)。
ii.異常値を除外してデータセットの分布を評価し、データセットが正常に分布していることが見出された(図5)。
iii.式:平均−2.33×SDを用いて、切断点の計算のためのパラメトリックアプローチを適用した。
iv.切断点の真数が決定された。
v.6回の実験で平均NSBを決定した。
vi.乗法正規化係数(NF)は、切断点/平均NSBの式を使用して決定された。
切断点データおよびデータ分析は、表6〜8に示される。
アッセイ感度を確立するために、正常なプールされたヒト血清において、陽性対照抗体の6個の希釈が4μg/ml〜0.5μg/mlで調製された。感度は、以下の表9に示された設計によって3日の分離した日における6の独立したアッセイの実験から決定された。

95%の整合性を有する感度は、式:平均+t0.05df×SDを使用して決定され、ここで、t0.05は、5%の偽陽性率に対応するt−分布から決定される臨界値であり、「df」は自由度(N−1)である。
LPCは、式:平均+t0.01df×SDを使用して感度データセットによって決定された。

高陽性対照(HPC)−4μg/ml
低陽性対照(LPC)−2.5μg/ml
感度データセットは、アッセイ制度を確立するために使用された。精度は、次の部分において評価された。
日内精度−3セットのPC調製物間の変動
日間精度−3日の異なる日における3つの調製物の平均間の変動
分析者間精度−2人の分析者による3つの調製物の平均間の変動
総体精度−2人の分析者による3日の各日において走行する全6セットの変動
MoRの不正確度は、変動係数(CV)として計算されて報告され、および式:標準偏差/平均×100を使用して決定された。
精度は、HPC(4μg/ml)およびLPC(2μg/ml)に対して決定された。
精度データは、表11〜13に示されている。
ウサギ抗ペプチド抗体をHPCおよびLPC(4および2.5μg/ml)レベルでNPHSにおいてスパイクし、抗RHI抗体とともにアッセイにおいて評価した。2つの特異性実験を2つの異なる日に繰り返した。
選択性は、正常なプールされたヒト血清サンプルと共に、10個の個々の正常ヒト血清サンプル(5匹の雄+5匹の雌)を用いて評価した。各サンプルに、抗RHI抗体をLPCおよびHPCレベルでスパイクし、NAbアッセイにて分析した。2つの異なる日に2回の実験が実施された。
結果
i.MoRを各選択性サンプルについて決定した。
ii.浮遊切断点を決定した。
iii.反応≦浮遊切断点を示すサンプルが陽性であると決定した。
選択性データは、表15に示されている。
HPCおよびLPCレベルにて、6つの個々のTI DM患者サンプル(男性4人および女性2人)に抗RHI抗体をスパイクすることによって、疾患マトリクスにおける選択性を評価した。
1型糖尿病患者のRHI療法におけるCmaxは、約100mU/Lであり、これは3.5ng/mlのRHIと同等である。両方のレベルでの抗RHI抗体は、500ng/mlまでのRHIと複合体化したが、これは予想されたCmaxより約140倍高い。HPCおよびLPCの複合体を5、10、50、100および500ng/mlのRHIで調製した。サンプル処理後のRHIの消耗および抗RHI抗体の回収の評価は、MSDベースのRHIアッセイおよびELISAベースの抗RHIアッセイによってそれぞれ実行された。
実験1では、HPCとLPCを10、100、および500ng/mlのRHIと複合体化させたが、実験2では、HPCとLPCを5、10、20、50および100ng/mlのRHIと複合体化させた。両方の実験において、複合体を含まない抗RHI抗体を対照として試験した。
サンプル処理後のRHIの消耗は、MSDベースのRHIアッセイによって評価された。このアッセイは、処理された、または未処理のサンプル中のRHIがMSDプレート上にコートされた抗RHIモノクローナル抗体によって捕捉され、さらにSulfoTag標識抗RHIモノクローナル抗体によって検出されるサンドイッチアッセイである。サンプル処理後のRHIの消耗データを表17に示す。
サンプル処理後の免疫複合体からのNAbの回収は、抗RHI ELISAによって決定された。
i.MoRを各薬物許容量について決定した。
ii.浮遊切断点を決定した。
iii.MoR≦FCPのサンプルがRHIに抗する中和反応に対して陽性であると決定した。
Claims (18)
- 血清サンプルを分析することにより組み換えヒトインシュリン(rHI)中和抗体の存在を検出するためのインビトロの方法であって、以下のステップ:
(a)血清サンプルを前処理すること;
(b)以下のステップ:
i.rHIの受容体を有する細胞を播種すること;
ii.細胞を飢餓させること;
iii.rHIおよび前処理された血清サンプルと細胞の集団を接触させること;
iv.細胞を溶解し、および透明な溶解物を調製すること;および
v.インシュリン受容体のリン酸化について溶解物の評価すること
を含む、細胞ベースのアッセイ;および
(c)リン酸化の量を浮遊切断点と相関させること、
を含む、前記方法。 - ステップ(a)が、以下のステップ;
i.酸解離および炭処理
ii.中和およびPEG処理、および
iii.ペレット再懸濁
を含む、請求項1に記載の方法。 - 酸解離が、血清サンプルを酢酸およびデキストラン炭溶液で処理することを含む、請求項2に記載の方法。
- サンプルが、室温で約30分間、約180rpmで振とうすることで培養される、請求項3に記載の方法。
- 中和およびPEG処理が、以下のステップ:
i.約4℃で約10,000rpmで約10分間スピンさせること;
ii.上清を移し、1Mトリス緩衝液および40%PEG溶液を添加すること;および
iii.約180rpmでオービタルシェイクしながら室温で約30分間培養すること、
を含む、請求項2に記載の方法。 - ペレット再懸濁が、以下のステップ:
i.約4℃で約6,000rpmで約20分間スピンさせること;

- 細胞株が、MCF 10AおよびMCF−7、より好ましくはMCF−7ヒト乳腺腺癌細胞株から選択される、請求項7に記載の方法。
- 細胞が、ウェル当たり0.05×106個〜0.5×106個、好ましくはウェル当たり約0.2×106個の範囲の密度を有する、請求項7に記載の方法。
- 細胞が、DMEM:F12増殖培地に播種され、約5%のCO2を含む加湿空気中で約37℃で12〜16時間の間培養される、請求項1に記載の方法。
- 細胞を飢餓させることが、グルコースを含まない増殖培地中において約18時間〜約26時間行われる、請求項1に記載の方法。
- 接触させるステップが、加湿CO2培養器中でグルコースを含まないDMEM中、室温で約5分〜約30分間行われる、請求項1に記載の方法。
- rHIの濃度が、約10ng/mL〜約30ng/mL、好ましくは20ng/mLである、請求項1に記載の方法。
- 溶解が、溶解緩衝液において室温で約10分間実行される、請求項1に記載の方法。
- インシュリン受容体のリン酸化が、電気化学発光(ECL)ベースのアッセイを使用して検出され、および評価される、請求項1に記載の方法。
- 相関させるステップにおいて、リン酸化の量が浮遊切断点より少ないかまたは等しいときにサンプルが陽性である、請求項1に記載の方法。
- 浮遊切断点の決定が、以下のステップ:
