Description
一般に、本明細書で使用するとき、以下の用語は以下の意味を有する。
図1を参照すると、痰試料を解析するための肺癌試験システムの機能概要が概略的に示される。試験システム5は、痰標本収集10用の装置及び方法、それに続く、例えばLuCED(商標)試験等の早期肺癌検出のための試験12を含む。早期肺癌試験12は、標本染色及び富化14、3D細胞撮像20、3D細胞分類22、及び異常候補細胞の臨床レビュー25用の装置及び方法を更に含む。
肺癌検出試験の一例において、痰標本は、解析に先立って、処理の3つの段階:1)痰細胞単離及び凍結保存;2)蛍光活性化細胞ソーティング(FACS:fluorescence activated cell sorting)による富化;及び3)光トモグラフィー撮像システムの光学コンポーネントにインデックスマッチングされる光学オイル内への富化済み細胞の埋め込みを受ける。
痰は、粘液溶解薬ジチオトレイトール(DTT:dithiothreitol)(Fisher Scientific(マサチューセッツ州ウォルサム所在))によって処理される。長期貯蔵の場合、標本は、その後、41μmナイロンネットを通してろ過され、15%ジメチルスルホキシド(DMSO:dimethyl sulfoxide)(Fisher Scientific(マサチューセッツ州ウォルサム所在))内で−80℃に維持される。ろ過後、保存される標本の最大100μLのアリコートが、肺癌検出試験解析のために取出される。最初に、痰細胞は、下流の肺癌検出試験撮像のためにヘマトキシリン(Electron Microscopy Sciences(ペンシルベニア州ハットフィールド所在))で染色される。細胞は、その後、気管支上皮細胞について富化させると共に、汚染性炎症細胞(好中球及びマクロファージ)を枯渇させるために選択される蛍光コンジュゲートを含む抗体カクテルで処理される。抗サイトケラチンFITCコンジュゲートカクテル(Cell Signaling(マサチューセッツ州ダンバース所在))は、正常及び悪性の両方の上皮細胞において発現するサイトケラチンをターゲットにする。抗CD45−APCコンジュゲート(Mylteni(ドイツ、ベルギッシュ グラッドバッハ(Bergisch Gladbach)所在))は、陰性選択のために炎症細胞をターゲットにする。細胞は、細胞ソーティングに先立って、DAPI(Life Technologies(ニューヨーク州グランドアイランド所在))によっても染色される。FACS富化の場合、DAPI陽性のマザーゲートが作成されて、ダブレット細胞及びデブリを排除し、それに続いて、高い側方散乱事象を排除する。ダブレット細胞は、主に口腔扁平上皮細胞である。その後、高サイトケラチン(高FITC)及び低CD45(低APC)のドーターゲートが引かれる。このドーターゲート内の細胞の集合は、Cell−CT(商標)光トモグラフィーシステム等の光トモグラフィーシステムを使用する、より効率的でかつ下流の肺癌検出試験解析についてソーティングされた富化済みターゲット上皮細胞である。
FACS富化に続いて、細胞は、エタノール中で脱水され、それに続いて、キシレン中で懸濁される。細胞は、その後、適した容積の光媒体に移送され、そこに埋め込まれる。光媒体は、光トモグラフィーシステムのために、整合する屈折率を有する粘性オイルである。埋め込まれると、細胞は、光トモグラフィーシステム上で撮像するため、使い捨てカートリッジ内に注入される。
ここで図5を参照すると、適正分類器を含む痰試料を解析するための肺癌試験の機能ブロック図が概略的に示される。幾つかの細胞分類アルゴリズムが含まれる。光トモグラフィーシステム技術の1つの利点は、人間レビュアーにとって微妙過ぎる又は複雑過ぎる細胞特徴を検出し得る自動化3D細胞解析に起因する。自動化分類は、非常に変動性のある標本の人間レビューをなくす。さらに、3D画像に基づく分類は、細胞疾病状態を包括的に識別するための本質的な画像情報を2Dスライスが保持しない場合があるため、標準的な2D固定焦点面画像に基づく分類に関連する固有の制限を克服する(非特許文献14)。細胞分類は、肺癌検出試験によって処理される何千もの正常細胞から少数の希少な異常細胞を自動的に識別する可能性を有する。
上述した細胞検出分類器の作成及び最適化は、そのプロセスが、参照又はグラウンドトゥルースに従って細胞を正確に診断することを目指すため、一般に、「分類器訓練(classifier training)」と呼ばれる。精度に対して2つの主要な態様が存在する:第1は特異度であり(正常細胞は分類器によって正常と呼ばれる)、第2は感度である(異常細胞は分類器によって異常であると呼ばれる)。アルゴリズム訓練法は、適用的ブースト式ロジスティック回帰(Adaptively Boosted Logistic Regression)(非特許文献18)及びランダムフォレスト(Random Forest)(非特許文献19)を含む。当業者は、テンプレート法、適応的処理等のような分類器のために他の古典的な訓練技法をどのように適用するかをよく知っているであろう。
1.図3A〜図3Cを参照して先に示したように、光トモグラフィーシステムによって生成される3次元細胞画像は、高い解像度を有し、正しい分類をサポートするクリティカルな特徴の的確な測定を可能にする。
2.分類において有用である幾つかの特徴は、3D画像においてのみ出現する(非特許文献17)。その結果、3D特徴セットは、細胞をより多く記述するだけでなく、3次元撮像に基づく分類を、2D撮像に対してより正確にする(非特許文献16)。
3.3次元画像区画化アルゴリズムは、背景から全細胞を、また、細胞から核を分離するために開発された。これらの区画化アルゴリズムの精度は、区画化済みトレースを、人間が導出する細胞又は核エンベロープトレースと比較することによって確認された。
4.特徴測定は、細胞、細胞核、細胞質、及び細胞核小体の種々の態様を記述する。試験システムの一例において、594の特徴が、各3D細胞画像についてコンピューター処理され、その特徴は、対象物の形状、容積、クロマチンの分布、及び、他のより微妙な形態要素を表す。これらの特徴のコンピューター処理は、細胞の向きに無関係であると確認された。
5.分類器訓練についての診断的真理(病理学の至適基準)は、2人の細胞検査技師及び1人の細胞病理学者によって提供される階層的細胞診断に基づく。CellGazer(商標)ワークステーションのユーザーは、例えば、正常又は異常であると宣告された細胞に関する性能を強調する訓練及び試験プログラムを最初に終了しなければならない。
第2に、本明細書の発明者らによって実施される1つの試験において、分類器訓練プロセスの精度は、3つの態様を包含した厳密なプロセスを通して保証された:
1.分類器を訓練するために使用されたデータベースは、2項分布95%信頼区間(非特許文献21)が、許容可能な限度内で性能推定の変動を維持することを保証するのに十分な材料を含むように調製された。
2.過剰訓練は、訓練プロセスの考えられる1つの落とし穴であり、そこでは、多過ぎる情報が分類器内に含まれる可能性があるため、結果は、訓練において使用されるデータに過剰に専用化される可能性がある。この状況は、分類器性能について過剰に楽観的な推定を生成する。過剰訓練のリスクは、訓練データの一部を取得するとともにこれを試験データとして使用することを伴う交差検証によって軽減され得る。分類器において使用され得る情報量の限界は、訓練データに基づく性能推定が試験データによる推定を超える場合に達せられる。
3.最後に、過剰訓練に対する更なる保証として、分類器は、訓練プロセスの一部でなかった細胞の第2のセットによるデータに対して試験された。
以下の考察は、異常細胞分類器42についての訓練を左右するパラメーターを規定するために使用された:
1.痰試料中の異常細胞が十分でなく、痰中の非診断要素が十分であるため、分類器は、高い感度及び非常に高い特異度で動作しなければならない。表4において後で述べるように、高い症例検出感度は、単一細胞分類器感度が75%であり、標本が2つ以上の異常細胞を含むときに維持される。
2.ワークロードを保証することは、妥当な限界内で維持され、特異度についての目標は99%に設定された。
3.下限2項分布95%信頼限界(非特許文献21)についての区間は、感度について70%以内に、また、特異度について98.5%以内に維持された。
痰が、患者ごとの非常に変動性のある標本であるため、肺癌検出試験によって解析される細胞が疾病検出について十分な肺サンプリングを含むかどうかを評価するプロセスが必要とされる。古典的な痰適正は、豊富な肺胞マクロファージの存在に基づいて評価される(非特許文献22)。しかし、これらの細胞タイプは、肺癌検出試験の細胞富化プロセスを通して保存されない。さらに、痰中のマクロファージの存在と異常細胞の存在との間の関係についての従来の評価は、この適正判定法において信頼を与えなかった。その結果、肺癌検出試験適正は、化生細胞及び円柱細胞を含む正常気管支上皮細胞等の参照細胞の列挙に基づく。先に述べたように、肺癌検出試験は、これらの細胞を自動的に列挙するため、適正についての別個のマニュアル解析は必要とされない。先に述べたように、肺癌検出試験標本処理は、痰中の非診断要素を取除く。この処理は、富化済み細胞ペレット内で細胞内容物をランダム化する効果を有する。これは、癌患者からの標本の肺癌検出試験解析中に異常細胞に遭遇する可能性が、試料中の異常細胞の数と正常細胞の数との比及び肺癌検出試験によって処理される正常細胞の数に主に依存することを意味する。この比は、病変サイズ、咳の動態等を含む多くの因子に依存する。症例検出は、その後、異常細胞は、同様に処理されるように十分な正常気管支上皮細胞を処理することに主に依存することになる。
一般に、特徴は、3Dトモグラムの種々の態様の数値表現を提供するためにコンピューター処理される。コンピューター処理される特徴は、対象物のエキスパート診断と共に使用されて、対象物タイプを区別し得る分類器を開発する。例えば、M個の3Dトモグラムを有するデータセットは、第1のタイプ、タイプ1の対象物についてコンピューター処理されてもよく、N個の3Dトモグラムを有するデータセットは、第2のタイプ、タイプ2の対象物についてコンピューター処理されてもよく、第1及び第2のタイプの対象物は、正常細胞及び異常細胞等である。ここで、「M」及び「N」はタイプ1及びタイプ2の値の数をそれぞれ表す。データセットは、好ましくは、光トモグラフィーシステムによって生成される。光トモグラフィーシステムは、例えば、細胞等の対象物の3D画像を含む3Dトモグラムを提供する。細胞は、通常、核小体等の細胞小器官を有する核等の他の特徴を含む。対象物タイプは、様々なタイプの細胞、細胞小器官、選択された疾病状態を示す細胞、プローブ、正常細胞、又は対象の他の特徴を含んでもよい。xの3D画像特徴のセットは、全てのM+Nの対象物についての3Dトモグラムに基づいてコンピューター処理される。次に、対象物タイプを最もよく弁別するyの3D画像特徴の洗練された特徴セットが見出される。ここで、「x」及び「y」は、各段階における3D画像特徴の数を表す。yの3D画像特徴の洗練された3D画像特徴セットが使用されて、その出力が対象物タイプに相関する分類器を構築する。1つの例示的な実施形態において、段階102において、3Dトモグラムのセットが組立てられ、組立てられたセットは、3D生物学的対象物タイプを区別するためエキスパートによって使用されることになる実質的に全ての重要なマーカーを表す。3Dトモグラムの代表的なセットを組立てて、3D画像特徴セットは、重要なマーカーを特徴付ける各対象物についてコンピューター処理されてもよい。
細胞等の生物学的対象物のトモグラムは、複数の観測可能でかつ測定可能な特性を示し、その特性の一部は、分類用の特徴として使用されてもよい。以下の表6は、特徴、すなわち分類目的を果たすために使用される重要なマーカーの概要を提供する。

Claims (29)
- 以下を含む、標本からの3D細胞分類のための細胞学的解析試験:
複数の細胞を前記標本から単離し保存する;
前記複数の細胞を富化し、複数の富化細胞を生産する;
前記複数の富化細胞を光媒体に埋め込む;
前記複数の埋め込んだ富化細胞をキャピラリーチューブ内に注入する;
疑似投影図観察サブシステムの視野内に少なくとも1の埋め込んだ細胞が現れるまで、前記複数の埋め込んだ富化細胞に圧力を印加する;
前記疑似投影図観察サブシステムを用いて、前記視野内の前記少なくとも1の埋め込んだ細胞の少なくとも1の疑似投影画像を取得する;
前記キャピラリーチューブをチューブ軸を中心に回転し、前記疑似投影図観察サブシステムを用いた、異なる視界におけるさらなる疑似投影画像を生成し、それぞれの画像化された埋め込まれた細胞のための疑似投影画像のセットを提供する;
それぞれの画像化された埋め込まれた細胞を、前記疑似投影画像のセットからのデータを用いて再構成し、3D細胞再構成のセットを生産する;
生体標本分類器を操作して、前記3D細胞再構成のセットから、参照細胞を検出する;
前記標本の前記検出された参照細胞を列挙する;
受信者動作カーブ(ROC)より特徴付けられる、予め決められた一細胞の感度値および、予め決められた一細胞の特異値で、第二の細胞分類器を操作して、標的細胞を検出する;および
適性分類器を操作し、列挙された参照細胞の閾値に対して、参照細胞の数を比較し、標本適性を判定する。 - 前記参照細胞が正常気管支上皮細胞を含み、前記標的細胞が異常肺細胞を含む、請求項1の方法。
- 前記標的細胞が、異常扁平上皮細胞、腺癌細胞、気管支肺胞上皮癌細胞、異常神経内分泌細胞、小細胞癌細胞、非小細胞癌細胞、肺円柱細胞、腫瘍細胞、新生細胞及び気管支肺胞上皮癌細胞、並びに痰中に見出される他の細胞及び対象物からなる群より選ばれる請求項1の方法。
- 前記閾値が統計的にあらかじめ決められた範囲である請求項1の方法。
- 前記閾値が250から800の正常気管支上皮細胞の範囲である請求項1の方法。
- 特異度および感度のための適性分類器訓練を用いた生体標本分類器を生成することをさらに含む請求項1の方法、ただし感度は標的細胞に関連し、特異度は前記標本の非希望細胞に関する。
- 適性分類器訓練は3次元画像区画化アルゴリズムを含む対象物を記述する特徴測定セットを用いて実行される請求項5の方法。
- 前記対象物は核を含む細胞であり、3次元画像区画化アルゴリズムは背景から全細胞を、また、細胞から核を分離する請求項7の方法。
- 適性分類器訓練は、細胞核、細胞質、及び細胞核小体、対象物の形状、容積、クロマチンの分布、及び、他の形態要素からなる群より選択される特徴を含む特徴測定セットを用いて実行される請求項7の方法。
- 特異度および感度のための分類器訓練を用いた前記第二の細胞分類器の生成をさらに含む請求項1の方法。
- 分類器訓練が異常細胞と正常対象物からの特徴を用いて実行される請求項10の方法。
- 前記標本を痰標本から得ることを含む請求項1の方法。
- 前記痰標本を富化することをさらに含む請求項12の方法。
- 前記痰標本の富化は蛍光活性化細胞ソーティングまたは免疫磁気富化を用いることを含む請求項13の方法。
- 以下を含む患者のために肺癌を示す試験を実行する方法:
前記患者から痰標本を得る;
前記標本から複数の細胞を単離し保存する;
前記複数の細胞を富化し、複数の富化細胞をえる生産する;
前記複数の富化細胞を光媒体に埋め込む;
前記複数の埋め込んだ富化細胞をキャピラリーチューブ内に注入する;
疑似投影図観察サブシステムの視野内に少なくとも1の埋め込んだ細胞が現れるまで、前記複数の埋め込んだ富化細胞に圧力を印加する;
前記疑似投影図観察サブシステムを用いて、前記視野内の前記少なくとも1の埋め込んだ細胞の少なくとも1の疑似投影画像を取得する;
前記キャピラリーチューブをチューブ軸を中心に回転し、前記疑似投影図観察サブシステムを用いた、異なる視界におけるさらなる疑似投影画像を生成し、それぞれの画像化された埋め込まれた細胞の疑似投影画像のセットを提供する;
それぞれの埋め込まれた細胞を、前記疑似投影画像のセットからのデータを用いて再構成し、3D細胞再構成のセットを生産する;
生体標本分類器を操作して、前記3D細胞再構成のセットから、参照細胞を検出する;
前記標本の前記検出された参照細胞を列挙する;
受信者動作カーブ(ROC)より特徴付けられる、予め決められた一細胞の感度値および、予め決められた一細胞の特異値で、第二の細胞分類器を操作して、標的細胞を検出する;および
列挙された正常気管支上皮細胞の閾値に対して、正常気管支上皮細胞の数を比較するために適性分類器を操作する。 - さらに前記患者に対し低線量CTを行うことを含む請求項15の方法。
- 前記閾値が統計的にあらかじめ決められた範囲である請求項15の方法。
- 前記閾値が250から800の正常気管支上皮細胞の範囲である請求項16の方法。
- 特異度および感度のための分類器訓練を用いた生体標本分類器の生成することをさらに含む請求項15の方法。
- 分類器訓練は、背景から全細胞を、また、細胞から核を分離する3次元画像区画化アルゴリズムを含む前記細胞を記述する特徴測定セットを用いて実行される請求項19の方法。
- 分類器訓練は細胞核、細胞質、及び細胞核小体、対象物の形状、容積、クロマチンの分布、及び、他の形態要素からなる群より選択される特徴を含む特徴測定セットを用いて実行される請求項19の方法。
- 特異度および感度のための分類器訓練を用いた前記異常細胞分類器を生成することをさらに含む請求項15の方法。
- 分類器訓練が異常細胞と正常対象物からの特徴を用いて実行される請求項19の方法。
- 前記標本を痰標本から得ることを含む請求項15の方法。
- 前記痰標本を富化することをさらに含む請求項15の方法。

- 前記参照細胞が正常気管支上皮細胞を含み、前記標的細胞が異常肺細胞を含む、請求項15の方法。
- 以下を含む標本からの3D細胞分類のための細胞学的解析試験システム
複数の細胞を前記標本から単離し保存する手段;
前記複数の細胞を富化する手段;
1以上の富化細胞を光媒体に埋め込む手段;
1以上の埋め込まれた細胞をキャピラリーチューブ内に注入する手段;
前記1以上の埋め込まれた細胞に、疑似投影図観察サブシステムの視野内に現れるまで、圧力をかける手段;
前記疑似投影図観察サブシステムを用いて、少なくとも視野内の埋め込んだ細胞の少なくとも1の疑似投影画像を取得する手段;
前記疑似投影図観察サブシステムを用い、異なる視界におけるさらなる疑似投影画像を生成し、それぞれの埋め込まれた細胞のための疑似投影画像のセットを提供するために、前記キャピラリーチューブをチューブ軸を中心に回転する手段;
それぞれの埋め込まれた細胞を、前記疑似投影画像のセットからのデータを用いて再構成し、3D細胞再構成のセットを生産する手段;
生体標本分類器を操作して、前記3D細胞再構成のセットから、参照細胞を検出する手段;
前記検出された標本の参照細胞を列挙する手段;
標的細胞を検出するために、予め決められた一細胞の感度値および、予め決められた一細胞の特異値で、受信者動作カーブ(ROC)より特徴付けられる、第二の細胞分類器を操作する手段;および
列挙された参照細胞の閾値に対して、参照細胞の数を比較し、標本適性を判定するために、適性分類器を操作する手段。
